背景技术
慢性阻塞性肺疾病(COPD)包括例如慢性支气管炎和肺气肿的情况。COPD目前单在美国就影响超过1500万人并且是该国第三大死亡原因。对于超过90%的COPD病例,COPD的主要原因是香烟烟雾的吸入。该病的经济和社会负担是实实在在的并且是正在增长的。
慢性支气管炎的特征在于具有痰液的慢性咳嗽。由于气道炎症、粘液高分泌、气道高反应以及气道壁的最终纤维化,导致显著的气流和气交换限制。
肺气肿的特征在于肺实质的破坏或损伤。肺实质的破坏导致弹性反冲维持气道通畅的栓带的损失。因为细支气管不受到类似更大气道的软骨的支撑,它们几乎没有内在的支撑,因此当栓带破坏发生时,对塌陷敏感,尤其是在呼气的过程中。
COPD的急性加重(AECOPD)经常需要急救以及住院护理。AECOPD由症状的突然恶化(咳嗽、喘息以及痰液变化的加重或者发病)所界定,该症状通常持续数天,但也能持续数周。AECOPD通常由细菌感染、病毒感染或者污染物引起,所述AECOPD可以由引起显著气道限制的气道炎症、粘液高分泌以及支气管收缩快速表现出来。

在哮喘中,气道内的慢性炎症过程在增加肺内气流阻力上起到了中心作用。许多细胞和细胞单元涉及炎症过程,包括但不限于肥大细胞、嗜酸细胞、T淋巴细胞、中性粒细胞、上皮细胞,甚至气道平滑肌本身。这些细胞的反应导致沿气道进行特定刺激的气道平滑肌细胞敏感性和超常反应上的相关增长。
哮喘的长期性也能导致气道壁的重构(例如气道壁增厚或者慢性水肿的结构变化),这将进一步影响气道壁的功能并且影响气道的超常反应。上皮剥脱将下伏的组织暴露给了不能另外正常接触下伏组织的物质,进一步加强了细胞破坏和炎症反应的周期。
在易感个体中,哮喘的症状包括复发性气短(呼吸困难)、喘息、胸闷和咳嗽。当前,哮喘通过避免刺激以及药理学进行管理。
支气管扩张是一种肺气道变得扩大、松弛和/或具有瘢痕的情况。在受损区域,经常产生粘液,引起阻塞和/或感染。重复感染的周期可以继续破坏气道并且引起更加大的粘液产生。支气管扩张能够导致例如呼吸衰竭、肺不张和心力衰竭的健康问题。

图1为具有塌缩形态的电极阵列或电极笼102的示例性导管100远端部的立体图。通过切口或者合适的自然开口,例如嘴和鼻,导管可以用来引入到病人的体内。在一个示例性的例子中,通过合适的导入鞘(未示出),例如内镜、支气管镜或者其他类型的镜,导管100可以前进到病人体内需要的位置。导管的近端可以连接到用于操作笼102的毂组件、手柄(未示出)或者另一种合适的致动器102。更加特别地,手柄或制动器可以用来使笼102在第一塌缩形态(例如,图1中所示)和第二扩张型态(例如,图5中所示)之间转变。手柄可以按照人体工程学来进行设计,并且可以包括各种构件,例如,用来选择定位导管100远端部的转向控制和拉丝。进一步地,手柄可以包括与用来***其他医疗装置的导入鞘的一个以上的工作通道连通的一个以上端口。
为了治疗,导管100的远端部可以用来消融或者将能量传送到病人体内的组织。如图1中所示,导管100的远端可以包括电极笼102、启动元件104(例如,拉丝)、扩张支架106和107,以及海波管或盖子108。正如下文所更加详细讨论的,电极笼102可以由互相可操作地容纳的两个独立的电极管形成。正如下文所描述的,电极管可以包括一个以上的电极带10和11,所述电极带中的每一条,可以通过纵向间隙18彼此分开,并且可以构造成传送能量组织的单极或双极形态。电极管的远端和近端可以包括允许启动部件104从导管100近端部穿过电极笼102向远端延伸的开口。不像传统的电极笼或电极阵列,可以不用在导管100上一起焊接电极10、11来构建电极笼102,并且通过将正极与负极隔离,可以以双极形态来对其进行操作。
延伸穿过电极笼102的启动元件104的长度也可以延伸穿过扩张支架106、107。扩张支架106、107,电极10、11之间的纵向间隙,和海波管108有助于电极10、11的屈曲。电极10、11的屈曲可以允许电极笼102在塌缩形态和扩张形态之间之间往复运动,在所述扩张形态中,电极笼的每个电极径向向外弯曲。在一些例子中,电极10、11可以包括储存电极10、11塌缩或者扩张形态的形状记忆材料。海波管或盖子108可以位于电极笼102的远端来支撑电极笼102并为其提供恢复力。
图2A-2H、3A-3E和4A-4B示出了组成图1中所描绘的电极笼102的构件和组件的各种示图。随后公开的内容描述了用电极板202和203形成电极笼102的示例性实施例(参照图2A-2H)。电极笼102可以包括热电偶(TC)元件304(例如,丝)的通道302(参照图3A-3C),和用来给电极笼102提供应变释放的收缩管306(参照图3D-3E)。电极板202和203能够连接到端盖402(图4A)、启动元件104(图4B)、扩张支架106、107和海波管108(图5)来制造导管100的远端部。
在一个例子中,电极板202、203中的每一个(参照图2A)可以由任何其性质既具有柔性又具有导电性的生物相容性材料的单薄板形成。薄板可以是矩形或者另一种合适的形状。生物相容性材料可以包括,但不限于不锈钢、镍钛诺、其他已知导电的手术/医疗材料,以及它们的任意结合。在制造过程中,电极板202可以由具有远端部22和近端部24的薄板形成,能够对该薄板进行加工或腐蚀来实现各种厚度的截面。类似的,电极板203可以由具有远端部23和近端部25的薄板形成。各种薄板的远端部22、23和近端部23、25也可以加工成,使得它们相对于薄板的剩余部分更加宽。

随后,可以卷制电极板202、203来形成部分管形部件(图2B),使得电极板202、203中的每一块都具有径向分布。当被卷起时,电极板202、203可以分别形成电极管204、205。径向分布可以在电极管204、205的纵向端生成C-形凸轮表面,允许电极管204环绕和紧密配合电极管205,形成电极笼102。例如,使用任何适合的机构(图2C),其中例如,焊接、同轴滑动、摩擦配合、螺纹配合以及胶合,电极管204的径向远端部22和近端部24可以可操作地容纳各自的电极管205的远端和近端部23、25。不像在传统的导管中,其中电极笼或电极阵列的形成可以通过将电极焊接成管束来实现,电极管204、205可以替代地相互容纳(例如,可以将一个***到由另一个部分界定的容纳空间中)。与此相一致的,电极笼102在其远端和近端部可以具有C-形开口。电极管204、205的外直径和内直径可以决定引起电极管204、205的互锁以形成电极笼102的有效作用力。电极管204、205的这种互锁可以允许围绕电极管204、205的远端部22、23和近端部24、25的电极笼102的扩张。另外,C-形径向端的互锁可以防止电极10、11、12和13相对于彼此的旋转。电极管204、205可以通过过盈配合或者摩擦配合相对于彼此固定。
进一步地,电极管204、205中电极10、11、12和13的角位置,能够通过它们之间纵向间隙18方位来控制。也就是说,电极管204、205可以相对于彼此旋转(如图2D和2E中所示)来,例如,控制电极10、11、12和13之间的间隔。电极管204、205远端和近端边的共面性可以保持,从而适当地使电极对齐。此外,在操作过程中纵向间隙18、20可以允许电极笼102的压缩和/或屈曲。
在电极笼102中,内电极管205可以可分离地连接到或者永久地连接到外电极管204,且电极10、11、12和13之间要么均匀的间隔(图2D)要么不均匀的间隔(图2E)。电极10、11、12和13可以沿电极笼102的圆周均匀地间隔以允许内电极管205相对外电极管204直径上的差异。电极笼102在其纵向端的圆周可以允许电极10、11、12和/或13之间的不均匀间隔。
基于电极202、203之间的绝缘设置,电极笼102可以是单极或双极。如果电极10、11、12和13在远端部22、23和近端部24、25中的一个或全部彼此不绝缘,电极笼102可以以单极方式进行工作(图2F)。如果电极10、11、12和13在远端部22、23和近端部24、25彼此绝缘,电极笼102可以以双极方式进行工作(图2G)。此外,如图2F和2G中所示,可以对电极11的部分P(由网纹表示)进行绝缘来形成位于电极10、11的内部的用于电能传送的集中区域。类似的,可以对电极12、13的部分进行绝缘来形成用于电能传送的集中区域。为了实现上文所描述的绝缘特征,可以用任何合适的绝缘材料对电极管204、205进行包覆,例如远端部22、23和近端部24、25接触区域中本领域已知的聚合物亲水层。其他合适的绝缘涂层可以包括陶瓷、有机硅、玻璃和任何其他非导电、双极材料。涂层可以在电极管204、205组装前产生。电极管204、205的涂层可以促进电极管204、205内部射频(RF)电能的定位。可以使用各种包覆工艺来应用绝缘材料,包括气相沉积、浸渍、喷涂或者其他小部件和薄膜导电的工艺。上文所描述的绝缘也可以用于电分离电极10-13,使得笼102可以以双极形态进行工作。
可以在电极管204、205(图2H)之间设置偏移X,并且可以将偏移X配置在电极笼102的远端26以协助电极笼102的组装。例如,可以允许内电极管205独立于外电极管204被压缩。进一步地,如果,例如在双极构造中,外电极管和内电极管204、205的最远表面28、30(参照图2H)分别暴露以接触正在传导电能的启动元件104,可以通过启动元件104将电能供应给内电极管205。在双极构造下,例如最远表面28可以绝缘于最远表面30,所述最远表面30与启动元件104接触并且从其接受电能。
参照图3A,电极管204、205的近端和/或远端可以为大体C-形。然而普通技术人员将认识到管204、205的近端和/或远端可以包括任何合适的构造。当电极管204、205互锁时,它们可以界定纵向延伸间隙302。由于间隙302的存在,电极管204、205的近端和/或远端可以部分界定不完全的环形通道303。在一个例子中,间隙302可以形成在电极管204、205的外部,并且可以用来为例如自导管100近端部延伸的热电偶(TC)元件304(例如,丝)提供进入通道303的入口。在手术过程中TC元件304可以用来测定电极10、11、12和13(或者电极可以接触的周围组织)的温度。在另一个例子中,给电极10、11、12和13供应电能的元件(例如,丝)可以路由送导管100的近端部通过间隙302穿过通道303。正如图3A中所示,电极笼102可以围绕其径向端扩张,因此与其他构造相比,钎焊或者焊接的TC元件304穿过电极笼102与电极10、11、12和13的接触可以相对简单。TC元件304可以跟随电极10、11、12和13上的绝缘涂层,并且可以在电极管204、205组装前布置在电极10、11、12和13上。进一步地,TC元件可以通过作用在各个电极管204、205上附加的压缩力而固定在通道内,或者,通过将TC元件304定位到通道303里并且随后通过粘结、焊接或钎焊,或者通过另一种合适的固定构件,将TC元件304连接到通道303的内表面。
参照图3E,电极管204、205的C-形端可以进一步界定纵向延伸间隙305以容纳收缩管306。间隙305可以基本上平行于间隙302,并且可以相对间隙302围绕电极管204、205C-形端的圆周换位。收缩管306可以为连接电极元件,例如TC元件304和电极10、11、12和13提供应变释放。在一个例子中(参照图3B-3E),间隙305可以接收TC元件304和收缩管306。在替代例子中(参照图3B-3E),电极管204、205可以定位在电极笼102中以在电极笼102的一端形成用来接收TC元件304和收缩管306的单通道302。通道302可以将电极,例如电极10,的一端与电极管204的剩余部分分开。电极管204、205的C-形端,可以界定在电极管204、205的一端(例如,近端或远端)将电极11和12分开的圆周延伸的间隙310。在另一个例子中(参照图3C),可以在电极10和布置于电极10较厚截面上的收缩管306之间形成通道311。在这个例子中,通道311可以沿电极10较厚截面以及电极笼102的C-形近端或远端,容纳TC元件304。收缩管306可穿过通道311以及沿TC元件304滑动(参照图3D)。可以随后加热收缩管306以使收缩管306永久贴附在TC元件304上来提供应变释放。通道311可以给电极提供额外的刚度,并且当扩张电极笼102时,可以生成改进的组织气道接触。参照图3E,外电极管204可以紧邻电极笼102的远端定位通道302。然而收缩管306可以紧握通道302,所述通道302可以在电极笼102的近端布置在电极10、11的最外层。
进一步地,一旦互锁电极管204、205以形成电极笼102,可以将端盖402(参照图4A)***到电极笼102的远端以维持电极笼102的内直径和方位。在一些例子中,盖402可以促使内电极管204径向向外并且抵靠外电极管205,因此在它们之间形成摩擦配合。可以形成布置在端盖402的同轴孔408以允许启动元件104朝远侧穿过该同轴孔408。端盖402也可以包括与通道302卡合的脊部404,所述通道302由电极管204、205在电极笼102的C-形远端形成。端盖402中的脊部404可以维持电极管204、205相对彼此的方位。脊部404也可以促使内电极管204的相对端彼此远离,因此扩大了内电极管204的直径,并且因此促使其抵靠外电极管205。端盖402可以改造或构造成包括绝缘材料,并且可以容纳电极管204、205之间的偏移X(参见图2H)。在具有电极10、11、12和13(它们在电极笼102的远端彼此绝缘)的C-形接触区域的双极形态下,改造的端盖402可以用来将电能从启动元件104传导到仅仅一个电极管,例如,电极管205。因此,在一些例子中,改造的端盖402与电极10、11、12和13任何组合的C-形接触区域的导电远端连通。
参照图4B,在至少一个例子中,启动元件104可以从电极笼102内穿过通道302朝远侧延伸。启动元件104的远端可包括阻挡部406,所述阻挡部406在远侧接合并且穿过端盖402中的同轴孔408。阻挡部406可以具有比启动元件104其余部分大的直径。端盖402可以***电极笼102的远端与其连通。在手术过程中,阻挡部406可以帮助提供稳定性、握力以及协助电极笼102从塌缩形态到扩张形态的往复运动。
图5示出了扩张状态下的电极笼102。可以给基于电极管204、205方位以及电极10、11、12和13与启动元件104之间间隔的电极笼102的构造提供扩张支架106和107,以协助电极屈曲方向并且防止电极倒转。扩张支架106和107也可以用作对齐部件,当电极10、11、12和13处在扩张形态时,以维持它们之间的间隔。扩张支架106和107中的每一个可以包括布置在其内用来容纳启动元件104的部分的纵向腔。
***可以包括控制器(未示出),以及包括导管100的标准柔性支气管镜。控制器可以用来将RF电能传送给位于导管100远端部分的电极10、11、12和13。在手术过程中,导管100可以以塌缩形态,通过例如鼻或嘴的身体自然开口,朝远侧进入到例如肺的支气管树或者气道的体腔。接着可以将柔性支气管镜导航到治疗靶位,例如,位于靶向支气管肺叶的最远端气道。一旦导管100的远端部分定位在治疗靶位,可以扩张电极笼102,使得电极10、11、12和13接触气道的内壁。当电极10-13与内气道壁接触时,可以限制其扩张。随后,可以启动控制器以将电能通过启动元件104以及电极10、11、12和13传送给治疗位置,所述控制器可以连接到位于近端的导管100的毂组件。通过将能量(例如,RF、超声波或者热能)传送给组织,例如肺组织,的电极10、11、12和13可以以单极或者双极的方式传送电能。可以控制电极10、11、12和13的每个这种启动,使得以某个功率、温度和/或时间段传送RF电能,实现某个治疗方法,例如,在单极应用的情况下,以10秒时间间隔,以大约65摄氏度的温度,以及最高至约15瓦的功率。应该注意到,如果需要,可以增加或者减少启动时间段。然而,相对于单极启动时间段,电极10、11、12和13的双极启动可以减少这种启动时间段。可以控制被传送的电能以形成热能到气道壁的精确传送,例如,消灭或减少过多的ASM,并且减少气道收缩的能力,因此减少了COPD或者其他支气管病情的严重程度。在一些例子中,这可以减少哮喘发作的频率。当肺组织或者ASM被充分减少,或者治疗完成,例如,可以停用控制器,并且可以使启动元件104松弛以释放电极10、11、12和13的屈曲,以使电极笼102回到塌缩形态,使得导管100可以从病人体内移走。
这种能量传送过程可以侵害最小化,并且可以在一次以上的门诊手术访问中执行,每一次对肺的不同区域进行治疗,并且时间间隔差不多一周以上(例如,三周)。

现在参照图6A-6D,图中描绘了按照本发明进一步的例子的另一种远端能量传送组件700。能量传送组件700可以包括本文所讨论的前述例子中的一个以上的特征,例如,电极笼102。

启动元件836可以从能量传送阵列800的近端801朝着远端802延伸。启动元件836与参照图5所描述的启动元件104基本上相似,并且可以连接到任何合适的致动器。启动元件836可以包括与扩张支架106、107基本上相似的扩张支架(未示出),以维持能量传递元件810和816之间的间隔。启动元件836可以以任何合适的方式连接到远端件814,来帮助能量传送阵列800在图7所示的塌缩形态到径向扩张形态(未示出,但是与图5中示出的电极笼102的扩张形态基本上相似)之间的移动。在一些例子中,可以向着近侧拉动启动元件836,因此向近侧移动远端件814和808,使得能量传递元件810和816中的每一个屈曲并且向外径向扩张。因此,启动元件836的纵向移动可以使得能量传送阵列800径向扩张和收缩。

在一些例子中,启动元件836可以用来将能量从控制器和/或电源传送到远端件814和第二极性组件804的能量传递元件816。进一步想到,其他启动元件(未示出)也可以将能量传送到第一极性组件802。在一些例子中,第一极性组件803和第二极性组件804可以是由电源供应的各自具有不同极性的RF电路的独立电极。在一些例子中,电极的极性可以在高频振荡。因为第二极性组件804绝缘于第一极性组件803,所以当将能量传送阵列布置在体腔和人体组织内,RF能量可以从第二极性组件804,穿过人体组织流到近端件806、远端件808和第一极性组件803的能量传递元件816,加热围绕能量传送阵列800的人体组织。另一种选择,能量可以从第一极性组件803,穿过人体组织,流到第二极性组件804。
图8-11描绘了能量传送阵列800的示例性组装顺序。图8描绘了被部分组装的第一极性组件803。正如图8中所示,可以将两个能量传递元件810连接到近端件806。远端件808,图8中显示未连接到能量传递元件810,可以通过上文所描述的任何合适的机构连接到能量传递元件810,以形成如图9中所示的第一极性组件803。可以将能量传递元件816(图10中显示已经连接到近端件812),朝着第一极性组件803的远端件808***穿过由近端件806界定的腔。如图11中所示,能量传递元件816可以穿过远端件808的腔朝着第二极性组件804的远端件814向远侧延伸。因此,在一些例子中,为了适应本文所描述的组装过程,与能量传递元件810相比,能量传递元件16可以更长。
可以将各种导管、电极阵列、能量传送阵列以及说明书中所公开的其他装置连接到一个以上轴和/或鞘,所述轴和/或鞘具有从大约0.5英尺到大约8.0英尺范围的长度,或者另一种合适的长度。在一些例子中,能量传送阵列和/或篮可以具有扩张篮直径,所述扩张篮直径范围从大约1mm到大约25mm,或者具有另一种合适的范围。在一些例子中,电极脚的暴露部分,例如,有源区826,长度可以是大约5mm。在其他的例子中,有源区826可以是大约1mm到大约50mm的长度范围,或者可以具有另一种合适的长度。
在本发明的各种例子中,可以将RF能量施加到界定体腔(例如,肺气道)的组织,持续范围为大约0.1秒到大约600秒的时间长度。在一个例子中,电源能够传送大约1到100瓦的RF能量,并且可以具有连续流动的能力。可以将界定肺气道的组织维持在小于、等于或者大于环境体温的温度。在一个例子中,可以将组织维持在至少大约60℃、70℃到95℃之间和/或70℃到85℃之间。RF功率级通常范围从大约0-30W,或者另一个合适的范围。在一些例子中,电源可以在不到75℃的设置下进行操作。在一些例子中,可以以离散激活的方式传送RF能量,例如5到10秒每次激活。RF能量的频率可以从300到1750kHz。应该注意到,也想到了能量传送次数、瓦特数、气道温度、RF电极温度以及RF频率其他合适的值。
此外,尽管文中已经一并示出和描述了具体的例子,应该意识到,设计用来实现相同或者相似目的的任何后续的布置可以取代文中描述以及示出的具体的例子。本发明打算覆盖各种例子任何和所有随后的改变或变化。例如,想到了上文所描述的示例性方法步骤能够彼此并且能够包括其他步骤地连续、同时或者以各种顺序发生。一旦本领域的普通技术人员了解了本发明,文中未具体描述的上述例子和其他例子的结合将是显而易见的。

Claims (14)
1.一种医疗装置,包括:
能在第一形态和第二形态之间往复移动的可扩张能量传送阵列,所述可扩张能量传送阵列包括:
第一组件,所述第一组件具有第一近端件、第一远端件以及一个以上的第一能量传递元件,所述第一能量传递元件在第一近端件和第一远端件之间延伸;和
第二组件,所述第二组件具有第二近端件、第二远端件以及一个以上的第二能量传递元件,所述第二能量传递元件在第二近端件和第二远端件之间延伸,其中所述第二近端件的最远部分在1)所述第一近端件的最近部分以及2)所述一个以上的第一能量传递元件中的每一个的最近端部的近侧,而所述第二远端件的最近部分在1)所述第一远端件的最远部分和2)所述一个以上的第一能量传递元件中的每一个的最远端部的远侧,其中所述第一和第二组件彼此电绝缘。
2.如权利要求1所述的医疗装置,其中所述第一和第二近端件由绝缘件隔开。
3.如权利要求2所述的医疗装置,其中所述第一和第二近端件由所述绝缘件沿纵向隔开。
4.如权利要求1所述的医疗装置,其中所述第一和第二远端件由绝缘件沿纵向隔开。
5.如权利要求1所述的医疗装置,其中所述第一和第二能量传递元件相对于所述医疗装置的纵轴线彼此径向交替。
6.如权利要求1所述的医疗装置,其中所述第二能量传递元件延伸穿过布置在所述第二远端件径向外表面上的缺口。
7.如权利要求1所述的医疗装置,其中所述第一能量传递元件布置成比所述第二能量传递元件离所述能量传送阵列的纵轴线更远。
8.如权利要求1所述的医疗装置,其中所述第二组件用来将能量经人体组织传送到所述第一组件。
9.如权利要求1所述的医疗装置,进一步包括启动元件,所述启动元件自所述医疗装置的近端延伸穿过所述第一和第二组件,并且所述启动元件连接到所述第二远端件。
10.如权利要求9所述的医疗装置,其中所述启动元件的纵向运动用来使所述能量传送阵列在第一和第二形态之间往复移动,其中所述第一形态为塌缩形态,而所述第二形态为径向扩张形态。
11.如权利要求9所述的医疗装置,其中所述启动元件用来将RF能量传送给所述第二组件。
12.如权利要求1所述的医疗装置,其中所述第一能量传递元件和所述第二能量传递元件中的每一个包括由绝缘区域在近侧部和远侧部界定的有源区域,其中当所述有源区域与人体组织接触时将RF能量传送给所述人体组织,并且所述绝缘区域在任何时候不用于将RF能量传送给人体组织。
13.如权利要求12所述的医疗装置,其中至少第一或第二能量传递元件的所述有源区域包括至少一个温度检测元件,所述温度检测元件用来检测所述有源区域或者人体组织的温度。
14.如权利要求1所述的医疗装置,其中所述能量传送阵列用来以双极形态将能量传送给人体组织。
