技术领域
本申请涉及生长激素化合物的药用组合物或制剂。
背景
生长激素(GH)为由哺乳动物的垂体前叶分泌的多肽激素。依物种而定,GH为包含约190个氨基酸残基,相当于分子量为约22 kDa的蛋白。GH通过细胞表面受体结合于GH受体(GHR)并发信号。GH在促进生长、维持正常身体组成、合成代谢和脂质代谢中起着关键作用。其对于中间代谢也有直接影响,比如减少葡萄糖摄取、增加脂类分解以及增加氨基酸摄取和蛋白质合成。激素也对包括脂肪组织、肝、肠、肾、骨骼、***和肌肉在内的其它组织发挥作用。

寻求具有增加的功能比如增加的半衰期的生长激素化合物目的在于减小所需要的化合物的量和治疗药物的给予频率。
尽管具有增加的半衰期的生长激素化合物可在实验环境下获得和起作用,但是必须使得患者能够以本文允许安全和便利地使用的制剂获得化合物。
概述

本发明的一个方面涉及包含生长激素白蛋白结合体缀合物的药用组合物。
药用组合物可作为液体组合物制备,其中液体组合物可为比如溶液剂、混悬剂或乳剂。

生长激素白蛋白结合体缀合物

表面活性剂可助于增加疏水性、油性物质的水溶性或者增加两种物质与不同疏水性物质的溶混性,因此增加相反的溶解度。表面活性剂可进一步助于降低液态药用组合物中的表面活性剂与蛋白分子之间由于相互作用造成的聚集。在本发明的一个实施方案中,组合物包含表面活性剂。

制备药用组合物的方法
作为制备药用组合物的起点,以溶液,通常以水溶液提供生长激素白蛋白结合体缀合物。这种起始溶液优选地为高度浓缩的,使得能够在制备药用组合物期间稀释。在一个实施方案中,生长激素白蛋白结合体缀合物的起始溶液大于15 mg/mL,比如15-30 mg/mL或者比如15-25 mg/mL。
常用的制备方法是使赋形剂(水中的缓冲剂、表面活性剂、等渗剂和防腐剂)溶解于2×缓冲剂溶液中,这种缓冲剂溶液以最终组合物的浓度的双倍浓度包括每一种赋形剂。依生长激素白蛋白结合体缀合物溶液(起始溶液)的浓度而定,加入适当体积的该种溶液,以包括达到最终药用组合物的浓度需要的量。可通过缓慢加入化合物溶液并确保经搅拌继续混合来实施混合。最后,加入水以达到总体积。pH可在不同的步骤进行调节,比如化合物溶液、2×缓冲液和最终药用组合物中的一种或多种。
技术人员应知道如何以各种方式修改方法,比如如果需要不同的混合关系或者如果在化合物溶液中已经存在最终组合物的一种或多种组分,则调节2×缓冲液的浓度,因此2×缓冲液中这种组分的浓度应小于最终浓度的二分之一。
本文的实施例8和9描述GH组合物的制备,并且对以上一般方法进行调节。关键的信息是需要避免混合GH与高浓度苯酚。
治疗方法
如在背景部分中描述的那样,生长激素产品适合于治疗生长激素缺乏症。基本上本发明的药用组合物可用于治疗其中患者得益于循环生长激素的活性升高的任何一种疾病或障碍。在当前的治疗中给予生长激素蛋白。作为备选方法,可给予具有延长的半衰期的生长激素化合物,以提供生长激素活性。

在一个实施方案中,本文的生长激素化合物和组合物用于治疗儿童GHD、成人GHD (AGHD)、特纳综合征(TS)、努南综合征、特发性矮小(ISS)、小于胎龄(SGA)、普拉德-威利综合征(PWS)、慢性肾功能不全(CRI)、骨骼发育不良、SHOX缺乏、AIDS消瘦、HIV相关性脂肪代谢障碍(HARS);短肠综合征,任选地包括类固醇依赖性疾病、囊性纤维化和纤维肌痛。

因为IGF-1反应是GH功能性的标志,治疗有效剂量可基于给定生长激素白蛋白结合体缀合物的IFG-1反应估计。如在图6中看到的那样,给予缀合物I导致剂量依赖性的IGF-1反应,在所有剂量下IGF-1水平升高证明这种化合物适合于每周一次给予。
在一个实施方案中,药用组合物用于一种以约0.01-2.0 mg/kg每剂量的量给予生长激素缀合物的治疗方法。在成人,组合物可用于给予每剂量0.02-0.10

实施例
生长激素白蛋白结合体缀合物可按照标准方法制备。在合适的宿主比如大肠杆菌表达生长激素蛋白并纯化。缀合反应可按照技术人员已知的本领域方法实施。以下呈现可用于制备这种缀合物的一般方法,包括与本文描述的具体缀合物有关的细节。技术人员可基于本领域的一般知识修改这些方法以制备备选缀合物。
用于制备生长激素蛋白的一般方法
把为生长激素蛋白编码的基因重组***到质粒载体。随后采用质粒载体转化合适的大肠杆菌菌株。人GH或GH变体可表达为具有N-末端蛋氨酸,或者表达为MEAE融合体,MEAE序列随后由该融合体裂解掉。
以25%甘油制备细胞储备液并储存于-80℃下。把甘油贮备菌株接种到LB板并随后在37℃下温育过夜。用LB培养基洗涤每一个板的内容物并稀释到500 mL LB培养基用于表达。培养物于37℃下温育,并且在220 rpm下振动,直到达到OD600为0.6。在25℃下采用0.2 mM IPTG实施随后诱导6小时。经离心分离最终收获细胞。
随后把细胞悬浮于含有0.05% Tween 20、2.5 mM EDTA和4 M尿素的10 mM Tris-HCl, pH=9.0中,并采用细胞粉碎机在30kPSI下进行粉碎。如果表达含有游离半胱氨酸(用于缀合)的GH分子,那么在悬浮缓冲液中包括10 mM半胱胺。经离心分离收集上清液,并随后经受色谱法纯化。
采用离子交换色谱法和疏水性相互作用实施纯化,随后采用自CHO细胞表达的人二肽基肽酶I (hDPPI)去除肽标记。经同种沉淀(isoprecipitation)和离子交换色谱法实现最终纯化。
纯化也可用但不限于离子交换色谱法、疏水作用色谱法、亲和色谱法、尺寸排阻色谱法和本领域技术人员已知的基于膜的分离技术实现。
包括GH(L101C)的单一cys GH变体的制备:
在如以上描述的初始纯化之后,变体可具有用谷胱甘肽和胱胺阻断的其游离半胱氨酸部分。在含有GSH和GSSG的平衡缓冲液中采用谷氧还蛋白II (Grx2)进行酶促解封闭。经在Sephadex G25柱上的缓冲液交换,分离解封闭的GH (L101C)与低分子量GSH/GSSG。
纯化的生长激素化合物的蛋白质化学特性
采用MALDI-MS分析完整的纯化蛋白,以确认观测到的质量与自氨基酸序列推导出的理论质量一致。
在二硫键用DTT还原的前后,采用胰蛋白酶和AspN消化,随后经MALDI-MS分析消化,可经肽图证实预测的键合二硫键。
白蛋白-结合体侧链制备
侧链(I)
4-(1H-四唑-16-基-十六碳酰基氨磺酰基)丁酰基-OEG-γGlu-γGlu-OEG-NƐ(C(O)CH2Br)Lys-OH(I):

在ABI433合成仪上,采用标准的Fmoc肽化学,以1mM规模,按照流程1,在固相载体上合成侧链(I)。采用Fmoc-OEG-OH和Fmoc-Glu-OtBu保护的氨基酸,在Fmoc-Lys(MTT)-Wang树脂上装配肽。采用DCM/NMP中的DIC/NHS 2当量把4-(16-1H-四唑-5-基-十六碳酰基氨磺酰基)丁酸手动偶联过夜,TNBS检测显示反应完成。然后按照流穿安排,用50 mL DCM/TFA/TIS/水(94:2:2:2)处理树脂,直到黄色消失,~20分钟,随后用DIPEA/DMF洗涤并中和。在DCM/NMP (1:1)中的溴乙酸(4 mM)用NHS和DIC的1
mM混合物活化,过滤并加入到树脂中,加入另外1 mM的DIPEA。1小时后反应完成。树脂用80 mL TFA/TIS/水(95:2.5:2.5)处理1小时。用N2流蒸发,通过加入Et2O沉淀并用Et2O洗涤,且干燥。在制备型HPLC (2次运行)上纯化粗品产物,梯度为30-80% 0.1 TFA/MeCN,对比0.1% TFA/水。收集级分并用~50% MeCN冻干,得到侧链(I)。TOF-MS: 质量1272.52 (M+1)。

偶联具有内部游离单一cys的GH蛋白(GH-SH)与如以上描述的白蛋白结合侧链(AB-卤素)。
a) 经用合适的选择性还原剂还原二硫化物(VI)释放GH-SH (VII):
b) 游离GH-SH (VII)用卤素活化的白蛋白结合体(VIII)烷基化,得到具有连接于单一Cys残基的“-S-”的AB的生长激素缀合物。
白蛋白结合体侧链与GH (101C)和侧链(I)的缀合
把白蛋白结合体侧链(78 mg/5当量)溶于含有5%羟丙基-ß-环糊精的170 mL HEPES/EDTA缓冲液中,并加入MTP (2.1 mL, 1%)和0.5 M NaCl (6.34
g)。
向该混合物中加入浓缩的GH (L101C) (1当量, 46 mL),并使混合物在室温下放置过夜。溶液一夜之间变为浑浊。由于HPLC 表明有未反应的起始原料,因此加入另外5当量溶于最小量NMP中的来自实施例5的白蛋白结合体。把得到的混合物在室温下搅拌另外16小时。关于缀合和纯化方法的进一步细节参照WO 11/089255。
采用以上描述的方法得到以下显示的生长激素白蛋白结合体缀合物I和II。

备选方法可用于制备如先前已在WO11/015649中描述的备选生长激素白蛋白结合体缀合物,这种方法包括N-末端C-末端缀合并在链中采用谷氨酰胺转胺酶反应位点特异性缀合于Gln或Lys残基。同样对于如以上描述的单一Cys缀合,可应用备选的缀合方法,比如在WO11/050923中描述的方法。
HMWP含量经尺寸排阻色谱法(SEC-HPLC)的测定
分析方法是尺寸排阻色谱法(SE-HPLC)测试,其中样品采用TSK G2000 SWxl柱分析,等度洗脱采用磷酸钠/异丙醇缓冲液,并随后在215 nm下进行紫外检测。相对于总积分面积计算% HMWP。
目视检查
在建筑师灯下和在光室中检查储存于玻璃小瓶或盒( I型)中的样品(最小体积2 mL)。样品的视觉外观通过目测评分量化,澄清度和颗粒的标准按分数从1到5,1为澄明且没有颗粒和5指具有可见沉淀的样品。
蛋白聚集体采用光密度(OD)的测定
样品的光密度采用Varian Cary 100 Bio UV-VIS分光光度计在340 nm下测量。
颗粒采用微流成像(MFI)的测定
采用Micro-Flow Imaging流动显微镜MFI 5000上的流动显微镜,实施未经稀释溶液(1 mL)中的颗粒按大小和形态的定量分析。实施>2 μm、>10μm和>25μm在显微镜下才能看到的颗粒的定量与表征。还应用去除最终气泡和硅微滴的滤器(环状0-0.85)。
实施例1
在不同的pH (pH 6.0和6.6)下,包括不同的表面活性剂和变化的表面活性剂与缓冲剂(组氨酸)浓度,研究以上描述的缀合物I的制剂稳定性。作为实验(DoE)设计的研究实施研究。
所测试的制剂除了生长激素缀合物I外还包括表面活性剂和以0.68 mg/mL或1.55 mg/mL的浓度存在的组氨酸缓冲剂,还包括40 mg/mL甘露糖醇和3.0 mg/mL苯酚。
在5℃下,在90天内,测量来自时间点0、30、60和90天的样品中的HMWP(高分子量蛋白)的量。在25℃下得到类似的数据。采用TSK凝胶G3000 SWXL柱、磷酸钠/异丙醇pH 7.0流动相,和等度洗脱,并随后在215 nm下进行紫外检测,经尺寸排阻色谱法(SEC-HPLC)测量HMWP含量。
图1显示泊洛沙姆188在制备的时候稳定制剂。发现在不含泊洛沙姆188的制剂中HMWP的量在时间为零(t=0)时增加。然而在聚山梨醇酯80制剂和不含表面活性剂的制剂中的HMWP水平降低至与在5℃下储存一个月后分析的含有泊洛沙姆的制剂相当的水平。

mg/mL泊洛沙姆188优于1.0 mg/mL聚山梨醇酯80。
实施例2
制备缀合物I的制剂实施可制造性研究。
液体缀合物I药物物质制剂(包括组氨酸缓冲剂)以体积为最终体积的约40%进行pH调节至pH 6.5和7.1。约60%体积的缓冲剂溶液(组氨酸、甘露糖醇、泊洛沙姆和苯酚)进行pH调节至pH
6.8。在混合两种溶液后,最终制剂进行pH调节至pH 6.8。自具有不同pH的制剂制备的pH 6.8下的制剂跟着在25℃下储存1个月期间进行稳定性研究。
分析在时间=0、1天、2周和1个月时取得的样品。
所有样品自目视检查显示澄明,并且自SE-HPLC测量的% HMPW提供在下表中。

6.8的评价
在最终产品制剂的制备期间,没有观察到制剂之间依GH缀合物DS溶液的pH而定的HMWP水平的明显差异。所有制剂在制备的时候呈现类似水平的HMWP和在其中测试25℃下储存1个月期间的稳定性的加速稳定性研究中呈现类似的稳定性。对于自具有pH 6.5的DS制备的制剂可能观察到一种轻微的趋势,要开发的制剂具有泊洛沙姆188的量越高(1.0 mg/mL和3.0 mg/mL), HMWP随着时间推移的量越高。
预计在混合缓冲剂和DS溶液后,由制剂中几乎相当的pH造成所有制剂几乎相似的稳定性,因为缓冲剂溶液(pH 6.8)占最终产品溶液的约60%。
实施例3
伴随改变包含甘露糖醇和苯酚的组氨酸缓冲液(0.68 mg/mL组氨酸、44 mg/mL甘露糖醇和3 mg/mL苯酚)中泊洛沙姆188的浓度(1.0、3.0和5.0 mg/mL),制备缀合物I的制剂。通过测量HMWP,测试30℃下,在pH 6.2、6.8和7.4下的稳定性。作为实验(DoE)设计的研究实施研究。
如自图3看到的那样,样品在时间0. (T=0)呈现同等数量的HMWP。在30℃下储存4周后,HMWP数据的统计分析没有发现泊咯沙姆的显著影响。然而,在pH 6.2和6.8两种情况下观察到一种趋势,要开发的制剂具有泊洛沙姆量增加, HMWP随着时间推移的量越高,表明如果组合物用于长期储存,那么应避免高浓度的泊洛沙姆188。在图中不包括来自pH 7.4的数据,因为导致HMWP形成通常偏低的pH效应占主导。
实施例4
在以下实验中研究pH 6.2、6.5和6.8下的效果。伴随改变泊洛沙姆188的浓度(0.0、1.0、3.0 mg/mL)和伴随改变pH (6.2、6.5和6.8),制备缀合物I制剂。所测试组合物的其它组分如在实施例3中那样。样品在室温下伴随剪切物理施压7天。在3和6小时后以及在1、2、5、6 和7天后取样。其它样品经移液施压25次。所有样品进行目视检查,并在340 nm下进一步分析OD (光密度)。
如自图4看到的那样,在振动和移液研究两种情况下,与含有1.0和3.0 mg/mL泊洛沙姆188的样品相比较,不含泊洛沙姆188的样品呈现OD 340的明显增大。这种趋势在所有pH水平下得到证实。图4A呈现全部结果,而图4B仅呈现低的OD 340测量。此外,视觉评分清楚地显示,不含泊洛沙姆的样品浑浊,并且在降低pH时浊度增大。
实施例5
采用3种另外的生长激素化合物,在pH 6.2和6.8下实施与实施例4类似的实验。野生型人生长激素(WT hGH)、缀合物I和包括101
Cys突变的生长激素变体(GH L101C),其也为缀合物I的GH蛋白。如自图5 A、5B和5C看到的那样,以上结论得到证实。低的表面活性剂含量在施加应力初始和之后两种情况下改善组合物的稳定性,如在7天期间测试的那样。对于缀合物I,不含泊洛沙姆的样品似乎特别依赖于pH。通过视觉评分,观察到随着pH降低浊度增大。
实施例6
微流成像(MFI)评价用于测量生长激素组合物中的颗粒形成。在Micro-Flow Imaging流动显微镜MFI 5000上测量样品(1 mL)。如上,在评价中包括以下化合物:野生型人生长激素(hGH)、缀合物I、缀合物II和生长激素变体L101C (GH L101C)。
在pH 6.2和pH 6.8下,用6.7 mg/mL生长激素化合物评价泊洛沙姆188的稳定作用。
作为应力稳定性研究建立研究,在室温下振动样品多达7天。在pH 6.2和pH6.8下的生长激素组合物包括0.0、1.0或3.0
mg/mL泊洛沙姆188,并在T=0和T=2 (2天)时进行测量。组合物全部包含0.68 mg/mL组氨酸、44.0 mg/mL和3 mg/mL苯酚。在T=0得到的数据包括在表1中,而两天后(T=2)得到的数据包含在以下表2中。

由此得出结论,泊洛沙姆188在所测试的所有生长激素化合物组合物中抑制颗粒形成。此外,较高浓度(3
mg/mL与1 mg/mL相比较)的泊洛沙姆188对颗粒形成具有较强的影响。对于GH的缀合物,pH似乎对颗粒抑制具有显著影响,因为在pH 6.8比在pH 6.2下得到更好的结果,减少高泊咯沙姆浓度的优势。通过目视检查得出类似的结论。
实施例7
在日本和非亚洲的健康男性受试者实施随机、双盲、安慰剂对照的多剂量和剂量递增试验,以评价安全性、耐受性和药代动力学(PK)及药效学(PD)参数。
16名受试者,4个组,经皮下给药,给予缀合物1
(n=12)或安慰剂(n=4)每周一次,连续4周(相等数量的日本和非亚洲受试者)。在单独组中剂量为0.02、0.08、0.16和0.24 mg/kg。化合物作为冻干的制剂提供,并在使用前重新组成。
在第4个(第4)剂量后评价缀合物I的药代动力学。观察到缀合物1的平均血浆浓度的剂量依赖性增加(数据未显示)。通过测量IGF-I反应(免疫诊断***(IDS))评价缀合物I给予的药效学参数。
观察到剂量依赖性的IGF-I和IGFBP-3反应,并且在所有剂量下IGF-I水平升高。平均IGF-I标准偏差得分(SDS)概况(图6)表明,缀合物I可能适合于每周一次给予。如果进一步显示,在剂量≤0.08 mg/kg下诱导临床相关的IGF-I反应。

在制备期间,自缀合物I和泊咯沙姆188溶液和自中间制剂和最终药用组合物取样。经SE-HPLC测量高分子量蛋白的含量(% HMWP) (表1),并发现在整个过程偏低。最终药用组合物显示澄明和无色。

实施例9
在另一项研究中,在苯酚-缓冲剂溶液中以不同浓度(5、10和30 mg/ml)包括苯酚,并引入到缀合物I制剂中。如以下看到的那样,使用30 mg/ml苯酚在最终制剂中给出% HMWP的增加。HMWP的水平取决于溶液中缀合物I的浓度和苯酚的浓度。

Claims (15)
1. 一种药用组合物,所述组合物包含2-20 mg/mL的生长激素白蛋白结合体缀合物(GH-AB)、缓冲剂、防腐剂和0.5-5.0 mg/mL的表面活性剂。
2. 权利要求1的组合物,其中所述组合物包含1 mg/mL的泊洛沙姆188作为表面活性剂。
3. 前述权利要求中任一项的组合物,其中所述组合物包含组氨酸缓冲剂。
4. 前述权利要求中任一项的组合物,其中所述组合物不包含甘氨酸。
5. 前述权利要求中任一项的组合物,其中所述组合物的pH为6.5-7.0,优选地为6.8。
6. 前述权利要求中任一项的组合物,其中所述组合物包含2.0-4.0 mg/mL的苯酚或3.0-4.0
mg/mL苯酚。
7. 前述权利要求中任一项的组合物,其中所述组合物包含等渗剂,比如40-45 mg/mL甘露糖醇。
8. 前述权利要求中任一项的组合物,其中AB-经GH中的cys残基连接于GH。
9. 前述权利要求中任一项的组合物,其中所述组合物用于生长激素缺乏症(GHD)或成人生长激素缺乏症(AGHD)的治疗方法。
10. 前述权利要求中任一项的组合物,其中所述组合物用于经每周一次给予的治疗方法。
11. 前述权利要求中任一项的组合物,其中所述组合物用于经每周一次给予0.01-0.08 mg/kg的剂量,来治疗生长激素缺乏症(GHD)或成人生长激素缺乏症(AGHD)。
12. 一种用于治疗生长激素缺乏症(GHD)或成人生长激素缺乏症(AGHD)的方法,所述方法包括给予有需要的患者治疗有效量的权利要求1-9中任一项的药用组合物。
13. 权利要求12的方法,其中所述组合物每周一次给予。
14. 权利要求12的方法,其中所述组合物每周一次给予患有生长激素缺乏症的成人患者。
15. 权利要求12或13的方法,其中所述组合物给予患有生长激素缺乏症的患者,其中每剂量给予0.01-0.08 mg/kg的所述生长激素白蛋白结合体缀合物。
