本发明涉及制备含有诱导因子或细胞因子的调整的血液成分的方法以及调整的血液成分及其治疗和预防人体或动物体疾病的应用。
背景技术
众所周知,血液成分,主要是蛋白质、因子或细胞因子,例如***、胰岛素或在血液或血清中含有的干扰素,具有治疗和预防疾病的效果。像白介素-1受体拮抗剂(IL-1Ra)这样的已知因子抑制了发炎引起的病变。同样众所周知的是,这样的血液成分一部分由血液组织自行生成或从血细胞中分泌到血液的血浆相。
特定的血液成分,如因子或细胞因子的生成或释放,可以由诸如培养从动物或人体内抽取的全血而得到积累。在培养后,经过培养的血液中常常会有更高浓度的特定因子。接下来,这种血液成分可以在必要时进行分离。这些含有诱导因子的血液,也可以从细胞成分中释放并作为所谓的诱导血清在上述动物或人体内(再)使用。
“诱导”指的是像因子或细胞因子这样的血液成分的生成或释放的积累过程。已知用于诱导全血中血液成分的基本过程是,从动物或人体内抽取全血,然后在一个含有专门用硌酸处理过的玻璃球而产生改变的一次性注射器中,在一段特定的时间内,在培养条件下进行培养(Meijer等,Inflamm,res.52(2003):1-4)。接下来分离这些细胞成分,从而获得一种含诱导因子或细胞因子的调整的血液成分。相应的,从人的静脉全血中获得了这样一种血清,其中抗发炎细胞因子白介素-1受体拮抗剂(IL-1Ra)、白介素-4(IL-4)和白介素-10(IL-10)的含量与新鲜采集的全血相比有提高。其中,全血的培养时间为24小时,培养温度约为37℃。
这样制备的血液成分用于治疗各种发炎的疾病和自体免疫性疾病,例如类风湿性关节炎。有显示,通过局部和/或全身地应用这样调整的血液成分,如类风湿性关节炎得到了缓解或治愈。不过,这种治疗效果仍需改进。
此外,还有其它一些疾病,例如肌肉损伤,至少在动物模型中,由于局部或全身地应用像IL-1Ra或类似成分的重组体细胞因子而得到治疗。在公开方式下制备的血清成分,在这里没有显示出效果或只显示出不充分的效果。而且,肌肉损伤在运动医学领域经常出现;在由运动导致的疾病或损伤中所占的比例最高达到30%。超过90%的这种肌肉损伤因挫伤,或因过度的肌肉拉伸应力造成。这种损伤经常会导致强烈的疼痛,并且导致无力再继续训练或无力继续短期或长期从事运动。因此,现有的技术需要改进。
马的一种需要重视的疾病是慢性或周期性的眼炎(马周期性复发葡萄膜炎,ERU)。根据现有技术,关于这种慢性炎症的假设是一种所谓的钩端螺旋体性葡萄膜炎,急性或慢性钩端螺旋体感染对于这种慢性眼炎的形成具有重要影响。在德国,这种寄生虫引起的感染率最大达到80%。在一部分被感染的动物中,已显示不同的病症,这其中也包括慢性眼炎。对于这种疾病的形成来说重要的因素似乎是,动物的免疫***是否能容忍这些钩端螺旋体作为寄生虫存在。
在马中,会出现由严重的炎症或腱鞘炎引起的跛行。引起跛行的另外一个原因也可以表现为在肌腱组织内部的退化性变化,也就是所谓的“核心病变”。这同样引发了很多炎症反应。通常可以使用“糖皮质激素”(可的松)、细胞浸渍剂()、血小板浓缩液( 等)或来自骨髓或脂肪组织中的细胞标本(干细胞)治疗发炎和跛行的症状。
神经性皮炎的症状,是由于免疫***的过度反应而引起的。不过,目前经常使用的含可的松的药膏的治疗产生了一些副作用。
此外,在德国居民中经常出现炎症,准确的说是带有大多数原因不明的神经***的发炎病变。其中,背痛表现出一个常见的症状。由于疼痛是引起发炎的原因,因此通过使用镇痛药,具体而言通过使用糖皮质激素(例如去炎松)只是在症状上进行治疗。
子宫内膜异位症是一种子宫粘膜的细胞或组织侵入腹腔中并因此导致大多为良性的肿瘤疾病。这种肿瘤通常对激素过敏,因此由于激素状况的不同会产生强烈的疼痛。外科手术切除或激素治疗可以解决疾病产生的痛苦从而给予病人帮助。不过,这经常会定期复发。这些肿瘤会慢慢发作,使得其它器官畸形生长并由此产生强烈的慢性疼痛。这种症状,经常只有服用大量的镇痛药才可以忍受。在***和绝经期之间的所有妇女中大约10%的人患有子宫内膜异位症,并伴有或多或少的明显症状。这种疾病的一种非常极端的症状会导致***症。
发明内容
因此,需要一种易于制备的、改进的可替代活性血液成分及其制备方法,用于有效地治疗前述的疾病,以及其它可以通过存在于血液中的因子或细胞因子来治疗的疾病。本发明所要解决的技术问题主要体现在,提供一种改进的方法,用于制备一种调整的血液成分,该血液成分含有特定的诱导因子或细胞因子,并可以用于有效地治疗和预防疾病。
本发明涉及的技术问题主要通过提供一种用于从血液中制备调整的血液成分的方法而得到解决,所述方法至少包括以下步骤:
在步骤(a),用已知的方法,优选通过静脉穿刺,从人体或动物体抽取新鲜血液,优选的是静脉全血。步骤(b),紧接着,这些抽取的血液在至少一个改进的容器中进行培养,用以诱导血液成分中的因子或细胞因子,即刺激这些血液组织中的因子或细胞因子的生成和释放。根据本发明,在所述改进的容器中血液培养过程的温度为10~40℃,优选的是25~40℃,更优选的是约37℃。在步骤(c),在所述改进的容器中获得富含诱导因子或细胞因子的调整的血液成分。
根据本发明,在血液培养过程中使用了至少一个改进的容器,这个容器具有下述特征:对于每毫升培养的血液,该容器具有至少约100mm2/ml或更多的内表面积、特别是104mm2/ml或更多、123mm2/ml或更多、131mm2/ml或更多、224mm2/ml或更多,或者283mm2/ml或更多。在一个优选的方案中,所述容器具有约200~750mm2/ml、特别优选的是约250~650mm2/ml的内表面积。
优选的,这个容器具有5ml或更多、10ml或更多、50ml或更多、60ml或更多,或者100ml或更多的容积。例如,计划抽取并培养大约50ml的血量,按照本发明所述的改进的容器,应该具有至少约6600mm2(66cm2)、优选约10000mm2~37500mm2(100~375cm2)的内表面积。例如,计划抽取并培养大约10毫升的血量,按照本发明所述的改进的容器,应该具有至少2300mm2(23cm2)、优选约2500mm2~7500mm2(25~75cm2)的内表面积。
这个容器的“内表面积”是指容器内部在培养过程中与要调整的血液成分接触的面积,即基本上会变湿的面积。
另外,本发明也提供了一种血液,从人体或动物体抽取,在具有内表面为200mm2/ml或更多的特定表面积指数的特殊的改进的容器中培养。发明人惊奇地发现,通过使用本发明所述的这个方法,在较短时间之后,在这个改进的容器中得到了一种调整的血液成分,该血液成分含有高比例的特定的诱导因子,例如含有高浓度的因子IL-6。因此,令人惊奇地,按照本发明所述的方法得到了一种在预防和治疗上有高活性的血液成分。通过按照本发明所述获得的调整的血液成分作为血清成分,用于患病的生物体或感染的器官,例如像在马中的关节炎和眼炎、肌腱损伤、神经损伤、子宫内膜异位症、神经性皮炎以及肌肉损伤可以得到治疗。
按照本发明所述的方法,可以从新鲜抽取的人类供体的全血中获得一种调整的血液成分,其中IL-6在新鲜抽取的人体中含量超过2000pg/ml。与此相比,IL-6在未调整的全血中的含量为大约0.5~15pg/ml。一般来说,按照本发明所述,IL-6的含量会达到大约200~4000倍的提升。
除了这个特别值得关注的因子IL-6之外,其它在治疗和预防上有效的成分,在调整的血液成分中的因子或细胞因子都获得了高浓度。除了已知的因子IL-4、IL-10和IL-I Ra之外,令人惊奇的是还包括像白介素-13(IL-13)、白介素-1(IL-1),特别是IL-1β、肿瘤坏死因子(TNF)、***(IGF)、转化生长因子(TGF)、血小板衍生的生长因子(PDGF)、成纤维细胞生长因子(FGF)和肝细胞生长因子(HGF)这样的因子。在按照本发明所述的可制备的调整的血液成分中,也存在着由不同的经过有效诱导的因子或细胞因子组成的“鸡尾酒”。不拘于理论,按照本发明所述含因子和细胞因子“鸡尾酒”展现了在治疗和预防上高效的有效成分。
其中,上述在血液成分中的有效成分也可以以泡囊、微泡囊或外来体的形式存在。泡囊或微泡囊理解为可以由免疫细胞的薄膜表面诱捕的亚细胞成分。外来体理解为亚细胞成分,所述亚细胞成分在纳米尺度下表现为多孔结构并通过所谓的“多泡体”的内陷和免疫细胞的分泌而形成。
“血液成分”理解为血液中的现有成分,主要由血浆、血清和血细胞组成,至少含有一种成分选自蛋白质,如因子和细胞因子。当前,血液成分也理解为血清成分。血清成分与血液成分的区别在于,血清成分不(再)含有任何细胞成分。例如,一种调整的血清成分从一种按照本发明所述的调整的血液成分得到,并借助离心分离、过滤或其它合适的方法把细胞成分从所述血液成分中分离出去,因此获得一个由血浆和血清成分组成的至少含有诱导因子和细胞因子的不含细胞的溶液。
相应地,在一个优选的实施例中,在下一个步骤中把这些细胞成分从已经获得的调整的血液成分中完全或基本分离出去,这样得到一种调整的血清成分。所述调整的血清,可以像按照本发明所述的血液成分那样使用,并且通常具有按照本发明所述的这种优势。本领域的技术人员会根据应用领域和有效性的不同使用这种调整的血清成分或调整的全血成分。优选的,他将使用这种调整的血清成分。
优选的,血液的培养在至少一个改进的容器中一直进行,直到因子或细胞因子的诱导足够成熟。这些诱导因子或细胞因子,基本上从培养开始时起就从血液组织中生成和分泌,这样在调整的血液成分中诱导因子或细胞因子的有效数量就会得到积累。
在本发明的一个实施例中,血液成分中的IL-6的含量显示了进一步诱导成功。特别的,其中IL-6的含量至少为30pg/ml。优选的,在改进的容器中进行培养,直到IL-6在血液成分中的含量至少达到30pg/ml。在其它优选的方案中,血液进行培养,直到IL-6在血液成分中的含量具有至少200pg/ml、优选500pg/ml、特别优选1000pg/ml。
在另外一个实施例中,培养时间为36小时或更少。在另外一个实施例中,培养时间为9小时或更少。在另外一个实施例中,培养时间为从2~36小时或更少,优选到2~9小时或更少。
在另外一个实施例中,血液的培养在低氧分压(pO2)下进行。特别的,氧分压在培养过程中低于5kPa,优选小于3kPa。在一个优选的方案中,血液的培养在改进的容器中、在隔绝氧气的情况下实现。
在一个优选的实施例中,这个改进的容器在其内部具有大表面积的特殊结构,以致于这个由容器的(外部)几何尺寸所产生的内表面由于这些特殊的结构而变大。这种由这些特殊的结构而引起的表面积增大比例优选从10~200%,也可以是从10~100%。其中,具有高表面/体积比的结构优选包括球和纤维,也包括像粉末和颗粒这样的其它微粒或它们的组合。这些结构的表面优选是光滑的。作为备选方案,在一些情况下也可以使用具有粗糙表面的结构。
专业人员将根据应用领域和有效性的不同选择这些内部结构的数量和形式。不言而喻的是,要选择上述提到的内部结构的形式和数量,使得由上述提到的内部结构的表面积与要改进的容器的内表面积组成的总和相互间进行调整,以获得按照本发明预期的表面/体积比(表面积指数)。
优选的,这个改进的容器具有无热原的内表面。优选的,这个改进的容器由无热原的材料制成。
如果使用微粒状的内部结构,如球、纤维、粉末、颗粒或它们的混合物,那么这种结构包括或优选由金属、金属氧化物或塑料和它们的混合物组成。优选的例子是玻璃、刚玉、石英、聚苯乙烯、聚氯乙烯、聚乙烯和聚丙烯以及它们的混合物。特别优选的是硼硅玻璃。这些材料优选无热原。
优选的,改进的容器至少在其内部含有玻璃球,这些玻璃球特别优选由无热原的硼硅玻璃组成,其(平均)直径为从0.5~5mm、优选1.5mm、2.5mm或3.5mm。特别优选的是,根据容器接受能力的不同,这个改进的容器中添加大约10~500个的玻璃球。如果一个容器预期要接收大约50ml血液,就应该优选添加大约30~300个球,特别的,大约50~250个球,其中球的直径优选为3.5mm。
在特别优选的实施例中,使用一个优选在输血医疗中已知的容器用于抽取或保存血液,如注射器、输血导管或血袋,其中这个容器通过添加一定比例的上述内部结构而进行改进,从而获得一个具有增大的内表面的改进的容器。相应地,本发明提供改进的容器的应用,该容器具有更大的按照本发明所述的表面积指数,该容器的内部结构选自球、纤维、粉末、颗粒、微粒或它们的混合物,用于制备一种调整的血液成分。
专业人员当然也可以使用其它或附加的方法来获得一个具有更大内表面的改进的容器。在另外一个优选的实施例中,人们设计了一个内容器壁具有***、空腔和/或突出部分的容器,从而达到按照本发明预期的表面/体积比(表面积指数)。
在一个优选的实施例中,所述改进的容器具有弹性的容器壁。当这个基本真空的改进的容器由于注入血液而膨胀,进而使得没有任何不需要的空气可以在容器中时,使得从动物或人体中真空抽取血液成为可能。不言而喻的是,在改进的容器中用于增大内表面而设计的内部结构的数量和面积并不针对弹性容器的最大容量,而是针对要培养的血液成分的体积而定。
优选的是,从在输血医学的血袋中选取这样一个弹性的容器,这些血袋优选为单袋、双联袋、三联袋或多袋***。其中,单袋***通常至少有一个开口用于填充或排空,双联袋、三联袋和多袋则具有多个相互连通的袋的排列,优选通过软管连接保持相互连通。优选的是,这样的血袋以简单的方式用两个相互焊接在一起的弹性片安装起来。
在一个特别优选的实施例中,按照本发明所述使用的改进的容器至少是一个血袋或调整的血袋***,所述调整的血袋***装入按照本发明所述数量和种类的微粒,优选玻璃球。
优选的是,这个容器是一个血袋***,它作为双联袋的血袋***用于离心分离新鲜抽取的血液。按照本发明,如果这个容器优选通过装入玻璃球而改进,那么可以实施按照本发明所述的用于制备一种调整的血液成分的方法。接下来,在血袋***中从所述调整的血液中分离这种血液成分,获得了不含“固体的”血液成分的调整的血清成分。
一个优选的双联袋血袋***含有至少一个主容器和至少一个次容器,它们构成了一个连通的容器***。主容器和次容器通过至少一个特别是可关闭的过渡管实现连接。结合本发明,一个“主容器”优选理解为一个容器,也就是料仓,在其中把要调整和要分离的血液成分加入并培养,并在需要时进行第一次分馏。特别优选的是,主容器、次容器和过渡管以有效的方式固定在一个载板上。特别优选的是,这个过渡通过至少一个可由阀门、齿轮和/或塞子组成的中断而关闭。“次容器”优选理解为一个容器,也就是料仓,在主容器中完全或部分分离的液体或悬浮液,再完全或部分地加入次容器并进行第二次分馏。这两个容器中的每一个,优选具有至少一个可关闭的输出和/或输入管,特别用于血液成分的加入(即输入或再使用)和/或血液成分的排出,即提取。优选的是,按照本发明所述的附加内部结构,如玻璃球,设计在主容器中或添加进其中。
在一个优选的实施例中,血袋或血袋***被***离心杯中,通过离心法从调整的血清成分中分离固体的血液成分。容器优选以弹性袋的形式在离心杯中拉伸,以致容器壁部分和/或完全贴在离心杯的内壁上。优选的是使用一个无菌烧杯。特别有利的是,容器壁和其中含有的细胞的拉伸应力在离心过程中得到了降低。离心烧杯的这种优选应用也允许把在力学上更轻、更细、更小的容器壁材料应用于优选的弹性血袋。
在一个优选的实施方法中,这些经过培养的血液成分是一种同种异源的血液成分,优选是一种以全血的形式从人体或动物供体中抽取并在按照本发明所述的方法实施之后作为调整的血液成分,优选作为由人类供体提供的调整的血清成分。在另外一种方法中,这种血液成分是自体同源的,即供体有机体和受体有机体是相同的。在这个特别优选的实施方法中,自体同源捐助的所有优点都可以体现出来。本领域的技术人员会根据应用和有效性的不同选择供体的种类和个体。其中,通常会考虑那些对于选择自体同源供体来说已知的标准和优势。
在一种备选的实施方法中,这种血液成分是异种的。这就是说,从一个异类的有机体中抽取血液。优选动物供体的有机体,例如一头猪中以全血的形式抽取非调整的血液成分,并在实施按照本发明所述的方法之后提供给其它属种,例如马、人或运动员中的待治疗个体。
本发明的另一方面是提供一种按照本发明所述的方法制备或优选制备的调整的血液成分。按照本发明所述,这种成分可以用于治疗、缓解、治愈或预防人体或动物体的疾病。按照本发明所述,这种血液成分含有诱导因子白介素-6至少30pg/ml,优选大于200、1000、5000、10000pg/ml,优选30~20000pg/ml。不言而喻的是,按照本发明所述的方法制备的调整的血液成分,除了作为诱导因子之一的白介素-6外,还含有其它诱导因子。令人惊奇地,这些在“鸡尾酒”中按照本发明所述获得的诱导因子的成分具有按照本发明所述的优点和效用。
优选的调整的血液成分,除了白介素-6(IL-6)之外,还含有至少一种成分,该成分选自:白介素-1受体拮抗剂(IL-1Ra)、白介素-4(IL-4)、白介素-13(IL-13)、白介素-1(IL-1)、白介素-10(IL-10)、肿瘤坏死因子(TNF)、***(IGF)、转化生长因子(TGF)、血小板衍生生长因子(PDGF)、成纤维细胞生长因子(FGF)和肝细胞生长因子(HGF)。
在一个实施例中,所述调整的血液成分含有白介素-1受体拮抗剂(IL-I Ra)的浓度为30~50000pg/ml。在另一个实施例中,所述调整的血液成分含有白介素-4(IL-4)的浓度为2~100pg/ml。在另一个实施例中,所述调整的血液成分含有白介素-13(IL-13)的浓度为2~100pg/ml。在另一个实施例中,所述调整的血液成分含有白介素-1(IL-1)的浓度为5~1000pg/l。在另一个实施例中,所述调整的血液成分含有白介素-10(IL-10)的浓度为5~1000pg/l。在另一个实施例中,所述调整的血液成分含有肿瘤坏死因子(TNF)的浓度为5~1000pg/ml。在另一个实施例中,所述调整的血液成分含有***(IGF)的浓度为100~15000pg/ml。在另一个实施例中,所述调整的血液成分含有转化生长因子(TGF)的浓度为10~20000pg/ml。在另一个实施例中,所述调整的血液成分含有血小板衍生生长因子(PDGF)的浓度为100~10000pg/ml。在另一个实施例中,所述调整的血液成分含有成纤维细胞生长因子(FGF)的浓度为50~10000pg/ml。在另一个实施例中,所述调整的血液成分含有肝细胞生长因子(HGF)的浓度为50~10000pg/ml。
令人惊奇的是,所述调整的血液成分中存在的诱导因子和细胞因子组成的“鸡尾酒”在预防和治疗上特别有效。按照本发明所述,通过使用这些调整的血液成分或从中获得的调整的血清成分,在一系列人体或动物体疾病或病症中产生了特别的疗效,疾病或病症被治疗、治愈或缓解,或者可以用它们针对这些疾病和病症进行预防。
按照本发明所述,这些按照本发明所述调整的血液成分或血清成分在肌肉疾病、骨骼和运动器官疾病以及在神经***炎症和病变,主要是肌腱组织疾病,如肌腱损伤、腱鞘炎、韧带损伤、肌腱萎缩和韧带萎缩中使用;以及用于快速治疗、缓解或预防针对食品或药品的不耐受和过敏症,涉及免疫***的疾病,主要是自体免疫性疾病,特别是类风湿性疾病以及由神经性皮炎引发的疾病;用于治疗和治愈慢性病,特别是糖尿病性溃疡,治疗子宫内膜异位症;以及治疗慢性眼炎或在马中的肌腱病变的再生或疼痛缓解。肌肉疾病包括:由于肌切开术引起的肌肉损伤以及在肌肉退化、肌肉损伤、肌肉萎缩、肌突出、肌肉营养不良中引起的肌纤维撕裂,或肌肉疲劳或肌肉酸痛。
另外,本发明涉及所述血液成分的应用,用来治疗或预防人体或动物体的疾病。优选被治疗、缓解、治愈或进行预防的人体或动物体的疾病选自:类风湿性疾病、肌肉骨骼***疾病以及与免疫***相关的疾病以及可导致急性或慢性疼痛的疾病。
不言而喻的,为了针对每种疾病进行合适的治疗,专业人员根据有效的应用类型选取按照本发明所述的调整的血液成分。优选的是,在身体和/或像关节、肌肉、肌腱、皮肤或神经中这样的相关器官中注射或注入调整的血液成分或血清成分,优选静脉、动脉、皮下、皮内、结膜下、局部、鞘内、脊椎硬膜,在中枢神经内和/或旁、在末梢神经内和/或旁、关节内和/或肌肉内。
本发明的另外一个方面,提供按照本发明所述的调整的血液成分用于制备治疗或预防人体或动物体疾病的药品。这些疾病如前文所述。
此外,按照本发明所述的血液成分的应用,也用作非治疗性的化妆品,也就是所谓的“抗衰老”药物。显而易见的是通过***和/或局部的给药,与年龄有关的身体机能,特别是上述症状会减缓或被治愈,而且会出现这样的情况:皮肤、头发、指甲变得更好。在本发明所述的另外一个方面,这种按照本发明所述的血液成分可以用于制备化妆品。
最后,本发明也涉及一种用于治疗或预防上述人体或动物体疾病的方法,至少包含下述步骤:在人体或动物体内以在治疗或预防上有效的剂量应用按照本发明所述的调整的血液成分。本领域的技术人员会根据应用领域和有效性的不同选择剂量和应用种类。

本发明通过下述实例和图示进行了进一步解释,其中这些实例可以不受限制的理解。本领域技术人员将从这些实例中了解本发明的基本原理和与之相联系的技术优势。他有能力把这些基本原理和技术优势运用在其它在这里没有明确指出的领域中。
图1显示了按照本发明所述的装置的一个优选的实施例的图示,该装置包括一个作为弹性血袋的容器10,该容器具有一个优选的半圆形下截面14和一个优选的带有至少一个输入和/或排出管11的尖端变窄的上截面15,所述管通向容器的漏洞型上截面15。在通向容器内腔10的下边缘,优选设置一个矛形阀13,即百叶阀。
实施例1:用于获取全血中调整的血液成分的试剂盒
一个灭菌的一次性使用的试剂盒,包括:
- 一个用于培养血液和分离固体的血液成分的血袋***(表1和2、图1),装入了直径为3.5mm的硼硅玻璃球(大约200个),
- 用于培养在抽血注射器中的抗凝血剂的20标准尺的针头,
- 用于抽血的60ml注射器,
- 用于抽血的蝴蝶针头,
- 用于抽取调整的血清成分的60ml注射器,
所有部件都是一次性用品,有包装并且经过了伽马射线消毒,同时整体具有经过消毒的外包装。

实施例2:获取全血中的调整的血清成分
a)抽血
用一个60ml Luerlock-注射器抽血。在这个注射器中缓慢充满60ml全血。要注意进行无气泡的填充,以便实际上在注射器中有正好60ml全血。
b)填充血袋***和培养
在注入完成之后,注射器没有被扭紧,用一个新的堵头重新将接口12封住。
容器10在大约37℃下储存9~36小时,优选悬挂存放。抽取的血液在容器中通过增大内表面的球进行培养。所述增大的内表面有约350mm2/ml培养的血液。
c)分离固体的血液成分
在一个无菌的离心悬挂装置中,把容器10放入到一个离心杯中。在控制好正确的重量平衡之后,在大约2500转/分钟下离心分离大约3分钟。在离心完成后,即从液体血液成分中分离细胞后,小心取回离心杯和容器10。
通过离心分离,血细胞,主要是红细胞(EZ)在容器10的下层聚集了起来。离心分离把血清从血块中分离出来。这些血清被转移到第二个血袋中,在需要时进行第二次离心分离。通过输入/排出管11的取样口,取出这些调整的血清成分。然后把充满的注射器拧开。
实施例3:分析这些调整的血清成分
制备五个在相同的方式下培养全血的试验装置A、B、C、D和E。
在装置A中,使用实施例1和2中描述过的一个商用血袋(OSTEOKIN,Orthogen,杜塞尔多夫),用210个硼硅玻璃材质()、直径3.5mm的球填充。通过加入这些内部结构,改进的容器的内表面积总共大约为18125mm2。在培养50ml血液时,改进的容器的表面/体积比(表面积指数)大约为360mm2/ml。
在另外一个装置B中,使用的是与在装置A中同样的血袋***,只是其中没有加入任何内部结构。这个未填充的血袋***具有一个大约10000mm2的内表面。在50ml血液中,表面积指数大约为200mm2/ml。
在另外一个装置C中,使用的是与在装置A中同样的血袋***,并用780个直径3.5mm的玻璃球填充。这样,其内表面积共计大约40000mm2。在50ml血液容量中,表面积指数大约为800mm2/ml。
在另外一种装置D中,使用一种具有主要是圆柱形状的采血***,用直径1.5mm的36个玻璃球填充。这样,其内表面积共计大约4050mm2。在10ml血液容量中,表面积指数大约为405mm2/ml。
在另外一种装置E中,使用一种具有主要是圆柱形状的采血***,用直径3.5mm的62个玻璃球填充。这样,其内表面积共计大约6200mm2。在10ml血液容量中,表面积指数大约为620mm2/ml。
在所有的试验装置中,每次都新鲜抽取静脉全血并注入到装置A、B和C中的容器中。这些容器在大约37℃下培养24个小时(t=24h)。此外作为对照,每次在采血后直接从同样的供体中重新采集大约10ml新鲜全血(t=0h)。
在培养结束以后,分析在血液成分中的这些成分IL-I Ra、IL-6、TNFa和IL-1β。
结果:表3显示了这个结果。

装置A和B显示出,在这种血液成分中经过分析的因子产生了明显的诱导,与此相比在装置C中,在培养过程中出现了溶血。显而易见的是,诱导的强度取决于表面积指数。在更大的表面积指数(更大的内表面)下,会含有更高比例的诱导细胞因子。同时,表面积指数有一个上限,如果超过了临界值,就会出现溶血。而有溶血问题的血液成分,不能再继续使用。在接近临界值的高表面积指数下,可以通过把培养时间从24小时缩短为6~9小时(数据未显示),把溶血抑制在一个特定的范围内。
实施例4:调整的血液成分中的细胞因子浓度

实施例5:神经性皮炎的治疗
为了治疗神经性皮炎,使用按照本发明所述制备的调整的血液成分,以注射和关节内注射的形式为病人治疗。在3周的时间内,以每2~3天为跨度注射2ml调整的血液成分。在3天之内就可以确定,神经性皮炎的症状有改善。在大约两个半月之后疾病重新发作时,同样用3次注射成功完成了治疗。
使用关节内注射,最初用于治疗膝盖疼痛(由关节炎和半月板负担引起)的病人中,经过6次注射之后,这种神经性皮炎症状也得到了改善。从那以后,也没有出现过神经性皮炎复发的情况。
实施例6:神经***炎症或病变的治疗
对至少6个月以来一直慢慢忍受根源性背痛的背痛病人(n=30),应用按照本发明所述制备的调整的血液成分,通过在神经根(硬脑外膜注射,gem,等)的局部注射进行了治疗。这种疼痛在几周后得到了缓解,并且疗效还可以持续六个月。与那些使用同样注射技术注射5mg或10mg糖皮质激素(去炎松)作为对比物质治疗的病人相比,这种方法显示出了至少相同甚至更好的效果。
实施例7:子宫内膜异位症的治疗
对于忍受痛苦的子宫内膜异位症的女病人(n=4),用4ml按照本发明所述制备的调整的血清通过腹膜内注射直接在由子宫内膜异位症引起的新生物和/或在腹腔中进行治疗。开始伴有强烈疼痛之后,疼痛在几个小时之内得到了明显的缓解。这个效果得到了延续,女病人在第二天几乎完全没有了疼痛。作为治疗的延续,每周通过皮下注射2ml调整的血清继续进行治疗。到现在为止,还没有观察到疼痛的复发情况。令人惊奇地,按照本发明所述的调整的血液成分缓解疼痛的效果,要比普通的镇痛药持续更长的时间。
实施例8:在马中的慢性眼炎
为了治疗在马中的慢性眼炎(马周期性复发葡萄膜炎,ERU),把按照本发明所述制备的调整的血液成分,注射到6匹马的眼中(结膜下)或滴到眼中(局部),这6匹马中的每3匹分成两个不同的对照组进行治疗。在直到10个月的复查时间内,都没有在任何一匹经过治疗的马中发现复发的情况。
实施例9:马肌腱发炎的恢复和疼痛减轻
在另外一个调整的血液成分的应用中,对由于腱鞘大规模发炎或刺激而导致形成腱鞘瘤而变瘸的马,在其腱鞘中注射3ml按照本发明所述的调整的血清进行治疗。为此,首先在第一步中刺破这个瘤以减轻这部分组织的压力并除去发炎的物质。在第一次注射之后,在一周之内瘸的情况有所减轻,在第二周时,可查出的瘤量明显减少。在4周,也就是在腱鞘中注射完第三次和最后一次的剂量之后,可以确定的是无论是瘸还是瘤都几乎完全没有了。
所谓的“核心病变”和/或“表面病变”,也就是肌腱组织中的退化性变化中的类似注射,也同样可以明显消除这些临床症状。某些时段,也观察到了通过胶原纤维重新补充损伤的情况。
实施例10:治疗在马中的伤口
一匹14岁、有多个关节变瘸的去势的马,在很多周以前就一直忍受着左前蹄上持续的伤口疼痛。为此,把按照本发明所述的调整的血液成分滴到这个伤口上(在大约1×3cm的面积上滴3滴)。接下来,这个伤口开始愈合了。经过4周之后,最后鉴定(在以周为单位的三次治疗之后)可以看出,该开放的伤口表面减小了大约三分之一。
Claims (36)
1、一种从血液制备调整的血液成分的方法,其特征在于包含下述步骤:
(a)从人体或动物体内抽取血液;
(b)所述血液在一个具有内表面的改进的容器中、在10~40℃的温度下培养,以通过诱导因子来调整血液,其中,所述改进的容器具有104~750mm2/ml培养的血液的内表面积;和
(c)在所述改进的容器中获得具有诱导的因子的调整的血液成分。
2、根据权利要求1所述的方法,其特征在于,如果白介素-6(IL-6)在血液成分中的含量达到至少30pg/毫升,则表示诱导成功。
3、根据前述任一项权利要求所述的方法,其特征在于,培养时间为2~36小时。
4、根据前述任一项权利要求所述的方法,其特征在于,培养过程中的氧分压(pO2)小于5kPa。
5、根据前述任一项权利要求所述的方法,其特征在于,还包括从所述调整的血液成分中分离细胞成分的步骤,并因此获得一种调整的血清成分。
6、根据前述任一项权利要求所述的方法,其特征在于,所述改进的容器具有大表面的内部结构,所述内部结构选自:球、纤维、粉末、颗粒、微粒以及它们的组合。
7、根据权利要求6所述的方法,其特征在于,所述内部结构含有至少一种材料,所述材料选自:金属、金属氧化物和塑料,诸如玻璃、刚玉、石英、聚苯乙烯、聚氯乙烯、聚乙烯、聚丙烯或它们的组合。
8、根据前述任一项权利要求所述的方法,其特征在于,所述改进的容器在其内部具有直径为0.5~5mm的玻璃球。
9、根据前述任一项权利要求所述的方法,其特征在于,所述改进的容器具有用于从动物体或人体真空地抽取血液的弹性容器壁。
10、根据权利要求9所述的方法,其特征在于,所述容器选自用于输血医学的血袋。
11、根据权利要求10所述的方法,其特征在于,所述容器选自单袋、双联袋、三联袋和多联袋***。
12、根据权利要求9~11中任一项所述的方法,其特征在于,所述弹性容器壁具有低的氧渗透性。
13、根据前述任一项权利要求所述的方法,其特征在于,所述血液成分是同种异源的。
14、根据前述任一项权利要求所述的方法,其特征在于,所述血液成分是自体同源的。
15、根据前述任一项权利要求所述的方法,其特征在于,所述血液成分是异种的。
16、一种根据前述任一项权利要求所述的方法制备的血液成分,用于治疗或预防人体或动物体的疾病,其特征在于含有30~20000pg/ml的白介素-6(IL-6)。
17、根据权利要求16所述的血液成分,其特征在于还含有至少一种成分,该成分选自:
-白介素-1受体拮抗剂(IL-1Ra),
-白介素-4(IL-4),
-白介素-13(IL-13),
-白介素-1(IL-1),
-白介素-10(IL-10),
-肿瘤坏死因子(TNF),
-***(IGF),
-转化生长因子(TGF),
-血小板衍生生长因子(PDGF),
-成纤维细胞生长因子(FGF),和
-肝细胞生长因子(HGF)。
18、根据权利要求16或17所述的血液成分,其特征在于,还含有至少一种成分,该成分选自:泡囊、微泡囊、外来体、免疫核糖核酸(iRNA)和它们的混合物。
19、根据权利要求16~18所述的血液成分,其特征在于,白介素-1受体拮抗剂(IL-1Ra)的浓度为30~50000pg/ml。
20、根据权利要求16~19所述的血液成分,其特征在于,白介素-4(IL-4)的浓度为2~100pg/ml。
21、根据权利要求16~20中任一项所述的血液成分,其特征在于,白介素-13(IL-13)的浓度为2~100pg/ml。
22、根据权利要求16~21中任一项所述的血液成分,其特征在于,白介素-1(IL-1)的浓度为5~1000pg/ml。
23、根据权利要求16~22中任一项所述的血液成分,其特征在于,白介素-10(IL-10)的浓度为5~1000pg/ml。
24、根据权利要求16~23中任一项所述的血液成分,其特征在于,肿瘤坏死因子(TNF)的浓度为5~1000pg/ml。
25、根据权利要求16~24任一项所述的血液成分,其特征在于,***(IGF)的浓度为100~150000pg/ml。
26、根据权利要求16~25中任一项所述的血液成分,其特征在于,转化生长因子(TGF)的浓度为100~20000pg/ml。
27、根据权利要求16~26中任一项所述的血液成分,其特征在于,血小板衍生生长因子(PDGF)的浓度为100~100000pg/ml。
28、根据权利要求16~27中任一项所述的血液成分,其特征在于,成纤维细胞生长因子(FGF)的浓度为50~10000pg/ml。
29、根据权利要求16~28中任一项所述的血液成分,其特征在于,肝细胞生长因子(HGF)的浓度为10~10000pg/ml。
30、根据前述权利要求16~29中任一项所述的血液成分的应用,用于治疗或预防人体或动物体的疾病,所述疾病选自:。
-肌肉疾病,
-肌腱组织疾病,
-过敏症,
-食品耐受不良,
-药品耐受不良,
-涉及免疫***的疾病,
-牛皮癣,和
-慢性病如糖尿病性溃疡。
31、根据权利要求30所述的应用,其特征在于,所述肌肉疾病为肌肉损伤、肌切开术、肌纤维撕裂、肌肉退化、肌肉损坏、肌肉萎缩、肌脱出、肌肉营养不良、肌肉疲劳或肌肉酸痛。
32、根据权利要求30或31所述的应用,其特征在于,所述肌肉疾病的治疗包括肌肉组织的再生。
33、根据前述权利要求16~29中任一项所述的血液成分的应用,其特征在于,用于治疗或预防人体或动物体的疾病,这些疾病选自:
-神经性皮炎,
-神经***炎症或病变,
-子宫内膜异位症,
-在马中的慢性眼炎。
34、根据权利要求30~33中任一项所述的应用,其特征在于,所述血液成分在需要时与药物赋形剂一起注射到身体或相关的器官。
35、根据前述权利要求16~29中任一项所述的血液成分的应用,其特征在于,用于制备治疗或预防权利要求30~33中任一项所述的人体或动物体的疾病的药品。
36、根据前述权利要求16~29中任一项所述的血液成分的应用,其特征在于,用作化妆品。
