Description
Der Erfindung betrifft allgemein in der Urologie bei verschiedenen pathologischen Zuständen angewendete Massnahmen zum Erzielen und Aufrechterhalten einer Drainage durch den Harnleiter. Die Intubation eines Harnleiter-Dehnkatheters ist häufig bei Patienten notwendig, die an bösartigen Erkrankungen, fortschreitender Azotämie und Verlegungen des Harnleiters leiden. Derartige Verlegungen können durch Prostata-, Blasen-, Gebärmutterhals-, Gebärmutter-, Eierstock- und Rektosigmoid-Krebs verursacht werden. Dehnkatheter werden im allgemeinen nur zeitweilig verwendet, bis eine Operation vorgenommen werden kann. Es gibt aber auch Dehnkatheter, die zum Zweck der Umleitung und zur Linderung oder zum Beseitigen von Schmerzen dauernd verwendet werden.
Die Verwendung von Dehnkathetern ist aus der medizinischen Literatur und der allgemeinen Praxis bekannt. Harnleiterdehn-Dauerkatheter unterstützen auch das Fluchten des Harnleiters und das Aufrechterhalten des Harnleiterlumens. Man kann die Katheter entweder mit Hilfe eines Endoskops oder bei einer Bauchoperation einsetzen, wenn die Entleerung der Niere in die Blase durch den anatomischen Harnleiter selbst nicht gewährleistet ist.
Harnleiter-Dehnkatheter sind in der Technik bekannt. In den letzten Jahren wurde besonderer Wert darauf gelegt, die Katheter mit Mitteln zu versehen, die ein Auf- oder Abwärtswandern des Katheters verhindern. Beispielsweise gemäss der US-PS 3 938 529 (Gibbons) kann dies beispielsweise durch die Verwendung von seitwärts abstehenden, flexiblen Widerhaken gewährleistet werden. Durch derartige Widerhaken wird aber der Durchmesser des Dehnkatheters ver-grössert und das Einführen des Katheters erschwert. In manchen Fällen bewirken diese Widerhaken, dass der Dehnkatheter aus der Blase herauswandert, so dass der Arzt mit Problemen konfrontiert wird. Gemäss der US-PS 4 212 304 (Finney) ist ein Harnleiter-Dehnkatheter mit einander entgegengesetzten hakenförmigen Festlegeelementen versehen. Derartige Dehnkatheter müssen in zahlreichen unterschiedlichen Längen vorhanden sein, aus denen die für den jeweiligen Patienten geeignete ausgewählt wird. Eine Harnleiterprothese ist in der US-PS 4 225 979 (Rey und Mitarb.) beschrieben.
Angabe und Aufgaben der Erfindung
Die Erfindung hat einen neuen, mit einer Wendel ausgebildeten Dehnkatheter geschaffen, der einfach ausgebildet und relativ leicht mit Hilfe eines Cystoskops einführbar ist und von dem Patienten gefahrlos verwendet werden kann. Dank der neuartigen Ausbildung des Dehnkatheters mit einer Wendel kann dieser nicht aufwärts oder abwärts wandern. Er hat jedoch eine nichttraumatische Federungswirkung, wenn sich der Patient bewegt.
Der neue Dehnkatheter braucht für jede französische Grösse (jeden Durchmesser) nur in einer Länge vorhanden zu sein. Dies wird durch die besondere Ausbildung des distalen oder blasenseitigen Endteils des Dehnkatheters ermöglicht. Dank dieser Ausbildung ist zum Unterschied von den bekannten Dehnkathetern vor dem Einführen des Katheters keine Röntgenuntersuchung zur Grössenbestimmung erforderlich und kann es nicht vorkommen, dass ein zu langer oder zu kurzer Dehnkatheter eingeführt wird.
Die Aufgabe der Erfindung besteht in der Schaffung eines neuartigen Harnleiter-Dehnkatheters, der mindestens an seinem einen Ende mit einer neuartigen Festlegeeinrichtung versehen ist.
Eine weitere Aufgabe der Erfindung ist die Schaffung eines neuartigen, mit einer Wendel ausgebildeten Harnleiter-Dehnkatheters, der leicht einführbar ist und nur in einer einzigen Länge vorgesehen zu werden braucht, so dass eine Röntgenuntersuchung zur Grössenbestimmung nicht erforderlich ist.
Ferner besteht eine Aufgabe der Erfindung in der Schaffung eines neuartigen, mit einer Wendel ausgebildeten Dehnkatheters, der relativ billig ist und gefahrlos und mit guter Wirkung verwendet und auf übliche Weise eingeführt werden kann.
Der erfindungsgemässe Harnleiter-Dehnkatheter ist im Anspruch 1 definiert.

Ausführliche Beschreibung Gemäss der Figur 1 ist der Harnleiter-Dehnkatheter 10 gemäss der Erfindung in dem befallenen Harnleiter 11 angeordnet, der das Nierenbecken 12 mit der Blase 13 verbindet. Entsprechend der üblichen medizinischen Terminologie wird das in die Niere eingeführte Ende des Dehnkatheters als dessen proximales Ende und das in der Blase befindliche, entgegengesetzte Ende des Dehnkatheters als dessen distales Ende bezeichnet.
Der Dehnkatheter kann aus jedem beliebigen biologisch verträglichen Material hergestellt werden und besteht vorzugsweise aus einem weichen, flexiblen, für medizinische Zwecke geeigneten Silikonkunststoff. Dieses Material verhindert ein Ankleben von Gewebe und eine Krustenbildung. Um die Einführung des Katheters zu erleichtern, ist der Werkstoff strahlenundurchlässig. Der Dehnkatheter kann verschiedene Aussendurchmesser haben, z.B. entsprechend den französischen Grössen 6,7 und 8. Für alle Zwecke ist jedoch nur eine einzige Länge erforderlich, und es hat sich für diesen Zweck eine Länge von 31 cm als ideal erwiesen. In der in der Figur 2 gezeigten, bevorzugten Ausführungsform der Erfindung ist der Dehnkatheter an seinem proximalen Ende mit einer relativ kurzen Wendel 14 ausgebildet, die beispielsweise nur eineinhalb Windungen hat. An seinem distalen Ende ist der Dehnkatheter mit einer längeren Wendel 15 ausgebildet, die in dem dargestellten Ausführungsbeispiel fünf Windungen besitzt. Man erkennt, dass die Längsachse der Wendeln zu dem Hauptteil 16 des Dehnkatheters parallel, in der bevorzugten Ausführungsform jedoch gegenüber diesem Hauptteil versetzt ist.
Über die ganze Länge des mit Wendeln ausgebildeten Dehnkatheters sind Drainageöffnungen 17 vorgesehen, die zu dem Lumen des Katheters führen. Bei der Herstellung des Dehnkatheters können beide Enden verschlossen werden. Nachstehend wird jedoch erläutert, dass beim Einführen des Dehnkatheters dessen distales Ende aufgeschnitten wird, damit ein Schubkatheter mit einer Federdrahtführung verwendet werden kann. In der Figur 2 ist die zum Einführen verwendete Federdrahtführung mit 18 und der aus Kunststoff bestehende Schubkatheter mit 19 bezeichnet.
In der in der Figur 4 gezeigten, abgeänderten Ausführungsform ist der rohrförmige Hauptteil 20 des Dehnkatheters auf der Achse der Wendeln zentriert.
Die Ausführungsform gemäss der Figur 5 entspricht im wesentlichen der bevorzugten Ausführungsform, doch sind gremäss der Figur 5 die an den Enden vorgesehenen Wendeln mit konstantem Radius ausgebildet, während sie in der bevorzugten Ausführungsförm spiralförmig sind.
In der in der Figur 6 gezeigten Ausführungsform der Erfindung ist die am distalen Ende ausgebildete Wendel wie gemäss der Figur 4 koaxial ausgebildet und ist am proximalen Ende anstelle der Wendel ein J-förmiger Haken vorgesehen.
In jeder der dargestellten Ausführungsformen verhindern die Festlegewendeln oder der Haken ein Aufwärts- oder Abwärtswandern des Dehnkatheters. Dabei ist dieser aber weich und geschmeidig und bewirkt er eine nichttraumatische Federung, besonders wenn sich der Patient bewegt.
Der Harnleiter-Dehnkatheter gemäss der Erfindung wird auf übliche Weise ausgewählt und eingeführt. Wie bei jedem Harnleiter-Dehnkatheter wählt der Chirurg die richtige französische Grösse aus. Der Harnleiter-Dehnkatheter gemäss der Erfindung kann in 11 bis 30 cm langen Harnleitern verwendet werden. Bei ganz gestrecktem Dehnkatheter muss mindestens eine Windung am distalen Ende des Dehnkatheters in der Blase bleiben, damit der Katheter nicht aufwärtswandern kann.
Wenn der Dehnkatheter mit Hilfe eines Endoskops eingeführt werden soll, wird er am distalen Ende aufgeschnitten, wie dies vorstehend angegeben wurde. Durch das aufgeschnittene Ende des Dehnkatheters wird die Federdrahtführung 18 in das Lumen des Katheters eingeführt. Dann wird der Katheter auf den Draht aufgefädelt. Dadurch werden die Festlegewendeln gestreckt. Es muss darauf geachtet werden, dass die Drahtführung nicht durch den proximalen Endteil des Dehnkatheters gedrückt wird. Danach wird der aus Kunststoff bestehende Schubkatheter aufgeschoben. Mit ihm wird der Dehnkatheter in Stellung gehalten, wenn nach dem Einführen des Dehnkatheters die Drahtführung aus dem Dehnkatheter herausgezogen wird.
Die so gebildete Anordnung wird wie ein üblicher Harnleiterkatheter eingeführt. Der Eintritt des Katheters in das Nierenbecken kann durch eine Röntgenaufnahme bestätigt werden. Dann wird die Federdrahtführung vorsichtig zurückgezogen, während der aus Kunststoff bestehende Schubkatheter in Stellung gehalten wird. Nach dem vollständigen Herausziehen der Drahtführung 18 und des Schubkatheters 19 bleibt der Dehnkatheter in Stellung und können gegebenenfalls in der Blase befindliche, überschüssige Windungen mit Hilfe eines Endoskops abgeschnitten und entfernt werden, wenn dies erwünscht ist. Der Chirurg muss jedoch darauf achten, dass mindestens eine Festlegewindung in der Blase bleibt, damit der Dehnkatheter nicht aufwärtswandern kann.
Der Dehnkatheter gemäss der Erfindung kann auch während einer Bauchoperation in den Harnleiter eingeführt werden. In diesem Fall wird das distale Ende nicht abgeschnitten und wird kein Schubkatheter 19 aus Kunststoff benötigt. Die Federdrahtführung 18 wird durch eine der seitlichen Drainageöffnungen 17 in den Dehnkatheter eingeführt. Die Wendel wird gestreckt und der Dehnkatheter wird durch einen chirurgisch hergestellten Einschnitt in den Harnleiter eingeführt. Wenn das blasenseitige Ende zuerst eingeführt wird, kann der Arzt den Eintritt des Dehnkatheters in die Blase kontrollieren, indem über eine am Dehnkatheter an dem der Eintrittsstelle in den Harnleiter nächstliegenden Drainageöffnung angebrachte Spritze Harn angesaugt wird. Auf dieselbe Weise wird der Dehnkatheter in das Nierenbek-ken eingeführt. Natürlich muss der Arzt entscheiden, ob der Dehnkatheter wieder entfernt wird oder nicht. Der Dehnkatheter gemäss der Erfindung kann mit Hilfe von Zangen unter Kontrolle durch ein Endoskop herausgenommen werden.
Im Rahmen der unter Schutz gestellten Erfindung sind verschiedene Abänderungen der vorstehend beschriebenen Ausführungsformen möglich. Beispielsweise kann man den Dehnkatheter gegebenenfalls mit Mitteln zum Versteifen der proximalen und distalen Wendel versehen. Zu diesem Zweck kann man in den Wendeln Faserstoffe oder Metall oder Kunststoff anordnen, um die Wendeln zu versteifen und ihr Wandern zu erschweren. Ferner kann man auf dem Dehnkatheter durch Aufmalen oder auf andere Weise Marken oder Zeichen aufbringen, die bei Röntgenuntersuchungen sichtbar sind.

Harnleiter-Dehnkatheter zum Gewährleisten einer Drainage zwischen der Niere und der Blase mit einem langgestreckten, flexiblen rohrförmigen Element, das über seine Länge mit Drairiagemitteln versehen ist, ferner mit einer am proximalen Ende des rohrförmigen Elements vorgesehenen Festlegeeinrichtung, die in die Niere einfuhrbar ist, und mit mehreren Wendelwindungen, die am distalen Ende de rohrförmigen Elements angeordnet sind und von denen mindestens eine geeignet ist, im Gebrauch in der Blase zu verbleiben, wobei die Mittellinie der von den Windungen gebildeten Wendel im wesentlichen parallel oder koaxial ist zu jenem Teil des rohrförmigen Elements, der sich zwischen den Windungen und der am proximalen Ende vorgesehenen Festlegeeinrichtung befindet.
- 2. Harnleiter-Dehnkatheter nach Anspruch 1, dadurch gekennzeichnet, dass das rohrförmige Element innen ein Lumen besitzt und die Drainagemittel aus mehreren Öffnungen besteht, die in das Lumen führen.
- 3. Harnleiter-Dehnkatheter nach Anspruch 2, dadurch gekennzeichnet, dass die Drainageöffnungen über die ganze Länge des Dehnkatheters verteilt und im Bereich des proximalen Endes stärker konzentriert sind.
- 4. Harnleiter-Dehnkatheter nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die am proximalen Ende angeordnete Festlegeeinrichtung aus mindestens einer Wendelwindung besteht, die mit den erstgenannten Windungen im wesentlichen koaxial ist.
- 5. Harnleiter-Dehnkatheter nach Anspruch 4, dadurch gekennzeichnet, dass die Achse der von den Windungen gebildeten Wendel gegenüber jenem Teil des rohrförmigen Elements versetzt ist, der sich zwischen den Windungen und der am proximalen Ende vorgesehenen Festlegeeinrichtung befindet.
- 6. Harnleiter-Dehnkatheter nach Anspruch 4, dadurch gekennzeichnet, dass die Achse der von den Windungen gebildeten Wendel mit jenem Teil des rohrförmigen Elements koaxial ist, der sich zwischen den Windungen und der am proximalen Ende vorgesehenen Festlegeeinrichtung befindet.
- 7. Harnleiter-Dehnkatheter nach Anspruch 1, dadurch gekennzeichnet, dass die Festlegeeinrichtung einen Haken aufweist, der sich über weniger als 360° erstreckt.Grundlage und Stand der Technik
