Abstract
本開示は、温度応答性ポリマーを含有する、眼圧上昇剤を提供する。本発明にかかる眼圧上昇剤を眼の特定領域に投与することで、眼圧を上昇させることができ、且つ、必要に応じて温度により眼圧を制御することができる。
Description
本発明は、眼圧上昇剤、特に、温度制御により薬効解消可能な眼圧上昇剤に関する。
眼球が正常の形態や機能を維持するためには一定の眼圧(例えば、10~20mmHg程度)が必要であると考えられている。これに対して、眼圧が上昇すると失明リスクの高い緑内障が発症および進行するため眼圧下降治療が積極的に行われている。しかし、逆に、眼圧が低下しても眼球に歪みが生じ、美容上の問題だけでなく、黄斑症、毛様体剥離等の深刻な視力障害が生じ得る(図1)。低眼圧症は深刻な視覚障害の原因となるが標準的な治療方法がないのが現状である。
眼圧は、毛様体上皮細胞における「眼房水の産生」と、角膜と虹彩の境界部にある隅角からシュレム管を経由する「眼房水の排出」で調節されている。低眼圧症の原因として、毛様体無色素上皮細胞障害による眼房水産生能の低下と、隅角部からの房水排出の亢進が挙げられる。眼房水産生能の低下に対しては細胞や組織の再生が必要になり極めて難易度が高いため、隅角部からの眼房水の排出を低下させることが現実的な手段である。
現状、医薬品として認可を受けた眼圧上昇剤は存在しない。外科的治療方法としては、(1)ヒアルロン酸等の粘弾性物質、空気の眼内注入、(2)隅角部の縫合及び(3)レーザー照射による隅角の癒着等が挙げられる。
非特許文献1は、動物実験レベルでは樹脂ビーズを房水に注入して隅角およびシュレム管に詰める方法を開示している。

非特許文献1が開示する方法は、房水排出口を樹脂ビーズにより物理的に封止するため、眼圧を上昇させるには適した手法である。しかし、樹脂ビーズは房水排出口から除去ができず、眼圧が上がりすぎた場合にその眼圧を下げることができないと考えられる。したがって、従来技術では、眼圧が上がり過ぎないように処置することが求められる。
本発明者らは、温度応答性ポリマーを含む眼圧上昇剤を房水に投与することにより眼圧を上昇させる方法、および、眼圧上昇が過剰であった場合には眼球を冷却することにより上昇した眼圧を低下させることができる方法、並びにこれらの方法において有効な温度応答性ポリマーを含む組成物を見出した。
本発明にかかる眼圧上昇剤を眼の特定領域に投与することで、眼圧を上昇させることができ、且つ、温度により眼圧を制御することができる。
上記眼圧上昇剤は、温度制御により薬効解消可能であってもよい。上記温度応答性ポリマーは、対象の眼組織に存在する眼内循環液の温度では凝集し且つ上記眼内循環液の温度を低下させるにつれて線状化(すなわち、可溶化)してもよい。上記対象の上記眼組織は、上記温度応答性ポリマーが投与される第一領域と、上記第一領域と流体連通する第二領域を有していてもよい。上記眼内循環液は、上記第一領域から上記第二領域に流れてもよい。上記第一領域から上記第二領域への上記眼内循環液の流れは、凝集した上記温度応答性ポリマーによって阻害されてもよい。上記眼内循環液の流れ阻害は、凝集した上記温度応答性ポリマーが線状化するにつれて解消してもよい。上記眼圧上昇剤は、上記第一領域から上記第二領域への上記眼内循環液の流れを阻害する量の上記温度応答性ポリマーを含んでいてもよい。上記第一領域は、隅角であってもよい。上記第二領域は、シュレム管であってもよい。上記温度応答性ポリマーは、オキサゾリンポリマーであってもよい。
また、本発明の別の目的は、上記眼圧上昇剤を含む、低眼圧症を治療するための医薬組成物を提供することである。
本発明にかかる医薬組成物は、低眼圧症を治療することができる。
また、本発明の別の目的は、低眼圧症の治療を必要とする対象の眼組織に上記眼圧上昇剤又は医薬組成物を投与する工程を有する低眼圧症を治療する方法を提供することである。
本発明にかかる治療方法によって低眼圧症を治療することができる。
本発明によれば、例えば、以下の発明が提供される。

本明細書では、眼圧上昇剤は、対象において眼圧を上昇させることに用いるための組成物を意味する。本実施形態にかかる眼圧上昇剤は、温度応答性ポリマーを含有する。本開示の眼圧上昇剤は、眼球、特に前房に注射され、眼圧を上昇させ得る。眼圧上昇剤は、温度応答性ポリマーの温度を制御すること(特に冷却すること)によって薬効、即ち、眼圧の上昇を解消することができる。すなわち、眼圧上昇剤は前房に注射されることにより眼圧を上昇させるが、上昇した眼圧を有する眼球を冷やすことによって眼圧の上昇を低下または解消することができる。この目的のため、0℃から体温(例えば、36℃~37℃)の温度範囲内にLCSTを有する温度応答性ポリマーが、好ましく用いられ得る。上記温度範囲にLCSTを有する温度応答性ポリマーは、体温において不溶化し、房水排出口を塞ぐ等して房水の眼球からの排出を阻害することができると共に、眼球を生理的に許容可能な限度でLCST未満に冷却すると、当該温度応答性ポリマーは可溶化して房水の眼球からの排出の阻害を停止する。このように、本開示によれば、0℃から体温(例えば、36℃~37℃)の温度範囲内にLCSTを有する温度応答性ポリマーを含む組成物を投与して、対象の眼圧を上昇させたり、眼球を冷却してその眼圧上昇を解消することができる。

投与される水性製剤は、LCST~体温(例えば、36~38℃)の温度を有し得る。また、水性製剤は、温度応答性ポリマーのLCST未満の温度では、温度応答性ポリマーを溶解した水溶液である。温度応答性ポリマーを溶解した水溶液を眼内に投与することができ、投与された温度応答性ポリマーは、眼球の温度まで温められて温度応答性ポリマーの粒子を形成することができ、これにより前記粒子は房水排出口からの房水の排出を阻害し、眼圧の上昇を誘発しうる。したがって、投与される水性製剤は、LCST未満(例えば、0℃からLCST未満)の温度を有し得る。
すなわち、本開示によれば、(a)対象を処置する方法であって、対象の眼球(特に眼球の前房)に温度応答性ポリマーを含む製剤化された水性製剤を投与することを含む方法(以下「方法(a)」という)が提供される。本開示によれば、方法(a)において用いるための温度応答性ポリマーを含む水性製剤(製剤化された水性溶液)が提供される。本明細書では、「水性製剤」は、眼内注射に適した組成を有する。方法(a)では、対象の眼球(特に眼球の前房)に温度応答性ポリマーを含む製剤化された水性製剤を投与することにより、製剤を投与された被検者の眼球の眼圧が上昇し得る。
図2は、本眼圧上昇剤の効果について模式的に示している。温度応答性ポリマーは、対象の眼組織に存在する眼内循環液の温度では凝集し且つ上記眼内循環液の温度を低下させるにつれて線状化する性質を有する。温度応答性ポリマーの凝集は、温度応答性ポリマーの親水性/疎水性のバランスが疎水性に傾くことで引き起こされてもよい。また、温度応答性ポリマーの線状化は、温度応答性ポリマーの親水性/疎水性のバランスが親水性に傾くことで引き起こされてもよい。温度応答性ポリマーは、複数の温度応答性ポリマー分子の集合体であるが、1つのポリマー分子であってもよい。複数の温度応答性ポリマー分子の集合体は、様々な種類の温度応答性ポリマー分子の集合体であってもよく、同じ種類の温度応答性ポリマー分子の集合体であってもよい。
本開示の方法において用いられる温度応答性ポリマーは、0℃から体温(例えば、36℃~37℃)の温度範囲内にLCSTを有し得る。例えば、本開示の方法において用いられる温度応答性ポリマーは、0℃以上、1℃以上、2℃以上、3℃以上、4℃以上、5℃以上、6℃以上、7℃以上、8℃以上、9℃以上、10℃以上、11℃以上、12℃以上、13℃以上、14℃以上、15℃以上、16℃以上、17℃以上、18℃以上、19℃以上、20℃以上、21℃以上、22℃以上、23℃以上、24℃以上、25℃以上、26℃以上、27℃以上、28℃以上、29℃以上、30℃以上、31℃以上、32℃以上、33℃以上、34℃以上、または35℃以上であり、かつ、体温以下のLCSTを有し得る。例えば、本開示の方法において用いられる温度応答性ポリマーはまた、体温以下、35℃以下、34℃以下、33℃以下、32℃以下、31℃以下、30℃以下、29℃以下、28℃以下、27℃以下、26℃以下、25℃以下、24℃以下、23℃以下、22℃以下、21℃以下、20℃以下、19℃以下、18℃以下、17℃以下、16℃以下、15℃以下、14℃以下、13℃以下、12℃以下、11℃以下、10℃以下、9℃以下、8℃以下、7℃以下、6℃以下、5℃以下、または4℃以下のLCSTを有し得る。例えば、本開示の方法において用いられる温度応答性ポリマーは、4℃~35℃、10℃~30℃、15℃~30℃、20℃~30℃、15℃~25℃、25℃~30℃、25℃~35℃、20℃~28℃、または20℃~25℃のLCSTを有し得る。
本開示の方法において用いられる温度応答性ポリマーは、例えば、オキサゾリンポリマーであり得る。オキサゾリンポリマーは、ポリ(2-オキサゾリン)であってもよい。オキサゾリンポリマーの数平均分子量(Mn, kDa)は、5、10、15、20、25、30、35、40及び45から選択する任意の値又は2値間の範囲であってもよい。オキサゾリンポリマーは、下記式(X)で示される構成単位を含む、もしくはからなるポリマーであり得る。オキサゾリンポリマーはまた、例えば、下記化学式(I)で示される構成単位を含む若しくはからなるポリマー、下記化学式(II)で示される構成単位を含む若しくはからなるコポリマー(ブロックコポリマー)、下記化学式(III)で示される構成単位を含む若しくはからなる統計コポリマー又は下記化学式(IV)で示される構成単位を含む若しくはからなるランダムコポリマーであってもよい。

図3Aは、眼組織1内に投与された温度応答性ポリマー10の概要図を示している。眼組織1は、眼圧の調節に直接的若しくは間接的に関与する又は影響を与える組織である。対象の眼組織1は、第一領域100と、第一領域100と流体連通する第二領域200から構成されている。上記眼内循環液は、矢印20が示す方向に流れる、即ち、上記第一領域100から上記第二領域200に流れる。開口部300は、第一領域100と第二領域200との境界領域に位置する。第一領域100の断面は、開口部300の断面はよりも大きい。第二領域200の断面は、開口部300の断面よりも小さくてもよく、大きくてもよく、同じであってもよい。第一領域100は、隅角であってもよい。第二領域200は、シュレム管であってもよい。眼内循環液は、例えば、房水である。
温度応答性ポリマー10を含む眼圧上昇剤は、例えば、注射器2によって第一領域100に投与される。眼圧上昇剤は、温度応答性ポリマー10が凝集する温度で注射してもよく、眼内循環液と約同じ温度で注射してもよい。第一領域100に投与された温度応答性ポリマー10は、対象の眼組織1に存在する眼内循環液の温度によって凝集する。眼圧上昇剤は、第一領域100から上記第二領域200への上記眼内循環液の流れを阻害する量の上記温度応答性ポリマーを含んでいてもよい。「眼内循環液の流れを阻害する」という表現には、「眼内循環液の流れを完全に止める」及び「眼圧を上昇させる程度に眼内循環液の流れを遅くする」が含まれる。
図3Bは、凝集した温度応答性ポリマー10Aが対象の眼組織1における眼内循環液の流れに影響及ぼすことを示す概念図である。凝集した温度応答性ポリマー10Aは、眼内循環液の流れによって開口部300に向かい、開口部300を部分的に又は完全に塞ぐ。凝集した温度応答性ポリマー1Aが開口部300を部分的に又は完全に塞ぐことで、上記第一領域100から上記第二領域200への上記眼内循環液の流れは、凝集した上記温度応答性ポリマーによって阻害される。眼内循環液の流れが阻害されることによって、眼圧が上昇する。
図3Cは、線状化した温度応答性ポリマー10Bを示している。氷などの低温物質や冷却装置によって眼組織1の眼内循環液の温度が低下すると、上記眼内循環液の流れ阻害は、凝集した上記温度応答性ポリマー10Aが線状化するにつれて解消する。線状化した上記温度応答性ポリマー10Bは、眼内循環液の流れによって上記第二領域200に流れる。眼内循環液の流れの阻害が解消されることによって、眼圧は元に戻る。
温度応答性ポリマーは、LCST以上の温度条件下では粒子を形成し得る。前記粒子の数平均粒径は、例えば、1~18μm程度であり得る。前記粒子の数平均粒径は、例えば、1~8μmであり得る。
水性製剤の投与は、眼球の前房まで第1の注射針を挿入することと、房水を回収することと、第1の注射針を眼球から抜くことと、水性製剤を含むシリンジの注射針(第2の注射針)を第1の注射針の通過した穴に差し込んで、その先端を前房に到達させることと、その後、水性製剤を前房に注入することを含んでいてもよい。第1の注射針を挿入することによって、穴が形成され、この穴を通して第2の注射針を挿入することにより水性製剤の投与が容易になる。また、第1の注射針から投与する房水を回収することによって、投与される水性製剤のための空間が確保され、これにより、投与後の眼球からの水性製剤の漏れを防ぎ得る。
本開示によれば、(b)対象を処置する方法であって、対象の眼球(特に眼球の前房)に温度応答性ポリマーを含む製剤化された水性製剤を投与することと、眼球を冷却することとを含む、方法(以下「方法(b)」という)が提供される。眼球を冷却することは、医師が対象の眼球を冷却することと対象に眼球を冷却させることのいずれかまたは両方を意味し得る。
方法(b)において、眼球の冷却は、生理学的に許容される方法で、生理学的に許容される温度にまで低下させるものである。すなわち、眼球の冷却は、眼球に損傷を与えたり、眼球に毒性を生じるような方法にはよらない。眼球の冷却は、温度応答性ポリマーが溶解するように行われる。眼球の冷却はまた、眼圧が低下するように行われる。温度応答性ポリマーは、眼球がLCST未満の温度に低下すると房水に溶解する。したがって、眼球の冷却では、眼球は、LCST未満の温度に冷却される。眼球の冷却は、例えば、LCST未満の温度を有する水溶液(例えば、生理食塩水)、好ましくは、氷水を含む水溶液(例えば、生理食塩水)で、まぶたを介して、またはまぶたを介さずに直接的に、眼球を冷却することによって達成され得る。
方法(b)における冷却では、生理学的に許容される温度は、0℃からLCSTの範囲の温度であり、例えば、LCST-25℃からLCSTの範囲の温度、例えば、LCST-20℃からLCSTの範囲の温度、例えば、LCST-15℃からLCSTの範囲の温度、例えば、LCST-10℃からLCSTの範囲の温度であり得る{但し、生理学的に許容される温度は0℃以上である}。方法(b)における冷却では、生理学的に許容される温度は、4℃からLCSTの範囲の温度、10℃からLCSTの範囲の温度、15℃からLCSTの範囲の温度、20℃からLCSTの範囲の温度であり得る。方法(b)における冷却では、眼球の温度をLCST未満に下げればよいので、LCST未満の温度を有する水溶液(例えば、生理食塩水)、好ましくは、氷水を含む水溶液(例えば、生理食塩水)で、眼球をLCST未満の温度に低下するまで冷却することを含み得る。
本開示によればまた、本開示の方法では、前記水性溶液は、前記温度応答性ポリマー(以下「第1の温度応答性ポリマー」という)に加えて、非電荷親水性ポリマーで修飾された温度応答性ポリマー(以下「第2の温度応答性ポリマー」という)をさらに含んでいてもよい。
非電荷親水性ポリマーとしては、例えば、ポリアルキレングリコール、例えば、ポリ低級アルキレングルコール、特にポリエチレングリコールまたはポリプロピレングリコール、好ましくは、ポリエチレングリコール(PEG)を用いることができる。温度応答性ポリマーに対する非電荷親水性ポリマーによる修飾は、温度応答性ポリマーの末端のオキサゾリン単量体に対してなされ得る。第2の温度応答性ポリマーは、非電荷親水性ポリマーと温度応答性ポリマーとは、例えば、1:1のモル比で含み得る。すなわち、第2の温度応答性ポリマーは、非電荷親水性ポリマーと温度応答性ポリマーとのブロックコポリマーであり得る。第2の温度応答性ポリマーにおいては、非電荷親水性ポリマーと温度応答性ポリマーは、直接、またはリンカーを介して間接的に連結されていることができる。
非電荷親水性ポリマーは、例えば、0.5kDa~10kDa、0.5~10kDa、0.5~5kDa、0.5~2kDa、または0.5~1.5kDaの重量平均分子量(Mw)を有し得る。
第2の温度応答性ポリマーは、好ましくは、PEG-ポリオキサゾリンであり得る。ここで、「PEG-ポリオキサゾリン」は、PEGとポリオキサゾリンとのブロックコポリマーを指す。第2の温度応答性ポリマーにおけるポリオキサゾリンは、好ましくは、式(X)で示される構造を有し得る。
この態様では、水性溶液は、第1の温度応答性ポリマーと第2の温度応答性ポリマーを含み、第1の温度応答性ポリマーと第2の温度応答性ポリマーのモル比は、例えば、p:100-pであり得る{ここで、pは、90~99であり得、好ましくは95~99であり得、より好ましくは97~99であり得、さらに好ましくは約98であり得る}。
ある側面では、本開示では、第1の温度応答性ポリマーと第2の温度応答性ポリマーを含み、第1の温度応答性ポリマーと第2の温度応答性ポリマーのモル比は、例えば、90:10~99:1であり得、好ましくは95:5~99:1であり得、より好ましくは97:1~99:1、最も好ましくは約98:約2であり得る水性溶液が提供される。水性溶液は、好ましくは、眼内投与(特に前房内投与)に適した組成を有し得る。この態様では、第1の温度応答性ポリマーは、好ましくは、ポリオキサゾリンであり得、第2の温度応答性ポリマーは、PEG-ポリオキサゾリンであり得る。
本開示の方法においては、前記水性溶液としては、第1の温度応答性ポリマーと第2の温度応答性ポリマーを含む上記水性溶液を用いてもよい。本開示の方法においては、好ましくは、ウイ溶液としては、第1の温度応答性ポリマーと第2の温度応答性ポリマーを含む水性溶液であって、第1の温度応答性ポリマーと第2の温度応答性ポリマーのモル比は、97:1~99:1である水性溶液を眼内、特に前房内に投与することができる。
本開示の方法は、(c)対象を処置する方法であって、対象の眼球(特に眼球の前房)に温度応答性ポリマーを含む製剤化された水性製剤を投与することと、眼球に対して、少なくとも1サイクルの眼球の冷却と昇温の処置をすることとを含む方法(以下「方法(c)」という)が提供され得る。ここで、冷却は、LCST以上の温度からLCST未満の温度への冷却であり、昇温は、LCST未満の温度からLCST以上の温度への昇温である。昇温は、特別な加温をしない自然昇温、または加温処置をすることにより達成され得る。昇温は、生理学的に許容される方法による、生理学的に許容される温度までの昇温である。昇温は、医師による昇温および対象自身による昇温、並びに自然昇温を含み得る。
眼球の冷却と昇温の処置のサイクルは、少なくとも1回なされ得る。眼球の冷却と昇温の処置は、例えば、2回以上なされ得る。眼球の冷却と昇温の処置は、例えば、3回以上なされ得る。眼球の冷却と昇温の処置のサイクルを繰り返すにつれて、眼圧の上昇幅が小さくなる。したがって、眼球の冷却と昇温の処置を繰り返して、対象の眼圧が適切な眼圧となるように調節することができる。別の言葉では、対象の眼圧が適切な眼圧となるまで、眼球の冷却と昇温の処置のサイクルを繰り返すことができる。
眼球の冷却と昇温の処置のサイクルは、少なくとも1回なされ得る。眼球の冷却と昇温の処置は、例えば、2回以上なされ得る。眼球の冷却と昇温の処置は、例えば、3回以上なされ得る。眼球の冷却と昇温の処置のサイクルを繰り返すにつれて、眼圧の上昇幅が小さくなる。したがって、眼球の冷却と昇温の処置を繰り返して、対象の眼圧が適切な眼圧となるように調節することができる。別の言葉では、対象の眼圧が適切な眼圧となるまで、眼球の冷却と昇温の処置のサイクルを繰り返すことができる。
本開示によれば、方法(a)において用いるための、温度応答性ポリマーを含む水性製剤または水性組成物が提供される。本開示によれば、方法(b)において用いるための、温度応答性ポリマーを含む水性製剤または水性組成物が提供される。本開示によれば、方法(c)において用いるための、温度応答性ポリマーを含む水性製剤または水性組成物が提供される。
ある態様では、上記水性溶液、水性製剤または水性組成物は、常温または環境温度(例えば、10℃から27℃)で保管されている。ある態様では、上記水性溶液、水性製剤または水性組成物は、0℃~10℃、好ましくは0℃~5℃で保管されている。ある態様では、上記水性溶液、水性製剤または水性組成物は、凍結されている。ある態様では、上記水性溶液、水性製剤または水性組成物は、凍結乾燥形態である。本開示によれば、上記水性溶液、水性製剤または水性組成物の凍結乾燥物と再構成用水との組合せキットが提供され得る。前記凍結乾燥物は、再構成用水により使用前に再構成され得る。再構成用水は、添加剤を含んでいてもよい。
第1の温度応答性ポリマーと第2の温度応答性ポリマーとは、LCST未満の温度を有する水性溶液中で混合され得る。混合は、LCST未満の温度を有する水性溶液中で実施することが望ましい。LCST未満の温度で保存された水性溶液は、好ましくはLCST以上の温度への昇温の前に十分に攪拌してから用いられ得る。LCST以上の温度で保存された水性溶液は、そのまま用いてもよいし、あるいは、使用前にLCST未満の温度にまで冷却して、十分に攪拌した後にLCST以上の温度に昇温させて用いることができる。

本開示の方法では、対象は、好ましくは、低眼圧を有する対象であり、例えば、低眼圧症を有する対象であり得る。低眼圧または低眼圧症を有する対象は、毛様体剥離、網膜剥離および黄斑変性などの網膜障害、または虹彩毛様体炎を有していても、有していなくてもよい。低眼圧または低眼圧症を有する対象は、合併症を有する対象であっても、合併症を有しない対象であってもよい。
低眼圧症を治療することに用いるための組成物または医薬組成物
上述の通り、本開示の方法によれば、対象の眼圧を上昇させることができる。したがって、本開示の方法によれば、その必要のある対象において低眼圧症を処置することができる。この態様では、対象は、方法(a)、(b)、または(c)により処置することができる。本開示の方法によれば、これにより合併症の発症リスクを低減する、または合併症を予防することができる。

本発明の組成物または医薬組成物は、薬学的または医薬的に許容される添加剤をさらに含んでいてもよい。薬学的または医薬的に許容される添加剤及び本発明の使用のための添加剤の最適濃度に関する選択は、実験によって当業者によって容易に決定され得る。本発明の使用のための医薬的に許容される添加剤には、溶剤、緩衝剤、キレート化剤、抗酸化剤及び/又は粘度増強剤が含まれる。
薬学的または医薬的に許容される添加剤とは、組成物または医薬組成物を投与することになる個体に対して実質的に無害である物質である。このような添加剤は通常、薬物に関する国家機関によって定められた必要条件を満たす。米国薬局方及び欧州薬局方等の公的薬局方は、よく知られた医薬的に許容される添加剤に関する基準を設定している。
溶剤の例としては、水及びアルコールが挙げられ、好ましくは水であるが、それらに限定されるわけではない。緩衝剤の例としては、クエン酸、酢酸、酒石酸、乳酸、炭酸水素塩、リン酸塩、ジエチルアミン等が挙げられるが、それらに限定されるわけではない。キレート化剤の例としては、EDTAナトリウム及びクエン酸が挙げられるが、それらに限定されるわけではない。抗酸化剤の例としては、ブチル化ヒドロキシルアニソール(BHA)、アスコルビン酸及びその誘導体、トコフェロール及びその誘導体、システイン、並びにそれらの混合物が挙げられるが、それらに限定されるわけではない。粘度増強剤の例としては、ポリビニルアルコール、ポリビニルピロリドン、メチルセルロース、ヒドロキシプロピルメチルセルロース、ヒドロキシエチルセルロース、カルボキシメチルセルロース及びヒドロキシプロピルセルロースが挙げられるが、それらに限定されるわけではない。
本組成物または本医薬組成物は、4.5~8.0の範囲(特に、5.0~6.0、6.0~7.0、または7.0~8.0の範囲、好ましくは、pH7.4~7.8の範囲、好ましくはpH7.6)のpHを有する。本組成物または本医薬組成物はまた、眼の房水および眼組織に相当する浸透圧値を有するように調剤することが好ましい。このような浸透圧値は一般に、約200~約400ミリオスモル/キログラム(「mOsm/kg」)の範囲であるが、好ましくは約250~約350mOsm/kg、好ましくは約300mOsm/kgである。
本組成物または本医薬組成物は、プレフィルドシリンジの形態で提供され得る。プレフィルドシリンジは、本組成物または本医薬組成物を含むプレフィルドシリンジを含み得る。プレフィルドシリンジは、無菌的に包装されていてもよい。プレフィルドシリンジは、シリンジキャップまたは注射針を備えていてもよい。この態様では、本組成物または本医薬組成物は、注射用に製剤化されている。
低眼圧症を治療する方法
本実施形態にかかる低眼圧症を治療する方法は、低眼圧症の治療を必要とする対象の眼組織に上記眼圧上昇剤又は本組成物もしくは本医薬組成物を投与する工程を有する。上記眼圧上昇剤又は本組成物もしくは本医薬組成物は、注射によって眼組織1に投与することができる。上記眼圧上昇剤又は本組成物もしくは本医薬組成物は、眼組織1の第一領域100に投与される。
本実施形態にかかる低眼圧症を治療する方法は、低眼圧症の治療を必要とする対象の眼組織に上記眼圧上昇剤又は本組成物もしくは本医薬組成物を投与する工程を有する。上記眼圧上昇剤又は本組成物もしくは本医薬組成物は、注射によって眼組織1に投与することができる。上記眼圧上昇剤又は本組成物もしくは本医薬組成物は、眼組織1の第一領域100に投与される。
本実施形態においては、温度応答性ポリマーを含む眼圧上昇剤を提供している。しかしながら、眼組織以外の組織(例えば、毛細血管)に対しても応用が可能である。
本開示によれば、低眼圧症を有する対象において低眼圧症を処置する方法に用いるための組成物または医薬組成物の製造における温度応答性ポリマーの使用が提供され得る。本開示によれば、低眼圧症を有する対象において低眼圧症を処置する方法に用いるための組成物または医薬組成物の製造における第1の温度応答性ポリマーおよび第2の温度応答性ポリマーの使用が提供され得る。本開示によれば、低眼圧症を有する対象において低眼圧症を処置する方法に用いるための組成物または医薬組成物の製造における第1の温度応答性ポリマーおよび第2の温度応答性ポリマーを含む上記水性製剤の使用が提供され得る。低眼圧症を有する対象において低眼圧症を処置する方法は、方法(a){但し、対象は低眼圧症を有する}を含み得る。低眼圧症を有する対象において低眼圧症を処置する方法は、方法(b){但し、対象は低眼圧症を有する}を含み得る。低眼圧症を有する対象において低眼圧症を処置する方法は、方法(c){但し、対象は低眼圧症を有する}を含み得る。
本開示によれば、低眼圧症を有する対象において低眼圧症を処置する方法に用いるための温度応答性ポリマーが提供され得る。本開示によれば、低眼圧症を有する対象において低眼圧症を処置する方法に用いるための第1の温度応答性ポリマーおよび第2の温度応答性ポリマーが提供され得る。本開示によれば、低眼圧症を有する対象において低眼圧症を処置する方法に用いるための第1の温度応答性ポリマーおよび第2の温度応答性ポリマーを含む上記水性製剤が提供され得る。
実施例1:オキサゾリンポリマー(以下「POx」ということがある)を含む組成物の投与による眼圧上昇と、投与後の眼球の冷却による眼圧の低下の観察
実験方法
オキサゾリンポリマー(Ultroxa(登録商標)、ポリ(2-プロピル-2-オキサゾリン)、製造番号: 900352-1G、Sigma-Aldrich;平均Mn:10,000;n=89)と生理食塩水を氷上で混合して10%(wt/wt)の水溶液(オキサゾリン水溶液)を準備した。ケタミンとキシラジンで麻酔したクローズドコロニー(SD)ラットの右眼の前房内にマイクロシリンジを用いてオキサゾリン水溶液を5μL注入した。処置後はタリビット眼軟膏を用いて感染とドライアイを予防した。
実験方法
オキサゾリンポリマー(Ultroxa(登録商標)、ポリ(2-プロピル-2-オキサゾリン)、製造番号: 900352-1G、Sigma-Aldrich;平均Mn:10,000;n=89)と生理食塩水を氷上で混合して10%(wt/wt)の水溶液(オキサゾリン水溶液)を準備した。ケタミンとキシラジンで麻酔したクローズドコロニー(SD)ラットの右眼の前房内にマイクロシリンジを用いてオキサゾリン水溶液を5μL注入した。処置後はタリビット眼軟膏を用いて感染とドライアイを予防した。
眼圧測定は、オキサゾリン水溶液注射10日後に麻酔下で処置眼(右眼)と未処置眼(左眼)に対して経時間的に実施した(図4)。眼圧測定のデータは変動が大きいため、一度の測定で5回ずつ測定した。未処置眼の測定期間における眼圧は、7~17mmHgであった。一方、オキサゾリン水溶液を注射した処置眼では麻酔の31分後及び44分後に共に55mmHg以上の顕著な眼圧の上昇が認められた。その後10分間冷却した後、麻酔の56分後に眼圧を測定したところ処置眼圧が12mmHgに低下した。常温で10分間静置したところ再度眼圧が上昇して48mmHgまで上昇した(66分後)。冷却-常温静置をその後2回繰り返したところ、同様に冷却後に著しく眼圧が低下し、常温静置後に高値に戻るが1度目よりも2度目以降の方が眼圧が低下する傾向が認められた。これにより投与により眼圧を上昇させ、冷却により眼圧を低下させるという当初の目的を達したことが明らかとなった。また、眼圧の上昇と低下は、繰り返し誘発させることが可能であり、眼圧の上昇と低下には、再現性が認められた。さらに、眼圧低下後に再上昇させた眼圧は、前回の眼圧よりも低下することも明らかとなった。これは、冷却によりポリマーが前房水に溶解して流れ出ることにより前房中での量が低下したことが理由の一因になっていると思われた。
一方で、ポリオキサゾリンによる眼圧上昇の誘導は、術者の手技等に依存して結果が変わる可能性があったため、製剤組成を改良することとした。
実施例2:非電荷親水性ポリマーとオキサゾリンポリマーとの共重合体の合成
以下の通り、非電荷親水性ポリマーとオキサゾリンポリマーとの共重合体を合成した。非電荷親水性ポリマーと共重合体とすることによりオキサゾリンポリマーの水溶性が高まると考えられる。

ノルマルプロピルオキサゾリン(nPrOx)を含むアセトニトリルを水素化カルシウムで脱水し、減圧蒸留または常圧蒸留により精製を行なった。p-トルエンスルホン酸メチル(TsOMe)を五酸化二リンで脱水し、減圧蒸留によって精製を行なった。TsOMe(50μL,0.33mmol)、nPrOx(3.9mL,33.1mmol)をアセトニトリル10mLに溶解し、42℃で7日間攪拌した。重合停止剤としてアジ化ナトリウム(215mg,3.3mmol)加え、70℃で24時間攪拌した。これらの操作は、全てアルゴン雰囲気化で行った。得られた重合溶液をメタノール、水に対してそれぞれ3回透析し(MWCO=3500)、凍結乾燥することでPnPrOx-N3(分子量8000)を得た。
分子量8000であるPnPrOx-N3 400mgとDBCO-PEG-COOH (MW=1000)100mgをメタノール6mLに溶解した。40℃で二日間攪拌し、メタノール、水に対してそれぞれ3回透析し(MWCO=3500)、凍結乾燥することで目的物(以下「POxPEG」という)を得た。POxおよびPOxPEGのLCSTは、20℃~25℃であった。
実施例3:POxとPOxPEGとの混合物の調製とその物性評価
POxとPOxPEGとを表示された比率(重量比)で含む水溶液を得て、37℃で観察した。具体的には、1重量%のPOxを含む生理食塩水と、1重量%のPOxPEGを含む生理食塩水を氷上で作成し、4℃で両者を様々な比率で十分に混合して、POx/POxPEG混合溶液を得た。第1のスライドグラス上に、中央に穴を有する厚さ100μmのシリコンゴムのシートを接着し、中央の穴にPOx/POxPEG混合溶液を添加して、第2のスライドグラスにより上から蓋をした。第2のスライドグラス上に透明なヒーターを乗せ、倒立顕微鏡を用いて下面からPOx/POxPEG混合溶液を観察した。
まず、ヒーターの温度を37℃に設定して、顕微鏡画像を取得した。結果は、図5に示される通りであった。図5に示されるように、POxのみを含む生理食塩水中では、POxによる比較的大きな粒径を有する粒子がスライドグラス上に付着した。これに対してPOxPEGのみを含む生理食塩水中では、比較的小さな粒径を有する粒子がスライドグラスに付着せずに浮遊していた。観察される粒子は混合比率に応じて両者の中間的な特徴を示した。
POxとPOxPEGとを表示された比率(重量比)で含む水溶液を得て、37℃で観察した。具体的には、1重量%のPOxを含む生理食塩水と、1重量%のPOxPEGを含む生理食塩水を氷上で作成し、4℃で両者を様々な比率で十分に混合して、POx/POxPEG混合溶液を得た。第1のスライドグラス上に、中央に穴を有する厚さ100μmのシリコンゴムのシートを接着し、中央の穴にPOx/POxPEG混合溶液を添加して、第2のスライドグラスにより上から蓋をした。第2のスライドグラス上に透明なヒーターを乗せ、倒立顕微鏡を用いて下面からPOx/POxPEG混合溶液を観察した。
まず、ヒーターの温度を37℃に設定して、顕微鏡画像を取得した。結果は、図5に示される通りであった。図5に示されるように、POxのみを含む生理食塩水中では、POxによる比較的大きな粒径を有する粒子がスライドグラス上に付着した。これに対してPOxPEGのみを含む生理食塩水中では、比較的小さな粒径を有する粒子がスライドグラスに付着せずに浮遊していた。観察される粒子は混合比率に応じて両者の中間的な特徴を示した。
次に、冷却実験を行った。結果は図6に示される通りであった。図6に示されるように、上記スライドグラス上のヒーターをオンのままにして、上部に氷水を含む袋をおいて試料含有部を冷却(アイシング)した。そうすると、観察された粒子が溶解して消える様子が確認された。これにより、得られた粒子は、冷却によって解消することができることが示唆された。氷水を含む袋を除去して、試料含有部を37℃に戻すと、冷却前と同様の形状の粒子が再形成された。これらの結果から、得られた混合溶液は、眼球への投与により房水排出口からの房水の排出を阻害することができる可能性が示唆された。これらの結果からはまた、冷却により房水排出の阻害能を解消することができること、および、これにより眼圧を再度低下させることができる可能性が示唆された。
実施例4:眼圧上昇および降下実験
白色家兎(2~3.5kg)を保定具で保定した後、覚醒下で手持ち眼圧計(Icare TONOVET Plus, M.E,Technica)を用いて眼圧を測定した後にケタミンとキシラジン(ketamine hydrochloride 60 mg/kg;Sankyo, Tokyo, Japan, and xylazine 10 mg/kg; Bayer, Munich,Germany)で麻酔し、麻酔下で改めて眼圧測定と顕微鏡で眼球を観察してから、片目のみに100μLの1wt%POxMixを前房内に注射した。注射直後、麻酔30分後、24時間後に顕微鏡観察と眼圧測定を実施した。
白色家兎(2~3.5kg)を保定具で保定した後、覚醒下で手持ち眼圧計(Icare TONOVET Plus, M.E,Technica)を用いて眼圧を測定した後にケタミンとキシラジン(ketamine hydrochloride 60 mg/kg;Sankyo, Tokyo, Japan, and xylazine 10 mg/kg; Bayer, Munich,Germany)で麻酔し、麻酔下で改めて眼圧測定と顕微鏡で眼球を観察してから、片目のみに100μLの1wt%POxMixを前房内に注射した。注射直後、麻酔30分後、24時間後に顕微鏡観察と眼圧測定を実施した。
結果は、図7に示される通りであった。図7に示されるように、POx:POxPEGが0:100~99:1または90:10~99:1の試料において眼圧上昇を引き起こす個体が確認されたが、個体によっては眼圧上昇の程度が低かった。また、POx:POxPEGが98:2である場合に特に優れた眼圧上昇を認めた。この混合比では手技による眼圧上昇幅のブレはほとんど無く、良好な投与が可能であった。
また、実施例1で用いたオキサゾリンポリマーを含む生理食塩水の投与では、投与直後に白濁し、経時的にオキサゾリンポリーマーが虹彩や水晶体表面に沈着して、24時間後も白い沈殿物として残存していた(図8上パネル参照)。POxPEGを含む生理食塩水の投与でも同様に投与直後は眼球が白濁したものの、30分後には白濁の程度が低下し、24時間後には透明性を回復したが(図8下パネル参照)、眼圧は上昇しなかった(図7)。これに対して、POx:POxPEG(98:2)を含む生理食塩水を投与した眼球では、POxPEGと同様に投与直後には眼球は白濁するものの、30分後には白濁の程度が低下し、24時間後にはほぼ白濁が消失しただけでなく、眼圧上昇も認められた(図7)。温度応答性ポリマーの粒子は、白濁の原因となるが、POx:POxPEG(98:2)では当該粒子が隅角に移動し、これにより水晶体や虹彩上から粒子が消失したことが白濁低下の理由であると考えられた。POxPEGで眼圧上昇が認められず、POx:POxPEG(98:2)で眼圧上昇が認められたのは、前者では粒子径が小さかったため隅角に留まらずに排出され、POx:POxPEG(98:2)は粒子が大きかったため隅角に留まったためであると考えられた。
さらに、1重量%のPOx:POxPEG(98:2)を含む生理食塩水を投与した兎個体の眼圧を測定したところ、図9に示されるように、注射30~40分後に2.5 mmHg程度眼圧が上昇し、24時間後には5 mmHg程度の眼圧上昇を達成した。これらの結果から、白濁が消失した眼球において眼圧上昇効果が達成されたことが示唆された。
その後保定具で家兎を保定して、90分間氷水で眼球を冷却した後に眼圧測定したところ、ΔΔIOPは0mmHgに下がった。この結果から、冷却により眼圧を低下させることもできることが示された。

Claims (14)
- 対象において眼圧を上昇させる方法において用いるための組成物であって、温度応答性ポリマーを含む組成物。
- 前記温度応答性ポリマーが、0℃から37℃の間に下限臨界溶液温度(LCST)を有する、請求項1に記載の組成物。
- 前記温度応答性ポリマーが、15℃から30℃の間に下限臨界溶液温度(LCST)を有する、請求項1または2に記載の組成物。
- 対象において眼圧を上昇させる方法が、前記温度応答性ポリマーを対象の眼球の前房に投与することと、投与後に対象の眼球を冷却することを含む方法である、請求項1~3のいずれか一項に記載の組成物。
- 非電荷親水性ポリマーと温度応答性ポリマーの共重合体をさらに含む、請求項1~4のいずれか一項に記載の組成物。
- 温度応答性ポリマーが、ポリオキサゾリンである、請求項1~5のいずれか一項に記載の組成物。
- 非電荷親水性ポリマーが、ポリアルキレングリコールである、請求項1~6のいずれか一項に記載の組成物。
- ポリオキサゾリン(POx)と、ポリエチレングリコールブロックとポリオキサゾリンブロックを含むブロック共重合体(POxPEG)を含む、組成物。
- POxとPOxPEGの重量比が、90:10~99:1である、請求項8に記載の組成物。
- 対象において眼圧を上昇させる方法において用いるための、請求項8または9に記載の組成物。
- 対象において眼圧を上昇させる方法であって、対象の眼球に温度応答性ポリマーを投与することを含む、方法。
- 前記温度応答性ポリマーが、0℃から37℃の間に下限臨界溶液温度(LCST)を有する、請求項11に記載の方法。
- 前記温度応答性ポリマーが、ポリオキサゾリン(POx)と、ポリエチレングリコールブロックとポリオキサゾリンブロックを含むブロック共重合体(POxPEG)を含む、請求項11または12に記載の方法。
- 温度応答性ポリマーの投与後に、対象の眼球を冷却することをさらに含む、請求項11~13のいずれか一項に記載の方法。
