Description of the invention
The invention relates to new substances based on synthetic highly dispersed silicas and 2 polyorganosiloxanes, which have a high wound-healing ability, which can be used in surgical practice for the treatment of purulent wounds and purulent-inflammatory diseases.
It is known about the highly hydrophilic wound-healing drug "Polysorb" ("Silix") based on highly dispersed silicon dioxide (1, 21), the therapeutic effect of which is based on the ability to adsorb pathogenic protein substances (up to 80 Ω/g), including microbial enzymes, antigens, endo - and exotoxins and microorganisms. 70 An essential component of the therapeutic effect of Polysorb is its high dehydrating ability, due to which edema is quickly eliminated. However, polysorb does not adsorb low- and medium-molecular toxic metabolites from the focus of inflammation, and too high hydrophilicity causes drying of tissues, which is why it is not advisable to use the drug in the second phase of the wound process.
The closest in terms of technical essence and therapeutic effect is the composite wound-healing drug 72 "Gentaxan", which is an organosilicon sorbent polymethylsiloxane (xerogel of methylsilicic acid) immobilized on it with the antibiotic gentamicin and a coordination compound of zinc with tryptophan (|Z). "Gentaxan" sorbs from the wound surface low molecular weight toxic metabolites, hydrophobic substances, waste products of microorganisms, peroxide compounds. However, it has insufficient adsorption activity for pathogenic proteins that are formed during the decay of microbial and somatic cells. In addition, gentaxan is characterized by hydrophobicity, due to which its adhesion to the wound contents is weakened, and with hyperexudation, the drug floats in the bandage. The antiedematous and drainage effect of the drug is not pronounced, therefore, in the first phase of the wound process, dressings must be performed 2 times a day.
Thus, there is a need to create a hydrophilic-hydrophobic wound-healing drug with a sorption-detoxification effect, which would differ from gentaxan by increased water absorption and a wider spectrum of adsorption capacity, namely the ability to adsorb from the focus of inflammation both low- and medium-molecular pathogenic substances, as well as pathogenic proteins - enzymes, antigens, microbial toxins, tissue decay products. The solution to this problem makes it possible to treat wounds of different nature at different stages of the wound process more effectively, using only one drug.
75The goal is achieved by the fact that the composite preparation based on polymethylsiloxane according to the invention also contains hydrophilic highly dispersed silicon dioxide (aerosil) and monoquaternary ammonium compound miramistin at this content of components, wt. 90: "polymethylsiloxane from 24.5 to 75 Ge"! highly dispersed silicon dioxide from 22.5 to 75

To 75.0 g of polymethylsiloxane, 750 g of О0.33906 percent aqueous solution of miramistin was added, thoroughly mixed at room temperature for 1.5 hours. After that, the resulting mixture was dried to a constant mass in a stream of warm air at a temperature of 802C, then dispersed. 22.5 g of highly dispersed silicon dioxide was added to the product thus obtained and stirred for 1.5 hours. The obtained powdered preparation was quantitatively transferred into a glass jar and tightly closed. 70 The output of the drug is 99.90 g.
1.Preparations according to examples 2-3 were obtained in the same way as according to example 1.
Example 4 (starting product - methylsilicic acid hydrogel).
200g of a 195% aqueous solution of miramistin was added to 500g of methylsilicic acid hydrogel (enterosgel), thoroughly mixed at room temperature for 1.5 hours. After that, the obtained mixture 75 was dried to a constant mass in a stream of warm air at a temperature of 702C, then dispersed. 22.5 g of highly dispersed silicon dioxide was added to the product thus obtained and stirred for 1.0 hour.
The obtained powdered preparation was quantitatively transferred into a glass jar and tightly closed.
The output of the drug is 99.89 g.
1-2.Preparations according to examples 5--6 were obtained in the same way as according to example 4. The ratio of components, concentration, temperature regime and time of mixing of the mixture of polymethylsiloxane (hydrogel of methylsilicic acid) - aqueous solution of miramistin, as well as the time of mixing the composition are given in tables 1-2.
We found that the monoquaternary ammonium compound miramistin is able to hydrophilize the hydrophobic surface of polymethylsiloxane in aqueous solutions, after which the suspension does not delaminate, and the dried Ge! the product is easily mixed with hydrophilic silicon dioxide in any ratio to form a stable product (claimed triple composition), which is easily stored for a long time (more than 3 years), does not clump together and, unlike highly dispersed silica and polymethylsiloxane, does not show significant dust-generating ability, which can be attributed to the new positive qualities of the composition.
Thus, in this case, miramistin was used as a modifying mediator between the hydrophobic and (ee) hydrophilic components of the composition, which made it possible to obtain a drug that has a complex of properties, the achievement of which was set as the goal of the invention.
бThe above functions of miramistin do not follow with obviousness from its known antimicrobial properties. b
The study of the composition also showed that there is a potentiation of the properties of each of its components (see Table 3). From the given data, it can be concluded that the adsorption capacity of polymethylsiloxane and silicon dioxide in the composition is significantly higher than that of each individual component.
3.The ratio between the components of the proposed drug is optimal for the manifestation of the maximum therapeutic effect. The composition of the drug in terms of miramistin content is optimized based on the need for complete lyophilization of the polymethylsiloxane surface, and on the other hand, increasing its content is impractical due to the possibility of exceeding the known therapeutic dose |b). - c The specific white absorption capacity of the proposed drug was determined as follows. A number of test tubes were filled with polysorbate, polymethylsiloxane, previously lyophilized by adding a few drops of ethyl alcohol, and the declared composition. 4 ml of 195 gelatin solution was added to the test tubes and mixed moderately on a shaker for 2 hours, after which the suspensions were centrifuged (300 rpm, 40 revolutions) and the equilibrium (residual) protein concentration was measured in the centrifuge. The amount of adsorption was determined as the difference between the initial and equilibrium protein concentrations, which was related to 1 g of sorbent. The results are shown in Table 3.
Similarly, the adsorption capacity of the proposed drug for metabolites of average molecular mass was determined (marker - Congo red, M.m.-696). The tests were carried out according to FS 42U 67-62-96 "Enterosgel". -1 50 Polymethylsiloxane was also pre-lyophilized by adding a few drops of ethyl alcohol (at the same time, the sorbent powder moved from the surface into the liquid volume). A measured amount of dye was added to the mixture
о Наведені результати свідчать, що в заявляємому інтервалі співвідношення компонентів ранозагоювальний препарат проявляє як високу білоксорбуючу здатність (від 83 до З15мг/г), так і здатність сорбувати іме) середньомолекулярні метаболіти (від 1,40 до З3,6б1мг/г).IR e) Congo red with a known initial optical density of the solution. After sorption for 1 hour with intense shaking, the optical density of the solution was re-measured. The value of the adsorption capacity in mg/g was determined by a special formula that takes into account the change in the optical density of the dye solution from the amount of sorption in the solution. The results of the research are shown in Table 3. o The given results indicate that in the declared interval of the ratio of the components, the wound-healing drug exhibits both a high protein-absorbing capacity (from 83 to З15mg/g) and the ability to sorb medium-molecular metabolites (from 1.40 to З3, 6b1mg/g).
The reproduction of such properties in one drug indicates the achievement of the goal of the invention and makes it possible to use the claimed drug for effective wound treatment practically during the time from the onset of the wound to its complete healing at all stages of the wound process.
The use of the claimed agent in the clinic revealed its higher therapeutic efficiency compared to the prototype (gentaxan). In addition to the rapid cleansing of the wound from necrotic tissues, the experimental group of patients observed an accelerated decrease in microbial contamination of the wound tissues, a rapid regression of 65 symptoms of inflammation and intoxication, including normalization of temperature, which previously appeared granulations and epithelization.
