Лікувальний план: - препарати тіоктової кислоти (діаліпон), вітаміни групи В (вітаксон), препарати, що покращують обмінні процеси в тканинах (актовегін); - фізіотерапевтичні процедури (використання гелій-неонового лазера з довжиною хвилі 0,632 нм, час експозиції 10 хв. вдень, протягом 10 днів); - пов'язки з синім йодом на поверхню ранового каналу 2 рази на добу; - дерматотропна мазь на основі наночасток діоксиду церію в рановий канал 2 рази вдень (зранку та вечері); - орошения ранового каналу перед закладанням мазі синім йодом; - антибіотикотерапія (лінкоміцин в/м на курс 21 день).Treatment plan: - thioctic acid preparations (Dialipon), group B vitamins (Vitaxon), preparations that improve metabolic processes in tissues (Actovegin); - physiotherapeutic procedures (use of a helium-neon laser with a wavelength of 0.632 nm, exposure time 10 minutes during the day, for 10 days); - bandages with blue iodine on the surface of the wound channel 2 times a day; - dermatotropic ointment based on nanoparticles of cerium dioxide into the wound channel 2 times a day (morning and evening); - irrigation of the wound channel before applying blue iodine ointment; - antibiotic therapy (lincomycin IV for a course of 21 days).

В результаті проведеного лікування в хворого зменшилися скарги на тупий біль у стопі, покращилася чутливість на стопах, трофічна виразка зажила за 28 днів.As a result of the treatment, the patient's complaints of dull pain in the foot decreased, the sensitivity of the feet improved, and the trophic ulcer healed in 28 days.
Після повного загоєння трофічної виразки хворий був виписаний додому з рекомендаціями про подальше лікування.After complete healing of the trophic ulcer, the patient was discharged home with recommendations for further treatment.
Клінічний випадок 2.Clinical case 2.
Після успішного лікування даний хворий був повторно госпіталізований в жовтні 2014 року в зв'язку із розвитком невропатичної виразки на іншій нозі. Після первинного огляду, ми поставили діагноз СДС, невропатична форма, трофічна виразка першого пальця на правій нозі (У/адпег клас 3) і запропонували пацієнту ту ж стратегію лікування.After successful treatment, this patient was re-hospitalized in October 2014 due to the development of a neuropathic ulcer on the other leg. After the initial examination, we made a diagnosis of SDS, neuropathic form, trophic ulcer of the first toe on the right foot (U/adpeg class 3) and offered the patient the same treatment strategy.

В результаті проведеного лікування в хворого зменшилися скарги на тупий біль у стопі, покращилася чутливість на стопах, трофічна виразка зажила за 50 днів.As a result of the treatment, the patient's complaints of dull pain in the foot decreased, the sensitivity of the feet improved, and the trophic ulcer healed in 50 days.
Після повного загоєння трофічної виразки хворий був виписаний додому з рекомендаціями про подальше лікування. Повторного утворення трофічної виразки не спостерігалося.After complete healing of the trophic ulcer, the patient was discharged home with recommendations for further treatment. Re-formation of trophic ulcer was not observed.

Таким чином, запропонований спосіб лікування хворих з невропатичною формою СДС, дозволяє підвищити ефективність лікування та профілактики цих хворих за рахунок системного комплексного медикаментозного та фізіотерапевтичного лікування паралельно з локальним призначенням гелю Церера (на основі наночасточок діоксиду церію) та синього йоду із заміною пов'язок кожен день, що прискорює повне загоєння ран за допомогою патогенетичних механізмів, які включають в себе посилення проліферації і міграції в зону ураження фібробластів та кератиноцитів, зниження спричиненого оксидативним стресом пошкодження клітинних мембран і білків, профілактику приєднання вторинної інфекцію і тим самим закріплювати отримані позитивні результати лікування.Thus, the proposed method of treatment of patients with a neuropathic form of SDS allows to increase the effectiveness of treatment and prevention of these patients due to systemic complex drug and physiotherapeutic treatment in parallel with the local appointment of Ceres gel (based on nanoparticles of cerium dioxide) and blue iodine with replacement of bandages each day, which accelerates the complete healing of wounds with the help of pathogenetic mechanisms, which include increasing the proliferation and migration of fibroblasts and keratinocytes to the affected area, reducing oxidative stress-induced damage to cell membranes and proteins, preventing the addition of secondary infection, and thereby consolidating the obtained positive treatment results.
