Abstract
本發明係關於將對於治療癌症有效之ALK阻礙劑及VEGF阻礙劑組合而成之用於治療或預防癌症之醫藥、治療或預防癌症之方法或抑制腫瘤增殖之方法。
Description
本發明係關於將對於治療或預防癌症有用之ALK阻礙劑及VEGF阻礙劑組合形成之醫藥及用於治療或預防癌症的方法及其製品等。
間變性淋巴瘤激酶(Anaplastic Lymphoma Kinase,ALK)係屬於胰島素受體家族之受體型酪胺酸激酶(tyrosine kinase)之一種(非專利文獻1、非專利文獻2)。
已有因ALK基因異常(易位、點突變及基因擴增)之結果,引起與其他基因融合之異常激酶的生成而涉及癌化之報告。
作為於日本或海外認可之ALK阻礙劑,可列舉阿雷仙紗TM(AlecensaTM)(學名:阿雷替尼鹽酸鹽、Alectinib)、賽可瑞TM(XalkoriTM)(學名:克唑替尼、Crizotinib)、立克迪(ZykadiaTM)(學名:色瑞替尼(Ceritinib))。
通常於抗癌劑的使用中,為了效果的增強、擴大可適用之腫瘤種類、或對已抗藥性化之癌症的應對或預防,使用作用機制不同之複數種抗癌劑。
有關於ALK阻礙劑之阿雷替尼與其他分子標靶藥併
用之報告。將阿雷替尼與埃羅替尼(Erlotinib)(EGFR(上皮成長因子受體)之酪胺酸激酶選擇性阻礙劑)或克唑替尼(ALK及c-Met(肝細胞增殖因子受體)之酪胺酸激酶阻礙劑)併用之結果,已報告有在經移植有抗阿雷替尼之肺癌細胞株之異種移植物模型(Xenograft model),確認有腫瘤之退縮效果(非專利文獻3:Cancer Res October 1,2014,74;3720,AACR Annual Meeting 2014;April 5-9,2014,Abstract 3720)。
另一方面,血管內皮生長因子(Vascular endothelial growth factor)(VEGF)係血管新生之主要調節因子,於大多數之人類腫瘤中表現亢進,參予腫瘤之增殖/轉移。藉由阻斷VEGF之信號傳達路徑,確認有抑制由於VEGF之腫瘤組織中之血管新生、阻礙腫瘤增殖、將脈管構造正常化、降低血管透過性、減低在腫瘤組織亢進之間質壓等作用,作為VEGF途徑阻礙劑,有癌思停TM(AvastinTM)(學名:貝伐珠單抗、bevacizumab)、蕾莎瓦TM(NexavarTM)(學名:索拉非尼、sorafenib)、福退癌TM(Votrienttm)(學名:帕唑帕尼鹽酸鹽、pazopanib hydrochloride)、紓癌特TM(SutentTM)(學名:蘋果酸舒尼替尼、sunitinib malate)、抑癌特TM(InlytaTM)(學名:阿西替尼、axitinib)、癌瑞格TM(StivargaTM)(瑞格菲尼水合物、regorafenib)等,作為抗癌劑之開發中的VEGF途徑阻礙劑有學名雷莫司單抗(ramucirumab)、學名蘋果酸卡博替尼(cabozantinib S-malate)、學名乙磺酸尼德達尼(nintedanib esylate)等作為抗癌劑被販售。
對於不能切除治癒之晚期/復發之結腸/直腸癌、扁平上皮癌除外之不能切除之晚期/復發之非小細胞肺癌、卵巢癌、不能手術或復發之乳癌,癌思停(貝伐珠單抗)係作為特定之標準化學療法或與化學療法劑併用之用法於日本被承認。然而,對於與VEGF途徑阻礙劑以外之分子標靶藥併用,癌思停(貝伐珠單抗)雖然目前未被承認,惟,已報告有VEGF途徑阻礙劑與EGFR之酪胺酸激酶選擇性阻礙劑之埃羅替尼的併用效果、藉由阻礙VEGF途徑與c-Met或HGF/c-Met途徑雙方,而抑制腫瘤之效果。(非專利文獻4:Lancet Oncol 2014;15:1236-44、非專利文獻5:Cancer Discov.2012 March;2(3):270-287、非專利文獻6:Mol Cancer Ther.2015 Jan;14(1):101-110、“Combination strategy targeting VEGF and HGF/c-Met in human renal cell carcinoma models”)。
惟,目前為止沒有對於VEGF途徑阻礙劑與ALK阻礙劑併用之報告文獻。除此之外,已報告有貝伐珠單抗的投予有降低併用之藥物對腫瘤之移行性之可能性(非專利文獻7:Cancer Cell 21,82-91,January 17,2012、非專利文獻8:Cancer Res.2013 Jun 1;73(11):3347-55”Bevacizumab-induced normalization of blood vessels in tumors hampers antibody uptake.”)、血管新生的抑制有減弱併用藥效果之可能性。臨床上,已報告有在併用EGFR1抗體之帕尼單抗(panitumumab)、貝伐珠單抗、化學療法劑之臨床試驗中無惡化存活期縮短(非專利文獻9:

因此,更期待在癌症的預防/治療上有用且具有優異藥效之醫藥的開發。
本發明之課題係提供用於各種癌症之治療或預防、或無惡化存活期之延長,將複數醫藥組合形成之新穎醫藥及使用該醫藥之癌症治療方法或製品等。
本發明人等將ALK阻礙劑及VEGF阻礙劑組合使用之結果,預料之外的發現該等藥劑之併用可發揮比單劑投予更優異之腫瘤縮小效果,進而完成本發明。

本發明係關於將對於治療癌症有效之ALK阻礙劑及VEGF阻礙劑組合形成之用於治療或預防癌症的醫藥、治療或預防癌症的方法、或抑制腫瘤增殖之方法。
本發明中使用之ALK阻礙劑係指可將ALK之活性直接或間接中和、阻斷、阻礙、降低或妨礙之物質。前述ALK活性之例可列舉酪胺酸激酶活性。前述物質包含低分子化合物、抗體、反義、轉錄阻礙劑、小干擾RNA(siRNA)等。
前述低分子化合物之例可列舉WO2010/143664、WO2004/076412、WO2006/021881、WO2008/073687中記載之化合物或其鹽等。
更具體之例可列舉選自由阿雷替尼(化合物名:9-乙基-6,6-二甲基-8-[4-(嗎啉-4-基)-哌啶-1-基]-11-側氧基-6,11-二氫-5H-苯并[b]咔唑-3-甲腈一鹽酸鹽)、克唑替尼(化合物名:3-[(1R)-1-(2,6-二氯-3-氟苯基)乙氧基]-5-[1-(哌啶-4-基)-1H-吡唑-4-基]吡啶-2-胺)、及色瑞替尼(化合物名:5-氯-N2-(2-異丙氧基-5-甲基-4-哌啶-4-基-苯基)-N4-[2-(丙烷-2-磺醯基)-苯基]-嘧啶-2,4-二胺所成群組之化合物或其鹽等。該等化合物或其鹽可藉由WO2010/143664、WO2004/076412、WO2006/021881、WO2008/073687等中記載之方法製造。該等化合物或其鹽亦包含水合物、製藥學
上容許之各種溶劑化物或結晶多形等。
阿雷替尼、克唑替尼及色瑞替尼之構造式表示於下述之式(I)至(III)。
本發明中使用之ALK阻礙劑之投予路徑可使用經口或非經口中之任一種,較佳為使用經口投予。作為經口投予使用之劑型可從例如液劑、散劑、顆粒劑、錠劑、腸溶劑及膠囊劑等任意劑型中適宜選擇。具有該等劑型之ALK阻礙劑可以所屬技術領域中具有通常知識者所習知之方法製劑化。例如與藥理學上容許之載體或介質,具體而言為與滅菌水或生理食鹽水、植物油、乳化劑、懸濁劑、界面活性劑、安定劑、香味劑、賦形劑、載體、防腐劑、黏合劑等適宜組合,以一般認定之製藥實施中所要求之單位用量形態混和後經由凍結乾燥、打錠成型等製劑化作業
製劑化。
ALK阻礙劑亦可以水或水以外之藥學上容許之液體之無菌性溶液或懸濁液劑等注射劑之形態非經口使用。該等製劑中之有效成分量可在所指示之範圍內適宜選擇可投予之適當用量。用於注射之無菌組成物可使用如注射用蒸餾水之載體,依照通常之製劑實施進行配製。作為注射用之水溶液可例示例如生理食鹽水、含有葡萄糖或其他補助藥之等張液,例如D-山梨糖醇、D-甘露糖、D-甘露糖醇、氯化鈉,可與適當之溶解補助劑例如醇,具體而言為乙醇、多元醇例如丙二醇、聚乙二醇,非離子性界面活性劑例如聚山梨酸酯80(TM)、HCO-50適當併用。作為油性液可例示芝麻油、大豆油,亦可與作為溶解補助劑之苯甲酸苯甲酯、苯甲醇併用。又,可與緩衝劑例如磷酸鹽緩衝液、乙酸鈉緩衝液;舒緩劑例如鹽酸普卡因;安定劑例如苯甲醇、苯酚;抗氧化劑適當調配。
作為本發明所述之ALK阻礙劑之投予量係例如每一次之投予,可以每1kg體重在0.0001mg至1000mg範圍選擇投予量。或例如每個病患可在0.001mg至100000mg/個體之範圍選擇投予量。惟,本發明ALK阻礙劑之投予量不限定於該等投予量。
作為前述阿雷替尼、其鹽或該等之水合物更具體之投予量,可列舉1日2次,且換算為游離態時,每一次為20mg、40mg、60mg、80mg、120mg、160mg、220mg、240mg、300mg、460mg、600mg、760mg或900mg。
本發明ALK阻礙劑之投予期間可適當根據症狀或副作用之程度決定,可在治療癌症或達到期望之治療效果為止之期間進行投予。具體而言,可在7日至3年之間連續投予,或者是,有在投予期間設定約1至14日之停藥期,將2日至3個月作為一循環,投予1至36循環之方法等。較佳為以14至30日為一循環,投予3至24循環之方法。
本發明中使用之VEGF阻礙劑係指可將VEGF之活性直接或間接中和、阻斷、阻礙、降低或妨礙之物質。VEGF阻礙劑亦可藉由對一個或複數個VEGF受體結合,將VEGF之活性中和、阻斷、阻礙、降低或妨礙。VEGF阻礙劑包含抗VEGF抗體與其抗原結合片斷、受體分子及VEGF特異結合,隔絕該等對一個或複數個受體結合之衍生物、抗VEGF受體抗體及抗VEGF受體拮抗劑(例如VEGFR酪胺酸激酶之低分子阻礙劑)。作為低分子VEGF阻礙劑之具體例可列舉選自由索拉非尼(Sorafenib、化合物名:N-(4-氯-3-(三氟甲基)苯基)-N'-(4-(2-(N-甲基胺基甲醯基)-4-吡啶氧基)苯基)脲[N-(4-Chloro-3-(trifluoromethyl)phenyl)-N'-(4-(2-(N-methylcarbamoyl)-4-pyridyloxy)phenyl)urea]、專利文獻:US7235576)、帕唑帕尼(Pazopanib、化合物名:5-[[4-[(2,3-二甲基-2H-吲唑-6-基)甲基胺基]嘧啶-2-基]胺基]-2-甲基苯磺醯胺)[5-[[4-[(2,3-dimethyl-2H-indazol-6-yl)methylamino]pyrimidin-2-yl]amino]-2-methylbenzenesulfonamide]、舒尼替尼

本發明中之「抗VEGF抗體」係指具有充分的親和性及特異性與VEGF結合之抗體。經選擇之抗體通常對於VEGF具有結合親和性,例如抗體可以100nM至1pM間之Kd值與hVEGF結合。抗體親和性可藉由表面電漿共振基礎分析(例如如WO2005/012359中記載之生物分子交互作用分析系統(BIAcore assay)等);酵素連結免疫吸附分析(ELISA);及競爭分析(例如RIA)判定。作為其他之實施態樣,本發明之抗VEGF抗體係將VEGF活性參予之疾病或症狀作為標靶,可作為防礙疾病或症狀時之治療劑使用。又,為了評估例如作為治療劑時之有效性,可在抗體實施其他生物學之活性分析。該等分析於該技術領域為已知,取決於標靶抗原及抗體之意圖之用途。其例有HUVEC阻礙分析、腫瘤細胞增殖阻礙分析(例如WO第89/06692號中記載者)、抗體依存性細胞傷害性(ADCC)及補體媒介性細胞傷害性(CDC)分析(美國專利第5500362號)及激動劑活性或造血分析(參照WO95/27062號)等。
「抗體」用語包含被最廣義使用之單株抗體(包含完全長或完整之單株抗體)、多株抗體、多價抗體、多重特異性抗體(例如雙重特異性抗體)及該等僅顯示期望的生物活性之抗體片段(參照下述)。
於本說明書中「Kd」或「Kd值」為於一實施態樣,在階段性力價之非標識VEGF存在下,以最小濃度之(125I)-標識抗原平衡Fab,藉由捕獲與抗Fab抗體塗覆盤結合之VEGF,測定對於VEGF之Fab的溶液結合親和性之
如以下述分析所示(Chen,等,(1999)J.Mol Biol 293:865-881),以使用Fab型抗體及VEGF分子實行之經放射性標識之VEGF結合分析(RIA)測定。於一例,為了決定分析之條件,將微孔盤(Dynex)以含有5μg/mL捕獲抗Fab抗體(Cappel Labs)之50Mm碳酸鈉(pH 9.6)塗覆一晚,之後以含有2%(w/v)牛血清白蛋白之PBS於室溫(約23℃)阻隔約2至5小時。於非吸附盤(Nunc#269620),將100pM或26pM之[125I]VEGF(109)與經階段稀釋之感興趣之Fab,例如Fab-12(Presta等,(1997)Cancer Res.57:4593-4599)混合。接著,將感興趣之Fab培養一晚,惟,培養要達到確實之平衡狀態或許需花費65小時。之後,將混合物移至捕獲盤,於室溫培養1小時。接著,去除溶液,將盤以含有0.1% Tween20之PBS洗淨8次。當盤乾燥後加入150μL/孔之閃光物質(MicroScint-20;Packard),將盤以Topcount γ計測器(Packard)計測10分鐘。選擇最大結合之20%或20%以下濃度之Fab,於各個競合結合分析中使用。若根據其他實施態樣,使用經固定之約10反應單位(RU)之hVEGF(8-109)CM5晶片,於25℃之BIAcore TM-2000或BIAcoreTM-3000(BIAcore,Inc.,Piscataway,NJ),使用表面電漿共振分析來測定Kd或Kd值。簡而言之,依照供給者之說明書將羧甲基化葡聚糖生物感測器晶片(CM5,BIAcore Inc.)以N-乙基-N'-(3-二甲基胺基丙基)-碳二亞胺鹽酸鹽(EDC)及N-羥基琥珀醯亞胺(NHS)活化。將人類VEGF以10mM乙酸鈉(pH4.8)稀釋為5μg/mL(約0.2μ
M),使經結合之蛋白質反應單位(RU)成為約10之方式以5μL/分鐘之流量注入。注入人類VEGF後,為了阻隔未反應之群而注入1M之乙醇胺。為了動態測定,將經2倍階段稀釋之Fab(從0.78nM至500nM)於25℃以約25μL/分鐘之流量注入至含有0.05% Tween20(PBST)之PBS中。使用單純一對一蘭格謬爾結合模型(simple one-to-one Langmuir binding model)(BIAcore Evaluation軟體版本3.2),藉由同時擬合締合及解離之感應圖來計算出締合速率(Kon)及解離速率(Koff)。平衡解離定數(Kd)以Koff/Kon比計算。參照例如Chen,Y.等(1999)J.Mol Biol 293:865-881。藉由前述之表面電漿共振分析,結合速度超過106M-1S-1時,可使用螢光消光技術來測定結合速度,即以分光計例如具備動力學停止流儀(Stopped-flow)之分光光度計(Aviv Instruments)或具備攪拌管(cuvette)之8000系列SLM-Aminco分光光度計(ThermoSpectronic)進行測定,在增加濃度之人類VEGF短型(8-109)或小鼠VEGF存在下,測定在PBS(pH7.2)、25℃之20nM抗VEGF抗體(Fab型)之螢光發光強度(激發=295nm;發光=340nm、通頻帶=16nm)的增加或減少。
於本發明中,抗VEGF抗體或其抗原結合片段係單株抗體、嵌合抗體(chimeric antibody)、完全人類抗體或人類化抗體。又,抗VEGF抗體或其抗原結合片段係缺失Fc部分、F(ab’)2、Fab或Fv構造之抗體。
作為本發明之一實施態樣,抗VEGF抗體雖無限定,
包含結合之抗原決定基與藉由融合瘤(Hybridoma)ATCC HB 10709產生之單株抗VEGF抗體A 4.6.1相同抗原決定基之單株抗體、依照Presta等(1997)Cancer Res.57:4593-4599產生之重組人類化抗VEGF單株抗體。於其他實施態樣,抗VEGF抗體係作為「rhuMAb VEGF」或「癌思停(註冊商標)」被已知之「貝伐珠單抗(BV)」。癌思停(註冊商標)係於特定之國家販售。貝伐珠單抗係包含源自阻隔人類VEGF在其受體結合之小鼠抗hVEGF單株抗體A.4.6.1之抗原結合互補性決定區域及經突變之人類IgG1框架區域。包含大部分框架區域之貝伐珠單抗之胺基酸序列約93%為衍生自人類IgG1,序列之約7%衍生自小鼠抗體A 4.6.1。另,於此處使用時,「貝伐珠單抗(BV)」用語係包含所有對應抗VEGF抗體或抗VEGF抗體片段,其係為了在選自美國、歐州及日本所成之國家群組中之國家或地區獲得作為相同或生物相似性藥品(biosimilars)製品之販售許可而必須滿足之要求。
為了篩選會與相關抗體所結合抗原上之抗原決定基結合之抗體,可實施常規交叉阻隔分析,例如A Laboratory Manual,Cold Spring Harbor Laboratory,Ed Harlow及David Lane(1988)中所記載者。
貝伐珠單抗及其他人類化抗VEGF抗體另記載於2005年2月26日發行之美國專利第6884879號中。又,亦包含如WO2005/012359號、WO2005/044853號及美國專利申請第60/991302號中記載之G6或B20系列抗體(例
如G6-31、B20-4.1)。除此之外,亦包含記載於美國專利第7060269號、美國專利第6582959號、美國專利第6703020號、美國專利第6054297號、WO98/45332號、WO96/30046號、WO94/10202號、歐州專利第0666868B1號、美國專利申請公開第2006009360號、美國專利申請公開第20050186208號、美國專利申請公開第20030206899號、美國專利申請公開第20030190317號、美國專利申請公開第20030203409號及美國專利申請公開第20050112126號及Popkov等,Journal of Immunological Methods 288:149-164(2004)之抗體。作為一實施態樣,上述抗體包括結合於人類VEGF上官能性抗原決定基者,其包含殘基F17、M18、D19、Y21、Y25、Q89、I91、K101、E103及C104,或包含殘基F17、Y21、Q22、Y25、D63、I83及Q89。
前述「G6系列抗體」為WO2005/012359第7圖24至26及34至35中之任一種G6抗體或從G6-衍生抗體序列衍生之抗VEGF抗體。另,亦包含WO2005/044853中記載之抗體。作為一實施態樣,G6系列抗體係結合於含有殘基F17、Y21、Q22、Y25、D63、I83及Q89之人類VEGF上之官能性抗原決定基。
前述「B20系列抗體」為WO2005/012359第27圖至第29圖中之任一種B20抗體或從B20衍生抗體序列衍生之抗VEGF抗體。另,亦包含WO2005/044853及美國專利申請第60/991302號中記載之抗體。作為一實施態樣,B20系列抗體係結合於含有殘基F17、M18、D19、Y21、
Y25、Q89、I91、K101、E103及C104之人類VEGF上之官能性抗原決定基。
前述「官能性抗原決定基」係指於抗體之結合性上賦予能量之抗原胺基酸殘基。藉由對賦予抗原能量之殘基中之任一個產生突變(例如野生型VEGF之丙胺酸突變或同源性突變),將破壞抗體之結合性,使抗體之相對親和性比(IC50突變型VEGF/IC50野生型VEGF)大於5(參照WO2005/012359號之實施例2)。於一實施態樣,相對親和性比係藉由溶液結合性噬菌體表現ELISA來測定。簡言之,將96孔Maxisorp免疫盤(NUNC)於4℃以存於PBS中之2μg/mL濃度之欲試驗之Fab型抗體包覆一晚,以PBS、0.5% BSA及0.05% Tween 20(PBT)於室溫下阻隔2小時。將存於PBT之噬菌體表現hVEGF丙胺酸點突變型(殘基8-109型)或野生型hVEGF(8-109)之連續稀釋物,首先於室溫下在經Fab塗覆之盤上培養15分鐘,將盤以PBS、0.05% Tween 20(PBST)洗浄。將經結合之噬菌體用以1:5000稀釋於PBT之抗M13單株抗體辣根過氧化酶(Amersham Pharmacia)結合物進行檢測,以3,3',5,5'-四甲基聯苯胺(TMB,Kirkegaard & Perry Labs,Gaithersburg,MD)基質使其顯色約5分鐘,以1.0M之H3PO4中止反應,以450nm之分光光度進行讀取。IC50值之比率(IC50,ala/IC50,wt)表示於結合親和性中之降低倍數(相對結合親和性)。
非人類(例如小鼠)抗體之「人類化」型為含有最少量源自非人類免疫球蛋白之序列的嵌合抗體
(chimeric antibody)。大部分之人類化抗體為人類免疫球蛋白(受體抗體),其受體高頻率可變區域之殘基藉由從小鼠、大鼠、兔子或具有所期望之特異性、親和性及能力之如非人類靈長類之非人類物種(捐贈者抗體)之高頻率可變區域之殘基置換。於一些情況中,人類免疫球蛋白之框架區域(FR)殘基藉由對應之非人類殘基置換。另,人類化抗體可包含未曾於受體抗體或損贈者抗體中發現之殘基。該等修飾係為了進一步改良抗體之特性。通常,人類化抗體係包含實際上全部至少一個且通常兩個可變結構區域,其中全部或實質上全部之高頻率可變環對應非人類免疫球蛋白之高頻率可變環,全部或實質上全部之FR係人類免疫球蛋白序列之FR。又,人類化抗體根據情況,包含至少一部分免疫球蛋白恆定區(Fc)、典型上包含至少一部分之人類免疫球蛋白恆定區(Fc)。更詳細可參照Jones人等,Nature 321:522-525(1986);Riechmann人等,Nature 332:323-329(1988)及Presta,Curr.Op.Struct.Biol.2:593-596(1992)。
其他人類化抗VEGF抗體之例可列舉雷莫蘆單抗(學名:Ramucirumab,商品名:欣銳札(Cyramza)TM、CAS登錄編號:947687-13-0)。
於本發明之其他實施態樣,抗VEGF抗體具有:含有以下胺基酸序列而構成之重鏈可變區:

VEGF阻礙劑可根據常法製劑化(例如Remington's Pharmaceutical Science,latest edition,Mack Publishing Company,Easton,U.S.A),可同時含有醫藥上容許之載體或添加物。可列舉例如界面活性劑、賦形劑、著色料、著香料、保存料、安定劑、緩衝劑、懸濁劑、等張化劑、黏合劑、崩解劑、潤滑劑、流動性促進劑、矯味劑等,惟,未限定於此,可適當使用其他之常用載體。具體而言,可列舉海藻糖、輕質無水矽酸、乳糖、結晶纖維素、甘露糖醇、澱粉、羧甲基纖維素鈣、羧甲基纖維素鈉、羥丙基纖維素、羥丙基甲基纖維素、聚乙烯縮醛二乙基胺基乙酸酯、聚乙烯吡咯啶酮、明膠、中鏈脂肪酸三酸甘油酯、聚氧乙烯硬化蓖麻子油60、白糖、羧基甲基纖維素、玉米澱粉、無機鹽類、聚山梨酸酯等。
本發明之一態樣為將ALK阻礙劑及VEGF阻礙劑組合而成之用於治療或預防癌症之醫藥。
於上述態樣,「將ALK阻礙劑及VEGF阻礙劑組合而成之用於治療或預防癌症之醫藥」係指在治療或預防癌症時用以將ALK阻礙劑及VEGF阻礙劑同時、個別或依序投予而組合之醫藥。本發明之醫藥可提供同時含有ALK阻礙劑及VEGF阻礙劑之複合劑形態。又,亦可提供分別含有ALK阻礙劑之製劑及含有VEGF阻礙劑之製劑,該等製劑可同時或依序使用。
於本發明,同時投予係指可將ALK阻礙劑及VEGF阻礙劑同時投予合併使用、可作為複合劑投予、可作為在投予使用時經調製之混合物投予、亦可將其他形態之製劑同時投予。同時投予使用時,可從個別路徑投予、亦可從同一路徑投予,投予劑型可相同亦可不同。
於本發明,將ALK阻礙劑及VEGF阻礙劑個別投予時,ALK阻礙劑及VEGF阻礙劑之投予順序係可在VEGF阻礙劑投予後投予ALK阻礙劑、可將ALK阻礙劑及VEGF阻礙劑同時投予、或可在ALK阻礙劑投予後投予VEGF阻礙劑。
本發明所述之ALK阻礙劑及VEGF阻礙劑之投予順序係指投予其中一個藥劑後再投予另一個藥劑,ALK阻礙劑及VEGF阻礙劑之投予間隔並無特別限制,可考慮投予路徑或劑型等主要因素而設定。例如,ALK阻礙劑與VEGF阻礙劑之投予間隔為0小時至168小時,較佳為0小時至72小時,更佳為0至24小時,又更佳為0小時至12小時。又,除了投予路徑或劑型等主要因素之外,
亦可酙酌本發明所述之ALK阻礙劑及VEGF阻礙劑在對象中之各別的殘留濃度。亦即,在投予VEGF阻礙劑之前投予ALK阻礙劑時,藉由VEGF阻礙劑可獲得所期待之效果之對象中該ALK阻礙劑殘留濃度被檢測之時間點,可投予VEGF阻礙劑。該濃度係可將從對象採取之試料使用各種層析儀等分離裝置以所屬技術領域具有通常知識者所習知之分析方法進行分析的結果為準而決定。
與此相反地,在投予ALK阻礙劑之前投予VEGF阻礙劑時,藉由ALK阻礙劑可獲得所期待之效果之對象中檢測到該VEGF阻礙劑殘留濃度之時間點可投予ALK阻礙劑。該濃度係可將從對象採取之試料以所屬技術領域具有通常知識者所習知之ELISA等免疫性測定法進行分析之結果為準而決定。
上述之醫藥中,以ALK阻礙劑及VEGF阻礙劑含於各個製劑之方式提供時,該等製劑之劑型可為相同之劑型,亦可為不同之劑型。例如,雙方為經口劑、非經口製劑、注射劑、點滴劑、靜脈內點滴劑中之一的彼此不同之劑型,亦可為雙方為經口劑、非經口製劑、注射劑、點滴劑、靜脈內點滴劑中之一的同種之劑型。又,上述之醫藥亦可將一種以上不同之製劑組合。
於其他之觀點,本發明提供含有ALK阻礙劑作為有效成分,與VEGF阻礙劑併用,用於治療或預防癌症之醫藥。「含有ALK阻礙劑作為有效成分,與VEGF阻礙劑併用,用於治療或預防癌症之醫藥」係指將與VEGF
阻礙劑併用作為條件,用於治療或預防癌症之含有ALK阻礙劑作為有效成分之醫藥。含有ALK阻礙劑作為有效成分之醫藥在與VEGF阻礙劑併用時,可與VEGF阻礙劑同時投予,亦可在VEGF阻礙劑投予前或投予後投予。在VEGF阻礙劑投予前或投予後投予ALK阻礙劑時,藉由測定對象中該VEGF阻礙劑之殘留濃度或例示於日本專利第4362225號等中之VEGF異形體之表現量,可將投予時期最適化。該濃度係可將從對象採取之試料以所屬技術領域具有通常知識者所習知之後述之ELISA等免疫性測定法為準而決定。
於其他之觀點,本發明提供一種含有VEGF阻礙劑作為有效成分,與ALK阻礙劑併用,用於治療或預防癌症之醫藥。於本發明,「含有VEGF阻礙劑作為有效成分,與ALK阻礙劑併用,用於治療或預防癌症之醫藥」係指將與ALK阻礙劑併用作為條件,用於治療或預防癌症之含有VEGF阻礙劑作為有效成分之醫藥。含有VEGF阻礙劑作為有效成分之本發明之醫藥在與ALK阻礙劑併用時,可與ALK阻礙劑同時投予,亦可在ALK阻礙劑投予前或投予後投予。在ALK阻礙劑投予前或投予後投予VEGF阻礙劑時,藉由測定對象中該ALK阻礙劑之殘留濃度,可將投予時期最適化。該濃度係可將從對象採取之試料使用各種層析儀等分離裝置以所屬技術領域具有通常知識者所習知之分析方法進行分析之結果為準而決定。
上述發明係指將ALK阻礙劑及VEGF阻礙劑同時投予
或使用(以下,簡稱為「投予」),對於投予之順序或投予間隔等並未限定。又,亦可使用作為本發明之ALK阻礙劑及VEGF阻礙劑組合之製品。進一步,根據本發明,相對於單獨使用任一方之投予量,將ALK阻礙劑及VEGF阻礙劑併用時,可期望各以較少之投予量進行投予。
本發明之醫藥可用於預防或治療如白血病(急性骨髓性白血病、慢性骨髓性白血病、急性淋巴性白血病、慢性淋巴性白血病等)、惡性淋巴瘤(何傑金氏淋巴瘤、非何傑金氏淋巴瘤等)、腦瘤、神經母細胞瘤、神經膠瘤、甲狀腺癌、骨髓異常形成症候群、頭頸部癌、食道癌、胃癌、大腸癌、結腸直腸癌、乳癌、卵巢癌、肺癌、胰臟癌、肝癌、膽嚢癌、皮膚癌、惡性黑色素瘤、腎癌、腎盂尿道癌、膀胱癌、子宮癌、睪丸癌、***癌之各種癌等疾病。另,本發明之化合物可用於預防或治療實體癌之浸潤/轉移。
特別是,本發明之醫藥可用於預防或治療ALK融合基因陽性之癌症。ALK融合基因陽性之癌症之例可列舉非何傑金氏淋巴瘤、神經母細胞瘤、炎性肌纖維母細胞瘤、腎癌及肺癌等,惟,不限定於此。
本發明中,「ALK融合基因陽性」係指存在有ALK與其他基因之融合基因。前述之其他融合基因可列舉EML4(棘皮動物微管結合蛋白4(echinoderm microtubule-associated protein like protein 4),GenBankTM登
錄編號NM_019063)、TFG(TRK-fused gene,GeneBankTM登錄編號NM_006070)等,惟,不限定於此。
將ALK融合基因陽性之癌症特定之方法係記載於WO2008/127248、日本特開2008-295444、「ALK基因檢査之入門」(日本肺癌學會生物標記(biomarker)委員會)等。作為用於檢測該融合基因之套組市售有Vysis LSI ALK Break Apart Rearrangement Probe Kit TM(Abbott公司)、Histofine ALK iAEPTM套組(NICHIREI公司)。
本發明之另一態樣為提供一種對於藉由ALK阻礙劑治療癌症之病患,藉由使用VEGF阻礙劑可增強該ALK阻礙劑之治療效果之方法。增強治療效果係指提升治療之效率、降低為了治療所投予之ALK阻礙劑之量及/或縮短藉由ALK阻礙劑之治療期間。又,本發明之另一態樣為提供一種包含將ALK阻礙劑及有效量之VEGF阻礙劑組合投予,延長對象之無惡化存活期之方法。又,本發明之另一態樣為提供一種VEGF阻礙劑之使用方法,其係用於製造含有ALK阻礙劑及VEGF阻礙劑作為有效成分之用於治療或預防癌症之醫藥組成物。
本發明中,含有ALK阻礙劑及/或VEGF阻礙劑作為有效成分係指含有ALK阻礙劑及/或VEGF阻礙劑作為主要之活性成分,ALK阻礙劑及/或VEGF阻礙劑之含有率並無限制。又,「治療」之用語係指藉由將本發明所述之醫藥對對象進行投予,癌細胞滅絕或其細胞數減少、抑制癌細胞增殖、改善由癌症引起之各種症狀。又,「預防」
係指防止經減少之癌細胞藉由再度增殖而造成癌細胞數增加、防止經抑制增殖之癌細胞再增殖。
本發明中,VEGF阻礙劑之投予方法可藉由經口、非經口投予中之任一方法實施。特佳為藉由非經口投予之投予方法,作為所述之投予方法之具體例較佳可例示注射投予、經鼻投予、經肺投予、經皮投予等。注射投予之例可例如藉由靜脈內注射、肌肉內注射、腹腔內注射、皮下注射等使本發明之治療用抗體可進行全身或局部投予。又,投予方法可根據病患之年齡、症狀而適當選擇。投予量為例如每一次之投予,可以每1kg體重在0.0001mg至1000mg之範圍選擇投予量。或,例如每個病患可在0.001至100000mg/個體之範圍選擇投予量。惟,本發明VEGF阻礙劑之投予量並不限定於該等投予量。
根據疾病類型及嚴重度,例如如貝伐珠單抗之抗VEGF抗體之較佳用量雖然沒有限定,可在包括5mg/kg、7.5mg/kg、10mg/kg或15mg/kg之約1μg/kg至約50mg/kg之範圍,最佳為約5mg/kg至約15mg/kg之範圍。投予頻率根據疾病類型及嚴重度而變動。對於數日或超過數日以上之反覆投予,根據症狀,治療係可藉由在該領域已知之方法測定,持續到癌症得以治療或達成所期待之治療效果為止。在一例中,抗VEGF抗體係以每週一次、每兩週一次或每三週一次進行投予,雖無限制,可在含有5mg/kg、7.5mg/kg、10mg/kg或15mg/kg之約5mg/kg至約15mg/kg之用量範圍投予。惟,亦適用於其他用量之方案。
VEGF阻礙劑可將上述每週一次、每兩週一次或每三週投予作為1個循環,在癌症得以治療或達成所期待治療效果為止之期間進行投予。具體而言,可投予1至36個循環。
本發明中,「無惡化存活期(PFS)」係指從治療(或隨機化)至最初之疾病惡化或死亡為止之時間。本發明之一態樣中,PFS可藉由實體腫瘤之治療效果判定基準(Response Evaluation Criteria in Solid Tumors,RECIST)進行評估。本發明之一態樣中,PFS可藉由CA-125含量作為惡化決定主要因素來評估。
本發明中,「延長無惡化存活期」之具體例可列舉與ALK阻礙劑單劑投予時之無惡化存活期相比,無惡化存活期延長。本發明中,「對象」並無特別限制,係指成為本發明醫藥投予之對象之人類或非人類哺乳動物,包含例如牛、馬、狗、羊或貓之哺乳動物。較佳之對象為人類。對象包含病患(包含人類及非人類哺乳動物)。
本發明之另一態樣為提供一種製品。一實施態樣中,製品係為了用於治療或預防對象之癌症而提供,包含(a)(1)含有VEGF阻礙劑之製劑、(2)容器、(3)標示有為了治療對象之癌症將前述VEGF阻礙劑與至少一種ALK阻礙劑組合並投予於對象一事之說明書或標籤或(b)(1)含有ALK阻礙劑之製劑、(2)容器、(3)標示有為了治療對象之癌症將前述
ALK阻礙劑及至少一種VEGF阻礙劑組合並投予於對象一事之說明書或標籤。
製品係包括含有製劑之容器,該製劑含有VEGF阻礙劑或ALK阻礙劑。
製品可進一步包括在容器上或容器中附帶有之標籤或說明書。
本發明中,「說明書」係指包含關於製劑使用之適應症、用法、投予量、投予、禁忌症及/或警告之情報,通常含於製劑之商業用包裝中之書類。「標籤」係指含有表示VEGF阻礙劑或ALK阻礙劑之製劑之製品名稱、用量、劑型、適應症等之片狀物品,直接貼在容器上者。
標籤或說明書表示製劑之適應症,亦即用於治療癌症等經選擇之疾病。一實施態樣中,標籤或說明書表示製劑可用於治療癌症。標籤或說明書可指示製劑可用於治療其他障礙。
適當之容器可列舉例如PTP、瓶、小瓶、注射筒、泡罩包裝等。容器可從玻璃或塑膠等各種材料形成。該等容器亦可梱包於印有上述標籤內容等之紙製外箱。
將ALK阻礙劑及VEGF阻礙劑併用時有無副作用可藉由周知之方法確認。此處,「副作用」係指在接受疾病治療的病患經觀察及/或測定在臨床、醫學、物理、生理學及/或生化學的影響,不屬於意圖的治療效果的一部分之影響。通常,前述影響不是被治療之病患之健康狀態及/或舒適感、被治療之病患而言之健康危險性及/或對於
被治療之病患之治療容許性所期望者。作為副作用之具體例可列舉嗜中性球減少、白血球減少、出血(消化道出血、肺出血、腦出血等)、高血壓、神經毒性、疲勞/倦怠感、食慾減退、噁心、口內炎、脫毛症、血小板減少、尿蛋白陽性、休克、過敏性反應、消化道穿孔、瘻孔、創傷治癒延遲、血栓栓塞症、高血壓性腦症、高血壓性危象、可逆性後白質腦症症候群、腎病症候群、骨髓抑制、感染症、鬱血性心不全、間質性肺炎、血栓性微小血管症、間質性肺病、肝功能障害、血中膽紅素增加、味覺異常、發疹、血中肌酸酐增加等。將併用時確認之副作用之頻率及程度等與單劑投予時之頻率及等級相比,可確認併用時是否可降低副作用。
本說明書中明示引用之所有專利及參考文獻內容都編入本說明書中作為參考。
以下,將本發明藉由實施例之記載加以具體說明,惟,本發明不限定於根據該記載所解釋者。
對於以每隻1×107細胞之方式移植有人類ALK融合基因陽性非小細胞肺癌株NCI-H2228之重症複合型免疫不全(SCID)小鼠,在細胞移植後第17日或第20日將阿雷替尼(游離態、3mg/kg、1日1次經口投予15日)與選自DNA交聯劑順鉑、微小管阻礙劑太平洋紫杉醇、DNA合成阻礙劑
吉西他濱及抗人類VEGF抗體貝伐珠單抗之抗癌劑併用。各自進行順鉑(5mg/kg)每週1次靜脈內投予(第17、24日)、太平洋紫杉醇(12.5mg/kg)每週1次靜脈內投予(第20、27日)、吉西他濱(60mg/kg)每週2次腹腔內投予(第17、21、24、27日)、貝伐珠單抗(5mg/kg)每週1次腹腔內投予(第20、27日)。第1圖表示各投予群之腫瘤體積(平均值±標準偏差)及體重之變化率(平均值±標準偏差)。
此外,第1圖中,横軸表示細胞移植後之經過日數,緃軸表示腫瘤體積(Tumor volume)及體重之變化率(Body weight change rate),對照組為溶劑投予對照群,併用組表示併用群。
其結果,相對於阿雷替尼單劑群,各併用群顯示較高的抗腫瘤效果。特別是確認到以血管新生作為標靶之貝伐珠單抗單劑在投予中之腫瘤體積增加,然而於阿雷替尼與貝伐珠單抗之併用群中確認到腫瘤縮小,相對於阿雷替尼單劑群,併用群顯示統計學上顯著較高的抗腫瘤效果(P=0.008)。細胞障礙性抗癌劑順鉑、太平洋紫杉醇、吉西他濱單劑,在投予中未確認到腫瘤體積有大變動。

- 如申請專利範圍第1項所述之醫藥,其中,ALK阻礙劑及VEGF阻礙劑係個別被投予者。
- 如申請專利範圍第3項所述之醫藥,其中,ALK阻礙劑及VEGF阻礙劑係同時或依序被投予者。
- 一種用於治療或預防癌症之醫藥,其係含有ALK阻礙劑作為有效成分,且係與VEGF阻礙劑併用者。
