Description
本発明は、癌の治療または予防に有用な、ALK阻害剤及びVEGF阻害剤を組み合わせてなる医薬および癌を治療または予防するための方法、ならびに製品等に関する。
ALKの遺伝子異常(転座、点変異および遺伝子増幅)の結果、他の遺伝子と融合した異常キナーゼの生成を引き起こし、癌化に関与することが報告されている。
日本もしくは海外にて承認を受けたALK阻害剤として、アレセンサTM(一般名:アレクチニブ塩酸塩、Alectinib)、ザーコリTM(一般名:クリゾチニブ、Crizotinib)、ZykadiaTM(一般名:セリチニブ、Ceritinib)が挙げられる。
一般的に、抗癌剤の使用において、効果の増強、適用可能な腫瘍の種類を広げるため、あるいは耐性化した癌への対応や予防のために、作用機序の異なる複数の抗癌剤を用いることが行われている。
ALK阻害剤であるアレクチニブと他の分子標的薬との併用について報告がある。アレクチニブとエルロチニブ(EGFR(上皮成長因子受容体)のチロシンキナーゼ選択的阻害剤)又はクリゾチニブ(ALK及びc−Met(肝細胞増殖因子受容体)のチロシンキナーゼ阻害剤)とを併用した結果、アレクチニブ耐性の肺癌細胞株を移植したゼノグラフトモデルにおいて腫瘍の退縮効果が確認されたことが報告されている(非特許文献3:Cancer Res October 1,2014,74;3720,AACR Annual Meeting 2014;April 5−9,2014,Abstract 3720)。
アバスチン(ベバシズマブ)は、治癒切除不能な進行・再発の結腸・直腸癌、扁平上皮癌を除く切除不能な進行・再発の非小細胞肺癌、卵巣がん、手術不能又は再発乳癌について、特定の標準化学療法又は化学療法剤との併用を用法として、日本で承認を受けている。しかし、アバスチン(ベバシズマブ)は、VEGFパスウェイ阻害剤以外の分子標的薬との併用については、現時点で承認を受けていないが、VEGFパスウェイ阻害剤とEGFRのチロシンキナーゼ選択的阻害剤であるエルロチニブとの併用の効果や、VEGFパスウェイとc−MetやHGF/c−Metパスウェイの両方を阻害することによる腫瘍の抑制効果が報告されている。(非特許文献4:Lancet Oncol 2014;15:1236−44、非特許文献5:Cancer Discov.2012 March;2(3):270−287、非特許文献6:Mol Cancer Ther. 2015 Jan;14(1):101−110,“Combination strategy targeting VEGF and HGF/c−met in human renal cell carcinoma models”)。
本発明は、各種癌の治療や予防や、無増悪生存期間の延長に用いるため、複数の医薬を組み合わせてなる新規な医薬、及びそれを用いた癌の治療方法や製品等を提供することを課題とする。
即ち、本発明は以下の発明;

前記低分子化合物の例としては、WO2010/143664、WO2004/076412、WO2006/021881、WO2008/073687に記載の化合物、またはその塩などが挙げられる。
更に具体的な例としては、アレクチニブ(化合物名:9−エチル−6,6−ジメチル−8−[4−(モルホリン−4−イル)−ピペリジン−1−イル]−11−オキソ−6,11−ジヒドロ−5H−ベンゾ[b]カルバゾール−3−カルボニトリル 一塩酸塩)、クリゾチニブ(化合物名:3−[(1R)−1−(2,6−ジクロロ−3−フルオロフェニル)エトキシ]−5−[1−(ピペリジン−4−イル)−1H−ピラゾール−4−イル]ピリジン−2−アミン)、およびセレチニブ(化合物名:5−クロロ−N2−(2−イソプロポキシ−5−メチル−4−ピペリジン−4−イル−フェニル)−N4−[2−(プロパン−2−スルホニル)−フェニル]−ピリミジン−2,4−ジアミンからなる群より選択される化合物またはその塩などが挙げられる。これらの化合物またはその塩は、WO2010/143664、WO2004/076412、WO2006/021881、WO2008/073687等に記載の方法により、製造することができる。これらの化合物またはその塩には、水和物、製薬学的に許容可能な各種溶媒和物や結晶多形等も含まれる。
アレクチニブ、クリゾチニブおよびセレチニブの構造式を式(I)〜(III)として以下に示す。
ALK阻害剤は、水もしくはそれ以外の薬学的に許容し得る液との無菌性溶液または懸濁液剤等の注射剤の形で、非経口的にも使用され得る。これら製剤における有効成分量は指示された範囲の適当な用量を投与し得るように適宜選択される。注射のための無菌組成物は注射用蒸留水のようなベヒクルを用いて通常の製剤実施に従って処方され得る。注射用の水溶液としては、例えば生理食塩水、ブドウ糖やその他の補助薬を含む等張液、例えばD−ソルビトール、D−マンノース、D−マンニトール、塩化ナトリウムが例示され、適当な溶解補助剤、例えばアルコール、具体的にはエタノール、ポリアルコール、例えばプロピレングリコール、ポリエチレングリコール、非イオン性界面活性剤、例えばポリソルベート80(TM)、HCO−50と適宜併用され得る。油性液としてはゴマ油、大豆油が例示され、溶解補助剤として安息香酸ベンジル、ベンジルアルコールと併用してもよい。また、緩衝剤、例えばリン酸塩緩衝液、酢酸ナトリウム緩衝液、無痛化剤、例えば、塩酸プロカイン、安定剤、例えばベンジルアルコール、フェノール、酸化防止剤と好適に配合され得る。
本発明に係るALK阻害剤の投与量としては、例えば、一回の投与につき体重1kgあたり0.0001mgから1000mgの範囲で投与量が選択され得る。あるいは、例えば、患者あたり0.001mgから100000mg/bodyの範囲で投与量が選択され得る。しかしながら、本発明のALK阻害剤の投与量はこれらの投与量に制限されるものではない。
前記アレクチニブ、その塩、またはそれらの水和物のより具体的な投与量としては、1日2回、1回につきフリー体に換算して、20mg、40mg、60mg、80mg、120mg、160mg、220mg、240mg、300mg、460mg、600mg、760mg又は900mgがあげられる。
本発明のALK阻害剤の投与期間は、適宜症状や副作用の程度に応じて決定され、癌が治療され又は所望の治療効果が達成されるまでの間投与することができる。具体的には、連続して7日から3年間にわたり投与してもよく、または、投与期間中に1〜14日程度の休薬期間を設け、2日〜3ヶ月を1サイクルとして、1〜36サイクルで投与する方法などがある。好ましくは、14〜30日間を1サイクルとして、3〜24サイクルで投与する方法である。
本発明に用いられるVEGF阻害剤は、VEGFの活性を直接的に又は間接的に中和し、遮断し、阻害し、低減し又は妨害することが可能な物質を意味する。VEGF阻害剤は、一又は複数のVEGFレセプターへ結合することにより、VEGFの活性を中和し、遮断し、阻害し、低減し又は妨害してもよい。VEGF阻害剤には、抗VEGF抗体とその抗原結合断片、レセプター分子及びVEGFに特異的に結合して一又は複数のレセプターへのその結合を隔絶する誘導体、抗VEGFレセプター抗体及び抗VEGFレセプターアンタゴニスト(例えばVEGFRチロシンキナーゼの低分子阻害剤)が含まれる。低分子のVEGF阻害剤の具体例としては、ソラフェニブ(sorafenib、化合物名:N-(4-Chloro-3-(trifluoromethyl)phenyl)-N'-(4-(2-(N-methylcarbamoyl)-4-pyridyloxy)phenyl)urea、特許文献:US7235576)パゾパニブ(pazopanib、化合物名:5-[[4-[(2,3-dimethyl-2H-indazol-6-yl)methylamino]pyrimidin-2-yl]amino]-2-methylbenzenesulfonamide)、スニチニブ(sunitinib、化合物名:N-[2-(Diethylamino)ethyl]-5-[(Z)-(5-fluoro-2-oxo-1,2-dihydro-3H-indol-3-ylidene)methyl]-2,4-dimethyl-1H-pyrrole-3-carboxamide、特許文献:特許3663382、US6573293、US7125905)、アキシチニブ(axitinib、化合物名:N-Methyl-2-({3-[(1E)-2-(pyridin-2-yl)ethen-1-yl]-1H-indazol-6-yl}sulfanyl)benzamide)、レゴラフェニブ(regorafenib、化合物名:4-(4-(((4-chloro-3-(trifluoromethyl)phenyl)carbamoyl}amino)-3-fluorophenoxy)-N-methylpyridine-2-carboxamide)、カボザンチニブ(cabozantinib、化合物名:1,1-Cyclopropanedicarboxamide, N'-[4-[(6,7-dimethoxy-4-quinolinyl)oxy]phenyl]-N-(4- fluorophenyl)-)、ニンテダニブ(nintedanib、化学名:(Z)-methyl 3-((4-(N-methyl-2-(4-methylpiperazin-1-yl)acetamido)phenylamino)(phenyl)methylene)-2-oxoindoline-6-carboxylate、WO01/27081参照)からなる群より選択される化合物、その塩、又はそれらの水和物、等があげられる。
「抗体」なる用語は、最も広義に使用され、モノクローナル抗体(完全長又は無傷のモノクローナル抗体を含む)、ポリクローナル抗体、多価抗体、多重特異性抗体(例えば二重特異性抗体)、及びそれらが所望の生物活性を示す限り抗体断片(以下を参照)も含む。
本発明の一実施態様として、抗VEGF抗体は、限定しないが、ハイブリドーマATCC HB 10709により産生されるモノクローナル抗VEGF抗体 A4.6.1と同じエピトープに結合するモノクローナル抗体;Presta等(1997) Cancer Res. 57:4593-4599 に従って産生された組換えヒト化抗VEGFモノクローナル抗体が含まれる。別の実施態様では、抗VEGF抗体は、「rhuMAb VEGF」又は「アバスチン(登録商標)」としても知られている「ベバシズマブ(BV)」である。アバスチン(登録商標)は特定の国で市販されている。それは、そのレセプターへのヒトVEGFの結合をブロックするマウス抗hVEGFモノクローナル抗体A.4.6.1からの、抗原結合相補性決定領域及び変異したヒトIgG1フレームワーク領域を含む。フレームワーク領域の殆どを含む、ベバシズマブのアミノ酸配列のおよそ93%が、ヒトIgG1から誘導され、配列の約7%がマウス抗体A4.6.1から誘導される。さらに、ここで使用される場合、「ベバシズマブ(BV)」なる用語は、米国、欧州及び日本から成る国の群から選択される国又は地域において同一又はバイオシミラー製品として販売承認を得るのに必要な要求を満たしている、全ての対応抗VEGF抗体又は抗VEGF抗体断片を包含する。

本発明の1つの態様は、ALK阻害剤及びVEGF阻害剤を組み合わせてなる、癌を治療または予防するための医薬である。
上記態様における、「ALK阻害剤とVEGF阻害剤とを組み合わせてなる、癌を治療または予防するための医薬」とは、ALK阻害剤およびVEGF阻害剤を、癌の治療または予防において同時に、別個に、または、順次に投与するために組み合わせた医薬を意味する。本発明の医薬は、ALK阻害剤およびVEGF阻害剤を共に含有する配合剤の形で提供することもできる。また、ALK阻害剤を含有する製剤とVEGF阻害剤を含有する製剤とが別々に提供され、これらの製剤が、同時に、または順次に使用されてもよい。
本発明において、同時に投与するとは、ALK阻害剤と、VEGF阻害剤とを同時期に投与して併用して用いることをいい、配合剤として投与してもよく、投与時に用時調整した混合物として投与してもよく、別形態の製剤を同時期に投与してもよい。同時に投与して用いる場合、別々の経路から投与してもよく、同一の経路から投与してもよく、投与剤型も同一であってもよく、別々であってもよい。
本発明において、ALK阻害剤およびVEGF阻害剤を別個に投与する場合、ALK阻害剤とVEGF阻害剤の投与の順番は、VEGF阻害剤の投与後にALK阻害剤が投与され得るし、ALK阻害剤とVEGF阻害剤とが同時に投与され得るし、また、ALK阻害剤の投与後にVEGF阻害剤が投与され得る。
これとは逆に、ALK阻害剤の投与前にVEGF阻害剤が投与される場合には、ALK阻害剤による所望の効果が得られるような、対象におけるVEGF阻害剤の残留濃度が検出される時点で、ALK阻害剤が投与され得る。当該濃度は、対象から採取された試料を当業者において公知のELISA等の免疫的測定法で分析した結果に基づいて決定され得る。
別の観点においては、本発明は、VEGF阻害剤を有効成分として含む、ALK阻害剤と併用して癌を治療または予防するための医薬を提供する。本発明における、「VEGF阻害剤を有効成分として含む、ALK阻害剤と併用して癌を治療または予防するための医薬」とは、ALK阻害剤と併用することを条件に癌を治療または予防することに用いられる、VEGF阻害剤を有効成分として含む医薬を意味する。VEGF阻害剤を有効成分として含む医薬がALK阻害剤と併用される際には、ALK阻害剤と同時に投与され得るし、ALK阻害剤の投与前または投与後に投与され得る。ALK阻害剤の投与前または投与後にVEGF阻害剤が投与される場合には、対象における当該ALK阻害剤の残留濃度を測定することにより、その投与時期が最適化され得る。当該濃度は、対象から採取された試料を各種のクロマトグラフィー等の分離装置を用いた当業者において公知の分析方法に基づいて決定され得る。
上記発明は、ALK阻害剤とVEGF阻害剤とが共に投与または使用(以下、単に「投与」と指称される。)されることを意味するものであって、投与の順番や投与間隔等が限定されて解釈されるものでない。また、本発明のALK阻害剤とVEGF阻害剤とが組み合わされた製品としても使用され得る。さらに、本発明にしたがって、ALK阻害剤とVEGF阻害剤が併用される場合には、いずれか一方が単独で用いられる投与量よりも、所望により各々が少ない投与量で投与され得る。
本発明の医薬は、白血病(急性骨髄性白血病、慢性骨髄性白血病、急性リンパ性白血病、慢性リンパ性白血病など)、悪性リンパ腫(ホジキンリンパ腫、非ホジキンリンパ腫など)、脳腫瘍、神経芽細胞腫、神経膠種、甲状腺癌、骨髄異形成症候群、頭頸部癌、食道癌、胃癌、大腸癌、結腸直腸癌、乳癌、卵巣癌、肺癌、膵臓癌、肝臓癌、胆嚢癌、皮膚癌、悪性黒色種、腎癌、腎盂尿管癌、膀胱癌、子宮癌、精巣癌、前立腺癌のような種々の癌などの疾患の予防または治療に有用である。さらに本発明の化合物は、固形癌の浸潤・転移の予防または治療に有用である。
特に、本発明の医薬は、ALK融合遺伝子陽性の癌の予防または治療に有用である。ALK融合遺伝子陽性の癌の例としては、非ホジキンリンパ腫、神経芽細胞腫、炎症性筋繊維芽細胞腫、腎がんおよび肺癌などがあげられるが、これらに限定されない。
本発明において、「ALK融合遺伝子陽性の」とは、ALKと他の遺伝子との融合遺伝子が存在していることをいう。前記他の融合遺伝子としては、EML4(echinoderm microtubule-associated protein like protein 4, GenBankTMアクセッション番号NM_019063)、TFG(TRK-fused gene, GeneBank TM アクセッション番号 NM_006070)などがあげられるが、これらに限定されない。
ALK融合遺伝子陽性の癌を特定する方法は、WO2008/127248、特開2008−295444、「ALK遺伝子検査の手引き」(日本肺癌学会バイオマーカー委員会)、等に記載されている。当該融合遺伝子を検出するためのキットとして、Vysis LSI ALK Break Apart Rearrangement Probe Kit TM(Abbott社)、ヒストファイン ALK iAEPTMキット(ニチレイ社)が販売されている。
疾患のタイプ及び重篤度に応じて、例えばベバシズマブのような抗VEGF抗体の好ましい用量は、限定されないが、5mg/kg、7.5mg/kg、10mg/kg又は15mg/kgを含む約1μg/kgから約50mg/kgの範囲であり得、最も好ましくは約5mg/kgから約15mg/kgの範囲であり得る。投与の頻度は疾患のタイプと重篤度に応じて変動する。数日又はそれ以上にわたる繰り返し投与では、症状に応じて、治療は、当該分野で知られている方法によって測定して、癌が治療され又は所望の治療効果が達成されるまで維持される。一例では、抗VEGF抗体は、毎週一回、2週毎、又は3週毎に、限定しないが、5mg/kg、7.5mg/kg、10mg/kg又は15mg/kgを含む約5mg/kgから約15mg/kgの用量範囲で投与される。しかしながら、他の用量レジメンも有用であり得る。
VEGF阻害剤は、上記毎週毎、2週毎、又は3週毎の投与を1サイクルとして、癌が治療され又は所望の治療効果が達成されるまでの間投与することができる。具体的には1〜36サイクルに渡り投与することができる。
本発明において、「無増悪生存期間を延長させる」の具体例としては、ALK阻害剤を単剤で投与した際の無増悪生存期間と比較して、無増悪生存期間が延長することが挙げられる。
本発明において、「対象」とは、限定しないが、本発明の医薬の投与対象となる、ヒト又は非ヒト哺乳動物、例えばウシ、ウマ、イヌ、ヒツジ、又はネコを含む哺乳動物を意味する。好ましくは、対象はヒトである。対象には患者(ヒト及び非ヒト哺乳動物を含む)が含まれる。
本発明の別の態様として、製品が提供される。一実施態様では、製品は対象における癌を治療または予防するために提供され、(a)(1)VEGF阻害剤を含有する製剤、(2)容器、(3)対象における癌を治療するために前記VEGF阻害剤と少なくとも一種のALK阻害剤とを組み合わせて対象に投与することを示す指示書又はラベル、または(b)(1)ALK阻害剤を含有する製剤、(2)容器、(3)対象における癌を治療するために前記ALK阻害剤と少なくとも一種のVEGF阻害剤とを組み合わせて対象に投与することを示す指示書又はラベル、を含む。
製品は、VEGF阻害剤またはALK阻害剤を含有する製剤を含む容器を含む。
製品は、容器上または容器に付随したラベルまたは指示書をさらに含み得る。
本発明において「指示書」とは、製剤の使用に関する適応症、用法、投与量、投与、禁忌症および/または警告についての情報を含む、製剤の商業用パッケージ中に通常含まれる書類を意味する。「ラベル」とは、VEGF阻害剤またはALK阻害剤を含有する製剤の製品名、用量、剤型、適応症等の表示を含むシート状の物品であり、容器に直接貼付されるものを意味する。
ラベルまたは指示書は、製剤の適応症、すなわち癌などの選択した疾患の治療に使用されることを示す。一実施態様では、ラベルまたは指示書は、製剤が癌を治療するために使用することができることを示す。ラベルまたは指示書はまた、製剤が他の障害を治療するために使用することができることを示し得る。
適切な容器には、例えば、PTP、ビン、バイアル、シリンジ、ブリスターパックなどが挙げられる。容器は、ガラスまたはプラスチックなどの種々の材料から形成し得る。これらの容器が更に上記ラベルの内容等を印刷した紙製の外箱に梱包されていてもよい。
以下、本発明を実施例の記載によって具体的に説明するが本発明は当該記載によって限定して解釈されるものではない。
ヒトALK融合遺伝子陽性非小細胞肺癌株NCI−H2228を1匹あたり1×107細胞移植した重症複合型免疫不全(SCID)マウスに対して、細胞を移植した後17日目または20日目にアレクチニブ(フリー体、3mg/kg、1日1回15日間経口投与)をDNA架橋剤シスプラチン、微小管阻害剤パクリタキセル、DNA合成阻害剤ゲムシタビン、および抗ヒトVEGF抗体ベバシズマブから選択される抗癌剤と併用した。シスプラチン(5mg/kg)は週1回静脈内投与(Days17、24)、パクリタキセル(12.5mg/kg)は週1回静脈内投与(Days20,27)、ゲムシタビン(60mg/kg)は週2回腹腔内投与(Days17、21、24、27)、ベバシズマブ(5mg/kg)は週1回腹腔内投与(Days20、27)をそれぞれ行った。各投与群の腫瘍体積(平均値±標準偏差)と体重の変化率(平均値±標準偏差)を図1に示した。
なお、図1において、横軸は、細胞移植後の経過日数、縦軸は、腫瘍体積(Tumor volume)と体重の変化率(Body weight change rate)を示し、controlは溶媒投与対照群、comboは併用群を示す。
その結果、各併用群は、アレクチニブ単剤群と比較して高い抗腫瘍効果を示した。特に、血管新生を標的とするベバシズマブ単剤では投与中に腫瘍体積の増加が確認されたものの、アレクチニブとベバシズマブとの併用群では腫瘍の縮小が確認され、アレクチニブ単剤群と比較して統計学的に有意に高い抗腫瘍効果を示した(P=0.008)。細胞障害性抗がん剤シスプラチン、パクリタキセル、ゲムシタビン単剤では、投与中に腫瘍体積の大きな変動は認められなかった。
Claims (15)
- ALK阻害剤とVEGF阻害剤とを組み合わせてなる、ALK融合遺伝子陽性の癌を治療または予防するための医薬であって、前記ALK阻害剤が、アレクチニブまたはその塩であり、前記VEGF阻害剤が、抗VEGF抗体であるベバシズマブである、医薬。
- 前記医薬が配合剤であることを特徴とする、請求項1に記載の医薬。
- ALK阻害剤とVEGF阻害剤とが別個に投与されることを特徴とする、請求項1に記載の医薬。
- ALK阻害剤とVEGF阻害剤が同時に又は順次に投与されることを特徴とする、請求項3に記載の医薬。
- ALK阻害剤を有効成分として含む、VEGF阻害剤と併用してALK融合遺伝子陽性の癌を治療または予防するための医薬であって、前記ALK阻害剤が、アレクチニブまたはその塩であり、前記VEGF阻害剤が、抗VEGF抗体であるベバシズマブである、医薬。
- VEGF阻害剤と同時に投与されることを特徴とする、請求項5に記載の医薬。
- VEGF阻害剤の投与前または投与後に投与されることを特徴とする、請求項5に記載の医薬。
- VEGF阻害剤を有効成分として含む、ALK阻害剤と併用してALK融合遺伝子陽性の癌を治療または予防するための医薬であって、前記ALK阻害剤が、アレクチニブまたはその塩であり、前記VEGF阻害剤が、抗VEGF抗体であるベバシズマブである、医薬。
- ALK阻害剤と同時に投与されることを特徴とする、請求項8に記載の医薬。
- ALK阻害剤の投与前または投与後に投与されることを特徴とする、請求項8に記載の医薬。
- アレクチニブまたはその塩が、1日2回、1回につきフリー体に換算して20mg、40mg、60mg、80mg、120mg、160mg、220mg、240mg、300mg、460mg、600mg、760mg又は900mg投与されることを特徴とする請求項1〜10に記載の医薬。
- 抗VEGF抗体が、1回につき、1μg/kgから50mg/kg投与されることを特徴とする請求項1〜11に記載の医薬。
- 抗VEGF抗体が、1回につき、5mg/kg、7.5mg/kg、10mg/kg又は15mg/kg投与されることを特徴とする請求項1〜11に記載の医薬。
- 抗VEGF抗体が、毎週一回、2週毎、又は3週毎に投与される、請求項1〜13のいずれかに記載の医薬。
