Изобретение относитс  к области переработки полимерных материалов, в частности к литьевым формам дл  изготовлени  изделий из пластмасс с наклонными отверсти ми . Цель изобретени  - упрощение конб струкции формы. Литьева  форма дл  изготовлени  полимерных изделий содержит неподвижную I и подвижную 2 плиты, плиту съема 3, расположенный в плоскости, перпендикул рной плоскости разъема, наклонный формующий знак, оканчивающийс  хвостовиком , один участок 5 которого взаимодействует с плитой съема, а другой 6 - с подвижной плитой, плиту выталкивани  7, св занную т гами 8 с плитой съема. Участок хвостовика знака, взаимодействующий с подвижной плитой, выполнен в виде наклонного стержн  с углом наклона к плоскости разъема , одинаковым с участком хвостовика, взаимодействующим с плитой съема.
