Abstract
Description
Изобретение относится к медицине, а именно к травматологии и может быть использовано при интрамедуллярном остеосинтезе бедренной кости. Необходимость в этом возникает в случаях блокирования динамического проксимального винта латеральным кортикальным слоем кости, что делает невозможным необходимую динамизацию внутрикостного штифта.The invention relates to medicine, namely to traumatology, and can be used for intramedullary osteosynthesis of the femur. Интрамедуллярные штифты первых поколений отличались тем, что блокирование их осуществлялось лишь статическим методом. За счет блокирования штифта на проксимальном и дистальном концах кости, значительно повышая жесткость системы кость-имплантат, что в ряде случаев приводило к нарушению механических и биологических факторов остеорепарации, результатом чего являлось нарушение консолидации перелома [Hu M, Zeng W, Zhang J, Feng Y, Ma L, Huang F, Cai Q. В дальнейшем данная концепция нашла отражение в разработке интрамедуллярных штифтов с наличием продольного отверстия для блокирования штифта [Eveleigh RJ. There was a displacement of the proximal bone fragment along the pin and, thus, closure of the bone fragments with each other was achieved, which was called the term “dynamization”.

В 2013 Biber et al. сообщили о 8 случаях, когда выполнение осевой межфрагментарной динамизации у пациентов было затруднено тем, что динамический винт упирался в латеральный кортикальный слой бедренной кости способствуя появлению боли, нестабильности имплантата, а в дальнейшем поломки его и нарушению консолидации перелома. Для решения данной проблемы авторы предложили и успешно реализовали хирургический способ, суть которого заключалась в кортикотомии латерального кортикального слоя сразу под динамическим винтом и блокирования штифта динамическим методом, назвав данную методику - латеральная кортикальная насечка (lateral cortical notching - LCN) [Biber R, Bail HJ, Stedtfeld HW. Epub 2013 Jan 18].In 2013 Biber et al. reported 8 cases where performing axial interfragmentary dynamization in patients was complicated by the fact that the dynamic screw rested on the lateral cortical layer of the femur, causing pain, instability of the implant, and subsequently its breakage and impaired consolidation of the fracture. To solve this problem, the authors proposed and successfully implemented a surgical method, the essence of which was corticotomy of the lateral cortical layer immediately under the dynamic screw and blocking the pin using a dynamic method, calling this technique lateral cortical notching (LCN) [Biber R, Bail HJ , Stedtfeld HW. Данный метод предполагает кортикотомию с использованием долота, что предполагает расширение операционной раны, которая была сформирована для установки динамического винта в шейку бедренной кости. Технический результат заявленного изобретения достигается за счет того, что для проведения послеоперационной динамизации проксимальных бедренных штифтов, включающий проведение латеральной фенестрации - кортикотомии, особенность заключается в том, что после полной установки проксимального бедренного штифта и демонтажа направляющего оборудования сквозь ранее сформированный операционный доступ для установки динамического винта в канал динамического винта вводят спицу и по ней, на установленный винт, повторно надевают установочную втулку, которая используется в качестве «защитника» винта при проведении фенестрации, далее, под установленной ранее втулкой, параллельно во всех плоскостях, до упора в латеральный кортикальный слой кости вводят аналогичную втулку с направляющим устройством и сквозь него в кость проводят спицу, по спице полым сверлом производят фенестрацию латерального кортикального слоя кости дистально от установленного ранее динамического винта.The technical result of the claimed invention is achieved due to the fact that for postoperative dynamization of proximal femoral pins, including lateral fenestration - corticotomy, the peculiarity is that after complete installation of the proximal femoral pin and dismantling of the guide equipment through the previously formed surgical access for installing a dynamic screw a pin is inserted into the channel of the dynamic screw and along it, onto the installed screw, the installation sleeve is re-putted, which is used as a “protector” of the screw during fenestration, then, under the previously installed sleeve, parallel in all planes, until it stops in the lateral cortical layer of the bone a similar sleeve with a guide device is inserted and a pin is passed through it into the bone; a hollow drill is used to fenestrate the lateral cortical layer of the bone along the pin, distal to the previously installed dynamic screw.

Способ осуществляется следующим образом.The method is carried out as follows.
Пациента укладывают на ортопедический стол, осуществляют тракцию за стопу травмированной конечности, что позволяет выполнить репозицию (при необходимости) костных фрагментов. Осуществляют блокируемый цефаломедуллярный остеосинтез проксимальным бедренным штифтом и демонтируют установочный инструментарий. Сквозь ранее сформированный операционный доступ (для установки динамического винта) в канал динамического винта вводят спицу и по ней, на установленный винт, повторно надевают установочную втулку (Фиг. 1), которая используется в качестве «защитника» винта при проведении фенестрации. Далее, под установленной ранее втулкой, параллельно (во всех плоскостях), до упора в латеральный кортикальный слой кости вводят аналогичную втулку (Фиг. 2) с направляющим устройством (Фиг. 3) и сквозь него в кость проводят спицу (Фиг. 4). По спице полым сверлом производят фенестрацию латерального кортикального слоя кости дистально от установленного ранее динамического винта (Фиг. 5). Раны послойно ушивают. Накладывают асептические повязки.The patient is placed on an orthopedic table, traction is applied to the foot of the injured limb, which allows repositioning (if necessary) of bone fragments. Next, under the previously installed bushing, parallel (in all planes), a similar bushing (Fig. 2) with a guide device (Fig. 3) is inserted into the lateral cortical layer of the bone until it stops, and a pin is passed through it into the bone (Fig. 4). Along the wire, a hollow drill is used to fenestrate the lateral cortical bone layer distally from the previously installed dynamic screw (Fig. 5). The wounds are sutured layer by layer. Apply aseptic dressings.

Пациент С. 66 лет, после выполненного цефаломедуллярного блокируемого остеосинтеза по поводу закрытого подвертельного перелома бедренной кости, спустя 9 месяц почувствовал боль в области тазобедренного сустава и был госпитализирован с диагнозом: несросшийся подвертельный перелом правой бедренной кости с наличием сломанного интрамедуллярного штифта. Пациент во время операции уложен на ортопедический стол. После удаления сломанного штифта произведено вытяжение таргетной конечности и проксимальный интрамедуллярный реостеосинтез бедренной кости. Далее произведена под динамическим финтом установленного блокируемого штифта фенестрация латерального кортикального слоя бедренной кости в соответствии с приведенной выше техникой оперативного вмешательства, так как винт, установленный в шейку бедренной кости, препятствовал последующей динамизации штифта.
