Abstract
Description
The invention relates to medicine, namely to proctology and emergency surgery, and can be used for surgical treatment, mainly of patients with acute paraproctitis.
The most common disease encountered in the practice of the department of emergency proctology is acute paraproctitis - inflammation of the perirectal tissue, usually emanating from the anal canal. The pathogenesis of acute paraproctitis is based on acute (cryptitis, papillitis) or chronic (hemorrhoids, anal fissures, etc.) lesions of anal crypts, which ultimately leads to the penetration of infection along the anal ducts into the cellular spaces of the small pelvis.
The main part of patients with acute paraproctitis comes with a clinical picture of a non-draining pararectal abscess. In these cases, emergency surgery is indicated. The operation should be performed as soon as possible from the time of diagnosis. The delay is fraught with the danger of the spread of the purulent process, the destruction of the muscle structures of the sphincter, the pelvic floor and the walls of the rectum. Operations are performed under general anesthesia, since the use of local anesthesia in a purulent process is inadequate in terms of the degree of anesthesia and may lead to the spread of infection.
The results of bacteriological monitoring during aerobic processes, carried out by puncture of the abscess. They showed that Gram-positive Staphylococcus aureus and Streptococcus are no longer monocauses or even leaders of microbial associations today. Their place was firmly taken by gram-negative microorganisms, in which, as you know, inflammatory processes develop more rapidly and destructively due to the action of enzymes and endotoxins released during their death.
This makes it necessary to take all possible measures to reduce the possibility of spreading the infection during the surgical intervention.
A known method of surgical treatment of acute paraproctitis with transsphincteric or extrasphincteric purulent fistulous tract [RU 2190972, C2, A61B 17/00, 20.10.2002], including opening and drainage of the abscess, and, through the purulent course, an elastic latex strip is carried out, its ends are fixed with tension on the skin, moving the place of fixation of the strip, ensuring its constant tension until the anal sphincter fibers erupt along with the purulent tract.
The disadvantage of this method is a relatively narrow scope, as well as a pronounced pain syndrome associated with the gradual eruption of tissues with a tension latex strip and extensive surgical trauma to the anal canal mucosa, a high risk of anal incontinence due to trauma to the rectal sphincter fibers and the formation of a scar at the site of the erupted latex strips, the impossibility of outpatient treatment and a long period of labor rehabilitation, the possibility of developing pronounced purulent-inflammatory changes in the mucosal suture due to the use of catgut threads, one of the most reactogenic, allergenic suture materials, with unpredictable resorption times and high absorption capacity.
The closest in technical essence to the proposed one is a method for the treatment of acute ischiorectal paraproctitis [RU 2691559, C1, A61B 17/00, 06/14/2019], including opening and draining the abscess, identifying the internal fistula opening, holding a free ligature, in which one of its ends is removed through the internal fistulous opening, rectal lumen and anus, removal of the free ligature and coagulation of the fistulous tract using FiLaC technology at a laser radiation wavelength of 1470 nm, while the other end of the free ligature is brought to the skin of the perineum in the projection of the affected crypt at a distance of 2 -3 cm from the anoderm, after which an even number of punctures are performed on the perineum along the length of the abscess under ultrasound control every 1.5-2 cm with a scalpel, through each pair of which an additional free ligature is carried out, which are removed with a decrease in the number of peripheral blood leukocytes in dynamics by 1.0×10 9 /l and more, normal body temperature and change in all ligatures from purulent cavities from purulent to serous exudate, the free ligature is removed after the formation of a fibrous capsule throughout the fistulous tract, after which the internal fistulous opening is sutured with an atraumatic absorbable thread with two separate interrupted sutures and the fistulous tract is coagulated with a laser radiation with the following parameters: pulse duration - 0.9 s, pulse repetition period - 1.0 s, while part of the fistulous tract over 10-15 mm from the internal fistula opening is coagulated with laser radiation of 10 W, and the rest - with laser radiation 8 W.
This method of treatment is used mainly for ischiorectal paraproctitis with fistula and is associated with the passage of a draining ligature through the fistulous tract.
The disadvantage of the closest technical solution is the relatively low efficiency of treatment, due to the relatively high possibility of infection during the operations of the method, including putrefactive and anaerobic forms.
The problem that is solved in the invention is aimed at creating a method that provides a higher efficiency of treatment and reduces the possibility of spreading infection during the operations of the method, including putrefactive and anaerobic forms.
The required technical result is to increase the effectiveness of treatment and reduce the possibility of infection spreading during the operations of the method, including putrefactive and anaerobic forms.
The problem is solved, and the required technical result is achieved by the fact that, in the method according to which an abscess is opened and drained, the internal fistulous opening is identified and necrotic tissues are excised in the abscess cavity, according to the invention, necrotic tissue is excised with an end light guide at a radiation power of 15 W and a wavelength of 1460 nm, and purulent cavities are exposed to scattered laser radiation with radial light guides with a lens for a bactericidal and bacteriostatic effect and an abacterial film is formed in the opening area, after which it is sanitized with a polyvalent pyobacteriophage solution and finally the wound is loosely plugged and drained.
The drawing shows a laser device used to implement the method.
3.The device contains a
The proposed method for the treatment of paraproctitis is as follows.
The method can be used in almost any form of paraproctitis subcutaneous-submucosal, retrorectal,
The method effectively prevents the spread of infection, including putrefactive and anaerobic forms. To implement the proposed method of treatment, it is possible to use radial light guides (see drawing) with both a club-shaped lens at the end and a conical lens. A 1460 nm diode laser with a power of 12-15 W is used as a radiation source. Drainage ligature is not carried out, surgical exposure from the rectum is not performed. The purulent cavity is sanitized and necrectomy is performed - excision of necrotic tissues in the abscess cavity. To do this, use an end light guide with a radiation power of 15 W and a wavelength of 1460 nm. In addition, all purulent cavities are exposed to scattered laser radiation using radial light guides with a lens for bactericidal and bacteriostatic effect, which leads to the formation of an abacterial film in the entire wound. The method is supplemented by debridement of the focus with a solution of a polyvalent pyobacteriophage, and at the end of the operation, the wound is loosely plugged and drained, without a ligature.
After the operation, outpatient therapy is prescribed, based on the culture obtained from the wound. Symptomatic therapy is performed and the wound is monitored for up to 14 days. In the delayed period, after stopping the acute process (acute paraproctitis, all its forms), after 14-30 days, the rectum is examined - rectoscopy, ultrasound of the rectum, MRI with contrasting of the rectum - in order to verify the purulent or fistulous course and subsequently determine the subsequent treatment tactics.

In total, 45 paraproctitis were treated during the analyzed period.
4 patients had complications in the postoperative period. Of these, 2 patients had necrosis of the reduced mucosa after plastic surgery and 2 patients had suppuration of the postoperative wound after suturing the sphincter.
Often, under the guise of acute paraproctitis, patients with purulent diseases of the perianal region of a different etiology are admitted to the department. Among the patients treated by the author, these were festering hematomas after various injuries and festering atheromas. When diagnosing festering atheroma, an abscess was opened and drained. In the presence of festering atheroma, excision was performed in all cases.
