Раневая инфекция в настоящее время является основным видом осложнений в хирургии, отмечается увеличение частоты гнойных заболеваний и послеоперационных нагноений, которая составляет 23-60% (Инфекция в хирургии: руководство / А.Н. Известен способ дренирования послеоперационной раны с помощью перфорированной трубки (Давыдов Ю.А., Ларичев А.Б. "Вакуум-терапия ран и раневой процесс". М.: Медицина, 1999, 160 с), заключающийся в следующем. Для дренирования авторы использовали силиконовые или полихлорвиниловые трубки диаметром 0,4-0,5 см. На боковой поверхности трубки, находящейся в раневой полости, вырезают отверстия на всем протяжении, после чего трубку укладывают на дно раны. Один из концов трубки выводят наружу через отдельный прокол кожи и фиксируют к ней узловым швом. Второй конец оставляют в ране. Затем частично, до соприкосновения, ушивают подкожную клетчатку и герметично - кожу. К выведенному наружу концу трубки подсоединяют вакуум-аспиратор, который работает постоянно, либо с перерывами. Дренаж из раны убирают на 3-4 послеоперационные сутки. Применяемое им закрытое аспирационно-промывное лечение обеспечивает изоляцию раневой полости от внешней среды. Швы накладывают только на кожу, оставляя все остальные слои гнойной раны не ушитыми. Перед ушиванием кожных краев раны в полость устанавливают через отдельный прокол кожи и подлежащих тканей двухпросветную дренажную трубку. К микроканалу трубки подключают систему капельного вливания, а к широкому каналу - система аспирации. Способ дренирования ушитых ран апробирован на 19 больных, находившихся на лечении в отделении гнойной хирургии Краевой клинической больницы и 12 пациентах хирургического отделения Центральной районной больницы, и показал хорошие результаты. Из 31 больного с использованием данного способа нагноения не произошли ни у одного пациента. На рану наложена вакуумная повязка. 15.09.20 и 18.09.20 производились этапные хирургические обработки раны со сменой вакуумной повязки. 21.09.21 на очередной перевязке отмечено полное очищение раны, появление зрелой грануляционной ткани, купирование воспаления. В условиях операционной произведено ушивание раны редкими широкозахватными отдельными узловыми швами монофиламентной нитью с захватом всех стенок и дна.
