Проблема лечения гнойных заболеваний мягких тканей, и в частности флегмон предплечья, остается одной из наиболее сложных и социально-значимых проблем современной хирургии. Основной метод лечения данного заболевания - хирургический, включающий вскрытие гнойного очага, дренирование гнойной полости для обеспечения адекватного оттока экссудата. Воспалительно-дегенеративные процессы, происходящие в тканях при длительном заживлении раны, как правило, приводят к выраженным рубцовым изменениям в зоне оперативного вмешательства, косметическим дефектам, снижению функциональной способности конечности и качества жизни пациентов.The problem of treating purulent diseases of soft tissues, and in particular phlegmon of the forearm, remains one of the most complex and socially significant problems of modern surgery. Для дренирования используют дренажную трубку из эластичного материала, выполненную гофрированной по спирали. В середине трубки на половину длины раневой полости по внутренней спирали наносят линейный разрез. Гнойную полость вскрывают, очищают, промывают после чего на ее дно укладывают трубку в сомкнутом состоянии витков спирали. Концы трубки выводят через отдельные проколы и подключают к системе капельного вливания. В процессе регенерации раны спираль каждые 12 часов растягивают за наружные отрезки трубки и поворачивают за выступающие части трубки на один шаг спирали гофра, с одной стороны - походу часовой стрелки, с другой - против хода часовой стрелки. При отсутствии отделяемого трубку удаляют. Способ выполняют следующим образом. Оба конца силиконовой трубки выводят наружу через дополнительные разрезы-проколы на расстоянии 3-5 см от углов раны, причем перфорационные отверстия оставляют полностью на дне раны. Рану ушивают, оба конца трубки фиксируют к коже лигатурой. На один конец трубки надевают заглушку, а на другой - гармошку. Периодически 1-2 раза в день заглушку и гармошку снимают, в полученном по дренажу экссудате исследуют уровень ферритина, через конец трубки, где стояла заглушка шприцем вводят 50-100 мл раствора антисептика, надевают на противоположный конец трубки гармошку и принудительно вручную растягивают ее, удаляя содержимое из послеоперационной раны. Затем гармошку снимают, промывают, и на оба конца трубки опять надевают заглушку и гармошку. В работе Ю.З. Лифшиц, Васецкий A.M., Зайченко П.А., Доскуч О.А. Успешное лечение больного по поводу флегмоны правого предплечья с применением метода отрицательного давления // Клiнiчна xipуpгiя - 2015, №3, с. 77-78 описана методика дренирования флегмоны предплечья. Для местного лечения раны применяют метод воздействия отрицательного давления с использованием системы VOUNDEX. При этом осуществляют 3 смены VAC-повязок с интервалом 72 часа при постоянном режиме отрицательного давления 120 мм рт.ст. Каждую смену VAC-повязки сопровождали наложением адаптационных наводящих швов на края раны из-за уменьшения отека тканей. На 9 сутки лечения выполнили аутодермопластику и наложили вакуумную повязку в постоянном щадящем режиме отрицательного давления. Экспозиция повязки - 78 часов. Общая продолжительность лечения - 32 дня. Недостатком данного способа является его сложность, длительность лечения, а также невозможность судить о его эффективности по результату единичного наблюдения.In the work of Yu.Z. Приводящую силиконовую трубку по длине раны 10 см перфорировали 4 рядами расположенных по кругу отверстий, каждое диаметром 3 мм, нанесенных вдоль трубки через 5 мм в шахматном порядке. Отводящую силиконовую трубку по длине раны 10 см перфорировали 5 рядами расположенных по кругу отверстий овальной формы, каждое длиной 6 мм и шириной 2 мм, нанесенных вдоль трубки через 7 мм в шахматном порядке. Приводящую и отводящую силиконовые трубки уложили на дно раны параллельно друг другу на расстоянии 12 мм между собой так, чтобы перфорационные отверстия на них располагались в шахматном порядке друг относительно друга. Проксимальные концы приводящей и отводящей силиконовых трубок вывели через два отстоящих от противоположных углов раны на расстоянии 3,5 см каждый разрез-прокола, после чего проксимальный конец каждой трубки зафиксировали к коже П-образным швом нитью Викрил 1/0. Края раны сблизили и зафиксировали наклеиванием пленочной повязки на раны Suprasorb F производства компании Lohmann & Rauscher (Австрия), после чего поверх раны наложили как повязку подкладочную синтетическую вату ЦЕЛЛОНА производства компании Lohmann & Rauscher (Австрия). Активное дренирование раны выполняли с помощью перфузора - инфузионной шприцевой помпы модели 2016 производства фирмы AITECS (Литва), с постоянной скоростью введения промывной жидкости 170 мл/час в течение 12 часов с интервалом 6 часов. При этом через приводящую силиконовую трубку вводили промывную жидкость - растворенные в 2000 мл 0,9% раствора натрия хлорида лекарственные препараты в дозах: диоксидин - 50 мл, пепсин - 20 мг, цефотаксим - 2 г. Данную промывную жидкость вводили до достижения воспалительного типа цитограммы отделяемого из раны. Воспалительный тип цитограммы определяли по методике, описанной в работе Кузин М.И., Костюченок Б.М. Раны и раневая инфекция - изд 2-е. - М.: Медицина, 1990. - 592 с. При этом цитологические отпечатки получали по методу Покровской и Макарова (Кузин М.И., Костюченок Б.М. Раны и раневая инфекция - изд 2-е. - М.: Медицина, 1990. - 592 с.). На 3 сутки после начала активного дренирования был достигнут воспалительный тип цитограммы отделяемого из раны, который определили по следующим показателям: лейкоциты в поле зрения - 8%, деструкция лейкоцитов - 69%, нейтрофилы - 49%, лимфоциты - 8%, моноциты - 1%, макрофаги - 16%, полибласты - 26%; микрофлора обнаружена в умеренном количестве внутриклеточно, в состоянии не полностью незавершенного фагоцитоза. После этого введение пепсина и цефотаксима прекратили, а введение с помощью перфузора - помпы шприцевой инфузионной модели 2016 производства фирмы AITECS (Литва) с постоянной скоростью введения 170 мл/час в течение 12 часов с интервалом 6 часов растворенных в 2000 мл 0,9% раствора натрия хлорида 50 мл диоксидина продолжали. На 7 сутки после начала активного дренирования, из отводящей силиконовой трубки начала отходить чистая промывная жидкость, после чего обе дренажные силиконовые трубки удалили. Active wound drainage was performed using a perfuser - an infusion syringe pump model 2016 manufactured by AITECS (Lithuania), with a constant rate of injection of washing liquid 170 ml / hour for 12 hours with an interval of 6 hours. At the same time, a washing liquid was introduced through a lead-in silicone tube - drugs dissolved in 2000 ml of 0.9% sodium chloride solution in doses: dioxidine - 50 ml, pepsin - 20 mg, cefotaxime - 2 g. С помощью заявляемого способа в районных больницах Песчано-копского и Сальского районов Ростовской области было выполнено дренирование глубоких (12 случаев) и поверхностных (8 случаев) флегмон предплечья у 20 больных.
