Изобретение относится к области медицины, в частности к сердечнососудистой хирургии, и может быть использовано как альтернативный малотравматичный способ лечения варикозной болезни нижних конечностей (ВБНК).The invention relates to medicine, in particular to cardiovascular surgery, and can be used as an alternative low-traumatic method for the treatment of varicose veins of the lower extremities (VBNA).
В настоящее время существуют три основных направления в лечении варикозной болезни нижних конечностей (ВБНК): хирургические открытые операции, термические тумесцентные методики (эндовенозная лазерная коагуляция (ЭВЛК) и радиочастотная облитерация (РЧО)) и нетермические нетумесцентные (НТНТ) методики (пенная склерооблитерация и цианоакрилатная облитерация). Недостатками открытых операций, несмотря на высокий процент успеха и хороший отдаленный результат, являются необходимость анестезии в том или ином виде (местная, спинномозговая, общая), чаще всего невозможность выполнения в амбулаторных условиях и, что самое главное, высокая травматичность в сравнении с малоинвазивными методами (ЭВЛК, РЧО, НТНТ) ввиду необходимости выполнения открытого доступа к сосудам и удаления (стриппинга) ствола большой или малой подкожной вены (БПВ и МПВ) на протяжении. В последнее время ЭВЛК и РЧО стали золотым стандартом в лечении ВБНК ввиду своей малотравматичности и возможности выполнения в амбулаторных условиях с очень хорошими отдаленными результатами облитерации стволов подкожных вен приближающимся к 100%. При этом термальные методы имеют ряд недостатков, основным из которых является необходимость выполнения тумесцентной анестезии, что вызывает у пациента значительный дискомфорт, а порой и выраженный болевой синдром во время процедуры. С целью минимизации болевых ощущений были разработаны НТНТ методы, для выполнения которых необходима только местная анестезия кожи в месте пункции вены. Одним из видов НТНТ методов является цианоакрилатная облитерация вен. В настоящее время в мире используются две основные системы: VenaSeal (Medtronic, Santa Rosa, США) и VaricloseVR (Biolas Inc., Ankara, Турция). В России зарегистрирована единственная методика цианоакрилатной облитерации - VenaSeal [https://www.youtube.com/watch?v=rOtBm-OFN3I&t=5s]. Цианоакрилатная облитерация показала хорошие отдаленные результаты в рандомзированном исследовании в течении двух лет, сопоставимые с ЭВЛК и РЧО [Eroglu Е, Yasim A. Recently, EVLK and RFO have become the gold standard in the treatment of VBNA due to their low-traumatic nature and the possibility of performing it on an outpatient basis with very good long-term results of obliteration of the saphenous vein trunks approaching 100%. At the same time, thermal methods have a number of drawbacks, the main of which is the need to perform tumescent anesthesia, which causes the patient considerable discomfort, and sometimes severe pain during the procedure. In order to minimize pain, NTNT methods were developed, for which only local anesthesia of the skin at the site of the puncture of the vein is necessary. One of the types of NTNT methods is cyanoacrylate venous obliteration. Currently, two main systems are used in the world: VenaSeal (Medtronic, Santa Rosa, USA) and VaricloseVR (Biolas Inc., Ankara, Turkey). In Russia, the only method of cyanoacrylate obliteration is registered - VenaSeal [https://www.youtube.com/watch?v=rOtBm-OFN3I&t=5s]. Заявляемый способ позволяет избежать вышеописанные недостатки за счет использования специальных приемов и применения отечественного биологического клея «Сульфакрилат», который имеет регистрационное удостоверение с 2000 г., прошел все клинические испытания и довольно широко применяется в хирургической практике [http://www.sulfakrilat.ru]. Поставленная задача решается способом, заключающимся в следующем: Положение пациента на операционном столе на спине, с несколько отведенной кнаружи и немного согнутой в коленном суставе оперируемой ногой с приподнятой нижней частью стола по типу положения Тренделенбурга (фото 1). Далее производится подача клеевой композиции по клеевому катетеру и постепенное выведение последнего из просвета вены (фото 15). По завершению процедуры производится У3-контроль заполнения клеевой композиции по вене (фото 16, 17). This leads to further emptying of the vein and comparison of the walls of the vessel around the adhesive catheter (photo 14). After completing the tightening of the introducer sheath, the faucet is set to the “closed” position, thus negative pressure ceases to act on the venous vessel. Next, the adhesive composition is delivered through the adhesive catheter and the latter is gradually removed from the lumen of the vein (photo 15). At the end of the procedure, a U3 control of filling the adhesive composition through a vein is performed (photo 16, 17). Then the patient leaves the operating room on his own.
В настоящее время проводится рандомизированное проспективное исследование сравнения предлагаемой методики и ЭВЛК. Исследование проводится на базе ГБУЗ РК «Евпаторийская ГБ», на базе ООО «Специализированная Клиника Инновационной Флебологии» г. Евпатория, совместно с кафедрой хирургии Института дополнительного профессионального образования ФГБОУ ВО ЮУГМУ Минздрава России. Научный руководитель д.м.н., профессор Фокин Алексей Анатольевич. На базе ГБУЗ РК «Евпаторийская ГБ» на койках сосудистого профиля с 30.10.2018 по 13.12.2018 с помощью предлагаемой методики оперировано 12 пациентов. 10 пациентов страдали варикозным расширением ЦПВ в бассейне БПВ, 2 пациента - в бассейне МПВ, что было подтверждено данными УЗДС вен. У 8 пациентов поражение правой нижней конечности, у 4 - левой. В раннем и/о периоде у 3 пациентов на 3-4 сутки после операции были выявлены признаки флебита обработанной вены. При этом у 2 из них это проявлялось болезненностью по ходу вены при надавливании. Еще у одного покраснением кожи по ходу обработанной вены. При этом, данные негативные эффекты не влияли на повседневный образ жизни. Все пациенты находятся на динамическом амбулаторном наблюдении. На 10 сутки ни у одного пациента не наблюдались признаки флебита. По данным контрольного УЗДС вен облитерация стволов ЦПВ достигнута в 100% случаев, рефлюкс зарегистрирован не был. 12.11.2018 выполнена операция - Вакуум ассистированная клеевая облитерация БПВ справа. В послеоперационном периоде осложнений не было. Во время операции уровень болевых ощущений соответствовал 2 баллам по ВАШ. Данные УЗДС вен были оценены на первые сутки и на 10 сутки после операции. Облитерация БПВ полная. Признаков тромбоза глубоких вен (ТГВ) не выявлено. У пациента на третьи сутки после операции был явления флебита обработанной вены в виде болезненности по ходу вены, покраснения кожи в нижней трети бедра в проекции вены. Было назначено местное лечение противовоспалительным гелем (Долобене-гель) и прием НПВС (Нимесил) в течении 5 дней. На фоне лечения явления воспаления купированы.
