Изобретение относится к клинической медицине, а именно к акушерству и гинекологии, как способ прогнозирования развития гестационного сахарного диабета у первородящих юного возраста.The invention relates to clinical medicine, namely to obstetrics and gynecology, as a method for predicting the development of gestational diabetes in primiparous young children.
Гестационный сахарный диабет (ГСД) является наиболее частым нарушением обмена веществ у беременных, с которым встречаются эндокринологи и акушеры-гинекологи и, следовательно, является важной междисциплинарной проблемой. Несмотря на достижения акушерской диабетологии, общая частота осложнений беременности и заболеваемость новорожденных при ГСД не снижается ниже 80%. Течение беременности при ГСД осложняется развитием гестоза в 25%-65% случаев /Ведмедь А.А. и др., 2009; Накоренок А.А. и др., 2010/, а тяжелые его формы отмечаются в 2,9-3,7% наблюдений /Ostlund I. и соавт., 2004; Boriboonhirunsarn и соавт., 2006/.Gestational diabetes mellitus (GDM) is the most common metabolic disorder in pregnant women with which endocrinologists and obstetrician-gynecologists meet and, therefore, is an important interdisciplinary problem. Despite the achievements of obstetric diabetology, the overall frequency of pregnancy complications and the incidence of newborns with GDM does not decrease below 80%. The course of pregnancy with GDM is complicated by the development of gestosis in 25% -65% of cases / Vedmed A.A. et al., 2009; Nakorenok A.A. et al., 2010 /, and its severe forms are noted in 2.9-3.7% of cases / Ostlund I. et al., 2004; Boriboonhirunsarn et al., 2006 /.

Сущность изобретения заключается в том, что определяются исходные (до 12 недели беременности) показатели роста и веса беременной, рассчитывается индекс массы тела (ИМТ) и, с помощью предложенной нами формулы, определяется вероятность развития ГСД.The essence of the invention lies in the fact that the initial (up to 12 weeks of pregnancy) indicators of growth and weight of the pregnant woman are determined, the body mass index (BMI) is calculated, and, using our formula, the probability of the development of GDM is determined.

Способ осуществляется следующим образом. С помощью общеизвестной методики рассчитывается индекс массы тела (ИМТ = вес (кг): (рост (м))2. Точка разбиения для прогноза ГСД равна 0,465. Значение для конкретной пациентки, полученное более 0,465, указывает на высокую вероятность развития СД во время беременности (позволяет отнести ее к группе с СД). При этом диагностическая точность (эффективность) полученной модели составляет 82,7%, чувствительность 74,0%, специфичность 87,6%.The break point for the forecast GDM is 0.465. Для доказательства объективности способа, после одобрения этического комитета ФГБОУ ВО "Амурская ГМА" от 21 сентября 2016 г., на базе Амурского областного перинатального центра было проведено ретроспективное исследование по изучению зависимости развития ГСД от исходного ИМТ.To prove the objectivity of the method, after the approval of the ethics committee of the FSBEI HE "Amur GMA" dated September 21, 2016, a retrospective study was conducted on the basis of the Amur Regional Perinatal Center to study the dependence of the development of HMD on the initial BMI.
Цель исследования: изучить возможность развития гестационного сахарного диабета у первородящих юного возраста.Objective: to study the possibility of developing gestational diabetes in primiparous young children.
Были отобраны 325 пациенток юного возраста родоразрешенных в Амурском областном перинатальном центре.325 patients of young age of delivery were selected in the Amur regional perinatal center.
Критериями включения пациенток в исследование являлись:The criteria for inclusion of patients in the study were:
1. Возраст от 13 до 17 лет включительно1. Age from 13 to 17 years inclusive
2. Первая беременность.2. The first pregnancy.
Критериями исключения были приняты:Exclusion criteria were adopted:
1. Тяжелая экстрагенитальная патология1. Severe extragenital pathology
2. Преэклампсия и эклампсия2. Preeclampsia and eclampsia
3. Многоплодная беременность3. Multiple pregnancy
4. Исходный сахарный диабет4. Initial diabetes

Исходные значения статистически значимо ниже у пациенток без СД. Из вышеуказанных результатов можно заключить, что риск развития СД связан с исходным уровнем ИМТ у юных беременных. Этот же факт отражает и график на фиг. 2.Baseline values are statistically significantly lower in patients without diabetes. Руководствуясь полученными результатами, мы предприняли попытку создания прогностической модели, включающей исходный ИМТ, которая позволила бы предсказать вероятность развития гестационного сахарного диабета у конкретной пациентки по ее исходным значениям ИМТ. Для проверки статистической значимости этой связи и разработки прогностической формулы был применен метод логистической регрессии, где откликом является наличие/отсутствие СД, а единственным предиктором - исходный уровень ИМТ юной пациентки. Использование заявляемого способа в Амурском областном перинатальном центре позволило достичь лечебные и диагностические результаты. Расчет вероятности развития гестационного сахарного диабета у первородящих юного возраста позволил обосновано с профилактической целью назначать диетотерапию и тем самым предупреждать развитие тяжелых форм гестационного сахарного диабета.Using the proposed method in the Amur regional perinatal center made it possible to achieve therapeutic and diagnostic results. The calculation of the likelihood of developing gestational diabetes mellitus in primiparous young children made it possible to prescribe diet therapy for the preventive purpose and thereby prevent the development of severe forms of gestational diabetes mellitus.
Таким образом, техническим результатом использования изобретения является исключение применения инвазивных методов при прогнозе гестационного сахарного диабета и возможность прогнозировать последний с высокой диагностической точностью (чувствительность 74,0%, специфичность 87,6%).Thus, the technical result of using the invention is the exclusion of invasive methods in the prognosis of gestational diabetes mellitus and the ability to predict the latter with high diagnostic accuracy (sensitivity 74.0%, specificity 87.6%).
