Благодаря деминерализирующим, ангиопротекторным, антибактериальным и фунгицидным свойствам препарата происходит остановка распространения остеонекротических повреждений и быстрое разграничение между жизнеспособными тканями и некротизированной костью (секвестральная полость).The method is implemented as follows. Пациент К., 38 лет, находился на стационарном лечении в клинике с диагнозом остеонекроз нижней челюсти справа травматического генеза. В анамнезе отмечал употребление синтетического метаамфетамина кустарного производства в течение трех лет. При поступлении основной жалобой являлось длительное гнойное выделение, свищевые ходы и обнажение костной ткани нижней челюсти справа (более 12 месяцев), болевой синдром в этой же области. На компьютерной томограмме в области угла справа определялся перелом без признаков консолидации, нижнечелюстная кость по обеим концам отломков имела выраженные признаки склероза по всей структуре правой ветви. После обследования пациенту проведено лечение по предложенному способу путем трехразового введения в свищевые ходы левомеколевой мази, содержащей Трилон Б в соотношении 1:1,5 по массе. На вторые сутки лечения отмечено исчезновение болевого синдрома и снижение интенсивности гнойного выделения. Пациент выписан в удовлетворительном состоянии на 5 сутки с рекомендацией проводить назначенное лечение (введение мазевой композиции в свищевые ходы в 3 раза в день в течение месяца). При повторной госпитализации через месяц на компьютерной томограмме в области ветви нижней челюсти справа отмечалась четко отграниченная секвестральная полость, ограниченная участками жизнеспособной кости с признаками снижения степени остеоклероза. Концы отломков без признаков консолидации. Произведена операция секвестрэктомии по стандартному протоколу. Послеоперационное течение гладкое. Пациент выписан на 7 сутки после операции и снятия швов в удовлетворительном состоянии. Наблюдение в течение 3-х месяцев без отрицательной динамики.Patient K., 38 years old, was hospitalized in a clinic with a diagnosis of osteonecrosis of the lower jaw on the right of a traumatic genesis. On the second day of treatment, the pain syndrome disappeared and the intensity of purulent discharge decreased. The patient was discharged in satisfactory condition on the 5th day with the recommendation to carry out the prescribed treatment (introduction of the ointment composition into the fistulous passages 3 times a day for a month). During a second hospitalization after a month, a clearly demarcated sequestral cavity was observed on a computer tomogram in the region of the mandible branch on the right, limited to areas of viable bone with signs of a decrease in the degree of osteoclerosis. The ends of fragments without signs of consolidation. A sequestrectomy was performed using a standard protocol. The postoperative course is smooth. The patient was discharged on day 7 after surgery and removal of sutures in a satisfactory condition. Observation for 3 months without negative dynamics.
