В медицинской технике известны конструкции тотальных эндопротезов тазобедренного сустава (ТБС), например, фирмы «MATHIS» (Австрия. Mathis Orthopadie GmbH, 2351 Wiener Neudorf. Info. at @ mathismedical.com), а также фирмы «Bicontact» (ООО «Б. Браун Медикал, Украина», www.b braun.ua), характеризующиеся наличием клиновидной ножки, сферической головки, полусферического вкладыша и полусферической вертлужной чашки. Установка вертлужной чашки тотального эндопротеза в вертлужной впадине таза требует, во-первых, удаления зачастую неповрежденного хрящевого слоя и придания впадине правильной сферической формы под сферическую наружную поверхность вертлужной чашки для обеспечения ее полного прилегания к костной ткани таза и, во-вторых, ее закрепления (фиксации), для чего используются костные шурупы или специальные конструктивные элементы в виде, например, резьбы с большим шагом на ее наружной сферической поверхности, усложняющие конструкцию и увеличивающие ее массу, что негативно отражается на биомеханике ТБС. В результате значительно увеличивается продолжительность операции, что не всегда допустимо по причине увеличения времени пребывания пациента под наркозом. Кроме того, необходимо иметь специальный медицинский инструментарий для подготовки вертлужной впадины, специальные костные шурупы, отвертки и воротки. Removal of the cartilaginous layer and part of the bone tissue, as well as its damage caused by mechanical elements fixing the cup, increase the patient's rehabilitation period.
Известна также конструкция однополюсного эндопротеза ТБС (Патент Украины, UA №93823 от 10.03.2011, Бюл. №5), включающего в себя пустотелую головку, состоящую из соединенных между собой с помощью самоуплотняющейся конической резьбы упругой сферической оболочки и шейки, снабженной центральным глухим коническим отверстием для сопряжения с конусом ножки ТБС. Сферическое подвижное соединение присущее ТБС образовано полой головкой и хрящевым слоем вертлужной впадины. Существенным конструктивным недостатком данного эндопротеза является имеющаяся возможность перерастания упругих деформаций тонкостенной сферической оболочки головки в пластические деформации при ударных или чрезмерных нагрузках, приводящих к необратимому изменению ее первоначальной правильной сферической формы или даже к разрыву, что объясняется отсутствием конструктивных элементов, являющихся ограничителями (упорами) ее прогибов в пределах упругих деформаций. The design of the endoprosthesis is characterized by a large mass, in particular the acetabular cup, which negatively affects the biomechanics (dynamics), which largely determines the degree of wear rate of the cartilaginous layer, and therefore the life of the TBS.
Недостатком прототипа также является плохая смазываемость открытого сферического подвижного соединения «головка - вкладыш», что приводит к ускоренному износу контактных поверхностей и образованию вследствие износа твердых фракций (титан, хирулен, сапфир, керамика и др.), попадающих в мышечные ткани человека и вызывающих вредные тканевые реакции (воспаления), а также негативно влияющих на работу как первичного, так и ревизионных эндопротезов. Задачи, которые решаются с помощью заявляемого изобретения, заключаются, во-первых, в создании закрытой зоны размещения и работы сферического соединения «головка - вкладыш», которая способна удерживать искусственную смазку и исключать попадание продуктов его износа в организм человека, во-вторых, в обеспечении условий адаптации сферической поверхности вертлужной чашки к поверхности хрящевого слоя вертлужной впадины, способных увеличить площадь их контакта с целью более равномерного распределения нагрузки, а в-третьих, в уменьшении массы эндопротеза для улучшения биомеханики движения.The tasks that are solved using the claimed invention are, firstly, to create a closed zone for the placement and operation of the spherical connection "head - liner", which is able to hold artificial lubricant and prevent the ingress of products of its wear into the human body, and secondly, providing conditions for adapting the spherical surface of the acetabular cup to the surface of the cartilaginous layer of the acetabulum, capable of increasing the area of their contact in order to more evenly distribute the load, and thirdly, to reduce endoprosthesis sy to improve the biomechanics of movement.
Для решения поставленных задач существующая конструкция биартикулярного эндопротеза ТБС, содержащая ножку клиновидной формы, сферическую головку, вертлужную чашку, вкладыш со сферической открытой наружу полостью под сферическую головку, снабжена трубчатой конической гофрированной упругой оболочкой, армированной по концам соответственно большим и малым кольцами, ножка имеет на проксимальном конце выступ с тремя участками в виде усеченных конусов: первого для фиксации сферической головки, второго переходного и третьего прилегающего к ножке, при этом верхнее меньшее основание третьего участка является большим основанием второго участка, а у последнего меньшее основание служит большим основанием первого участка. Вертлужная чашка выполнена в виде двух концентрично расположенных с зазором и соединенных между собой по краям полусфер, при этом зазор заполнен биоинертной жидкостью, наружная полусфера представляет собой упругую тонкостенную сферическую оболочку, внутренняя полусфера имеет на выпуклой стороне «вафельную» поверхность, а со стороны вогнутой поверхности имеет край с конической поверхностью, вкладыш закреплен неподвижно на внутренней полусфере и снабжен расположенным по краю кольцевым упорным буртиком, имеющим с выпуклой стороны вкладыша цилиндрическую поверхность, а с вогнутой - коническую поверхность с расширением наружу для упора во второй участок выступа с возможностью ограничения углового перемещения ножки. The acetabular cup is made in the form of two hemispheres concentrically arranged with a gap and interconnected at the edges, the gap being filled with bioinert liquid, the outer hemisphere is an elastic thin-walled spherical shell, the inner hemisphere has a “waffle” surface on the convex side, and a concave surface on the convex side has an edge with a conical surface, the liner is fixed motionless on the inner hemisphere and is equipped with an annular thrust shoulder located on the edge, having on the convex side breathing cylindrical surface, and a concave - conical surface extension outwardly to abut the second protrusion section to limit the angular displacement legs. The small ring of this shell has an inner conical surface for a tight self-locking connection with the third portion of the protrusion, and the large ring has an inner cylindrical surface for connection with the cylindrical surface of the specified collar of the liner and an outer conical surface for a tight self-braking connection with the conical surface of the inner hemisphere. The cavity formed by the specified tubular shell and the surfaces of the protrusion, liner and head communicating with it is filled with the indicated bioinert antifriction nanofluid.
Между совокупностью заявляемых признаков изобретения и техническими результатами, обеспечиваемыми предлагаемым изобретением, существует причинно-следственная связь, заключающаяся в том, что, во-первых, подвижное сферическое соединение «вкладыш - головка» размещено в герметичной полости, охватывающей его трубчатой конической гофрированной упругой оболочкой, способной удерживать искусственную смазку (антифрикционную жидкость), уменьшающую трение, а следовательно, и износ подвижного сферического соединения и исключить попадание продуктов износа в ткани человека, во-вторых, наличие в конструкции вертлужной чашки контактной поверхности в виде упругой полусферы позволяет ей за счет упругих деформаций адаптироваться к форме поверхности хрящевого слоя, что приводит к увеличению площади контакта, более равномерному распределению нагрузки и снижению интенсивности его износа, в-третьих, выполнение одной из поверхностей внутренней полусферы вертлужной чашки «вафельной» и замещение части ее материала с большим удельным весом жидкостью, обладающей меньшим удельным весом, что позволяет уменьшить ее массу без снижения прочностных характеристик и улучшить тем самым биомеханические характеристики эндопротеза.Between the totality of the claimed features of the invention and the technical results provided by the proposed invention, there is a causal relationship, namely, that, firstly, the movable spherical connection "liner - head" is placed in a sealed cavity, covering it with a tubular conical corrugated elastic shell, able to retain artificial lubricant (anti-friction fluid), which reduces friction and, consequently, the wear of a movable spherical joint and prevent product wear patterns in human tissue, and secondly, the presence in the design of the acetabular cup of the contact surface in the form of an elastic hemisphere allows it to adapt to the shape of the surface of the cartilaginous layer due to elastic deformations, which leads to an increase in the contact area, a more uniform distribution of the load and a decrease in the intensity of its wear thirdly, the implementation of one of the surfaces of the inner hemisphere of the acetabular cup "waffle" and replacing part of its material with a large specific gravity with a liquid having a lower specific gravity, which allows to reduce its mass without reducing the strength characteristics and thereby improve the biomechanical characteristics of the endoprosthesis.

В процессе ходьбы сферическая тонкостенная оболочка 12, деформируясь, постоянно адаптируется к анатомической форме поверхности хрящевого слоя, а «вафельная» поверхность внутренней полусферы 11 вертлужной чашки является ограничителем ее упругих прогибов. Наличие упругой полусферы позволяет демпфировать импульсные нагрузки на ножку эндопротеза, приводящие к ее расшатыванию в интрамедулярном канале тазобедренной кости и вероятному перелому. Изменением давления жидкости в щелевой полости (в зазоре) можно изменять жесткость упругой оболочки, адаптируя эндопротез к весу пациента, т.е. к действующей на ТБС нагрузке, с учетом характера его повседневной жизнедеятельности.The action of the force F on the thin-
