Известны различные конструкции эндопротезов тазобедренного сустава, в которых для уменьшения износа трущихся поверхностей головки и впадины используют смазочную жидкость. В эндопротезах тазобедренного сустава [1, 2] смазочная жидкость, депонированная в емкостях базального отдела шейки, посредством плунжера подается к суставной щели по каналам, выполненным в шейке и головке. Оба устройства под базальными отделами шейки имеют амортизаторы, демпфирующие осевые нагрузки на головку в опорную фазу шага, а в одном из этих устройств [2], кроме того, имеется амортизирующая прокладка, размещенная между двумя сферическими, концентрически расположенными чашами впадины.Various designs of hip arthroplasty are known in which a lubricating fluid is used to reduce wear on the rubbing surfaces of the head and cavity. В процессе нагрузки впадина несколько удлиняется и возвращается в исходное положение в фазу переноса конечности, т.е. жесткая впадина не работает в резонанс с костью вертлужной впадины таза. Поэтому в эндопротезах тазобедренного сустава стали использовать пластичные полиэтиленовые прокладки [3]. Однако по сравнению с другими материалами, используемыми для изготовления эндопротезов, полиэтилен имеет максимальный линейный износ [4]. Поэтому четко наметилась тенденция отказа от комбинированных пар трения жесткий/эластичный материал, в пользу пар трения металл/металл, что уменьшило количество наблюдавшихся случаев асептического расшатывания эндопротезов. От концентрического расположения чаш с амортизирующей прокладкой между ними, также нельзя ожидать дифференцированного распределения нагрузок на впадину в зависимости от положения шейки бедра, начиная от момента перехода к опорной фазе шага и дальнейшего ее углового перемещения до момента перехода к фазе переноса. In addition, the shock-absorbing pad is subject to hysteresis and is not durable enough.

Первый слой, закрепленный на дне впадины, изготовлен из упругоэластичного материала микропористой структуры, а второй слой, закрепленный на первом, выполнен в форме полигональных пластин из трибологически устойчивого материала, разделенных между собой капиллярными щелями. Поверхности пластин, прилегающие к первому слою прокладки, конгруэнтны его поверхности, а прилегающие к головке - конгруэнтны поверхности последней. Между внутренним краем торца впадины и буртом шейки размещен, закрепленный на них, упругоэластичный чехол, образующий герметичную полость, заполненную смазочной жидкостью и сообщающуюся через микропоры первого слоя прокладки с капиллярными щелями, разделяющими пластины. Эта сущность заключается в том, что эндопротез тазобедренного сустава содержит впадину, головку, шейку, ножку, прокладку, состоящую из подвижных элементов, отделенных друг от друга зазорами, размещенную между поверхностью головки и дном впадины, и гофрированный чехол из упругого материала, герметизирующий полость сустава, заполненную смазочной жидкостью. Элементы прокладки выполнены в виде колец, уложены друг на друга плоскими контактирующими между собой поверхностями, параллельными торцу впадины. Наружные поверхности колец конгруэнтны внутренней поверхности впадины, внутренние - поверхности головки, а элемент прокладки, расположенный между полюсами головки и впадины, выполнен в виде вогнуто-выпуклого мениска со сферическими, концентрически расположенными поверхностями. Верхняя, выпуклая поверхность этого элемента, конгруэнтна внутренней поверхности впадины, а вогнутая в средней части, нижняя поверхность - поверхности головки. Плоские нижние края мениска сопряжены с плоскими краями нижележащего кольцевидного элемента. Зазоры допускают возможность качательных перемещений элементов прокладки между поверхностями головки и впадины, между собой, а также вращательных движений вокруг собственной оси. При этом элементы прокладки и головка удерживаются во впадине фланцем с отбортовкой, закрепленным на торце впадины через герметизирующую прокладку. Гофрированный чехол закреплен по краю отбортовки и на бурте шейки. The gaps allow the possibility of rocking movements of the gasket elements between the surfaces of the head and cavity, between themselves, as well as rotational movements around its own axis. At the same time, the gasket elements and the head are held in the cavity by a flange with a flange mounted on the end of the cavity through a sealing gasket. The corrugated cover is fixed along the edge of the flanging and on the collar of the neck. The cavity of the cover, filled with lubricating fluid, is connected through the holes in the flange flange to the joint cavity formed by the gaps.

Головка 1 и элементы 3,4 прокладки удерживаются во впадине 2 через герметизирующую прокладку 5, фланцем 6 с отбортовкой 7 вокруг его центрального отверстия. По краю отбортовки 7 и бурта 8 шейки 9 закреплен гофрированный чехол 10 конической формы, изготовленный из упругого материала. В отбортовке 7 выполнены отверстия 11, соединяющие полость чехла 10 с полостью сустава, образованную зазорами, окружающими элементы прокладки. Along the edge of the flange 7 and the collar 8 of the neck 9, a corrugated cover 10 of a conical shape, made of elastic material, is fixed. In the flanging 7, holes 11 are made, connecting the cavity of the cover 10 with the joint cavity formed by the gaps surrounding the gasket elements. The cavities are filled with lubricating fluid.
