Abstract
Description
The invention relates to medical equipment and can be used in orthopedics and traumatology in the treatment of injuries, diseases and injuries of the hip joint.
Mathis Orthopadie GmbH, 2351 Wiener Neudorf. Info.at @ mathismedical.com), In medical technology, designs of total hip arthroplasty (TBS) are known, for example, MATHIS (Austria. Mathis Orthopadie GmbH, 2351 Wiener Neudorf. Info.at @ mathismedical.com), as well as Bicontact (B. Ltd.) Brown Medical, Ukraine ”, www.b braun.ua), characterized by the presence of a wedge-shaped leg, a spherical head, a hemispherical insert and a hemispherical acetabular cup. The leg is installed and fixed in the intramedullary canal of the hip bone, and the acetabular cup is installed without the possibility of movement in the acetabulum
Installing an acetabular cup of a total endoprosthesis in the acetabulum of the pelvis requires, firstly, removing the often undamaged cartilaginous layer and giving the cavity a regular spherical shape under the spherical outer surface of the acetabular cup to ensure its full fit to the bone tissue of the pelvis, and secondly, its fixing ( fixation), for which bone screws or special structural elements are used in the form, for example, of thread with a large pitch on its outer spherical surface, complicating the design and increasing its mass, which negatively affects the biomechanics of TBS. As a result, the duration of the operation significantly increases, which is not always permissible due to the increase in the patient's time spent under anesthesia. In addition, it is necessary to have special medical tools for the preparation of the acetabulum, special bone screws and knobs. Removal of the cartilaginous layer and part of the bone tissue, as well as its damage caused by mechanical elements fixing the cup, increase the patient's recovery time.
The construction of a single-pole TBS endoprosthesis is also known (Patent of Ukraine, UA No. 93823 dated 03/10/2011. Bull. No. 5), which includes a hollow head consisting of an elastic spherical shell and a neck connected to each other by a self-sealing conical thread, equipped with a central blind conical a hole for interfacing with the cone legs TBS. The head cavity is filled with bioinert liquid under overpressure.
The design of this endoprosthesis is designed to improve biomechanics due to weight reduction, as well as to increase its service life due to the damping by the elastic head of shock loads that occur when walking, which violate the stability of the leg due to its loosening in the intramedullary canal of the hip bone.
Essentially, this design is a hip head endoprosthesis, i.e. only part of the total TBS endoprosthesis, since there is no acetabular cup installed in the acetabulum. The spherical movable joint inherent in TBS is formed by a hollow head and a cartilaginous layer of the acetabulum. The insufficiency of the synovial fluid secreted by only one cartilaginous layer, which functions as a natural lubricant for human TBS, leads to more intensive wear (abrasion) than a spherical head with much greater wear resistance. Thus, being functionally suitable, the endoprosthesis due to the wear of the cartilage layer is characterized by a short life, which leads to the need for revision surgery.
A significant structural drawback of this endoprosthesis is the possibility of the elastic deformation of a thin-walled spherical shell of the head developing into plastic deformations under shock or excessive loads, leading to irreversible change in its initial regular spherical shape or even to rupture, which is explained by the absence of structural elements that limit it (stops) deflections within elastic deformations. In fact, such a limiter is the surface of the acetabulum, but as soon as a part of the head shell during rotation goes beyond its limits, in accordance with Pascal’s law, all the load acting on the head will be redistributed and cause increased deformation (buckling) of this particular part of the shell.
A design flaw should also include the technological complexity of manufacturing a thin-walled shell, which actually has the shape of a ball and different wall thicknesses. In addition, the conical self-sealing thread used as a connecting thread does not allow for a change in pressure in the head cavity by screwing it on the neck (or screwing), because the tightness of the conical threaded connection is achieved only with a certain relative arrangement with an interference fit of the threads.
BEZNOSKA Ltd (Delnicka 2727, 27201 Kladno, Czech Republic; www.beznoska.cz; mailbox @ beznoska.cz). As a prototype of the claimed invention, the biarticular endoprosthesis of TBS manufactured by Messrs is adopted by the design and principle of operation. BEZNOSKA Ltd (Delnicka 2727, 27201 Kladno, Czech Republic; www.beznoska.cz; mailbox @ beznoska.cz). The endoprosthesis includes a wedge-shaped leg with a cone for installing a spherical head, an insert with a locking ring, which is the limiter of the angle of rotation of the legs, and an acetabulum placed in the acetabulum with the possibility of angular movements.
The negative consequences of the use of this endoprosthesis are manifested in the form of intensive wear of the cartilaginous layers of the acetabulum in contact with the spherical surface of the acetabulum. This is due to the fact that, unlike the spherical surface of the acetabulum, which has the correct one, i.e. strictly spherical shape, the anatomical spherical surface of the cartilaginous layer of the acetabulum has an irregular spherical shape, and is characterized by the presence of sections of different curvature, recesses and protrusions. The design of the endoprosthesis is characterized by a large mass, in particular the acetabular cup, which negatively affects the biomechanics (dynamics), which largely determines the degree of wear rate of the cartilaginous layer, and therefore the life of the TBS.
The disadvantage of the prototype is also the poor lubricity of the open spherical movable connection “head-liner", which leads to accelerated wear of contact surfaces and the formation due to wear of solid fractions (titanium, chirulen, sapphire, ceramics, etc.) that enter human muscle tissue and cause harmful tissue reactions (inflammation), as well as negatively affecting the operation of both primary and revision endoprostheses. The ingress of wear products into the moving head-cartilage layer joint leads to more intense wear of the latter.
The tasks that are solved using the claimed invention are, firstly, to create a closed zone for the placement and operation of the spherical connection "head - liner", which is able to hold artificial lubricant and prevent the ingress of products of its wear into the human body, and secondly, providing conditions for adapting the spherical surface of the acetabular cup to the surface of the cartilaginous layer of the acetabulum, capable of increasing the area of their contact in order to more evenly distribute the load, and thirdly, to reduce endoprosthesis sy to improve the biomechanics of movement.
To solve the tasks, the existing design of the biarticular endoprosthesis of the TBS, containing a wedge-shaped leg, a spherical head, an acetabular cup, an insert with a movable spherical connection with the head, is equipped with a tubular conical corrugated elastic shell reinforced at the ends with smaller and larger rings respectively, the leg has a proximal the end is a protrusion with three sections in the form of truncated cones: the first for fixing the spherical head, the second transitional and the third threaded adjacent on to the leg, wherein the upper smaller base of the third portion is larger base of the second portion, and at first the smaller base is the large base of the third portion. The acetabular cup is made in the form of two hemispherical shells concentrically arranged with a gap and connected to each other along the edges, forming a slit-like cavity, and the outer hemispherical shell is made elastic, while the slot-like cavity is connected with the holes in the connection zone of the hemispherical shells with a conical tubular cavity corrugated elastic shell, the inner hemispherical shell has an edge with a conical threaded surface on the side of the concave surface, and on the convex with Oron - "wafer" surface, in recesses which radial channels are made. The insert is made with an outer “waffle” surface in the recesses of which radial channels are made and fixed with a mushroom-shaped elastic latch in the inner hemispherical shell of the acetabular cup, the lower part of the insert is made with an annular collar having a tapered end surface that extends outward to abut the second transition section of the protrusion legs with the possibility of limiting the angle of rotation of the protrusion with the leg, while the lower part of the liner on the convex side has grooves at the end with the transition to voro Cochin. Radial channels made in the recesses of the "waffle" surfaces of the liner and the inner hemispherical shell are made coaxial. The smaller ring of smaller diameter of said tubular conical corrugated elastic shell has an internal conical self-sealing thread for tight connection with the third portion of the protrusion, and the larger ring of larger diameter has an external conical self-sealing thread for tight connection with the conical thread surface of the edge of the inner hemispherical shell of the acetabular cup. The cavity formed by the tubular conical corrugated elastic shell is filled with bioinert antifriction nanofluid with the possibility of circulation and communicates with the surfaces of the protrusion, liner, and through said openings and grooves with a slit-like cavity of the acetabulum, and then through the said channels with a movable spherical connection of the head and liner.
Between the totality of the claimed features of the invention and the technical results provided by the invention, there is a causal relationship, namely, that, firstly, the movable spherical connection "liner-head" is placed in a sealed cavity formed by an acetabular cup, a tubular conical corrugated elastic the shell and the protrusion of the legs, capable of holding artificial lubricant (antifriction fluid) circulating through the channels (holes), which reduces friction, and therefore wear of the movable spherical joint, and to prevent the ingress of wear products into human tissue, and secondly, the presence of an external elastic hemispherical shell in the acetabular cup design allows it to adapt due to elastic deformations to the surface shape of the cartilaginous layer of the acetabulum, which leads to an increase in the contact area, more uniform distribution of the load and a decrease in the intensity of its wear, and thirdly, the implementation of the convex surface of the inner hemispherical shell of the acetabular cup and the outer the surface of the liner "waffle" and replacing part of their material with a large specific gravity with a liquid having a lower specific gravity, allows to reduce their mass without reducing the strength characteristics and thereby improve the biomechanical characteristics of the endoprosthesis.

The composition of the structural elements of the claimed endoprosthesis (Fig. 1) includes a wedge-shaped leg 1, which has a protrusion in the form of a truncated cone with three sections at the proximal end: the first threaded 2 adjacent to the leg, forming, with an internal conical self-sealing thread, a smaller ring of
The insert, the convex spherical surface of which is made “waffle” (Fig. 2), is fixed using a mushroom-shaped
The inner hemispherical shell has an edge on the concave surface with a conical threaded surface forming a tight threaded connection with the outer conical self-sealing thread of the
The slit-like cavity of the acetabular cup communicates with the
In addition, these two cavities are interconnected by means of radial holes (channels) 20 coaxially made in the recesses (pockets) 18 and 19 and a guaranteed radial clearance of the movable spherical connection.
-230 с.); Lashneva V.V., Dubok V.A., Tkachenko Ju.G. Physico-chemical properties of fullerene С60 based coatings // Book of abstract, VIII International Conference - ICHMS'2003, Sudak, Crimea, Ukrain, 14-20 Sept., 2003. p. 718-721).All cavities of the endoprosthesis are filled with bioinert antifriction fluid containing unmodified carbon fullerene C 60 nanoparticles, which contributes to a significant (8 ... 10 times) decrease in the friction coefficient in the movable spherical head-insert connection (Churilov GN Fullerenes: Synthesis and formation theory / G.N. Churilov, N.V. Bulina, A.S. Fedorov; Russian Academy of Sciences, Siberian Branch, Novosibirsk: Publishing House SOR AN, 2007.230 s.); Lashneva VV, Dubok VA, Tkachenko Ju.G. Physico-chemical properties of fullerene C 60 based coatings // Book of abstract, VIII International Conference - ICHMS'2003, Sudak, Crimea, Ukrain, 14-20 Sept., 2003. p. 718-721).
When walking, the spherical head 6 makes angular displacements α x and α y relative to the axis of the acetabular cup with insert 7 (Fig. 1), and the acetabulum relative to the axis of the acetabulum performs additional angular displacements α x , α y and angular displacement α z . Thus, the design of the endoprosthesis provides three degrees of mobility inherent in the human anatomical TBS.
The presence in the design of the acetabular cup of the external elastic
When using a biarticular endoprosthesis taken as a prototype with a rigid acetabular cup, the contact of its external regular spherical surface with a radius of R1 (Fig. 3) and the cartilaginous layer of thickness δ of radius R2, due to its anatomically irregular spherical shape, occurs in small areas with a diameter of D1 limited contact angle ϕ 1 , which also perceives the force F created by the person’s static weight and dynamic (shock) loads arising from his movement. As a result, these sections of the cartilage layer are subject to more intense wear.
The action of the force F on the elastic
During walking, the elastic
The recesses (pockets) 19 of the “waffle” surface of the liner act as accumulators that provide the necessary recharge of the movable spherical joint with a lubricating antifriction fluid, which, through the guaranteed gap, removes solid wear products from it, thereby reducing the wear rate of its elements. An increase in the life of the endoprosthesis is also due to the fact that non-modified carbon (fullerene) nanoparticles with high hardness and wear resistance, located in a liquid lubricant, penetrate into the material, for example, of a chirulen liner, i.e. by sharpening it, they increase the hardness and wear resistance of the contact surface of the liner.
The presence of an elastic hemispherical shell allows damping impulse loads on the leg of the endoprosthesis, leading to its loosening in the intramedullary canal of the hip bone and damage. By changing the fluid pressure in the slit cavity, the stiffness of the elastic membrane can be changed, adapting the endoprosthesis to the patient’s weight, i.e. to the load acting on TBS, taking into account the nature of its daily life.
Traditional materials are used for the manufacture of endoprosthesis elements: leg, acetabular cup, tubular conical corrugated elastic shell with reinforcing rings - titanium alloy, including elastic, or bioinert steel; head - ceramics, artificial sapphire; insert - ultra-high molecular weight Chirulen polyethylene, artificial sapphire.
The inability to get the wear products of the spherical mobile joint into the muscle tissues of the human body avoids the occurrence of inflammatory processes and additional time spent on their removal from the tissues when replacing the endoprosthesis.
