Abstract
Description
The present invention relates to medicine, in particular to combustiology, and can be used to prepare donor skin sites before grafts.
С.178-180).There is a method of preparing donor skin sites, which consists in the subcutaneous injection of oxygen to infiltrate donor sites during excision of skin flaps (Fistal S. Ya, Lavrukhin Yu.N., 1970. “Burn disease.” Kiev, due to “Health”, 1970. S. 178-180).
However, the known method requires special equipment (oxygen system, reduction gear). In addition, with the introduction of oxygen, it can enter the bloodstream, which leads to the risk of embolic complications.
Б.С. М. С.126-127).As a prototype, the method of preparing donor sites for performing skin plastic surgery was chosen, which consists in introducing a 0.9% NaCl solution into the subcutaneous fat (see "Burns", manual for doctors, edited by B. S. Vikhriev, V. M. Burmistrova. L.: Medicine, 1986. S. 126-127).
The disadvantage of this method is that infiltration with an isotonic solution and its leakage onto the surface of the donor wound creates a risk of infection and the formation of inflammatory infiltrates, since it does not have bactericidal properties. Along with this, when the solution enters under the glue dermat, it does not stick to the skin. A 0.9% NaCl solution introduced into the subcutaneous fatty tissue does not affect the microcirculation in the donor area and tissue oxygenation, which reduces the endurance of the grafts taken, often contributes to their lysis, and also extends the healing time of donor wounds.
The task of the invention is to improve microcirculation in the area of the donor site, ensure bactericidal activity, increase oxygenation of tissues and cut grafts.
2.The method is as follows: after treatment of the donor skin with antiseptics (1% iodopiron solution, alcohol solution of chlorhexidine, etc.) 10-15 minutes before cutting the grafts using a regular 10-20 gram syringe, they are injected into the subcutaneous fat tissue of the donor site 150 -200 ml of ozonized physiological solution with an ozone content of 160-200 mcg. Ozone is produced using the medical ozonizer Medozons-BM. The injected solution easily spreads under the skin, which allows for 5 minutes to achieve the formation of an elastic smooth surface and to prepare a donor site for excision of transplants on an area of 600-800 cm 2 .
Clinical example
б. Patient N., 3 years old (IB No. 269704), was admitted to the burn center of Nizhny Novgorod with burns with a flame of ShAB-IV degree of the trunk and limbs on an area of 50% of the body surface. Thermo-inhalation injury. Combustion poisoning.
After preparing burn wounds, the patient underwent 4 stages of skin plasty. When performing surgical interventions, the preparation of donor sites (400-500 cm 2 ) consisted of introducing into the subcutaneous fat tissue the proposed site for cutting transplants of the ozonated solution at a concentration of 160-200 μg. Despite the child’s severe tissue hypoxia due to extensive burns, thermo-inhalation injury, poisoning by combustion products, the transplanted grafts healed completely, and donor wounds healed without suppuration within 8-10 days, which, with a significant shortage of donor sites, allowed transplantation of the transplants in place of the healed donor wounds. The skin of the child has been completely restored and after 42 days he was discharged for outpatient treatment at the place of residence with good cosmetic and functional results.
The introduction of an ozonized solution with a high bactericidal effect helps to increase oxygenation of the tissues of the donor site and, accordingly, cut grafts due to improved microcirculation in this area, as well as the multicomponent effect of ozone on tissue metabolism.
The method allows to reduce the period of epithelization of donor wounds formed on the site of the taken grafts, which healed on average after 8-10 days without signs of suppuration, by 3-4 days, and also to ensure complete engraftment of the transplanted grafts on the wound bed. Reducing the healing time of donor wounds by 3-4 days allows the use of donor skin for re-cutting transplants, which is especially important in case of deficiency of skin resources in seriously burned and weakened patients.
