Патология пародонта занимает одно из ведущих мест среди стоматологических заболеваний. Согласно современным представлениям наряду с метаболическими нарушениями и иммунологическими сосудистый фактор признан одним из ключевых в патогенезе заболеваний пародонта. Болезни пародонта являются тяжелым недугом и по распространенности занимают четвертое место среди неинфекционных заболеваний, уступая лишь патологии сердечно-сосудистой системы, онкологическим заболеваниям и сахарному диабету. Указанный результат достигается тем, что в известном способе хирургического лечения воспалительных заболеваний пародонта, включающем отслаивание слизисто-надкостничного лоскута, его одномоментную деэпителизацию, удаление грануляций и поддесневых отложений ультразвуком, а также обработку поверхностей корней зубов и внутрикостных карманов, обработку поверхностей корней зубов и внутрикостных карманов производят ультразвуком в течение 10-18 минут с использованием полирующей суспензии, после чего производят заполнение костных карманов остеопластическим материалом - аллоплантом. Экспериментально установлено, что при обработке ультразвуком корней зубов и внутрикостных карманов менее 10 минут их очищение не происходит, а обработка ультразвуком более 18 минут не усиливает эффект очистки, и поэтому производить обработку ультразвуком более указанного времени нецелесообразно. На ОПТГ- глубина костных карманов более 4 мм. Поставлен диагноз: хронический генерализованный пародонтит, тяжелой степени. Назначено комплексное лечение, включающее в себя профессиональную гигиену с одновременной полировкой и шлифовкой шеек зубов, депульпирование зубов, избирательное пришлифовывание по методике Дженкельсона и шинирование подвижных зубов flex-дугой. После этого проводилось хирургическое лечение-остеогингивопластика в области 1.3-2.3 зубов. Под инфильтрационной и резцовой анестезией сформированы и отслоены слизисто-надкостничные лоскуты с одномоментной деэпителизацией, кюретаж, обработка костных карманов и поверхностей корней ультразвуком в течение 10 мин, частотой 25 кГц и суспензией гидроксилапатита - размер гранул 10 мкм, деэпителизация лоскутов, заполнение костных карманов порошком аллоплант, погруженным в кровяной сгусток.The patient was examined clinically and radiologically. В послеоперационном периоде заживление осуществлялось первичным натяжением. Через 3 месяца уменьшилась подвижность, зубы стали устойчивы. Пациентка Н., 38 лет, обратилась в клинику с жалобами на кровоточивость десен, неприятный запах изо рта, подвижность зубов. При обследовании у больной выявлено: ПК 4-5 мм, выделения слизисто-гнойного характера, подвижность зубов I-II степени, по данным ОПТГ - глубина костных карманов более 4 мм, ИГ - 1,8. Комплексное лечение было начато с проведения профессиональной гигиены полости рта, обучения больного индивидуальной гигиене, антибактериальной и противовоспалительной терапии, временного шинирования подвижных зубов. После предоперационной подготовки проведена операция: остеогингивопластика в области 4.4-3.3 зубов, под проводниковой анестезией сформированы и отслоены слизисто-надкостничные лоскуты, кюретаж, деэпителизация лоскутов, обработка 18% раствором лимонной кислоты, обработка поверхностей корней и костных карманов ультразвуком в течение 18 мин, частотой 25 кГц в сочетании с порошком гидроксилапатита, имеющим размер частиц около 10 мкм, в качестве водной среды была использована стерильная дистиллированная вода. Далее проводилось заполнение костных карманов порошком аллоплант, который помещали в кровяной сгусток. Next, bone pockets were filled with alloplant powder, which was placed in a blood clot. Wound healing was carried out by primary intention.
Через 3 месяца уменьшилась подвижность зубов, ПК до 3 мм, отделяемого нет, слизистая десны плотная, бледно-розовая. В послеоперационный период больным назначают противовоспалительную, десенсибилизирующую терапию, орошение полости рта антисептическими растворами.
