Description
Den foreliggende oppfinnelse vedrører midler som inneholder, i forbindelse, minst ett immunotoksin og klorokin.
I de tidligere franske patentsøknader nr. 7 827 838,
7 924 655, 8 107 596 og 8 121 836 har man beskrevet fremstil-ling av anticancerøse produkter, dvs. konjugater fått ved kob-ling, ved kovalent binding, av kjede A i ricin med antistoffer eller fragmenter av antistoffer rettet mot et antigen båret av en destruksjonscelle. Produktene av denne type er i den foreliggende søknad presentert under det generiske navn immunotoksiner.
I den franske søknad 8 121 836 har man dessuten beskrevet egenskapen ved ammoniumioner (i form av et hvilket som helst av deres salter og spesielt klorid) til på eri effektiv måte å potentialisere den cytotoksiske virkningen til disse immunotoksiner.
I en annen tidligere søknad i Frankrike, nr. 8 202 091, har søkeren beskrevet egenskapen til stoffer som tilhører klassen av karboksylionoforer, til å potentialisere aktiviteten til immunotoksinene og akselerere deres aksjonskinetikk i henhold til retningslinjer lik dem som allerede er beskrevet for ammoniumioner.
Alle disse potentialiserings- og akselereringsstoffer kan anvendes for å forbedre virkningen av immunotoksinene såvel in vitro som in vivo hos mennesker eller dyr i et terapeutisk øye-med.
Den foreliggende oppfinnelse har til hensikt å fremstille sterke cytotoksiske midler idet den anvender potentialisering av den selektive cytotoksiske virkning av immunotoksinene beskrevet i patentsøknadene nevnt ovenfor.
Etter at tallrike andre stoffer har vært studert uten hell er det blitt funnet at klorokin anvendt i form av et hvilket som helst av dets farmasøytisk akseptable salter, var et spesielt in-teressant stoff for å potentialisere og akselerere den selektive cytotoksiske virkning av immunotoksinene. Klorokin har da også, og særlig for terapeutisk anvendelse in vivo, den fordel å ha vært anvendt som medikament for menneskeri adskillige år. Dets retningslinjer for terapeutisk anvendelse er altså meget godt kjent. Det er et middel som tolereres godt opptil relativt høye doser (omkring 2 g i et eneste inntak hos voksne) og som repre-senterer særlig interessante farmakokinetiske karakteristika som skyldes en langsom urinutskillelse, for 70% av dosen i uendret
form, og en meget lang serum-halveringstid (40 til 70 timer).
I den foreliggende oppfinnelse har det vært funnet at klorokin, på en overraskende måte, anvendt i doser hvor det selv ikke viser noen særlig cytotoksisitet for de studerte linjer, potentialiserer på en meget viktig måte (faktor på 2 500) den spesifikke cytotoksisiteten til immunotoksinene.
Det følgende ikke-begrensende eksempel tillater å forstå rekkevidden av oppfinnelsen bedre.
EKSEMPEL
i
Dette eksempel viser pbtentialiseringen av den selektive cytotoksisitet av immunotoksin anti-T65 (slik som beskrevet i søkerens søknad nr. 81 21 836) overfor T-lymfoblastoide humane celler av linje CEM som bærer antigen T65.
I disse forsøk er cytotoksisiteten funnet ved å måle inkor-14 o poreringen av C-leucin i celler etter 18 t inkubering ved 37 c i nærvær av kjente mengder av det studerte immunotoksin eller av cytotoksiske referansestoffer i fravær av eller i nærvær av klorokin. 1 - Potentialisering av den cytotoksiske virkning av klorokin Resultatene av disse forsøk er presentert i form av kurver dose/virkning som i ordinaten viser den cytotoksiske virkning funnet som angitt ovenfor ved inkorporering av markør regnet i % av verdien oppnådd på kontrollceller uten cytotoksiske stoff, og i abscissen de molare konsentrasjoner i toksisk underenhet av de studerte cytotoksiske stoffer. Klorokin ble testet i en konsentrasjon på 60 mikromol. Det er tidligere blitt bekreftet at klorokin ikke er spontant cytotoksisk for de anvendte celler i den angitte konsentrasjon.
Fig. 1 viser virkningen av klorokin på cytotoksisiteten av ricin og på dens isolerte A-kjede, tatt som referansestoffer. Verdiene av de molare konsentrasjoner (CI50)som svarer til 50% inhibering av inkorporering av markør, er angitt i tabell I.
På denne figur 1 har man overført henholdsvis resultatene fått for ricin (R), og blandingen ricin og klorokin (R-C), kjede A av ricin (A) og blandingen av kjede A og ricin med klorokin
(A-C).
Disse resultater viser at det praktisk talt ikke er noen potentialiseringseffekt av klorokin på cytotoksisiteten av ricin (potentialiseringsfaktor på 1,6) og av kjede A (potentialiseringsfaktor på 1,4).
Fig. 2 viser den sammenlignede potentialiseringseffekt av NH*-ioner (10 mM) og av klorokin (60 mM) på cytotoksisiteten av immunotoksin anti-T65 overfor celler av linje CEM. Verdiene av de molare konsentrasjoner (CI50) som svarer til 50% inhibering av inkorporering av markør, er sammenfattet i tabell II. I fig. 2 er innført henholdsvis resultatene for konjugat anti-T65 (AT6 5), kjede A av ricin (A) og blandingene av konjugat anti-T65 med et fireverdig ammoniumsalt (AT65 N4) resp. klorokin (AT65 C).
Disse resultater viser at potentialiseringseffekten av klorokin på aktiviteten av immunotoksinet overfor dens spesifikke mål er nær den av ammonium og omkring 2 500. Dette betyr at i nærvær av klorokin er immunotoksin anti-T65 overfor sitt spesifikke mål et cytotoksisk stoff som er sterkere enn ricin selv.
Dessuten viser klorokin den bemerkelsesverdige egenskap ikke bare å potentialisere aktiviteten, men også å forsterke selektiviteten til immunotoksinet. Hvis man nemlig tar som kri-terium for selektivitet av virkning av immunotoksinet utbyttet av CI50 av fri kjede A og av immunotoksin, er dette utbytte på
10 i fravær av klorokin og nær 44 000 i nærvær av klorokin.
2 - Akselerering av cytotoksisitetskinétikken med klorokin Virkningen av klorokin begrenser seg ikke til å forsterke den cytotoksiske aktivitet og selektiviteten av immunotoksinene på en betraktelig måte. Dette stoff tillater også på en meget viktig måte å akselerere cytotoksisitetskinétikken og selektiviteten av immunotoksinene, som følgende eksempel viser: I dette forsøk har man som tidligere målt inkorporeringen av radioaktiv markør i cellene, men denne gang som funksjon av inkuberingstiden av cellene med immunotoksin i fravær og i nærvær av 6 0 yM klorokin som potentialiseringsstoff. Dette forsøk er blitt utført på den cellulære modell som består av den humane lymfoblastoide linje CEM med immunotoksin anti-T65 i konsentrasjon på 50 nM. Resultatene er vist på fig. 3.
På denne figur er de oppnådde resultater med konjugat anti-T65 (AT6 5) og med blandingen av konjugat anti-T65 med klorokin (AT6 5 C) vist som funksjon av tiden (timer).
For denne linje viser det seg at i fravær av potentialisering er cytotoksisitetskinétikken meget langsom som vist i kurve a. Andre forsøk under samme betingelser har vist at den nødven-dige tid for å oppnå 50% reduksjon av inkorporering av radioaktiv markør var omkring 20 t. Derimot, i nærvær aviklorokin ma-nifesterer en forbløffende akselerering av kinetikken seg (kurve b), da den nødvendige tid for å oppnå 50% reduksjon av inhibering av inkorporering bare er omkring 1,5 time.
En slik akselereringseffekt er av den største viktighet for all anvendelse av immunotoksinene og særlig for terapeutisk anvendelse in vivo, for virkningshastigheten av medikamentet er alltid en meget gunstig faktor i effektiviteten av behandlingen .
Man kan altså i egenskap av middel i human-terapi anvende forbindelsen som består av immunotoksin og klorokin (i form av
basen eller et hvilket som helst av dens farmasøytisk akseptable salter). Det kan altså brukes til behandling av lidelser, can-cerøse eller ikke, som vil være følsomme mot antistoffet anvendt til fremstillingen av immunotoksinet.
Med det for øye å eliminere cancercellene i sin helhet vil behandlingen måtte utføres med en tilstrekkelig dose immunotoksin bundet til en mengde klorokin som vil kunne variere fra 10 mg til 2 g (uttrykt i base) for hver administrering av immunotoksin. Varigheten av behandlingen vil måtte bli bestemt i hvert tilfelle avhengig av pasienten og naturen av lidelsen som skal behandles.
De nye midler i henhold til oppfinnelsen er fabrikert for
å kunne anvendes under betingelser som er tilpasset deres bruk. Immunotoksinet vil bli administrert via injiserbar vei og fortrinnsvis via intravenøs vei. Klorokin vil fortrinnsvis bli administrert via injiserbar vei unntatt hvis dens anvendelse via oral vei viser terapeutiske fordeler.
