Description
Denne oppfinnelsen omfatter optimalt substituerte N<6->oksa, tia, tioksa og azacykloalkylsubstituerte adenosinderivater som er selektive adenosintype 1 reseptoragonister og på denne måten er potentielt anvendelige midler for fremstilling av et preparat mot kardiovaskulære sykdommer og sentralnervesystemforstyrrelser.
Det er to undergrupper av adenosinreseptorer i hjertet: Ai og A2. Hver undergruppe påvirker forskjellig fysilogiske funksjoner. Stimulering av Ai-adenosinreseptoren induserer to bestemte fysiologiske responser. Den første er inhiberingen av de stimulerende effektene av katekolamin. Denne effekten blir formidlet via inhiberingen av cyklisk AMP-syntese. Den andre effekten formidlet ved Ai-reseptorer er nedsettelse av hjerterytmen og impulspropagering gjennom AV-noden. Effekten er uavhengig av cAMP-metabolismen og er tilknyttet Ai-adenosinreseptoreraktivering av den innvendig rektifiserende K+-kanalen. Denne effekten er unik for Ai-reseptoren; A2-reseptoren har ingen rolle i modulering av funksjonen av denne kanalen. Stimulering av adenosin A\-reseptoren vil følgelig forkorte varigheten og nedsette amplituden av aksjonspotenialet av AV-modale celler og deretter forlenge refratærperioden av cellene. Konsekvensen av disse effektene er å begrense antallet impulser som utføres fra atria til ventriklene. Dette danner basis for den kliniske utnyttelsen av Ai-reseptorantagonister ved behandlingen av supraventrikulær tachycardiaen, inkluderende atrieflimmer, atrieflutter og AV-nodal reentrant tachycardia.
Den kliniske nytten av Ai-agonister vil derfor være ved behandling av akutte og kroniske sykdommer vedrørende hjerterytmen, særlig de sykdommene som kjennetegnes ved rask hjerterytme hvor hastigheten drives av abnormaliteter i arteria. Sykdommene inkluderer, men er ikke begrenset til, atrieflimmer, supraventrikulær tachycardia og atrieflutter. Utsettelse for Ai-agonister gir en reduksjon i hjertehastigheten og en regularisering av den abnormale rytmen og dermed gir tilbake forbedret hemodynamisk blodflyt.
Ai-agonister vil ha fordelaktige effekter ved hjertefeil hvor øket sympatetisk tone som forårsaker økt cAMP har tilknytning til øket sannsynlighet for ventrikulære arrhytmis og brå død, gjennom deres evne til å inhibere den katakolamininduserte økningen i cAMP.
Et formål med denne oppfinnelsen er nye heterocyklisk substituerte adenosinderivater.
Et annet formål med denne oppfinnelsen er nye heterocykliske substituerte adenosinderivater som er anvendelige som A)-reseptoragonister.
Et ytterligere formål ved denne oppfinnelsen er nye heterocyklisk substituerte adenosinderivater som er anvendelige ved behandling av suptraventrikulær tachycardias, inkluderende atrieflimmer, atrieflutter og AV-nodal reentrant tachycardia.
I en utførelsesform angår oppfinnelsen en forbindelse som har formelen:
hvori Ri er kaprolaktam.
I en annen utførelsesform angår oppfinnelsen anvendelse av en terapeutisk effektiv mengde av en forbindelse ifølge oppfinnelsen for fremstilling av en farmasøytisk sammensetning for stimulering av koronar aktivitet hos et pattedyr som opplever en koronar elektrisk forstyrrelse som kan behandles ved å stimulere en Ai hjerteadenosinreseptor.
I en ytterligere utforming er denne oppfinnelsen en farmasøytisk sammensetning av materiale innbefattende sammensetningen ifølge oppfinnelsen og en eller flere farmasøytiske eksipienter. Fig. 1 er et plot av effekten av konsentrasjonen av forbindelse II i eksempel 2 på atrial AV-nodal konduktans for Ai-adenosinreseptoren (-•-) og for A2-adenosinreseptoren (-0-). Fig. 1 er et plot av effekten av konsentrasjonen av forbindelse I i eksempel 2 på atrial AV-nodal konduktans og spesifikt på responsen til Ai-adenosinreseptoren (-•-) og på responsen til A2-adenosinreseptoren (-0-).
Sammensetningen ifølge oppfinnelsen anvendelig som Ai-reseptoragonister for behandlingen av kardiak elektriske forstyrrelser slik som supraventrikulær tachycardias, inkluderende atrieflimmer, atrieflutter og AV-nodal reentrant tachycardia. Sammensetningene kan administreres oralt, intravenøst, gjennom epidermisen eller ved en hvilken som helst annen måte kjent i litteraturen for administrering av terapeutiske midler.
Metoden for behandling innbefatter administrering av en effektiv mengde av den valgte forbindelsen, foretrukket dispergert i en farmasøytisk bærer. Doseringsenheter for den aktive ingrediensen blir generelt utvalgt fra området på 0,01 til 100 mg/kg, men vil enkelt bli bestemt av fagfolk avhengig av administrasjonsmåte, alder og tilstanden til pasienten. Disse doseringsenhetene kan administreres en eller ti ganger daglig for akutte og kroniske sykdommer. Ingen uakseptable toksikologiske effekter forventes når forbindelser ifølge oppfinnelsen blir administrert i overensstemmelse med den foreliggende oppfinnelsen.
Hvis den endelige forbindelsen i denne oppfinnelsen inneholder en basisk gruppe, kan et syreaddisjonssalt bli fremstilt. Syreaddisjonssalt av forbindelsene blir fremstilt på vanlig måte i et egnet løsemiddel fra morforbindelsen og et overskudd av syre, slik som saltsyre, hydrobromsyre, svovelsyre, fosforsyre, eddiksyre, maleinsyre, ravsyre eller metansulfonsyre. Hydrokloridsaltformen er særlig anvendelig. Hvis den endelige forbindelsen inneholder en sur gruppe, kan kationiske salter fremstilles. Vanligvis blir morforbindelsen behandlet med et overskudd av en alkalisk reagens, slik som hydroksid, karbonat eller alkoksid, inneholdende det hensiktsmessige kationet. Kationer slike som Na<+>, K<+>, Ca<+2> og NH4<4>" er eksempler på kationer tilstedeværende i farmasøytisk akseptable salter. Noen av forbindelsene danner indre salter eller Zwitter-ioner som også er akseptable.
Farmasøytiske sammensetninger inkluderende forbindelser ifølge oppfinnelsen, og/eller derivater derav, kan formuleres som løsninger eller lyofiliserte pulvere for parenteral administrasjon. Pulvere kan bli rekonstituert ved tilsetning av et egnet fortynningsmiddel eller andre farmasøytisk akseptable bærere før anvendelse. Hvis det anvendes i flytende form, er sammensetningene ifølge oppfinnelsen foretrukket tatt opp i en bufret, isotonisk vandig løsning. Eksempler på egnede fortynningsmidler er normal isotonisk saltløsning, standard 5% dekstrose i vann og bufret natrium eller ammoniumacetatløsning. Slike væskeformuleringer er egnede for parenteral administrasjon, men kan også bli anvendt for oral administrasjon. Det er ønskelig å tilsette eksipienter slike som polyvinylpyrrolidon, gelatin, hydroksycellulose, acacia, polyetylenglykol, mannitol, natriumklorid, natriumcitrat og en hvilken som helst annen eksipient kjente for fagfolk til farmasøytiske sammensetninger inkluderende forbindelser ifølge oppfinnelsen. Alternativt kan farmasøytiske forbindelser bli innkapslet, tablettert eller fremstilt i form av en emulsjon eller sirup for oral administrasjon. Farmasøytisk akseptable faste eller flytende bærere kan tilsettes for å øke eller stabilisere sammensetningen, eller for å lette fremstillingen av sammensetningen. Flytende bærere inkluderer sirup, peanøttolje, olivenolje, glycerin, saltvann, alkoholer og vann. Faste bærere inkluderer stivelse, laktose, kalsiumsulfat, dihydrat, teffa alba, magnesiumstearat eller stearinsyre, talkum, pektin, acacia, agar eller gelatin. Bæreren kan også inkludere et vedvarende avgivelsesmateriale slik som glycerolmonostearat eller glyceroldistearat, alene eller med en voks. Mengden av fast bærer varierer, men vil være foretrukket mellom ca. 20 mg til ca. 1 g pr. doseringsenhet. De farmasøytiske doseringene blir laget ved anvendelse av vanlige teknikker slik som maling, blanding, granulering og sammenpressing, når nødvendig, for tablettformer; eller maling, blanding og fylling for harde gelatinkapselformer. Når en flytende bærer blir anvendt, vil fremstillingen være i form av en sirup, eleksir, emulsjon eller en vandig eller ikke-vandig suspensjon. En slik flytende formulering kan administreres direkte eller fylles i myke gelatinkapsler.
Eksemplene som følger tjener til å illustrere oppfinnelsen. Eksemplene er ikke ment på noen måte å begrense omfanget av oppfinnelsen, men er gitt for å vise hvordan man lager eller anvender forbindelser ifølge oppfinnelsen. I eksemplene er alle temperaturer i grader Celcius.
EKSEMPEL 1
Forbindelsene ifølge oppfinnelsen kan fremstilles ved vanlige fremgangsmåter i organisk kjemi. Reaksjonssekvensen gitt nedenfor, er generell fremgangsmåte anvendt ved fremstilling av forbindelser ifølge oppfinnelsen.
I henhold til denne fremgangsmåten blir oksacykloalkylkarboksylsyre varmet opp i en blanding av dioksan, difenylfosforyazid og trietylamin i 1 time. Til denne blanding blir tilsatt benzylalkohol og reaksjonen blir varmet ytterligere over natten for å gi intermediatforbindelsen 1. Forbindelse 1 blir oppløst i metanol. Deretter blir konsentrert Hel, Pd/C tilsatt og blandingen anbrakt under hydrogen ved 1 atm. Blandingen blir rørt over natten ved romtemperatur og filtrert. Residuet blir rekrystallisert som gir intermediatforbindelse 2. 6-klorpurinribosid blir tilsatt en blanding av forbindelse 2 oppløst i metanol og deretter med trietylamin. Reaksjonen blir varmet til 80°C i 30 timer. Isolering og rensing gir forbindelse 3.
EKSEMPEL 2
Forbindelser ifølge oppfinnelsen fremstilt i henhold til fremgangsmåten i eksempel 1 ble testet i to funksjonsmodeller spesifikke for adenosin Ai-reseptoragonistfunksjonen. Den første var Ai-reseptorformidlet hemmingen av isoproterenolstimulert cAMP-akkumulering i DDT-celler. EC50 av hvert derivat er vist i tabell 1. Også vist i tabell 1 er evnen til hvert derivat til å stimulere cAMP-produksjon i PC 12 celler, en funksjon av agoniststimulering av adenosin A2-reseptorer. Andelen av det relative potentialet til hver forbindelse til å stimulere enten en Ai-reseptor eller en A2-reseptoreffekt er angitt som selektiviteten til hver forbindelse for A)-reseptoren. Som det fremgår av tabell 1, er hvert derivat relativt selektivt som en Ai-reseptoragonist. Anvendelsen av å måle cAMP-metabolisme som en bestemmelse av adenosin Ai-reseptorfunksjon har tidligere blitt beskrevet (Scammells, P., Baker, S., Belardinelli, L., og Olsson, R., 1994, Substituted 1,3-dipropylxanthines som irreversible antagonists of Ai adenosine receptors, J. Med. Chem 37:2794-2712, 1994).
Forbindelser ble også testet i en helorganmodell med Ai-reseptoraktivering med hensyn til atrial og AV-modalfunksjon. I denne modellen ble guineagrisehj erter isolert og gjennomstrømmet med saltvann inneholdende forbindelse mens atrialhastighet og AV-nodalkonduksjonstid ble bestemt ved elektrografisk målin av atrialcykluslengde og AV-intervaller, som beskrevet i Belardinelli, L, Lu, J. Dennis, D. Martens, J. og Shryock J.
(1994); kardiaeffektene av en ny Ai-adenosinreseptoragonist hos guineagris isolert hjerte. J. Pharm. Exp. Therap. 171:1371-1382 (1994). Som vist i fig. 1, er hvert derivat effektivt i å nedsette artrialhastigheten og forlenge AV-nodalkonduktivitetstiden av spontant slående hjerter på en konsentrasjonsavhengig måte. EKSEMPEL 3
Fremstilling av N-benzyIoksykarbonyl-4-aminopyran
En blanding av 4-pyranylkarboksylsyre (2,28 g, 20 mmol), difenylfosforylazid (4,31 ml, 20 mmol), trietylamin (2,78 ml, 20 mml) i dioksan (40 ml) ble varmet i 100°C oljebad under tørr nitorgen i 1 time. Benzylalkohol (2,7 ml, 26 mmol) ble tilsatt og oppvarmingen ble fortsatt ved 100°C i 22 timer. Blandingen ble avkjølt, filtrert fra et hvitt precipitat og konsentrert. Residuet ble oppløst i 2N HC1 og ekstrahert to ganger med EtOAc. Ekstraktene ble vasket med vann, natriumbikarbonat, saltvann og deretter tørket over MgS04 og konsentrert til en olje som ble fast ved henstand. Oljen ble kromatografert (30% til 60% EtOAc/Heks) som ga 1,85 g av et hvitt fast stoff (40%).
Fremstilling av 4-aminopyran
N-benzyloksykarbonyl-4-aminopyran (1,85 g, 7,87 mmol) ble oppløst i MeOH (50 ml) sammen med konsentrert Hel og Pd-C (10%, 300 mg). Kolben ble tilført hydrogen ved 1 atm. og blandingen ble rørt i 18 timer ved romtemperatur. Blandingen ble filtrert gjennom celitt og konsentrert. Residuet ble gjenfordampet to ganger med MeOH/EtO/Ac og rekrystallisert fra MeOH/EtOAc som ga 980 mg (91%) av hvite nåler (smeltepunkt 228-230°C).
Fremstilling av 6-(4-aminopyran)-purinribosid
En blanding av 6-klorpurinribosid (0,318 g, 1,1 mmol), 4-aminopyran-HCl (0,220 mg, 1,6 mmol) og trietylamin (0,385 ml, 2,5 mmol) i metanol (10 ml) ble varmet til 80°C i 30 timer. Blandingen ble avkjølt, konsentrert og residuet kromatografert (90:10:1, CH2Cl2/MeOH/PrNH2).
De ønskede fraksjoner ble samlet og rekromatografert ved anvendelse av et kromatron (2 mm plate, 90:10:1, CH2Cl2/MeOH/PrNH2) som ga et off-white skum (0,37 g, 95%).
EKSEMPEL 4
Fremstilling av N-benzyloksykarbonyl-3-aminotetrahydrofuran
En blanding av 3-tetrahydropyroslimsyre (3,5 g, 30 mmol), difenylfosforylazid (6,82 ml, 32 mmol), trietylamin (5 ml, 36 mml) i dioksan (35 ml) ble rørt ved romtemperatur i 20 minutter og deretter varmet til 100°C på etoljebad under tørr nitorgen i 2 timer. Benzylalkohol (4,7 ml, 45 mmol) ble tilsatt og oppvarmingen fortsatte ved 100°C i 22 timer. Blandingen ble avkjølt, filtrert fra et hvitt precipitat og konsentrert. Residuet ble oppløst i 2N HC1 og ekstrahert to ganger med EtOAc. Ekstraktene ble vasket med vann, natriumbikarbonat, saltvann og deretter tørket over MgS04 og konsentrert til en olje som ble fast ved henstand. Oljen ble kromatografert (30% til 60% EtOAc/Heks) som ga 1,85 g av et hvitt fast stoff (40%).

Oljen ble kromatografert (30% 60% EtOAc/heks) som ga 3,4 g av en olje (51%).
Fremstilling av 3-aminotetrahydrofuran
N-benzyloksykarbonyl-3-aminotetrahydrofuran (3,4 g, 15 mmol) ble oppløst i MeOH (50 ml) sammen med konsentrert HC1 og Pd-C (10%, 300 mg). Kolben ble tilsatt hydrogen ved 1 atm. og blandingen ble rørt i 18 timer i romtemperatur. Blandingen ble filtrert gjennom celitt og konsentrert. Residuet ble gjenfordampet to ganger med MeOH/EtOAc og rekrystallisert fra MeOH/EtOAc som ga 1,9 g av et gult fast stoff.
Fremstilling av 6-(3-aminotetrahydrofuranyl)purinribosid
En blanding av 6-klorpurinribosid (0,5 g, 1,74 mmol), 3-aminotetrahydrofuran
(0,325 mg, 2,6 mmol) og trietylamin (0,73 ml, 5,22 mmol) i metanol (10 ml) ble varmet til 80°C i 40 timer. Blandingen ble avkjølt og konsentrert. Residuet ble filtrert gjennom en kort kolonne av silikagel eluert med 90/10/1 (C^CVMeOH/PrNHi) og fraksjonene
som inneholdt produktet ble kombinert og konsentrert. Residuet ble kromatografert på en kromatron (2 mm plate, 92,5/7,5/1, CH2Cl2/MeOH/PrNH2). Det resulterende hvite faste stoffet ble rekrystallisert fra MeoH/EtOAc som ga 0,27 g av hvite krystaller (smeltepunkt 128-130°C).
EKSEMPEL 5
Gjenvinning av 3-aminotetrahydrofuranhydroklorid
En blanding av 3-aminotetrahydrofuranhydroklorid (0,5 g, 4 mmol) og (S)-(+)-10 kamforsulfonylklorid (1,1 g, 4,4 mmol) i pyridin(10 ml) ble rørt i 4 timer ved romtemperatur og deretter konsentrert. Residuet ble oppløst i EtOAc og vasket med 0,5N HC1 natriumbikarbonat og saltvann. Det organiske sjikte ble tørket over MgS04, filtrert og konsentrert som ga 1,17 g av en brun olje (97%) som ble kromatografert på silikagel (25% til 70% EtOAc/heks). Det hvite faste stoffet som ble oppnådd ble rekrystallisert en gang til fra aceton og krystallene og supernatanten samlet opp til det oppnåes en økning større enn 90% med 1H NMR.
Fremstilling av 3-(S)-aminotetrahydrofuranhydroklorid
Sulfonamid (170 g, 0,56 mmol) ble oppløst i konsentrert HCl/AcOH (2 ml hver), rørt i 20 timer ved romtemperatur, vasket tre ganger med CH2C12 (10 ml) og konsentrert til tørrhet som ga 75 mg (kvantitativt) av et hvitt fast stoff.
Fremstilling av 6-(3-(S)-aminotetrahydrofuranyl)purinribosid
En blanding av 6-klorpurinribosid (30 mg, 0,10 mmol), 3-(S)-aminotetrahydrofuranhydroklorid (19 mg, 0,15 mmol) og trietylamin (45 ml, 0,32 mmol) i metanol (0,5 ml) ble varmet til 80°C i 18 timer. Blandingen ble avkjølt, konsentrert og kromatografert med 95/5 (CH2Cl2/MeOH) som ga 8 mg (24%») av et hvitt fast stoff.

Anvendelse av en terapeutisk effektiv mengde av en forbindelse ifølge krav 1 for fremstilling av en farmasøytisk sammensetning for stimulering av koronar aktivitet hos et pattedyr som opplever en koronar elektrisk forstyrrelse som kan behandles ved å stimulere en Aj hjerteadenosinreseptor.

Anvendelse ifølge krav 3, hvori den terapeutisk effektive mengden varierer fra 0,01 til 100 mg/kg vekt av pattedyret.

Anvendelse ifølge krav 3, hvori forbindelsen blir administrert til et pattedyr som opplever en koronar elektrisk forstyrrelse utvalgt fra gruppen som består av supraventrikulær tachycardias, atrie-flimmer, atrieflutter og AV nodal re-entrant tachycardia.
