Description
Den foreliggende oppfinnelse angår orale preparater som virker mot plaque, i form av et munnskyllevann med en lav sur pH av 3-5, omfattende farnesol kombinert med benzosyre,
et konserveringsmiddel og polycarboxylater for å hemme dannelsen av tannplaque, enkeltvis eller i kombinasjon med hverandre .
Oppfinnelsens bakgrunn
Tannplaque er en bakteriefilm som henger fast på tennene. Det er blitt fastslått at det er en forbindelse mellom tannplaque og gumbetennelse (gingivitt). På grunn av at plaque fjernes utilstrekkelig ved børsting av tennene er det stor interesse for kjemisk hemming av plaquedannelse.
I teknikkens stand er anvendelsen av kationiske antimikrobielle midler beskrevet, som klorhexidin, cetylpyridiniumklorid, sanguinarinklorid og lignende, i orale preparater for å redusere tannplaque og gingivitt. Virkningen av de kationiske forbindelser antas å skyldes at de henger fast på orale overflater og at de blir langsomt frigjort i løpet av en lang tidsperiode. Et iboende problem ved innarbeidelse av kationiske forbindelser i orale preparater er at de førstnevnte er uforenlige med anioniske bestanddeler, som f.eks. overflateaktive midler, fluorid og søt-ningsstoffer. Andre problemer som er forbundet med de kationiske forbindelser, innbefatter tilflekking av orale overflater og bitter eftersmak.
Det er i teknikkens stand også blitt beskrevet anvendelse av etheriske oljer, som thymol, methylsalicylat og eucalyptol,ienvandig-alkoholisk munnskyllevannbærer for å redusere plaque og gingivitt, i Journal of Clinical Periodontology 1987; 14:285:288; Clinical Preventive Dentistry, Vol. 5, nr. 6, november-desember 1983; Journal of Clinical Periodontology 1985:12:697-704; og Journal of
Dent. Res. 65:274 (Abstact 941), 1986. Virkningen av disse forbindelser antas å stå i forbindelse med deres antiseptiske egenskaper. Imidlertid er munnskyllevann fremstilt med disse forbindelser karakterisert ved en sterk, ubehagelig
smak.
I teknikkens stand er også den bakteriostatiske aktivitet til enkelte australske etheriske oljer, som sandSltreolje som inneholder farnesol og santalol, beskrevet i en artikkel av Beylier i Perfumer and Flavorist, 4 (april-mai) , 1979, s. 23-25. Da den bakteriostatiske aktivitet til australsk sandeltreolje oppviser hemming av vekst av prøveorganismen Staphylococcus aureus, ble det foreslått at sandeltreolje skulle kunne innarbeides i kosmetika, som kremer, lotions, deodoranter, sjampoer og badeoljer, på
grunn av dens antiseptiske aktivitet.
I tysk patent nr. 3315058 (13. juni 1985) til Brunke et
al beskrives syntetisk farnesol, syntetiske farnesolblandinger og isomerer av farnesol som kan anvendes som bakteriostater i kosmetiske produkter for beskyttelse og pleie av den menneskelige hud, spesielt i en deodorantpumpedusj eller deodorantstift. Merkbar antibakteriell virkning mot de undersøkte bakteriearter Staphylococcus epidermis, Staphylococcus aureus og Cornebacterium ga seg tilkjenne ved at kroppslukt ble unngått.
I en artikkel av Brunke, E.J., Jellinek, J.S. og Koester, F.W., i Pollena - TSPK, 5-8 / 8 6XXX, s .151-155 , beskrives at fire stereoisomerer av farnesol har bakteriostatisk aktivitet overfor forskjellige bakteriekulturer in vitro og at stereoisomerer av bisabolol oppviser antiflammatorisk aktivitet i kaniner.
I US patent nr. 4214909 beskrives seskviterpen-alkoholer, som farnesol, nerolidol og organiske estere av disse, som grohemmende midler for å kontrollere vanlige organismer, som alger, andeskjell, sjøplanter, slim etc.
I US patent nr. 4775534 beskrives et middmiddel mot edderkoppmidd og som omfatter farnesol og/eller nerolidol impregnert inn i et substrat i fast eller flytende form som gir regulert frigjøring. De faste substrater er porøse partikkelformige materialer, som silika, talkum, leire, gelatin og geler, polymerer, nylon, cellulose, finmalte maiskolber og lignende. Flytende former av substrat som gir regulert frigjøring, innbefatter vegetabilske og/eller vegetabilske oljer, fortrinnsvis inneholdende overflateaktive midler, fuktemidler, emulgerende midler, disperger-ende midler og lignende. Farnesol og/eller nerolidolen funksjonerer som tiltrekkende eller modifiserende kjemi-kalier for å tiltrekke edderkoppmidd, hvorved spredningen av disse reduseres.
I ingen av publikasjonene i henhold til teknikkens stand som det er vist til ovenfor beskrives et oralt antiplaqueprodukt som inneholder lave konsentrasjoner av en seskviterpenalkoholsmaksforbindelse som antiplaquemiddel i nærvær av minst én av benzosyre, konserveringsmiddel eller et polymert polycarboxylat i en oral bærer med en lav pH
av 3-5.
Oppsummering av oppfinnelsen
Det har nå vist seg at orale antiplaquepreparater som inneholder meget lave konsentrasjoner av farnesol i nærvær av benzosyre, konserveringsmiddel, et anionisk lineært polymert carboxylat, eller et anionisk tverrbundet polycarboxylat, i en oral bærer med en lav pH kan gi betydelig antimikrobiell aktivitet overfor orale dentalplaqueproduserende bakterier hos mennesker.
Det er derfor et hovedmål ved oppfinnelsen å tilveiebringe et oralt antiplaaueorodukt som inneholder farnesol som virker mot plaque og har
antimikrobiell aktivitet ved lave konsentrasjoner.
Det er et annet mål ved oppfinnelsen å tilveiebringe
et oralt produkt som inneholder farnesol for å hemme dannelsen av tannplaque i nærvær av benzosyre og en lav pH.
Det tas ved oppfinnelsen også sikte på å tilveiebringe
et oralt ikke-flekkdannende produkt som virker mot plaque og inneholder et antiplaquemiddel av farnesol med behagelig smak og forenlig med anioniske komponenter i det orale preparat.
Ytterligere mål, fordeler og nye særtrekk ved oppfinnelsen vil bli angitt delvis i den nedenstående beskrivelse og vil delvis fremstå for fagfolk innen dette tek-niske -område ved et studium av det følgende, eller de kan læres ved praktisering av oppfinnelsen. Formålene og for-delene ved oppfinnelsen kan virkeliggjøres og oppnås ved hjelp av de midler og kombinasjoner som er spesielt påpekt i de vedføyede patentkrav.
For å oppnå de ovenstående og andre formål og i overensstemmelse med den foreliggende oppfinnelse slik denne er beskrevet ved utførelsesformer og generelt i den foreliggende beskrivelse, oppviser det nye orale antiplaqueprodukt ifølge oppfinnelsen forsterkede antimikrobielle egenskaper og inneholder som det vesentlige antiplaquemiddel en lav konsentrasjon av seskviterpenalkoholsmaksforbindelsen farnesol i nærvær av benzosyre, et konserveringsmiddel, et polymert lineært eller tverrbundet carboxylat, eller en kombinasjon derav, i en oral bærer med en lav pH av 3-5, fortrinnsvis 3-4.
Det ikke-flekkdannende, smakstiltalende, orale antiplaquepreparat ifølge oppfinnelsen i form av et munnskyllevann er kjennetegnet ved at det omfatter farnesol i en konsentrasjon av 0,05-1,0 vekt%, og 0,1-2,0 vekt% av minst ett hjelpestoff valgt fra gruppen bestående av benzosyre, organisk konserveringsmiddel, et anionisk lineært polymert carboxylat og et anionisk tverrbundet polycarboxylat, idet munnskyllevannet har en pH på 3-5.
Detaljert beskrivelse av oppfinnelsen
Det har overraskende vist seg at smaksforbindelsen farnesol innarbeidet i munnskyllevann gir antimikrobiell aktivitet ved meget lave konsentrasjoner. Denne smaksfor-bindelse er forskjellig fra vanlig anvendte fenoliske forbindelser, som thymol, ved at den oppviser antimikrobiell aktivitet ved lavere konsentrasjoner, har lavere giftighet og har mindre utiltalende smak. Denne seskviterpenalkohol byr på de fordeler at den er forenlig med anioniske komponenter i blandingen og har bedre smak og lavere giftighet.
Det har også vist seg at seskviterpenalkoholens antibakterielle aktivitet økes i nærvær av benzosyre i et oralt preparat med en lav pH. Tilsetningen av komponenter som
øker pH, som pyrofosfat og natriumbenzoat, fører til ned-
satt antibakteriell aktivitet.
Det har i tillegg vist seg at tilsetningen av et organisk konserveringsmiddel eller et polymert carboxylat til et farnesolholdig oralt preparat med en lav
pH av 3-5 oppviser øket mikrobiell aktivitet og overlegen antiplaqueaktivitet. Egnede konserveringsmidler innbefatter propyl-parabeh (propyl-parahydroxy-benzoat), methylparaben, dehydroeddiksyre, sorbinsyre og lignende konserveringsmidler i mengder av 0,1-2,0 vekt%.
De polymere carboxylater anvendt i henhold til den foreliggende oppfinnelse i mengder av 0,1-2,0 vekt%, er anioniske lineære eller tverrbundne polymere polycarboxylater med en molekylvekt av 1000-1000000, fortrinnsvis i form av vannopp-løselige alkalimetall (kalium og natrium)- eller ammonium-salter. De lineære polycarboxylater er fortrinnsvis 1:4-
4:1 kopolymerer av maleinsyreanhydrid eller maleinsyre med en annen polymerisérbar ethylenisk umettet monomer, fortrinnsvis methylvinylether (methoxyethylen) med en molekylvekt (m.v.) av 30000-1000000. Disse kopolymerer er tilgjengelige for eksempel som 'Gantrez AN 139" (m.v. 500000), "A.N. 119" (m.v. 250000) og fortrinnsvis '5-97' farmasøytisk kvalitet (m.v. 70000) fra GAF Corporation. Andre lineære polymere polycarboxylater innbefatter 1:1 kopolymerer av maleinsyreanhydrid med ethylacrylat, hydroxyethylmethacrylat eller methylvinylether, 1:1 kopolymerer av acrylsyre med methyl- eller hydroxyethylmethacrylat, methyl- eller ethylacrylat, isobutylvinylether eller N-vinyl-2-pyrolidon, kopolymerer av maleinsyreanhydrid med styren, isobutylen eller ethylvinylether, polyacryl-, polyitacon- og poly-maleinsyrer, og sulfoacryloligomerer. Kopolymerene inneholder tilstrekkelig med carboxyliske saltgrupper til at
de er vannoppløselige.
De tverrbunine carboxylater er fortrinnsvis carboxy-vinylpolymerer fremstilt av B.F. Goodrich og gitt vare-merkene Carbopol<®> 934, Carbopol<®> 940 og Carbopol<®> 941.
Hvert av disse produkter består i det vesentlige av en kolloidalt vannoppløselig polymer av acrylsyre tverrbundet med 0,7 5-2,0 0% av et tverrbindingsmiddel bestående av poly-allylsakkarose eller polyalkylpentaerythritol.
Kombinasjoner av konserveringsmidler og"Gantrez"kan også anvendes som hjelpestoffer for farnesol-smaksforbindelsen. Likeledes kan benzosyre kombineres med konserveringsmidler, som f.eks. parabener eller dehydroeddiksyre.
Ved én undersøkelse under anvendelse av SIKT-prøvningen ble farnesolskyllevann uten benzosyre fremstilt og regulert til pH 3, 4, 5 og 6 med fortynnet HC1. Resultatene i Tabell I viser at det er ingen antibakteriell aktivitet ved disse pH-verdier.
SIKT-prøvningen er en grunnleggende mikrobiologisk prøvning som innbefatter en fast kontakttid. 1 ml tann-rensemiddel eller skylleoppløsning blandes med et på forhånd bestemt inokulat av Actinomyces viscosus T14V (mellom 1 x 10g og 1 x 10 7 cfu/ml) i en kontakttid av 60 sekunder. Systemet ble derefter nøytralisert for å hemme ytterligere antimikrobiell aktivitet. De overlevende bakterier telles under anvendelse av platetellingsmetodikk. Reduksjonen i tellinger sammenlignet med en vannkontroll er grunnlaget for å ut-trykke aktivitet. En tre log -reduksjon blir vanligvis be-traktet som signifikant.
Farnesol-skyllevann uten benzosyre ved forskjellige pH-verdier viste mangel på antibakteriell aktivitet med farnesol alene selv ved lav pH. Ytterligere analyse av disse prøver avslørte imidlertid at de ikke var tilstrekkelig pufret til å holde pH stabil efter fortynning med fosfatpuffer. Det har vist seg at innarbeidelse av syrer, som sitronsyre, ascorbinsyre eller fosforsyre, gir tilstrekkelig pufferkapasitet til å holde pH mellom 3 og 4 efter fortynning.
Tilstrekkelig pufrede.farnesolskyllevann til å holde lav pH stabil efter fortynning med en fosfatpuffer oppviser derfor antimikrobiell aktivitet in vitro, som vist i Tabell
II.
I henhold til en annen undersøkelse ble farnesol-skyllevannene inneholdende benzosyre fremstilt ved tre forskjellige pH-verdier av henholdsvis 3, 5 og 6. Resultatene i Tabell III bekrefter den antibakterielle aktivitet til farnesol i nærvær av benzosyre og ved en lav pH
(prøve 3).
Disse resultater antyder et synergistisk forhold mellom farnesol og benzosyre ved en sur pH av 3,15 (Prøve 3).
Farnesolen utgjør 0,05-1,0 vekt%
av det orale preparat. Benzosyrebestanddelen utgjør 0,1-2,0 vekt% av det orale preparat. Den synergistiske blanding av farnesol og benzosyre i et surt medium med en pH av 3-5
i et oralt preparat oppviser overlegen antiplaqueaktivitet in vivo som vist i Tabell IV. En metode for å måle mengden av plaquevekst på mennesketenner efter 24 timer ble anvendt som en kvalifiseringsprøving for å undersøke antiplaquemunnskyllevann. Et personpanel renser sine tenner ved om-hyggelig børsting av tennene med et inert tannpussemiddel. Den gjenværende plaque avsløres bg fotograferes for poeng-notering (under oppnåelse av et 0 timepoeng). Prøveskylle-vannet anvendes i 30 s, hvorefter ingen oral hygiene til-lates i 24 timer. Efter 24 timer avsløres plaqueveksten, fotograferes og poengsettes (oppnåelse av poeng efter 24 h). Når poengtallet ved 0 h trekkes fra poengtallet efter 24 h, fås plaquevekstpoengtallet efter 24 h. Denne metode gjen-tas i fire dager i rekkefølge (mandag til fredag) for å
få fire plaquevekstpoengtall efter 24 timer pr. paneldel-tager for hvert undersøkt produkt. En tre dagers utvaskings-
periode over weekenden hindrer overføringseffekter til det neste prøveprodukt. Resultatene av denne sorterings-prøving har vist seg å overensstemme med kliniske plaque-undersøkelser over lengre tid.
Farnesolmunnskyllevannet ble prøvet i forhold til et placeboskyllevann og kommersielt tilgjengelige produkter som inneholdt 0,26% etheriske oljer, som thymol, mentol, methylsalicylat, eucalyptol, 0,03% sanguinariaekstrakt og 0,05% cetylpyridiniumklorid.
Resultatene viser at munnskyllevannet med 0,08% farnesol (Eksempel 1) har utmerket ahtiplaqueaktivitet sammenlignet med produkter som er tilgjengelige i handelen, dvs. sanguinariaekstraktmunnskyllevann og munnskyllevannet med etheriske oljer, og betydelig høyere antiplaqueaktivitet enn placebomunnskyllevannet og cetylpyridiniumkloridmunn-skyllevannet. Det fremgår også at mengden av 0,26% etheriske oljer i det nevnte munnskyllevann er over tre ganger større enn innholdet, 0,08%, av farnesol i farnesolmunnskyllevannet.
Den synergistiske blanding av farnesol og konserveringsmidler eller'XSantrez" i et surt medium med en lav pH av 3-5 oppviser også sterkere antimikrobiell aktivitet in vitro enn farnesol ved en lav pH alene, som vist i Tabell V og
Figur 1.
Den synergistiske blanding av farnesol og Carbopol<®> i et surt medium oppviser også større antimikrobiell aktivitet in vitro enn farnesol alene, ved en lav pH, som vist i Tabell VI.
In vitro prøving av farnesol påviser dens antimikrobielle aktivitet overfor en dominerende stamme av bakterier, Actinomyces viscosus, som produserer tannplaque hos mennesker, i Tabell VII, under anvendelse av SIKT-prøvingen som er blitt tidligere beskrevet i Tabell I.
I Tabell VIII sammenlignes in vitro-aktiviteten for farnesol i forhold til thymol ved forskjellige konsentrasjoner. Resultatene viser at farnesol drepte plaquebak-terier i betydelig omfang ved 1/10 av den effektive konsentrasjon av thymol.
Anvendelsen av farnesol i munnskyllevann istedenfor kationiske antimikrobielle midler i henhold til teknikkens stand byr på flere fordeler, Farnesol er forenlig med anioniske overflateaktive midler, fluorid og søtningsstoffer. Det forekommer ingen bitter eftersmak eller tanntilflekking som er forbundet med kationiske antimikrobielle midler.
I tillegg byr farnesol på fordeler fremfor andre antimikrobielle etheriske oljer, som f.eks. thymol og eucalyptol, ved at den har en mer tiltalende smak og en lavere giftighet som vist i Tabell IX. Giftighetsverdiene er rapportert i litteraturen. LD50 er den mengde av midlet som er nødvendig for å drepe 50% av forsøksdyrene.
Bæreren i munnskyllevannet som ofte betegnes som en dental bærer, inneholder væsker som i alminnelighet omfatter vann og/eller et fuktighetsbevarende middel, som f.eks. glycerol, sorbitol, propylenglycol eller polyethylenglycol innbefattende egnede blandinger derav. Det er som regel fordelaktig å anvende en blanding av både vann og ett eller to av de fuktighetsbevarende midler. De foretrukne fuktighetsbevarende midler er glycerol, sorbitol og polyethylenglycol. Munnskyllevannet ifølge den foreliggende oppfinnelse kan også inneholde midler som befordrer antiplaqueaktivitet, i minimale mengder av opptil 5 vekt%, som f.eks. poly-vinylfosfonsyre som avsettes på tannoverflater og hemmer ved-hengning av plaque til overfalten. Farnesol virker ved en annen mekanisme, dvs. at som et antimikrobielt middel hemmer det bakteriereproduksjon og dermed vekst av plaque. De to forskjellige mekanismer for å bekjempe plaque vil derfor kunne være komplementære og vise en additiv eller endog synergistisk virkning.
Munnskyllevannet ifølge den foreliggende oppfinnelse
kan også inneholde vanlige bestanddeler, som f.eks. farge-midler, smaks- og/eller søtningsmaterialer, fluorider som natriumfluorid, toverdig tinnfluorid og natriummonofluor-fosfat. Disse ytterligere bestanddeler kan hver tilsettes i minimale mengder av opp til 5 vekt%, fortrinnsvis opp til 1 vekt%, forutsatt at de ikke innvirker uheldig på det ferdige produkts antiplaqueegenskaper.
Munnskyllevann inneholder typisk 20-99 vekt% av en vandig lavere alifatisk alkohol, fortrinnsvis med 10-30 vekt% alkohol, som ethanol, n-propyl- eller isopropylalkohol, vann og, ofte, 5-35% fuktighetsbevarende middel.
Slike preparater blir typisk anvendt ved å skylle munnhulen i 30-90 s eller ved anvendelse av et munndusjmiddel ved dusjing i kontakt med orale overflater minst én gang daglig. Typiske orale preparater ifølge oppfinnelsen som kan anvendes på denne måte, er gjengitt nedenfor.

Munnskylle vannblandinger >
Munnskyllevannet fremstilles ved at to adskilte opp-løsninger dannes hvorav den ene er en ethanoloppløsning som inneholder farnesol, benzosyre og smaksmiddel, og den annen er en vandig oppløsning som inneholder natriumlauryl-sulfat, glycerol, natriumsaccharin og fargemiddel, og de to oppløsninger kombineres med hverandre under blanding for å ferdigfremstille munnskylle:vannet.
Nærmere bestemt blir benzosyren, farnesolen og smaks-midlet oppløst i ethanol og blandet i 5 minutter. I en egen beholder blir natriumlaurylsulfatet og vannet blandet langsomt i 5 minutter. Glycerol, saccharin og fargemiddel tilsettes til vannoppløsningen og blandes i ca. 10 minutter. Mens den vandige oppløsning blandes, blir alkoholoppløs-ningen langsomt tilsatt til denne, og det hele blir blandet i 10 minutter.
Det erholdte munnskyllevann er et klart,
flytende produkt.
Munnskylle:vannet som inneholder 0,08% farnesol i nærvær av 0,1% benzosyre og ved en sur pH av 3,2, oppviser utmerket antiplaquevirkning sammenlignet med tidligere kjente anti-plaquemunnskyllemidler, som vist i den ovenstående Tabell
IV.
Eksempel 3
Dette munnskyllevann fremstilles i overensstemmelse med metoden ifølge Eksempel 1, bortsett fra at polyvinyl-fosfonsyren og sorbitolen oppløses i den vandige oppløsning. Det anvendte Pluronic<®> oppløses i ethanoloppløsningen.
Dette antiplaquemunnskylleyann oppviser også overlegen antiplaqueaktivitet.

Claims (9)
1. Munnskyllevann,
karakterisert ved at det omfatter farnesol i en konsentrasjon av 0,05-1,0 vekt%, og 0,1-2 vekt% av minst ett hjelpestoff valgt fra gruppen bestående av benzosyre, et organisk konserveringsmiddel, et anionisk lineært polymert carboxylat og et anionisk tverrbundet polycarboxylat, idet munnskyllevannet har en pH på 3-5.
2. Munnskyllevann ifølge krav 1, karakterisert ved at det dessuten omfatter en vandig lavere alifatisk alkoholblanding.
3. Munnskyllevann ifølge krav 1 eller 2, karakterisert ved at det inneholder et fuktighetsbevarende middel.
4. Munnvann ifølge krav 1-3, karakterisert ■ ved at det også inneholder et anionisk overflateaktivt middel.
5. Munnskyllevann ifølge krav 1-4, karakterisert ved at hjelpestoffet er benzosyre .
6. Munnskyllevann ifølge krav 1-4, karakterisert ved at hjelpestoffet er pro-pylparaben.
7. Munnskyllevann ifølge krav 1-4, karakterisert ved at hjelpestoffet er dehydroeddiksyre.
8. Munnskyllevann ifølge krav 1-4, karakterisert ved at hjelpestoffet er metyl-vinyleter-maleinsyreanhydrid-kopolymer.
9. Munnskyllevann ifølge krav 1-4, karakterisert ved at hjelpestoffet er car-boxyvinylpolymer.
