Description
Foreliggende oppfinnelse angår en fremgangsmåte for fremstilling av et konjugat av et antitumorlegemiddel og et monoklonalt antistoff hvor konjugatet er innrettet for å
avgi cytotoksiske midler til tumorceller.
Det er i de senere år blitt rettet betydelig oppmerk-somhet mot anvendelsen av tumor-tilknyttede monoklonale antistoffer som bærere for cytotoksiske midler (Moller, 1982). Formålet med mye av dette arbeid har vært å forbedre effektiviteten av anticancerlegemidler samtidig med at de uønskede og ofte toksiske bivirkninger forminskes. Undersøkelser med henblikk på å forbedre effektiviteten ved anvendelse av antistoff-legemiddelkonjugater hvor antistoffet tjener til å levere anticancerlegemidlet til tumoren, har vært foretatt eller foreslått.
For at en slik fremgangsmåte skal være virknings-full, er det nødvendig at antistoffet er meget tumorselek-tivt og at legemidlet avgis i en aktiv, cytotoksisk form. Legemidler som methotrexat (Endo, 1987), daunomycin (Gallego et al., 1984), mitomycin C (MMC) (Ohkawa et al., 1986) og vincaalkaloider (Rowland et al., 1986) er bundet til antistoffer, og de fremkomne konjugater er undersøkt med henblikk på antitumoraktiviteter. I mange tilfeller kan de sporadiske aktiviteter av slike konjugater tillegges den forminskede aktivitet av legemidlet når dette er kovalent bundet til antistoffet. I teknikken forekommer mange eksempler som illustrerer bindingen mellom antistoffer og legemidler ved hjelp av forholdsvis stabile kjemiske bindinger som gjennom-går langsom ikke-spesifikk nedbrytning, f.eks. hydrolyse.
Ytterligere problemer kan imidlertid oppstå når legemidlet frigis fra antistoffet i en kjemisk modifisert form. Selv om legemidlet nå kan ha adgang til dets virk-ningssete, kan det kjemisk modifiserte legemiddel være vesentlig mindre virkningsfullt.
På grunn av disse forhold foreligger et behov for utvikling av nye bindingsstrategier, dvs. hittil ukjente legemiddel-antistoffkonjugater som kan frigjøre kjemisk umodifisert legemiddel fra antistoffet på en slik måte at legemidlet kan utøve sitt maksimale aktivitetsnivå. Under-søkelser har vist at prolegemiddelforbindelser som er benzylcarbamat-disulfidderivater av mitomycin C (MMC), mitomycin A (MMA) og daunomycin, frigjør kjemisk umodifisert legemiddel når disulfidbindingen reduseres (se US patentsøknad nr. 124.314, fig. 1).
Det har vist seg at en prolegemiddelstrategi som av-henger av disulfidbindingsreduksjon for frigivelse av legemiddel, kan være velegnet for levering av legemidler til tumorer med tumor-tilknyttede antistoffer, fordi mange faste tumorer har vist seg å forekomme i oxygenmanglende omgivelser og å være i besittelse av forhøyede innhold av reduksjonsmidler som glutathion, NADH og NADPH (Sartorelli, 1986). Disse reduksjonsmidler kan fremkalle frigivelsen av et fritt legemiddel fra benzylcarbamatdisulfid-legemiddelkonjugater ved reduksjon av disulfidbindingen.
Anvendelsen av benzylcarbamatdisulfidlinkere til legemiddel-antistoffkonjugater kan også være anvendelig ved intracellulær frigivelse av legemidler i tilfeller hvor antistoffet tas opp inne i cellen ved reseptor-formidlet endocytose. Intracellulære thioler som glutathion, vil deretter kunne redusere de disulfidbundne konjugater.
Foreliggende oppfinnelse angår et legemiddel-antistof f kon jugat hvor antistoffet og legemidlet er forbundet ved anvendelse av disulfidbenzylcarbamat, f.eks. et MMC-antistoffkonjugat, eller disulfidbenzylcarbonat, f.eks. et etoposid-antistoffkonjugat.
Ifølge et annet aspekt angår oppfinnelsen en fremgangsmåte for levering av et aktivt antitumorlegemiddel til setet for tumorceller hos et pattedyr, ved å administrere til pattedyret et legemiddel-monoklonalt antistoffkonjugat ifølge oppfinnelsen.
Det har vist seg at disulfidbindingsreduksjonen innleder en legemiddel-fragmenteringsprosess hvorved stam-legemidlet, dvs. det umodifiserte legemiddel, frigis i en aktiv, cytotoksisk form. Videre kan hastigheten av lege-middelfrigivelse kontrolleres ved sterisk hindring av disulfidet. Ved anvendelse av kjemien beskrevet i US patent-søknad nr. 124.314, forekom intet vesentlig tap i legemiddel-aktivitet. Vesentlig samme fremgangsmåte har vist seg å være anvendelig ved binding av amingruppeholdige legemidler og tilsvarende hydroxylgruppeholdige legemidler og protein-toksiner til antistoffer for sete-rettet immunterapi. Fig. 1 illustrerer reaksjonsveien for eliminering av MMC fra det tilsvarende pro-legemiddel. Fig. 2 illustrerer fremstillingen av et representa-tivt legemiddel-monoklonalt antistoffkonjugat ifølge oppfinnelsen. Fig. 3 illustrerer sammenlignende HPLC-analyse-resultater for et autentisk legemiddel og et konjugat i fravær og i nærvær av reduksjonsmiddel. Foreliggende oppfinnelse angår en fremgangsmåte for fremstilling av et konjugat av et antitumorlegemiddel og et monoklonalt antistoff med den generelle formel

Eksempler på aminogruppeholdige legemidler er mitomycin C, mitomycin A, daunomycin, adriamycin, aminopterin, actinomycin, bleomycin og derivater derav, og etoposid er et
eksempel på de alkoholgruppeholdige legemidler.
De anvendte forkortelser er som følger: MMC, mitomycin C; MMA, mitomycin A; DAU, daunomycin, PBS, fosfat-bufret, fysiologisk saltvann; HPLC, høytrykksvæskekromato-grafi, DDT, dithiothreitol; og Ab, monoklonalt antistoff.
Fremgangsmåten ifølge oppfinnelsen er kjennetegnet ved at man omsetter en hydroxysuccinimidester med formelen

D, R<1>, R2, R<3>, R<4> og m har den ovenfor beskrevne betydning, med et monoklonalt antistoff Ab hvor det er tilknyttet en aminogruppe, i et inert organisk oppløsningsmiddel.
Konjugatet fremstilt ifølge foreliggende oppfinnelse kan tilveiebringes som et farmasøytisk preparat for å behandle en vert, spesielt en pattedyrvert, som f.eks. en forsøksdyr-vert, som er påvirket av en tumor. Det farmasøytiske preparat inneholder en antitumorvirksom mengde, dvs. en tumorvekst-inhiberende mengde, av konjugatet ifølge foreliggende oppfinnelse, og en farmasøytisk akseptabel bærer, og eventuelt konvensjonelle, farmasøytisk akseptable tilsetninger og hj elpestoffer.
Antistoffkomponentene av immunokonjugatet
omfatter et hvilket som helst antistoff som bindes spesifikt til et tumor-forbundet antigen. Eksempler på slike
antistoffer omfatter, men er ikke begrenset til, antistoffer som bindes spesifikt til antigener som finnes på carcin-omer, melanomer, lymfonomer og knokkel- og bløtdelssarcomer samt andre tumorer. Antistoffer som forblir bundet til celle-overflaten i lengre tidsrom eller som internaliseres, fore-trekkes. Disse antistoffer kan være polyklonale eller for-trinnsvis monoklonale og kan fremstilles ved velkjente fremgangsmåter i teknikken (se f.eks. R.A. DeWeger et al., "Eradication of Murine Lymphoma and Melanoma Cells by Chlorambucil-Antibody Complexes", Immunological Rev., 62,

Den farmasøytiske bærer vil vanligvis foreligge i flytende tilstand for tilveiebringelse av flytende preparater, da flytende preparater vil forventes å være de mest foretrukne, fordi faste preparater vil forventes å ha lavere absorpsjon fra fordøyelseskanalen. De beskrevne konjugater kan tilveiebringes som sterile, oppløselige konjugater eller preparater som kan oppløses eller suspenderes i sterilt vann eller et annet flytende medium, for å tilveiebringe oppløs-ninger og suspensjoner og emulsjoner til oral eller parenteral administrering. Eksempler på flytende bærere som er egnet til oral administrering, omfatter vann, alkohol, polypropylenglycol, polyethylenglycol og blandinger av to eller flere av de ovennevnte midler. Eksempler på flytende bærere som er egnet for parenteral anvendelse, omfatter vann-til-injeksjon, fysiologisk saltvann og andre egnede, sterile injeksjonsmedier. Egnede buffere for anvendelse med den flytende bærer for å tilveiebringe en generelt egnet, bufret isotonisk oppløsning, omfatter trinatrium-orthofosfat, natriumbicarbonat, natriumcitrat, N-methylglucamin, L(+)-
lysin og L(+)-arginin.
Det farmasøytiske preparat vil inneholde en mengde av minst ett konjugat med formel (I) eller en blanding av én eller flere av forbindelsene eller en blanding derav med et annet antitumormiddel. Den antitumor-virksomme mengde av forbindelsen med formel (I) kan varieres eller innstilles innenfor et bredt område avhengig av den spesielle anvendelse, formen og styrken av det spesielle konjugat som anvendes, og den ønskede konsentrasjon av konjugat i prepar-atet. Mengden av aktiv bestanddel ligger vanligvis i området fra 0,5 til 90 vekt% basert på den samlede preparat-vekt.
Ved terapeutisk anvendelse for behandling av en pattedyrvert, f.eks. en forsøksdyrvert som er påvirket av en tumor, ondartet eller godartet, administreres konjugatene ifølge oppfinnelsen i en mengde som er virksom for å inhi-bere veksten av tumoren, dvs. en tumorvekst-inhiberende mengde i området fra 0,1 til 15 mg/kg kroppsvekt/dag. Det går tydelig frem at den faktisk foretrukne dose av konjugatet varierer i vidt omfang avhengig av behovene hos dyret som skal behandles, av det anvendte preparat samt adminis-treringsmåten. Mange faktorer som modifiserer virkningen av det anti-neoplastiske middel, vil tas i betraktning av den dyktige tekniker, f.eks. dyrevertens alder, legemsvekt og kjønn, samt diett, tidspunkt for administrering, ut-skillelseshastighet, vertens tilstand og sykdommens alvor. Administrering kan utføres på én gang eller periodisk innenfor den maksimalt tolererte dose. Optimal administrering (eller anvendelse) under gitte forhold kan lett fast-settes av den dyktige tekniker ved anvendelse av konvensjonelle dosebestemmelsestester.
De følgende eksempler belyser foreliggende oppfinnelse. Medmindre annet er anført, er alle deler og prosent-angivelser basert på vekt, og temperaturer er i grader Celsius. Forbindelsene og konjugatene er nummerert i over-ensstemmelse med fig. 1 og 2.
Protein A renset, monoklonalt antistoff benevnt
L6 (IgG2a), som virker som et glycolipidantigen på humane lungecarcinomer (Hellstrom et al., 1986) er tilveiebrakt av dr. K.E. og dr. I. Hellstrom (Oncogen, Seattle). Den humane tumorcellelinje A549, er tilveiebrakt av dr. J. Catino (Bristol-Myers Co., Wallingsford).
Konjugatbindingsforsøk
Immunokonjugater ble seriefortynnet i vekstmedium,
og 100 pliter aliquoter ble inkubert ved 4°C med lxlO<6> celler i 100 pliter vekstmedium. Etter 1 time ble cellene vasket to ganger og resuspendert i 100 yliter medium som inneholdt 1:40 fortynnet geit-anti-mus IgG-FITC (Boehringer-Mannheim)
i 20 minutter ved 4°C. Cellene ble vasket og analysert på et "Coulter Epics V"-fluorescens celleanalyseapparat. Til hvert forsøk ble det anvendt lignende fortynnet MAb som en ikke-konjugert, positiv bindingskontroll.
Cytotoksisitetsforsøk in vitro
A549-celler i 0,4 ml McCoys komplette medium ble anbrakt med 1000 celler/brønn på vevskulturplater med 12 brønner og inkubert natten over ved 37°C. Cellene ble vasket med RPMI-medium, og konjugat i 0,4 ml McCoys medium ble tilsatt. Med periodevise intervaller (1, 3, 6 og 24 timer) ble cellene vasket med RPMI for å fjerne eventuelt ubundet konjugat eller legemiddel, friskt McCoys medium ble tilsatt, og inkuberingen ble fortsatt ved 37°C i totalt 24 timer. Koloniene ble farget med krystallfiolett og tellet med et "Optimax 40.10"-bildeanalyseapparat.
Generell fremgangsmåte - Fremstilling av 1 og 2
En oppløsning av hhv. 137 mg (0,55 mmol) para-
eller ortho-mercaptobenzylalkohol og 0,044 ml pyridin (0,55 mmol) i 1 ml tørt dioxan ble i løpet av 3 minutter tilsatt til en omrørt oppløsning av 0,032 ml (0,275 mmol) tri-klormethylklorformiat i 0,5 ml dioxan. Etter omrøring i 15 minutter ble en oppløsning av MMC (92 mg, 0,275 mmol) og
triethylamin (0,153 ml, 1,1 mmol) i 4 ml dioxan tilsatt hurtig. Etter 5 minutter ble oppløsningsmidlene avdampet.,
og en oppløsning av bunnfallet i CH2C12 ble ekstrahert med mettet NaHC03, NaCl og tørket (MgS04). Produktet ble renset ved lynkromatografi på en 2 x 20 cm Si02~kolonne ved først å adskille upolart materiale med 30% ethylacetat i petroleumsether (300 ml) og deretter eluere carbamatet med 5% methanol i kloroform. Produktet, hhv. 1 og 2, ble erholdt som et
■ amorft, blått, fast stoff som ble oppløst i 3 ml CH2C12 og tilsatt dråpevis til 30 ml petroleumsether. Både i tilfelle av 1 og 2 ble et fast produkt med følgende egenskaper erholdt:

MMC benzylcarbamatdisulfid 2: utbytte 63% blått pulver,

Fremstilling av hydroxysuccinimidester 6
Til en oppløsning av 125 mg (0,205 mmol) av 1 i 5 ml aceton ble tilsatt 18 uliter (0,205 mmol) 3-mercaptopropionsyre. Ytterligere 18 uliter 3-mercaptopropionsyre ble tilsatt etter 1 time, og reaksjonen var fullstendig etter 1 1/2 time. Oppløsningsmidlet ble fjernet under vakuum, og bunnfallet ble renset ved lynkromatografi (Si02) under eluering med 10% methanol i methylenklorid. Syren (5) ble anvendt i neste trinn uten ytterligere rensning.
En oppløsning av syren (5, 0,186 mmol), N-hydroxysuccinimid (43 mg, 0,372 mmol) og dicyclohexylcarbodiimid (77 mg, 0,372 mmol) i 2 ml tørr DMF ble omrørt i 3 timer. Bunnfallet ble filtrert fra og vasket med ethylacetat. Etter at oppløsningmidlene var fjernet under vakuum ble bunnfallet renset ved preparativ TLC (Si02) under eluering med 10% isopropanol i methylenklorid. Hydroxysuccinimidesteren (6) ble erholdt som et fint blått, fast stoff (26 mg).

Fremstilling av hydroxysuccinimidester 8
Hydroxysuccinimidesteren 8 ble fremstilt ut fra pyridyldisulfidet 1 og 2-mercapto-2-methylpropionsyre som beskrevet ved fremstillingen av 6. Det ble erholdt et fint blått, fast stoff (55 mg) ut fra 100 mg av 1.

Fremstilling av hydroxysuccinimidester 10
Hydroxysuccinimidesteren 10 ble fremstilt ut fra pyridyldisulfidet 2 og 2-mercapto-2-methylpropionsyre som beskrevet for fremstillingen av 6. Produktet 10 ble erholdt som et blått, fast stoff (10 mg) ut fra 71 mg av 2.

Fremstilling av legemiddel- antistoffkonjugater 11- 13
Oppløsninger av hydroxysuccinimidesterne 6, 8 og 10 (2,9-3,7 mM) i acetonitril ble tilsatt til L6-antistoff (2,61 mg/ml) i 1,5 ml 50 mM boratbuffer (pH 8,5) som inne-
holdt 100 mM NaCl ved 30°C. Hydroxysuccinimidesterne 8
og 10 (20-dobbelt samlet molart overskudd) ble tilsatt i 4 like store porsjoner med 10 minutters intervaller, mens 6 (10-dobbelt samlet molart overskudd) ble tilsatt i 2 like store porsjoner ved 0 og 10 minutter. Etter 40 minutter ble bunnfallet fjernet ved sentrifugering, og supernatantene ble dialysert natten over mot PBS ved 4°C. Dialysatene ble for-siktig rotert med ca. 0,5 g "SM-2"-polystyrenperler (BioRad) i 10 minutter ved 4°C, og ble deretter filtrert sterilt for. å fjerne eventuelt legemiddel som ikke var kovalent bundet til antistoffet. HPLC-analyser (beskrevet nedenfor) av konjugatene viste at fritt legemiddel eller frie legemiddel-derivater ikke var til stede. Sammensetningen av de således erholdte konjugater ble bestemt ved legemiddelabsorbansen ved 365 nm (£ = 21086) og antistoffabsorbansen ved 280 nm (Abs 280 nm, 1 mg/ml = 1,4) og viste følgende: 11, 0,86 mg/ml Ab (1,29 mg ialt), legemiddel/Ab = 4,38/1, 12, 0,70 mg/ml Ab (1,05 mg ialt), legemiddel/Ab = 5,14/1, 13, 1,07 mg/ml Ab (1,65 mg ialt), legemiddel/Ab = 5,25/1.
Et antitumor-legemiddel-antistoffkonjugat hvor lege-middelkomponenten er det alkoholgruppeholdige legemiddel etoposid som er bundet til disulfidbenzyldelen ved en car-bonatbinding istedenfor en carbamatbinding, ble fremstilt ved i det vesentlige å følge de ovennevnte fremgangsmåter ved at det først ble tilveiebrakt et etoposid-benzylcarbonat-disulfid ved å anvende etoposid (162 mg, 0,0275 mmol) istedenfor MMC, og deretter anvende det dannede etoposid-benzylcarbonat-disulfid istedenfor MMC-benzylcarbamat-disulfid under de resterende trinn av konjugatfremstillingen.
MMA, daunomycin, adriamycin og de andre aminogruppeholdige legemidler kan naturligvis anvendes istedenfor MMC
i de ovenfor beskrevne eksempler.
Stabilitet av konjugater
Konjugatene 11-13 ble fortynnet med et tilsvarende volum vekstmedium som inneholdt RPMI og 10% serum fra kalvefoster. Oppløsningene ble inkubert ved 37°C. Porsjoner (0,25 ml) ble innført på protein-A-kolonner (0,25 ml) med periodevise intervaller, og kolonnene ble vasket med PBS for å fjerne eventuelt ubundet materiale. De bundne konjugater ble eluert med 100 mM eddiksyre som inneholdt 150 mM NaCl (0,5 ml) og hurtig nøytralisert. Spektrofotometrisk ana-lyse ble anvendt for å bestemme sammensetningen av konjugatene.
Reaksjon av konjugatene med dithiothreitol
Til en oppløsning av legemiddel-antistoffkonjugatene 11-13 i PBS ble det tilsatt dithiothreitol (sluttkonsentra-sjon 0,2 mM). Etter 19 timer ved romtemperatur ble aliquoter analysert med HPLC ved anvendelse av en 10 cm "Whatman Partasil 5 ODS-3" revers fase-C-18-kolonne og følgende gradi-entsystem: 30% CH30H i 0,1% acetat (pH 6) til 95% CH30H i 6 minutter, fortsatt i 8 minutter, gjennomstrømmingshastig-het 2 ml/minutt, påvisning ved 340 nm.
Fremstilling av konjugater
Hydroxysuccinimidesterne 6, 8 og 10 ble fremstilt fra mitomycin C benzylcarbamatpyridyldisulfider 1 og 2 ved en totrinns-fremgangsmåte som omfattet disulfidombytting med en thiolsubstituert syre etterfulgt av forestring av syren med N-hydroxysuccinimid (fig. 2). Reaksjon av hydroxysuccinimidesterne med antistoffet L6 ved pH 8,5 førte til dannelsen av antistoff-MMC-konjugater 11-13. Konjugatene var frie for eventuelt ikke-kovalent bundet MMC og ble karakterisert ved HPLC og uv/vis-spektroskopi. Det var mulig å binde inntil 6 MMC-molekyler til L6 under anvendelse av den beskrevne kjemi. Antagelig fortrengte aminogrupper på antistoffet hydroxysuccinimidesterne på de aktive legemidler, og amidbindinger ble dannet mellom antistoffet og legemiddel-derivatene.
Frigivelse av mitomycin C fra konjugatene
Benzylcarbamat-disulfid-legemiddelderivatenes evne til å gjennomgå legemiddeleliminering ved disulfidbindingsreduksjon er undersøkt i detalj (se Senter ovenfor). Den antatte mekanisme for fragmenteringsreaksjonen ved reduksjon av disulfidbindingen i konjugatene 11-13 ville føre til frigivelse av MMC.
L6-MMC-konjugatet 11 ble redusert med en overskyt-ende mengde dithiothreitol, og reaksjonen ble overvåket ved HPLC. Det ble iakttatt at MMC-frigivelse forekom, noe som ble dokumentert ved sammenligning med en autentisk prøve (fig. 3). I fravær av et reduksjonsmiddel var konjugatene i PBS fullstendig stabile under reaksjonsbetingelsene.
Det var av interesse å bestemme om sterisk hindring av disulfidbindingen ville ha noen virkning på stabiliteten av legemiddel-antistoffkonjugatene. Det er tidligere beskrevet at ricin-A-kjede-immunotoksier med hindrede di-sulfider var mer stabile in vitro enn tilsvarende ikke-hindrede immunotoksiner (Worrell et al., 1986).
Konjugatene ble inkubert ved 37°C i 1:1 oppløsninger av PBS og vekstmedier inneholdende RPMI og 10% serum fra kalvefoster. Den mengde legemiddel som forble bundet til antistoffet, ble bestemt etter at konjugatene var reisolert på en protein-A-affinitetskolonne. Etter 24 timer viste det seg at 11, 12 og 13 frigjorde hhv. 40%, 21% og 9% av det bundne legemiddel. Derfor kan forhøyet konjugatstabil-itet oppnås ved forhøyelse av graden av hindring av disulfidbindingen.
Binding og in vitro- cytotoksisitet av konjugatene
Konjugatene ble undersøkt med henblikk på deres
evne til å bindes til reseptorer på A549 lungecarcinom-cellelinjen.
Fluorescensaktivert cellesortering viste at alle tre konjugater bandt seg like godt til cellene som det umodifiserte antistoff. Kjemien anvendt til legemiddelbinding, påvirket tilsynelatende ikke virkningen av antistoffets egenskaper med hensyn til binding til cellereseptorene.
Den cytotoksiske aktivitet hos konjugatene overfor A549-cellelinjen ble bestemt for en rekke eksponeringstider (tabell 1). Det minst hindrede konjugat 11 utviste vesentlig vekstinhibering etter eksponering i 3 timer, mens det for de mer hindrede konjugater 12 og 13 gikk betydelig lengre tid før en cytotoksisk virkning kunne iakttas. Etter 24 timer var alle tre konjugater meget cytotoksiske. De erholdte IC5Q-verdier for konjugatene 11, 12 og 13 etter en ekspon-eringstid på 24 timer var hhv. 55 nM, 64 nM og 59 nM. IC,-0-verdiene for det frie MMC etter eksponering i 24 timer var 50 nM. Konjugasjonskjemien bevarer derfor aktiviteten av legemidlet.

1. Fremgangsmåte for fremstilling av et konjugat av et antitumorlegemiddel og et monoklonalt antistoff med den generell formel
hvor D er en antitumor-legemiddeldel hvor det til skjelettet av denne er knyttet en kjemisk reaktiv funksjonell gruppe, og ved hjelp av denne gruppe er legemiddelskjelettet bundet til di-sulf idbenzyloxycarbonylgruppen, idet D er avledet fra gruppen bestående av en primær aminogruppe med formelen R^-NH-, en sekundær aminogruppe med formelen R<X>R<2>N- og en alkoholgruppe med formelen R<1>©-, hvor R<1> er skjelettet av legemiddeldelen når D er avledet fra en primær aminogruppe, en sekundær aminogruppe eller en alkoholgruppe, hvor R<1> og R2 er innbyrdes uavhengige, i tilfelle av en sekundær aminogruppe,R<2>, når R<1> og R2 er innbyrdes uavhengig, er valgt blant usubstituerte og substituerte, forgrenede og uforgrenede alkylgrupper med 1-10 carbonatomer hvor substituenten er valgt blant 1-3 alkoxygrupper med 1-3 carbonatomer og 1-3 halogenatomer, usubstituert og substituert fenyl hvor substituenten er valgt blant 1-3 alkylgrupper med 1-3 carbonatomer, 1-3 alkoxygrupper med 1-3 carbonatomer og 1-3 halogenatomer, og usubstituert og substituert fenylalkyl hvor fenyldelen, i tilfelle av substitusjon, er substituert på samme måte som beskrevet ovenfor for substituert fenyl, og alkyldelen er en polyalkylengruppe med 1-3 carbonatomer, R<1> og R<2> sammen i en funksjonell gruppe avledet fra et sekundært amin, represen-terer skjelettet av legemiddeldelen D hvor en divalent gruppe er bundet kjemisk til nitrogenatomet som inngår i den sekundære aminogruppe,
