Description
FARMACEUTISCHE SAMENSTELLING VOOR DE BEHANDELING VAN HERPES
De onderhavige uitvinding heeft betrekking op een farmaceutische samenstelling voor de behandeling van herpes.
Herpes simplex infectie op de huid veroorzaakt oppervlakkige pijnlijke plekken en blaasjes, die uiteindelijk open gaan, waardoor lesies ontstaan. Herpes wordt voornamelijk veroorzaakt door Herpes simplex virus (HSV) type 1, maar kan ook het gevolg zijn van HSV type 2, die gewoonlijk genitale herpes veroorzaakt. Omgekeerd wordt genitale herpes in zo'n 30% van de gevallen veroorzaakt door HSV 1.
Infecties van de mond worden aangeduid met de term herpes labialis, ook wel koortslip genoemd. Ook andere delen van het gezicht, zoals ogen en neus kunnen aangetast zijn. Men spreekt dan over faciale herpes simplex. De infectie kan zich eveneens op andere delen van het lichaam manifesteren.
Ongeveer 70-90% van de mensen wordt primair met HSV besmet. Deze mensen zijn dan drager van het virus. Wanneer een individu eenmaal met het HSV geïnfecteerd is, zal dit virus daarna latent in het lichaam aanwezig blijven. In latente toestand bevindt het virus zich in zenuwcel-lichaampjes in de ganglia. Door bepaalde prikkels, zoals influenza-infectie of andere respiratoire aandoeningen, gastro-intestinale infecties, stress, moeheid, menstruatie, zwangerschap, allergie, zonlicht en koorts kan het latente virus geactiveerd worden. Het zal zich vanuit de ganglia langs wel bepaalde zenuwbanen naar het huidoppervlak begeven en zich daar vermenigvuldigen en de symptomen veroorzaken. Deze recurrente vorm van herpes komt voor bij ongeveer 40% van de geïnfecteerde personen.
Een op zichzelf redelijk onschuldige herpes-sim-plexinfectie kan leiden tot veel ernstiger infecties, zoals keratitis of encephalitis. Het is daarom van belang dat de virushaarden, ter plaatse van de infectie, doeltreffend vernietigd worden.
Het meest toegepaste middel tegen HSV-infecties op dit moment is acyclovir, dat bijvoorbeeld onder de merknaam Zovirax verkocht wordt. Acyclovir is een guanine-analoog dat interfereert met het DNA-polymerase van het virus en daardoor virale DNA-replicatie remt. De vermenigvuldiging van het virus wordt tegengegaan, maar het virus zelf wordt echter niet gedood. Het virus kan zich daardoor weer terugtrekken in de ganglia, waardoor opnieuw latentie ontstaat.
Transmissie van HSV-1 gebeurt door direct contact via speeksel of de geïnfecteerde plaats op de huid. Ook deze transmissie wordt door acyclovir niet voorkomen.
Een ander probleem is het optreden van secundaire bacteriële infecties, bijvoorbeeld impetigo, veroorzaakt door staphylococcen en/of streptococcen.
Vanwege al deze, al dan niet bijkomende, problemen van herpesinfectie bestaat er een duidelijke behoefte aan een farmaceutisch preparaat dat zowel het optreden van latentie, als de transmissie en de secundaire bacteriële infecties kan verminderen of zelfs voorkomen.
Volgens de onderhavige uitvinding is nu gevonden dat een farmaceutische samenstelling, die bestaat uit een quaternaire ammoniumverbinding als een eerste component en/of een antiviraal middel als een tweede component en/of een plantenextract als een derde component, waarbij de drie componenten onderling compatibel zijn, in een farmaceutisch aanvaardbare basis, de bovengenoemde problemen kan oplossen.
Om de vicieuze cirkel van latentie, recurrentie en opnieuw latentie van het virus te doorbreken is het nodig om het virus zelf te doden. Hierdoor kan bij primaire of recur-rente infectie minder of zelfs geen latentie meer optreden omdat het dode virus zich niet kan terugtrekken in de ganglia. De belangrijkste component van de onderhavige samenstelling volgens de uitvinding is derhalve een virucidale substantie waardoor het virus gedood wordt.
Antivirale middelen zijn onder te verdelen in een aantal categorieën (Colgate, S.M. & Molyneux, R.J., Bio-active natural products, 410, CRC Press, Inc. (1993)), aangeduid met de classificatie groep Ia tot groep Vb. Vol- gens de uitvinding is nu gevonden dat voor een zo volledig mogelijke aanpak van HSV infecties bij voorkeur één of meer substanties uit groep IVb gecombineerd moeten worden met één of meer substanties uit groep Va. Substanties uit groep IVb vertonen virucidale activiteit, terwijl verbindingen uit groep Va virustatische en antivirale activiteit vertonen. Door de combinatie van beide typen substanties wordt de virusreplicatie geremd, en worden tevens de reeds aanwezige virussen gedood.
Om er voor te zorgen dat ook de secundaire besmetting van de door HSV infectie veroorzaakte lesie door andere micro-organismen, zoals bacteriën, voorkomen wordt gaat de voorkeur er naar uit dat ofwel de eerste component en/of de tweede component desinfecterende eigenschappen heeft.
Om de begeleidende symptomen van pijn en ontsteking te verminderen wordt daarnaast bij voorkeur een plan-ten-extract gebruikt. Dit extract heeft naast ontstekings-remmende eigenschappen bij voorkeur tevens een verzachtende werking.
Produkten uit groep IVb die tevens een desinfecterende werking hebben en derhalve toegepast kunnen worden in de farmaceutische samenstelling volgens de uitvinding kunnen op een voor de deskundige bekende wijze geselecteerd worden. Hierbij wordt gebruik gemaakt van de eindpunt-titratietech-niek (EPTT), zoals beschreven door Colgate & Molyneux (supra. p. 413), waarmee de virucidale activiteit van een produkt getest kan worden. De activiteit wordt uitgedrukt in virustiter-reductie, welke in vitro bepaald wordt door incubatie van een herpesvirus-suspensie met verdunningen van het te testen produkt bij 37°C gedurende 5 minuten. Een goed produkt zal minimaal een titerreductie van 103 geven.
Uit een algemene screening van bekende desinfec-tantia op hun virucidale werking tegen Herpes simplex type 1 en Herpes simplex type 2 stammen (klinische isolaten) is volgens de uitvinding gevonden dat enkele produkten een zeer hoge activiteit vertonen, zelfs nog bij 25°C. Dit is zeer voordelig omdat de temperatuur op de huid in het algemeen lager is dan 37°C, en zelfs lager dan 34°C. Gevonden is dat de quaternaire ammoniumverbindingen cetrixnide en benzalkoni-umchloride in een concentratie van respectievelijk 50-200 jig/ml, respectievelijk 100-200 Mg/xnl een titer-reductie van tenminste 103 vertoonden gedurende een testperiode van 5 minuten bij 25°C. Andere bekende desinfectantia, zoals alcoholen, fenolen, peroxiden, biguaniden, aldehyden, chloorverbindingen etc. vertonen een minimaal 5 maal lagere activiteit. Alleen kwikverbindingen en bepaalde zware metaalverbindingen hebben een betere activiteit. Vanwege hun grote toxiciteit zijn deze verbindingen voor de bedoelde toepassing echter niet geschikt.
In principe kunnen alle quaternaire ammoniumver-bindingen gebruikt worden met de algemene formule.
waarin R1, R2, R3, en R4 gelijk of verschillend kunnen zijn en staan voor een alifatische of aromatische groep, waarbij tenminste één van Rv R2, R3 of R4 een alifatische keten van 8 of meer koolstofatomen is; en waarin X een laagmoleculair anorganisch anion of een hoogmoleculair organisch anion is. Een overzicht van geschikte quaternaire ammoniumverbindingen wordt bijvoorbeeld gegeven door A. Dauphin & J.C. Darbord in "Hygiëne hospita-lière pratique", Editions medicales internationales, Parijs (1988). De meeste voorkeur gaat uit naar cetrimide.
Quaternaire ammoniumverbindingen zijn goed oplosbaar in water en alcohol maar kunnen niet met elk willekeurig ander produkt gecombineerd worden. Een belangrijke eis voor de tweede component is daarom dat hij compatibel moet zijn met de eerste component. Anionische produkten, zepen, nitraten, zware metalen, oxiderende produkten, rubber, eiwitten en dergelijke zijn niet compatibel met quaternaire ammoniumverbindingen. Dergelijke produkten zijn derhalve niet geschikt voor toepassing in de samenstelling volgens de uitvinding. De tweede component is bij voorkeur niet terato-geen en kan in een hoge dosering gegeven worden zonder daarbij toxisch te zijn. Bij voorkeur vertoont de tweede component verder een goede penetratie van de huid.
Als tweede component kunnen bekende anti-virale produkten worden toegepast, zoals acyclovir, BVDU, 3FT, Idoxuridine etc., zolang deze compatibel zijn met de eerste component. Daarnaast kunnen niet-toxische natuurprodukten met antivirale activiteit gebruikt worden.
De geschiktheid van de tweede component kan eveneens met een op zichzelf bekende EPTT-methode getest worden (Colgate & Molyneux, supra, p. 414). Hierbij wordt gebruik gemaakt van VERO-cellen. Volgens de uitvinding is op deze wijze gevonden dat gallaten in vitro een goede anti-virale werking hebben. Zo heeft propylgallaat in een concentratie van 100 Mg/ml een titerreductiefactor van 104. In een concentratie van 25 Mg/ml is de reductiefactor van de infec-tieuze partikels in VERO-cellen 103. Ethylgallaat heeft een reductiefactor van 104 in een concentratie van 100 μg/ml en van 102 bij 25 μg/ml. Ethylgallaat is dus iets minder actief.
De compatibiliteit van de eerste component en de tweede component kan bepaald worden door een mengsel van beide componenten te testen op HSV-1. Gebleken is dat een mengsel van propylgallaat en cetrimide (2:1) in een concentratie van 250 jug propylgallaat op 12 5 /xg cetrimide nog steeds een virucidale werking van 103 heeft na een testperiode van 5 minuten bij 25°C. Voor ethylgallaat en cetrimide geldt hetzelfde. Een mengsel van cetrimide en BVDU of acyclovir blijft een anti-virale activiteit vertonen na uitverdunnen in vitro tot 5 μg/ml BVDU of acyclovir en 0,1 μg/ml cetrimide.
Bij voorkeur bevat de farmaceutische samenstelling tevens een plantenextract, waarmee de pijn en gevolgen van de ontstekingsreactie verminderd kunnen worden. Ook hiervoor geldt dat het extract de activiteit van de eerste en de tweede component niet mag verstoren.
Hiertoe werden diverse plantenextracten getest met cetrimide en propylgallaat of benzalkoniumchloride en propylgallaat om te kijken of de virucidale werking behouden blijft en de componenten compatibel zijn. Gebleken is dat extracten van kamille en Calendula bruikbaar blijven in vitro. Ook extracten van Aloë vera, Echinaceae-wortel, driekleurig viooltje, Senna, melisse en/of meidoorn zijn in staat de ontstekingsreactie of pijn te verminderen terwijl de virucidale activiteit behouden blijft.
Bepaalde plantenextracten veroorzaken echter een duidelijke vermindering van de virucidale werking van sommige quaternaire ammoniumverbindingen. Gevonden is bijvoorbeeld dat 10% hamamelis de virucidale en anti-bacteriële werking van benzalkoniumchloride (0,5%) volledig inactiveert. De activiteit van 0,1% benzalkoniumchloride tegen herpes en bacteriën wordt door salie en grote klis-extract 4 tot 8 maal verminderd. Een combinatie van cetrimide met een tweede component en een plantenextract van kamille, Calendula, salie, grote klis en senna bleek in vitro wel actief te blijven tegen zowel Herpes simplex virus als bacteriën.
De compatibiliteit van het toe te passen plantenextract dient derhalve vooraf bepaald te worden. Een dergelijke bepaling ligt binnen het bereik van de gemiddelde deskundige.

De farmaceutische samenstelling volgens de uitvinding, die bestaat uit een combinatie van één of meer quaternaire ammoniumverbindingen met een virucidale en antiseptische werking en/of een antiviraal produkt dat compatibel is met de quaternaire ammoniumverbinding en de virusreplicatie remt en/of een plantenextract, dat eveneens compatibel is met beide eerder genoemde produkten en de bijkomende effecten van de herpes-infectie, zoals pijn, jeuk, zwelling, tinteling etc. vermindert, verschaft een totale behandeling van herpes-infectie. De virucidale activiteit van de samenstelling doodt de virussen, waardoor latentie niet of verminderd zal optreden. De antivirale activiteit remt de virusreplicatie en daarmee de vermenigvuldiging van het virus. De antiseptische activiteit voorkomt secundaire bacteriële infecties. De ontstekingsremmende activiteit tenslotte, gaat ontstekingen tegen en verzacht bovendien de pijn en de jeuk.
De componenten van de samenstelling volgens de uitvinding zijn bij voorkeur opgenomen in een farmaceutisch aanvaardbare basis. Ook voor de basis geldt dat hij de werking van de drie componenten niet mag verminderen. Om te testen of een basis geschikt is kunnen de virucidale werking van de eerste component en de desinfecterende werking bepaald worden. De desinfecterende werking wordt bijvoorbeeld op een binnen het bereik van de deskundige liggende wijze gedurende 5 minuten bij 25°C op gram-negatieve kiemen, bijvoorbeeld van Pseudomonas aeruginosa of Escherichia coli. bepaald.
Voorbeelden van geschikte bases zijn polyethyleen-glycolen in verschillende molecuulgrootten of mengsels daarvan, esters van vetzuren of mengsels daarvan, al dan niet gemengd met emulgerende en/of verzorgende bestanddelen. Ook mengsels van polyethyleenglycolen en/of esters van vetzuren met of zonder de emulgerende en/of verzorgende bestanddelen zijn geschikt.
De farmaceutische samenstelling volgens de uitvinding kan de vorm hebben van een poeder, suspensie, oplossing, spray, emulsie, zalf of crème en kan gebruikt worden voor lokale toediening. Dergelijke samenstellingen kunnen bereid worden door het combineren (d.w.z. mengen, oplossen etc.) van de actieve componenten in de vorm van een vrij zuur of zout met farmaceutisch acceptabele excipienten met een neutraal karakter (zoals waterige of niet-waterige oplosmiddelen, stabilisatoren, emulgatoren, detergentia, additieven), en verder indien noodzakelijk kleur-, geuren/ of smaakstoffen. De concentratie van het (de) actieve ingrediënt(en) in een farmaceutische samenstelling kan variëren tussen 0,001% and 100% (g/v), afhankelijk van de aard van de behandeling en de wijze van toedienen. De voorkeur gaat uit naar een zalf.
De samenstelling van de uitvinding bevat bijvoorbeeld 10-90% van een niet-waterige basis, 1-20% van-een alcohol, 0,1-5% van één of meer quaternaire ammoniumver-bindingen, en/of 0,01-2% van één of meer antivirale middelen, en/of 0,5-10% van één of meer plantenextracten. De gebruikte hoeveelheid plantenextract hangt sterk samen met de werkzaamheid daarvan en van de concentratie werkzame bestanddelen in het extract.
Een voorkeurssamenstelling volgens de uitvinding, bevat 20-60%, bij voorkeur 52% PEG 400, 10-40%, bij voorkeur 26% PEG 4000, 2-20%, bij voorkeur 9% glycerol, 0,5-1,8%, bij voorkeur 1% cetrimide, 0,2-3%, bij voorkeur 1% cetylalcohol, 5-15%, bij voorkeur 8% kamille-extract en 0,2-1%, bij voorkeur 0,25% propylgallaat.
De onderhavige uitvinding zal verder worden verduidelijkt aan de hand van de bijgaande voorbeelden, die echter niet de bedoeling hebben de uitvinding op enigerlei wijze te beperken.
VOORBEELDEN VOORBEELD 1 I. Bereiding van bet antiberpes preparaat

Propylgallaat 1 % werd een zalf bereid door de ingrediënten onder A te smelten bij 75°C en de ingrediënten onder B op te lossen. Vervolgens werd het na smelten verkregen mengsel van de ingrediënten onder A onder langzame afkoeling homogeen gemengd en werd het mengsel van B voor het stollen van het mengsel van A toegevoegd. Vervolgens werd verder gemengd, totdat het totale mengsel volledig was afgekoeld. Het mengsel werd 93J12 genoemd.
Polyethyleenglycol 400 en 4000 waren afkomstig van Pharmaceuticals, waardoor zij verkocht worden onder de merknamen Macrogol 400 - Purna en Macrogol 4000. Glycerol was van p.a. kwaliteit en afkomstig van Merck (4094), cetrimide A 11936 werd geleverd door Pharmachemic NV, cholinechloride (C1879) door Sigma. Cetylalcohol wordt onder de merknaam Lanette 16 door Henkel KGaA verhandeld. Het vloeibare kamille-extract was afkomstig van Conforma en propylgallaat P3130 van Sigma.
II. Bereiding en samenstelling van M-VDB medium

Fenolrood stockoplossing 1 ml (d.i. 1% fenolrood in een waterige oplossing van 0,05N NaOH ) werd een onderhoudsmedium voor celculturen bereid door alles op te lossen in 960 ml gedestilleerd water, vervolgens de pH op 7,4 te stellen met NaOH (10N) en te filtreren tot steriel door een 0,2 μ membraanfilter. Vervolgens werd 2% Newborn Calf Serum (voor VERO cellen) of 2% foetal kalfs serum (voor andere cellen) toegevoegd.
VOORBEELD 2 Antivirale activiteit 1. Antivirale activiteit van de componenten
In dit voorbeeld wordt gebruik gemaakt van VERO cellen die afkomstig waren van de American Type Culture Collection (toegangsnummer CCL 26). VERO cellen werden gekweekt in Medium 199 (Gibco) met Earle's zouten en 5 % kalfsserum. Als virusbron werd Herpes Simplex Virus 1 (HSV-1, getypeerd klinisch isolaat) gebruikt, welke werden opgekweekt in M-VDB met 100 Mg/ml penicilline, 100 Mg/ml streptomycine en 2 % kalfsserum.
Op een monolaag van VERO cellen, die gekweekt werd in microtiterplaten (96 openingen met vlakke bodem), werd 100 μΐ van een verdunningsreeks van 10'1 tot 10'6 van HSV-1 in M-VDB aangebracht. Men liet het virus gedurende 60 tot 90 minuten aan de cellen adsorberen. Vervolgens werd een verdunningsreeks van het testprodukt in M-VDB aan de geïnfecteerde cellen toegevoegd. Om de cytotoxiciteit van het produkt te controleren werden de verdunningen van het pro-dukt eveneens toegevoegd aan niet geïnfecteerde cellen. Na de incubatie bij 37°C in een vochtige kamer gedurende 5 dagen, werd microscopisch het cytopathogene effect (CPE) beoordeeld.
De antivirale activiteit van het produkt wordt uitgedrukt als de reductiefactor (RF) van de hoogste niet toxische concentratie van het monster. De reductiefactor (RF) is de verhouding van de concentratie virus in de controle vergeleken met de concentratie levend virus in de pro-duktverdunningen. Een RF van > 101 wordt als significant beschouwd. De resultaten van het experiment met propylgal-laat staan weergegeven in tabel 1.

Uit het bovenstaande blijkt, dat de antivirale activiteit van propylgallaat voor HSV-1 tot een concentratie van 25 μg/ml nog steeds significant is.
De virucidale activiteit werd bepaald door virussuspensies, die 106 cfu HSV-1 bevatten, te mengen met een gelijk volume van de testproduktoplossingen in verschillende concentraties. Dit mengsel werd gedurende een bepaalde tijd bij gecontroleerde temperatuur geïncubeerd. Een verdunnings-reeks van 10'1 tot 10'6 van elk mengsel werd op een monolaag van VERO cellen gebracht. Na incubatie gedurende vijf dagen bij 37°C werd het CPE beoordeeld. Het resultaat werd uitgedrukt in RF. Opnieuw werd een titerreductie van > 101 als significant beschouwd.
De resultaten van de virucidale activiteit ten opzichte van HSV-1 bij 34°C gedurende 15 minuten staan weergegeven in tabel 2.

Uit het bovenstaande blijkt, dat propylgallaat geen virucidale activiteit vertoont, en dat Cetrimide virucidale activiteit heeft tot een concentratie van 100 μg/ml. Het mengsel van propylgallaat en Cetrimide behoudt zijn virucidale activiteit ondanks een overmaat propylgallaat (2/1). 1
Antivirale eigenschappen van het testmenqsel
Op overeenkomende wijze werd de antivirale activiteit van het in voorbeeld 1 bereide testmengsel bepaald. De resultaten van de virucidale activiteit van de zalf bij 25°C gedurende 15 minuten staan weergegeven in tabel 3.
Uit tabel 3 blijkt, dat de zalf ten opzichte van HSV-1 tot een verdunning van 1/100 bij 25°C nog steeds virucidale activiteit vertoont. De cetrimide behoudt dus zijn volledige activiteit, zelfs bij 25°C.

Antibacteriële activiteit
De antibacteriële activiteit van zowel de afzonderlijke componenten als het eindprodukt werd bepaald met de testorganismen Candida albicans (ATTC 10231), Escherichia coli (ATTC 8739), Pseudomonas aeruginosa (ATTC 15442) en Staphylococcus aureus (ATTC 6538) gebruikt. De gebruikte media waren Tryptic Soy Broth (TSB), Tryptic Soy Agar (TSA) en Steriele fosfaatbuffer (PBS).
I. Antibacteriële activiteit van componenten en menasels Om de minimale remmende concentratie (MIC) te bepalen werd van elk micro-organisme een overnachtcultuur gekweekt in TSB bij 37°C. Van de gebruikte testprodukten werd een 1/2 verdunningsreeks, eveneens in TSB gemaakt. Er werd van elke verdunning 100 μΐ in een microtiterplaat gebracht. De overnachtculturen werden 1/1000 verdund in TSB. Van elke bacteriesuspensie werd eveneens 100 μΐ aan de produktverdunningen toegevoegd. Als controles werden pro-duktverdunningen zonder toevoeging van kiemen gebruikt ter controle van de steriliteit en bacterieverdunningen zonder toevoeging van produktverdunningen. De microtiterplaten werden gedurende 24 uur bij 37°C in een vochtige kamer geïncubeerd. De bacteriegroei werd beoordeeld aan de hand van de troebeling van de suspensie. De MIC waarde is de laagste produktconcentratie die de normale groei van een micro-organisme kan verhinderen, dat wil zeggen geen troebe-ling tot gevolg heeft.
De resultaten worden weergegeven in tabel 4.
Uit het bovenstaande blijkt, dat het kamille-extract een zeer beperkte antibacteriële activiteit heeft ten opzichte van sommige micro-organismen en slechts in hoge concentraties. Propylgallaat en cetrimide daarentegen hebben een significante antibacteriële activiteit. In de mengsels blijkt cetrimide zijn antibacteriële activiteit te behouden.
II. Antibacteriële activiteit van de testzalf
Er werden drie afzonderlijke porties zalf bereid, zoals beschreven in voorbeeld 1, welke de lotnummers 94A12, 94D21 en 94E05 meekregen. Als placebo werd dezelfde samenstelling bereid echter met weglating van cetrimide. De placebo wordt aangeduid met 94D27. Als controle werd 1% cetrimide in steriel gedestilleerd water gebruikt. Alle testmonsters werden bewaard bij kamertemperatuur.
Ha. Bepaling van minimale inhiberende concentratie (MIC).
De minimale inhiberende concentratie is die concentratie van het testprodukt, waarbij bacteriën net niet meer groeien. De hoogste gebruikte testconcentratie (1/10 verdunning) werd bereid door één gram zalf te suspenderen in 9 ml steriel gedestilleerd water. De verdunningen werden bij de bacteriën gevoegd, welke vervolgens gedurende 24 uur gekweekt werden. De gebruikte testorganismen waren Candida albicans (Ca), in een hoeveelheid van ΙΟ5, 106 Escherichia coli (Ec), 105 Pseudomonas aeruginosa (Pa), en 106 Staphylococcus aureus (Sa).
Tabel 5 geeft de verdunning waarin de bacterie-groei geremd wordt.
Uit de tabel blijkt dat alle testzalven hun anti-bacteriële activiteiten behouden. De MIC van deze zalven is vergelijkbaar met de MIC waarde van 1% cetrimide in water.
In de geteste concentraties heeft de placebo geen anti-bacteriële werking.
Ilb. Bepaling van de bactericide en fungicide activiteit
De bactericide activiteit wordt getest met Candida albicans (105), Escherichia coli (106) , Pseudomonas aeruginosa (105) en Staphylococcus aureus (106) . De bactericide activiteit wordt bepaald volgens de voorschriften van de Europese Farmacope. Deze houden in dat het te testen produkt wordt gemengd met het micro-organisme en enige tijd geïncu-beerd, waarna dit mengsel wordt verdund en bepaald wordt bij welke van de verdunningen er nog bacteriegroei optreedt.
Van elk micro-organisme werd een verdunning bereid tot een eindconcentratie van 106 - 107 CFU. De zalven worden onverdund en in een 1/2 en 1/10 verdunning in steriel gedestilleerd water getest. Aan een gram van de zalf of haar verdunning werd 100 μΐ van één verdund inoculum gevoegd en gemengd. Na 15 minuten incubatie bij 25° C werd van 100 mg van dit mengsel een 1/10 verdunningsreeks bereid in TSB (Tryptic Soy Broth (Gibco)). Elke verdunning werd uitgeplaat op TSA (Tryptic Soy Agar (Gibco)). De platen werden gedurende 24 uur bij 37°C geïncubeerd. Als controles werden een onbehandelde cultuur van de micro-organismen en 1% cetrimide in gedestilleerd water activiteit gebruikt. Na incubatie werden alle platen geëvalueerd op groei. De tests uitgedrukt in Reductiefactor (Rf) werden in drievoud uitgevoerd. De resultaten worden weergegeven in tabel 6.
Uit de bovenstaande tabel blijkt dat alle testzal-ven ook in verdunningen hun bactericide activiteit behouden.
Alle kiemen werden gedood. De placebo bleek een zeer beperkte bactericide-activiteit te hebben.
lie. Fungicide activiteit
Voor de bepaling van de fungicide activiteit werden testor-ganismen Eoidermophvton floccosum (RV 69635) en Trichophvton rubrum (RV58124) gebruikt.
De fungicide activiteit werd op dezelfde wijze bepaald als de bactericide activiteit.
Hiertoe werd van elk micro-organisme wordt een verdunning bereid tot een eindconcentratie van 104 - 105 CFU. De zalven werden onverdund getest en in een 1/2 en 1/10 verdunning in steriel gedestilleerd water. Aan één gram van de zalf of haar verdunning worden 100 μΐ van een verdund inoculum gevoegd en vermengd. Na 15 minuten incubatie bij 25° C wordt van 100 mg van dit mengsel een 1/10 verdunnings-reeks bereid in SAB (Sabouraud Bouillon (Gibco)). Elke verdunning werd uitgeplaat op SABA (Sabouraud Agar (Gibco)). De platen werden gedurende 5 dagen bij 25°C geïncubeerd. Als controles werden een onbehandelde cultuur van de micro-organismen en 1% cetrimide in gedestilleerd water. Na incubatie werden alle platen geëvalueerd op groei. De tests werden in drievoud uitgevoerd. De resultaten, uitgedrukt in Reductie factor (Rf), worden getoond in tabel 7.
Uit de bovenstaande tabel blijkt dat de testzalven hun fungicide activiteit, die vergelijkbaar is met die van 1% cetrimide in gedestilleerd water, behouden. De placebo heeft een beperkte fungicide activiteit.
lid. Virucide activiteit
De virucide activiteit werd bepaald volgens de micromethode beschreven door D. Vanden Berghe et al. in "Methods in Plant Biochemistry", Vol. 6 "Assays for Bioactivity", p. 49-67, Academie Press Limited 1991. Bij 300 μΐ produktverdunningen werden 300 μΐ onverdunde virussuspensie gevoegd. Deze mengsels werden gedurende 15' geïncubeerd bij 34°C. Verdunningsbuisjes werden voorzien van 0,9 ml gekoeld kweekmedium voor virus en in een ijsbad geplaatst. Van de geïncubeerde mengsels werd 100 μΐ in de verdunningsbuizen gebracht. Hieruit werd een 1/10 verdunningsreeks bereid. Van deze verdunningen werd 200 μΐ op de VERO cellen gebracht. Controles waren onbehandeld virus (= viruscontrole) en onbehandelde cellen (= celcontrole). De platen werden ge-incubeerd bij 37°C in een vochtige incubator gedurende 5 tot 7 dagen. De cellen werden microscopisch geëvalueerd op cytotoxisch en cytopathogeen effect (CPE). De resultaten worden uitgedrukt in Reductiefactor (Rf). De reductiefactor is de verhouding van de concentratie residueel virus ten opzichte van de initiële virusconcentratie. De testen werden in drievoud uitgevoerd. De resultaten uitgedrukt in Rf, staan weergegeven in tabel 8.
Uit de bovenstaande tabel blijkt dat de testzalven virucide werken tot in de verdunning 1/50. De verucide activiteit is vergelijkbaar met de activiteit van 1% cetri-mide. De placebo heeft geen virucide activiteit.
VOORBEELD 4
In vivo activiteitsproef
De testgroep bestond uit vijftien patiënten met koortsblaasjes aan de lippen. Zij werden behandeld met een zalf met de volgende samenstelling: PEG 400 52% PEG 4000 26% glycerol 9% cetrimide 1% choline (eventueel) 2% cetylalcohol 1% kamille-extract 8% propylgallaat 0,25%
De resultaten van het experiment staan weergegeven in de hierna volgende tabel 9.
In een tweede experiment met 2 mannelijke patiënten werd de preventieve werking van de zalf getest. Daartoe werd op de plaats waar normaal gesproken lesies optraden gedurende 6 maanden éénmaal per dag de zalf aangebracht. Het gevolg van deze behandeling was dat er geen zogeheten "opstoten" (herhalingen van de symptomen) meer optraden, zelfs niet bij verkoudheden. Voorafgaand aan de behandeling had één van beide patiënten tenminste één maal per maand een koortslip terwijl bij beiden verkoudheden altijd gepaard gingen met symptomen.
De onderhavige uitvinding verschaft een nieuwe farmaceutische samenstelling voor de behandeling van herpesinfecties, in het bijzonder faciale of labiale herpes veroorzaakt door HSV-1 of -2. Het grote verschil met andere middelen is dat de meeste patiënten die het middel curatief of preventief gebruiken later op dezelfde plaats geen opstoten meer krijgen.
Claims (10)
1. Farmaceutische samenstelling voor de behandeling van herpes simplex virus infecties, omvattende een quaternaire ammoniumverbinding als een eerste component en/of een antiviraal middel als een tweede component en/of een plantenextract als een derde component, waarbij de drie componenten onderling compatibel zijn, in een farmaceutisch aanvaardbare basis.
2. Farmaceutische samenstelling volgens conclusie 1, met het kenmerk, dat de eerste component wordt gekozen uit de groep, die bestaat uit cetrimide, benzalkoniumchlori-de.
3. Farmaceutische samenstelling volgens conclusie 1 of 2, met het kenmerk, dat de tweede component wordt gekozen uit de groep, die bestaat uit acyclovir, broomvinyl-desoxuridine (BVDU), 3-fluorthymidine (3FT), idoxuridine, propylgallaat, ethylgallaat, proanthocyaniden, glucosamine.
4. 6. Farmaceutische samenstelling volgens conclusie 1, 2 of 3, met het kenmerk, dat de derde component wordt gekozen uit de groep, die bestaat uit extracten van kamille, Calendula, Senna in geval de eerste component benzalkonium-chloride is.
7. Farmaceutische samenstelling volgens één der conclusies 1-6, omvattende: 0.1-5% van een quaternaire ammoniumverbinding; 0,01-3% van een antiviraal middel; en 1-20% van een plantenextract; in een farmaceutisch aanvaardbare drager.
8. Farmaceutische samenstelling volgens conclusie 7, omvattende: PEG 400 52% PEG 4000 26% glycerol 9% cetrimide 1% choline (eventueel) 2% cetylalcohol 1% kamille-extract 8% propylgallaat 0,25%
9. Gebruik van een farmaceutische samenstelling volgens één der conclusies 1-8 voor de behandeling van HSV-1 infecties.
10. Gebruik van een farmaceutische samenstelling volgens één der conclusies 1-8 voor de behandeling van HSV-2 infecties.
