Abstract
본 발명은 지속적 방출형 안과용 제제에 관한 것으로서, 약70%의 폴리옥시에틸렌 단위를 갖고 평균 분자량 약12,500인 폴리옥시 에틸렌-폴리옥시프로필렌 공중합체(폴록사머)와 물의 혼합물로 형성된 고점도 용액의 형태중에 활성성분을 포함하는 안과용 제형에 있어서, 안정제로서 비이온성 계면활성제를 더욱 포함하는 것을 특징으로 하고 있다.
Description
본 발명은 포스콜린(forskoline) 또는 안과에 사용되는 다른 활성성분을 포함하는 지속적 방출형 안과용 제제에 관한 것이다.
안과용 제형은 일반적으로 활성성분 또는 성분들을 용액 또는 현탁액의 형태로 포함하고 있다. 후자의 제형은 용액에 비하여, 예를들어 재현탁의 곤란으로 부터 파생되는 단점을 보여 주며 결과적으로 용량의 항상성 결여와 생체이용율 저하를 수반한다. 게다가 현탄액중의 재형은 입자 크기의 증가 및 결정 특성의 변동과 관련되는 문제를 야기하여 결과적으로 눈의 자극 및 제형의 안정성을 손상할 가능성도 있다.
통상적인 수용액 형태로서 안과용 약제를 투여하는 것은 활성성분의 물리화학적 특성에 따라 허용되며, 이러한 단점을 없애줄 수 있으나, 이번에는 반사유루(reflex lachrymation), 눈깜박임 및 배출과 같이 투입되는 약제의 농도 및 저류시간을 신속히 감소시키는 눈의 보호적 생리작용에 기인한 다른 단점을 나타낼 수 있다. 따라서 긴 작용시간을 가지며 여러가지 형태의 활성성분을 포함하는 안과용 제형에 대한 의학적 필요성이 있게 된다.
본 발명의 주제는 활성성분이 고점도 용액중에 포함되는 안과용의 약제학적 제형이다. 이러한 제형은 점액성 액체 또는 겔과 같은 외양을 나타낼 수도 있는데 정상적인 사용 조건하에서 안정하고 지속적 작용을 한다.
제안된 제형은 투여 용량과 빈도를 감소시키기 위한 방식으로 결막낭내에서 용액중에 활성성분이 지속되도록 하는데, 환자 순응도 및 국소내용성을 개선시킨다.
미국특허 제4,100,271호 및 4,188,373호는 점막에 국소 적용하기 위한 "폴록사머(poloxamers)"에 기초한 제제를 개시하고 있는데 이것은 상온에서는 액체이고 체온에서는 겔이 된다.
본 발명의 예시는 포스콜린의 안과용 제형에 관련된다.
포스콜린은 7β-아세톡시-8, 13-에폭시-1α, 6β, 9α-트리히드록시-라브드-14-엔-11-온 인데, 라브단 류(labdane family)의 디테르펜으로서, 콜레우스 포스콜리(Coleus forskolii)의 추출물로부터 얻는다. 국소 투여된 포스콜린의 안압저하 작용은 토끼, 원숭이 및 사람에게서, 건강한 대상 및 녹내장에 걸린 대상에게서 충분히 시사되었다(란셋지(The Lancet), i, 958, 1983 ; 임상안과학지(Clin. Ocul.), 9, 173, 1988). 안압 저하 작용은 눈의 후방으로부터 나오는 안내액의 모양체 혈관으로의 흡수와 함께, 모양체 상피의 클리어 셀내에 있는 아데닐 사이클라제 시스템(adenylcyclase system)의 자극에 의해 유도되는 방수(aqueoushumour)의 생산 감소에 기인한다(미국 안과학회지(Am. J. Ophtalm.) 100, 194, 1985).
포스콜린은 실질적으로 물에 불용성인 고체이고 현재까지는 수성현탁액의 형태로서 눈에 투여되고 있다(란셋지 (Lancet), i, 958, 1983 ; 임상 안과학지(Clin. Ocul.), 9, 173, 1988).
최근에, "폴록사머"와 "사이클로 덱스트린(cyclodextrins)"을 용해제로 사용하여 수성매체 중에 포스콜린을 용해시키는 기술(안과양학지(J. Ocul. Pharmacol.), 5, 111, 1988)이 개시되었다.
특히 폴록사머 407은 폴리옥시에틸렌 단위가 총 모노머 단위 수의 약 70%이고 평균 분자량이 약 12,500인 폴리옥시에틸렌-폴리옥시프로필렌으로서, 물에서 높은 용해성을 가지며 적절한 농도에서는 열가역성 겔화특성을 갖는다.
물중에 용해되는 포스콜린의 양은 폴록사머 407의 농도에 직접적으로 비례한다. 폴리머의 최대 테스트 농도(25% w/v)에서는 5℃에서 최대 0.12%의 포스콜린이 용해된다. 그러나 이러한 제형은 안정하지 않아서, 예를들어 열자극과 같은 어떤 저장 조건하에서는 활성성분이 침전한다. 자극조건하에서의 저장시험 [에스.에스.데이비스 : 의약품의 안정성시험, 더블류, 그림 및 케이.토마, 과학출판사 스투트가르트 1987(S.S Davis in : Stability Testing of Drug Products, W.Grimm and K. Tomae, Eds. Wissenschaftliche Verlagsgesellschaft GmbH Stuttgart 1987)]은 약제학적 제제의 잠재적 불안정성을 보여주기 위하여 추천되는데, 이러한 불안정성은 저장 및 운반도중에 명백하게 될 수도 있다. 게다가 식품약품국(Food and Drug Administration)의 "사람의 약물 및 생물학적제제의 안정성 연구를 위한 지침"에서는 약제학적 제제가 온도변화주기 처리를 받도록 함으로써 의약품이 유통경로 도중 받게 되는 변화를 모의 실험하도록 권하고 있다.
폴록사머 중에 있는 포스콜린 또는 다른 활성성분의 수용액중에, 안과용도로서 생리적으로 배합가능한 비이온성 계면활성제를 적정량 가할때, 상기의 자극 조건하에서도 활성성분의 재침전을 막는다는 것이 발견되었다. 사용될 수 있는 비이온성 계면활성제 중에서 바람직한 것은 특히 폴리옥시에틸렌 소르비탄과 지방산의 에스테르(바람직하게는 폴리소르베이트 20 및 폴리소르베이트 80) 및 폴리에톡실화된 식물유(바람직하게는 약40몰의 에틸렌 옥사이드를 갖는 폴리에톡실화된 수소화 피마자유)이다. 이들 계면활성제는 본 발명의 주제를 형성하는 조성물중에 5%(w/v)보다 적지 않은 양으로 존재한다.
사실, 이보다 적은 농도에서는 저온에서 단지 한정된 기간 동안 그리고 어떤 조건에서는 시장 유통을 허용할 수 없을 정도로 짧은 기간동안만 활성성분의 침전을 막을 뿐이다.
폴록사머는 제형중에서 15 내지 25% w/v의 범위로 존재하며, 반면에 활성성분은 0.01 내지 0.1% w/v의 범위로 존재한다. 경이롭고도 본 발명의 주된 장점 중의 하나를 구성하는 것은 본 발명의 조성물로서, 기간이 보다 연장되고 용량의 포스콜린 또는 10배 높은 용량의 다른 활성성분을 포함하는 공지의 조성물로부터 얻어지는 것과 비교하여 보다 현저한 치료효과를 얻는 것이 가능하게 된 것이다.
게다가 계면활성제의 존재는 제제의 누액과의 혼화성을 개선시켜 준다. 본 발명의 주제를 형성하는 제형은 또한 용해성이고 생리적으로 배합가능한 적절한 염을 포함하여, 제형을 누액과 등장으로 만들고 그의 pH를 생리적 범위내에서 조절하고 미생물 오염으로부터 제제를 보호한다.
포스콜린외에도 본 발명의 제형에는 다음과 같은 다른 활성성분을 포함할 수 있다 :
항녹내장 약제(예를들어 필로카핀, 티몰롤) ;
혈관수축제(예를들어 나파졸린, 테트라존린) ;
스테로이드성 항염제(예를들어, 히드로코르티손, 프레드니졸론, 덱사메타손, 베타메타손 및 이들의 에스테르) ;
비스테로이드성 소염제(예를들어 플루비프로펜, 디클로페낙, 케토프로펜, 벤다작) ;

항생제(예를들어 네오마이신, 클로람페니콜, 옥시테트라사이클린) ;
국소마취제(예를들어 테트라카인).
본 발명의 제형은 상기 언급된 활성성분의 두가지 또는 그 이상을 조합하여 포함할 수 있다.

상기 제형의 제조에는, 활성성분이 완전히 용해될때까지 포스콜린과 폴록사머 407의 혼합물을 계속 교반하면서 60~70℃까지 가열하는 것을 포함한다. 별도로 남은 부형제를 70℃에서 물에 용해시키고, 이 용액을 상기의 용융물에 교반하면서 가한다.
5℃까지 냉각하여 투명한 균질액을 얻는데, 이것은 졸-겔 전이 온도가 16~18℃를 넘지 않는다.
실시예 2~5에 기술된 제제에도 동일한 방법을 사용한다.

이 제형의 제조에는 필로카핀, 폴록사머 407 및 폴리소르베이트20의 혼합물을 계속적으로 교반하면서 70℃까지 가열하는 것을 포함한다. 별도로, 남은 부형제를 70℃에서 물에 용해시키고, 이 용액을 상기의 용융물에 교반하면서 가한다.
5℃까지 냉각하여 투명한 균질액을 얻는다.
이 제형은 실온에서는 점성의 액체로, 체온에서는 겔로 나타난다.

염산필로카핀 4g

이 제형은 실시예6에 기술된 방법에 따라 제조된다.
얻어진 제형은 졸-겔 전이온도가 16~18℃보다 높지 않다.
실시예 1 및 2의 제형과 비교겔(안과 약물학지(J. Ocul. Pharmacol.), 5, 111, 1989)의 안정성을 두가지 테스트로 검사하였다 :
- 시험 A는 3~6℃범위내의 온도에서 제제를 저장하는 것을 포함한다 ;
- 시험 B는 3~6℃에서 24시간 및 실온에서 24시간의 냉각/가열 주기처를 포함한다(주기시험).
표 1은 저장 조건 A 및 B전후 실온 및 37℃에서 제제의 외관과 관련된 데이타를 보여준다.
본 발명의 주제를 형성하는 제형의 안내압(Intraocular pressure : IOP)과 관련된 활성은 토끼의 눈에 대하여 정상 안압에서 국소적용후에 시험하였다.
토끼는 시험동안 구속 장(restrain cage)에 넣고 오른쪽 눈에 시험중인 제형 100㎕를 적용하기 직전과 0.5, 1, 2, 4, 8 및 10시간후에 Schiotz 안압계를 사용하여 IOP를 측정하였다. 비교를 위하여 메틸 셀룰로스중 0.1 및 1% 포스콜린 현탁액과 안과약물학지(J. Ocul. Pharmacol.), 5, 111, 1989에 따라 제조된 0.12%겔을 사용하였다.
표 2에 나타낸 결과는 0.1% 현탁액이 투여 30분 후부터 최대 기간 1~2시간 동안 IOP를 약3㎜Hg만큼 감소시키는 것을 보여준다. 이에 반하여 실시예1에 따라 제조된 포스콜린 동등량의 투여는 적어도 8시간동안 지속되는 IOP의 감소를 야기한다. 최대효과는 1 내지 2시간의 범위내에 있으며 약7㎜Hg의 IOP감소를 수반한다. 실시예1의 제형은 1% 현탁액 및 0.12% 대조겔과 비교하여도 더 큰 활성과 특히 보다 긴 작용기간을 가짐을 보여준다.
IOP와 관련된 활성은 또한 오른쪽 눈의 후방에-키모트립신 300단위/㎖를 포함하는 용액 0.5㎖를 주사하여 고안압으로 만든 토끼의 눈에 있어서도 평가되었다(미국 안과학회지(Am. J. Ophtalm.), 77, 378, 1974). 다음 15일 동안 처리된 눈의 검사와 IOP의 측정을 매일 시행하였다. 안과조직의 비정상화나 과도하게 낮은 혈압값을 나타내는 동물들은 실험으로 부터 제외하였다. 16일째 토끼들은, 시험중인 제형 100㎕를 처리된 눈에 적용하기 직전과 0.5, 1, 2, 3, 4, 5, 6, 7 및 24시간 후에 IOP의 측정을 받았다.
표 3에 나타낸 결과는 1% 포스콜린 현탁액이 투여 30분후에 5㎜Hg의 감소를 보여주고 이러한 작용은 2시간 이내에 소실되는 것을 지적한다.
실시예 4 및 5의 제형은 0.05% 포스콜린을 포함하는데(상기 현탁액보다 20배 낮은 농도) 신속한 발현과 긴 지속시간을 특징으로하는 현저한 안압저하 작용을 갖는다. 사실상 최고효과는 약-9㎜Hg로서 1시간후에 나타나며 현저한 효과는 투여 8시간 후까지도 존재한다.
표 4의 결과는 4% 염산필로카핀을 포함하여 실시예6 및 7의 제형에 대한 유사한 특성을 보여준다.
제형 1, 4 및 5의 안 내용성을, 대응하는 위약과 메토셀(MethocelR)중의 0.1% 포스콜린 현탁액과 비교하여 검사하였다. 이 제제 100㎕를 6마리 토끼의 오른쪽 눈의 결막낭에 투여하였다. 투여받지 않은 왼쪽눈은 대조군으로 사용된다. 적용후 두개의 눈꺼풀을 수초동안 덮어서 약물이 없어지지 않도록 하였다. 5분, 1,4,6 및 24시간후 눈을 검사하였다. 결막에 영향을 주는 병변(충혈,부종 및 유루)의 강도는 드레이즈 스케일(Draize scale)에 따른 값에 의해 평가하였다(약물학실험 치료지(J. Pharmacol. Exp. Ther.), 81, 377, 1944).
표 5에 나타낸 결과는 시험제형의 내용성이 좋음을 입증한다. 0.1% 포스콜린 현탁액은 투여 바로 5분후에 약간 자극성을 나타낸다.
이러한 결과는 문헌(Kin. Mol. Augenheilk., 185, 522, 1984)에 기재된 바와 일치하고 활성성분의 혈관확장 특성에 기인하는 것이다. 이러한 자극효과는 지속시간이 짧고 투여된 현탁액을 신속히 세척해 버리는 눈의 능력과 관련된다.
본 발명의 주제를 형성하는 제형이 보다 장기간 나타나고 6시간동안 지속되며 시간에 따라 감소되는 약간 자극성을 유발한다 하더라도, 이러한 현상은 활성성분을 조절된 방식으로 장기간에 걸쳐 방출하는 본 발명의 주제를 형성하는 제형의 능력을 확인해 주는 것이 된다. 약간 자극성이 포스콜린에 기인한다는 것을 더욱 확인해 주는 것으로서 위약의 제형은 자극효과가 전혀 없음을 보여준다.
실시예 1~5의 제형은 적절한 양의 비이온성 계면활성제의 첨가에 의하여, 열자극 조건하에서도 포스콜린이 폴록사머 407 용액중에 유지되도록 허용한다.
제안된 제형은 겔 또는 점액성 액체의 형태로 있는데, 따라서 저온에서 뿐 아니라 의약품이 유통경로중 있게 되는 열의 변화 조건하에서도 안정하다. 이러한 혁신적인 제형은 선적 및 저장기간 중 지키기 힘든 특별한 저장 주의를 요하지 않으면서 저장기간 동안 안과용제제의 안정성을 보장한다.
게다가 폴록사머의 농도를 조정함으로써 점성의 점안제의 형태 또는 겔의 형태로 제제의 점도를 조절하는 것이 가능하다. 특히 실시예1, 2 및 3에 사용된 폴록사머 407의 양은 용해되는 활성성분의 양 및 안과용도에 최적의 특성을 갖는 겔을 얻을 수 있는 필요성과 관련하여 선택된다.
제안된 제형의 최적 점도특성은 포스콜린의 활성농도 및 투여빈도의 극적인 감소(10~20배)라는 장점과 함께, 본 제제의 각막 상피와의 접촉시간 연장이라는 점에 반영된다. 이것은 환자의 순응도와 제제의 국소적 내용성에 있어서의 개선이라는 결과를 초래하게 된다.
이러한 신규한 안과용 제형은 활성성분을 안정한 용액중에 유지하여, 그의 용량을 감소시키고 그의 생체내 이용율과 안내용성을 증진시킴으로써 특히 원하는 약물학적 효과를 지속시키는데 특히 유효하다는 것을 입증한다.
이러한 특성은 매우 다양한 안질환의 치료에 특히 장점을 발휘한다.

- 약70%의 폴리옥시에틸렌 단위를 갖고 평균분자량 약12,500인 폴리옥시에틸렌-폴리옥시프로필렌 공중합체(폴록사머)와 물의 혼합물로 형성된 고점도 용액의 형태중에 활성성분을 포함하는 안과용 제제에 있어서, 안정제로서 비이온성 계면활성제를 안과용 제제 전체 중량에 대해 5-6중량%의 양으로 포함하고 있는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 활성성분을 0.01% 내지 4% 함유하는, 높은 치료효능 및 지속적 방출성을 갖는 안과용 제제.
- 제1항에 있어서, 비이온성 계면활성제가 폴리옥시에틸렌 소르비탄과 지방산의 에스테르 또는 폴리에톡실화된 식물유인 것을 특징으로 하는 안과용 제제.

- 제1항에 있어서, 녹내장 치료를 위한 활성성분으로서 포스콜린을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 녹내장 치료를 위한 활성성분으로서 티몰롤을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 눈의 전방부 염증의 치료를 위한 활성성분으로서 덱사메타손을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 눈의 전방부 염증 및 수술 후 염증의 치료를 위한 활성성분으로서 플루비프로펜을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 결막 충혈 증상 치료를 위한 활성성분으로서 나파졸린을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 안 감염 치료를 위한 활성성분으로서 클로람페니콜을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 결막의 알러지 및 염증 증상 치료를 위한 활성성분으로서 톤질아민을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 표면 마취 유도용 의약품의 활성성분으로서 테트라카인을 포함하는 것을 특징으로 하는 안과용 제제.
- 제1항에 있어서, 녹내장 치료를 위한 활성성분으로서 필로카핀을 포함하는 것을 특징으로 하는 안과용 제제.
