본 발명은 신규한 아폽토시스 조정제(Apoptosis regulating composition)에 관한 것이다.
본 발명의 아폽토시스 조정제는, 활성성분으로서, 하기 일반식(1)로 표시되는 카르보스티릴 유도체(이후에는 화합물(1)로서 언급한다) 및 이의 염의 하나 이상을 함유한다:
(상기식에서, R은 페닐환상에 치환기로서 저급 알콕시기를 가질 수 있는 벤조일기이고 카르보스티릴 골격의 3 및 4 위치에서 탄소-탄소 결합은 단일결합 또는 이중결합을 나타낸다)
상기 화합물(1) 및 이의 제조에 유용한 방법들에 관해서는, 일본국 특허 공고 공보 제 43747 / 1989호에 기술된 것을 참조할 수 있다. 또는 화합물(1)은 강심제로서 유용하다고 알려져 있다.
본 발명의 발명자들은 화합물(1)에 관하여 추가로 조사한 결과, 이들이 상술한 공지의 활성으로 부터 거의 예견할 수 없는, 항암활성 및 세포분화 유발 활성 등을 포함하는 아폽토시스 조정 또는 변성(억제 또는 촉진) 활성을 갖고 있다는 것을 발견하였다.
한편, 세포 죽음에는 두가지 형태의 메카니즘이 있다고 알려져 있다. 하나는 소위 괴사(necrosis)라 불리는 전형적인 형태의 세포죽음이다. 형태학적으로, 괴사는 미토콘드리아의 현저한 팽창, 세포질의 팽창 및 핵변형에 이어서, 세포파괴 및 자기분해로 특정되어진다. 이는 수동적으로나 우연히 일어난다. 조직괴사는 예를 들면, 일반적으로 세포에 대한 물리적 외상 또는 화학적 해독에 의하여 야기된다.
다른 형태의 세포죽음은 아폽토시스(예정된 세포죽음) [Kerr. J. F. R. and Wyllie, A. B., Br. J. Cancer, 265, 239(1972)]라 불리는 것이다. 아폽토시스는 다양한 생리학적 상태하에서 일어날 수 있다. 형태학적으로, 아폽토시스는 근접 세포와의 접촉실패, 세포질 농축, 핵내소체활성 - 연관 크로마틴 농축 및 세포농축, 및 핵의 분리 등으로 특정되어진다. 또한 세포표면으로부터 미세융모의 사라짐 및 세포표면의 평탄화(세포표면상에 소포형성 : 막기포)가 관찰된다. 핵내소체 활성에 기인하여 DNA의 뉴클레오좀 단위의 180-200 염기 DNA 단편으로의 세분도 추가로 관찰된다. 아폽토시스 체세포의 최종 단편은 이웃세포에 의하여 식균되어진다. 이러한 메카니즘은 듀발 및 윌리 [Duvall, E. and Wyllie, A. H., Immunology Today, 7(4), 115∼119(1986); Science, 245, 301∼305(1989)]에 의하여 제안되었다. 또한 윌리는 티모시테스의 글루코코르티코이드 - 유발 아폽토시스는 세포내의 핵내 소체 활성을 포함하고 있다고 보고하였다 [Wyllie, A. H., Nature, 284, 555∼556(1986)]. 핵내소체 활성은 아폽토시스가 진행되는 세포내에서 올리고 뉴클레오티드 수준으로 DNA의 단편화를 초래하며 이것은 아가로스겔(agarosegel) 전기영동에 의하여 쉽게 확인될 수 있다.
아폽토시스는 성장, 분화과정에서 또는 조직의 변절과정에서 나타나는 예정된 세포죽음으로서 고려될 수 있다 [Wyllie, A. H., et al., Int. Rev. Cytol., 68, 251∼306(1980)].
티오시테스에서, 칼슘 이온 운반체(ionophore)내에서 칼슘농도의 증가 또는 cAMP농도의 증가는 상술한 아폽토시스의 특징인 상기와 같은 DNA 단편화의 촉진을 유도하며 [Wyllie, A. H., et al., J. Pathol., 142, 67∼77(1984)], 따라서 칼슘이온 및/또는 cAMP도 아폽토시스의 메카니즘에 관련이 있다고 생각된다. 지금까지 보고된 바에 따르면, HL-60 세포의 분화가 레티노산 또는 칼슘 이온 운반체에 의하여 유발되는 이들의 아폽토시스를 언급할 수 있다 [Martin, S. J. et al., J. Immunol., 145, 1859∼1867(1990); Martin, S. J., et al., Clin. Exp. Immunol., 79, 448∼453(1990)].
보고된 바에 따르면, 아폽토시스는 활성 세포주기에서 정상 세포의 배자성육 및 생리적 죽음의 과정에서 (예컨대, 간, 부신피질 및 전립선 세포) 생리적 세포죽음 뿐만 아니라, 글루코코르티코이드 치료에 의하여, 시토톡스 T세포에 의한 세포 손상에 의하여, 호르몬 - 의존조직의 쇠퇴에 의하여, 또는 방사, NK 세포, 킬러 세포, 종양 괴사 인자(TNF), 림포톡신(LT), 다른 시트키네스 등에 의하여 유발된다 [Wyllie, A. Immunol., 20, 779(1990)]. 또한 아폽토시스는 악성 종양의 자발적인 퇴행에서 얻어질 수 있는 바와 같이 나까무라 등의 발견사실에 의하여 확인될 수도 있다 [Nakamura, Y., et al., Rinsho Hifuka (Jpn, J. Clin, Dermatol.), 35(4), 289∼295(1981)].
한편, 아크티노마이신 D(RNA 합성 억제제), 시클로헥시미드(단백질 합성 억제제) 및 칼슘이온(Ca2+) 킬레이트제 등은 아폽토시스를 억압할 수 있는 것으로 보고되어 있으며, 또한 시클로스포린 A(면역 억압제), 헤마토포이에트계 시토키네스 [IL-3, GM-CSF(과립세포 대식세포 군체 자극제), G-CSF(과립세포 군체 자극제)], IL-2, bcl-2 유전 생성물 등도 아폽토시스를 억압한다고 보고되어 있다 [Cohen, J. J., J. Immunol., 132, 38(1984); Wyllie, A. H., et al., J. Pathol., 142, 67(1984); Shi, Y., et al., Nature, 339, 625(1989); Williams, G. L., et al., Nature, 343, 76(1990); Nielo, M. A., J. Immunol., 143, 4166(1989); Vaux, D. L., et al., Nature, 335, 1440(1988)]. 반면에, 시클로헥시미드 및 악티노마이신 D의 경우에, 시클로헥시미드에 의한 급성 백혈병 세포에서, 악티노마이신 D에 의한 소정 소여포 세포에서, 및 이들 두가지에 의한 HL-60세포에서 아폽토시스 유발을 기술한 보고가 있다 [Martin, S. J., et al., J. Immunol., 145, 1859∼1867(1990)]. 다른 한편, 시클로헥시미드는 X-선 조사전에 나타나며 X-선 조사에 의하여 증가되는 림포시트 종양 세포의 아폽토시스를 오히려 억압하고, 아크티노마이신 D는 촉진시킨다고 보고되었다. 따라서, 세포의 종류, 상태, 및 다른 메카티즘이 아폽토시스의 억압 또는 촉진에 연관되어 있다는 것을 제안하였다 [Igarashi, T., et al., Nippon Ketsueki Gakkaishi (Acta Hematol. Jpn.), 51(2), 144(1988)]. 하여튼, 현재로는 세포의 분화, 성장 및 성숙이 아폽토시스와 밀접하게 연관되어 있고 이러한 분화, 성장 등에 어느정도 역할을 할 수 있는 물질도 또한 아폽토시스와 연관되어 있다고 고려되고 있다.
최근에, 안티-Apo-I항체에 의한 암치료는 아폽토시스-관련치료로서 시도되었다. 판시토페니아가 지배적인 미엘로다이스 플라스틱 신드롬(MDS), 불응성 빈혈(RA) 및 불응성 빈혈 위트링(wiht ring) 사이드로블라스트(RARS)는 레티노산 또는 비타민 D3의 합성물로 치료되는 것이 바람직하고, 이러한 합성물은 헤모포이에트 세포에 대한 분화 유발제이며, GM-CSF 또는 IL-3은 혈소판 생성 세포의 과잉 아폽토시스를 억압하는 아폽토시스 조정제로서 역활을 하는 반면에, 아세토(blast) 성장이 활성적인 RAEB(과잉의 아세포에 의한 불응성 빈혈) 및 RAEB-t(REAB 변형)에서는, 레티노산 및 비타민 D3이 아세포의 성숙 혈액세포로 분화를 유발하는 분화 유발제로서 작용한다고 알려져 있으며, 에토포시스 및 아클라루비신은 아세포 성장을 억압하는 (따라서, 아폽토시스를 축진하는) 아폽토시스 조정제로서 작용한다고 알려져 있다 [Shibuya, T., J. Clin. Exp, Med., 160(5), 319∼323(1992)].
무라까미 등의 보고에 따르면, 항적혈구 자기항체 발현 자기면역을 나타내는 유전자변형 (transgenic) 마우스의 절반가량이 자기내성의 결핍결과로서 질병이 생기고 이것은 정상 마우스에서와 같은 자기 항원-자기항체 형성세포에 의한 아폽토시스 유발로부터 야기되는 자기항체 형성 세포를 제거할 수 있는 능력의 부족이 기인한다고 하였다 [Murakami, M., et al., Nature, 357, 77∼80(1992)].
와따나베-후꾸나가 등은, MRL 1pr/1pr 마우스의 경우에, 아폽토시스와 관련된 Fas 분자가 비정상적이고 자기반응 T-세포의 네거티브관계(apoptosis) 메카니즘이 흉선에서 적절히 작용하지 않는다고 제안하였다. 결국, 자기면역 질병이 발생한다 [Watanabe-Fukunaga, R., et al., Nature, 356, 314∼317(1992)].
몬타그니어 등에 따르면, 아폽토시스 DNA 밴드가 HIV-감염 환자로부터의 T 임파세포 추출물에서 관찰되었다. 이러한 현상은 90%의 비증후적 HIV-감염 환자에게서 관찰되며 100%의 AIDS 환자 및 ARC (AIDS-related complex) 환자에게서 관찰되고, HIV-감염 환자에게서도 마찬가지로 증가된 아폽토시스 유발을 지적하고 있다 [Montagnier, L., et al., Sixieme Colloque des Cent Gardes, 9∼17(1991)].
병아리의 성장단계 세포죽음에 관하여, NGF (신경 성장인자 ; 신경세포 신경절에서 세포 이상비대 및 신경섬유 신장을 촉진시키는 단백질)의 사전 투여는 이러한 성장 단계에서의 신경 세포 죽음의 완전한 억제를 도모할 수 있는 [Hamburger, V., et al., J. Neurosci., 1, 60(1981)] 반면에, NGF에 대한 항체의 투여는 역으로 약 90%의 유약 교감 신경세포의 손실을 야기한다 [Levi-Montalchini, R. and Booker, B., Proc. Nat1. Acad. Sci. USA, 46, 384(1960)].
클락(clark)은 자발적인 뉴우런 죽음을 세가지 형태로 분류하고 형태 1를 아폽토시스라고 입증하였는데, 그 이유는 형태 1의 뉴우런 죽음의 경우, 형태학적 특징이 아폽토시스의 특징과 동일하며 DNA 단편화와 함께 형태 1 세포죽음이 성장인자의 박탈에 의하여 야기되는 세포죽음에 포함되기 때문이다 [Clark, P. G. H., Anat, Embryol., 181,195(1990) ; J. Neurosci., 1, 60(1981) ; Proc. Nat1, Acad. Sci. USA, 46, 384(1960) ; Rawson, C. L., et al., J. Cell. Biol., 113, 671(1991)].
에드워드 등의 보고에 따르면, NGF는 교감 신경 세포의 예정된 죽음을 억제할 수 있으므로 아마도 NGF는 아폽토시스를 억제할 수 있다는 것이다 [Edwards, S. N., et al., J. Neurochemistry, 57(6), 2140∼2143(1991)].
피셔 등에 따르면, 습득 질병을 갖는 성년 쥐는, NGF를 투여할 경우, 알쯔하이머 질병으로 손상될때 발견된다고 알려진 전뇌기본 영역 콜린 신경세포에 작용하는 NGF 의 결과로서 습득질병으로부터 회복될 수 있다 [Fischer, W., et al., Nature, 329, 65(1987) ; Barde, Y. -A., Neuron, 2, 1525(1989) ; Hatanaka, H., Develop. Brain Res., 30, 47(1986) ; Hatanaka, H., et al., Develop. Brain Res., 39, 85(1988)]. 하따나까 등은 NGF는 분화, 성숙, 생명지탱 및 노화방지, 손상으로부터 신경세포 보호, 손상된 신경세포의 회복 촉진 및 뇌의 노화와 관련한 신경계질환, 특히 알쯔하이머 질환에서 신경세포 죽음의 억제에 효과적일 수 있다는 가능성을 제안하였다 [Hatanaka, H., Taisha(Metabolism), 28, 891∼899(1991)].
약제 내성 바이러스 간장염의 간병변의 경우, 면역계와 직접 또는 간접적으로 관련된 아폽토시스의 촉진이 간병변에 포함되어 있다고 고려된다.
다른 한편으로, 간에서, 미토겐은 헤파토시테스의 성장을 유발하여 이상중식 상태를 발생시키고, 이러한 상태는 헤파토시테스의 쇠퇴 및 괴사, 즉 아폽토시스로 정상화된다 [Kerr, J. F., et al., Br. J. Cancer, 26, 239∼257(1972)]. 간이 관련되어 있는 한, 아폽토시스는 간 이상증식, 이상증식 결핵형성, 간암등에서 관찰될 수 있는 [Columbano, A., et al., Lab. Invest., 52, 670∼675(1985) ; Columbano, A., et al., Am. J. Pathol., 116, 441∼446(1984)] 반면에, 케르등에 따르면, 아폽토시스는 염증 또는 섬유조직형성을 수반하지 않는다 [Kerr, J. F., et al., Lancet, 2, 827∼828(1979)].
상기의 보고내용에 따라, 본 발명가들은 간장염 환자는, 급성이든 만성이든간에, 아폽토시스가 억제될 경우 치료될 수 있다는 것을 인식하였다. 또한 이들은, 만성간장염이 간경변증에 이어서 간암으로 발전하는 과정의 환자의 경우에, 아폽토시스는 억제된 상태이므로 시토톡스 T 세포는 헤파토시테스 염증을 유발시킨 다음, 섬유조직증식을 유발시켜 간 경변증으로의 악화를 야기할 수 있다는 것을 인식하므로써, 아폽토시스를 촉진할 경우 간장염을 억압하고 간경변으로의 발전을 방지할 수도 있다는 것을 인식하게 되었다.
본 발명은 활성성분으로서, 하나 이상의 일반식(1)의 화합물 및 이의 염의 유효량을 이를 위한 약리학적 허용담체와 배합되어 함유하는 아폽토시스 조정제를 제공한다.
본 발명의 아폽토시스 조정제는 지금까지 언급한 바와 같이, 아폽토시스를 조정하거나 억제할 수 있으며, 이의 작용으로 인하여 항암제, 안티레트로 바이러스제, 및 자기면역 질환용, 혈소판감소증용, 알쯔하이머 질환용 및 다양한 형태의 간장염용 치료제, 종양 병소전위 억제제 등으로서 의약분야에 유용하다.
일반식(1)에서, 페닐환상에 치환기로서 저급 알콕시기를 가질 수 있는 벤조일기는 페닐환상에 치환된 1 내지 3개의 직쇄 또는 측쇄 C1-6알콕시기를 갖는 벤조일기, 예컨대, 벤조일, 2-메톡시벤조일, 3-메톡시벤조일, 4-메톡시벤조일, 2-에톡시벤조일, 3-에톡시벤조일, 4-에톡시벤조일, 4-이소부톡시벤조일, 4-헥실옥시벤조일, 3,4-디메톡시벤조일, 3,4-디에톡시벤조일, 3,4,5-트리메톡시벤조일, 2,5-디메톡시벤조일 등이 있다.
본 발명에 따른 화합물(1)의 활성 성분중에서, 6-[4-(3,4-디메톡시-벤조일)-1-피페라지닐]-3,4-디히드로카르보스티릴이 가장 바람직하다.
본 발명에 따른 활성 성분으로서 제공되는 화합물(1)은 통상적인 산과 함께 약리학적 허용염을 용이하게 형성시킬 수 있다. 이러한 산으로는, 무기산, 예컨대 황산, 질산, 염산 및 수소화브롬산, 및 유기산 예컨대 아세트산, p-톨루엔술폰산, 에탄술폰산, 옥살산, 말레산, 푸마르산, 시트르산, 숙신산 및 벤조산이 있다. 이러한 염들은 또한 일반식(1)의 화합물 자체와 같이, 본 발명의 활성 성분 화합물로서 사용될 수 있다.
화합물(1) 및 이의 염은 통상의 약학 제제로 일반적으로 제형화될 수 있다. 이런 제제는 통상의 충진재, 증량제, 결합제, 보습제, 붕괴제, 계면활성제, 윤활제 등 희석제 또는 부형제를 사용하여 제조된다. 이들 약학제제는 치료 목적에 따라 각종 제형으로 선택될 수 있으며, 그 대표적 예로는 정제, 환제, 산제, 액제, 현탁제, 유제, 과립제, 캡슐제, 좌제, 주사제(액제, 현탁제 등) 및 안제가 있다.
정제 제조에 있어서는 이 분야에서 종래 공지인 각종 담체를 사용할 수 있다. 따라서, 락토스, 수크로스, 염화나트륨, 글루코스, 요소, 전분, 탄산칼슘, 카올린, 결정성 셀룰로스 및 규산 같은 전달체 또는 부형제, 물, 에탄올, 프로판올, 단시럽, 글루코스액, 전분액, 젤라틴액, 카르복시메틸셀룰로스, 쉘락, 메틸셀룰로스, 인산칼륨 및 폴리비닐피롤리돈 같은 결합제, 건조 전분, 알긴산 나트륨, 한천 분말, 라미나란 분말, 탄산수소나트륨, 탄산칼슘, 폴리옥시에틸렌 소르비탄 지방산 에스테르, 소듐 라우릴 술페이트, 스테아르산 모노글리세라이드, 전분 및 락토스 같은 붕괴제, 수크로스, 스테아린, 카카오버터 및 수소첨가유 같은 붕괴억제제, 4차 암모늄 염기 및 소듐 라우릴 술페이트 같은 흡수 촉진제, 글리세롤 및 전분 같은 보습제, 전분, 락토스, 카올린, 벤토나이트 및 콜로이드성 실리카 같은 흡착제, 및 정제 활석, 스테아르산염, 붕산 분말 및 폴리에틸렌 글리콜 같은 윤활제를 사용할 수 있다. 필요에 따라서는 정제를 통상의 제피를 수행시켜 당의정, 젤라틴 피복정, 장용피정, 필름코팅정 또는 이중정 또는 다층정으로 할 수도 있다.
환제 제조에 있어서는 당분야에 공지인 각종 담체를 사용할 수 있다. 그 예로는 글루코스, 락토스, 전분, 카카오버터, 경화식물유, 카올린 및 활석 같은 전달체 또는 부형제, 아라비아 고무 분말, 트라가칸트 고무 분말, 젤라틴 및 에탄올 같은 결합제 및, 라미나란 및 한천 같은 붕괴제가 있다.
좌제 제조에 있어서는 종래 공지인 각종 담체를 사용할 수 있다. 그 예로는 폴리에틸렌 글리콜, 카카오버터, 고급 알코올, 고급 알코올 에스테르, 젤라틴 및 반합성 글리세라이드가 있다.
주사제 제조에 있어서는 액제 또는 현탁제를 살균시키고 혈액과 등장인 것이 바람직하며 이러한 제형으로 제조하기 위해서는 이 분야에 사용되는 희석제를 모두 사용할 수 있다. 따라서, 물, 에틸 알코올, 프로필렌 글리콜, 에톡실화 이소스테아릴 알코올, 폴리옥실화 이소스테아릴 알코올 및 폴리옥시에틸렌 소르비탄 지방산 에스테르를 예로 들 수 있다. 이 경우, 약학 제제는 등장성 용액을 제공하기에 충분한 양의 염화나트륨, 글루코스 또는 글리세롤을 함유할 수 있다. 통상의 용해제, 완충제, 진정제 또는 국부 마취제 등을 첨가할 수 있다.
또한, 필요에 따라 약학 제제는 착색제, 보존제, 향료, 풍미제, 감미제 등의 기타 의약품을 함유할 수 있다.
본 발명의 약학 제제중의 활성 성분 화합물의 비율은 특별히 한정되지 않으며 광범위하게 적당히 선택될 수 있다. 그러나, 통상 약 1∼약 70중량%, 바람직하게는 약 1∼약30중량% 범위내에서 선택되는 것이 좋다.
본 발명의 약학 제제의 투여경로도 특별히 제한되지 않으며, 제형, 환자의 연령, 성별 및 기타 조건, 질환의 정도 등에 따라 선택된다. 따라서, 정제, 환제, 액제, 현탁제, 유제, 과립제 또는 캡슐제 형태로 제공시는 경구 투여한다. 주사제의 경우는 단독으로 또는 글루코스, 아미노산 등을 함유하는 통상의 비경구 주입액과의 혼합물로 정맥내 투여된다. 필요에 따라 단독으로 근육내, 피내, 피하 또는 복강내 투여될 수도 있다. 좌제는 직장 투여된다. 안제 용액은 눈에 소량식 방울로 투여된다.
본 발명의 약학 제제의 투여량은 투여방법, 환자의 연령, 성별 및 기타조건, 질환의 정도 및 기타 조건에 따라 적당하게 선택될 수 있다. 그러나, 각 활성 성분 화합물의 1일 투여량은 통상 체중 1kg당 약 0.5∼약 30mg인 것이 바람직하다. 활성 성분 화합물은 각 투여단위 형태에 약 10∼약 1,000mg의 양으로 함유되는 것이 바람직하다.
본 발명의 아폽토시스 조정제가 세포 분화 유발 및 그에 따른 다른 활성에 근거하여 효과적일 것으로 기대할 수 있는 질병은 암, AIDS, ARC(AIDS related complex), ATL(성인 T세포 백혈병), 모발세포 백혈병, 척수병(HAM/TSP), 호흡기 장애(HAB/HABA), 관절증(HAAP), 포도막염(HAU), 다른 HTLV-1(human T cell leukemia virus type I) 관련 질병, SLE(Systemic lupus erythematosus)와 같은 자기면역질병, 루마티스성 관절염(RA)과 같은 콜라겐 질병, 궤양성 결장염, 스죄그렌(Sjogren) 증상, 초기 담즙간 경변증, 자아병성 트롬보시토펜 자반(ITP), 자기면역 용혈 빈혈증, 마이스테니아 그라비스(maysthenia gravis), 하시모토(Hashimoto) 질병, 및 인슐린 의존(형태 I) 당료병 멜리터스가 있다. 또한 본 발명의 아폽토시스 조정제는 혈소판 감소증에 수반되는 다양한 질병, 예컨대 미엘로다이스플라스틱 증상, 주기적 혈소판 감소증, 아플라스틱 빈혈증, 자아병성 혈소판 감소증, 산재된 혈관내 응집등에 적용될 수 있다. 추가로, 본 발명의 조정제는 다양한 형태의 간장염(예컨대, 형태 C, A, B 및 F), 알쯔하이머 질병, 신근염, ARDS(성인 호흡기 고통증상), 감염성 질병, 간 경변증, 전립선 이상비대, 자궁근종, 기관지 천식, 동맥경화, 다양한 선천적 불구, 신장염, 노인성 백내장, 만성 피로증상, 및 근이소증에 적용될 수 있다.
본 발명의 아폽토시스 조정제는, 항암제로서 투여될 경우, 예를 들면, 암세포의 분화를 유발한 후, 부수적으로 아폽토시스 유발을 촉진하거나 억제하고, 또는 직접적으로 아폽토시스 유발을 촉진하거나 억제하므로써 항암효과를 발생시킨다. 이 경우에, 본 발명의 조성물은, 제형 및/또는 투여경로에 관계없이, 암화학치료제 및/또는 방사선 치료로서 알려진 다양한 항암제중의 어느 하나와 결합되어 사용될 수 있다. 따라서, 우수한 항암효과를 나타낼 수 있는 본 발명의 활성 성분 화합물은 혼합하여 사용된 다른 항암제(들)의 효과를 놀라울 정도로 촉진시켜 상승효과를 발휘할 수 있다. 따라서, 병용되는 다른 (상대방) 항암제(들)이 보통적인 투여량보다 상당히 소량의 양으로 사용되더라도, 만족스러운 항암효과를 얻을 수 있어, 상대방 항암제(들)의 부작용을 최소화할 수 있다. 이러한 화학치료제로서는, 예컨대, 5-플루오로우라실(5-FU : Kyowa Hakko Kogyo), 미토마이신 C(Kyowa Hakko Kogyo), 푸트라풀(FT-207 : Taiho Pharmaceutical), 엔독산(Shionogi Co.) 및 토요마이신 (Takeda Chemical Industries)을 언급할 수 있다.
혈소판 감소증의 치료에 사용될 경우, 본 발명의 아폽토시스 조정제는 세포분화 유발 촉진작용을 일으키면서 동시에 RA 또는 RARS와 같은 MDS 환자에게 아폽토시스 억제작용을 일으킬 수 있으므로, 조혈세포의 증식을 촉진하고 정상적인 분화 및 성숙을 야기시킬 수 있다. RAEB 또는 RAEB-t와 같은 MDS 환자의 경우에, 본 발명의 조정제의 투여는 아세포의 유발된 분화를 발생시키고 아세포복합을 억제할 수 있으므로써, 성숙 세포의 증식을 야기시킬 수 있다. 또한 본 발명의 조정제는 원시거대핵세포 및 거대핵세포상에 작용하여 이들의 분화 및 성숙을 촉진하여, 전구형성을 촉진시키는 것이 기대될 수 있다.
혈소판 감소증의 치료에 사용될 경우에, 본 발명의 아폽토시스 조정제는 전구형성 촉진제로서의 하나 이상의 다른 공지 약제와 배합되어 사용되므로써 이들 상대방 약제의 효과를 촉진시킬 수 있다. 따라서, 몇몇 경우에, 상대방 약제가 적당하게 감소된 투여량으로 사용될 경우조차도, 만족스러운 치료효과를 얻을 수 있으므로 이러한 약제의 부작용을 감소시킬 수 있다.
본 발명의 아폽토시스 조정제는 또한 알쯔하이머 질병의 치료 및 예방제로서 유용하다. 이 경우에, 전형적인 알쯔하이머 질병 또는 알쯔하이머 형태의 노인성 치매증의 환자에게도, 본 발명의 조성물은 아폽토시스의 억제를 통한 NGF형 작용을 보여줌으로써, 상술한 치료 및 예방 효과를 얻게 된다. 또한, 이 경우에, 본 발명의 조성물은 대뇌 순환 회복제 및 대뇌 신진대사제와 같은 알쯔하이머 질병의 통상적인 치료제중의 어느 것과 혼합하여 사용될 수 있으므로, 이들의 효과를 촉진시킬 수 있고 몇몇 경우에 이들의 부작용을 감소시킬 수 있다.
본 발명의 아폽토시스 조정제는 약제-유발 간장염 또는 바이러스성 간장염의 환자에게서 아폽토시스를 억제하는 경변증 예방제로서 사용될 수 있으므로, 간장염의 치료효과를 확장시키고 피브로겐네시스로부터 헤파토시테스를 예방할 수 있다.
본 발명의 아폽토시스 조정제의 몇가지 제형예 및 활성성분 화합물에 대한 약리적 연구결과를 하기에 기술하였다.

본 발명의 활성 성분 화합물, 아비셀, 옥수수 전분 및 마그네슘 스테아레이트를 혼합 연마하고, 생성 혼합물을 당의 R 10mm 펀치로 타정한다. 수득된 정제를 히드록시프로필메틸셀룰로스, 폴리에틸렌 글리콜 6000, 피마자유 및 메탄올로 구성된 필름코팅 조성물로 피복하여 필름코팅정을 수득한다.

본 발명의 활성 성분 화합물, 시트르산, 락토스, 인산이칼슘, 플루로닉 F-68 및 소듐 라우릴 술페이트를 혼합한다.
60호 스크린을 사용하여 크기 선택후, 혼합물을 폴리비닐피롤리돈, 카르보왁스 1500 및 카르보왁스 6000을 함유하는 알코올성 용액으로 습식 입상화한다. 필요에 따라 알코올을 가하여 분말을 페이스트상 괴로 만든다. 이어서, 옥수수 전분을 가하고 균일한 입자가 형성될 때까지 혼합을 계속한다. 혼합물을 10호 스크린을 통과시키고 트레이에 넣어 100℃ 오븐에서 12∼14시간 건조시킨다. 건조 입자를 16호 스크린으로 체질하고, 건조 소듐 라우릴 술페이트 및 건조 마그네슘 스테아레이트를 가하고, 혼합후 혼합물을 타정기를 사용하여 목적크기 및 형태로 압축한다.
상기 심부를 바니시(varnish) 처리하고 활석을 산포시켜 습기 흡수를 방지한 후 심부 주변에 하도층을 피복한다. 내복용으로 충분한 횟수로 바니시 피복을 반복 실시한다. 정제를 완전히 둥글고 평활하게 하기 위해 하도층 및 평활 피복을 추가로 실시한다. 원하는 색이 얻어질 때까지 착색 피복한다. 건조후 피복 정제를 닦아 균일한 광택의 정제를 수득한다.
후술하는 약리적 시험에서는 다음의 시험 화합물을 사용하여 수행하였다.

[약리시험예1]
[아폽토시스 조정효과]
CMK 세포를 사용하여 10% 태아 송아지 혈청으로 보충된 RPMI-1640 매질내에 1x105세포수/ml의 현탁액을 제조하고 화합물1를 30㎍/ml의 농도로 가한다. 혼합물을 37℃의 배양 플라스크내에서 2일 또는 4일간 배양시킨다. 비교용으로, 용매만을 가한 매질을 동일하게 배양시킨다.
4℃에서 10분동안 200xG 속도로 원심분리하여 세포를 거두어드린다. 혈장완충제(10mM Tris-HCl, 1mM EDTA 및 0.2% Triton x 100, pH 7.5)내에 펠렛을 용해시키고 용액을 4℃에서 10분 동안 13000 x G 속도로 원심분리한다. 상충액을 수거하고 -20℃의 50% 이소프로필 알콜-0.5M NaCl 내에서 밤새동안 방치한다. 얻어진 침전물을 4℃에서 10분 동안 13000 x G 속도로 원심분리하고, 상충액을 버리고 침전물을 건조시킨다. 건조 침전물을 10mM Tris-HCl-1mM EDTA(pH 7.4)에 재현탁시킨다. 상충액에 0.1mg/ml의 최종 농도로 RNase를 가하고 혼합물을 37℃에서 1시간동안 배양시킨다.
1x106의 세포수에 상응하는 시료에 15mM EDTA, 2% SDS, 50% 글리세롤 및 0.05% 브로모페놀 블루를 함유하는 완충제 1/5 부피를 가하고, 혼합물을 65℃에서 10분동안 배양시킨다.
이로써 얻어진 배양시료에 1.5% 아가로스겔에서 100V로 100분동안 전기영동시킨다. 얻어진 DNA는 에티디움(ethidium) 브롬화물로 염색된다.
그 결과를 제1도에 나타내었다.
제1도에서, 양쪽끝에 있는 레인은 기호표 1(120,600 및 1350bp) 및 기호표 2(72,420 및 930bp)를 각각 나타낸다. 좌측으로부터 두번째 레인은 화합물1로 처리한 2일 후의 패턴을 보여주고, 좌측으로부터 세번째 레인은 비교처리 2일후의 패턴을 보여주며, 좌측으로부터 네번째 레인은 화합물1로 처리한 4일후의 패턴을 보여주고, 좌측으로부터 다섯번째 레인은 비교처리 4일후의 패턴을 보여준다.
따라서, DNA 단편의 발현이 시간에 의존하여 전기사진 상에 나타나는 것이 분명하다. 이것은 화합물1이 CMK 세포의 아폽토시스를 촉진한다는 것을 의미한다.
[약리시험예2]
[암세포 성장억제]
[1) 인간 원시골수구 백혈병 세포(HL-60)]
인간 원시골수구 백혈병 세포 라인 HL-60은 로버트 갈로(Robert Gallo) 등에 의하여 확인된 인간 백혈병 세포라인이며 이의 전형적인 특성은 문헌 [Gallo, R. C. et al., Blood, 54, 713(1979)]에 기술되어 있다. 이는 수탁번호 ATCC No. CCL-240으로 아메리칸 타이프 컬쳐 콜렉션(ATCC)으로부터 입수할 수 있다.
HL-60세포의 농도를 10%의 FCS(태아 송아지 혈청 : GIBCO)로 보충된 RPMI-1640 매질(GIBCO)을 사용하여 1x105세포수/ml로 조정하였다.
[2) 인간 위암세포(KATO-III)]
세포농도를 (1)에서 사용된 것과 같은 매질을 사용하여 1x105세포수/ml로 조정한다.
[3) 시험 화합물 용액]
시험 화합물1을 아세트산에 용해시키고, 이어서 상기 (1)에서 사용된 것과 같은 매질로 희석시키고 2N NaOH로 중화시켜 최종농도가 100㎍/ml, 10㎍/ml 및 1.0㎍/ml로 한다.
[4) 지엠자 염색용액]
지엠자(Giemsa) 시약(Merck)을 인산염 완충액(pH 6.4)으로 50∼100배로 희석하여 지엠자 염색 용액을 제조한다.
[5) 에스테라세 염색시약]
나프톨 AS-D 클로로아세테이트 방법에 따라 에스테라세(esterase) 염색(Muto Kagaku)용 시약을 사용한다.
[6) 안티-CD33 모노클로날 항체 My9]
미성숙 과립세포와 특별하게 반응할 수 있는 모노클로날 항체 My9(Coulter)를 유리병당 0.5ml의 증류수를 가하여 용해시킨다.
[7) 결과]
(a) 상기 (3)에 기술된 바와 같이 제조된 시험 화합물 용액을 상기 (1)에 기술된 바와 같이 제조된 최종 농도가 10㎍/ml 및 1.0㎍/ml인 HL-60 세포 현탁액에 가한다. 2-웰(well) 배양판(Costar)을 사용하여, 배양을 37℃에서 유지되는 5% 이산화탄소가스 배양기내에서 3∼4일동안 수행한다. 시험화합물이 없는 매질을 비교용으로 사용한다. 이어서, 각각의 웰내에 있는 세포현탁액을 시료로하고 0.2% 트라이팬 블루-함유 인산염 완충액과 혼합하여, 현미경하에서 염색되지 않은 생육가능한 세포수를 헤아린다. 계산결과에 기준하여, 생육가능한 세포농도를 1x105세포수/ml로 재조정하여, 배양을 동일한 조건하에 계속한다.
27일 및 33일째 되는날의 세포성장 억제효과를 표 1에 나타내었다.
상기 (2)에 기술된 바와 같이 제조된 KATO-III 세포를 동일한 방법으로 배양한다. 24일 및 33일째 되는 날의 세포성장 억제효과를 표 2에 나타내었다.
표 1 및 표 2에 나타낸 데이타로부터, 종양 세포의 성장은 시험 화합물의 농도에 의존하여 억제된다는 것이 명백하다.
[(b) HL-60 세포의 분화]
상기 (1)에서와 같이 제조된 HL-60 세포를 (a)에서와 동일한 과정으로 배양한다. 27일째에 수거된 세포를 슬라이드유리상에 넣고 지엠자 염색을 행하고, 현미경하에서 형태학 관찰을 행한다. 시험 화합물을 가할 경우, 세포가 과립세포로 분화하는 것을 시험화합물이 없는 경우와 비교하여 형태학적 변화로서 관찰한다. 나프톨 AS-D 클로로아세테이트법에 의한 에스테라세 염색 결과는 70∼85%의 HL-60 세포가 과립세포로의 분화를 보여준다.
[(c) 안티-CD 33 모노클로날 항체 My9와의 반응성]
상기 (1)에서 기술된 바와 같이 제조된 HL-60 세포를 (a)와 동일한 과정으로 배양한다. 27일째에, 세포농도를 1x60 세포수/ml로 조정하고, 각각 100㎕ 엘리쿼트 (aliquot)를 10㎕의 플루오레스친(FITC)-표시된 My9와 반응시킨 다음, 유동 세포계산을 행한다. 이로써 얻어진 양성비율 데이타를 표 3에 나타내었다.
표 3에 나타낸 데이타로부터, 시험화합물을 가할 경우, 비분화된 HL-60 세포의 비율은 감소하는 반면에 분화된 세포의 비율은 증가한다.
상기 결과는 본 발명의 활성 성분 화합물은 종양세포에 대한 성장억제 및 분화 유발 효과를 갖는다는 것이 명백하다.
[약리시험예3]
[분화유발 시험]
[(8) 모노클로날 항체 FH-6]
시알릴 Le 를 인식할 수 있는 모노클로날 항체 FH-6을 플루오레스친(FITC)-표시하고, 본 발명의 활성 성분 화합물과 FH-6과의 반응성을 다음과 같이 유동 세포계산으로 조사한다.
[(9) 모노클로날 항체 FH-6과의 반응성]
상기 (3)에 기술된 바와같이 제조된 시험 화합물 용액을 상기 (1)에서와 같이 제조되고 최종농도가 1.0㎍/ml 또는 10㎍/ml인 HL-60 세포에 가한다. 배양을 37℃의 5% 이산화탄소가스 배양기내에서 2시간 동안 행한다. 이어서, 세포농도를 1x10 세포수/ml로 조정하여 각각 100㎕의 엘라쿼트를 10㎕의 플루오레스친-표시된 FH-6용액과 반응시킨 다음 유동 세포계산을 행한다. 이로써 얻어진 양성비율 데이타를 표 4에 나타내었다.
상기 결과로부터, 본 발명의 활성 성분 화합물의 첨가는 HL-60 세포표면으로부터 시알산의 손실을 야기시킨다는 것을 알 수 있다. 이것은 HL-60 세포의 분화를 의미하며 시알리다세 활성의 증가를 의미한다.
[약리시험예4]
[인간의 원시골수구 백혈병 세포(HL-60)에 대한 효과]
HL-60 세포를 37℃에서 5% CO하에 10%의 FCS로 보충된 RPMI-1640 매질에서 배양시키고, 세포농도를 5x10 세포수/ml로 조정한다.
이어서, 30㎍/ml의 각 시험 화합물을 함유하는 상기 매질을 6-웰 미세판(Costar) 각각에 가한 다음, 37℃에서 5% CO하에 3일동안의 배양을 행한다. 비교군으로서, 상기 매질만을 가하고 동일한 과정으로 배양을 수행한다. 이어서, 각각 배양된 세포 현탁액을 에펜도로프관내로 샘플링하고, 0.2% 트라이팬 블루-함유 인산염 완충액으로 염색한 후, 생육 가능한 세포를 혈구측정기를 사용하여 헤아린다.
상기 세포를 1x10 세포수/ml로 조정한다. 100㎕의 세포현탁액에 5㎕의 플루오레스친 이소티오시아네이트(FITC)-표시된 안티-휴먼 CD 11b 항체(Mol ; Coulter)의 용액 5㎕를 가하고 반응을 어두운 곳에서 빙냉하에 30분동안 진행시킨다. 이어서, 세포를 0.1%의 BSA(소 혈청 알부민 ; 시그마)를 함유하는 PBS(인산염-완충 염수 ; Nissui Pharmaceutical)로 두번 세척하고, 500㎕의 BSA-함유 PBSㅇ 최종적으로 현탁한 후 프로파일 II(Coulter)를 사용한 유동 세포계산으로 형광강도를 측정한다. 이로써 얻어진 결과를 표 5 및 6에 나타내었다.
상기 표에 나타낸 데이타로부터, 각 시험 화합물군에서, 세포성장억제 효과가 발생하는 반면에 CD 11 b의 발현이 증가하므로써, 과립세포, 단일세포 및 대식세포 계열로의 분화유발이 촉진된다는 것을 알 수 있다.
[약리시험예5]
[M12 흑색종 세포 성장억제]
이러한 시험연구는 10 Balb/c 누드마우스의 군에서 수행되었다.
시험(0일)을 행한 후 첫번째일에, 2x10 M12 세포종을 시험군에 있는 마우스에 이식시킨다. 종양부피가 100mm 에 도달할 경우, 시험 화합물1을 일일당 10mg/kg의 투여량으로 마우스에 경구투여한다. 이러한 처리를 25일째부터 55일째까지 30회 수행한다. 종양직경을 종양세포의 이식후 매일 캘리퍼스를 사용하여 측정한다. 종양부피를 다음식에 따라 계산한다.
종양부피(mm ) = (장직경) x (단 직경) x 1/2
그 결과를 제2도에 나타내었다.
제2도에는, 비교군에서는 상응하는 결과도 나타내었다. 그림에서, * p 0.05(Student's t test)에서 상당한 차이를 볼 수 있다.
제2도로부터 알 수 있는 바와같이, 시험 화합물의 투여는 M12 흑색종 세포의 성장을 상당하게 억제하였다.
[약리시험예6]
[ATL-감염세포 성장 억제]
시험 화합물로서 화합물1을 사용하여 하기의 약리시험을 행하였다.
[(a) 인간의 말초혈액 급성 임파구 백혈병 세포(Molt-4)의 제조]
인간의 말초 혈액 급성 임파구 백혈병 세포(Molt-4) [J. Minowada, et al., J. Nat1. Cancer Inst., 49, 891∼895(1972) : ATCC CRL 1582]를 사용하여 ATL 바이러스-비감염 세포로서 사용하기 위한 10% 태아 송아지 혈청(FCS)으로 보충된 RPMI-1640 매질(Gibco)중의 1x10 세포수/ml의 현탁액을 제조한다.
[(b) 인간 성인 T 세포 백혈병 유발 세포(5S)의 제조]
인간 성인 T 세포 백혈병 세포(5S)를 사용하여 ATL 바이러스-감염세포로서 사용하기 위한 10% 태아 송아지 혈청으로 보충된 RPMI-1640 매질(FCS, Gibco)중의 1x10 세포수/ml의 현탁액을 제조한다.
[(c) 시험화합물의 제조]
화합물1을 염산에 용해시키고, 상기와 동일한 매질로 희석한 후, 2N NaOH로 중화하여 1㎍/ml, 10㎍/ml 및 30㎍/ml의 농도로 조정한다.
[(d) 약리시험]
상기 (a) 및 (b)에서 제조된 세포를 24-웰 배양판의 웰내에 각각 위치시킨 다음, (c)에서 제조된 시험 화합물(최종 농도 : 1㎍/ml, 10㎍/ml 또는 30㎍/ml)을 첨가하고 5% 이산화탄소 배양기에서 37℃에서 4일동안 배양한다. 비교용으로, 시험화합물의 첨가가 없는 매질을 사용한다. 배양완결후, 각 웰내의 세포현탁액을 취하여 생육가능한 세포수를 결정한다.
그 결과를 표7에 나타내었다.
표7로 부터 알수 있는 바와같이, 본 발명의 활성 성분 화합물(화합물1)은 1㎍/ml 이상의 농도에서 ATLV-감염 세포의 성장에 대한 잠정적 억제효과를 발휘하지만, ATLV-비감염세포에는 영향을 주지 않았다.
[약리시험예7]
[안티레트로바이러스 활성시험]
(a) 인간의 변형된 정상 T 세포(MT-4) [Nagumo, T. and Hoshino, H., Jpn. J. Cancer Res. 79, 9∼11(1988)]를 사용하여 RPMI-1640 매질(Gibco) 보충 10% FCS 중의 2x10 세포수/ml의 현탁액을 제조한다.
[(b) 시험 화합물의 제조]
화합물1를 염산에 용해시키고, 상기와 동일한 매질로 희석시킨 후 2N NaOH로 중화하고 1㎍/ml, 10㎍/ml, 30㎍/ml 및 100㎍/ml의 농도로 조정한다.
[(c) 약리시험]
상기 (a)에서 제조한 세포 및 상기 (b)에서 제조한 시험 화합물(최종 농도 : 1, 10, 30 또는 100㎍/ml)을 12-웰 배양판(Costar)의 웰내에 위치시킨다. 이어서, HIV 현탁액 50㎍/well을 가하여 각 웰내의 세포를 감염시킨 다음, 5% 이산화탄소 배야기내에서 37℃로 3일동안 배양시킨다. 비교용으로, 시험 화합물을 첨가하지 않은 매질을 사용한다. 배양완결후, 각 웰내의 세포현탁액을 취하여 생육가능한 세포수를 헤아린다.
이어서 각 웰내의 세포현탁액을 원심분리(1500rpm x 5분)하고 750㎕의 배양 상충액을 얻는다.
상기에서 얻어진 750㎕의 배양 상충액을 에펜도르프관(1.8ml)에 위치시키고 4M NaCl 및 PEG-6000(Sigma)을 가한다. 혼합물을 0℃에서 2시간 동안 반응되도록 방치하여 바이러스를 침전시키고 원심분리(15000 rpm x 20분)로 바이러스를 거두어들인다. 이러한 바이러스 펠렛에 용액 I [50% 글리세롤(Wako Pure Chemical), 25mM 트리스-HCl(pH 7.5), 50mM KCl, 0.025% 트리톤 x 100(Sigma) 및 5mM DTT(Sigma)] 및 용액 II [0.9% 트리톤 x -100(Sigma) 및 1.5M KCl]를 용해 붕괴를 위하여 0℃에서 30분동안 가한다. 얻어진 10-㎕ 바이러스 용액에 5mg/ml 소 혈청 알부민(BSA, Sigma), 1M 트리스-HCl, 100mM DTT(Sigma), 1M KCl, 10mM Poly A(Sigma), 0.15mM TTP(Sigma), 200mM MgCl, 0.15mM Oligo(dT)(NEN) 및 [ H]-TTP(NEN)을 가하고 혼합물을 37℃에서 1시간 동안 방치하여 반응시킨다. 이어서 혼합물을 얼음에 넣어 반응을 중단시키고 전체량을 막 여과지(DE-81Filter, Whatman)상에 가하고 건조시킨다. 건조후, 여과지를 0.5M NaHPO로 세 번 세척하고 에탄올로 헹군 다음 건조시킨다. 막상의 DNA에 혼입된 [ H]-TTP의 방사능을 액체 신틸레이션 계수기로 측정하여 역전사효소활성(단위, cpm)을 측정한다.
상기에서 얻어진 생육가능한 세포수(x10 세포수) 및 역전사효소 활성(cpm)을 표8에 나타내었다.
표8로부터, 본 발명의 활성 성분 화합물(화합물1)은 30 및 100㎍/ml에서 세포성장을 억제할 뿐만 아니라 놀라울 정도로 역전사효소 활성을 억제한다는 것을 알수 있다.
상기 약리 시험예6 및 7의 결과는 본 발명의 활성성분 화합물로서 사용된 화합물이 높은 안티레트로바이러스 활성을 갖는다는 것을 입증한다.
[약리시험예8]
[자기면역 질병의 동물 모델에서 본 발명의 활성성분 화합물의 효과]
W/BF: (NZW x B x SB)F마우스는 자발적인 자기면역 질병을 갖는 마우스이다. 이들은 고빈도로 이상긴장 및 심근경색을 수반하는 난창 신장염의 동물모델로서 알려져 있다 [Hang, L. M., et al., J. Exp. Med., 154, 216∼221(1981)]. 이들은 나이가 증가함에 따라 항-혈소판 항체를 발현하고, 심한 혈소판 감소증을 보여주면서, 특발성 혈소판감소자반(1TP)의 동물모델로서도 사용된다 [Oyaizu, N., et al., J. Exp. Med. 167, 2017∼2077(1988)]. 보고에 따르면, 90%를 초과하는 모델 동물들이 생후 8개월내에 심근경색 또는 신장부전증으로 죽는다 [Ikehara, S. Metabolism and Disease, (Suppl) 26, 169∼176(1989)].
본 발명의 화합물1의 생명-연장 효과 및 혈액 세포와 항-혈소판 항체에 대한 동일 화합물의 효과는 이들 W/BF: (NZW x B x SB)F마우스에서 조사하였다.
14 주된 W/BF: (NZW x B x SB)F마우스(Kiwa Experimental Animal 로부터 구입)를 사용하였다. 10마리의 마우스의 군을 각 실험에서 사용하였다.
각 군의 마우스에, 이러한 마우스에게 보통 주어지는, 일상적인 음식물(Oriental Yeast) 및 물을 준다.
0.5% 카르복시메틸셀룰로스 용액(CMC, Cellogen)에 현탁시킨 화합물1(Lot No. 9D 87M)을 사용한다. 화합물1은 5-일 투여 및 2-일 중단 계획표에 따라 14주째되는 때부터 20주째되는 때까지 6주동안 100 및 300mg/체중 kg/day의 투여량으로 경구 유동식 인공영양으로 마우스에 투여된다. 비교용으로, 비처리군을 제공한다.
(1) 생명-연장효과의 조사를 위하여, 14주 내지 20주째되는 동물등의 생사여부를 관찰한다. 통계적 분석은 비처리군에 대하여 일반화된 윌콕슨(Wilcoxon) 시험에 의하여 행하여졌다. 그 결과를 제3도에 나타내었다.
제3도에서, 세로좌표는 생존율(%)을 나타내고 가로좌표는 주단위의 나이(14주째 내지 20주째)를 나타낸다.
도면으로부터 알수 있는 바와 같이, 본 발명의 화합물1의 100mg/kg/day로 투여된 군에서는 20주째될때까지 죽지 않았다. 300mg/kg/day군에서는, 두마리가 18주째에 치사된 것을 발견하였다. 비처리군에서는, 두마리, 한마리 및 세마리가 각각 17주째, 18주째 및 19주째에 치사된 것을 발견하였다. 따라서, 본 발명의 활성 성분 화합물(화합물1)의 생명-연장 효과가 자기면역 질병에 걸린 마우스 모델에서 확인되었다.
(2) 혈액세포 분석을 위하여, 혈액을 14주째, 16주째, 18주째 및 20주째되는 펀더스 오쿨리(fundus oculi)로 부터 샘플링하였다. 혈액세포(백혈구, 적혈구 및 혈소판) 수의 변화를 자동 혈액 세포 분석기(Coulter JR, Coulter)를 사용하여 결정하였다. 식별(호중구 및 임파구) 계수는 리그트-지엠자 염색으로 염색된 혈액 도말표본을 사용하여 결정하였다. 통계적 분석을 위하여, 각 혈액 샘플링 요일에 비교용으로서 비처리군을 취하는 스튜던트 T 시험을 사용하였다.
그 결과를 표9에 나타내었다.

이께하라의 상기 보고서에 기술된 바에 의하면, W/BF마우스의 혈액학적 특징은 나이가 증가함에 따라 백혈구 증다증 및 혈소판 감소증의 발달에 있다. 비처리군 및 본 발명의 화합물1이 투여된 군에서, 백혈구세포수는 나이가 증가함에 따라 증가하는 경향이 있고, 본 발명의 화합물은 이러한 인자에 영향을 주지 않는다는 것을 보여준다. 비처리 비교군에서의 혈소판 수는 14주째(1144.5 ± 121.3/mm )에 감소하기 시작하였고, 20주째에는, 14주째에 발견된 수의 약 1/5 (228.0 ± 90.0/mm )로 감소하였다. 그러나, 본 발명의 활성 성분 화합물(화합물1)의 300mg/kg/day로 투여된 군에서는, 혈소판수의 감소억제가 처리개시후 즉각적으로 나타나기 시작하였으며 20주째 (823.3 ± 24.5/mm )에서는 비처리군과 비교하여 상당한 억제를 발견하였다 (228.0 ± 90./mm ). 이와 유사한 결과가 적혈구 세포수에서도 발견되었다.
(3) 오야이즈(Oyaizu) 등은, W/BF마우스에서, 혈소판감소증은 항-혈소판 항체의 출현과 연관되어 있다고 보고하였다. 그러나, 상술한 (2)의 경우와 같이, 본 발명의 화합물1의 투여는 혈소판수에서 감소를 억제하였고, 항-혈소판 항체의 효력검정을 20주째에서 수행되었다.
20주째까지 처리된 Balb/c 마우스로부터 수거된 혈소판 및 W/BF마우스로부터 수거된 원형질을 함께 실온에서 30분동안 배양하였다. 혼합물을 두번 세척하고 FITC-표시된 안티-마우스 IgG(Tago, Code No. 6250)와 반응시키고, 염색후, 항-혈소판 항체를 FACS 분석기(EPICS-profile II, Coulter)로 효력검정을 하였다. Balb/c 마우스로부터의 원형질이 98% 음성적이기 위하여 음성적 비교군을 설정하였다. 각 시료의 양성비율을 결정하였다.
그 결과를 표 10에 나타내었다.
표에서, N. T.는 측정시에 치사되었기 때문에 측정안된 것을 의미한다. 숫자는 양성비율(%)을 나타낸다.
상기 표는 비처리된 군에서 양성비율이 19.6 ± 5.5%인 반면에, 본 발명의 활성 성분 화합물(화합물1)의 100mg/kg/day로 투여된 군에서는 13.2 ± 2.7%이고, 300mg/kg/day로 투여된 군에서는 10.7 ± 2.2%이므로, 항-혈소판 항체의 투여량-의존성 억제를 보여준다.
상술한 바와 같이, W/BF마우스는 인간의 만성적 ITP에 대한 자발적 ITP의 동물모델이다. 이러한 동물은 생후 3개월때 또는 이후에 난창신장염으로 발전하며 이들중 90%를 넘는 동물이 생후 8개월전에 심근경색 또는 신장부전증으로 죽는다. 이러한 마우스는 정상적인 Balb/c 마우스로부터 얻어진 골수를 이식하므로써 치료될 수 있다고 보고되고 있다 [IKehara, S., et al., Proc. Nat1. Acad. Sci. USA, 82, 2483∼2487(1985) ; Yasumizu, R., et al., Proc. Nat1. Acad. Sci. USA, 84, 6555∼6557(1987) ; Ikehara, S., et al., Proc. Nat1. Acad. Sci, USA, 56, 3306∼3310(1989)]. 임상적 사용에서도 역시, ITP를 위하여 오늘날 구입가능한 가장 효과적인 치료양상은 골수이식으로 고려되고 있다.
상기 (1)∼(3)의 결과로부터 알수 있는 바와 같이, 본 발명의 활성성분 화합물은 W/BF마우스에서의 상당한 생명-연장 효과를 나타내고 이러한 종류의 마우스의 혈액학적 특징인 항-혈소판 항체의 발현으로 인간 혈소판 감소증을 억제하므로, 이러한 화합물은 자기면역 질병-개선효과를 갖고 있다는 것을 의미한다.
[약리시험예9]
[혈소판감소증-개선효과]
[(1) 시험 화합물의 제조]
화합물1을 1N 염산에 용해시키고 태아 송아지 혈청(FCS, Gibco)으로 희석하여 1mg/ml 용액을 얻는다. 이 용액을 RPMI-1640 매질(Flow Laboratories) 보충 10% FCS에 가하고 혼합물을 1N NaOH로 중화시켜 사용전에 30㎍/ml의 농도로 조절한다.
[(2) CMK 세포의 제조]
CMK 세포는 티. 사또(T. Sato)등에 의하여 급성 거대핵 세포성 백혈병의 환자로부터 확인되었다. 이들은 항-혈소판 항체(안티-글리코프로테인 IIb/IIIa 항체, 안티-글리코프로테인 Ib 항체 및 P1t-1 항체)와 반응한다고 알려져 있다. 더우기, 이는 α-과립의 형성 및 원형질내의 분할막과 같은 거대핵세포형 성질을 갖는다. 따라서, 이러한 세포는 거대핵세포의 성장 및 분화 분석을 위하여 사용되어 왔다 [Sato, T., et al., Exp. Hematol., 15, 495(1987) ; Sunami, S., et al., Blood, 70, 368(1987) ; komatsu, N., et al., Blood, 74(1), 42∼48(1989) ; Fuse, A., et al., British J. haematology, 77, 32∼36(1991) ; Acta haematologia Japonica 53(2), 294∼295, 317∼318(1990)].
CMK 세포는 지바 의과대학교의 소아과의학부에 있는 다께유끼 사또박사에 의하여 공급되었다. 이러한 세포 (5x10 세포수/ml)는 RPMI-1640 매질 보충 10% FCS에서 배양되고 매 사일동안 이차배양된다.
[(3) CMK 세포의 혈소판-연관 항원의 발현에 대한 화합물1의 효과]
CMK 세포 (1x10 cells/ml)를 30㎍/ml의 화합물1을 함유하는 10% FCS-첨가 RPMI-1640 매질내에서 4일동안 배양하고 세포표면상의 혈소판-연관 항원의 발현을 FITC-표시된 Plt-1 항체(Coulter)를 사용한 유동 세포계수(Coulter)에 의하여 측정한다. 화합물1을 함유하지 않은 매질을 비교용매로서 사용된다.
세포 표면 항원의 분석을 위하여, 1x10 cells/100㎕의 CMK 세포를 시험관 (Fischer)에 취하여 5㎕의 FITC-표시된 Plt-1 항체와 4℃에서 30분동안 반응시킨다. 반응 혼합물을 0.1% 소혈청 알부민을 함유하는 인산염 완충액으로 3회 세척하고 양성세포비율(%) 및 형광강도를 측정한다.
상기의 Plt-1 항체(Coulter)는 혈소판-연관 항원, 글리코프로테인 IIb/IIIa를 인식하는 항체이며 CMK 세포의 분화 또는 성숙에 따라 증가하는 것으로 보고되었다 [Komatsu, N., et al., Blood, 74(1), 42∼48(1989)].
그 결과를 표 11에 나타내었다.
상기 표는 사용된 CMK 세포의 약 96%가 Plt-1 항체에 의하여 인식된 혈소판-연관 항원에 대하여 양성 세포임을 보여준다. 30㎍/ml의 화합물1의 첨가는 양성 세포비율에 변화를 일으키지 않았다.
그러나, 세포당 항원 발현의 양을 표시하는, 평균 플루오레신 강도의 분석은, 비교용매 세포의 평균 플루오레신 강도를 100%로 취할 경우, 30㎍/ml의 화합물1로 처리된 세포의 형광강도는 약 191%임을 보여준다. 따라서, 화합물1의 첨가는, CMK 세포의 표면상에, Plt-1 항체로 인식되는 혈소판-연관 항원의 발현에서 2배 증가를 야기하는 것이 명백하다.
[(4) CMK 세포의 형태학적 관찰]
CMK 세포(1x10 cells/ml)를 화합물1을 함유하는 10% FCS-첨가 RPMI-1640 매질내에서 4일동안 배양하여 상대조 현미경하에서 세포의 형태를 관찰한다. 세포빈도가 많은 부분을 채취하여 사이토스핀제제를 제조하고 세포형태의 측정을 위하여 리그트-지엠자 염색으로 염색한다.
그 결과를 제4도 내지 제7도에 나타내었다. 따라서, 제4도 및 제6도는 각각 비교용 매질의 상대조 현미경 사진 및 염색지도를 나타내고, 제5도 및 제7도는 30㎍/ml의 화합물1로 처리된 세포에 대한 상응하는 도면이다.
30㎍/ml의 본 발명의 화합물1로 처리된 세포에서, 상대조 현미경 사진은 혈소판 생성과정에서와 같은 세포의 단편화를 나타낸다. 염색지도는 본 발명의 화합물1로 처리된 세포내에서 세포막의 발아 및 핵의 분엽을 보여준다.
상기 결과는 카르보스티릴 유도체, 즉 본 발명의 활성 성분은 CMK 세포의 표면상에 혈소판-연관 항원의 발현을 증가시키고, 혈소판 생성과정과 유사한 세포의 단편화를 초래하며, 더우기 세포막의 발아 및 세포핵의 분엽을 야기시키는 것을 보여주고 상기 화합물은 CMK 세포의 분화 또는 성숙을 촉진시킨다는 것을 암시한다. 따라서, 화합물1은 원시거대핵세포 또는 거대핵세포상에 작용하여 이들 특징적인 작용으로 이들의 분화 및 성숙을 시킴으로써 혈소판 생성을 자극하고 혈소판감소증을 완화시킨다.
[약리시험예10]
[자기 면역 질병의 치료효과]
5마리의 MRL/MP-1pr/1pr 암컷 마우스(Charles River Japan, Inc. 로부터 7주째되는 것을 구입)의 군을 19주째되는 시점에서 사용한다.
이러한 종류의 쥐는 인간의 전신 낭창 홍반(SLE)-형 면역 콤플렉스 사구체신염, 임파종, 맥관염 및 다발성관절염과 같은 자기면역 질병으로 자발적으로 발전한다고 알려져 있다 [Theofilopolos, A. and Dixon, F. J., Adv. Immunology, 37, 269(1985) ; Murphy, E. D., Roths, J. B., Autoimmunity and Iymphoproliferation, Elsevier North Holland, New York, 207∼221(1978) ; Taniguchi, Y., Gendai Iryo, 21, 131(1989) ; Ade, C.].
화합물1-처리군 및 비교군(비처리군) 양쪽에 있는 마우스에게 마우스 다이어트(Oriental Yeast) 및 물을 준다.
0.5% 카르복시메틸셀룰로스(CMC, Dai-ichi Kogyo Seiyaku Co., Ltd.) 용액에 현탁시킨 화합물1을 사용한다.
화합물1을 5-일 투여 및 2-일 중단 계획표(화합물1-처리군)에 따라 19주째부터 25주째까지 일일당 300mg/체중 kg의 투여량으로 경구 유동식인공영양으로 투여한다.
각 군에서의 마우스에 대하여 비뇨 단백질, 임파종 직경, 이개의 맥관염, 및 모발 손실의 변화를 조사한다.
그 결과에 따르면, 화합물1-처리군은 비교군과 비교하여 비뇨단백질의 증가억제 경향을 보여주었다. 임파종 크기의 경우는, 화합물1-처리군의 직경은 7.8 ± 1.3mm인 반면에 비교군의 직경은 9.0 ± 0.8mm이므로 임파종의 성장이 약간 억제되었다는 사실을 알 수 있다.
제8도는 양족 시험군간의 맥관염, 모발손실 및 임파종의 정도를 비교한 것이다.
이 도면은 비교군과 비교하여 화합물1-처리군에서 맥관염 및 모발 손실에서 증진과 함께 임파종의 감소를 보여준다.
상기 결과는 화합물1의 투여가 이의 아폽토시스-개선 작용에 근거하여 자기면역 질병을 경감시킬 수 있다는 기대감을 갖도록 하고 있다.
[약리시험예11]
[PC12 세포에 대한 효과]
5% 열-비활성(56℃,30분)말 혈청 및 10% 태아 송아지 혈청으로 보충된 덜베코-변성-이글 매질(D-MEM)에서 이차배양된 PC12 세포를 교원질-피복 플라스틱 페트리 접시(직경 35mm)상에 6x10 cells/3ml 매질의 농도로 이식되었다. 이식 2일째에 배양매질을 화합물1, 신경 성장인자(NGF, Wako pure Chemical) 또는 FCS(비교용)의 다양한 농축물로 보충된 D-MEM으로 대체하여 배양을 추가로 계속한다. 3일째되는 날에 상대조 현미경하에 세포의 형태학적 변화를 관찰한다.
그 결과를 제9도 및 제10도에 나타내었다.
제9도에서, 상부 사진은 FCS 만을 첨가한 비교군을 나타내고 하부 사진은 30㎍/ml의 화합물1 처리군을 나타낸다. 그림으로부터 알 수 있는 바와 같이, 전형적인 형태학적 변화, 즉, 신경 섬유-형 형성과정이 상부사진과 비교하여 하부사진에서 발견되며 가장 뚜렷한 신경 섬유-형 형성과정이 FCS 0.3%(하부, 좌쪽)의 존재에서 관찰된다. FCS의 농도를 증가시켰을 경우, 다른 형상의 세포가 나타났다 (하부, 중앙 및 우쪽). 따라서, 형태학적 변화가 30∼50%의 전체 세포수에서 유발되었다.
제10도에서, 상부 사진은 NGF 단독군(그러나, 0.3% FCS 첨가됨)을 나타내고 하부사진은 30㎍/ml의 화합물1 군을 나타내다. 이러한 그림은 상부사진에서 볼 수 있는 바와같이 NGF 자극에 기인한 신경-형 형성과정과 같이, 신경-형 형성과정이 화합물1군에서도 마찬가지로 관찰됨을 보여준다. 하부 사진은 화합물1 및 NGF(그러나, 0.3% FCS 첨가됨)의 합성물이 화합물1 또는 NGF 단독과 비교하여 과정의 연장을 촉진하고 있음을 보여준다.
[약리시험예12]
[간 부전증에 대한 효과]
피, 아크네스(P. acnes)(오오사까시 의과대학교, 내과 제3실, Dr. Mizoguchi로부터 받은 선물)의 열-치사 세포를 Balb/c 수컷 마우스(Japan SLC로부터 구입한 7주째되는 것)에 3mg/체중의 투여량으로 정맥내로 투여한다. 7일후, 용매군(0.5% CMC, Dai-ichi Kotyo Seiyaku Co., Ltd) 및 비처리군 뿐만 아니라, 경구적으로 10 또는 100mg/kg의 화합물1이 투여된 시험군을 마련한다. 화합물1의 투여 한시간후, 5㎍/체중의 리포폴리사카라이드(LPS, Sigma Co., Ltd.)를 정맥내로 투여하여 급성간부전증을 유발시킨다.
LPS 투여후 7일까지 각 군에서의 마우스의 생존율을 관찰한다.
그 결과를 제11도에 나타내었다.
그림에서 보여주는 바와 같이, 생존율은 10mg/kg의 화합물1군(시험군)에 대해서는 30%이고 100mg/kg의 화합물1군(시험군)에 대해서는 70%이다. 비교용으로, 비처리군 및 용매군에 대한 생존율은 각각 0 내지 10%였다. 이러한 결과는 화합물1이 급성 간장염의 모델에서 투여량-의존성 및 우수한 생명-연장 효과를 발휘한다는 것을 의미한다.
[약리시험예13]
[폐성 병소전위의 실험모델에 대한 B16 흑색종 세포의 효과]
B16 마우스 흑색종 세포 [Koren, S. and Fleischmann, W. R., J. Interferon Research, 6, 473∼482(1986)] (2x10 세포수)를 3 또는 10㎍/ml의 화합물1이 미리 첨가되거나 되지 않은 상태로 10% FBS(Gibco)로 보충된 이글 MEM 매질(Nissui Pharm. Co., Ltd.)에 현탁시킨다. 현탁액을 37℃에서 5% 이산화탄소하의 인큐베이터(National Appliance Model 15300)내의 75-㎠ 플라스크(Corning)에서 4일 동안 배양한다. 이어서 세포를 덜베코 PBS 용액(Nissui Pharm. Co., Ltd.)으로 세척하고, 0.05% 트립신(Flow Laboratories)을 가하여 세포를 플라스크로부터 떼어놓는다.
세포를 원심분리적으로(Hitachi 05PR-22, 1200rpm, 5분) 두번 세척하고 0.2% 트라이판 블루(Wako Pure Chemical Industries Ltd.)의 첨가후, 생육가능한 세포를 혈구계산기(No. J7796, Kayagaki Works)로 헤아리고 세포를 한크 용액(Flow Laboratories)으로 5x10 cells/ml의 농도까지 희석시킨다.
상기에서 얻어진 세포를, 각각 3x10 세포수의 빈도로, 꼬리혈관을 통하여 7마리의 C57BL/6 마우스(Charles River Japan, Inc. 로부터 구입한 7주째되는 암컷)군에 이식시킨다. 12일후, 목탈구로 마우스를 치사시킨 다음 폐에 병독전이된 군체의 수를 측정한다.
그 결과를 제12도 및 하기 표 12에 나타내었다.

도면 및 표로부터 알수 있는 바와 같이, 폐성 병소전위의 명백한 억제가 3 또는 10㎍/ml의 화합물1을 함유하는 매질내에서 배양된 세포를 이식시킨 양쪽 군내에서 발견되었다.
본 발명의 아폽토시스 조정제는 아폽토시스 조정활성 및 세포 분화 유발활성의 특성에 의하여 암화학치료제로서 유용하게 사용될 수 있다. 또한 본 발명의 조정제는 이의 안티레트로 바이러스 활성에 의하여 AIDS, ARC, ATL, 다른 관련 질병 및 HTLV-I-관련 질병에 대한 치료제로서, 자기면역 질병에서 이의 생명-연장 활성에 의하여 자기면역 질병에 대한 치료제로서, 항원 관련 혈소판의 발현을 증가시키는 이의 효과에 의하여 혈소판 감소증의 치료제로서, 알쯔하이머 질병에 대한 치료제로서, 헵파토르기(hepatorgy) 내의 이의 생명-연장 활성에 의하여 간장염의 치료제로서 및 폐성 병소전위의 억제제로서 효과적이다.

- 제1항에서 정의한 하나 이상의 화합물의 유효량을 약리학적으로 허용되는 담체와 배합되어 함유하는 안티레트로바이러스제.
- 제1항에서 정의한 하나 이상의 화합물의 유효량을 약리학적으로 허용되는 담체와 배합되어 함유하는 자기면역 질병에 대한 치료제.
- 제1항에서 정의한 하나 이상의 화합물의 유효량을 약리학적으로 허용되는 담체와 배합되어 함유하는 혈소판 감소증에 대한 치료제.
- 제1항에서 정의한 하나 이상의 화합물의 유효량을 약리학적으로 허용되는 담체와 배합되어 함유하는 알쯔하이머 질병에 대한 치료제.
- 제1항에서 정의한 하나 이상의 화합물의 유효량을 약리학적으로 허용되는 담체와 배합되어 함유하는 간장염에 대한 치료제.
