본 발명은 중합체 재료, 바람직하게는 동물 몸통에 흡수 가능한 중합체로 이루어지며 작은 뼈 조각을 고정하는데 사용하는 뼈핀(bone pin)에 관한 것이다. 본 발명의 뼈핀은 뼈 혹은 뼈조각에 직접 삽입될 수 있도록 핀의 중합체 부분의 한쪽 단부에 고정된 절단(cutting) 혹은 드릴링(drilling) 장치를 갖고 있다.
뼈핀은 보통 부작용 없이 장기간 동안 동물 몸통내에 배치할 수 있는 의료용 금속으로 제조된다. 상기 금속 뼈핀은 보통 뼈를 아물게한 후 몸통으로부터 제거된다. 상기 금속 뼈핀, 특히 키르시너(Kirschner) 와이어라 불리우는 뼈핀 장착장치는 뼈를 관통하여 삽입하도록 드릴 포인트로써 사용되는 날카로운 단부를 갖고 있다.
뼈핀의 재료로써 폴리에틸렌같은 플라스틱을 사용하는 것도 제안되어 있다.
또, 몸통에 흡수 가능한 중합체 재료로된 뼈핀도 제안되어 있다. 이들 뼈핀은 풀리글리콜라이드(polyglycolide) 또는 폴리락티드(polylactide) 중합체 혹은 공중합체(copolymer) 또는 글리콜라이드 및 락티드나 폴리- 디옥사논(poly-dioxanone) 혹은 다른 흡수 가능한 중합체로 제조할 수 있다. 미합중국 특허 제4,052,988호에 기술된 폴리-디옥사논으로된 뼈핀은 한때 상업적으로 입수 가능한 것이었다.
폴리-디옥사논 뼈핀은, 뼈조각을 통해 온전한 뼈내로 또는 서로 유지되야 하는 인접한 2개의 뼈조각들 사이 또는 그들을 관통하도록 홀을 드릴링 하여 사용된다.
적절한 직경의 구멍이 뼈를 통하여 드릴된후, 상기 드릴은 제거되고 폴리-디옥사논핀이 상기홀을 통해 삽입되며, 뼈의 표면 너머로 연장된 핀부분은 매스나 다른 기구로 절단하여 제거된다.
상기 절차가 갖는 문제점으로 뼈를 통해 초기에 구멍을 드릴할 때 배열되었던 뼈조각들이, 핀을 삽입하기 위해 드릴을 제거한 후에는 배열이 흐뜨러져 핀의 올바른 삽입을 어렵게 한다는 점이다.
본 발명은 중합체 핀에 부착된 드릴 포인트를 갖는 중합체 뼈핀을 제공하는 것이다. 본 발명은 핀이 뼈의 한쪽 표면으로부터 대향 표면으로 완전히 연장하는 곳에서 그 절치를 특히 유효하게 한다. 본 발명의 뼈핀을 중공 드릴을 사용하여 위치될 수 있다. 상기 핀의 드릴 포인트는 서로 결합될 뼈를 통하여 드릴된다. 상기 드릴 포인트는 뼈의 말단부 표면을 관통하여 연장된다. 그런다음, 상기 핀은 뼈를 통하여 눌러지게 되며 뼈 표면을 지나서 연장된 드릴 포인트와 핀의 중합체 부분은 제거된다. 본 발명의 뼈핀을 사용하는 상기 절차는 상기 핀이 뼈를 통하여 드릴 포인트 뒤에서 직접 삽입되기 때문에 뼈 조각의 배열이 흐트러진 문제를 해결한다.
제1도는 본 발명의 사용법을 도시하였다. 핀은 뼈(10)의 부분을 함께 고착하는데 사용한다. 핀(11)은 드릴 부분(13)이 부착된 중합부(12)를 포함한다. 드릴부분은 드릴 포인트(14)를 가지며 접착제 및 다른 방법과 함께 스웨이징(swaging)에 의해 중합 부분에 부착된다.
핀의 중합 부분에 사용되는 바람직한 비흡수성 중합체로는 폴리에틸렌이 있으며, 바람직한 흡수성 중합체로는 미합중국 특허 제4,052,988호에 기술된 폴리-디옥사논이다.
상기 핀의 중합 부분은 1mm의 길이당 0.005 내지 0.05mm로 테이퍼된다.
핀(11)은 약 100내지 200mm의 길이를 갖는다. 핀의 중합 부분은 약 50내지 100mm의 길이를 가지며, 장치의 절단 부분은 약 50내지 100mm의 길이를 갖는다.
절단 장치는 가장 작은 직경을 갖는 핀의 중합 부분의 단부에 부착된다. 상기 장치의 중합 부분은 스웨이징(swaging)에 의해 절단 부분에 고정되거나 에폭시 또는 다른 적합한 시멘트로 상기 두부분을 함께 시멘팅 함으로써 연결핀과 함께 고정되거나 또는 상기 공정을 조합하여 고정될 수 있다. 절단 부분은 장치의 흡수 가능한 부분을 수용하는 와이어의 뒤에서 드릴된 구멍을 갖는 한 조각의 키르시어 와이어일 수 있다. 장치의 절단부분의 드릴링 포인트(14)는 중공 외과용 드릴을 사용하면 뼈에 구명을 낼 수 있다. 중공 외과용 드릴을 사용함에 있어서, 핀의 절단부분은 드릴 척으로 고정되며, 핀의 중합체 부분은 척의 후방까지 드릴 몸체로 연장된다. 도면에 도시되었듯이, 장치의 흡수 가능 부분은 1mm 길이당 약 0.005mm 내지 0.05mm의 테이퍼를 갖는다. 테이퍼 mil 길이당 직경의 차이다. 장치의 절단부분은 장치의 중합체 부분의 최소 직경보다 크지만 중합체 부분의 최대 직경보다는 크지 않은 직경을 갖는다는 점에 주의해야 한다. 핀의 테이퍼에 의하여, 핀은 뼈에 드릴된 구멍으로 점차적으로 들어갈 수 있다. 결국에는 핀의 직경은 뼈의 구멍보다 크거나 같으며, 핀을 뼈속에 고정하도록 뼈에 꼭 맞어 들어간다.
핀이 자리잡고 난후, 제3도처럼 뼈밖으로 뻗은 핀 부분을 메스나 다른 적당한 절단 기구로 잘라내어 핀을 뼈크기와 동일하게 한다.

Claims (5)

  1. 중합체 재료로 된 원통형 부분과 이 중합체 부분의 한 단부에 부착된 절단 부분을 포함하는 삽입 가능한 핀을 구비하며, 상기 핀의 중합체 부분이 1mm 길이당 0.005에서 0.05mm 의 테이퍼를 갖는 것을 특징으로 하는 뼈핀.
  2. 중합체 재료로 된 원통형 부분과 이 중합체 부분의 한 단부에 부착된 절단부분을 포함하는 삽입가능한 핀을 구비하는 뼈핀으로서, 상기 핀의 중합체 부분이 1mm 길이당 0.005에서 0.05mm 테이퍼를 가지며, 상기 핀의 중합체 부분은 동물 몸체에 흡수 가능한 중합체로 만들어진 것을 특징으로 하는 뼈핀.
  3. 제1항에 있어서, 상기 절단 부분 포인트가 금속인 것을 특징으로 하는 뼈핀.
  4. 제2항에 있어서, 상기 핀의 중합체 부분이 절단 부분에 스웨이징되어 있는 것을 특징으로 하는 뼈핀.
  5. 제2항에 있어서, 흡수 가능한 중합체가 폴리-디옥사논인 것을 특징으로 하는 뼈핀.
