Description

【発明の詳細な説明】 本発明は移植により関節軟骨の欠陥と骨の欠陥とを修復
するに使用するための組)a物に関するものである。本
発明の組成物はそうした修復用に使用できる。それらは
、それとして、または天然または人工骨をキャリヤとし
て一諸にして、ゲルの形で提供される。以下の記述は主
として関節軟骨の修復に関連しているが、これは最も問
題であり繊細な生物学構造であるからである。本発明の
新規な組成物は、軟骨形成誘因因子の影響により潜在的
に軟骨細胞に変換されうるところの萌芽性の軟骨細胞な
いし成る間葉繊細胞を組合せて含んで居り、細胞は適量
の線維素片、トロンビン、及びアンチプロテアーゼを組
合せて膠として役立つように作られている。
結果としてできるゲルは組織培養貯蔵条件下に長期間に
渉って貯蔵でき、ト匈送でき、容易に取扱いできる。
本発明の好ましい態様によれば、細胞外基質(ECM 
) 、または、SM 、 FGF 、 CGF 、 B
DGFの如き成る生長ファクターまたはこれらの何れか
の4ra合せをそうしたゲル中に内蔵させられうる。
骨の成る欠陥の修復のためには、そうしたゲルで満して
適当な形状にした骨部祠を含む夢植物を使用しうる。
本発明の他の更に別の特徴は今後明かとなるであろう。
関節軟骨が外傷、感染ないし退化性操作によって損ぜら
れると、そうした損傷は一般に治し得す、改良さえもで
きない。これまで、骨軟骨植接にたよることや、種々の
形の補綴を用意することに種々の試みが行われて来たが
、長期的結果は貧弱であり落胆すべきものであった。軟
骨移□植の源泉として培養した軟骨細胞を使用する試み
も報告されたが移植物と隣接する軟骨との一体化は一般
に不満足であった。
本発明によれば、直ちに使用可能な組成物が提供され、
萌芽性の軟骨細胞または、間葉織起源の細胞、何等かの
ifs[のもので、軟骨形成誘因ファクターの影Uによ
り、試P倹管内または生体内で軟骨細胞へ変換されつる
もので、゛ゲル″を形成する適当な生物学的媒体中で大
人の関節軟骨及び骨への損傷の修理に対して、損傷の場
所への移植により行・うのに使用しうるものが提案され
ている。また、そうした移植可能材料の生産用の操作も
また提案されている。
また、軟骨の欠陥の修理に使用する組成物も提案されて
いる。その8M物は、適当な萌芽性の組織の機械的破砕
と共にトリプシン化による萌芽性軟骨細胞(若さを留め
た軟骨細胞)の遊離で作り、ついで適当な媒質上で培養
し、細胞を収穫し、それらを線維素片、アンチプロテア
ーゼ及びトロンビンを加えてその結果として、培養器中
で限られた時間の期間貯蔵ができるゲルを得、または、
収穫された細胞が望むゲルへ使用する前に深冷及び氷解
によって長期間保存ざnうるようになるようなゲルを得
る。ゲルは、欠陥を満すようにトロンビン溶液を噴霧し
た移植の場所に適用するtjijに線維素片の溶液中に
浸されている。動物実験(鳥類及び哺乳類種につき)に
於て、軟骨と骨との欠陥は2乃至12ケ月の4v]川]
後の検証したとところでは、切筋損傷の侶れた修復を示
して居て、正しく満されたことが示された。
本発EI+]の組成物は、外傷ないし老齢のせいでの人
ILJ関変軟骨または骨の損傷の修復に価値がある。そ
うした損傷は4fi々の型の軟骨及び骨で、接合部の退
化性傷吾や破砕を含んでいる。
本発明の好ましい態様によれば、本発明の新規な処方は
、以下に於て開示される如くに特殊技法で調製される萌
芽性軟骨細胞の或:(1の細、抱外i、質(ECM )
を含んでいる。
ECMを設けることで、新規なaK物が植付けられた時
に軟骨細胞の成長を高めるが、これらが望む天然環境に
似ている事が最も確からしい。
軟骨細胞は、移植される材料が適用きるべき種の適当な
萌芽性置端m織から得られる。異質形成細胞は、HLA
タイプ化にたよる必要なしに全く満足な結果を与える。
人間での使用に対しては、人間萌芽性軟骨細胞が培呑さ
れた。
毎単位容積当りの軟骨細胞数は成る値を超えないもので
あるべきである。それでないと、IB胞の類壊死が生じ
、より劣る結果が得られる。
軟骨細胞濃度の代表的値は、ゲルの毎ミリリットル当り
約100000から500000である。毎ミリリット
ル当り約5から50単位のトロンビンと約25から80

される。一般にプロテアーゼ抑制剤を、ゲルの速い溶解
を防ぐか、抑えるため組成物へ加える。天然または合成
プロテアーゼ抑制剤を使ってもよい。適切なプロテアー
ゼ抑制剤は、ゲルの約10から20■/ meの量で使
用されるε−アミ/カプロイン酸、ゲルの約1から2m
’i / 7の量で使用されるトランエキゼミン酸の如
きである。アンチ・トリプシン(チキン卵白、シグマ、

抑制剤を使ってもよい。ゲルの遅い形成が望まれる時は

きる。本発明によるゲルは、直ちに使用でき、下記に示
す如くに、損傷された、または、欠陥個所に直接に適用
しうる。
成る修復に対しては、トロンビン、アンチグロテアーゼ
、線維葉片及び一つ以上の成長ファクターおよび/また
は上に鵡(定された型の細胞の小濃度を伴うか、または
、そうした細胞なしでさえも、ホルモンから成るゲルで
満した骨構造(天然または人工)から+mる移イ1)を
利用することが推奨される。後者の場合、棋llからの
細胞が移植内に徐々に滲透し、環」〕)と移植との間に
凝集牲一体物を形成する。
本発明を下記の例を参J’−t;4 シて描写するが、
これは非制限的な具合に解釈ざるべきものである。
例1:軟骨修復用廻成物の記]製 出発材料として長い骨(脛骨、大屁骨)の置端を使用し

萌芽性軟骨細胞(若さを留めた軟骨細fril )の遊

芸天(Ham −F −12含有0.6%寒天)を塗布

秋骨約胞九1殖が巳る。炊・く寒天板からの細8.4は
更にスピンナー曇内のF −t 2の、ニー!:、濁培
養内にもりされ、更に310L−1の明ijJ生長させ
られる。スピンナーl云内の、犬長ずた件は再び線惟芽
細胞よりも款骨絢゛個を好んで支持する。スピンナー9
からの細胞は遠心分に9により採取され、直」灰に映用
されるか、または、液体膝紫中にて長明間冷凍保存され
る(20%胎児子牛血清、10%ジメチルスルフォキサ
イド(IIMSO)及び7o%r−12)。
出つ:り上った軟骨ゴ硼iI凪のペレットは、41・色
素、原(b Ox7g7 me ’)または回前なる他
の血1管凝精蛋白及ヒl−!Jプシン抑制剤(50μ/
m(+シグマ49. !II )式たは他のアンチグロ
テアーゼを含む小容遺の燐酸塩緩衝塩水中に再懸濁され
る。
細胞(培養の期間と大きさとに従って涙度が1−5X1
05細胞/mlの[t、1に渉っている)と、線維素片
とトリプシン抑制剤とを含んでいる最後の溶液を溶液A
とする。ミクロテスト板(蓋付き95FNuncデンマ
二り)才たは、炙岐井戸に、トロンビン溶液(溶液B)
 (40mMcac12のI U / 30 me )
の30マイクロリツトルを各井戸へ加え(底及び側壁上
に平均に分布して)、それから溶液Aを90マイクロリ
ットル加え、そして混合物を固化(ゲル化)させる。
需要に従って、最後の母は貧化出来、かくて、溶液Aと
Bの比を3:1(v/v)に保つ。F−12の0.2 
tr=eで蔽ったゲル(Jそれから或期間(約14日)
、5−10%C02を空気に入れて平衡化した培養器中
か、または冷凍保存されて、保たれることができる。
移植では、損傷した場所をトロンビン溶液で繊細に噴霧
し、他方、移植に先立つゲルは線維素片とトリプシン抑
制剤を含む(それぞれ50yv / r;tl及び50
μg/ gに)溶液中に浸漬し、そし・て試料を損傷し
た場所に圧入し欠陥を滑らかに読す。
萌芽性軟骨細胞の代りに、萌芽性軟骨細胞(段階24)
また:ま青嵐・痒辿胞を使ってもよい。
加うるに、トラップされた非差異化間葉繊細胞をつけた
11ij何なる成人運結狙゛雁を使用してもよく、培、
連中に細胞を発生し、それが最炭に自己差A化により軟
骨細胞に変形され、または軟骨jli l泡ファクター
により指向される。
先天性移毛に対しては、受1四トドの欣人徂織内にトラ
ップされた同質江聞葉獄型1.!■胞で、皮膚、1.λ
堆芽′酊珂包、生倹により得られる骨髄、’:’ill
胞の叩きを・吏ってもよ(、これらは、培養中をこ細、
泡を生じ、それらが軟骨?iIl胞に変セ1さiする。
スピンナー壜(14−21日培;詭中)からの萌芽性ひ
よこ軟骨細胞を、2X105a胞/35心皿のm期濃度
で板にし、フィブロネクチンを塗布した皿上にアスコル
ビン酸(50μf/ml)で補足したF−12媒質中に
維持した。培養らは6日以内に合流になり、それから!
柴3”tを更新し、また培養を更に追加の6日間培養し
た。三組の培養の組はトリプシン化され、Can1te
r計数器(モデルエ蟇D)により細胞計数を調べる為に
使用した。末9;1tでの細胞計数はlX106i:Q
胞/板の周りの範囲であった。総ての他の板をそれから
燐酸緩箭塩水(PBS ”lで洗浄し、20分間蒸溜水
中で20 m M N H40Hに::i、r した。
そして3回PBSで再洗滌I7、溜水水で2 [ii 
;ニアと1゛、4洗滌し、細胞体質ないし核が、皿に倹
7′IデシたもとのままのECMと組合されて認められ
ることがないようにした。ECMはゴムポリスマンで(
:足長して洟δ霜真空乾燥した。乾燥粉末化ECMの収
AXは0.3−1.0即/板の間の範囲であった。
ECMは5乃至20巧毎リツトルのがで混ぜ合せた。最
良の結果は約10πi’ / fneで得られた。
軟骨細胞ゲルを種々の坪(鳥類及び咄乳頬)内の移植と
して実験し、巨視的篤察、相1”;I’に学的断面及び
生化学試験により採集したデータは、移植の場所に於て
、二三ケ月以内に、欠陥は活性の増殖する軟骨細胞で正
しく満され(関節軟骨の表面を含め゛C,環境にシ:j
歪されてう、特徴的な(表現型の)代謝性体質と古いし
・“−接軟骨組織と積胆った井戸(線維軟骨、才たは他
の軟かい組織かヘリに無い)とを表しでいた。

Claims (1)

  1. 【特許請求の範囲】 1、 移植によって関節の軟骨および骨の欠陥を修復す
    るためのものであって、それのH=物は、その組成物が
    移植されるべき楢の萌芽性軟骨細胞または間葉繊細胞で
    、線維累原、トロンビン、−及び天然ないし化学的プロ
    テアーゼ防止剤の成る量とから成る生物学的膠による軟
    骨形成誘因因子により軟骨細胞に変換されうるものを含
    んでいるところの、組成物。 2、 毎ミリリットルごとに、100000がら500
    000個の軟骨細胞または間M繊細路を含んでいるとこ
    ろの、特許請求の範囲jHI項記載の組成物。 3.5乃至20〜/lnlの細胞外基質を含んでいると
    ころの、特許請求の範囲第1項記載の組成物。 4、局部的生長ないしホルモン生長因子剤を含んでいる
    ところの、特許請求の範囲第1項記載の組成物。 5、局部的生長因子が軟骨起源生長因子(CDF)また
    は骨起源生長因子(BDGF )であるところの、特許
    請求の範囲第4項記載の組成物。 6、 ホルモン性成長因子が血清または下垂体からのン
    マトメジンであるところの、特許請求の範囲第4項記載
    の組成物。 7、細胞が軟骨細胞または未分化間葉織捕捉細飽を含む
    結ケ組織から導出された細胞、培養中に軟骨細胞になる
    ように増殖し変換されるよう誘導される細胞であるとこ
    ろの、特許請求の範囲第1項記載の組成物。 8、 ゲルが、適当な形状にしlた親水化された(キー
    ル骨)または人工的骨中に包埋されているところのζ特
    許請求の範囲第7項記載の組成物。 9、 特許請求の範囲第1項記載の組成物を欠陥組織内
    に植付けることを含むところの、管端軟骨及び骨修復の
    操作。 10、細胞外基i (FC! >または適当な局部的な
    いしホルモン性成長型因子で、特許請求の範囲第1項記
    載の細胞の少量を含むか、またはそうした細胞を欠いて
    いるものと共に、トロンビン、アンチプロテアーゼ及び
    線維素片とから成る生物学的ゲルで含浸された適当に形
    状づけられた骨構造を含むところの、軟骨ないし骨修復
    用の移植。
