発明の分野

び生化学の技術分野に属する。それは、in vitro で培養された軟骨細胞の使用
を伴い及び伴わない、軟骨損傷の修復のために用いる製品及び方法の両方である
。
発明の背景
傷ついた関節軟骨が限られた自己修復能力しか有さないことは、文献に十分に
報告されている。関節軟骨は相対的に無血管でありかつ無神経鞘(aneural)な
ので、表面組織の損失は永久的な瘢痕部位をもたらすことになる。より大きな血
管供給を有する軟骨下骨を骨折する損傷は、損なわれた部位を線維質軟骨組織で
充填しながら炎症/修復応答を行うことになる(Convery ら,1972)。いずれの
場合も、軟骨の生化学的及び生体機械的特徴が変化するにつれて、機能が害され
そして慢性的な痛みが普通の予後となる。最近の治療手順は、外科手術(研磨関

された(Sokoloffら,1978)。この可能性が示されたので、軟骨損傷を治癒する
ための自家移植又は異系移植組織及び組織類似物の研究が発展してきた。
1)骨軟骨移植片(DePalma ら,1963);2)軟骨細胞(Grandeら,1989);
3)骨膜(Hommingaら,1990);及び4)無機質脱落骨(Dah1berg と Kreicber
gs,1991)の移植の如き、自家組織を用いる技術が開発された。これら技術は、
全体関節又は部分関節を移植するために用いられてきた。例えば、幾人かの研究
者は、骨端プレートから分離された軟骨細胞並びに関節細胞を用い、これら細胞
がその高められた代謝によってより大きな成功の機会を有するであろうという仮
説の下に、軟骨欠損症を治癒させようと試みた(Itayら,1987)。培養した細胞
を用いる臨床的検討で、術後2〜4年後にかなりの痛みの減少と正常機能の回復
を示すという優れた結果が報告された(Hoika ら,1990;Hommingaら,1990)。

用いて軟骨欠損を効果的に修復した。これらアプローチは、未充填部位又は材料
だけで充填された部位のいずれよりも正常な軟骨に近似した修復組織を与えたが
、かなりの量の線維質軟骨形成があることが明らかになった。
今日まで、関節軟骨修復の成功が限られていたのは、部分的には内部に外科的
に固定する適当な方法を欠いていたためである。Grandeら,1989 により用いら
れた初期の技術は、骨膜弁を縫い込むことを必要とする。骨膜片を脛骨基部から
新たに分離して、9-0 Vicryl 縫合糸で膝蓋骨欠損部の末梢に縫い付けた。この
技術は、外科的に煩雑で、欠損部の大きさや配置に関して限界があることが分か
った。幾人かの専門化は、インプラントを軟骨欠損部に固定するためにプレスフ
ィット(Press-fit)技術を用いた(例えば、Couttsら,1994;Freed ら,1993
;Nixon ら,1994)。この技術は、体重を支えない条件で有効であり得るが、イ
ンプラントは動いている間に、特に移植後の最初の2週間の間にずれる可能性が
ある。加えて、プレスフィット技術を用いて関節軟骨表面に関してインプラント
の高さを調節するには、その融通性に限界がある。
発明の目的
従って、この発明の一般的な目的は、軟骨損傷を修復するための、この先行技
術の欠点を克服する多段階コラーゲンインプラントを固定する方法を提供するこ
とである。
この発明の更なる目的は、軟骨損傷を修復するための効果的で安全な多段階コ
ラーゲンインプラントを固定する方法を提供することである。
この発明のもう一つの目的は、軟骨損傷を修復するための吸収性成分を有する
多段階コラーゲンインプラントを固定する方法を提供することである。
発明の要旨
軟骨修復テンプレートを軟骨欠損部位に保持させるためにある取り付け方法が
用いられる。この方法は、縫合糸を軟骨下プレートを通して骨性組織の中にアン
カーで固定し、2本又はそれを越える糸をその表面から出現させることからなる
。アンカーで固定されたそれら縫合糸は、軟骨修復テンプレートを軟骨欠損部位
に固定するために用いられる。
図面の説明
この発明の他の目的及び多くの付随する特徴は、添付の図面と関連付けて考慮
すれば以下の詳細な説明を参照することによってより十分に理解されるので、す
ぐに分かるであろう。
図1は、正常な軟骨と欠損又は損傷部位4の解剖学的構造を示し、次の符号が
付されている:
1は関節表面を表し、2はヒアリン軟骨を表し、3は網状骨及び骨髄を表し、
4は欠損部を表し、5は細胞外マトリックスを表し、6は軟骨細胞を表し、7は
基準線を表し、8は石灰化軟骨を表し、そして9は軟骨下プレートを表す。
図2は、欠損部位4において網状骨3の中に埋め込まれた縫合糸10が付いた
アンカー11を示す。
図3は、多孔質コラーゲンマトリックス13と密なコラーゲンメンブラン14
を有するコラーゲンテンプレート12の、そのテンプレート12にアンカーの付
いた縫合糸10を通す際の位置関係を示す。
図4は、アンカーの付いた縫合糸10で上部保護層15及びテンプレート12
を固定する際のその上部保護層15の位置関係を示す。
好ましい態様の説明
アンカーで固定する本発明の利点は、イヌモデルにおける負荷を支える条件下
での軟骨修復で証明されるように、インプラント12を軟骨再生の部位4に固定
して移動を阻止する点である。移動の阻止は重要である。というのは、損傷部位
において必要となるインプラントの組織再生局面のためだけでなく、関節の生体
力学が、関節の滑らかな動きを部分的又は完全に妨げる物体により崩壊してしま
わないからである。本発明のもう1つの利点は、治療される欠損症のプレスフィ
ットサイズ限界を克服する点である。
軟質組織、特に関節の周り及び関節内の筋肉、靭帯、及び鍵を骨に固定するた
めに、これまでアンカーが用いられてきた。それらアンカーは、生体適合性の金
属及びプラスチックから作られ、種々の大きさのものが入手可能である。損傷部
位内にアンカーを入れる前に、縫合材料をアンカーの遠位面に固定できることが
分かった。ドリル穴を欠損部のほぼ中央に作る。次いで、アンカーをその作った
部位にセットして、縫合糸が関節表面から自由に伸びるようにする。発明を使用する例
1.軟骨修復インプラント又はテンプレート12の大きさより僅かに小さな
外科的に画成された部位4を作る。
2.欠損部位4のほぼ中央にほぼ大きさを合わせた穴(例えば、直径1mm
)をあける。その穴は、アンカー11が正しく収まりかつ縫合糸10がそれを通
れるような十分な直径のものであるべきである。
3.適切な本数の縫合糸10、例えば、2本の縫合糸をアンカー11(例え
ば、Mitek Surgical Products 社,Norwood により製品番号 Mitek No.210193G
-11 MINI ANCHOR として販売されている製品であって、米国特許第4,898,1
56号及び4,946,468号に開示されている製品;なお、この特許の全開示
内容は、参照により本明細書中に組み入れられるものとする)に取り付け、等し
い大きさのストランド、例えば4本がアンカー11の遠位面から出てくるように
する。縫合糸10は生体吸収性であり、かつサイズが 5-0 又はそれ未満のもの
であるべきである。
4.アンカー11をドリル穴の中に入れ、これら吸収性縫合糸ストランド1
0、例えば4本が外科的に作った部位の中央から出てくるようにする(図2)。
5.縫合糸を軟骨修復インプラント12を通して引っ張る(図3)。
6.縫合糸をくくって、結び目がインプラント表面の下に又はもし使うなら
保護カバーの下に隠れて、関節表面1に露出しないようにする(図4)。
関節軟骨の欠損症は、「軟骨損傷の修復に用いるための多段階コラーゲン基剤
テンプレート又はインプラント」という名称の本出願人の出願に記載した、多孔
質コラーゲンスポンジ(コラーゲンマトリックス)を密なコラーゲンメンブラン
と組み合わせることにより形成された再生テンプレートを用いることによって治
癒することができる。なお、その全開示内容は、参照により本明細書中に組み入
れられるものとする。密なコラーゲンマトリックスは、1マイクロメーター未満
の気孔サイズで作られ、そして強度を増しかつ吸収時間を長引かせるために非細
胞毒性物質で架橋されている。この密なコラーゲンメンブランの故に、本発明は
、軟骨下プレートを横断する欠損症を含む全層性欠損症に用いることができる。
密なコラーゲンメンブランは、軟骨欠損部の表面に載せられ、軟骨下プレート及
び血管系からの細胞移動を阻止する。このコラーゲンメンブランは、軟骨再生に
必要な体液、栄養、サイトカイン及び他の因子の移入及び交換を可能にする。テ
ンプレート12の部材の厚さは、使用する状況に依存して変動させることができ
る。例えば、密なコラーゲンメンブランの厚さは、50〜200マイクロメータ
ー又はそれを越えてもよく、多孔質コラーゲンマトリックスの厚さは、0.5〜
8ミリメーター又はそれを越えてもよい。
本テンプレートのコラーゲンマトリックス部材は、細胞、特に軟骨細胞の付着
及び増殖を可能にするために開発された。in vitro 検討を用いてテンプレート
の多孔質コラーゲンマトリックス部材の最適な気孔サイズを決定した。コラーゲ
ンマトリックスは、in vitro で軟骨細胞を不動化してその後の細胞増殖を支援
するために用いることができる。次いで、細胞数が in vitro で拡大すると、コ
ラーゲンマトリックスは、それら細胞を修復部位に運搬して移植に服する位置に
保持する。
これまでの検討で、コラーゲンマトリックスの気孔サイズは、分散pH、コラ
ーゲン濃度及び凍結乾燥サイクル(凍結時間、温度範囲、及びサイクル時間)を
変動させることによってコントロールできることが示された(Dillion ら,1986
)。コラーゲンマトリックスは、球状高分子に対するそれらの透過性によっても
特徴付けられてきた。例えば、約15マイクロメーターの気孔構造は106ダル
ト
ンより大きな分子を排除する。密なコラーゲンメンブランは、7×104ダルト
ンの分子量排除性を有する(liら,1987)。平均マトリックス気孔サイズの細胞
増殖速度への効果を確認するために、軟骨細胞を種々の気孔構造のI型コラーゲ
ンマトリックス上で増殖させた。気孔構造は、12日後の細胞増殖の速度に影響
しないことが分かった。しかしながら、軟骨細胞浸潤は、100マイクロメータ
ーよりも大きな平均気孔サイズについてより大きかった。線維芽細胞を用いる平
行検討で、類似の細胞増殖結果が示された。密なコラーゲンメンブラン上での線
維芽細胞の増殖速度は、多孔質マトリックスとほぼ同じであったが、そのメンブ
ランを通過する細胞の移動は排除された(Pachenceら,1991)ということに注目
することが重要である。
密なコラーゲンメンブランは、1)体内吸収性縫合糸;又は2)密なコラーゲ
ンメンブランが形成中にコラーゲンマトリックス内に組み入れられることを要す
る融合技術;を用いて、細胞培養前又は移植前にコラーゲンマトリックスに取り
付けることができる。
一連の in vivo 検討を通して、軟骨細胞を添加し及び添加していないテンプ
レートがウサギモデルに外科手術で誘発した軟骨損傷のある全層性欠損症の治癒
を促進することが示された。軟骨細胞播種テンプレートは、取り出したインプラ
ント−組織部位の組織学的、生化学的、及び機械的分析の使用を通して、ヒアリ
ン軟骨であると考えられる修復組織をもたらすことが明らかになった。軟骨欠損
部におけるテンプレートの方向は、成功を収めるために重要である。密な層を欠
損部内の“下の方に”配置して、骨に接触させ、そして多孔質層を天然軟骨の平
面内に横たわらせる。密な層は、線維質軟骨の形成を阻害することが実験的に示
された。
更に説明しなくても、上に述べたことが本発明を十分に説明しているであろう
。当業者は、現時点での又は将来の知見を応用することによって、本発明を種々
の役務の条件下で用いるために適合させることができる。

Claims (1)
