Description
本質的に米国特許第6,120,477号明細書(Campbell他)に教示されている方法を用いて、構造カバーを調製した。20層の高フィブリル化された5ミクロン厚のePTFEフィルム(Bacinoの米国特許第5,476,589号明細書)をチューブ軸線に対して83.4°の角度を成して螺旋状に7mmのステンレス鋼マンドレルに巻き付けることによって、フィルムチューブを形成した。10層のePTFEを一方の方向に巻き付け、10層を反対方向に巻き付けた。マンドレルを炉装置内で、380℃で6分間にわたって焼成することにより層を融合させた。その結果生じたチューブをマンドレルから取り外し、そして、2.2mm未満の直径まで「ネッキングした」(延伸させた)。ネッキングしたこのチューブを2.2mmのステンレス鋼マンドレルに置き、約5層の犠牲ePTFEフィルムを上から巻き付けることにより、後続工程でチューブに皺が生じるのを防止した。次に、チューブ集成体を元の長さの約65%まで均一に圧縮した。集成体を380℃の炉装置内に1分間にわたって置き、続いて犠牲ePTFE層を除去した。この集成体をマンドレルから取り外し、そして、65.0mm長に切断した。別の実施態様では、この構造層は下側に位置するバルーンの再圧密化を支援するためにエラストマーを含んでもよい(例えばCampbell他の米国特許第6,120,477号明細書を参照)。
セミコンプライアント型のバルーンカテーテルをBavaria Medizin Technologie, Oberpfaffenhofen, Germanyから購入した(モデル番号BMT-035、品番08PL-604A、バルーン寸法6.0mm×40mm)。バルーンの仕様は以下の通りである:公称膨張圧力6気圧(atm)、定格破裂圧力14atm、公称直径6mm、バルーン作業長40mmのナイロンバルーン、0.9mmのガイドワイヤ適合カテーテルに載置。
ポリビニルアルコール(PVA、USPグレード、Spectrum Chemicals & Laboratory Products, Gardena, CA)の5wt%水溶液を調製した。この溶液をここでは溶液3と呼ぶ。実施例2に記載されているように構造チューブをバルーンカテーテル上に組み付け、そして回転させながら溶液3で30秒間にわたって浸漬被覆した。30秒後、デバイスを溶液3から取り出した。デバイスを回転させながら、ヒートガンを使用して(約40℃の)熱風をデバイスに約3分間にわたって吹きつけた。次いでこのプロセスをさらに2回繰り返した。次に、デバイスを60℃の炉装置内に約10分間にわたって入れた。
以下の方法を用いて外側シース層を調製した。図11Aに示されているように、(Branca他の米国特許第5,814,405号明細書に記載されるような)薄型ePTFEフィルムを50%オーバーラップさせた状態でチューブ軸線に対してほぼ45°の角度を成して6mmのステンレス鋼製マンドレルに少なくとも1つのパスで螺旋状に巻き付けることによって、外側シース層を調製した。ePTFE層を含むマンドレルを炉装置内で380℃で3分間にわたって焼成することにより層を融合させた。図11Bに示されているように、その結果生じたチューブをマンドレルから取り外した。マンドレルから取り外した後、チューブを平坦化し、バルーン、構造層、及び被膜の周りに螺旋状に巻き付けることによって、外側シース層を形成し、これをバルーンの遠位端及び近位端に取り付け、余剰の長さをトリミングして取り除いた。具体的には、結合された区域をLoctite 7701プライマー (Henkel AG & Co. KgaA, Duesseldorf, Germany)で湿潤した。次いで、5層の6.4mm幅のePTFEフィルムを使用して、外側シース層の端部をバルーンに固定的に取り付けた。具体的には、ePTFEフィルム層をバルーン端部の周りに周方向に巻き付ける一方、Loctite 4981 (Henkel AG & Co. KgaA, Duesseldorf, Germany)をフィルムに塗布した。薬物送達バルーンは図11Cに非拡張状態で示されている。
Claims (23)
- 医療デバイスであって:
a.外面を有する拡張可能な部材と、
b.前記拡張可能な部材の周りに配置された治療薬を含む被膜と、
c.前記拡張可能な部材の外側表面に沿って、平坦化された形態を有するネッキング可能なエレメントを含むシースと、
を含み;
ここで、前記ネッキング可能なエレメントは、離散した1層の材料層であり、該離散した1層の材料層は、非拡張状態にある前記拡張可能な部材の周りに螺旋状、長手方向、又は周方向に巻き付けられているとき、前記離散した1層の材料層の縁が互いに隣接しかつオーバーラップするか又は触れるよう、前記材料層の縁により画定される幅を有する;
d.前記拡張可能な部材が拡張されると、前記ネッキング可能なエレメントが伸長して幅が少なくとも2分の1に減少し、前記離散した1層の材料層の縁が互いに離れるようになり、これにより前記外面の少なくとも一部を露出させるように構成されている、

- 前記チューブ形態が螺旋状に巻き付けられている、請求項2に記載の医療デバイス。

- 前記治療薬が疎水性薬剤である、請求項1に記載の医療デバイス。
- 前記ネッキング可能なエレメントが、前記治療薬の大量転移を制限して水和を可能にする透過性マイクロ構造を含む、請求項1に記載の医療デバイス。
- 前記シースが、圧潰された形態では、下側に位置する表面の100%を覆い、拡張時には前記シースは該下側に位置する表面の30%未満を覆う、請求項1に記載の医療デバイス。
- 前記シースが、圧潰された形態では、下側に位置する表面の100%を覆い、拡張時には前記シースは該下側に位置する表面の20%未満を覆う、請求項1に記載の医療デバイス。
- 前記シースが湿潤剤で処理されている、請求項1に記載の医療デバイス。
- 前記湿潤剤が、ヘパリン被膜、ポリビニルアルコール、ポリエチレングリコール、ポリプロピレングリコール、デキストラン、アガロース、アルギネート、ポリアクリルアミド、ポリグリシドール、ポリ(ビニルアルコール−コ−エチレン)、ポリ(エチレングリコール−コ−プロピレングリコール)、ポリ(ビニルアセテート−コ−ビニルアルコール)、ポリ(テトラフルオロエチレン−コ−ビニルアルコール)、ポリ(アクリロニトリル−コ−アクリルアミド)、ポリ(アクリロニトリル−コ−アクリル酸−コ−アクリルアミジン)、ポリアクリル酸、ポリリシン、ポリエチレンイミン、ポリビニルピロリドン、ポリヒドロキシエチルメタクリレート、及びポリスルホン、及びこれらのコポリマーから成る群から、単独又は組み合わせて選択される、請求項10に記載の医療デバイス。
- 前記湿潤剤がポリビニルアルコールである、請求項10に記載の医療デバイス。
