Description
デュシェンヌ型筋ジストロフィー(DMD)とベッカー型筋ジストロフィー(BMD)が知られている。
DMDは、新生男児3,500人に1人の割合で発症する。臨床症状としては、幼児期に発症し、10歳前後で歩行不能となり、20歳代で呼吸不全または心不全により死に至る病状を示す。そのため、30代過ぎまで生存することは稀である。
なお、「BMD」は、ストップコドンを生じない突然変異に起因する疾患であり、ジストロフィンタンパク質自体は発現するが、その構造が異常な場合に発症する。BMDの症状は、DMDよりも軽微であり進行性も遅い。
アンチセンス核酸を用いた治療は、目的のスプライス部位に相補的な配列のオリゴマーを投与することによって、ジストロフィン遺伝子に配列特異的なエクソンスキッピング(エクソン上の変異によって生じたストップコドンの読み飛ばし)を誘導し、そのタンパク質発現を回復させる技術である。
さらに、PMOをアンチセンス核酸として用いた場合、通常のRNAベースのオリゴマーとは異なり、生体内で分解され難い非常に安定な物質であるという利点がある。
PMOは、細胞透過性が極めて低いため、臨床応用を実現するためには、筋組織に豊富に存在する毛細血管壁を突破させ、筋繊維(筋細胞)への導入効率を大幅に改善させる手段の開発が必須である。
現段階では、PMOの血管投与を介した全身治療はできず、筋肉内に局所投与を継続的に行う必要があり、患者のQOLを著しく低下させていた。
また、DMDの直接の死因となる心不全や呼吸困難の原因となる心臓や横隔膜の筋繊維(筋細胞)に対しては、導入できないのが現状である。
これにより本発明では、DMDの病状を劇的に改善させる治療薬を提供することを課題とする。
また、本発明者らは、血管投与によって、全身の骨格筋、心臓や横隔膜の筋組織の筋繊維(筋細胞)おけるDMDのジストロフィン欠損を顕著に回復できること(全身治療が可能であること)を見出した。
即ち、〔請求項1〕に係る本発明は、PEG-リポソームの内部にパーフルオロ炭化水素を封入したものであり、且つ、平均粒子径50〜500nmのものである、バブルリポソームの表面に;下記(A1)〜(A3)の性質を有するモルフォリノオリゴマーを結合させたバブルリポソーム;を有効成分として含有してなり、且つ、下記(B)に記載の用法で用いられることを特徴とする、デュシェンヌ型筋ジストロフィー治療薬、に関するものである。
(A1):変異によってストップコドンを有するジストロフィン遺伝子のmRNA前駆体に対して、スプライシング過程で当該変異を有するエクソンをスキッピングさせた成熟mRNAを生成させる機能を有するモルフォリノオリゴマー。
(A2):前記エクソンのスプライシング促進配列を含む領域に対する相補的塩基配列からなるモルフォリノオリゴマー。
(A3):重合度15〜50merのモルフォリノオリゴマー。
(B):前記モルフォリノオリゴマーを表面に結合させたバブルリポソームを、筋組織内又は血管内に投与した後、体外から筋組織に超音波照射することにより、前記筋組織の筋細胞内に前記モルフォリノオリゴマーを高効率で導入させる用法。
また、〔請求項2〕に係る本発明は、前記エクソンが、エクソン2, 8, 23, 43〜46, 50〜53のいずれかである、請求項1に記載の筋ジストロフィー治療薬、に関するものである。
また、〔請求項3〕に係る本発明は、前記投与が血管を介した全身投与である、請求項1又は2に記載の筋ジストロフィー治療薬、に関するものである。
また、〔請求項4〕に係る本発明は、前記筋組織が心臓および横隔膜の筋組織である、請求項3に記載の筋ジストロフィー治療薬、に関するものである。
また、本発明によれば、これまで治療が不可能であった心臓や横隔膜の筋組織(DMDの進行によって心不全や呼吸困難を引き起す筋組織)に対してもDMDの症状を改善させることが可能となる。
本発明は、特定のモルフォリノオリゴマー(PMO)を表面に結合させたバブルリポソームを有効成分として含有してなり、且つ、筋組織内又は血管内に投与した後、体外から筋組織に超音波照射することにより、前記筋組織の筋繊維(筋細胞)に前記PMOを高効率で導入させることを特徴とする、デュシェンヌ型筋ジストロフィー治療薬に関する。
本発明によって治療可能な病状は、デュシェンヌ型筋ジストロフィー(DMD)である。
本発明のDMD治療薬は、「特定のPMOを表面に結合させたバブルリポソーム」を有効成分として含むものである。なお、本願明細書では、単に「PMO搭載バブルリポソーム」と記載する場合がある。PMO搭載バブルリポソームの一態様を図1に示す。
ここで、PMOは、バブルリポソーム内部に封入されたものではなく、バブルリポソームの表面に結合した状態を指す。
なお、具体的には、この場合の結合とは、荷電状態による電気的な結合、水素結合、共有結合等だけでなく、PMOがバブルリポソームの表面の脂質類分子によって形成される溝に入り込んだ状態、なども含むものである。
本発明における‘モルフォリノオリゴマー’(PMO、;ホスホロジアミデートモルフォリノオリゴマー)とは、モルフォリノサブユニット(単量体)が重合したオリゴマーを指すものである。
モルフォリノサブユニットは、ヌクレオチド類似体であり、RNAの構成単位であるリボヌクレオチドのリボース(構成糖)全体が、モルフォリノ環に置き換わって構造のものを指す。参考として、図2に当該構成単位が重合した構造図を示す。
PMOは、DNAやRNAと同様に塩基配列を有するものであり、アンチセンス核酸として機能する分子である。
またPMOは、RNase等の分解を受けないため、生体内や自然界において極めて安定な物質であり、アンチセンス核酸を用いた治療に有効な分子として用いることができる。
なお、スプライシング促進部位とは、mRNA前駆体からスプライシングによってイントロンが切り出される際に重要な役割を果たす領域である。当該部位は、イントロンとエクソンの両方に存在するが、本発明においてはエクソン上の部位を対象とする。
そして、最終的に、スプライシング過程において当該エクソン(ストップコドンを有するDMDの原因となるエクソン)をスキッピングさせ、ジストロフィンタンパク質の発現を可能とする成熟mRNAが生成される。
なお、エクソンスキッピングの機構の一態様を図3に示す。
また、具体的なアンチセンスPMO(当該DMD原因エクソンのスプライシング促進部位を含む領域の相補的配列PMO)としては、配列番号1(エクソン46に対応), 配列番号2(エクソン50に対応), 配列番号3(エクソン51に対応)、に記載の塩基配列からなるPMOを挙げることができる。
なお、これらの分子種は「PMO」であるため配列表に記載可能な分子種が存在しない。そこで、「DNA」として記載した配列番号1〜3の塩基配列情報を引用することで、本発明のPMO(アンチセンス配列のモルフォリノオリゴマー)を記載した。
例えば、マウスのmdx変異体の場合、エクソン23にDMDの原因となる変異が存在するため、当該エクソンをスキッピングさせる機能を有するアンチセンスPMOを用いることができる。具体的には、配列番号4に記載の塩基配列からなるPMOを挙げることができる。
また、導入効率の観点からは、重合度が低いものが好ましく、40mer以下、さらには30mer以下のものが好適である。
なお、本発明のPMOとしては、3’側及び/又は5’側に修飾分子が結合したものであってもよい。例えば、蛍光物質や抗原物質等の標識分子を結合させたものであっても用いることができる。
本発明における‘バブルリポソーム’とは、粒子サイズが微小なリポソームにパーフルオロ炭化水素(超音波造影剤)を封入したものを指す。
当該リポソームとしては、常法によって調製した粒子径の大きなリポソームを、ナノフィルターなどを通過させて微小化させたものであればよい。具体的にはPEG-リポソームを挙げることができる。
特にリン脂質である、DPPC, DSPE-PEG, DSPE-PEG-NHS, EPC, POPC, DSPC, DSPE, PS, PG, PI, DMPG, DMPC, などを挙げることができる。
ここで、粒子径が大きすぎる場合、毛細血管内の移動に適さず、血管投与に好適に用いることができない。また、組織深部への移行性も悪くなる。
具体的には、パーフルオロプロパンを好適に用いることができる。
本発明のDMD治療薬としては、前記したように有効成分としてPMO搭載バブルリポソームを含むものであれば如何なる形態のものでも良いが、投与する際に液体の形状であれば良い。例えば、液体アンプル、濃縮液等の形状とすることができる。
また、必要に応じて、pH調製剤、抗生物質等を混合したものでも良い。また、賦型剤等と混ぜた粉末、顆粒、カプセル状であっても、使用の際に液体であれば問題なく用いることができる。
本発明におけるDMD治療薬は、筋肉組織への局所投与、血管内投与によって体内に投与することを要する。具体的には注射や点滴等によって行うことができる。
特に、血管内投与(具体的には静脈投与)を行うことで、当該治療薬の有効成分を毛細血管を通して全身の筋組織に送達する(行き渡らせる)ことができ好ましい。
投与量としては、体重60kgの成人の場合、PMOの量に換算して、0.1〜500 mg、1〜250 mg、好ましくは10〜100 mgを投与することで十分な効果を得ることができる。
本発明においては、投与後に体外から筋組織に‘超音波照射’することを必須とするものである。
本発明は、当該処理でバブルリポソームが任意の部位で崩壊させ、それに伴って発生した衝撃波(キャビテーション)によってPMOに血管壁(血管内皮細胞や筋内膜)を透過させ、PMOを筋繊維(筋細胞)の細胞内に高効率に取り込ませる(導入する)ことを可能とする技術である。図4に当該処理の模式図を示す。
また、臨床用の超音波診断装置を利用することもできる。ここで超音波診断装置とは、目的部位にプローブを当てて体外から超音波を照射し、組織等からの超音波反射を検出して内部を画像化する装置を指す。本発明では、当該装置の超音波照射機能を利用することで、本発明の超音波照射が可能となる。
また、癌治療等で用いる集束超音波装置(HIFU)、骨折治療等に用いる低周波超音波装置も、照射条件を調節することで、本発明に用いることが可能である。
また、照射強度(intensity)としては、0.1〜10 W/cm2、好ましくは0.1〜3 W/cm2、さらに好ましくは0.5〜3 W/cm2の条件で行うことができる。
照射時間としては、バブルリポソームを崩壊させることが可能な時間を行えばよいが、例えば10秒以上、好ましくは30秒以上、特に60秒以上、を挙げることができる。また、上限としては特にないが、例えば、360秒以下、好ましくは180秒以下、特に120秒以下を挙げることができる。
なお、マウス等の動物に対して当該処理を行う場合、毛等を刈ってから行うことで照射効率を向上させることができる。
本発明では、超音波を照射した付近の広範囲の筋組織に対して、高効率のPMO導入が可能となる。これにより、ジストロフィンが欠損していた筋組織の筋繊維(筋細胞)に対して、当該タンパク質の発現を劇的に回復させることが可能となる。
例えば、下肢上部に止血帯を施して静脈注射を行い、下肢部に超音波照射を行った場合、下肢部の骨格筋全体(‘広範囲’)の治療が可能となる。
また、本発明では、‘最低1回’の血管投与を行うことでも、DMDの症状を劇的に改善することが可能となる。
特に、従来法ではPMO導入がほぼ不可能であった‘心臓や横隔膜’の筋組織(DMDの進行によって心不全や呼吸困難を引き起す筋組織)に対しても、これらの部位に超音波照射を行うことでDMD治療が可能となる。
(1)リポソームの調製
基本脂質として、dipalmitoylphosphatidylcholine(DPPC:日本油脂製)及びdistearoylphosphatidylethanolamine-PEG2000-OMe(DSPE-PEG2000-OMe:日本油脂製)の2種を使用し、脂質組成がDPPC:DSPE-PEG2000-OMe=94:6(molar ratio)となるように、クロロホルム:ジイソプロピルエーテル:PBS = 1:1:1の溶液に溶解した。
これをプローブ型ソニケーター(20kHz)を用いて超音波処理した後、ロータリーエバポレーターを用いて有機溶媒を留去し、リポソーム(PEG-リポソーム)を得た。
得られたリポソームは凍結融解を行った後、Extruder (Lipex Biomembrane Inc.)を用いてpore size 0.2μmのpoly carbonate membraneを通すことで、粒子径を約100〜200nmに調製した。その後、pore size 0.45μm のcellulose acetate syringe filterを用いて、濾過滅菌を行ったものを実験に用いた。
前記調製したリポソーム(脂質濃度:1mg/mL)2mLを、パーフルオロプロパン置換下の5mLバイアル瓶に封入した。
これをバス型ソニケーター(42kHz)を用いて超音波処理することで、パーフルオロプロパンを封入したバブルリポソームの懸濁液を調製した。
滅菌水に溶解した配列番号4に記載の塩基配列のPMO(mdxマウスのジストロフィン遺伝子におけるエクソン23に対するPMO、GENE TOOLS, LLC、濃度:10μg/10μL)を、バブルリポソーム(総脂質量:30μg/30μL)に添加し、一定時間微振動を与えることで、PMOを搭載させたバブルリポソームを調製した。当該PMO搭載バブルリポソームの模式図を図1に示す。
なお、当該PMOは、バブルリポソームの内部に封入されているのではなく、表面の溝に入り込んだ状態(結合した状態)で、搭載されていると推測される。
バブルリポソーム(総脂質量:60μg/60μL)に対して、FITC標識したPMOを用いて添加量を0, 25, 50, 100, 150, 200 pmolとなるように添加して、添加量の異なる各PMO搭載バブルリポソームを調製した。なお、その他の調製手順は、調製例1に記載の方法と同様にして行った。
そして、調製した各バブルリポソームに搭載されたPMO量を、FACS(Fluorescence Activated Cell Sorting)解析により測定した。結果を図5に示す。
(1)PMOの導入
調製例1で調製したPMO搭載バブルリポソーム(PMO量:10μg)の溶液を、5〜6週齢のmdxマウス(ジストロフィータンパク質欠損マウス)の前脛骨筋(Tibialis)に筋肉内投与(局所投与)した。
その後、直ちに、前脛骨筋(下肢部)に超音波照射装置(SONITRON 2000, NEPA GENE, CO, LTD)のプローブを当て、体外から超音波照射(Frequency:1MHz, Duty:50%, Intensity:2W/cm2, Time:60sec.)を行った。
そして、超音波照射後、常法にて2週間飼育した。また、比較対象としては、同量のPMOのみをmdxマウスに投与する処理を行い同様にして飼育した。また、対照として、通常のmdxマウスを同様にして飼育した。
PMO導入処理から2週間経過後、解析例1の方法により、PMO導入によってジストロフィンmRNAのエクソン23(mdxマウスでは変異によってストップコドンを有する)がスキッピングされる効率を解析した。結果を図6に示す。(なお、以下、エクソンスキッピングを単に「ES」と略す場合がある。)
また、そのバンド強度は、PMOのみを投与した場合(比較対象:図6 レーンC)と比べて、大幅に増加することが示された。
また、PMO導入処理から2週間経過後、解析例2の方法により、PMO導入によってmdxマウス筋組織で誘導されたジストロフィン発現を検出した。結果を図7, 8に示す。
また、ジストロフィンタンパク質を発現する筋繊維の数は、PMOのみを投与した場合(比較対象:図7 (C))と比べて、大幅に増加することが示された(図8)。
(1)PMOの導入
5〜6週齢のmdxマウスの下肢上部に止血帯を施し、調製例1で調製したPMO搭載バブルリポソーム(PMO量:50μg)の溶液を、シリンジポンプ(KDS100, kdScientific)により一定流速で大伏在静脈より投与(経静脈投与)した。
その後、直ちに、下肢部骨格筋(下肢部)に超音波照射装置(SONITRON 2000, NEPA GENE, CO, LTD)のプローブを当て、体外から超音波照射(Frequency:1MHz, Duty:50%, Intensity:2W/cm2, Time:120sec.)を行った。当該操作の模式図を図9に示す。
そして、超音波照射後、常法にて2週間飼育した。また、比較対象としては、同量のPMOのみをmdxマウスに投与する処理を行い同様にして飼育した。また、対照として、通常のmdxマウスを同様にして飼育した。
なお、以下で解析した下肢部骨格筋の種類を表1に示す。また、各骨格筋の模式図を図10に示す。
PMO導入処理から2週間経過後、解析例1の方法により、PMO導入によってジストロフィンmRNAのエクソン23がスキッピングされる効率を解析した。結果を図11, 12に示す。
その結果、PMOのみを投与した群(比較対象)と比べて、PMO搭載バブルリポソームを投与した群(本発明)では、下肢部骨格筋の広範囲にわたって、ジストロフィン遺伝子におけるエクソンスキッピングが顕著に誘導されることが示された。
また、PMO導入処理から2週間経過後、解析例2の方法により、PMO導入によってmdxマウス筋組織で誘導されたジストロフィン発現を検出した。結果を図13に示す。なお、筋ジストロフィンを発症していない健常なC57BL/6マウスの下肢部骨格筋組織を、Positive controlとして用いた(健常マウス:図13 (P))。
(1)PMOの導入
5〜6週齢のmdxマウスに調製例1で調製したPMO搭載バブルリポソーム(PMO量:500μg)の溶液200μLを尾静脈投与した。
その後、直ちに、心臓(胸部)に超音波照射装置(SONITRON 2000, NEPA GENE, CO, LTD)のプローブを当て、体外から超音波照射(Frequency:1MHz, Duty:50%, Intensity:2W/cm2, Time:60sec.)を行った。当該処理の状況を撮影した写真像図を図14に示す。
PMO導入処理から2週間経過後、解析例2の方法により、PMO導入によってmdxマウス筋組織で誘導されたジストロフィン発現を検出した。結果を図15に示す。なお、筋ジストロフィンを発症していない健常なC57BL/6マウスの心筋組織を、Positive controlとして用いた(健常マウス:図15 (P))。
i )PMO搭載バブルリポソームと投与し対象部位に超音波処理を行うことによって、従来のPMOのみを投与した場合と比べて、ジストロフィンの発現が大幅に回復できることが明らかになった。
なお、当該ジストロフィンの発現回復は、PMOが働きエクソン23がスキッピングされたmRNAが合成されたことに起因することが確認された。
ii)PMO搭載バブルリポソームを静脈注射(全身投与)し、任意の部位に超音波を照射することによって、‘任意’且つ‘広範囲’の筋組織で、ジストロフィン発現を回復できることが示唆された。
iii)また、従来は頻回投与が必須であったが、‘1回のみ’の投与で劇的な回復効果が得られることが示された。
iv)従来ではPMO導入が困難であった‘心臓(心筋)’に対しても、ジストロフィン発現を回復できることが示された。
マウスジストロフィン遺伝子のエクソン20とエクソン26上に設計した2組のプライマーセットを用いてnested RT-PCRを行うことにより、PMO導入によるエクソンスキッピング誘導効率を解析した。
前記マウスから筋組織を回収し、RNAiso Plus(TaKaRa)を加えてホモジネートをした。そして、12,000g, 4℃, 5分遠心後、上清を回収した。
上清にクロロホルムを加え混合し、室温で5分放置した後,12,000g, 4℃, 15分遠心し、上層の水層を回収した。
これにイソプロパノールを加えて12,000 g, 4℃, 10分遠心し、75%エタノールを加え洗浄し、7,500 g, 4℃, 5分遠心後に上清を捨て、乾燥させた。
得られた沈殿をDEPC処理水に溶解しtotal RNA溶液とした。RNAの濃度は、260nmの吸光度を測定することにより算出した。
Total RNA(1μg)に、Oligo dT(Invitrogen)1μL, dNTP mix(Invitrogen)1μL, DEPC処理水を加えてtotal 10μLとした。そして、65℃, 5分処理することで熱変性を行い、即座に氷冷することで、RNA分子の高次構造形成を阻害した。
上述の液に、5×Prime Script(TaKaRa)4μL, RNase Out inhibitor (Invitrogen) 0.5μL, Prime Script (RTase, TaKaRa) 0.5μL, DEPC処理水 5μL を加え、total 20μLとした。そして、42℃, 60分の伸長反応を行い、70℃, 15分でRTaseを失活させて反応を停止させ、cDNA溶液とした。
前記cDNA溶液 2μLに、10μM forward primer(Exon20Fo:エクソン20上に設計された配列番号5に記載のプライマー)1μL,10μM reverse primer(Exon26Ro:エクソン26上に設計された配列番号6に記載のプライマー)1μL,Ex Taq(TaKaRa)0.125μL, 10×Ex Taq buffer 2.5μL, 10mM dNTP Mixture 2.0 μL,滅菌蒸留水16.375μLを加え、total 25μLとした。
そして、95℃, 30秒間の熱変性、;55℃, 1分間のアニーリング、;72℃, 2分間の伸長反応、;を、30サイクル行った。
上記検出したPCR産物のバンド強度をImageJ softwareを用いて解析した。そして、エクソンスキッピング(ES)誘導効率は、得られたバンド強度の数値から次の式(1)に従って算出した。
ジストロフィンタンパク質の抗体を用いて免疫染色を行うことで、PMO導入によってmdxマウス筋組織で誘導されたジストロフィン発現を検出した。
その後、凍結組織からクリオスタットを用いて切片を作成した。TBSTにて洗浄後、一次抗体NCL-DYS2(Novocastra)を室温で1時間反応させた。TBSTを用いて洗浄し、二次抗体Alexa Fluor 546 (MolecularProbes)を室温で1時間反応させた。
TBSTで洗浄後、VECTASHIELD Hard・Set Mounting Medium with DAPI(フナコシ)で封入し、蛍光顕微鏡(Axiovert 200 M: Carl Zeiss, KEYENCE: BZ8100)で観察することで、ジストロフィンタンパク質の発現を検出した。
これにより、本発明は、医薬分野や製薬分野において貢献できることが期待される。

Claims (4)
- PEG-リポソームの内部にパーフルオロ炭化水素を封入したものであり、且つ、平均粒子径50〜500nmのものである、バブルリポソームの表面に;下記(A1)〜(A3)の性質を有するモルフォリノオリゴマーを結合させたバブルリポソーム;を有効成分として含有してなり、且つ、下記(B)に記載の用法で用いられることを特徴とする、デュシェンヌ型筋ジストロフィー治療薬。
(A1):変異によってストップコドンを有するジストロフィン遺伝子のmRNA前駆体に対して、スプライシング過程で当該変異を有するエクソンをスキッピングさせた成熟mRNAを生成させる機能を有するモルフォリノオリゴマー。
(A2):前記エクソンのスプライシング促進配列を含む領域に対する相補的塩基配列からなるモルフォリノオリゴマー。
(A3):重合度15〜50merのモルフォリノオリゴマー。
(B):前記モルフォリノオリゴマーを表面に結合させたバブルリポソームを、筋組織内又は血管内に投与した後、体外から筋組織に超音波照射することにより、前記筋組織の筋細胞内に前記モルフォリノオリゴマーを高効率で導入させる用法。
- 前記エクソンが、エクソン2, 8, 23, 43〜46, 50〜53のいずれかである、請求項1に記載の筋ジストロフィー治療薬。
- 前記投与が血管を介した全身投与である、請求項1又は2に記載の筋ジストロフィー治療薬。
- 前記筋組織が心臓および横隔膜の筋組織である、請求項3に記載の筋ジストロフィー治療薬。
