Description
本出願はアメリカ合衆国において2007年9月28日提出の、本明細書中で、その全体を参考として援用する仮出願60/960,408に対する優先権及び利益を主張するものである。
アメリカ合衆国政府は、保健社会福祉省により授与されたNIH助成HL60942号の条件に基づく規定により、本発明において全額支払済み実施権及び限定された状況において特許権者に妥当な条件で他に実施権を付与させる権利を有する。
本発明は、肥満症の治療及び予防に有用な組成物及び方法に関する。
肥満症は、極めて一般的かつ有害な代謝病であり、アメリカ合衆国や世界中で公衆衛生上に極めて深刻な脅威を与える。この疾患の有病率は1980年代半ば頃から著しく増加し始めた。アメリカ合衆国では成人の50%が過体重であり、30%が肥満である。さらに深刻なことに、肥満の有病率とそれにと関連する糖尿病は小児においても急速に増加している。個人や家族そして社会に与える肥満の影響は、特に、保健資源の費用および利用の点からも大変深刻である。これらのことより、体重管理は学問的な話題になるばかりではなく、社会的関心も高まっている。
本発明は、肥満細胞が、炎症促進性サイトカインおよび他のメディエーターの産生および放出におそらく起因して、肥満症の発症において重要な役割をすることを示唆する実験に基づく。マウスに高脂肪食(Western diet)を与えたところ、正常対照群マウスよりも、肥満細胞欠損マウスでの体重の増加が実質的に低かった。しかし、正常なマウスに肥満細胞安定化薬も投与することにより、体重増加が実質的に回避される。
本発明は、例えば以下の項目を提供する。
(項目1)
ヒトまたは動物被験体における肥満症の治療または予防する方法であって、該方法は、該被験体に肥満細胞を安定化させる薬物の有効量を投与する工程を包含する、方法。
(項目2)
前記肥満細胞を安定化させる薬物が、肥満のヒト被験体に、治療開始時の総体重の少なくとも15%を減少させるのに少なくとも該被験体に十分な期間投与される、項目1に記載の方法。
(項目3)
前記ヒト及び動物被験体が、治療開始時においてアレルギー、心臓疾患及び糖尿病を有していない、項目1に記載の方法。
(項目4)
前記薬物がクロモリン、ネドクロミル、ケトチフェン、及びロドキサミドからなる群より選択される、項目1に記載の方法。
(項目5)
前記薬物が、1日あたり用量50〜1500mgにて経口投与される、項目4に記載の方法。
(項目6)
前記薬物が、1日あたり用量5〜100mgにて経鼻投与される、項目4に記載の方法。
(項目7)
前記薬物が、1日あたり用量200〜1000mgにて経口投与されるクロモリンナトリウムまたはクロモリン二ナトリウムである、項目4に記載の方法。
(項目8)
前記薬物が1日あたり用量5〜50mgにて経鼻投与されるネドクロミルナトリウムである、項目4に記載の方法。
(項目9)
前記薬物が1日あたり用量50〜500mgにて経口投与されるネドクロミルナトリウムである、項目4に記載の方法。
(項目10)
前記薬物が1日あたり用量1〜200mgにて経口投与されるフマル酸ケトチフェンである、項目4に記載の方法。
(項目11)
前記薬物が1日あたり用量1〜200mgにて経口投与されるロドキサミドトロメタミンである、項目4に記載の方法。
(項目12)
治療構成物であって、該治療構成物は:
a)完成した薬学的容器内の肥満細胞安定化薬、及び
b)該肥満細胞安定化薬をヒトまたは動物患者に投与して肥満症を治療するための説明書
を含む、治療構成物。
(項目13)
前記薬物がクロモリン、ネドクロミル、ケトチフェン、及びロドキサミドからなる群より選択される、項目12に記載の方法。
(項目14)
前記薬物がクロモリンナトリウムまたはクロモリン二ナトリウムである、項目13に記載の方法。

本発明は、肥満細胞安定化薬が肥満症を制御するのに使用され得ることを示す実施例セクションに以下のようにまとめられた実験に基づいている。これらの薬物は、肥満のヒトが体重を減らすことを補助するか、ヒトを肥満になることから予防するためのいずれかに使用され得る。
肥満細胞を安定化する薬物は、アレルギーの治療との関連で広く研究された。そしてこれらの薬物のいくつかは商品として使用されている。もっとも好まれる肥満細胞安定化薬はクロモリン、ネドクロミル、ケトチフェン、ロドキサミドであり、これらは、購入され得るか、または当該分野で周知の方法を使用して合成され得る。さらに、当該分野で記載される、任意のその他の薬学的に受容可能な肥満細胞抑制剤についても本発明で使用され得る。これらとしては、アメリカ合衆国特許(6,207,684;4,634,699;6,207,684;4,871,865;4,923,892;6,225,327;7,060,827)に開示される化合物が挙げられる。化合物を調製するための方法が、化合物を精製し得る方法と使用され得る形態についての情報と共に、これらの米国特許の各々に提示される。これらの化合物は、任意の薬学的に受容可能な塩を含む、任意の薬学的に受容可能な形態で患者に与えられ得、最も好ましい薬物はクロモリンナトリウムまたはクロモリン二ナトリウムのいずれかである。
肥満細胞を安定化させる薬物は、当該分野で標準的な方法にしたがって薬学的組成物に組み入れられる(例えば、Remington’s Pharmaceutical Sciences, Mack Publishing Co.,(1990)を参照のこと)。処方物は、当該分野で一般的に使用される任意の経路によって送達されるように設計され得、経口または経鼻送達のために設計された調製物が好ましい。例えば錠剤、カプセルのような経口組成物について、肥満細胞安定化薬は一般的に1〜500mgで存在すべきである。経鼻送達用の組成物において、安定薬は一般的に0.5〜50mg/mlとし、1〜20mg/mlにするのがより好ましい。経口的に摂取される液剤に、同様の濃度範囲が使用され得る。好ましくはないが、投与の他の経路も使用され得る。
本発明は、経口、経口(peroral)、内用(internal)、直腸内、経鼻、舌下、経皮、経膣、静脈内、動脈内、筋肉内、腹腔内、皮内、そして皮下経路を含む任意の投与経路に適合性がある。使用され得る剤形は、錠剤、カプセル、粉末薬、エアロゾル、坐薬、皮膚用パッチ剤、非経口的な薬、除放性調製物、そして懸濁液、溶液および乳濁液を含む経口性の液体が挙げられる。もっとも好ましい投与経路は経口投与および経鼻投与である。望ましい場合、組成物(特に、注射用組成物)は、投与前に再構成される、フリーズドライおよび凍結乾燥したものであり得る。剤形は、唯一の活性成分として肥満細胞安定化薬を含み得るか、他の活性薬剤も含んでもよい。すべての剤形は、当該分野で標準的であり、かつRemington’ Pharmaceutical Sciences(Osol, A, ed., Mack Publishing Co.(1990))のような参考研究に教示される方法に基づいて調製され得る。
本明細書に書かれている治療目的は、患者の体重を減らすこと、またはさらなる体重の増加を防ぐことである。体重の増加を防ぐために使用される場合、最適な投薬量は、動物実験(本明細書に記載されるような実験)の結果に基づき、臨床研究は、当該分野で周知の方法を使用して実行される。肥満細胞安定化薬は、すでに他の疾患症状、特にアレルギーに利用されており、既存の投薬量は、肥満症の予防または治療に有効な投薬量を評価するための開始点として役立ち得る。既存の知識に基づき、経口送達方法を使用して、代表的に、患者が、肥満細胞安定化薬の50〜1,500mg/日の経口用量で、好ましくは少なくとも2個の等しい用量に分けて受容することが期待される。薬物が経鼻投与される場合、5〜100mg/日の量の安定化薬を毎日投与され、再度、この量が、いくつかの等しい用量に分割されることが期待される。
本発明に述べられている実験は主として肥満症に関することであるが、特定の局面は、同じ方法がI型糖尿病の治療または予防にも適用可能であることが示唆された。糖尿病治療における目的はインスリン依存性を減らす、もしくは無くすことである。好ましい薬物および投薬量は肥満に対するものと同じである。
これまで述べられてきたように、肥満細胞安定化薬に含まれる薬学的組成物は、完成した薬学的容器内に配置され得、組成物の使用に関する医師への説明書とともに販売され得る。経鼻送達用の調製物において、薬学的組成物は、一般にスプレーとして組成物を送達するように設計されたデバイスに包装された液剤または粉末である。この方法で薬物を送達するための当該分野で公知のあらゆるデバイスは、この発明に適合性がある。送達の意図された経路に依存して、他の容器は、ボトル、バイアル、アンプル、ブリスターパックなどを含み得る。
本発明は、肥満細胞を安定化する能力をもとに肥満症の治療または予防において化合物の使用の可能性を評価するための方法も含んでいる。これらのアッセイは、当該分野で周知であり、またアレルギーの治療において有用な薬剤の同定と組み合わせて使用される。
マウス
野生型(C57BL/6)、Il6−/−(C57BL/6,N11)、およびIfng−/−(C57BL/6,N10)マウスはJackson Laboratories(Bar Harbor, ME)から購入した。類遺伝子性のTnf−/−(C57BL/6,N10)マウスと肥満細胞欠損KitW−sh/W−shマウス(C57BL/6,N>10)は、記載されるようにC57BL/6マウスとのもどし交配によって発生させた(Sunら、Nat. Med. 13:719−724(2007);Woltersら、Clin Exp Allergy 35:82−88(2005))。すべての動物実験のプロトコールはHarvard Medical Schoolの常任委員会に是認され、すべてのマウスは病原体のない設備に収容された。肥満を引き起こすために、6週齢のマウス(メス及びオス)に高脂肪食(Research Diet, New Brunswick, NJ)を12〜20週間与えた。マウスの体重は週に1回モニタリングした。高脂肪食消費の各々の経過の終わりまで、耐糖能とエネルギー消費量のアッセイを以前の報告(Yangら、Nat. Cell Biol. 9:970−977(2007))に従って実行した。マウスの血液サンプルは血清アディポカインの測定のために採取された。皮下脂肪と内臓脂肪と褐色脂肪と骨格筋は別個にタンパク抽出およびパラフィン切片の調製のために集められた。組織タンパクの抽出物はELISA、イムノブロット分析、およびシステイニルカテプシン活性部位の標識に用いられた。イムノブロット解析では30μgのタンパクを、220kDaフィブロネクチン(マウスモノクローナル抗体, 1:100, NeoMarkers, Fremont, CA)と200kDaインスリン受容体(マウスモノクローナル抗体, 1:100, Calbiochem, San Diego, CA)を検出するために8%SDS−PAGE上で分離したか、またはGlut4(マウスモノクローナル抗体, 1:200, R&D Systems, Minneapolis, MN)、UCP1(ラビットポリクロ―ナル抗体, 1:3000, Abcam, Cambridge, MA)、アクチン(マウスモノクローナル抗体, 1:500, Abcam)、グリセルアルデヒド−3−リン酸デヒドロゲナーゼglyceraldehyde−3−phosphate(GAPDH, ラビットポリクロ―ナル抗体, 1:1000, Santa Cruz Biotechnolog, Santa Cruz, CA)を検出するために12%SDS−PAGE上で分離した。カテプシン活性部位の標識については、以前に記載されたように、30μgのタンパクを1μlの600mM のジチオスレイトールと1μlの[125I]−JPMをpH5.5の緩衝液中でインキュベートした。37℃で1時間反応混合物をインキュベートした後、標識されたタンパクを12%SDS−PAGEで分離した。ゲルはクマシーブルーで染色し、脱色、乾燥させX線フィルムに曝露した。肝臓および胃腸管は組織学的分析のために回収した。
この研究では80人の肥満症の被験体が、2003年〜2007年の間にDepartment of Nutrition of Hotel−Dieu Hospital(Reference Center for the Medical and Surgical Care of Obesity, Paris, France)において先を見越して集められ登録された(平均年齢37.5歳、範囲20−66歳、平均BMI 48.5kg/m2、範囲32−85 kg/m2;女性/男性比69/11)。これらの被験体はAssistance Pubrique−Hopitaux de Parisにより支持される臨床研究プログラムにおける、食事介入あるいは胃の手術のどちらかの候補者たちである。これらの被験体からは、炎症性疾患あるいは感染症、がん、アルコール乱用、腎疾患の者は除外された。胃の手術の際、本発明者らは、肥満患者の一部の臍周囲の領域から皮下脂肪組織を採取した(N=6, ボディマス指数=51.71±1.40kg/m2, グルコース: 5.38±0.13 mmol/l, インシュリン:13.32±1.06μU/ml)。同じような地理的領域にいる、健康で、非肥満の年齢が対応する(P=0.2)の個体がコントロールとして集められた(N=32、平均年齢;41.4歳、範囲、20−62歳;平均BMI、22.8kg/m2;範囲、19.9−28.4kg/m2;女/男比、22/10)。10人の肥満のないコントロール(BMI=23.67±0.48kg/m2;グルコース:4.82±0.45mmol/l、インシュリン:7.20±1.56μU/ml)において、臍周囲の領域において皮下脂肪組織を針生検により採取した。代謝の研究に参加したボランティアは、介入の前、3か月間、体重は安定していた。絶食後の朝に採取した血液サンプルおよび血清は使用時まで−80℃で凍結した。組織サンプルはホルマリンで固定し、パラフィン包埋した。Hotel−Dieu hospitalの倫理委員会によりこの臨床研究は承認され、すべての被験体に書面でのインフォームドコンセントを行なった。
ヒトおよびマウスのWATと筋肉組織は10%ホルマリンで一晩固定し、パラフィン包埋し5μmの切片を作成した。抗ヒト肥満細胞トリプターゼ抗体(マウスモノクローナル抗体, 1:100, Dako, TRAPPES CEDEX, France)、抗マウス肥満細胞CD117抗体(ラットモノクローナル抗体, 1:10, eBiosciences, Inc., San Diego, CA)、および抗マウスCD31抗体(ラットモノクローナル抗体, 1:400, Pharmingen, San Diego, CA)でWAT、筋肉および肝臓の切片上の肥満細胞と微細血管を免疫染色した。研究者は、組織の起源がわからないようにして、ヒトトリプターゼ陽性肥満細胞とCD117陽性肥満細胞をカウントし、データは細胞数/mm2で示した。WATと筋肉におけるCD31陽性の面積は、記載されるようにImage−Pro Plus softwareを用いて同様に測定し、そしてデータはCD31陽性面積のパーセンテージで示した(Wangら、J. Biol. Chem. 281:6020−6029 (2006); Wild, Microvasc Res. 59:368−376 (2000))。ヘマトキシリン‐エオジン染色は消化管の組織学的評価を補助した。
凍結したマウスのWATは粉々に砕き、プロテアーゼ阻害剤カクテル(Calbiochem, San Diego, CA)を含むRIPA緩衝液(Pierce, Rockford, IL)で溶解した。WATおよび血清サンプルは、製造者の説明書に従ってIL6(BD Biosciences)、TNF−α(PeproTech, Rocky Hill, NJ)、IFN−γ(PeproTech)、MCP−1 (PeproTech)、アディポネクチン(R&D Systems)、MMP−9 (R&D Systems)、Cat S (R&D Systems)およびCat L (Bender MedSystems Inc, Burlingame, CA)についてのELISAに供した。
マウスの前駆脂肪細胞(pre−adipocyte)3T3−Ll(The American Type Culture Collection, Manassas, VA)は、前述のように6−wellあるいは24−wellプレートで培養し、インスリン、デキサメタゾン、イソブチルメチルキサンチン中で脂肪細胞に分化した(Yangら、Nat. Cell Biol. 9:970−977 (2007))。 3T3−Llの分化における肥満細胞の役割を評価するために、生きている肥満細胞(6−well plateに 2x106細胞/wellまたは24−well プレートに5x105細胞/well)あるいはそれらの脱顆粒タンパク抽出物(生きている細胞数と同等の)を100%コンフルエントの3T3−Ll細胞に添加した。共培養は7〜9日間維持され、生きている肥満細胞または肥満細胞のタンパク抽出物を含む培養培地は、2日毎に交換した。オイルレッドO染色により脂肪沈着を定量化し、データはOD510nm読み取り値により示した。分化した3T3−Ll細胞は前述のとおり、pH5.5緩衝液に細胞を溶解することによって、カテプシン活性部位の標識にも用いた(Shiら、J. Biol. Chem. 267:7258−7262(1992))。
BMMCは、本発明者らが以前報告したようにWT、Il6−/−、Tnf−/−、Ifng−/−マウスの骨髄から調製した(Sunら、Nat. Med. 13:719−724 (2007); Sunら、J. Clin. Invest. 117:3359−3368 (2007))。組換えマウスIL3(PeproTech)と幹細胞因子(PeproTech)での5週間分化させた(Sunら、Nat. Med. 13:719−724 (2007))後、細胞の純度および形態はそれぞれCD117媒介FACS分析およびトルイジンブルー染色によって確認した。マウスにおける肥満症での肥満細胞の役割を調べるために、6週齢のオスのKitW−sh/W−shマウスの尾静脈に、それぞれのタイプのマウス由来のBMMC(1x107/マウス)を注射した。BMMC再形成の2週間後、それらのマウスに高脂肪食を消費させて肥満症と糖尿病を誘導させた。マウスの体重は毎週記録され、タンパク抽出物調製のためのWATを採集前に耐糖能アッセイを行なった。
マウスから得られたすべてのデータは平均値±SEとして示し、本発明者らのデータのサイズが小さいことと異常なデータ分布に起因する、統計的有意差はノンパラメトリック検定(Mann−Whitney検定)を用いて判定した。ヒト血清のキマーゼおよびトリプターゼのデータは平均値±標準誤差で示した。Shapiro−Wilcoxon検定により、すべての臨床的および生物学的パラメーターは正規分布を示した。非対称の変数(キマーゼおよびトリプターゼ)は統計処理の前に、その分布を正規化するために対数変換し、検証した。グループ間の比較について、Srudent’s t test、分散分析(ANOVA)、非連続の値についてのカイ2乗検定を使用した。統計解析はJMPの統計ソフトウェア (SAS Institute Inc., Cary, NC)により行なった。P<0.05を有意であるとみなした。
肥満の被験体のWATには、脂肪細胞に加えてマクロファージとリンパ球が含まれていた(Weisbergら、J. Clin. Invest. 112:1796−1808 (2003); Wuら、Circulation 115:1029−1038 (2007))。これらの炎症性細胞はWATでサイトカインや成長因子、ケモカインおよびプロテアーゼを産生する (Wuら、Circulation 115:1029−1038 (2007); Fantuzzi, J. Allergy Clin. Immunol. 115:911−919 (2005))。ただし、肥満症および関連する代謝性合併症である糖尿病の病因におけるこれらの細胞の役割に関しては依然として不明である。他の認識されていない細胞が決定的な一因となっているのかもしれない。ヒトの脂肪組織切片において、脂肪細胞特異的なトリプターゼモノクローナル抗体による免疫染色から、脂肪の少ない(lean)ドナー由来のWATより肥満の被験体のWATでの肥満細胞の数が多いことが明らかになった。肥満のドナーの血清中の肥満細胞プロテアーゼのレベルがより高いことは、この脂肪細胞含量がより多いことに付随した。ELISAを使用して、肥満のドナー(ボディマス指数≧30kg/mm2)は、性別についての補正前(P<0.01)と後(P<0.01,多変量解析)で脂肪の少ない被験体(ボディマス指数<30kg/mm2)よりも有意に高い血清トリプターゼ濃度を有した。これらの結果は肥満症における肥満細胞の役割を示唆する。
肥満症の発症には脂質生成、血管新生、マトリックスリモデリングを含まれる。血管新生は肥満症において特定の重要性を有する。栄養成分を白色脂肪に提供する以外に、微細血管は、アディポカインの放出が続く白血球浸潤の経路を提供する。マウスでは血管新生の抑制により脂肪組織発達が抑制される(Rupnickら、Proc. Natl Acad. Sci. USA 99:10730−10735 (2002))。肥満のWTマウス由来白色脂肪と筋組織は、内皮細胞のマーカーであるCD31についての実質的な免疫染色を示した。対照的に、高脂肪食を給餌したKitW−sh/W−shマウス、あるいはDSCGを受容したマウスは、固形飼料を給餌した脂肪の少ないマウスと同様なCD31陽性領域を有した。血管新生の低減により白血球の侵潤が減少し、したがって炎症メディエーターのWAT産生も低下した可能性がある(Skouraら、J.Clin.Invest.117:2506−2516(2007))。この概念と一致して、KitW−sh/W−shマウスとDSCGを受容したマウスの血清および/または白色脂肪は、IL6、TNF−α、IFN−γ、MCP−1、マトリックスメタロプロテアーゼ(MMP−9)およびカテプシンS(Cat S)のより低いレベルを有したが、いくつかのアディポカイン(アディポネクチンやCat Lなど)が有意な変化をしなかった。マトリックスのタンパク分解は血管新生促進ペプチド(pro−angiogenic peptide)の放出によって血管新生に寄与する可能性がある(Xuら、J.Cell Biol.154:1069−1079(2001))。本発明者らは以前、抗血管新生ペプチドの分解と血管新生促進性ラミン−5 断片γ2の生成によって血管新生においてCat Sが重要な役割を果たすことを明らかにした(Wangら、J. Biol. Chem. 281:6020−6029 (2006))。KitW−sh/W−shマウスおよびDSCG治療したマウスにおける血管新生の減少は、白色脂肪および血清中の低いCat Sレベルを伴っていた。活性なカテプシンを選択的に標識する[125I]−JPMと白色脂肪タンパク抽出物を一緒にインキュベートした結果、KitW−sh/W−shマウスの白色脂肪においてCat S、Cat K、Cat Bを含む活性なカテプシンの減弱が見出された。興味深いことに、DSCG治療したマウスの白色脂肪は、WTコントロールに類似した、活性なCat SおよびCat Kのレベルを依然として有し、このことは、これらの組織内の肥満細胞の数の多さと一致していた。Cat SのELAISAにより、本発明者らは、異なるマウス間の局所および全身レベルを測定することが可能であった。JPM標識実験からの観察と同様に、オスおよびメスの両方のKitW−sh/W−shマウスは、WTコントロールよりも低い血清中および白色脂肪中のCat Sレベルを示した。対照的に、DSCG治療は、血清中のCat Sレベルは有意に減少させたが、白色脂肪では減少しなかった。このことはこの抗アレルギー物質は効果的に肥満細胞を不活性化することが示唆した。
Claims (11)
- ヒトまたは動物被験体における肥満症を治療または予防するための組成物であって、該組成物は、肥満細胞を安定化させる薬物の有効量を含み、該薬物は、クロモリンおよびケトチフェンからなる群より選択される、組成物。
- 前記組成物が、肥満のヒト被験体に、該被験体について治療開始時の総体重の少なくとも15%を減少させるのに少なくとも十分な期間投与されることを特徴とする、請求項1に記載の組成物。
- 前記ヒト及び動物被験体が、治療開始時においてアレルギー、心臓疾患及び糖尿病を有していない、請求項1に記載の組成物。
- 1日あたり用量50〜1500mgにて経口投与されることを特徴とする、請求項1に記載の組成物。
- 1日あたり用量5〜100mgにて経鼻投与されることを特徴とする、請求項1に記載の組成物。
- 前記薬物が、クロモリンナトリウムまたはクロモリン二ナトリウムであり、前記組成物が1日あたり該薬物の用量200〜1000mgにて経口投与されることを特徴とする、請求項1に記載の組成物。
- 前記薬物がフマル酸ケトチフェンであり、前記組成物が、1日あたり該薬物の用量1〜200mgにて経口投与されることを特徴とする、請求項1に記載の組成物。
- 肥満症を治療または予防するための治療用パッケージであって、該治療用パッケージは:
a)完成した薬学的容器内の肥満細胞安定化薬物であって、該薬物がクロモリンおよびケトチフェンからなる群より選択される、肥満細胞安定化薬物、及び
b)該肥満細胞安定化薬をヒトまたは動物に投与して肥満症を治療するための説明書
を含む、パッケージ。 - 前記薬物がクロモリンナトリウムまたはクロモリン二ナトリウムである、請求項8に記載のパッケージ。
- 前記薬物がフマル酸ケトチフェンである、請求項8に記載のパッケージ。
- 前記肥満細胞安定化薬物が、単位用量形態で薬学的組成物の一部であり、単位用量当たり1〜500mgで存在する、請求項8に記載の治療用パッケージ。
