前記表面シート及び裏面シートは左右方向に沿って設けられた横断線を境として、上下方向に分けられた第１領域と第２領域とされ、
前記表面シートの前記第１領域に第１情報記録部が、前記第２領域に第２情報記録部が形成され、
前記裏面シートの前記第１領域に、前記横断線の両端からそれぞれ内側に位置する第１端点を起点として、ほぼコ字状の線状の第１切り離し部が形成され、
その第１切り離し部を境として、その裏面シートの周辺の部分を除去して、前記表面シートの周辺粘着部を露出可能であり、
前記第１情報記録部が、前記第１切り離し部より内側に設けられ、前記表面シート及び裏面シートを貫通して形成した切り離しミシン目線部を境にして、前記第１領域より切り離し可能とされ、
