Description
本発明は、癌患者が化学療法での治療反応に対して有益な反応を有する可能性が高いかどうかを予測するために有用な遺伝子発現情報を提供する。
遺伝子発現研究
癌専門医には、彼らが利用できるいくつかの治療選択肢、例えば、「標準治療」と位置づけられる、化学療法薬の種々の組み合わせ、および個々の癌の治療のための表示事項はないが、その癌における有効性の証拠があるいくつかの薬物がある。良好な治療成果の最良の尤度を得るには、転移性疾患の最高の危険にある患者を同定し、最適な利用可能な癌治療を与えることが必要である。詳しくは、「標準治療」治療薬、例えば、シクロホスファミド、メトトレキサート、5−フルオロウラシル、アントラサイクリン、タキサンおよび抗エストロゲン薬、例えば、タモキシフェンに対する患者の反応の尤度を調べることが重要であるが、これは、これらには限定された有効性しかなく、重篤な副作用の範囲を有することも多いからである。したがって、利用可能な薬物を必要とし、これに対して反応する可能性が最も高い、または最も低い患者を同定することにより、より賢明な患者選択によって、これらの薬物が提供しなくてはならない正味の有効性を増大させ、正味の死亡率および毒性を低減することができる。

過去2年間にわたって、Genomic Health, Incおよび共同研究者(Esteban et al., Proc Am Soc Clin Oncol 22: page 850, 2003 (abstract 3416)、Soule et al, Proc Am Soc Clin Oncol 22: page 862, 2003 (abstract 3466)、Cobleigh et al. Soc Clin Oncol 22: page 850, 2003 (abstract 3415)、Cronin et al, Am J Pathol 164(l):35−42 (2004))が、再発リスクに対する分子署名を見い出すことを目的として、初期乳癌における遺伝子発現についていくつかの予備的臨床研究を報告した。これらの研究では、定量的RT−PCRを用いて、臨床記録と関連付けられた、凍結、パラフィン包埋(FPE)組織試料において250種の候補遺伝子マーカーが試験された。多様な患者グループにわたって再発のリスクと一貫して関連している遺伝子が同定され得るかどうかを調べるために3つの研究すべてにわたる分析が実施された。これらの一変数の結果に基づいて、複数の遺伝子モデルが設計され、3つの研究にわたって分析された。臨床評価研究において予測的に試験される、16種の癌関連遺伝子および5種の参照遺伝子からなる、単一の複数遺伝子アッセイが開発された。これら21種のmRNA種の測定値を用い、100点尺度で再発リスクを報告する、再発スコア(RS)と呼ばれるアルゴリズムが作製された。

一般に、遺伝子発現プロファイリング法は2つの大きなグループに分けることができる:ポリヌクレオチドのハイブリダイゼーション分析に基づく方法およびポリヌクレオチドの配列決定に基づく方法。サンプル中のmRNA発現の定量化のために当技術分野で公知の最も良く用いられる方法として、ノーザンブロッティングおよびin situ ハイブリダイゼーション(Parker & Barnes, Methods in Molecular Biology 106:247−283 (1999))、RNアーゼ保護アッセイ(Hod, Biotechniques 13:852−854 (1992))および逆転写ポリメラーゼ連鎖反応(RT−PCR)(Weis et al, Trends in Genetics 8:263−264 (1992))が挙げられる。あるいは、指定の二本鎖、例えば、DNA二本鎖、RNA二本鎖およびDNA−RNAハイブリッド二本鎖またはDNA−タンパク質二本鎖を認識できる抗体を使用することもできる。配列決定に基づく遺伝子発現解析の代表的な方法として、遺伝子発現の連続分析(SAGE)および超並列的シグニチャー配列決定(MPSS)による遺伝子発現解析が挙げられる。
上記に列挙した技術のうち、最も感度が高く、最も適応性がある定量的な方法は、RT−PCRであり、これを用いて種々のサンプル集団において、薬物治療を施したか施していない正常および腫瘍組織において、mRNAレベルを比較して、遺伝子発現のパターンを特性決定でき、密接に関連しているmRNA間を識別でき、RNA構造を解析できる。
マイクロアレイ技術を用いても、差次的遺伝子発現を同定または確認できる。したがって、乳癌関連遺伝子の発現プロフィールは、マイクロアレイ技術を用いて新鮮腫瘍組織またはパラフィン包埋腫瘍組織のいずれかで測定できる。この方法では、注目するポリヌクレオチド配列(cDNAおよびオリゴヌクレオチドを含む)を、マイクロチップ基材上にプレーティングするか、または配列させる。次いで配列させた配列を、注目する細胞または組織に由来する特異的DNAプローブとハイブリダイズさせる。ちょうどRT−PCR法におけるように、mRNAの供給源は通常、ヒト腫瘍または腫瘍細胞株および対応する正常組織または細胞株から単離した全RNAである。したがって、RNAは種々の原発腫瘍または腫瘍細胞株から単離できる。mRNAの供給源が原発腫瘍である場合には、mRNAは、例えば、毎日の臨床診療において日常的に調製され保存される、凍結または保管されたパラフィン−包埋および固定(例えば、ホルマリン固定)組織サンプルから抽出できる。
遺伝子発現の連続分析(SAGE)は、各転写物に個々のハイブリダイゼーションプローブを提供する必要なく、多数の遺伝子転写物の同時かつ定量的な解析を可能にする方法である。まず、転写物を一意的に同定するのに十分な情報を含む短い配列タグ(約10から14bp)を作製する。ただし、タグは各転写物内の独特の位置から得る。ついで、多数の転写物を互いに結合して長い連続分子を形成するが、これは配列決定でき、複数のタグの同一性を同時に示すことができる。いずれかの転写物の集団の発現パターンは、個々のタグの量を測定することと、各タグに対応する遺伝子を同定することとによって定量的に評価できる。より詳細については、例えば、Velculescu et al., Science 270:484−487 (1995)およびVelculescu et al, Cell 88:243−51 (1997)参照のこと。
Brenner et al, Nature Biotechnology 18:630−634 (2000)によって記載されたこの方法は、分離している直径5μmのマイクロビーズ上で非ゲルベースのシグニチャー配列決定(signature sequencing)を、何百万もの鋳型のin vitroクローニングと組み合わせる配列決定アプローチである。まず、in vitroクローニングによってDNA鋳型のマイクロビーズライブラリーを構築する。この後、高密度(通常、3×106個マイクロビーズ/cm2より多い)で、フローセルにおいて鋳型を含有するマイクロビーズの平面アレイの構築を行う。各マイクロビーズ上のクローニングされた鋳型の遊離末端を、DNA断片の分離を必要としない蛍光ベースのシグニチャー配列決定法を用いて同時に分析する。この方法は、酵母cDNAライブラリーから1回の操作で、同時に、正確に何十万の遺伝子のシグニチャー配列を提供することがわかっている。
固定され、パラフィン包埋された組織をRNA供給源として用いて遺伝子発現をプロファイリングするための代表的なプロトコールの工程、例えば、mRNA単離、精製、プライマー伸長および増幅は、種々の公開学術論文に提供されている(例えば、T.E. Godfrey et al,. J. Molec. Diagnostics 2: 84−91 [2000];K. Specht et al., Am. J. Pathol. 158: 419−29 [2001]))。要約すると、代表的なプロセスは、パラフィン包埋した腫瘍組織サンプルの約10μm厚切片を切断することで開始する。次いで、RNAを抽出し、タンパク質およびDNAを除去する。RNA濃度を分析した後、必要に応じてRNA修復および/または増幅工程が含まれる場合があり、遺伝子特異的プロモーターを用いてRNAを逆転写し、その後RT−PCRを行う。最後に、データを分析して、調べた腫瘍サンプルにおいて同定された特徴的な遺伝子発現パターンに基づき、癌再発の予測される尤度に応じて、患者が利用できる最良の治療選択肢を同定する。
本発明の重要な態様は、予後情報または予測情報を提供するために乳癌組織による特定の遺伝子の測定された発現を使用することである。この目的には、アッセイされたRNA量の相違および用いたRNAの質の変動の双方について補正する(正規化する)ことが必要である。したがって、アッセイは、通常、特定の正規化遺伝子、例えば、以下の実施例において示されるように、周知のハウスキーピング遺伝子、例えば、ACTB、GAPD、GUSB、RPLOおよびTFRCの発現を測定し、組み込む。あるいは、正規化は、アッセイされた遺伝子のすべてまたはその大きな部分集合の平均または中央値シグナル(CT)に基づくものである場合もある(グローバルノーマライゼーションアプローチ)。以下、特に断りのない限り、遺伝子発現とは正規化された発現を意味する。
本発明の一態様によれば、PCRプライマーおよびプローブは、増幅される遺伝子中に存在するイントロン配列に基づいて設計する。したがって、プライマー/プローブ設計における第1の工程は、遺伝子内のイントロン配列の描写である。これは入手可能な公開ソフトウェア、例えば、Kent, W.J., Genome Res 12(4):656−64 (2002)によって開発されたDNA BLATソフトウェアによって、または変法を含むBLASTソフトウェアによって行うことができる。その後の工程は、PCRプライマーおよびプローブ設計の十分に確立された方法に従う。
本発明は、同時係属出願第10/883,303号に記載される特定のアルゴリズムおよび統計的手法を利用する。
癌治療に用いられる化学療法薬は、その作用機序に応じていくつかのグループに分けることができる。いくつかの化学療法薬はDNAおよびRNAに直接損傷を与える。このような化学療法薬は、DNAの複製を混乱させることによって、複製を完全に停止するか、ナンセンスDNAまたはRNAの生成をもたらす。このカテゴリーには、例えば、シスプラチン(プラチノール(登録商標))、ダウノルビシン(セラビジン(Cerabidine)(登録商標))、ドキソルビシン(アドリアマイシン(登録商標))およびエトポシド(ベペシド(登録商標))が含まれる。癌化学療法薬のもう1つのグループは、ヌクレオチドまたはデオキシリボヌクレオチドの形成を干渉し、その結果RNA合成および細胞複製が阻止される。この種類の薬物の例としては、メトトレキサート(アビトレキサート(Abitrexate)(登録商標))、メルカプトプリン(ピュリネソール(登録商標))、フルオロウラシル(アドルシル(登録商標))およびヒドロキシウレア(ハイドレア(登録商標))が挙げられる。第3の種類の化学療法薬は紡錘体の合成または分解を達成し、結果として、細胞***を妨げる。この種類の薬物の例としては、ビンブラスチン(ベルバン(Velban)(登録商標))、ビンクリスチン(オンコビン(登録商標))およびタキセン、例えば、パクリタキセル(タキソール(登録商標)およびトセタキセル(タキソテール(登録商標))が挙げられる。トセタキセルは、従前の化学療法が失敗した後の局所進行または転移性乳癌の患者および従前の白金ベースの化学療法が失敗した後の局所進行または転移性非小細胞肺癌の患者を治療するために米国で現在認可されている。これらのすべておよびその他の化学療法薬に対する患者の反応の予測は、明確に本発明の範囲内にある。
本発明の方法に用いる材料は、周知の手順に従って製造されるキットの調製に向いている。したがって、本発明は、予後転帰または治療に対する反応を予測するための開示された遺伝子の発現を定量化するための遺伝子特異的または遺伝子選択的プローブおよび/またはプライマーを含み得る薬剤を含むキットを提供する。このようなキットは、場合により、腫瘍サンプル、詳しくは、固定され、パラフィン包埋された組織サンプルからRNAを抽出するための試薬および/またはRNA増幅用試薬を含み得る。さらに、本キットは、場合により、本発明の方法におけるその使用に関連する識別説明書または表示または使用説明書を有する試薬を含み得る。本キットは、例えば、既製のマイクロアレイ、バッファー、適当なヌクレオチド三リン酸(例えば、dATP、dCTP、dGTPおよびdTTPまたはrATP、rCTP、rGTPおよびUTP)、逆転写酵素、DNAポリメラーゼ、RNAポリメラーゼおよび本発明の1種以上のプローブおよびプライマー(例えば、RNAポリメラーゼに関して反応性であるプロモーターと連結している適当な長さのポリ(T)、遺伝子特異的またはランダムプライマー)を含む方法において用いられる、各々、1種以上の種々の試薬(通常、濃縮型で)を含む容器(例えば、本方法の自動実行において使用するのに適したマイクロタイタープレート)を含み得る。
この研究は、化学療法に対して感受性がある患者または耐性の患者を予測する遺伝子または遺伝子群を同定するために実施した。本研究では、NSABP研究B−20「A Clinical Trial to Determine the Worth of Chemotherapy and Tamoxifen over Tamoxifen Alone in the Management of Patients with Primary Invasive Breast Cancer, Negative Axillary Nodes and Estrogen−Receptor−Positive Tumors」Fisher et al., J Natl Cancer Inst 89(22): 1673−1682 (1997)から得た組織およびデータを用いた。
患者組み入れ基準:NSABP研究B−20に登録されたもの。患者除外基準:NSABPアーカイブにおける初期診断から利用可能な腫瘍ブロックがないもの、ブロック中に腫瘍がないまたは極めて小さい腫瘍と、病理学者によるH&Eスライドの検査によって評価されるもの、RT−PCR解析には不十分なRNA(<275ng)、5種の参照遺伝子の平均非正規化CT<35、臨床不適格者または追跡調査がないもの。
B−20研究においてTAMおよび化学療法で研究登録時に治療された最大600人の患者から得た固定され、パラフィン包埋された***腫瘍組織試料を分析した。B−20研究においてTAM単独で研究登録時に治療された最大252人の患者から得た固定され、パラフィン包埋された***腫瘍組織から先に抽出したRNAを再分析した。TaqMan(登録商標)RT−PCRを用い、各患者について、16種の癌関連遺伝子および5種の参照遺伝子の発現を定量的に評価した。これは反応あたり2ngのRNAインプットを用い3連で実施した。
グループ1:腫瘍がないか極めて小さい腫瘍(正常上皮、繊維嚢胞性変化またはDCIS/LCISなどのその他の上皮要素によって占められる部分と比較した浸潤性癌細胞によって占められる部分が<5%)を有する場合は研究から除外した。
原発分析のために、遠隔再発のない生存率(DRFS)は、手術から最初の遠隔再発までの時間(年)に基づくものとした。対側性疾患、その他の二次原発癌および遠隔再発の前の死亡は打ち切り事象と考えた。
再発スコアの算出に用いた21種の遺伝子の発現レベルは、GHIアッセイから得た値として報告した。表1は16種の試験遺伝子および5種の参照遺伝子の同一性を示す。遺伝子発現値は、5種の参照遺伝子の平均に対して正規化した。参照遺伝子は、乳癌ならびに種々のサンプリングおよび加工条件下において比較的不変であることがわかっており、このために外来作用について正規化するのに有用なものとなっている。参照によって正規化した発現測定値は、通常、1単位増が通常RNA量の2倍の増大を反映する、0〜15の範囲である。解析のために21種の予め指定した遺伝子を表1に列挙する。
再発スコアは予後因子および予測因子の両方を含む。乳癌における治療予測遺伝子を同定する目的で、主目的は、治療された患者における遺伝子発現とDRFSの間の関係を調査することである。このような解析には、治療された患者および治療されていない患者双方から得たデータを用いて、治療予測遺伝子と純粋に予後的な遺伝子を区別した。化学療法治療予測遺伝子を同定するために、NSABP研究B−20から、TAMのみで治療された患者とTAMおよび化学療法の両方で治療された患者の双方を含めた。
0〜100の尺度での再発スコア(RS)は、以下のように参照によって正規化した発現測定値から導く:

RSを用いて、各患者の再発リスクグループを決定した。低、中および高リスク再発グループの間のカットオフ点は以下のように定義する:

表2は、試験した変数のうち6、すなわち、RS、増殖グループ閾値スコア(Pro1Thres)、MYBL2、浸潤グループスコア、SCUBE2およびESR1グループスコアが、統計的に有意(P<0.1)に有益な化学療法反応と相互作用したことが10年DRFSによって測定されたことを示す。表2中、RS/50の項によって示されるように、RSの相互作用分析を全部で100点の範囲の下方半分にかけて実施した。
Claims (21)
- ESR1陽性の乳癌と診断された被験体の化学療法に対する有益な反応の尤度を予測する方法であって、
(a)該被験体から得られた癌細胞を含む生体サンプルにおいて、下記(i)〜(vi)の変数のうち1以上の値を決定するのに必要であり、なおかつBIRC5、MKI67、MYBL2、CCNB1、STK6、MMP11、SCUBE2、ERBB2、GRB7、ESR1、PGR、BCL2、CTSL2、GSTM1、CD68及びBAG1からなる群から選択される少なくとも1の遺伝子の、RNA転写物もしくは対応する発現産物の発現レベルを測定し、
(b)以下の変数(i)〜(vi)のうち1以上を定量的に決定し、
(i)再発スコア
(ii)ESR1グループスコア
(iii)浸潤グループスコア
(iv)増殖グループ閾値スコア
(v)MYBL2のRNA転写物、または対応する発現産物の発現レベル、
(vi)SCUBE2のRNA転写物、または対応する発現産物の発現レベル、
(c)該被験体の化学療法に対する有益な反応の尤度を予測することを含み、
(c1)前記変数(i)が決定された場合、変数(i)の増加が前記被験体における前記化学療法に対する有益な反応の尤度の増大を示し、
(c2)前記変数(ii)が決定された場合、変数(ii)の増加が前記被験体における前記化学療法に対する有益な反応の尤度の低下を示し、
(c3)前記変数(iii)が決定された場合、変数(iii)の増加が前記被験体における前記化学療法に対する有益な反応の尤度の増大を示し、
(c4)前記変数(iv)が決定された場合、変数(iv)の増加が前記被験体における前記化学療法に対する有益な反応の尤度の増大を示し、
(c5)前記変数(v)が決定された場合、変数(v)の増加が前記被験体における前記化学療法に対する有益な反応の尤度の増大を示し、
(c6)前記変数(vi)が決定された場合、変数(vi)の増加が前記被験体における前記化学療法に対する有益な反応の尤度の低下を示す、
方法。 - 前記ESR1グループスコア、浸潤グループスコア、増殖グループ閾値スコア、MYBL2の発現レベル、及びSCUBE2の発現レベルが、有益な化学療法に対する各々の寄与によって重み付けされる、請求項1に記載の方法。
- 前記のRNA転写物またはそれらの発現産物の発現レベルは、1種以上の参照遺伝子またはそれらの発現産物の発現レベルに対して正規化される、請求項1に記載の方法。
- 前記参照遺伝子が、ACTB、GAPD、GUSB、RPLP0およびTFRCからなる群から選択される、請求項3に記載の方法。
- 前記化学療法がアジュバント化学療法である、請求項1に記載の方法。
- 前記化学療法がアントラサイクリン誘導体の投与を含む、請求項1に記載の方法。
- 前記アントラサイクリン誘導体がドキソルビシンまたはアドリアマイシンである、請求項6に記載の方法。
- 前記化学療法がタキサン誘導体の投与を含む、請求項1に記載の方法。
- 前記タキサン誘導体がパクリタキセルまたはドセタキセルである、請求項8に記載の方法。
- 前記化学療法がトポイソメラーゼ阻害剤の投与を含む、請求項1に記載の方法。
- 前記トポイソメラーゼ阻害剤がカンプトセシン、トポテカン、イリノテカン、20−S−カンプトセシン、9−ニトロ−カンプトセシン、9−アミノ−カンプトセシンおよびGI147211からなる群から選択される、請求項10に記載の方法。
- 前記化学療法がヌクレオチド生合成の阻害剤の投与を含む、請求項1に記載の方法。
- 前記のヌクレオチド生合成の阻害剤がメトトレキサートおよび/または5−フルオロウラシル(5−FU)である、請求項12に記載の方法。
- 前記組織が固定パラフィン包埋されているか、または新鮮なものであるか、または凍結されている、請求項1に記載の方法。
- 前記生体サンプルが細針生検、コア生検またはその他の種類の生検に由来する、請求項1に記載の方法。
- 前記発現レベルの決定が定量的RT−PCRを含む、請求項1に記載の方法。
- 前記発現レベルの決定が免疫組織化学又はプロテオミクス技術による決定を含む、請求項1に記載の方法。
- 前記有益な反応の尤度を要約する報告書の作成をさらに含む、請求項1に記載の方法。
- 増殖グループスコアが決定された場合、閾値を適用して増殖グループ閾値スコアを決定することをさらに含み、増殖グループ閾値スコアは、増殖グループスコアが6.5未満である場合には6.5に等しく、増殖グループスコアが6.5以上である場合には増殖グループスコアに等しい、請求項1に記載の方法。
