Abstract

本発明は、抗腫瘍免疫療法の分野に関する。特に本発明は、ネオアジュバント療法における免疫毒素自体による、または免疫チェックポイント阻害剤と組み合わせた免疫毒素を用いるがん処置に関する。
これらおよび他の態様は、本明細書を読むことで当業者に明らかになり、がんを処置する新しい治療レジメンを当技術分野に提供するであろう。
本発明の説明で使用される用語は、腫瘍学および医学において当業者によって十分に理解されると考えられるが、本明細書で提供される定義は、本発明の説明を容易にし、かつ用語の使用のために例示的な実施例を提供するために明記される。
本明細書で使用される場合、「a」、「an」、および「the」という用語は、単数が明示的に指定されない限り、「1つまたは複数」を意味する(例えば、単数とは、「a single agent(単一の薬剤)」などという句で明示的に指定される)。
「アジュバント療法」とは、本明細書では、腫瘍の切除の手術後、または腫瘍負荷を減少する他の方法が最初に実施された後にがん療法を投与することを指すために使用される。
「維持療法」とは、本明細書では、疾患の進行または再発の可能性を低減するために与えられる治療レジメンを指すために使用される。維持療法は、治療への応答を評価するための臨床パラメーターの評価に応じて、任意の長さの期間提供されることが可能である。
上記の開示は、本発明を一般的に説明するものである。以下の特定の実施例を参照することにより、より完全な理解を得ることができ、これらは、例示のみを目的として本明細書に提供され、かつ本発明の範囲を限定することを意図するものではない。
免疫毒素D2C7−ITを単独で使用して、腫瘍を有する個体において第I相臨床試験を実施する。腫瘍は再発性神経膠芽腫(GBM)であり、腫瘍切除後にD2C7−ITを単独で投与した(アジュバント療法)。2018年4月11日の時点で、41人の患者はD2C7−ITの単回腫瘍内投与の第I相用量漸増試験で処置されていた。17個の用量レベルが調査されていた(用量レベル1=40ng/mL)。用量レベル17(35,032ng/mL)を用量制限として識別した。耐用量の中央値(第2相用量)を確認するために、追加の患者が用量レベル16(23,354ng/mL)において現在登録している。用量レベル2(80ng/mL)の1人の患者は、32.7か月を超えた後に、D2C7−IT注入以来に追加治療なしで無病のままである。さらに、追加処置なしの腫瘍応答は、用量レベル10(2,050ng/mL)で処置された1人の患者および用量レベル13(6,920ng/mL)で処置された1人の患者において、処置後の現時点でそれぞれ12.8か月および6.8か月を超えて観察されている。3人の患者が現時点で死亡しており、23.7か月、23.2か月、および21.4か月間生存した。2人の追加の患者がD2C7−ITの腫瘍内注入後18.9か月および18.2か月を超えて生存している。
D2C7−(scdsFv)−PE38KDEL免疫毒素の構築、発現、および精製。D2C7のVHドメインのカルボキシル末端を、15個のアミノ酸ペプチドの(Gly4Ser)3リンカーによってVLドメインのアミノ末端に連結した。安定なITを得るためには、再生の間にVHがVLの近くに配置されることが不可欠である。これは、安定化ジスルフィド結合を形成するために、各鎖の単一の鍵となる残基をシステインに変異することによって達成した。分子モデリングおよび他のdsFv組換えITでの経験的データを使用した予測に基づいて、本発明者らは、システインに変異する各鎖における1つのアミノ酸を選択した。これらは、VHのフレームワーク領域2(FR2)における残基44およびVLのFR4における残基100である(Kabat番号付けに従う)。したがって、本発明者らは、ペプチドリンカーと、VHのSer44およびVLのGly100を置換するシステイン残基によって生成されるジスルフィド結合の両方を含有するFvを調製した。次いで、D2C7(scdsFv)PCR断片をPseudomonas外毒素AのドメインIIおよびIIIのDNAに融合した。本明細書で使用されるPseudomonas外毒素Aの型であるPE38KDELは、その細胞内滞留を増加させ、次いで細胞毒性を高める改変C末端を有する。D2C7−(scdsFv)−PE38KDEL(DNA配列 配列番号12)は、T7プロモーターの制御下で大腸菌(E.coli)において発現し、封入体として回収した。
この実施例は、がんのアジュバント療法を説明するものである。D2C7−IT抗原マウスEGFRvIII(dmEGFRvIII)を過剰発現するマウス神経膠腫株であるCT−2A−dmEGFRvIII−Lucを確立した。CT−2A−dmEGFRvIII−Luc(ホタルルシフェラーゼまたは「FFLuc」を発現するように改変された腫瘍細胞)細胞に対するD2C7−ITの反応性および治療効果を、フローサイトメトリーおよびインビトロでの細胞毒性アッセイによってそれぞれ測定した。CT−2A−dmEGFRvIII−Lucを、フローサイトメトリーによってMHCクラスIおよびPD−L1の発現についてさらに分析した。D2C7−ITもしくはαCTLA−4もしくはαPD−1の単剤療法またはD2C7−IT+αCTLA−4もしくはD2C7−IT+αPD−1の組み合わせ療法のインビボでの有効性を頭蓋内CT−2A−dmEGFRvIII−Luc神経膠腫保持C57BL/6免疫応答性マウスで評価した。この腫瘍のアジュバント処置のために、60匹のマウスを6つの処置群(ビヒクル対照、D2C7−IT、αPD−1、αCTLA−4、D2C7−IT+αPD−1、およびD2C7−IT+αCTLA−4、10〜12匹のマウス/群)に無作為化し、そして6〜9日目に、対流増強送達(CED)により総用量0.1μgのD2C7−IT/ビヒクル対照で治療した。CT−2A−dmEGFRvIII−Lucの移植後、250μg/用量のラットIgG2aアイソタイプ対照抗体、αPD−1抗体、または100μg/用量のαCTLA−4抗体を5回の投与で、6、9、12、15、および18日目に腹腔内注射により送達した。処置に対する頭蓋内(ic)腫瘍の抗腫瘍応答を、特定の神経学的終点(発作活動、反復旋回、または食欲不振などの他の微細な変化)または死亡までの時間の増加パーセントによって評価した。動物は、苦痛の徴候または神経学的症状の発症が1日に2回観察され、その時点でマウスを安楽死させた。D2C7−IT+αPD−1の組み合わせ療法群において、有意な腫瘍増殖遅延(生存の中央値において120%増加)および治癒率(4/12マウス)が観察され、そしてD2C7−IT+αCTLA−4の組み合わせ療法群において、有意な腫瘍増殖遅延(生存の中央値において80%増加)が観察された(図1)。
この実施例は、この処置後に個体における腫瘍負荷が減少する有効量の免疫毒素(例えば、D2C7−IT)を個体に投与することを含む、個体における腫瘍のネオアジュバント療法を説明するものである。雌のC57Bl6/J(約20g;7〜8週)マウスに、100μlのPBSに懸濁した3x106個のCT2A−mEGFRVIII−D2C7−FFLuc細胞を右脇腹に皮下注射した。移植された腫瘍が50〜100mm3に達したときに腫瘍負荷を減少するための手術あり、または手術なしのビヒクルまたはネオアジュバント(D2C7−IT)を投与することについて、治療群あたり10匹のマウスを無作為に選択した。試験マウスを12日目(腫瘍接種後)に20μlのPBSに希釈した4μgのD2C7−(scdsFv)−PE38KDELの単回腫瘍内(i.t.)注射で処置した。対照マウスは同じ様式で取り扱い、20μlのPBSのみで処置した。腫瘍接種後18日目およびネオアジュバント療法の7日後に、対照およびネオアジュバント療法マウスの1セットを未治療のままにし、一方で対照およびネオアジュバント療法マウスの第2のセットを、0.5〜1.5mm3の推定腫瘍体積を残す腫瘍の部分切除に供した。腫瘍を手持ち式デジタルキャリパーで週に3回測定し、腫瘍体積を次の式を使用して立方ミリメートルで計算した:([長さ]×[幅2])/2。腫瘍体積が1500〜2000mm3に達した場合、または腫瘍潰瘍(開放創)が発生した場合にはこの研究から除外して動物を試験した。図2に示すとおり、PBSで処置し、その後の腫瘍の切除が行われないマウス(−■−、PBS+切除なし)では、腫瘍が完全に退縮したマウスは0/10匹であった。D2C7−ITで処置され、その後の腫瘍の切除が行われないマウス(−▲−、D2C7−IT+切除なし)では、腫瘍が完全に退縮したマウスは0/10匹であった。PBSで処置し、その後の腫瘍の切除が行われるマウス(−▼−、PBS+切除あり)では、腫瘍が完全に退縮したマウスは2/10匹であった。そして、D2C7−ITで処置し、その後の腫瘍の切除が行われるマウス(−◆−、D2C7−IT+切除あり)では、腫瘍が完全に退縮したマウスは5/10匹であった。したがって、腫瘍を保持する個体を処置するためにネオアジュバントアプローチを使用する場合、腫瘍の退縮の増加という治療上の利益が観察される。この実施例におけるネオアジュバント療法アプローチの別の例は、EGFRwtおよびEGFRvIIIを標的とする有効量の免疫毒素の投与と組み合わせた(連続した)有効量の免疫チェックポイント阻害剤の投与を含むことであり、腫瘍の外科的切除、または腫瘍負荷を減少するための他の方法の前に両方とも投与される。D2C7−ITを単独で使用するネオアジュバントアプローチが治療上の利益をもたらすことを考慮すると、免疫チェックポイント阻害剤との組み合わせにおいてD2C7−ITと免疫チェックポイント阻害剤とを使用するネオアジュバントアプローチを使用する治療上の利益は(一部、このような組み合わせ療法から治療上の利益が観察されるため)明らかになるであろう。
腫瘍の外科的切除前に、治療有効量のEGFRwtおよびEGFRvIIIを標的とする免疫毒素を個体に投与し、次いで個体から腫瘍を切除する手術を行うことを含む、腫瘍を有する個体を処置する方法が提供される。ネオアジュバント療法のこの方法は、腫瘍の切除の前に、治療有効量の免疫チェックポイント阻害剤を、腫瘍を有する個体に投与することをさらに含み得る。例えば、抗PD−1抗体による処置が開始されてから数日後、次いで免疫毒素(D2C7−IT)が腫瘍内に投与される(神経膠芽腫の個体について、腫瘍内投与には点滴が使用されるであろう)。免疫毒素の投与から1週間後に、別の用量の免疫チェックポイント阻害剤を個体に投与する(例えば、抗PD−1抗体、240mg)。免疫毒素の投与から3週間後に、別の用量の免疫チェックポイント阻害剤を個体に投与する(例えば、抗PD−1抗体、240mg)。免疫毒素の投与から4週間後に腫瘍を外科的に切除する。免疫毒素の投与から5週間後かつ外科的切除から1週間後に、別の用量の免疫チェックポイント阻害剤を個体に投与する(例えば、抗PD−1抗体、240mg)。免疫毒素の投与から7、9、11、および13週間後に、別の用量の免疫チェックポイント阻害剤を個体に投与する(例えば、抗PD−1抗体、240mg)。次いで免疫チェックポイント阻害剤を投与することを含む維持療法は、医学的に保証されるとおり進めることが可能である。例えば、免疫毒素の投与から17、21、25週間後、そしてその後の約101週間まで4週間ごとに、免疫チェックポイント阻害剤を投与することが可能である(例えば、抗PD−1抗体を240mgにて2週間ごとに4か月間投与し、次いで480mgにて4週間ごとに最大2年間投与することが可能である)。
テキスト形式の配列表は同時に提出され、本出願の一部として参照により組み込まれる。配列表は、免疫毒素の成分のアミノ酸および核酸の配列を提供する。特に、配列表は、VHのCDR1(配列番号1)、VHのCDR2(配列番号2)、VHのCDR3(配列番号3)、VLのCDR1(配列番号4)、VLのCDR2(配列番号5)、およびVLのCDR3(配列番号6)のアミノ酸配列、可変重鎖(VH)のアミノ酸配列(配列番号7)、可変軽鎖(VL鎖)のアミノ酸配列(配列番号9)、配列番号8のとおりのVHおよびVL鎖を連結する適切なアミノ酸リンカー、免疫毒素D2C7−scdsFv−PE38KDEL全体をコードする核酸配列(配列番号12)とともに免疫毒素D2C7−scdsFv−PE38KDELの完全なアミノ酸配列(配列番号5)、ならびにD2C7−scdsFv部分の核酸配列(配列番号13)およびPE38KDEL部分の核酸配列(配列番号14)を提供する。
Claims (21)
- 患者の腫瘍を処置する方法であって、
(a)腫瘍の外科的切除の前に、治療有効量の免疫毒素、または免疫毒素と免疫チェックポイント阻害剤とを個体に投与すること、
(b)任意に腫瘍を切除する手術を実施することまたは放射線で処置することにより、腫瘍負荷を減少させること、および
(c)腫瘍切除後に、任意に免疫チェックポイント阻害剤を個体に投与すること、
を含み、免疫毒素が、PE38トランケート型Pseudomonas外毒素に融合された、一本鎖可変領域抗体またはその抗原結合断片であってもよい抗体またはその抗原結合領域を含み、抗原結合領域が、配列番号1〜6からなる群から選択される少なくとも2つのCDR領域を含む、方法。 - 抗体またはその抗原結合領域が、配列番号1〜6からなる群から選択される少なくとも4つのCDR領域を含む、請求項1に記載の方法。
- 抗体またはその抗原結合領域が、配列番号1〜6の6つのCDRを含む、請求項1または2に記載の方法。
- 抗体またはその抗原結合領域が、一本鎖可変領域抗体であり、かつ配列番号7のVHおよび配列番号9のVLを含む、請求項1〜3のいずれか1項に記載の方法。
- PE38トランケート型Pseudomonas外毒素が、KDELペプチドに融合されている、請求項1〜4のいずれか1項に記載の方法。
- 抗体またはその抗原結合領域が、一本鎖可変領域抗体であり、かつ配列番号11を含む、請求項1〜5のいずれか1項に記載の方法。
