Abstract
Description
(a)N末端シグナルペプチドを伴う又は伴わない配列番号1のアミノ酸配列を含むか又はそれからなる、即ち配列番号21で示されるような成熟ペプチド配列を含み得るか又はそれからなり得るもの;又は
(b)(配列番号21に示されるような)N末端シグナルペプチドを伴う又は伴わない配列番号1のアミノ酸配列に対して1つ以上のアミノ酸置換、修飾、欠失又は挿入を有する誘導体であり、配列番号20のアミノ酸配列など、グレムリン−1の活性を保持するもの。
(c)その変異体、このような変異体は、典型的には、配列番号1(又は配列番号20又は21)と少なくとも約60%、70%、80%、90%、91%、92%、93%、94%又は95%の同一性を(又は約96%、97%、98%又は99%の同一性さえも)保持する。言い換えれば、このような変異体は、配列番号1と約60%〜約99%の同一性、適切には配列番号1と約80%〜約99%の同一性、より適切には配列番号1と約90%〜約99%の同一性、最も適切には配列番号1と約95%〜約99%の同一性を保持し得る。変異体は以下でさらに記載する。
本発明によるグレムリン−1活性の阻害剤は、グレムリン−1の活性を低下させるか又は遮断する薬剤である。本発明による阻害剤は、グレムリン−1活性を部分的又は完全に阻害し得る。本発明での使用の阻害剤としては、グレムリン−1に若しくはグレムリン−1をコードする核酸分子に結合可能であるか又はグレムリン−1の発現を阻害可能である阻害剤が挙げられるが限定されない。このような阻害剤は、タンパク質、ポリペプチド、ペプチド、ペプチド模倣物、核酸(例えばDNA、RNA、アンチセンスRNA及びsiRNA)、炭水化物、脂質及び低分子であり得るが限定されない。

本発明での使用のためのグレムリン−1活性の阻害剤は、医薬組成物中で提供され得る。本医薬組成物は通常は無菌であり、典型的には薬学的に許容可能な担体及び/又はアジュバントを含む。本発明での使用のための医薬組成物は、薬学的に許容可能なアジュバント及び/又は担体をさらに含み得る。
本発明によるグレムリン−1活性の阻害剤は、骨折又は骨欠損の処置のために提供される。骨折は、骨組織における破損又は亀裂であり、転倒又は衝撃など、外傷の結果であり得るが、骨の完全性に影響を及ぼす疾患の結果としても起こり得る。骨欠損は、外傷又は疾患による骨の喪失である。
例1−タンパク質の発現、精製、再折り畳み及び構造決定
タンパク質発現及び封入体の調製
BamHI/XhoIを使用してE.コリ(E.coli)での発現に最適化された切断型ヒトグレムリン−1コード配列(配列番号20)を改変pET32aベクター(Merck Millipore)にクローニングし、N末端6His−TEVタグ付きのグレムリン配列(pET−hグレムリン1)をコードするベクターを作製した。
封入体を変性緩衝液(8M尿素、100mM Tris、1mM EDTA、10mM Na2S4O6及び100mM Na2SO3、pH8.5)中で再懸濁し、室温で16時間撹拌し、21,000gで15分間遠心分離することにより清澄化した。
可溶化した封入体を8M尿素、50mM MES、200mM NaCl、1mM EDTA、pH6.0中で平衡化したSephacryl S−200 26/60カラム(120mL)に載せた。グレムリン−1タンパク質を含有する分画を6M尿素、20mM MES、pH6.0で希釈し、HiTrap SP HP陽イオン交換カラムに載せ、10カラム体積(10CV)にわたり1M NaCl勾配で溶出した。精製変性hグレムリン−1タンパク質を含有する分画をプールした。
変性させ精製したグレムリン1タンパク質を再折り畳み緩衝液(50mM Tris、pH8.5、150mM NaCl、5mM GSH及び5mM GSSG、0.5mMシステイン、5mM EDTA、0.5Mアルギニン)に0.1mg/mLの最終濃度になるまで滴下して添加し、5日間にわたり一定の撹拌を行いながら4℃で温置した。5日後、グレムリン−1タンパク質を20mM HEPES、100mM NaCl、pH7.5に対して透析した。
6.6mg/mLのグレムリン1溶液及びpH4.1の0.1Mクエン酸、1M塩化リチウム及び27%ポリエチレングリコール(PEG)6000を1:1の比で混合することにより、ハンギングドロップ法を使用して、グレムリン1タンパク質結晶を成長させた。データ収集前に、結晶化緩衝液に20%グリセロールを添加することによって結晶を凍結保護した。回折データはダイヤモンド光源で収集し、XDSを使用して処理した(Kabsch,Wolfgang(2010)Acta Crystallographica Section D 66,125−132)。回折データの統計を以下の表でまとめる:

背景
Hek Id1レポーター遺伝子アッセイは、クローン12 Hek293−Id1レポーター細胞を使用する。この細胞株に対してId1転写因子で安定的に遺伝子移入を行った。Id1は、BMPシグナル伝達経路における転写因子である。グレムリンはBMPに結合して、その受容体への結合を妨げ、レポーター遺伝子からのルシフェラーゼシグナルを減少させることが知られている。従って、このレポーターアッセイを使用して、抗グレムリン抗体をスクリーニングし、グレムリンとBMPとの相互作用を遮断するものが存在するか否かを確認することが可能である。この相互作用の遮断がある場合、これらの細胞においてルシフェラーゼシグナルの回復が見られる。
クローン12細胞は、10%FCS、1xL−グルタミン及び1xNEAAを含有するDMEM中で培養した。細胞がId1遺伝子発現を失わないことを確実にするために、ハイグロマイシンB(200μg/mL)の存在下でも細胞を増殖させた。細胞は、0.5%FCS、1xL−グルタミン及び1xNEAAを含有するDMEM中でアッセイした。ハイグロマイシンBは、細胞をアッセイしている短時間では不要である。
BMP4/7に対するブロッキング活性を確認するために、グレムリン1の全長及び切断型をHek−Id1レポーター遺伝子アッセイで試験した。
グレムリン1は、Hek−Id1レポーター遺伝子アッセイにおいてBMP 4/7シグナル伝達を阻害可能であった。
例1に記載のように精製グレムリン−1を使用する免疫付与により、及びライブラリパニングにより、抗グレムリン−1抗体を得た。ライブラリは、献血から増幅されたV領域を有するナイーブヒトライブラリとして社内で作製された。
例3に記載のHek−Id1レポーター遺伝子アッセイを使用して、及びSMADリン酸化を測定することによって、抗体をスクリーニングした。SMAD1、5及び8は、BMPシグナル伝達時にリン酸化される。従って、グレムリン−1の阻害剤はSMADリン酸化を増加させる。
Hek−Id1レポーター遺伝子アッセイにおいて、免疫付与由来の抗体による明らかなヒットはなかった(BMP4/7ヘテロ二量体に対して10倍過剰の抗体を試験)。結果を図1で示す。

Biacore T200 system、GE Healthcare Bio−Sciences ABの表面プラズモン共鳴(SPR)技術を使用したビアモレキュラー(biamolecular)相互作用分析によって、ヒトグレムリン1に対する抗グレムリンmIgGの親和性を測定した。固定化抗マウスFc表面により抗グレムリンmIgGが捕捉され、捕捉されたmIgGに対してグレムリン1を滴定した。
5回の測定に対する平均KD値と見なされる結合親和性は、100pMを下回ることが分かった。
8.1材料及び方法
ラット骨折モデル及び薬物投与
長骨の部分的欠損モデルは、骨の治癒及び再生の研究に広く使用されてきた(Sato et al;2014)。本研究では、10週齢の雄ラットにおいて3mmの大腿骨欠損を作製し、8穴PEEKプレート(RIS.602.105,RISystem、Switzerland)を使用して安定化させた。骨に穴をあけてネジで固定する前に、骨に対してプレートを骨幹の中央に鉗子により固定した。0.44mmのギグリーソーを使用して、3mmの骨折間隙を作製した。欠陥のサイズ/一貫性/固定化は、Faxitron(MX−20−DC5,Faxitron Bioptics LLC,USA)を使用してX線イメージングによって品質管理し、この時点を第0日として定めた。
マイクロCT(SkyScan 1076)を使用して17.2μmの解像度で大腿骨(骨折側)をスキャンした。骨折部を中央にして仮骨のおよそ15mmの領域を得た。走査はSkyscan NRECONソフトウェア(1.7.10)を使用して再構築し、次に再構築したスライスをさらにセグメント化して、大腿骨骨折欠損の中間点から計算された3mmの定められた領域で固定ピンを取り除いた。3Dにおける骨折仮骨の組織形態計測分析は、SkyScanソフトウェア(v.1.13.1)によって実行した。3mmの大腿骨骨折欠損内の中間点を決定し、測定した各四肢について、中間点に対して1.5mm遠位及び近位のスライスをセグメント化した。続いて、再構築したデータセットの2値化及びセグメント化を2つの定義済みしきい値に従って実行し、一方は低石灰化仮骨を表し(従って新たに形成された骨を定量)、他方は成熟骨を定める。大腿骨欠損の不完全な架橋の基準を満たすことに基づいて低レスポンダーとして、又は骨折部位の架橋を示す高レスポンダーとして分類された動物の大腿骨において、これらのデータのさらなるセグメント化を行った。
大腿骨を24時間にわたり10%中性緩衝ホルマリン中で固定し、脱水し、低温にてメタクリル酸メチル(MMA)中で包埋した。厚さ50μmの切片をトルイジンブルーで染色して、治癒ギャップ欠損部(gap defect)の骨成分を定量した。組織形態計測パラメータは、骨折欠損部位の海綿骨において測定した。画像解析により測定を行った。
結果は平均値±SDとして提示した。統計分析は、特に明記されない限り、GraphPad Prismソフトウェアで両側マンホイットニーU検定を使用して実行した。
試験の生存期間中に得られたX線画像の分析から、抗グレムリン−1抗体が骨折治癒を加速し、対照群及び処置群では25日後に有意に分かれることが示された(P<0.05)。この効果は、本試験の残りの期間について明らかであった(図5)。
中和抗グレムリン−1抗体を使用してグレムリン−1活性を阻害した結果、骨折修復が加速され、25日後には対照群と処置群との間で有意差が明らかになった(抗体3回投与)。さらに、骨折部位の末端分析から、低レスポンダー動物における骨組織の形成促進が示されたが、これらの動物ではそうでない場合は非癒合が生じたであろうと思われる。従って、グレムリン−1活性の阻害は、非癒合骨折の予防又は処置のための有望な治療法であり、非癒合発生の傾向がある骨折、例えば、脛骨、橈骨遠位端、大腿骨頸部及び舟状骨の処置のために特に価値があり得る。

- 骨折又は骨欠損が癒合遷延又は非癒合骨折である、請求項1〜7のいずれかに記載のグレムリン−1活性の阻害剤。
- 骨折又は骨欠損が、骨の完全性に影響を与える疾患に関連する、請求項1〜8のいずれかに記載のグレムリン−1活性の阻害剤。
- 骨の完全性に影響を与える疾患が、骨粗しょう症、骨形成不全症、糖尿病、骨のページェット病、関節リウマチ、強直性脊椎炎、多発性骨髄腫、原発性骨癌、骨に転移する癌、広汎性特発性骨増殖症、骨髄炎、腎疾患、デュシェンヌ型筋ジストロフィー又は重症型サラセミアである、請求項9に記載のグレムリン−1活性の阻害剤。
- 骨折又は骨欠損の処置のための医薬の製造のためのグレムリン−1活性の阻害剤の使用。
- 阻害剤が、抗グレムリン−1抗体又はその機能的活性断片、変異体若しくは誘導体である、請求項11に記載の使用。
- 機能的活性抗体断片が、Fab、Fab’、F(ab’)2、Fv又はscFvである、請求項12に記載の使用。
- 抗体が、配列番号3、4、5及び6から選択される少なくとも1つのHCDR配列及び/又は配列番号7、8及び9から選択される少なくとも1つのLCDR配列を含む、請求項12〜13のいずれかに記載の使用。
- 抗体が、配列番号10の重鎖可変領域(HCVR)及び/又は配列番号11の軽鎖可変領域(LCVR)を含む、請求項12〜14のいずれかに記載の使用。

- 機能的活性抗体断片が、Fab、Fab’、F(ab’)2、Fv又はscFvである、請求項22に記載の方法。
