はその医薬的に許容される塩と白金又はパラジウム化合物との複合体(特に、酸化グルタチオンの二ナトリウム塩とシス−ジアミノジクロロ白金から成る複合体)の開発も知られている[ロシア特許第2144374(C1)号、2000年1月20日公開;ロシア特許第2153350(C1)号、2000年7月27日公開;米国特許第6,312,734(B1)号、2001年11月6日公開]。
、サイトカイン及び造血因子の内因性産生を制御する。アスコルビン酸、ジメチルスルホキシド、イノシン(ヒポキサンチン−9−D−リボフラノシド)、シスタミン(2,2’−ジチオビス[エチルアミン])、白金化合物(例えば、塩化白金)が、酸化グルタチオンの作用の延長剤として用いられる。
強作用を有する新規な医薬組成物を提供することである。特に、本目的は、より少ない用量を用いて、必要とされる治療効果を、それを必要とする患者に吸入、経腸、非経口によって、単独及び他の薬理活性物質と組み合わせての単一剤形の両方での外用投与で投与された場合に得ることを可能とする目的で、薬物動力学を最適化し、最終的には、GSSGの薬物動力学を最適化することである。薬理活性物質は、薬物動力学及び/若しくは薬物動態の最適化、並びに/又は毒性の低下が実現されることになる、抗微生物薬及び抗ウィルス薬、抗凝固剤、第Xa因子阻害剤;細胞膜イオンチャネルの作用の修飾剤;他の医薬品を含むいかなる薬物療法剤群から選択されてもよい。
− 抗凝固剤、第Xa因子阻害剤、特に、アミジン塩酸塩;
− 抗微生物薬及び抗ウィルス薬、特に、モキシフロキサシン、抗狂犬病ワクチンの抗原性物質、インターフェロンα;
− 細胞膜カルシウムチャネルの作用の修飾剤、特に、ニフェジピン。
グルタチオンジスルフィド(又は酸化グルタチオン、GSSG)は、トリペプチドグル
タチオン、γ−グルタミルシステイニルグリシンの二量体であり、前記トリペプチドの二分子が、システイン残基間のジスルフィド共有結合を介して互いに連結されている。本発明によると、アルカリ金属又はアルカリ土類金属との塩の形態であるグルタチオンジスルフィドは、本技術分野において公知のいかなる方法によって調製されてもよい[ロシア特許第2144374(C1)号、2000年1月20日公開]。

方法A
L−グルタチオンから誘導したグルタチオンジスルフィドのナトリウム塩の溶液に(例2)、手順(例1)に従って合成したグルタチオンジスルフィドS−オキシドを添加した。グルタチオンジスルフィドS−オキシドの量は、全組成物に対して0.1〜10重量%であってよい。実際の実施形態では、特に例3及び4では、グルタチオンジスルフィドS−オキシドの量は、全組成物の重量に基づいて、それぞれ2%及び4%であった。提供された方法により、グルタチオンジスルフィドS−オキシドの含有量を高い精度で制御することが可能となる。
水酸化ナトリウム水溶液中のグルタチオンジスルフィドナトリウム塩の得られた溶液に、低下された温度で、通常は0〜5℃で、過剰の過酸化水素を添加して、グルタチオンジスルフィドS−オキシドをin situで生成した。1つの実施形態(例5)では、+3℃以下の温度で、6%の過酸化水素の127gを添加した。グルタチオンジスルフィドS−オキシドの量は、全組成物に対して5重量%であった。

還元L−グルタチオン物質(GSH)100gの100mlの水による溶液に、150mlの酢酸中30%過酢酸溶液を、撹拌しながら、0〜5℃の温度で30〜40分間かけて滴下した。滴下後、反応物を5℃以下の温度で1時間にわたって撹拌し、その後、凍結し、24時間にわたって凍結乾燥した。110gの物質が白色フォームとして得られ、それは、HPLC分析によると、成分の混合物を含有している(40% GSO3H、55
% GS(O)SG、5% GS(O2)SG)。
7Lの水による2760gの還元L−グルタチオンの懸濁液に、2245gの16%水酸化ナトリウム溶液を、17℃以下の温度で撹拌しながら添加した。グルタチオンが完全に溶解した後、混合物を冷却し、2546gの6%過酸化水素を、30〜50ml/分の速度で、撹拌しながら+15℃以下の反応物温度で添加した。過酸化物の添加後、得られた溶液を、所定の温度でさらに1時間にわたって撹拌した。反応の完了後(HPLCで制御)、11.5Lの水に2.95kgのグルタチオンジスルフィド二ナトリウム塩を含有する溶液が得られ、3℃に冷却した。生成物、グルタチオンジスルフィド二ナトリウム塩の化学純度は、98.5%超であり(HPLCで制御)、生成物単離のための追加の手順の必要はない。
例2で作製したグルタチオンジスルフィド二ナトリウム塩(11.5Lの水に2.95kg)に、60gの例1に従って得たグルタチオンジスルフィドS−オキシドを、3〜5℃の温度で、5分間にわたって充分に混合しながら添加し、この溶液を、120分間にわたって5℃の温度で放置し、その後凍結乾燥した。
120gの例1に従って作製したグルタチオンジスルフィドS−オキシドを、例2で作製したグルタチオンジスルフィド二ナトリウム塩(11.5Lの水に2.95kg)に、5分間にわたって充分に混合しながら添加し、この溶液を、120分間にわたって5℃の温度で放置し、その後凍結乾燥した。

本出願の例3に従って得た組成物を、抗凝固剤の第Xa因子阻害剤である薬理活性剤アミジン塩酸塩の治療有効性を高める能力について調べた。試験物質のアミジン塩酸塩(例えば、ユーラシア特許第015918(B1)号、2011年12月30日公開の例2に従って得られる)又はアミジン塩酸塩物質とアジュバント(アジュバントは、本出願の例
3で得た組成物であった)との混合物を、尾の基部へ1/3近づいた領域の外側尾静脈に、30G針を取り付けた1mlのインスリンシリンジで静脈内投与した。各動物に対する個々の投与量は、体重に基づいて算出し、各々の体重測定後に補正した。物質の投与は1回とした。前記混合物を、アミジン塩酸塩物質とアジュバントとの混合物の動物への静脈内投与用に調製した。この目的のために、アミジン塩酸塩物質及びアジュバントを、別々に蒸留水に溶解し、次にこれらの溶液を混合した。溶液の調製は、動物への投与の直前に行い、調製後の10分以内に注射した。ラットに対する投与量は、0.31〜0.42mlであった。

33591、に対して調べた。
測定することによって評価した。4×106の微生物細胞を含有する懸濁液のアリコートを、0.15MのNaClを含有しpH7.4の10mMリン酸ナトリウム緩衝液中の滅菌1%アガロース溶液の10mlと、42℃で混合する。得られた混合物を、直径90mmの滅菌プラスチックペトリ皿に注ぎ入れ、固化するまで室温で放置する。pH7.4の10mMリン酸ナトリウム緩衝液による製剤の段階希釈物である5μlの量の分析サンプルを、アプリケータによって作製したウェル(直径3mm)に添加し、エアーサーモスタット中、37℃で3時間インキュベートした。次に、6%TGSを含有する1%アガロースをプレートに注ぎ入れ、37℃で18時間インキュベートする。増殖阻害ゾーン(ウェル周囲の微生物のいないゾーン)の直径を、抗微生物作用の1慣用単位を0.1mmとして取って、測定値からウェル自体の直径に相当する30慣用単位を差し引くことによって測定する。用いた製剤の濃度は、64μg/ml、32μg/ml、16μg/ml、8μg/ml、4μg/ml、2μg/ml、1μg/mlであった。

乾燥濃縮精製不活化細胞由来抗狂犬病ワクチン及び本出願の例3に従う組成物の組み合わせの調製
PBS中0.5mg/mlの滅菌水酸化アルミニウム溶液を調製した。本出願の例3に従って得た組成物の12gを、得られた溶液の60mlに添加した。得られた溶液を、細孔径0.44μmのフィルターに通すことによって滅菌した。0.5mg/mlの水酸化アルミニウム濃度のPBS中、20mg/ml、10mg/ml、5mg/ml、1mg/mlの濃度の希釈溶液(AD)を、得られた溶液から調製した。

本実験の目的は、本出願の例3で得た組成物の、細胞膜イオンチャネルの作用及びカルシウムチャネル阻害剤の作用に対する効果を試験することである。
試験化合物及び組成物を、+4℃で保存し、これらの物質を、実験の開始直前に脱イオン水(super Q)に溶解した。調製した溶液を、+4℃で5時間以下にわたって保存した。化合物を、実験すべき最終濃度まで細胞培養培地に添加した。

200μMのATPをラット腹膜マクロファージのインキュベーション媒体に添加することによって、P2u受容体活性化によって引き起こされる貯蔵庫からのCa2+動員に主として伴う初期の短期ピークと、顕著な長期の「定常」フェーズとから成る2フェーズのCa2+シグナルが引き起こされる。この定常フェーズは、外部媒体からのCa2+の流入に起因し、P2u及びP2z受容体が同時に活性化されていることを反映しているものと推定される。図4(1)は、生理食塩水中の40〜50のマクロファージの集団における、細胞外ATP(200μM)によって誘導された特徴的なCa2+シグナルを示す
。
のアンタゴニストとして及びニフェジピンの毒性を中和する対応策として作用する方向へと、低下又は阻害することができる。
例5に従って得た1gのサンプル(グルタチオンジスルフィドS−オキシド含有量5%)を、水(9ml)に溶解した。得られた溶液へ、還元L−グルタチオンのナトリウム塩の溶液(2.5mg/ml)の1mlを撹拌しながら添加した。この反応物を、5分間撹拌し、HPLCによって分析した。得られた反応溶液中には、グルタチオンジスルフィドS−オキシドは検出されなかった。
試験化合物として以下を用いる:
1− グルタチオンジスルフィドS−オキシド(例1で得た化合物);
2− グルタチオンジスルフィド(例2で得た化合物);
3− グルタチオンジスルフィドS−オキシドとグルタチオンジスルフィドとの組成物(例5で得た組成物);
4− グルタチオンジスルフィドS−オキシドとグルタチオンジスルフィド及び金属、白金化合物Pt−Sシスプラチン、との組成物(例6で得た組成物)。
No.1− 試験化合物の溶媒(生理食塩水)の注射を受けた未処理動物(溶媒コントロール);
No.2− CPを受け、次に治療剤として生理食塩水を受けた動物(コントロール);
実験群:
No.3− 10日間にわたって、毒性剤CPを投与した30分後に、生理食塩水中の試験化合物1を、10mg/kgの用量での腹腔内投与で受けた動物;
No.4− 10日間にわたって、毒性剤CPを投与した30分後に、生理食塩水中の試験化合物2を、0.1mg/kgの用量での腹腔内投与で受けた動物;
No.5− 10日間にわたって、毒性剤CPを投与した30分後に、生理食塩水中の
試験化合物3を、10mg/kgの用量での腹腔内投与で受けた動物;
No.6− 10日間にわたって、毒性剤CPを投与した30分後に、生理食塩水中の試験化合物4を、10mg/kgの用量での腹腔内投与で受けた動物。
毒性物質の作用に対する許容性を与える複合的な分子反応の実験の結果は、グルタチオンジスルフィドS−オキシド(1)、グルタチオンジスルフィド(2)、それらの組成物(3及び4)による、生体異物解毒の第2相の酵素であるグルタチオンレダクターゼ(XE1.6.4.2)、グルタチオンペルオキシダーゼ(XE1.11.1.9)、グルタチオン−S−トランスフェラーゼ(XE2.5.1.18)の活性、及びそれらに伴う還元グルタチオンの交換を誘導する能力を示している(表6)。

スルフィド及びグルタチオンジスルフィドS−オキシドの組成物が生体異物解毒の第2相の酵素の活性を誘導する能力を高め、それらに伴う重要な代謝物還元グルタチオンの交換の強度を増加させた。
グルタチオンジスルフィドS−オキシド及びその組成物(例1、2、5、6)のインターフェロンの抗ウィルス作用に対する効果の実験を、感染細胞の培養物に対して行う。
Viral Preparations(MRIVP)から入手した、腫瘍、性病、肝炎、結核、遺伝的及び先天性異常を有しない28歳女性からの11週ヒト中絶胚の肺細胞由来のヒト胚性肺の二倍体細胞株である。
Scientific Research Institute for Standardization and Control of Medical Biological Preparations(SISC)と共に、MRIVP RAMSにおいて、免疫生物学的製剤の調製について認可されている。
を、等量部の0.02%Versene液及び0.25%トリプシン溶液から成る特別媒体を用いて懸濁させた。これを行うために、細胞の単層を形成したバイアルから完全増殖培地を排出し、単層を特別媒体(トリプシンを含むVersene液)で2回洗浄し、37℃で5分間インキュベートした。この間に、線維芽細胞の単層がプラスチックから剥離した。剥離した細胞を、完全増殖培地で希釈し、複数回のピペット操作によって細胞集合体をばらした。滅菌遠心分離チューブに細胞を移し、1200rpmで10分間遠心分離
した。上澄を排出し、細胞を完全増殖培地に移した。次に、Gorjaevチャンバーで細胞数を計数し、この実験で用いた。
実験のために、滅菌条件下でガラスアンプルに密封包装された凍結乾燥水疱性口内炎ウイルス(VSV)を用いた。
0.2mlの体積中5×104細胞/ウェルの濃度の完全培地1に懸濁させて調製したЛ−68株の細胞を、96ウェルプレート(「Costar」型)に添加した。この後、プレートを、5%CO2を含有する雰囲気のCO2インキュベータ中、37℃で1日間インキュベートした。この間に、細胞はウェルを覆い、連続単層を形成した。1日後、培地を滅菌条件下でデカントし、予め調製しておいたウィルスの2倍希釈物を、4つの反復サンプルとしてプレートのウェルに添加した。VSVウィルスを、0.2mlの体積の完全培地に添加した。次に、上記で述べた条件下でプレートをインキュベートした。インキュベーションの終了後(1日後)、培地をデカントし、20%メタノール中のクリスタルバイオレットの0.2%溶液0.05mlを、ウェルに添加した。10分後、染料を取り除き、プレートを水流下で洗浄し、乾燥した。さらに、染料を溶液中に溶出させるために、0.1mlの溶解緩衝液をプレートに添加した。染色の強度を、マイクロプレートリーダー上、595nmで記録した。
完全増殖培地中、標準活性サンプルの2倍希釈物(予測される力価の上及び下)を調製し(42−28−119−96P;L.A.Tarasevich SISC)、その活性は、国際単位IUで表した。標準の希釈は、0.1mlの体積で96ウェルプレート(「Costar」型)中で行い、各希釈に対して少なくとも4つのウェルを用いた。プレートの1つの列は、培地のコントロール(4ウェル)及びVSVウィルスの用量のコントロール(4ウェル)に残した。これらのウェルに、0.1mlを添加した。標準の希釈後、0.1mlの体積中5×−104細胞/ウェルの濃度の完全培地1に懸濁させて調製したЛ−68株の細胞を、プレートに添加した。この後、実験画分:
1− グルタチオンジスルフィドS−オキシド(例1で得た化合物);
2− グルタチオンジスルフィド(例2で得た化合物);
3− グルタチオンジスルフィドS−オキシドとグルタチオンジスルフィドとの組成物(例5で得た組成物);
4− グルタチオンジスルフィドS−オキシドとグルタチオンジスルフィド及び金属、白金化合物Pt−Sシスプラチン、との組成物(例6で得た組成物)
を、希釈標準を含む列の一部に、ある特定の時間間隔で、0.0015μmol/ml、0.015μmol/ml、及び0.15μmol/mlの濃度で添加した。次に、各プレートを、5%CO2を含有する雰囲気のCO2インキュベータ中、37℃で1日間インキュベートした。この間に、細胞はウェルを覆い、連続単層を形成する。1日後、完全増殖培地を滅菌条件下でデカントし、所定の感染力価のVSVウィルスを各プレートのウェルに添加した。完全培地中、0.2mlの体積のVSVウィルスを添加した。VSVウィルスを含まない同じ培地の0.2mlを、培地コントロールとしてウェルに添加した。この後、各プレートを、上記で述べた条件下でインキュベートした。インキュベーションの終了後(1日後)、培地をデカントし、20%メタノール中のクリスタルバイオレットの0.2%溶液0.05mlを、ウェルに添加した。10分後、染料を取り除き、ディッシュを水流下で洗浄し、乾燥した。培地コントロールウェルでは、着色した単層は、破壊の徴候を示していないはずである。さらに、染料を溶液中に溶出させるために、0.1mlの溶解緩衝液をプレートに添加した。染色の強度を、マイクロプレートリーダー上、595nmで記録した。
細胞を各試験化合物と共に予めインキュベートすることは、インターフェロンの抗ウィルス作用に対してまったく影響を与えないことが実験的に確立された。

てより低い治療用量でインターフェロンαを用いることの可能性は、インターフェロンαに対する様々な副作用及び毒性の用量依存性の反応を大きく低減することを可能とするものである。
Claims (23)
- 感染性疾患及び非感染性疾患の治療において、薬理活性化合物の用量関連毒性を排除するため及び治療活性を向上させるための医薬組成物であって、該医薬組成物が、グルタチオンジスルフィド又はその医薬的に許容される有機若しくは無機塩、及び以下の構造のグルタチオンジスルフィドS−オキシド:
又はその医薬的に許容される有機若しくは無機塩を含む、医薬組成物。 - グルタチオンジスルフィドS−オキシドの量が、前記全組成物に対して0.01〜10重量%である、請求項1に記載の組成物。
- 前記組成物が、金属(Me)をさらに含み、該金属(Me)が、Me−S−グルタチオン結合を含有する配位化合物として示される、請求項1に記載の組成物。
- 前記金属が、白金族から選択される、請求項3に記載の組成物。
- 前記金属が、白金である、請求項3に記載の組成物。
- 前記組成物中のd−金属の量が、前記組成物の1kgあたり1×10−10モル〜1×10−3モルの範囲内である、請求項3に記載の組成物。
- 前記組成物中のd−金属の量が、前記組成物の1kgあたり1×10−5モルである、請求項3に記載の組成物。
- 感染性疾患及び非感染性疾患の治療において、薬理活性化合物の用量依存性の毒性を排除するため及び治療活性を向上させるための薬理学的組み合わせであって、該薬理学的組み合わせが、請求項1〜7のいずれか一項に記載の組成物、及び抗凝固剤、第Xa因子阻害剤、抗微生物剤又は抗ウィルス剤、カルシウムチャネル阻害剤の群から選択される薬理活性化合物を含む、薬理学的組み合わせ。
- 前記組み合わせを、血栓症の治療法に用いることができ、前記薬理活性化合物が、抗凝固剤、第Xa因子阻害剤のアミジン塩酸塩である、請求項8に記載の組み合わせ。
- 前記組み合わせを、大腸菌(Escherichia coli)、シュードモナス・エルジノーサ(Pseudomonas aeruginosa)、アシネトバクター・バウマンニ(Acinetobacter baumannii)
、及びリステリア・モノサイトゲネス(Listeria monocytogenes)EGD、スタフィロコッカス・アウレウス(Staphylococcus aureus)、MRSA−メチシリン耐性スタフィロ
コッカス・アウレウスを含むグラム陰性細菌並びにグラム陽性細菌によって引き起こされる感染性疾患の治療に用いることができ、前記薬理活性化合物が、抗微生物剤のモキシフロキサシンである、請求項8に記載の組み合わせ。 - 前記薬理活性化合物が、広範囲の抗ウィルス作用を有する抗ウィルス剤−インターフェロンアルファである、ウィルス性疾患の治療法のための、請求項8に記載の組み合わせ。
- 前記薬理活性化合物が、抗狂犬病ワクチンの抗原性物質である、狂犬病を予防するための、請求項8に記載の組み合わせ。
- 感染性疾患及び非感染性疾患の治療において,薬理活性化合物の用量関連毒性を排除するため及び治療活性を向上させるための医薬であって、該医薬が、治療有効量の請求項1〜7のいずれか一項に記載の少なくとも1つの組成物を、医薬的に許容される賦形剤と共に含む、医薬。
- 感染性疾患及び非感染性疾患の治療において、薬理活性化合物の用量関連毒性を排除するため及び治療活性を向上させるための医薬であり、該医薬が、治療有効量の請求項8〜12のいずれか一項に記載の少なくとも1つの組み合わせを、医薬的に許容される賦形剤と共に含む、医薬。
- 感染性疾患及び非感染性疾患の治療において、薬理活性化合物の用量関連毒性を排除するため及び治療活性を向上させるための、外用、吸入、経腸、又は非経口投与用に製造することができる、請求項13又は14に記載の医薬。
- 感染性疾患及び非感染性疾患の治療において、薬理活性化合物の用量関連毒性を排除するため及び治療活性を向上させるための、請求項1〜7のいずれか一項に記載の医薬組成物の使用。
- 前記組成物が、さらに、好ましくは白金族からの、さらにより好ましくは白金であるd−金属(Me)を含み、該d−金属が、Me−S−グルタチオン結合を含有する配位化合物の形態で示され、患者に投与される前記d−金属配位化合物の量が、10−3〜10−15mol/kg体重である、請求項16に記載の使用。
- 感染性疾患及び非感染性疾患の治療において、薬理活性化合物の用量関連毒性を排除するため及び治療活性を向上させるための、請求項8〜12のいずれか一項に記載の薬理学的組み合わせの使用。
- 前記薬理活性化合物が、血栓症の治療法のための、抗凝固剤、第Xa因子阻害剤のアミジン塩酸塩である、請求項18に記載の使用。
- 前記薬理活性化合物が、大腸菌、シュードモナス・エルジノーサ、アシネトバクター・バウマンニ、及びリステリア・モノサイトゲネスEGD、スタフィロコッカス・アウレウス、MRSA−メチシリン耐性スタフィロコッカス・アウレウスを含むグラム陰性細菌並びにグラム陽性細菌によって引き起こされる感染性疾患の治療のための、抗微生物剤のモキシフロキサシンである、請求項18に記載の使用。
- 前記薬理活性化合物が、ウィルス性疾患の治療のための、抗ウィルス剤のインターフェロンアルファである、請求項18に記載の使用。
- 前記薬理活性化合物が、狂犬病を予防するための抗狂犬病ワクチンの抗原性物質である、請求項18に記載の使用。
- 前記薬理学的組み合わせの投与が、吸入、経腸、又は非経口投与によって行われる、請求項18に記載の使用。
