腫瘍制御における免疫系、特に、T細胞媒介性細胞傷害の役割はよく認識されている。T細胞が、疾患の早期及び末期の両方で、腫瘍増殖及び癌患者の生存率を制御するというエビデンスが益々増えている。しかし、腫瘍特異的T細胞応答は、癌患者において増大させ、維持することは困難である。
PD−L1とは対照的に、細胞傷害性Tリンパ球抗原4(CTLA−4)は、調節T細胞により構成的に発現され、活性化T細胞上で上方制御される。CTLA4は、共阻害剤としての役割を果たし、CD28媒介性T細胞活性化の後にT細胞応答を阻止する。T細胞受容体(TCR)結合の後、CTLA4は、ナイーブ及びメモリーT細胞の早期活性化の大きさを調節すると共に、抗腫瘍免疫及び自己免疫の両方に作用する中央阻害経路の一部であると考えられる。CTLA4は、主としてT細胞上に発現され、そのリガンドCD80(B7.1)及びCD86(B7.2)の発現は、抗原提示細胞、T細胞、及びその他の免疫媒介細胞に大部分が限定される。腫瘍浸潤免疫細胞(IC)上のCD80又はCD86に対するCTLA−4の結合は、T細胞活性化の阻害を引き起こす。CTLA4シグナル伝達経路を遮断する拮抗性抗CTLA4抗体は、T細胞活性化を増強することが報告されている。こうした抗体の1つであるイピリムマブは、転移性黒色腫の治療のために、2011年にFDAにより承認された。別の抗CTLA4抗体であるトレメリムマブは、進行性黒色腫の治療のために、第III相試験で試験されたが、当時の標準治療(テモゾロミド又はダカルバジン)と比較して、患者の全生存率を有意に増加しなかった。
PD−L1活性に結合して、PD−L1活性(例えば、PD−1及び/又はCD80との結合)を阻害する抗体は、膀胱癌(例えば、UC)の治療に有用である。

局所進行性UCは、通常、根治的膀胱切除術とネオアジュバント及びアジュバント化学療法により治療されている。プラチナ薬を用いた全身併用化学療法は、ネオアジュバント及びアジュバントセッティングの双方における選択レジメンである。ネオアジュバント又はアジュバント療法後12ヵ月超の時点で疾患の再発を有する患者は、後述する通り、第1選択(1L)レジメンとしてプラチナベースの療法による再治療を受ける可能性がある。しかし、約25%の患者が、療法の1年以内に疾患の再発を有する。これらの患者は、標準的プラチナベースの療法に対して適格ではないため、限定的な治療オプションしかない。
局所進行性又は転移性UCを有する患者の場合、30年近く前に導入された全身シスプラチン含有併用化学療法が、現在のところ標準治療(SoC)である。第1選択(1L)化学療法は、シスプラチン/ゲムシタビン又はメトトレキサート、ビンブラスチン、ADRIAMYCIN(商標)(ドキソルビシン)、及びシスプラチン(MVAC)を含む。これらの治療レジメンは、1L治療レジメンにおいて、46%〜60%の客観的奏効率(ORR)、7〜8ヵ月の無増悪生存(PFS)中央値、及び13〜15ヵ月の全生存率(OS)中央値をもたらした。しかし、患者の約半数が、低いパフォーマンスステータス、腎臓機能障害、又は共存症のためにシスプラチン含有化学療法には不適格であり;こうした患者には、概して、カルボプラチンベースのレジメン又は単剤のタキサン若しくはゲムシタビンを用いた緩和療法が投与される。これらの治療は、シスプラチンベースの化学療法ほど有効ではなく、約30%〜40%の客観的奏効率(ORR);約3〜6ヵ月のPFS中央値;及びほぼ10ヵ月以下のOS中央値をもたらした。
ILセッティングの臨床利益にもかかわらず、化学療法を中断した後の患者の場合、化学療法に初め応答した患者でも、疾患の進行が常に起こる。米国では、年間約6,500人の患者が、局所進行性又は転移性UCのために第2選択(2L)療法に進む。
本明細書に記載の方法は、膀胱癌、例えば、UCを有し、治療を必要とする対象に、対象の膀胱癌、例えば、UCを治療するのに有効な量で、デュルバルマブなどの抗PD−L1抗体の投与に伴う医学的及び臨床的利益を提供する。UCなどの膀胱癌の単剤療法又は第1選択(1L)治療としての抗PD−L1抗体、とりわけ、デュルバルマブの治療利益は、本明細書に記載し、実施例で実証する。
治療方法の選択過程で、膀胱癌(例えば、UC)に罹患している対象をPD−L1ポリヌクレオチド又はポリペプチド発現について試験してもよい。無視できる程度若しくは低PD−L1を発現する(例えば、Ct若しくはIHC−Mスコアにより定義される通り)腫瘍を有する、又は標準対照レベルよりも低い(低)若しくは検出不可能なレベルのPD−L1を有するとして同定された対象は、デュルバルマブなどの抗PD−L1抗体、又は抗PD−L1抗体とトレメリムマブなどの抗CTLA4抗体の併用による治療に対して応答性であると同定される。こうした対象は、トレメリムマブと組み合わせて、デュルバルマブなどの抗PD−L1抗体又はその抗原結合断片を投与される。
この実施例は、デュルバルマブの臨床薬理学の概要を提供すると共に、抗PD−L1抗体又はその抗原結合断片が、UCなどの膀胱癌を有する対象の尿路上皮癌(UC)などの膀胱癌を治療する記載の方法での使用に対するその好適性を支持する特性及び特徴を呈示することを実証する。
デュルバルマブの薬物動態(PK)は、固形腫瘍を有する1,337人の患者で試験し、患者は、Q2Wに投与される0.1〜10mg/kgの範囲、又は3週間毎(Q3W)に投与される15mg/kg、又は4週間毎(Q4W)に投与される20mg/kgの用量でデュルバルマブを受けた。10mg/kg Q2Wでのデュルバルマブ処置の後、PKは、線形範囲内で、典型的なモノクローナル抗体(mAb)と合致すると予想された。PK解析の主要な結果から、下記のことが明らかにされた:
・デュルバルマブは、恐らく可飽和標的介在性クリアランスのために、用量<3mg/kgで非線形PKを示し、用量≧3mg/kgで線形PKを示した。濃度−時間曲線(AUC;0〜14日)下の面積は、0.1〜3mg/kgの用量範囲の場合、用量比例性を超えて増加し、用量≧3mg/kgでは、用量に比例して増加した。最大濃度(Cmax)は、試験した用量範囲内で用量比例的に増加した。
・反復投与の約16週目後に定常状態が達成された。試験した用量範囲にわたって、Cmaxの全身貯留及びトラフ濃度(Ctrough)は、それぞれ、0.64〜1.87倍及び3.39〜4.93倍であった。
・平均全身線形クリアランス及び中心の分布容積は、226mL/日及び3.51Lであり、それぞれ29.3%及び21.2%という控え目な対象間変動を伴った。予想半減期は、10mg/kg Q2Wの用量で投与されるデュルバルマブ後約21日であった。
・非線形クリアランスの場合の半値最大容量を表すデュルバルマブ濃度(Km)は、約0.5μg/mLであった。この推定値に基づき、デュルバルマブの約50μg/mL濃度で、>99%標的飽和が予想された。10mg/kgの用量は、>95%の患者において標的曝露を達成すると予想された。
・患者のベースラインデモグラフィックス又は疾患の特徴に基づいて提案されたデュルバルマブの用量10mg/kg Q2W IVに、用量調節は推奨されなかった。集団PK分析によれば、デュルバルマブのPKに対して、年齢、体重、性別、陽性抗薬物抗体(ADA)、ステータス、アルブミンレベル、LDHレベル、クレアチニンレベル、可溶性PD−L1(sPD−L1)レベル、腫瘍タイプ、人種、軽度から中度の腎障害、軽度の肝障害、又はECOGステータスの臨床的に有意な作用はなかった。
・集団PKモデル化は、750mg Q2W IVの有望な固定投与レジメン又は同等であるが、頻度が低い1500mg Q4W IVの固定投与レジメンの使用を支持した。
全体として、デュルバルマブは、低い発生率(2.6%;37/1,412)の治療下発現ADAを示し、10mg/kg Q2W IV用量後にPKに対して臨床的に有意な作用はなかった。
標的結合:遊離可溶性PD−L1(sPD−L1)の用量依存性抑制が、デュルバルマブのIV投与後に観察された。デュルバルマブの投与後、用量レベル≧0.3mg/kg前後で完全なsPD−L1抑制が観察された。10mg/kg Q2Wでデュルバルマブを投与後、>93%の患者が、投与間隔全体を通して完全なsPD−L1抑制を示した。
抗PD−1/PD−L1療法の最適な期間はまだ不明だが、臨床試験における抗PD−1/PD−L1処置の期間は、12ヵ月、24ヵ月以下、又は疾患の進行までと、変動する。理論に拘束されることは意図しないが、抗PD−1/PD−L1抗体は、腫瘍細胞(TC)又は腫瘍関連免疫細胞(IC)上でのPD−L1の発現を改変することなく、主としてPD−1とPD−L1の相互作用を生物学的に阻止するため、連続的な遮断によって、T細胞の疲弊を最適に防ぐことができる。デュルバルマブを含む以前のプロトコルは、再処置のオプションを含む限定的な処置期間を使用した。許容可能な安全性プロフィールが長期の投与について実証され、新たなAEの発生は、12ヵ月の処置の前に水平状態になることから、全てのデュルバルマブプロトコルが、疾患の進行まで継続的な処置を包含する。全てを考慮すると、デュルバルマブから利益を受けている患者が、12ヵ月を超えて処置を継続することを可能にする潜在的な利益は、進行中のリスクに基づいて正当化される。この理由で、実施例2に記載する試験は、疾患の進行までの対象の膀胱癌(例えば、UC)の処置を含む。
A.全体的試験設計
本明細書に記載の方法に関連するデータは、進行中の試験1108、第1/2相、単一アーム、オープンラベル、多施設臨床試験(ClinicalTrials.gov Identifier:NCT01693562)により提供される。試験の用量漸増段階は、尿路上皮膀胱癌(UC)コホートをはじめとする17の異なる腫瘍別コホートが含まれた。用量漸増段階で患者は、10mg/kg Q2Wのデュルバルマブで処置され、その処置を継続する。
適格な患者は、組織学的又は細胞学的に確認された局所進行性又は転移性UCを有する年齢≧18歳であった。以前の選択療法に基づく適格性基準は、上の(A)に記載の通りである。患者は、0又は1のECOGパフォーマンスステータススコア、適切な臓器及び血液機能、並びにPD−L1試験に使用できる新鮮な腫瘍生検及び/又は腫瘍組織記録を有した。この試験では、免疫不全の病歴;全身免疫抑制を必要とする病状;重度の免疫媒介性有害反応の病歴;未処置の中枢神経系転移;並びにヒト免疫不全ウイルス、活動性結核症、又は肝炎B若しくはC感染症を有する患者を除外した。
中間有効性解析
103人のUC患者が、デュルバルマブ10mg/kg Q2Wで処置され、少なくとも13週間経過観察される機会を有した(すなわち、RECIST v1.1に従って6週目及び12週目に少なくとも2回のフォローアップスキャンを受け、さらに1週間の来院許容期間を受ける機会を有した)後、有効性の中間解析を実施した。
主要有効性評価項目は、BICRにより決定される通り、RECIST v1.1に従い確定されたCRの最良総合効果として定義される客観的奏功(OR)又は部分奏功(PR)であった。ORを有する患者の割合として定義されるORRを計算し、Clopper−Pearson法を用いて、ORRの両側95%正確CIを推定した。ベースライン;デュルバルマブ療法の開始後6、12、及び16週目;続いて、処置中、8週間毎に、疾患評価のために患者をスキャンした。治療の中止後、1年にわたり2ヵ月毎、続いて、客観的な疾患の進行が確認されるまで3ヵ月毎に患者をスキャンした。
副次的有効性評価項目は、BICR及び主治医判定、及びOSにより決定される通り、RECIST v1.1に従うDoR、DCR、奏功までの時間、PFS、標的病変サイズの変化を含んだ。UC患者におけるデュルバルマブの抗腫瘍活性に関するPD−L1発現を評価するために、検証済のVENTANA PD−L1(SP263)アッセイの評価で熟練した病理学者により決定された腫瘍細胞(TC)中の膜PD−L1染色及び腫瘍関連免疫細胞(IC)の全PD−L1染色の評価としての、PD−L1発現に基づいて識別された亜群別でも有効性データを分析した。合計103人のUC患者について、ORRが20%〜30%の範囲にあると予想されたとき、両側正確95%CIの観察ORRとその下限の間の幅は、7%〜9%の範囲であった。
PD−L1発現カットオフに基づいて応答及び非応答患者を識別するためのスコアリングアルゴリズムが、本発明者らによって開発された。腫瘍細胞(TC)及び免疫細胞(IC)双方でのPD−L1発現レベルと、デュルバルマブ療法に対する応答との関係を、試験1108のUCコホートの処置済患者からの腫瘍サンプルで評価した。この解析によって、TC及びICのPD−L1染色の組合せ評価となる最適アルゴリズムを確立した。
1.PD−L1及び応答の生物学:高い膜性腫瘍細胞PD−L1発現は、PD−1/PD−L1経路をターゲティングする薬物による優れた臨床応答と相関することが証明されており;
2.UCにおける優勢:市販の膀胱癌サンプル中のPD−L1発現の評価、ここで、PD−L1はいくつかの症例のTCに高度に発現されることが判明し、これは、PD−L1の先天的又は適応性過剰を示唆しており;
3.UCの臨床転帰データ:部分奏功(PR)を有する対象の1人及び最良変化が−38.5である別の対象は、PD−L1≧25%をTCに有した。
1.UCにおけるPD−L1及び応答の生物学:ICにおける高いPD−L1発現は、抗PD−L1薬であるアテゾリズマブによる優れた臨床応答と相関することがわかっており;
2.UCにおける優勢:市販の膀胱癌サンプルにおいて、多くの症例のICでPD−L1が高度に発現されることが判明したが、これは、PD−L1の適応過剰発現を示唆しており;
3.UCにおける臨床転帰データ:PRを有する4人の対象全員が、ICにPD−L1≧25%を有することがわかる。
主要有効性集団中の103人のUC患者のうち、94人は、ネオアジュバント/アジュバントセッティングで治療法を受けて12ヵ月以内に進行した患者を含むプラチナベースの療法中、または療法後に進行した2L+プラチナ後患者であった。他の9人は、治療に対してナイーブであるか、又はネオアジュバント/アジュバントセッティングでプラチナベースの療法を受け、療法の最後の投与から12ヵ月超で進行した患者で、1L患者と称された。9人の治療ナイーブ/1L患者のうち、7人は、試験で使用したシスプラチン適格性基準に基づき、シスプラチン不適格とみなされた
客観的奏効率(ORR)、奏功期間(DoR)、及びBICRによる疾患制御率
デュルバルマブ単剤療法は、臨床利益を予測する可能性が高い評価項目(ORR、DoR)に基づき、局所進行性又は転移性UCを有する患者において有意義な利益を実証した。主要有効性集団では、デュルバルマブ単剤療法は、全UC患者においてRECIST v1.1に従いBICRにより20.4%(95%CI:13.1%,29.5%)のORR、並びに2L+プラチナ後亜群において20.2%(95%CI:12.6%,29.8%)のORRをもたらした(表3)。3人の追加患者は、下記のように、DCO時点でBICRにより未確定の奏功を有し、これは確定される可能性がある:依然として処置中の3人の患者のうち1人は、DCO後に続くスキャンに基づき、BICRにより確定されたPRを有し;まだ処置中の3人のうち2番目の患者は、2回の連続したスキャンでPRと評価されたが、2回のスキャンの間の間隔は、26日(<4週間)であり;まだ処置中の3人のうち3番目の患者は、2回目のスキャン(12週目)で初期PRを有し、これは確定される可能性がある。
デュルバルマブは、PD−L1高及びPD−L1低/neg亜群の両方において臨床活性及び奏功の持続性を明らかにしたが、VENTANA PD−L1(SP263)AssayでTC/ICを用いる組合せアルゴリズムは、高TPR(85.7%)及び高NPV(92.3%)を確かに示した。対照的に、TCのみ又はICのみのアルゴリズムは、より低いTPR及びNPVにより示されるように、それほど識別力はなかった。これらのデータは、デュルバルマブに応答する可能性が高い対象を同定するために組み合わせたTC又はICに基づくアルゴリズムの有用性を支持する。加えて、アッセイの高NPVを考慮すれば、これは、デュルバルマブに対する応答の可能性を患者に知らせる上で特に役立つ。すなわち、VENTANA PD−L1(SP263)AssayによりPD−L1高と分類される対象は、PD−L1低/negであった対象よりも高いORRを有する傾向があった。組合せアルゴリズムは、デュルバルマブに応答する可能性が最も高い対象を同定するためにこのアッセイの有用性を支持するが、応答する可能性がある対象を完全に除外するわけではない。スポンサーは、アッセイの高いNPVは、デュルバルマブに対する応答の可能性を患者に知らせる上で特に役立つが、デュルバルマブで処置した3人のPD−L1低/neg対象は、臨床応答(1人がPR及び2人が完全奏功)を有し、奏功はDCOの時点でも継続していることから、治療法から対象を除外するために使用するべきではないことを考慮した。
補助的有効性集団では、ORRは、全UC患者について28.6%(95%CI:17.9%,41.3%)であり、2L+プラチナ後患者の場合は29.6%(95%CI:18.0%、43.6%)であった(表4)。応答期間の中央値は到達されなかった。主要有効性集団と同様に、PD−L1高及びPD−L1低/neg亜群において臨床活性を観察した。ORRは、PD−L1高亜群で40.0%(16/40;95%CI:24.9%,56.7%)であり、PD−L1低/neg亜群では8.7%(2/23;95%CI:1.1%,28.0%)であった。双方の亜群においてCRが観察された:PD−L1高、2人(5.0%)の患者;PD−L1低/neg、2人(8.7%)の患者。両亜群で観察された奏功は、持続性であった。
主要有効性集団における無増悪生存(PFS)中央値は、2.2ヵ月(95%CI:1.4,2.7)であった。6ヵ月時点のPFS率は、28.3%(95%CI:19.3%,37.9%)であった。PFS中央値は、PD−L1高亜群で2.5ヵ月、PD−L1低/neg亜群では1.5ヵ月であった。6ヵ月時点のPFS率は、PD−L1高亜群で39.2%(95%CI:26.4%,51.8%)であり、PD−L1低/neg亜群では13.4%(95%CI:4.2%,28.1%)であった。
主要有効性集団における全生存率(OS)中央値は、14.1ヵ月(95%CI:4.5、推定不能)であった。6ヵ月、9ヵ月、及び12ヵ月時点のOS率は、それぞれ60.3%(95%CI:48.7%、70.1%)、55.7%(95%CI:43.2%,66.4%)、及び52.4%(95%CI:39.1%,64.1%)であった(表5)。OS中央値は、PD−L1高群で到達されず、PD−L1低/neg亜群では3.3ヵ月であった。図4は、OSのカプラン−マイヤー推定値を示す。
客観的奏効(OR)は、例えば、内蔵転移若しくは肝臓転移を有する対象のような予後が不良の対象を含む、全ての対象について、ベースラインデモグラフィックス又は疾患の特徴に基づいて観察した。主要有効性集団では、内蔵転移、肝臓転移、及びリンパ節のみの亜群におけるORRは、それぞれ19.4%(19/98;95%CI:12.1%,28.6%)、10%(5/50;95%CI:3.3%,21.8%)及び40%(2/5;95%CI:5.3%,85.3%)であった。いくつかの亜群における点推定の数値に差が認められたが、サンプルサイズは小さく、95%CIは、中央値を下回る腫瘍負荷を有する患者が、より高いORRを有した腫瘍負荷を除き、ほとんどの亜群比較について重なった。地域、以前の選択療法、ベースラインクレアチニンクリアランス、原発性腫瘍の部位、及び以前のプラチナ療法をはじめとする主要亜群について比較可能なORRが観察された。
客観的奏効
デュルバルマブ単剤療法は、一般に使用される治療法に対する臨床利益を予測し得る評価項目である客観的奏効率(ORR)及び奏功期間(DoR)に基づき、局所進行性若しくは転移性UCを有する患者における有意義な利益を実証した。より具体的には、主要有効性集団(≧13週間のフォローアップの機会を有した全てのUC患者)において、ORRは、RECIST v1.1に従うBICRにより、20.4%(21/103;95%CI:13.1%,29.5%)であり、CRの最良の奏功を有する5人の患者(4.9%)を含んだ。2L+プラチナ後UC患者のORRは、20.2%(19/94;95%CI:12.6%、29.8%)であった。さらに、3人(全員が、DCOの時点でまだ処置中であった)が、DCOの時点で未確定の奏功を有した。補助的有効性集団(≧24週間のフォローアップの機会を有したUC患者)において、ORRは、28.6%(18/63;95%CI:17.9%,41.3%)であった。2L+プラチナ後UC患者のORRは、29.6%(16/54;95%CI:18.0%,43.6%)であった。
膀胱癌を治療するための試験1108では、奏功が処置の早期に起こり、持続性であった。奏功までの時間中央値は、1.41ヵ月(範囲、1.2〜7.2ヵ月)であった。DoR中央値にはまだ到達されなかった(範囲、1.4+〜19.9+ヵ月)。DCOの時点で、21人の奏功者のうち18人(85.7%)が、継続奏功を有し:16人の患者は、DoR>6ヵ月を有し;また、9人の患者が、DoR>9ヵ月を有した。RECIST v1.1に従うBICRにより進行した3人の患者の全員が、デュルバルマブによる12ヵ月の処置を完了した。これらの患者のうち1人は標的病変の増大のために進行したが、BICRにより後にCRを有し、これは、650日目のDCOの時点で最後のフォローアップまで維持された。
主要有効性集団におけるPFS中央値は、2.2ヵ月(95%CI:1.4%、2.7%)であった。6ヵ月時点のPFSは、28.3%(95%CI:19.3%、37.9%)であった。
OS結果は、多種類の治療歴を有するUC集団において励みとなるものだった。OS中央値は、14.1ヵ月(95%CI:4.5、推定不可能)であり;6ヵ月、9ヵ月、及び12ヵ月時点のOS率は、それぞれ、60.3%(95%CI:48.7%,70.1%)、55.7%(95%CI:43.2%,66.4%)、及び52.4%(95%CI:39.1%,64.1%)であった。
主要有効性集団における臨床活性は、予後不良の者を含め、PD−L1高及びPD−L1低/neg亜群(検証済VENTANA PD−L1[SP263]アッセイを用い、TC又はIC25%PD−L1発現により定義される)並びに予後不良の亜群を含む、様々なベースライン疾患亜群において認められた。PD−L1高亜群では、ORRは、29%(18/61;95%CI:18.5%、42.6%)であった。PD−L1低/neg亜群では、ORRは、7.7%(3/39;95%CI:1.6%、20.9%)であった。双方の亜群においてCRが観察された:PD−L1高、3(4.9%)及びPD−L1低/neg、2(5.1%)。ORRは、内蔵転移、肝臓転移、及びリンパ節のみの亜群において、それぞれ、19.4%(19/98;95%CI:12.1%、28.6%)、10%(5/50;95%CI:3.3%、21.8%)及び40%(2/5;95%CI:5.3%、85.3%)であった。
膀胱癌、例えばUCを治療するためのデュルバルマブとトレメリムマブの併用
検証的臨床試験(DANUBE)は、UCの治療にデュルバルマブを使用する実施例2に記載の臨床試験を支持する。補助的臨床試験は、第3相のランダム化、オープンラベル、比較、多施設、国際試験であり、ここでは、デュルバルマブ単剤療法(4週間毎に1.5gIV(Q4W));又はデュルバルマブ(1.5g IV Q4W)とトレメリムマブ(75mg IV Q4W)の併用を各々最大4用量/サイクル、続いてデュルバルマブ(1.5g IV Q4W)の有効及び安全性を、標準治療(SoC):(シスプラチン適格性に応じて、シスプラチン+ゲムシタビン又はカルボプラチン+ゲムシタビンダブレット)1L化学療法と比較して決定する。この試験では患者は、尿路上皮(腎盂、尿管、膀胱、及び尿道を含む)の組織学的若しくは細胞学的に立証された、切除不能なステージIV移行細胞癌(移行細胞及び混合型移行/非移行細胞組織学)を有し、治療に対してナイーブである。患者は、デュルバルマブ+トレメリムマブ併用療法、デュルバルマブ単剤療法、又はSoC治療による処置にランダムに分けた(1:1:1)。主要な目的は、全UC患者におけるPFS及びOSに関してSoC治療に対するデュルバルマブ+トレメリムマブ併用療法の有効性を評価すること、並びにPD−L1高(PD−L1高UC患者)として同定されたUC癌又は腫瘍細胞若しくは組織を有するUC患者におけるOSに関して、SoC治療に対するデュルバルマブ単剤療法の有効性を評価することである。重要な副次的目的は、PD−L1高UC患者のPFSに関して、及び全UC患者におけるPFS及びOSに関して、SoC治療に対するデュルバルマブ単剤療法の有効性の評価である。この試験では、ランダム化した625人の患者のデータを更新する必要があり、その登録は継続中である。試験の進行と共に、中間OSを含むPFSを分析し、最終OS評価は試験の終わりに実施する。
以上の説明から、本明細書に記載する本発明に変更及び修正を加えて、それを様々な用途及び条件に採用することができることが明らかになるであろう。こうした実施形態も、以下に示す特許請求の範囲に含まれる。

Claims (75)
- 治療が必要な対象の膀胱癌を治療する方法であって、以下:前記膀胱癌を治療するために、前記対象に、抗PD−L1抗体又はその抗原結合断片を10mg/kg〜20mg/kgの量で2〜4週間毎(Q2W〜Q4W)に投与するステップを含む方法。
- 前記抗PD−L1抗体又はその抗原結合断片が、デュルバルマブ又はその抗原結合断片である、請求項1に記載の方法。
- 前記膀胱癌が、尿路上皮癌(UC)並びに/又は尿管、尿道、尿膜管及び/若しくは腎盂を含む膀胱の関連構造及び組織の癌である、請求項1又は2に記載の方法。

- UCを有する対象の少なくとも20%が、全奏効率を達成する、請求項22に記載の方法。
- 前記対象が、初期治療後から1〜12ヵ月後に治療応答を達成する、請求項22に記載の方法。

- 前記対象のUCが、PD−L1−高として同定される、請求項33に記載の方法。
- 前記投与が、静脈内注入によって行われる、請求項33〜35のいずれか1項に記載の方法。
- 前記対象が、プラチナ薬又は別の抗癌剤と組み合わせたプラチナ薬を含む第1選択癌療法を以前受けたことがある、請求項33〜35のいずれか1項に記載の方法。
- デュルバルマブ又はその抗原結合断片の投与と同時に、又は異なる時間に、有効量の抗CTLA抗体又はその抗原結合断片を前記対象に投与するステップをさらに含む、請求項33に記載の方法。
- プラチナ薬又は別の抗癌剤と組み合わせたプラチナ薬を含む第1選択癌療法による治療後に癌が進行した膀胱癌を有する対象を治療する方法であって、前記第1選択癌療法による治療を以前受けたことがある前記対象に、抗PD−L1抗体又はその抗原結合断片を10mg/kg〜20mg/kgの量で2〜4週間毎(Q2W〜Q4W)に投与するステップを含む方法。
- 前記第1選択癌療法が、シスプラチン、シスプラチンとゲムシタビン、シスプラチンとメトトレキサート、ビンブラスチン、ADRIAMYCIN(商標)(ドキソルビシン)、又はカルボプラチン及びゲムシタビンダブレット併用から選択される治療法を含む、請求項39に記載の方法。
- 前記抗PD−L1抗体又はその抗原結合断片が、デュルバルマブ又はその抗原結合断片である、請求項39又は40に記載の方法。
- 前記抗PD−L1抗体又はその抗原結合断片が、前記対象に、10mg/kgの量で2週間毎(Q2W)に投与される、請求項41に記載の方法。
- 前記抗PD−L1抗体又はその抗原結合断片が、前記対象に、10mg/kgの量で4週間毎(Q2W)に投与される、請求項41に記載の方法。
- 前記抗PD−L1抗体又はその抗原結合断片が、前記対象に、20mg/kgの量で2週間毎(Q2W)に投与される、請求項41に記載の方法。
- 前記抗PD−L1抗体又はその抗原結合断片が、前記対象に、20mg/kgの量で4週間毎(Q2W)に投与される、請求項41に記載の方法。
- 前記抗PD−L1抗体又はその抗原結合断片が、前記対象に、静脈内注入により投与される、請求項42〜45のいずれか1項に記載の方法。
- 前記膀胱癌が、尿路上皮癌(UC)、進行性UC、又は転移性UCである、請求項39〜45のいずれか1項に記載の方法。
- 前記対象のUCが、PD−L1−低又はPD−L1−低/negとして同定される、請求項47に記載の方法。
- 前記対象のUCが、PD−L1−高として同定される、請求項47に記載の方法。
- 癌細胞の25%未満が、PD−L1染色を呈示するとき、前記膀胱癌は、PD−L1−低又はPD−L1−低/negである、請求項48に記載の方法。
- 前記抗PD−L1抗体又はその抗原結合断片の投与と同時若しくは異なる時間に、抗CTLA4抗体又はその抗原結合断片の有効量が、膀胱癌を有する前記対象に共投与される、請求項39〜45のいずれか1項に記載の方法。
- 前記抗CTLA4抗体又はその抗原結合断片が、トレメリムマブ又はその抗原結合断片である、請求項51に記載の方法。
- 前記PD−L1が、免疫組織化学を用いて検出される、請求項18〜20、27、28、34、35、48、又は49のいずれか1項に記載の方法。
- 前記免疫組織化学が、ホルマリン固定及びパラフィン包埋された癌細胞に対して実施される、請求項53に記載の方法。
- 前記処置によって、前記対象における全生存率、無増悪生存、又は完全奏功の増加がもたらされる、請求項1、21、33、又は39のいずれか1項に記載の方法。
- 前記治療応答までの時間中央値が、約1ヵ月〜8ヵ月である、請求項1、21、33、又は39のいずれか1項に記載の方法。

- 治療が必要な対象の膀胱癌を治療する方法であって、膀胱癌を治療するために、2〜4週間毎に50〜150mgの量でトレメリムマブ又はその抗原結合断片を対象に投与するステップと組み合わせて、デュルバルマブ又はその抗原結合断片を、2〜4週間毎(Q2W〜Q4W)に50〜2000mgの量で膀胱癌を有する前記対象に投与するステップを含む方法。
- 前記デュルバルマブ又はその抗原結合断片が、4週間毎(Q4W)に1500mgの量で投与され、前記トレメリムマブ又はその抗原結合断片が、4週間毎(Q4W)に75mgの量で投与される、請求項60に記載の方法。
- 前記デュルバルマブ及びトレメリムマブ又はそれらの抗原結合断片が、1サイクルにつき最大4用量まで投与される、請求項60又は61に記載の方法。
- 前記デュルバルマブ及びトレメリムマブ又はそれらの抗原結合断片が、同時又は異なる時間に投与される、請求項60又は62に記載の方法。
- 前記対象が、より低い若しくは低レベルのPD−L1発現(PD−L1−低)、又は無視できる程度から低いPD−L1発現(PD−L1−低/neg)を伴う膀胱癌を有するとして同定される、請求項60に記載の方法。
- 前記対象が、高レベルのPD−L1発現(PD−L1−高)を伴う膀胱癌を有するとして同定される、請求項60に記載の方法。
- 前記膀胱癌が、尿路上皮癌(UC)並びに/又は尿管、尿道、尿膜管及び/若しくは腎盂を含む膀胱関連構造及び組織の癌である、請求項60〜65のいずれか1項に記載の方法。
- 前記膀胱癌が、尿路上皮癌、進行性UC、又は転移性UCである、請求項60〜65のいずれか1項に記載の方法。
- PD−L1−高として同定される膀胱癌を有し、治療を受けた対象の65%超が、6ヵ月時点で全生存奏功を有する、請求項19、28、35、49、又は65のいずれか1項に記載の方法。
- PD−L1−高として同定される膀胱癌を有し、治療を受けた対象の約65%が、9ヵ月時点で全生存奏功を有する、請求項19、28、35、49、又は65のいずれか1項に記載の方法。
- PD−L1−高として同定される膀胱癌を有し、治療を受けた対象の約60%が、12ヵ月時点で全生存奏功を有する、請求項19、28、35、49、又は65のいずれか1項に記載の方法。
- PD−L1−低/negとして同定される膀胱癌を有し、治療を受けた対象の40%超が、6ヵ月時点で全生存奏功を有する、請求項18、27、34、48、又は64のいずれか1項に記載の方法。
- PD−L1−低/negとして同定される膀胱癌を有し、治療を受けた対象の35%超が、9ヵ月時点及び12ヵ月時点で全生存奏功を有する、請求項18、27、34、48、又は64のいずれか1項に記載の方法。
- 治療を受けた対象の約60%が、6ヵ月時点で全生存奏功を有する、請求項1、21、33、又は39のいずれか1項に記載の方法。
- 治療を受けた対象の55%超が、9ヵ月時点で全生存奏功を有する、請求項1、21、33、又は39のいずれか1項に記載の方法。
