Abstract

コア208は、吸収材料を含み得るか、または吸収材料からなり得、吸収材料は液体を保持、安定化および/または固化するために使用される材料であり得る。吸収材料は親水性材料であり得る。吸収材料は、流体を組織部位から引き離すまたは吸い上げるために吸収勾配を提供し得る。組織部位からの流体は吸収勾配によって引かれ得、次いで吸収材料によって吸収され得る。吸収材料は、吸収材料に対する物理的または化学的変化と共に流体を保持し得るか、または流体と結合し得る。吸収材料は、例えば組織部位からの流体と接触すると半固体状の液体になり得るか、粘度が増大し得るか、または厚くなり得、それにより流体を捕捉する。吸収材料の吸上げ特性は、組織部位から流体、例えば滲出液を迅速に引き離すことを促進し得、その結果、組織部位またはその近くに流体が蓄積することを防止し得る。
いくつかの実施形態では、吸収材料は、高吸収性材料を含み得る。例えば、吸収材料は、カルボキシメチルセルロース(CMC)、またはカルボキシメチルセルロース塩、架橋親水性アクリル、または架橋親水性アクリル塩、またはアクリルアミドなど、1種または複数種の高吸収性材料を含み得る。いくつかの実施形態では、例えば、高吸収性材料は、一般にヒドロゲルまたは親水コロイドと呼ばれる種類のものなどの高吸収性ポリマー(SAP)であり得る。一般に、それらの質量に比して、SAPは大量の液体、特に水を吸収し保持することができる。キャニスタまたはドレッシングなどの医療用使い捨て製品は、創傷流体および他の滲出液を保持し、かつ安定化または固化するためにSAPを使用する場合がある。
SAPは、いくつかの方法で、例えば、ゲル重合、溶液重合または懸濁重合によって形成され得る。ゲル重合は、アクリル酸、水、架橋剤および紫外線(UV)開始剤化学物質の混合を伴い得る。混合された混合物は反応器に入れられ得、そこで混合物は、SAPを形成する架橋反応を引き起こすためにUV光に暴露される。混合物は乾燥かつ破砕され得、次いで梱包および/または配送される。溶液重合は、反応物重合ゲルの塊を生成する水ベースモノマー溶液を必要とし得る。モノマー溶液は、モノマーの架橋を促進する発熱性の反応を経る可能性がある。架橋プロセス後、反応物ポリマーゲルは、細断、乾燥および粉砕され得、その最終の粒状サイズにされる。懸濁重合は、炭化水素ベース溶液中に懸濁される水ベース反応物を必要とし得る。
一般に、SAPは、水素結合を介して水分子と結合することによって液体を吸収する。水素結合は、極性水素原子と電気的陰性原子との相互作用を伴う。その結果、SAPは、電気的陰性イオン成分を有するSAPの親水性ポリマーと結合する各水分子中の水素原子の能力に基づいて水を吸収する。高吸収性SAPは、塩または遊離酸の形態のアクリルまたはアクリルアミドなどのイオン性架橋親水性ポリマーから形成可能である。
SAPはイオン性であるため、それらは吸収される溶液中の可溶性イオン性成分によって影響を及ぼされる可能性があり、および例えば、純水と比べて食塩水を吸収しない可能性がある。この食塩水のより低い吸収率は、SAPの水吸収部位の一部を遮断するナトリウムイオンおよび塩化物イオンによって引き起こされる。SAPによって吸収される流体が、溶解されたミネラルイオンを含有する溶液である場合、溶液中の水分子のより少ない水素原子が自由にSAPと結合し得る。このように、流体を吸収しかつ保持するSAPの能力は、流体のイオン濃度に依存し得る。例えば、SAPは脱イオン水を乾燥SAPの重量の500倍まで吸収かつ保持し得る。体積の観点では、SAPはSAPの乾燥体積の30〜60倍程度の高さの流体体積を吸収し得る。より高いイオン濃度を有する他の流体は、より低い量で吸収され得る。例えば、SAPは、0.9%塩である溶液(NaCl)を、乾燥SAPの重量の50倍まで吸収しおよび保持するのみであり得る。
創傷流体はナトリウム、カリウムおよびカルシウムなどの塩を含有する場合があるため、SAPの吸収能力は、創傷処置に使用されるときに著しく低減される可能性がある。吸収能力の低減は、追加的な量のSAPの使用を必然的に伴う可能性があり、これはドレッシングまたは流体貯蔵装置の嵩全体を大幅に増す可能性がある。さらに、多くの治療システムにおいて、追加量のSAPを使用することは、ドレッシングまたは容器の寸法制限が原因で不可能であり得る。
本明細書に記載されるシステム、装置および方法は、吸収繊維にイオン交換媒体(IEM)を提供することによってこれらの欠点およびその他の欠点を克服することができる。その結果、高吸収機能と組み合わされた創傷流体媒体からイオン物質を除去するための簡素化された設計が提供される。そのような設計は、創傷ドレッシング、流体取扱いキャニスタまたは貯蔵容器の一部として組み込むことができる。
図2をなおも参照すると、例えば、コア208は、少なくとも部分的にフィルタ層210に取り囲まれ得、IEM212はフィルタ層210内に配置され得る。いくつかの実施形態では、フィルタ層210は、コア208の外側にコーティングされ得、水溶性または水膨張性ポリマーであり得る。水溶性または水膨張性ポリマーは、架橋ポリビニルアルコール、ポリビニルピロリドン、ポリウレタン、ポリアクリルアミド、およびポリエチレンオキシドを含み得る。IEM212は、ビーズまたは他の粒子の形態のイオン交換樹脂であり得、それはフィルタ層210の表面にグラフトされ得る。代替的に、イオン交換樹脂は、ポリスチレンにグラフトされ得、およびフィルタ層210全体に分散され得る。繊維106の直径は、約1〜50マイクロメートルの範囲内、より典型的には約5〜25マイクロメートルの範囲内であり得、特定の使用に望まれ得るまたは適切であり得るコア208およびフィルタ層210の寸法に依存する。
IEM212は、コア208と接触するように進入する創傷流体内のイオン化粒子の数を低減し得る。例えば、IEM212は、およそ1%〜80%の範囲内で創傷流体中のイオン化粒子の数の低減を実現し得る。いくつかの好ましい実施形態において、IEM212は、創傷流体中のイオン化粒子濃度の約25%の低減を実現し得る。IEM212は、2つの電解質間または電解液と錯体との間のイオン交換を実現するように適合され得る。電解質は、水などの適切なイオン化溶媒中で溶解されるとイオン化する化合物である。電解液はNaClなどの溶解された塩を含有し得る。錯体は、リガンドまたは錯化剤として知られる固定分子またはアニオンの周囲配列を有する原子またはイオンであり得る。電解質または電解液がIEMと相互作用するとき、IEMは電解質または電解液中のカチオンおよびアニオンを置換することができる。カチオンは正味の正の電荷を有するイオン、例えばNa+である。カチオンは、電解質または電解液中でIEMの水素(H+)イオンと置換され得る。アニオンは、正味の負の電荷を有するイオン、例えばCl−である。アニオンは、電解質または電解液中でIEMのヒドロキシル(OH−)イオンと置換され得る。H+およびOH−イオンは、電解質または電解液中で組み合わされて水を形成し得る。
いくつかの実施形態では、IEM212は、酸性ポリマーでドープまたはグラフトされる、ポリスチレンなどの架橋ポリマーから形成された多孔性ビーズであり得る。酸性ポリマーの例は、ポリ(2−アクリルアミド−2−メチル−1−プロパンスルホン酸)またはポリAMPSを含み得る。ポリAMPSは、正に荷電された塩イオンをH+と交換することができる。アルカリ性ポリマーの例は、ポリ(アクリルアミド−N−プロピルトリメチルアンモニウムクロリド)またはポリAPTACを含み得る。ポリAPTACは負に荷電された塩イオンをOH−と交換する。
フィルタ層210のIEM212はまた、アニオンおよびカチオンの両方を同時に吸収することができる混合床イオン交換媒体を形成するために、カチオン交換媒体214およびアニオン交換媒体216の両方の混合物を含み得る。混合床媒体の非限定的な例は、AMBERLITE IRN150およびTMD−8を含む。IEMは、例えば、イオン交換樹脂、ゼオライト、モンモリロナイト、ベントナイト、粘土、または土壌腐植質から形成され得る。イオン交換樹脂(イオン交換ポリマーとしても知られる)は、通常、有機ポリマー基体から形成される小さいビーズの形態の不溶性マトリックスである。イオン交換樹脂は、イオンを捕捉かつ放出する細孔を表面に有し得る。イオン交換樹脂は、例えば架橋ポリスチレンを含むことができる。ゼオライトは微孔性のアルミノケイ酸鉱物である。ゼオライトは多孔質構造を有し、それにより、例えばNa+、K+、Ca2+およびMg2+などのカチオンがゼオライトに収容されることを可能にする。一般的なゼオライトは、例えば、ホウフッ石、リョウフッ石、クリノプチロライト、輝沸石、ソーダフッ石、フィリップス沸石、およびタバフッ石を含む。上記の物質に加えて、他のイオン交換媒体は、例えば粒子および布または不織布の形態の両方である活性化された炭、およびChemviron Carbonとしても知られるZorflexを含む。Chemviron Carbonはまた、100%活性化された炭素としても知られ得る。
コア208は、アルギン酸または一緒に融着された粒子、例えばカルボキシメチルセルロース(CMC)またはポリアクリル酸から形成されたものなど、高吸収性繊維の形態であり得る。いくつかの実施形態では、高吸収性繊維は、非限定的に、湿式紡糸、乾式紡糸、溶融紡糸、ゲル紡糸または電解紡糸など、様々な紡糸プロセスによって形成または製造され得る。プロセスの第1ステップとして、繊維形成ポリマーは流動状態に変換され得る。ポリマーは、溶融されるか、溶媒中で溶解されるか、または化学的に処理されて、溶解性誘導体を形成することができる。溶融されたポリマーは、紡糸口金、多孔性装置を通して押出され、繊維を形成し得る。
いくつかの実施形態では、繊維106は、共押出プロセスを用いて、高吸収性コア材料と、イオン交換媒体を含有するフィルタ層材料とを同時に押出すことによって形成され得る。例えば、いくつかの実施形態では、共押出プロセスは、イオン交換媒体を含有し得る外側フィルタ層材料を形成する押出し機と、外側フィルタ層材料に高吸収性コア材料を注入または充填するための付属システムとの使用を伴う。
図3をより詳細に参照すると、繊維マトリックスの別の例示的実施形態を示す概略図が示されている。図3に示される繊維マトリックス302は、繊維106と同様または類似する特性を有する短繊維306を含み得る。例えば、繊維306は、フィルタ層210などのIEM層によって包み込まれたコア208などのコアを含み得る。いくつかの例示的実施形態において、繊維306は、支持体318などの支持材料中に分散され得る。繊維306は、組織部位によって放出されると予想される液体量を含む様々な要因に依存して様々な寸法または形状を有し得るが、好ましくは、約1〜50マイクロメートルの範囲内、より典型的には約5〜25マイクロメートルの範囲内の直径を有し得る。いくつかの実施形態では、支持体318は、連続ポリマー水透過性マトリックスであり得、負圧の通過を許容するように穿孔されている。そのようなポリマーは、ポリビニルアルコール、ポリアクリル酸およびその塩(ナトリウムおよびカリウム)、カルボキシメチルセルロース(CMC)、他のセルロース系材料、ポリアクリルアミド、ポリウレタン、ならびにポリエチレンオキシドおよびグリコールのポリマーおよびコポリマーを含み得る。いくつかの実施形態では、支持体はまた、織られたまたは織られない繊維、例えば、ポリ−およびコポリ−エステル、ポリオレフィン、セルロース系材料、またはこれらの材料の何れかのブレンドから形成されたテキスタイルであり得る。
図4は、繊維マトリックスの別の例示的実施形態を示す。図4の繊維マトリックス402は、複数の層を含み得る繊維420を含み得る。例えば、図4の実施形態において、繊維420は、コア422などの吸収性ストランドと、IEMを有するフィルタストランド423とを含み得る。いくつかの実施形態では、コア422は、高吸収性ストランドの形態であり得、フィルタストランドはコア422の周囲で撚られ得る。いくつかの実施形態では、繊維420などの繊維は、支持体418などの支持材料中に分散され得る。フィルタストランド423は、酸交換媒体428を含み得る酸交換ストランド424と、アルカリ交換媒体430を含み得るアルカリ交換ストランド426とを含み得る。いくつかの実施形態では、フィルタストランド423は、カチオン交換樹脂またはアニオン交換樹脂を含み得る。他の実施形態では、フィルタストランド423は、カチオンおよびアニオン交換樹脂の組合せを含み得る。
図4の例示的実施形態において、フィルタストランド423は、流体がコア422と接触する前に流体からイオン性材料を低減または除去することができる。3つの機能的要素、すなわち、高吸収性コア、酸性イオン交換媒体、およびアルカリ性イオン交換媒体のそれぞれを、別々のストランドとして含むことにより、繊維420の製造を簡素化することができる。例えば、単一の複合繊維を製造するため、互いに一貫性を保持するために4種類以上のフィードストックストリームが必要とされる繊維製造または紡糸方法は必要ないことがあり得る。
繊維106または繊維420など、イオン交換媒体を有する吸収性繊維は、多くの有益または有利な用途を有し得る。例えば、繊維マトリックス104、繊維マトリックス302または繊維マトリックス402など、イオン交換媒体を有する吸収性繊維を有する吸収層は、減圧による創傷の処置に特に有利であり得る。減圧による創傷または他の組織の処置は、一般に「減圧治療」と称され得るが、他の名称でも知られ、例えば「負圧創傷治療」、「負圧治療」、「陰圧治療」および「陰圧補助閉鎖」が含まれる。減圧治療は多くの恩恵をもたらし得る治療であり、それらの恩恵には上皮および皮下組織の移動、改善された血流、および創傷部位における組織の微小な変形が含まれる。これらの恩恵が合わさることにより肉芽組織の発生を増大することができ、および治癒時間を短縮することができる。
図5は、イオン交換媒体を有する吸収性繊維を含み得る減圧治療システム500の例示的実施形態の概略図である。例えば、減圧治療システム500は、組織部位の近くに位置付けることができるドレッシング532と、減圧をドレッシング532に提供するための減圧源534とを含み得る。いくつかの実施形態では、ドレッシング532は、マニホルド540と、吸収層502と、ドレープ538とを含み得る。吸収層502は、繊維マトリックスまたは本明細書に記載されるイオン交換媒体を有する繊維の適切な特徴、構造または要素の全体的または部分的な任意の組合せを組み込み得る。例えば、吸収層502は、繊維マトリックス104、繊維マトリックス302、繊維マトリックス402、またはこれらの実施形態の特徴の何れかの適切な組合せを組み込み得る。ドレッシング532は、減圧源534に流体結合され得る。減圧治療システム500はまた、ドレッシング532および減圧源534に結合された、容器542などの滲出液容器を含み得る。
用語「組織部位」は、骨組織、脂肪組織、筋肉組織、神経組織、皮膚組織、血管組織、結合組織、軟骨、腱、または靭帯を含むが、それらに限定されない組織の上または中に位置する創傷または欠損を指す場合がある。組織部位は、例えば慢性、急性、外傷性、亜急性、および裂開性創傷、部分的に厚いやけど、潰瘍(糖尿病性、圧迫性、または静脈不全性潰瘍など)、皮弁および移植片を含み得る。用語「組織部位」はまた、必ずしも創傷も欠損もないが、追加的な組織の成長を追加または促進することが所望され得る領域である何れかの組織の領域を指す場合もある。例えば、減圧は、採取され得るまたは別の組織箇所へ移植され得る追加の組織を成長させるために特定の組織領域で使用され得る。
減圧源534などの減圧源は、減圧の空気貯蔵庫であり得、または例えば陰圧ポンプ、吸引ポンプ、多くの医療施設で利用可能な壁の吸引ポートまたはミクロポンプなど、密閉体積内の圧力を低減可能な手動装置または電動装置であり得る。減圧源は、センサ、処理ユニット、警報指示器、メモリ、データベース、ソフトウェア、ディスプレイ装置、またはユーザインターフェースなど、減圧治療をさらに促進する他の構成要素内に収容されてもよく、またはそれらと併せて使用されてもよい。組織部位に適用される減圧の量および性質は治療要件に従って変化し得る一方、圧力は約−5mmHg(−667Pa)〜約−500mmHg(−66.7kPa)の低い陰圧であり得る。いくつかの実施形態では、圧力は約−75mmHg(−9.9kPa)〜約−300mmHg(−39.9kPa)であり得る。
一般に、滲出液および他の流体は流体経路に沿ってより低い圧力の方へ流れ得る。さらに流体は、各透過性材料間で増大する親水度または吸収度の経路に沿って各透過性材料を通って流れるように引かれ得る。従って、用語「下流」は、「上流」と称され得る構成要素よりも流体経路に沿って遠くにある構成要素を指す場合がある。同様に、特定の特徴を、流体の「入口」または「出口」の観点から、そのような基準枠で記載することが好都合であり得る。この方向性は、一般に、本明細書の負圧治療システムの様々な特徴および構成要素を記載するために仮定される。しかしながら、流体経路はまた、使用によっては(負圧源の代わりに正圧源を使用することによってなど)逆転される場合もあり、この記述的な規則は、限定的な規則として解釈するべきでない。
「減圧」は、密封治療環境の外部の局所的環境中の周囲圧力などの局所的周囲圧力より低い圧力を指す場合がある。局所的周囲圧力は、患者が置かれている大気圧でもあり得る。さらに圧力は、組織部位における組織に関連付けられる静水圧未満であり得る。特段の指示のない限り、本明細書に記載される圧力の値はゲージ圧力である。同様に、減圧の増大への言及は絶対圧力の低減を典型的に指し、一方で、減圧の低減は絶対圧力の増大を典型的に指す。
減圧治療システム500の構成要素は、直接または間接的に結合され得る。構成要素は、構成要素間で流体(例えば液体および/または気体)を移送するための経路を提供するように互いに流体結合され得る。いくつかの例示的実施形態では、構成要素は、チューブなどの導管と流体結合され得る。「チューブ」は、本明細書で使用される際、パイプ、ホース、導管、または2つの端部間で流体を搬送するように作られた1つまたは複数の管腔を有する他の細長い構造を指す場合がある。いくつかの例示的実施形態では、構成要素は、追加的または代替的に、物理的近接性によって結合され得、単一構造物に一体化されるか、または同一材料片から形成される。結合は状況によっては機械的、熱的、電気的または化学的な結合(化学結合など)も含み得る。
マニホルド540は、組織部位、吸収層502およびドレープ538と流体連通するように適合され得る。例えば、マニホルド540は、組織部位の中に、それを覆って、その上にまたはその近くに配置され得る。吸収層502は、組織部位またはマニホルド540とドレープ538との間に配置され得る。ドレープ538は、マニホルド540および吸収層502を覆って配置され得、組織部位の近くの組織に封じ付けられ得る。組織部位の近くの組織は、組織部位の近くの未損傷の表皮であり得る。その結果、ドレッシング532は、組織部位の近くに、外部環境から組織部位を実質的に隔絶する密封治療環境を提供することができる。減圧源534は、密封治療環境内の圧力を低減することができる。密封治療環境内のマニホルド540を介して均一に適用された減圧は、組織部位内に大きい歪みおよび微小な歪みを誘発可能であり、かつ滲出液および他の流体を組織部位から取り除くことができる。
いくつかの実施形態では、マニホルド540は組織部位と接触するように適合され得る。マニホルド540は組織部位と部分的または完全に接触し得る。組織部位が例えば組織表面から組織中へ延在する場合、マニホルド540は部分的または完全に組織部位を満たし得る。他の例示的実施形態において、マニホルド540は、組織部位の上に配置され得る。マニホルド540は、実行される処置の種類または組織部位の性質および寸法など、様々な要因に依存して、多くの形態を取り得、また多くの寸法、形状または厚さを有し得る。例えば、マニホルド540の寸法および形状は、深く不規則な形状の組織部位の輪郭に適合され得る。
マニホルド540は、組織部位全体にわたって減圧を分配するように、流体を組織部位全体から除去するように、またはその両方を実行するように適合された物体または構造物を含み得る。いくつかの例示的な実施形態では、マニホルド540はまた、例えば流体経路が逆転されるか、または第2の流体経路が設けられる場合、組織部位全体にわたって流体を送達することを促進し得る。マニホルド540は、マニホルド540の周囲の組織部位へ提供され、そこから除去される流体を分配する流れチャネルまたは経路を含み得る。1つの例示的な実施形態において、流れチャネルまたは経路は、組織部位へ提供されるか、または組織部位から除去される流体の分配を改善するように相互に接続され得る。例えば、気泡質発泡体、連続気泡発泡体、多孔性組織集積物、およびガーゼまたはフェルトマットなどの他の多孔性材料が、流れチャネルを形成するように配置された構造要素を含み得る。液体、ゲルおよび他の発泡体もまた、組織部位全体にわたり流体を分配するように構成された流れチャネルを含み得るか、または含むように硬化され得る。
1つの例示的な実施形態では、マニホルド540は、実質的に均一に組織部位全体にわたり減圧を分配するように適合された相互接続気泡または細孔を有する多孔質発泡材料であり得る。発泡材料は、疎水性または親水性の何れかであり得る。1つの非限定的な例では、マニホルド540は、Kinetic Concepts,Inc.(San Antonio,Texas)から入手可能なGranuFoam(登録商標)ドレッシングなどの連続気泡型網状ポリウレタン発泡体であり得る。
マニホルド540が親水性材料から作られ得る例示的な実施形態において、マニホルド540はまた、組織部位から流体を吸い上げ得る一方で、減圧組織部位に分配し続ける。マニホルド540の吸上げ特性は、毛管流または他の吸上げ機序によって組織部位から流体を引き離し得る。親水性発泡体の例は、Kinetic Concepts,Inc.(San Antonio,Texas)から入手可能なV.A.C.WhiteFoam(登録商標)ドレッシングなどのポリビニルアルコールの連続気泡発泡体である。他の親水性発泡体はポリエーテルから作製されたものを含み得る。親水特性を示し得る他の発泡体は、親水性をもたらすように処理またはコートされた疎水性発泡体を含む。
マニホルド540はさらに、密封治療環境内の圧力が低減されるとき、組織部位において肉芽形成を促進し得る。例えば、減圧がマニホルド540を通して組織部位に適用される場合に組織部位に微小な歪みおよび応力を誘発できる、平らでない、粗いまたは鋸歯状のプロファイルを、マニホルド540の表面の何れかまたは全てが有してもよい。

ドレープ538は、密封部材の例である。密封部材は、治療環境と局所的外部環境との間など、2つの構成要素間または2つの環境間の流体シールを提供するために構成され得る。密封部材は、例えば、所与の減圧源の減圧を組織部位で維持するのに適したシールを提供できる不透過性または半透過性の弾性フィルムであり得る。半透過性材料に関して、その透過性は、一般に、所望の減圧を維持できるように十分に低くあるべきである。密封部材を、未損傷表皮、ガスケットまたは別の密封部材などの取付表面に取り付けるために取付装置が使用され得る。取付装置は多くの形態をとり得る。例えば、取付装置は、密封部材の周辺、一部または全体の周りに広がる医療的に許容可能な感圧性接着剤であり得る。取付装置の他の例示的実施形態は、両面テープ、糊、親水コロイド、ヒドロゲル、シリコーンゲル、オルガノゲルまたはアクリル系接着剤を含み得る。

また、吸収層502は気体透過性であると好ましい。例えば、いくつかの実施形態では、吸収層502は繊維106を含み得、繊維106中、コア208がSAPを含み得る。減圧は、吸収層502のそのような実施形態において、繊維106間の空間または空隙によっておよびそれを通して伝達され得る。
容器542は、組織部位から引かれた滲出液および他の流体を管理するために使用可能な容器、キャニスタ、パウチまたは他の貯蔵構成要素の代表的なものである。多くの環境において、流体の収集、貯蔵および処分に剛性容器が好まれ得るか、または必要とされ得る。他の環境において、流体は剛性容器での貯蔵なしに適切に処分され得、再利用可能容器が、減圧治療に関連付けられるごみおよび費用を低減することができる。減圧治療システム500のいくつかの例示的実施形態は、容器542を含まなくてもよい。代わりに、治療システム500のこれらの例示的実施形態は、ドレッシング532および吸収層502を用いて流体の貯蔵に対処する。

いくつかの例示的な実施形態において、吸収シート656は内側654を実質的に満たし得る。他の例示的実施形態において、吸収シート656は内側654の一部のみを満たし得る。容器542の内側654は、吸収シート656が組織部位から流体を吸収する場合に吸収シート656が拡張することを可能にするように適合され得る。いくつかの実施形態において、容器542は剛性であり得、吸収シート656は内側654を実質的に満たさず、吸収シート656が拡張するための空間を残す場合がある。いくつかの実施形態において、容器542は柔軟性であり得、内側654は、吸収シート656の拡張に適応するために拡張するように構成され得る。加えて、内側654内の吸収シート656の量および配置は、容器542を通過してドレッシング532に至る減圧の流体連通を可能にするように必要に応じて変化し得る。
容器542は、流体入口648および流体出口652をさらに含み得る。いくつかの実施形態において、流体入口648は容器542の壁を通過し、内側654と、容器542の外側の領域との間の流体連通を提供し得る。いくつかの実施形態において、チューブ646が流体入口648に流体結合し得、その結果、チューブ646はドレッシング532と内側654との間の流体連通を提供し得る。流体出口652は、容器542の壁を貫いて伸び、本体内側654と容器112の外側領域との間の流体連通を提供し得る。いくつかの実施形態において、チューブ650は流体出口652と流体結合され得、減圧源534などの減圧源にさらに流体結合され得、減圧源と内側654との間の流体連通を可能にする。流体入口648および流体出口652は、図6に示されるように容器542に配置され得る。他の実施形態では、流体入口648および流体出口652は、減圧治療システム500の追加の構成要素を収容するために他の位置に配置され得る。例えば、容器542は減圧源と統合され得る。この例では、流体入口648および流体出口652は、統合された容器542および減圧源の様々な構成要素の配置に対応するように容器542の他の部分に配置され得る。
作動中、減圧源534は、減圧を、マニホルド540を含むドレッシング532に、最終的に組織部位に供給し得る。減圧は組織部位から流体を引き出し得、マニホルド540は流体を組織部位からドレッシング532へ分配し得る。組織部位から引かれた流体は高いイオン濃度を有する場合がある。ドレッシング532に到達すると、流体は、繊維106、繊維306または繊維420など、イオン交換媒体を有する繊維を含み得る吸収層502と接触し得る。創傷流体が例えば繊維106と接触する場合、フィルタ層210中のイオン交換媒体212は、ナトリウムおよび塩化物イオンなどの塩イオンを流体と交換することができる。流体のイオン塩含量は、コア208と接触する前に除去または実質的に低減され得、その結果、コア208は有意により多くの量の流体を吸収することが可能になる。
いくつかの実施形態において、組織部位から引かれた流体の大部分が吸収層502に吸収され得る一方、一部の流体はドレッシング532を通して引かれ得る。他の実施形態では、ドレッシング532は吸収層502を含まない場合があり、または吸収層502はイオン交換媒体を含まない場合がある。ドレッシング532に保持されない流体は容器542に、および容器542の内側654内へ引かれ得る。内側654に引き込まれた流体は容器542の内側654内の吸収シート656と接触し得る。吸収シート656は、上に記載したようなイオン交換繊維を有する吸収性繊維を含み得る。イオン交換繊維は、流体が吸収繊維のコアによって吸収される前、上に記載したように流体のイオン濃度を低減することができる。コアに進入する流体は低減されたイオン濃度を有し得、これにより吸収シート656による吸収効果を高めることが可能になり、これはまた容器542の収容能力を増大することもできる。
本明細書に記載したシステム、装置および方法は著しい利点を提供し得、そのいくつかは既に記載した。例えば、イオン交換繊維は、吸収繊維の吸収能力を大幅に向上させることができる。向上した吸収能力により、ドレッシングまたは容器中で使用される吸収材の量を低減することが可能になり得、これは例えばより小さいドレッシングまたは容器の使用を可能にする。加えて、より少ない吸収材料は、潜在的な費用削減を実現することができる。例えば、SAPの使用を低減することは、吸収材を製造する費用を低減し得、および例えばドレープまたは吸上げ層など、他の材料の費用を低減し得る。ドレッシングのグラム対グラムの流体処理能力は、蒸発などの他の要因に依存することなく改善可能である。
加えて、高吸収材を有するイオン交換繊維は、低プロファイルドのドレッシングを製造するのに有利であり得る。例えば、上に記載した方法で高吸収性コアとIEMとを一緒に組み合わせることにより、吸収ドレッシングの正味効果は著しく改善され得る。いくつかの例では、例えば、正味効果は、ドレッシングが圧縮されているかどうかにかかわらず、約25%増大することができる。さらに、IEMを含む水透過層を高吸収性コアの周囲に配置することにより、構造的一体性が吸収構造に追加され得、これは完全吸収能力時またはドレッシング交換中、構造が分離することを防止し得る。同様に、IEMを吸収繊維に結合することにより、IEMが遊離するまたは移動可能になる可能性が低減される。
いくつかの例示的実施形態が減圧治療用途に関連して記載される一方、特徴および利点の多くは他の環境および産業に容易に適用可能である。
特定の説明に役立つ非限定的な例示的実施形態を提示してきたが、様々な変更形態、交換形態、置換形態および修正形態が、付随の請求項の範囲から逸脱することなくなされ得る。何れか1つの例示的実施形態に関連して記載された何れかの特徴は、他の何れかの例示的実施形態にも適用可能であり得る。
さらに、本明細書に記載される方法のステップは、任意の適切な順番で、または適切な場合には同時に実行されてもよい。
Claims (41)
- 繊維において、
吸収コアと、
前記吸収コアの周囲に配置されたフィルタ層と、
前記フィルタ層中に配置され、かつ流体のイオン濃度を低減するように適合されたイオン交換媒体と
を含むことを特徴とする繊維。 - 請求項1に記載の繊維において、前記フィルタ層が前記吸収コアにコーティングされることを特徴とする繊維。
- 請求項1に記載の繊維において、前記フィルタ層が前記吸収コアを取り囲むことを特徴とする繊維。
- 請求項1乃至3の何れか1項に記載の繊維において、前記イオン交換媒体がカチオン交換媒体を含むことを特徴とする繊維。
- 請求項1乃至3の何れか1項に記載の繊維において、前記イオン交換媒体がアニオン交換媒体を含むことを特徴とする繊維。
- 請求項1乃至3の何れか1項に記載の繊維において、前記イオン交換媒体が、
カチオン交換媒体と、
アニオン交換媒体と
を含み、前記カチオン交換媒体および前記アニオン交換媒体が前記フィルタ層全体に分散されることを特徴とする繊維。 - 請求項1に記載の繊維において、前記イオン交換媒体が、酸性ポリマーでドープまたはグラフトされた架橋ポリマーから形成された多孔性ビーズを含むことを特徴とする繊維。
- 請求項7に記載の繊維において、前記架橋ポリマーがポリスチレンを含み、および前記酸性ポリマーが、ポリ(2−アクリルアミド−2−メチル−1−プロパンスルホン酸)およびポリ(アクリルアミド−N−プロピルトリメチルアンモニウムクロリド)を含むことを特徴とする繊維。
- 請求項1に記載の繊維において、前記イオン交換媒体がアルカリ性ポリマーを含むことを特徴とする繊維。
- 請求項9に記載の繊維において、前記アルカリ性ポリマーがポリ(アクリルアミド−N−プロピルトリメチルアンモニウムクロリド)であることを特徴とする繊維。
- 請求項1に記載の繊維において、前記イオン交換媒体がゼオライトを含むことを特徴とする繊維。
- 請求項1乃至11の何れか1項に記載の繊維において、前記フィルタ層が水透過性であることを特徴とする繊維。
- 請求項1乃至11の何れか1項に記載の繊維において、前記フィルタ層がポリマーコーティングを含むことを特徴とする繊維。
- 請求項13に記載の繊維において、前記ポリマーコーティングが、架橋ポリビニルアルコール、ポリビニルピロリドン、ポリウレタン、ポリアクリルアミド、およびポリエチレンオキシドの少なくとも1つを含むことを特徴とする繊維。
- 請求項1乃至14の何れか1項に記載の繊維において、前記吸収コアが高吸収性材料を含むことを特徴とする繊維。
- 請求項15に記載の繊維において、前記高吸収性材料が高吸収性ポリマーであることを特徴とする繊維。
- 請求項16に記載の繊維において、前記高吸収性ポリマーがヒドロゲルであることを特徴とする繊維。
- 組織部位を処置するための吸収ドレッシングにおいて、
前記組織部位からの流体のイオン濃度を低減するように適合されたイオン交換媒体と、
前記低減されたイオン濃度を有する流体を吸収するように適合された吸収材と
を含むことを特徴とする吸収ドレッシング。 - 請求項18に記載の吸収ドレッシングにおいて、前記吸収材が複数の吸収繊維へと形成されることを特徴とする吸収ドレッシング。
- 請求項18または19に記載の吸収ドレッシングにおいて、前記イオン交換媒体がカチオン交換媒体を含むことを特徴とする吸収ドレッシング。
- 請求項18または19に記載の吸収ドレッシングにおいて、前記イオン交換媒体がアニオン交換媒体を含むことを特徴とする吸収ドレッシング。
- 請求項18乃至21の何れか1項に記載の吸収ドレッシングにおいて、前記イオン交換媒体が前記吸収材の周囲のフィルタ層中に分散されることを特徴とする吸収ドレッシング。
- 請求項18に記載の吸収ドレッシングにおいて、
前記吸収材が吸収ストランドを含み、および
前記イオン交換媒体が前記吸収ストランドの周囲で撚られたストランド中に分散されることを特徴とする吸収ドレッシング。 - 請求項18乃至23の何れか1項に記載の吸収ドレッシングにおいて、前記吸収材が高吸収性材料を含むことを特徴とする吸収ドレッシング。
- 請求項24に記載の吸収ドレッシングにおいて、前記高吸収性材料が高吸収性ポリマーであることを特徴とする吸収ドレッシング。
- 請求項18に記載の吸収ドレッシングにおいて、前記吸収材が繊維コアであり、および前記イオン交換媒体が前記繊維コアの周囲の層中に分散されることを特徴とする吸収ドレッシング。
- 請求項26に記載の吸収ドレッシングにおいて、前記イオン交換媒体が前記繊維コアの周囲のポリマーコーティング中に分散されることを特徴とする吸収ドレッシング。
- 組織部位を処置するためのシステムにおいて、
減圧源と、
前記減圧源と流体連通するように適合された吸収層であって、
前記組織部位からの流体を吸収するように適合された吸収材料、および
前記流体のイオン濃度を低減するように適合されたイオン交換媒体
を含む吸収層と、
前記吸収層を覆うように適合されたドレープと
を含むことを特徴とするシステム。 - 請求項28に記載のシステムにおいて、容器をさらに含み、前記容器は、その中に配置された吸収シートを有し、かつ前記組織部位および前記減圧源と流体連通するように適合されていることを特徴とするシステム。
- 請求項28または29に記載のシステムにおいて、前記吸収層と前記組織部位との間に配置されるように適合されたマニホルドをさらに含むことを特徴とするシステム。
- 請求項28乃至30の何れか1項に記載のシステムにおいて、前記イオン交換媒体が、酸性ポリマーでドープまたはグラフトされた架橋ポリマーから形成された多孔性ビーズを含むことを特徴とするシステム。
- 組織部位からの流体を貯蔵するための容器において、
内側、流体入口および流体出口を有する本体と、
前記内側内に配置された吸収層であって、
吸収材料、および
前記内側内の流体のイオン濃度を低減するように適合されたイオン交換媒体
を含む吸収層と
を含むことを特徴とする容器。 - 請求項32に記載の容器において、前記イオン交換媒体が前記吸収材料の周囲に配置されて繊維を形成することを特徴とする容器。
- 請求項32に記載の容器において、前記イオン交換媒体が、酸性ポリマーでドープまたはグラフトされた架橋ポリマーから形成された多孔性ビーズを含むことを特徴とする容器。
- 請求項34に記載の容器において、前記架橋ポリマーがポリスチレンを含み、および前記酸性ポリマーが、ポリ(2−アクリルアミド−2−メチル−1−プロパンスルホン酸)およびポリ(アクリルアミド−N−プロピルトリメチルアンモニウムクロリド)を含むことを特徴とする容器。
- 請求項32に記載の容器において、前記イオン交換媒体がゼオライトを含むことを特徴とする容器。
- 組織部位を処置するための方法において、
前記組織部位の近くに吸収層を配置するステップと、
ドレープを用いて前記組織部位に前記吸収層を取り付けるステップであって、前記吸収層が、
吸収材料、および
前記吸収層に引き込まれる流体のイオン濃度を低減するように適合されたイオン交換媒体
を含む、ステップと
を含むことを特徴とする方法。 - 請求項37に記載の方法において、
前記組織部位と前記吸収層との間にマニホルドを配置するステップと、
減圧源を前記マニホルドに流体結合するステップと、
減圧を前記組織部位に供給して流体を前記組織部位から前記吸収層へ引くステップと
をさらに含むことを特徴とする方法。 - 請求項38に記載の方法において、容器を前記マニホルドと前記減圧源との間で流体結合するステップをさらに含み、第2吸収層が前記容器内に配置されることを特徴とする方法。
- 組織部位を処置するためのシステムにおいて、
減圧を提供するための減圧源と、
流体を前記組織部位から収集するために前記組織部位および前記減圧源と流体連通するように適合された流体貯蔵部材と、
減圧を受け、かつ前記減圧を前記組織部位全体へ分配するように適合されたマニホルドと、
前記マニホルドを覆い、かつ前記組織部位に実質的に流体密のシールを提供するように適合されたドレープと、
前記減圧源と流体連通するように適合された吸収層であって、
支持体、および
複数の多層繊維であって、各多層繊維が吸収コアの周囲に配置されたイオン交換媒体を含む、複数の多層繊維
を含む吸収層と
を含むことを特徴とするシステム。 - システム、装置および方法において、実質的に本明細書に記載されるようなものであることを特徴とするシステム、装置および方法。
