Abstract
Description
(1)本発明によって抗生剤とともにリゾホスファチジルコリンまたはこれの類似体を併用投与すれば、免疫増強または細菌性感染疾患の治療効果の面で相乗効果を発揮する。
(2)本発明は感染を通じて免疫系が抑制された疾患、及び免疫系が抑制された後に感染された疾病の治療などに有用に利用されることができ、既存の抗生剤またはリゾホスファチジルコリンの治療効果を顕著に向上させることができる。
DW286([7−[3−(aminomethyl)−4−(methoxyimino)−3−methyltetrahydro−1H−1−pyrrolyl]−1 cyclopropyl−6 fluoro−4−oxo−1, 4−dihydro[1,8] naphthyridine−3−carboxylic acid hydrochloric acid salt], Lpt, 20021)は同和薬品(Anyang, Korea)によって合成された新規のフルオロ−ナフチリジン抗生剤であり、LPC(リゾホスファチジルコリン;18:0 94%、GmbH PHISPHOLIPID、ドイツ)を使用した。LPCの異性体(isomer)である1−オレオイル−2−ヒドロキシ−sn−グリセロ−3−ホスホコリン(1−Oleoyl−2−hydroxy−sn−glycero−3−phosphocholine, 1−Oleoyl;18:1 LPC)及びシプロフロキサシン塩酸塩水和物(Ciprofloxacin hydrochloride hydrate, Cipro,)、ベンジルペニシリンカリウム(Potassium benzylpenicillin, Peni)、セフトリアキソンナトリウム(Ceftriaxone sodium, Ceft)、ドリペネム(Doripenem, Dori)、塩酸バンコマイシン(Vancomycin hydrochloride, Vanco)、活性型ドロトレコジンアルファ(Drotrecogin alfa(activated), Xigris)、コリスチン(colistin)、トブラマイシン(tobramycin)、及びフシジン酸(fusidic acid, fusidin)はそれぞれシグマなどから購入した。LPC、DW286、シプロフロキサシン塩酸塩水和物、ベンジルペニシリンカリウム、セフトリアキソンナトリウム、ドリペネム、塩酸バンコマイシン及び活性型ドロトレコジンアルファはデシケーターに保管されて光と湿気による変性から保護した。
2−1.CPA誘導された免疫抑制マウスモデルの準備
手術3日及び1日前にそれぞれ150mg/kg及び110mg/kgの CPA(Cyclophosphamide・H2O、Sigma、USA)を生理食塩水に溶かして10ml/kgの濃度で単回腹腔注射して免疫抑制を誘発させた。INTACT control群ではCPAの代わりに同一量の生理食塩水を同一方法で投与した。
免疫抑制剤としてよく知られているCPAを、多菌性感染を増幅させるためにCLP実施の3及び1日前にそれぞれ150mg/kgと110mg/kgのCPA(Sigma, USA)を生理食塩水に溶かして10ml/kgの濃度で単回腹腔注射して免疫抑制を誘発させた。このように、CPA処理後にCLPを実施すれば、細菌の大量感染モデルとして使用することができる。CLPのためにマウスをケタミン塩酸塩(Ketamine hydrochloride, ICN Biochemicals Inc., USA)及びキシラジン塩酸塩(Xylazine hydrochloride, Wako Pure Chemical Industries Ltd., Japan)の麻酔下に開腹して盲腸を露出し、露出した盲腸は回盲弁(Ileocecal valve)のすぐ下を二重結紮した後、22ゲージの注射針を用い2回貫通させた後、通常の方法に準じて腹腔を閉鎖した。INTACT及びCPA control群では盲腸露出後、再度腹腔を閉鎖し、INTACT control群ではCPAの代わりに同一量の生理食塩水を同一方法で投与した。
3−1.CPA誘発免疫抑制マウスモデル
免疫抑制を誘発させるために、150mg/kgと110mg/kgのCPAを生理食塩水に溶かしてCLP手術の3及び1日前または最初投与時に単回腹腔内に投与した。INTACT control群では、CPAの代わりに同一量の生理食塩水を同一方法で投与した。
マウスはケタミン(Ketamine)及びキシラジン(Xylazine)によって麻酔下に開腹して盲腸を露出し、露出した盲腸は回盲弁(Ileocecal valve)のすぐ下を二重結紮した後、22ゲージの注射針を用いて2回貫通させた後、通常の方法に準じて腹腔を閉鎖した。INTACT及びCPA control群では盲腸露出後、再度腹腔を閉鎖し、INTACT control群ではCPAの代わりに同一量の生理食塩水を同一方法で投与した。CLPはCPAの二回目の投与後、一日後に行った。
CPA−CLPモデルで、体重、生存数及び死亡数は5日間1日に1回ずつ automatic electronic balances(Sartorius Co., Ltd., USA)測定し、CPA処理モデルでは2日間1日に1回ずつ測定した。増体量は次のように計算した:

CPA−CLP model:犠牲当時の体重−CLP時の体重(5days)、
CPA−treat model:犠牲当時の体重−最初投与時の体重(2days)。
CPA処理モデルの犠牲時に脾臓及び胸腺の湿重量(wet−weight)を絶対重量で測定し、その後、相対的な臓器重量(体重の%)を下記式によって計算した:
相対的な臓器重量=[(絶対的な臓器重量/犠牲当時の個別的な体重)×100]。
血液はCPA処理モデルの大静脈から収集し、血中総白血球数はカウンティングチャンバーを用いて計算し、希釈ピペット(diluting pipette)と希釈溶液としてTurk溶液を使用して計算した。全ての数字は×103/mm3で計算した。さらに、ギムザ液で染色された塗抹血液サンプル内で好酸球、好中球、単球及び好塩基球の細胞数を総100白血球中で計算した。
CPA処理モデルの骨髄標本は次のように作製された。つまり、骨髄細胞は非活性化されたウシ胎児血清3ml内の大腿骨から収集し(GIBCO BRL, USA)、遠心分離して、スライドに塗抹した。標本は乾燥させ、absolute methanolに浸して10−20分間固定した。固定されたスライドは下記のような方法で染色した:
メイ・グリュンワルド染色(May−Grunwald stain) 3分、
メイ・グリュンワルド染色(May−Grunwald stain)(1:1 diluted) 2分、
ギムザ染色(1:6 diluted) 10分。
スライドは無作為にコードされ、2名の他の専門家によって1000倍拡大下に観察した。PCE(多染性赤血球、Polychromatic erythrocyte)の直径の1/5から1/20の範囲のサイズの小さくて丸いか楕円形の胴体は小核赤血球数として計数した。結果は1000PCE内のMNPCE(小核多染性赤血球、PCE with one or more nuclei)の数として表現した。MNPC±S.D.の平均数字を計算した。さらに、PCE/(PCE+NCE)の割合は細胞毒性の可能性を探知するために500赤血球を計算した。
器官の重量を測定した後、胸腺と脾臓をサンプル化した。サンプル組織は、10%中性緩衝ホルマリンに固定した。パラフィンで包埋させた後、3−4μm標本を準備した。代表的な部分は光学顕微鏡検査のためにヘマトキシリンとエオシン(H&E, Hematoxylin & Eosin Stain)で染色した。その後、個別的な器官の組織病理学的プロファイルを観察した。
脱パラフィン化した後に、CD3抗原決定基の検索は10mM Tris−1mM EDTAバッファー(pH9.0)で行い、1mM EDTAバッファー(pH8.0)及びTNF−α in 10mM Citrate Buffer(pH6.0)でCD4及びCD8の検索を上記方法と同一に行った。本実験に使用された1次抗血清は下記表1のとおりである。
平均及び標準偏差(平均±S.D.)を計算した。統計学的分析はMann−Whitney U−Wilcoxon Rank Sum W test(MW test) with SPSS for Windows(登録商標)(Release 6.1.3., SPSS Inc., USA)を使用して行った。CPA処理モデルでINTACT及びCPA対照群、CPA対照群及びテスト群間の差に対するテスト物質の効率の理解を助けるために、次の式を使用した:
INTACT controlに対する変化率(%)=[((CPA controlのデータ−INTACT controlのデータ)/INTACT controlのデータ) × 100]、
CPA controlに対する変化率(%)=[((テスト群のデータ−CPA controlのデータ)/CPA controlのデータ) × 100]。
CPAで免疫抑制を誘発させた後、CLP(Cecal ligation及びpuncture)で敗血症を誘発させたマウスを用いて、LPCの投与用量別の薬効を確認した。5用量(1、2.5、5、10及び20mg/kg)のLPCをCPA−CLP敗血症マウスにCLP手術の6時間後から12時間間隔で4回皮下投与した後、死亡率及び体重の変化を観察した。LPCは5% human albumin(緑十字、Korea)に溶解させて10ml/kgでそれぞれ投与した。このような投与群と投与順序を図1に簡単に示す。
CPAで免疫抑制を誘発させた後、CLPで敗血症を誘発させたマウスを用いて、LPCとXigris、LPA、18:1 LPC、18:0 LPC及びDW286AAの薬効を比較した。5mg/kgのLPC、LPA、18:1 LPC、18:0 LPC、0.4及び2mg/kgのXigris、10及び20mg/kgのDW286AAをそれぞれCPA−CLP敗血症マウスにCLP手術の6時間後に単回静脈投与した後、死亡率及び体重の変化を観察した。全ての実験物質は滅菌生理食塩水に溶解させて10ml/kgでそれぞれ投与した。このような投与順序と投与群は図2に簡単に示す。
CPAで免疫抑制を誘発させた後、CLPで敗血症を誘発させたマウスを用いて、LPCとCipro(Ciprofloxacin hydrochloride hydrate)、Peni(Potassium benzylpenicillin)及びCeft(Ceftriaxone sodium)の併用効果を比較した。LPC(5mg/kg)、Cipro(20mg/kg)、Peni(60mg/kg)、Ceft(25mg/kg)、LPC+Cipro(5+20mg/kg)、LPC+Peni(5+60mg/kg)及びLPC+Ceft(5+25mg/kg)をそれぞれCPA−CLP敗血症マウスにCLP手術の6時間後に単回静脈投与した後、死亡率及び体重の変化を観察した。また、DW286AA(5mg/kg)を対照薬物として使用し、全ての単一実験物質は滅菌生理食塩水に溶解させて10ml/kgでそれぞれ投与し、全ての複合組成群ではそれぞれの薬物をLPC(0.5mg/ml)が溶解されている溶液に直接適切用量をそれぞれ溶解させて投与した。このような投与順序と投与群は図3に簡単に示す。
CPAで免疫抑制を誘発させた後、CLPで敗血症を誘発させたマウスを用いて、LPC、Dori(Doripenem)、Vanco(Vancomycin hydrochloride)及びXigris(Drotrecogin alfa(activated))の併用効果を比較した。LPC(5mg/kg)、DW286AA(5mg/kg)、Dori(200mg/kg)、Vanco(10mg/kg)、Xigris(2mg/kg)、LPC+DW286AA(5+5mg/kg)、LPC+Dori(5+200mg/kg)、LPC+Vanco(5+10mg/kg)及びLPC+Xigris(5+2mg/kg)をそれぞれCPA−CLP敗血症マウスにCLP手術の6時間後に単回静脈投与した後、死亡率及び体重の変化を観察した。全ての単一実験物質は滅菌生理食塩水に溶解させて10ml/kgでそれぞれ投与し、全ての複合組成群ではそれぞれの薬物をLPC(0.5mg/ml)が溶解されている溶液に直接適切用量をそれぞれ溶解させて投与した。このような投与順序と投与群は図4に簡単に示す。
CPAで免疫抑制を誘発させた後、CLPで敗血症を誘発させたマウスを用いて、LPCとColistin、Tobra(Tobramycin)及びFusidin(fusidic acide sodium)の併用効果を比較した。LPC(5mg/kg)、colistin(5mg/kg)、tobramycin(4mg/kg)、fusidin(80mg/kg)、LPC+colistin(5+5mg/kg)、LPC+tobramycin(5+4mg/kg)及びLPC+fusidin(5+80mg/kg)をそれぞれCPA−CLP敗血症マウスにCLP手術の6時間後に単回静脈投与した後、死亡率及び体重の変化を観察した。また、DW286AA(5mg/kg)を対照薬物として使用し、全ての単一実験物質は滅菌生理食塩水に溶解させて10ml/kgでそれぞれ投与し、全ての複合組成群ではそれぞれの薬物をLPC(0.5mg/ml)が溶解されている溶液に直接適切用量をそれぞれ溶解させて投与した。このような投与順序と投与群は図5に簡単に示す。
敗血症に対する新しい治療剤の開発過程の中でLPC及びDW286AAの最適な併用投与方法を捜すために、CPA−CLP及びCPAで誘発された免疫抑制マウスを用いて評価した。CPA−CLPマウス及びCPA誘発免疫抑制マウスにLPC及びDW286AAを単独、混合(LPC:DW286AA 2:1)、LPC先投与後(2回) DW286AA後投与(2回)、DW286AA先投与後 LPC後投与後の死亡率、胸腺及び脾臓重量、血中白血球数、胸腺及び脾臓内CD3+、CD4+、CD8+及びTNF−α+細胞の数的変化を骨髄内小核多染性赤血球数の変化とともに評価した。
上記実施例3−3の方法で、死亡数、生存数及び体重を評価した。その結果、INTACT及びCPA CONTROL群では観察期間中に死亡例が全然認められなかったが、CPA−CLP CONTROL群ではCLP1日及び2日後にそれぞれ6及び3例の死亡例が認められ、CLP2日後に全ての実験動物(9/9, 100%)が死亡した(表12)。一方、LPC、LPC−DW286及びDW286群ではCLP4日後に全ての実験動物が死亡し、Mix及びDW286−LPC群ではCLP5日後に全ての実験動物が死亡し、Mix群に比べてDW286−LPC群でより高い初期生存率が認められた(表12)。
全てのCPA処理群はINTACT controlに比べて有意的な体重の減少を見せた(p<0.01)。本発明のCPA control群に比べて全てのテスト群で体重の意味ない変化が感知された。さらに、CPA controlを含む全てのテスト群で犠牲物に最初の投与後、類似する体重の増加を観察した(表14)。
上記実施例3−4の方法で臓器重量を測定した。その結果、INTACT controlに比べてCPA controlで相対的で絶対的な脾臓及び胸腺の重量の減少を観察した。しかし、有意的ではない胸腺重量の増加をLPC、Mix及びDW286で観察し、有意的な脾臓重量の増加を(p<0.01)、CPA controlに比べてLPC、Mix及びDW286−LPCでそれぞれ観察した(表15)。
上記実施例3−5の方法で血液収集及びWBCを計算した。その結果、INTACT controlに比べて全てのCPA誘発群では有意的な(p<0.01)血中総白血球数の減少が生じ、CPA controlに比べてDW286投与群を除く全ての投与群で顕著な総白血球数の増加が観察されたが、有意的な(p<0.01)増加はDW286−LPC投与群に限って見られた(表16)。
分別countの結果、CPAによる白血球の減少は主にリンパ球減少による変化と観察され、相対的に好中球及び単球の割合の増加がもたらされたものと観察された(表16)。一方、本実験の結果、LPC及びDW286−LPC併用投与はCPA投与によって誘発される白血球の減少を比較的効果的に抑制するものと観察され、LPC単独投与よりもDW286−LPC投与群でより優れた効果が認められた。
上記実施例3−6の方法で骨髄小核多染性赤血球の変化を観察した。その結果、CPAは突然変異発生率を高める物質と知られており、骨髄内の小核多染性赤血球を顕著に増加させるものと知られている。INTACT controlに比べてCPA controlでは小核多染性赤血球(MNPCEs)は有意的に増加し(p<0.01)、PCE/(PCE+NCE)は減少した。全ての投与群でCPA controlに比べて意味ある小核多染性赤血球及びPCE割合の変化が生じなかった(表17)。
上記実施例3−7の方法で組織病理的変化を観察した。その結果、胸腺皮質でリンパ球細胞の減少及び顕著な脾臓萎縮がCPA controlで探知された。しかし、このような萎縮性の変化はLPC、LPC−DW286及びDW286−LPCの処理によって効果的に抑制され、DW286−LPC群は他の群に比べて最も高い抑制傾向を見せた(図7及び8)。組織形態計測法で、脾臓での白脾髄の数は(p<0.01)、INTACT controlに比べてCPA controlで有意的に減少されたが、DW286単独投与群を除く全ての投与群で有意的に増加した。さらに、胸腺皮質でのリンパ性細胞の減少はやはり、DW286群を除くCPA controlに比べて全ての投与群で顕著に抑制された(表18)。
上記実施例3−8の方法で免疫組織化学分析を行った。その結果、脾臓のCD3+、CD4+、CD8+及びTNF−α+細胞の有意的な減少(p<0.01)はINTACT controlに比べてCPA controlで観察され、CD3+及びTNF−α+細胞の有意的な減少は胸腺の皮質及び髄質でそれぞれ観察された。しかし、このような細胞はDW286群を除く全ての投与群でCPA controlに比べて有意的に増加した。特に、LPC単独投与群よりもDW286−LPC群で類似するかより優れた減少抑制効果が認められた(表19及び図9ないし16)。
Claims (2)
- 免疫増強用の薬剤学的組成物であって、
(i)L−α−リゾホスファチジルコリン、ステアロイル、及び(ii)抗生剤を有効成分として含み、
前記抗生剤が、DW286、シプロフロキサシン塩酸塩水和物、セフトリアキソンナトリウム、ドリペネム、塩酸バンコマイシン、ドロトレコジンアルファ、コリスチン、トブラマイシン、及びフシジン酸から構成された群より選択されることを特徴とする免疫増強用の薬剤学的組成物。 - 免疫増強用のキットであって、
(i)L−α−リゾホスファチジルコリン、ステアロイル、及び(ii)抗生剤が、単一または別個の容器に入れられており、
前記抗生剤が、DW286、シプロフロキサシン塩酸塩水和物、セフトリアキソンナトリウム、ドリペネム、塩酸バンコマイシン、ドロトレコジンアルファ、コリスチン、トブラマイシン、及びフシジン酸から構成された群より選択されることを特徴とする免疫増強用のキット。
