Abstract
Description
本願は、2008年12月30日出願の米国特許出願第61/141,421号の優先権を主張するものであり、出願の全体が参照により本明細書に組み込まれる。
本発明は、血清バイオマーカーを使用して、強直性脊椎炎と診断された患者の抗−TNFα生物学的治療を用いた処置に対する応答を予測するための方法及び手順に関する。
「バイオマーカー」は、Biomarkers Definitions Working Group(Atkinsonら、2001 Clin Pharm Therap 69(3):89〜95)による、正常な生物学的過程、発病過程、又は治療的介入に対する薬理応答の客観的指標として、客観的に測定及び評価される特徴として定義される。したがって、解剖学的又は生理学的過程は、タンパク質、遺伝子発現(mRNA)、小分子、代謝産物又は無機質のレベルが可能なように、例えば挙動の範囲のようなバイオマーカーとしての役割を果たすが、但しバイオマーカーと、関連する生理学的、毒物学的、薬理学的又は臨床成績との間に、検証された関係が存在することを条件とする。

1.ASを有する患者におけるゴリムマブ等の抗−TNF処置に対する応答又は非応答に関連したバイオマーカーを同定するための血清の使用。
2.抗−TNF療法を開始する前に、診断されたAS患者からの血清中に存在するバイオマーカーを使用して、ゴリムマブ等の抗−TNFα Mab処置に対する応答又は非応答を予測する能力。
3.抗−TNF療法で処置されたASを有する患者における成果を予測するためのアルゴリズム。
a.第14週におけるAS患者の抗−TNFαに対する臨床応答又は非応答は、抗−TNF療法の開始前に、診断されたAS患者の血清中に存在するバイオマーカーを使用して、評価時(第0週)に予測され得る。
b.第14週におけるAS患者の抗−TNFα処置に対する臨床応答又は非応答は、治療法の開始前(第0週)に得られたベースライン値からのバイオマーカーと、治療法の開始後の第4週でのバイオマーカーとの変化を使用して予測され得る。
c.第14週におけるAS患者の抗−TNFα処置に対する臨床応答又は非応答は、治療法の開始前(第0週)に得られたベースライン値からのバイオマーカーの変化を、治療法の開始後の第4週でのバイオマーカーの変化と組み合わせて使用して予測され得る。
4.本発明のマーカーを使用して、AS患者の抗−TNFα療法に対する応答又は非応答を予測する手段を含む装置、システム及びキット。
任意の好適な学習アルゴリズムを用いて、適切な参照又は訓練データセットを使用して、分類に使用されるべき分析プロセスのパラメータを決定し、即ち予測的モデルを開発する。

本発明の一態様では、ASと診断された患者における血清マーカーの分析は、バイオマーカーのベースライン値と抗−TNFα療法に対する応答との間の有意な関係に焦点を当てた。本発明の別の態様では、ASと診断された患者における血清マーカーにおいて、ベースライン(抗−TNFα療法の前)から治療後の第4週への血清マーカーにおける変化の分析は、後の時間(第14週)の患者の臨床応答又は非応答に関連していた。
予測的アルゴリズムをベースラインバイオマーカー血清濃度値のみを含むデータセットから構築した際、抗−TNFα療法で処置したAS患者の臨床応答を、臨床応答を評価するASAS20及びBASDAI等の2つ以上の方法と相関付け、マーカーはレプチン、TIMP−1、CD40リガンド、G−CSF、MCP−1、オステオカルシン、PAP,及びインスリンを含んでいた。
臨床応答を評価するASAS20及びBASDAI等の2つ以上の方法において、臨床応答と相関することが見出された、AS患者における、ベースライン血清レベルから第4週へのバイオマーカー変化は、レプチン、VEGF、補体3、ICAM−1及びフェリチンを含む。
診断されたAS患者の抗−TNF療法に対する応答を予測するための血清バイオマーカーの測定は、本発明に記載される標準的な免疫化学的及び生物物理学的方法を使用して、臨床検査室又は研究所、又は病院若しくは病院以外の場所の中心研究所で実行されてもよい。マーカー定量化は、WBCカウント、血小板及びESR等の他の標準的な測定と同一の時間に実行されてもよい。分析は、市販のキットを使用して個別に若しくはバッチで、又は個々の患者サンプルに多重化分析を用いて実行されてもよい。
いくつかの実施形態では、ニューラルネットワークが使用される。ニューラルネットワークは、マーカーの選択されたセットのために構成され得る。ニューラルネットワークは、二段階回帰又は分類モデルである。ニューラルネットワークは層構造を有し、層構造は、重み層により出力ユニットの層に接続された入力ユニット(及びバイアス)の層を含む。回帰のために、出力ユニットの層は、典型的には1つのみの出力ユニットを含む。しかしながら、ニューラルネットワークは、途切れのない様式で、多数の定量的応答を取り扱うことができる。
別の態様では、本発明は、ゴリムマブ等の抗−TNFα剤を用いた処置に応答するか又は応答しないかを決定するためのキットを提供し、キットは、本発明に従って血清マーカーを検出するのに使用される。キットは、AS患者内で異なるように存在する血清マーカー及びマーカーの組み合わせの存在に関してスクリーニングする。
本発明は、ASと診断された患者において検出されたバイオマーカーを分析することによる、ゴリムマブ等の抗−TNFα剤による治療法に対する応答性を予測する方法を提供する。本発明の方法では、患者は、主観的及び客観的基準を使用して、経験豊かな専門家によりASと最初に診断される。
ASの診断は、1984 Modified New York Criteria(van der Linden S,Valkenburg HA,Cats A:Evaluation of diagnostic criteria for ankylosing spondylitis.A proposal for modification of New York criteria.Arthritis Rheum 27:361〜368,1984)により規定されるいくつかの撮像技術による仙腸関節炎の臨床的特徴と証拠との組み合わせにより行われる。赤血球沈降速度(ESR)及びC−反応性タンパク質(CRP)レベル等の、疾病の研究室マーカーは、疾病活性の評価、又は処置に対する応答の監視には役立たないことが示されている(Spooorenberg Aら、1999 J Rheumatol 26:980〜4)。
ASASは、ASにおける抗−TNF療法の必要性に関する合意書を準備している(Braunら、2003 Annals Rheumatic Diseases 62:817〜824)。軸方向疾病、末梢関節及び腱付着部炎のASの3つの全提示に関して、処置の失敗を少なくとも3ヶ月の標準的なNSAID処置の試験として定義した。抗−TNF療法を開始する前、患者は、最大推奨又は最大許容抗−炎症投与量の使用に基づいて、これらの薬物が矛盾しない限り、少なくとも2つのNSAIDの十分な治療的試験を有している必要がある。
症候性軸方向疾病では、抗−TNF療法の開始前に追加の処置を必要としない。
症候性末梢関節では、少関節炎において関節内コルチコステロイド処置(少なくとも2回の注射)の失敗が通常必要とされる。矛盾せず又は許容不可能でない限り、3g/日迄のm最大許容投与量における、スルファサラジンを用いた標準的なDMARD処置を4ヶ月間処方される必要がある。
症候性腱付着部炎では、少なくとも2回の局所ステロイド注射からなる十分な治療的試験が、これらの注射が矛盾しない限り、通常必要である。
インフリキシマブ等の抗−TNFα剤は市販されており、数年に亘りASの処置に使用されている。抗−TNFα剤は、強直性脊椎炎の劇的な改善をもたらし、疾病の異なる症状を寛解させ、生活の質を向上させることが示されている。AS患者は、臨床評価を越える追加の基準、場合により、NSAIDs及び理学療法、スルファサルジン(sulfasalzine)又はメトトレキサート又はビスホスホネート等の等の代替療法に応答しないことに基づいて、抗−TNFα療法の候補者として考慮され得る。
抗−TNF療法に対する早期応答を予測又は評価するための本発明の方法では、抗−TNF療法の開始前の「ベースライン訪問」時に、抗−TNF療法で処置される患者からベースライン、即ち、「第0週」のサンプルを獲得する。サンプルは、本発明の方法に関連したバイオマーカーを評価し得る任意の組織であってもよい。一実施形態において、サンプルは、血液、血清、尿、***及び便からなる群から選択される流体からなる群から選択される流体である。特定の実施形態では、サンプルは、直接静脈穿刺又は静脈カテーテルを介した標準的な方法により引かれた患者の血液から得られた血清サンプルである。
Centocor Protocol C0524T09、多施設ランダム化、二重盲、プラセボ対照、3−アーム試験に登録した患者から血清サンプルを獲得し評価した。3つのグループは、プラセボと、抗−TNFα Mab処置の2つの用量レベルとからなり、ゴリムマブ50mg、又はゴリムマブ100mgが活動性強直性脊椎炎を有する患者内の4週間毎のSC注射として投与される。一次有効性評価は、第14週及び第24週に行った。バイオマーカー試験用の血清サンプルを、100人の患者からベースライン(第0週)、第4週及び第14週にて収集した。
所定のマーカーが多重試験メニューに含まれなかったため、Rules Based Medicine多重分析に加えて単一のEIA方法を使用して、血清バイオマーカーデータの追加のセットを生成した。追加のマーカーを多重バイオマーカーデータセットと組み合わせて、単一及び多重マーカーの組み合わせに基づくモデル正確性を決定した。これらのデータは、予測的モデルの一部のみとして含まれた。
予測的モデル又はアルゴリズムを構築するために、マーカーデータを試験の臨床エンドポイントに関連して評価した。この試験では、ASAS20第14週、ASAS20第24週、BASMIにおける変化第14週、BASFIにおける変化第14週、及びBASDAIにおける変化第14週と規定される6つの臨床エンドポイントが存在した。これらの試験エンドポイントは、患者の疾病状態を評価するための一般に受け入れられている臨床的方法である。タンパク質バイオマーカーのサブ試験における100人の患者と、収集された試験エンドポイントとを下記に示す(表6)。
ベースライン、第4週、及び第14週においてバイオマーカーの関連性に関して評価した。これらの分析から、数個の知見が現れた。調べた92個のマーカーのうち、臨床応答と有意に関連したものは殆ど存在しなかった。有意な効果を示したマーカーと、これらのマーカーに関するマーカーとエンドポイントとの関係は、数個の一次及び二次エンドポイントの全体で概ね一貫していた。臨床成課上に用量効果が存在しなかったため、使用したデータはゴリムマブ処置群(全患者はゴリムマブを受容)と組み合わせた。バイオマーカーをベースライン、第4週及び第14週において、関連性に関して評価した。
ゴリムマブ処置群において観察されたバイオマーカー/臨床エンドポイント関連性とは対称的に、プラセボ群ではバイオマーカー値と臨床エンドポイント応答との関連性が、あったとしても、殆ど存在しなかった(図示せず)。この結果は、ゴリムマブバイオマーカー分析で見られたより有意なバイオマーカーに対する内部標準又はベンチマークとしての役割を果たす。
処置に対する患者の長期臨床応答の予測に使用できるバイオマーカーの決定に使用する分類及び回帰木(CART)予測的モデルを開発した。全予測モデルは、一つ抜き交差確認(leave-one-out cross-validation)を使用した。CARTモデルは、決定木(図1〜6)の形態で示されている。木のノードは、クラス予測(肯は、予測された臨床エンドポイント応答者、否は、予測された臨床的非応答者)及び2つの数字(x/y、xは、そのノードに属する、試験での非応答者の実際の数、yは、そのノードに属する、試験での答者の実際の数)で標識されている。モデルの全体的な正確性は、「否」エンドノード全体のxの数、及び「肯」エンドノード全体のyの数である。モデルは、一次臨床エンドポイント、第14週におけるACR20、及び選択された二次臨床エンドポイントに関して開発された。一般に、二次エンドポイントモデルは、それらの感度及び特異性において、一次エンドポイントモデルと非常に類似していた。
多重データを使用する追加の予測モデルを開発して、処置の第4週におけるバイオマーカーの変化が、第14週における臨床成果の予測を含み得るか否かを決定した。ASAS20を予測するアルゴリズムを、図5に示す。ASAS20を予測するベースラインのみのアルゴリズムと同様、ベースラインレプチンが初期分類子である。3.8(対数目盛り)以上のレプチンを有する患者は非応答者として予測され、3.8未満のレプチンを有する患者は、2つの追加の予測物、i)補体3における変化、及びii)ベースラインVEGF、に基づいて更に分類される。このモデルでは、正確性は第14週での臨床応答(ASAS20)に関して64/76(84%)であった。モデルの感度は92%、特異性は81%であった。
Claims (28)
- 強直性脊椎炎の診断を有する患者の、抗−TNFα療法に対する応答を予測するための方法であって、
a)レプチン、CD40リガンド、TIMP−1、前立腺酸性ホスファターゼ(PAP)、G−CSF、MCP−1、補体成分3、VEGF、オステオカルシン、フェリチン及びICAM−1からなる群から選択される少なくとも1つの血清マーカーの濃度を測定し、
b)前記測定された濃度を、抗−TNFα療法を受容し、1つ以上の臨床エンドポイントに基づいて応答者又は非応答者として分類された、ASと診断された患者からのマーカーの血清濃度の値のセットを分析することにより決定されたカットオフ値と比較する、ことを含む、方法。 - 前記患者からの血液又は血清サンプル中のハプトグロビン、血清アミロイド、CRP、α−1アンチトリプシン、フォン・ヴィレブランド因子及びインスリンからなる群から選択される前記血清中の追加のマーカー濃度が測定される、請求項1に記載の方法。
- 強直性脊椎炎の診断を有する患者の、抗−TNFα療法に対する応答を予測するための方法であって、
a)前記患者からの血液又は血清サンプル中のレプチン及びCD40リガンドの濃度を測定し、
b)前記ASサンプル中のレプチンの前記濃度をレプチンカットオフ値と比較し、前記濃度が前記カットオフ値以上の場合、前記患者は抗−TNFα療法に対する非応答者として予測され、レプチンの前記血清値が前記カットオフ値未満の場合、
c)前記患者のサンプル中のCD40リガンドの前記濃度をCD40リガンドカットオフ値と比較し、前記CD40リガンドカットオフ値以上のCD40の濃度は、前記患者がTNFα治療に応答することを示し、前記CD40リガンド値未満の値及び前記レプチンカットオフ値未満のレプチンは、TNFα中和治療に対する非応答者を予測する、ことを含む、方法。 - 前記サンプルが血清である、請求項3に記載の方法。
- 前記血清中のレプチンの濃度がlog変換され、前記レプチンカットオフ値が3.804である、請求項4に記載の方法。

a)前記患者からの血液又は血清サンプル中のオステオカルシン、前立腺酸性ホスファターゼ及びインスリンの濃度を測定し、
b)前記ASサンプル中のオステオカルシンの前記濃度をオステオカルシンカットオフ値と比較し、前記濃度が前記カットオフ値以上の場合、前記患者は抗−TNFα療法に対する非応答者として予測され、オステオカルシンの前記血清値が前記カットオフ値未満の場合、
c)前記患者のサンプル中の前立腺酸性ホスファターゼの濃度を前立腺酸性ホスファターゼカットオフ値と比較し、前記前立腺酸性ホスファターゼカットオフ値以上の前立腺酸性ホスファターゼの濃度は、前記患者がTNFα治療に対する応答者であると予測し、前記前立腺酸性ホスファターゼカットオフ値未満の値と、場合により、
d)ASAS20により評価された臨床成績に基づいて、前記患者を非応答者と予測されたと分類し、又は更に、BASDAIにより評価して、前記患者血清中のインスリンの濃度をインスリンカットオフ値と比較することにより前記患者を分類し、前記インスリンカットオフ値未満のインスリン値は、前記患者を応答者と予測されたと分類し、カットオフ値以上のインスリン値は、前記患者をTNFα中和治療に対する非応答者と予測されたと分類する、ことを含む、方法。 - 前記サンプルが血清である、請求項10に記載の方法。

c)前記患者のサンプル中の前立腺酸性ホスファターゼの前記濃度を前立腺酸性ホスファターゼカットオフ値と比較し、前記前立腺酸性ホスファターゼカットオフ値以上の前立腺酸性ホスファターゼの濃度は、前記患者がTNFα治療に対する応答者であると予測し、前記前立腺酸性ホスファターゼカットオフ値未満の値と、
d)により評価された前記臨床成績に基づいて、前記患者を非応答者であると分類する、ことを含む、方法。 - 強直性脊椎炎の診断を有する患者の、抗−TNFα療法に対する応答を予測するための方法であって、
a)前記患者からの血液又は血清サンプル中のTIMP−1及び前立腺酸性ホスファターゼ、GCSF及びMCP−1の濃度を測定し、
b)前記ASサンプル中のTIMP−1の前記濃度をTIMP−1カットオフ値と比較し、前記濃度が前記TIMP−1カットオフ値以上の場合、前記患者は更に分類され、TIMP−1の前記血清値が前記カットオフ値未満の場合、
c)前記患者のサンプル中の前立腺酸性ホスファターゼの濃度を前立腺酸性ホスファターゼカットオフ値と比較し、前記前立腺酸性ホスファターゼカットオフ値未満の前立腺酸性ホスファターゼの濃度は、前記患者がTNFα治療に対する応答者であると予測し、前記前立腺酸性ホスファターゼカットオフ値以上の値は、前記患者が更に分類されることを要し、

- 強直性脊椎炎の診断を有する患者の、抗−TNFα療法に対する応答を予測するための方法であって、
a)ベースラインサンプル及び第4週サンプルから補体成分3(C3)濃度における変化、ベースラインにおけるフェリチン、及び前記患者からの血液又は血清サンプル中のCAM−1のベースラインからの第4週のマーカー濃度の変化を測定し、
b)抗−TNF療法の開始前に採取した前記AS患者の血液サンプル中のC3の前記濃度の、抗−TNF療法の開始後の第4週に採取した前記AS患者の血清サンプル中のC3の前記濃度に対する変化をC3カットオフ値と比較し、濃度における前記変化が前記C3カットオフ値未満と決定された場合、前記患者は抗−TNF療法に応答すると予測されたと分類され、前記C3カットオフ値以上のC3の血清濃度における変化を有する患者を、フェリチンカットオフ値と比較される前記患者のサンプル中のフェリチンのベースライン値を使用して分類し、前記カットオフ値以上の値により、前記患者は抗−TNFα療法に対する応答者であると予測され、フェリチンレベルの前記血清値が前記カットオフ値未満の場合、
c)抗−TNF療法の開始前に採取された前記AS患者の血液サンプル中のICAM−1の前記濃度の、抗−TNF療法の開始後の第4週に採取した前記AS患者の血清サンプル中のICAM−1の前記濃度に対する変化をICAM−1カットオフ値と比較し、ICAM−1濃度における前記変化が前記ICAM−1カットオフ値以上と決定された場合、前記患者は抗−TNF療法に応答すると予測されたと分類され、ICAM−1濃度における前記変化が前記ICAM−1カットオフ値未満であると決定された場合、前記患者は予測された非応答者と分類される、ことを含む、方法。 - 前記C3変化のカットオフ値が−0.233である、請求項21に記載の方法。

コンピュータ読み取り可能なフォーマットにおける患者データセットを受信及び加工するためのコンピュータステーションであって、前記コンピュータステーションが前記患者データセットを加工し、出力分類を生成するための学習ずみニューラルネットワークを含み、前記学習ずみニューラルネットワークが患者データセットを前処理する方法により学習され、前記方法が、
a)ASに関連した患者バイオマーカーを選択する工程と、
b)統計的に及び/又はコンピュータ的に、前記選択された患者バイオマーカーの区別力(power)を、線形及び/又は非線形の組み合わせにおいて個別に試験して、臨床エンドポイントに基づいて患者の応答又は非応答を示す工程と、
c)二次入力の誘導のための統計的方法を、元の又は変換されたバイオマーカーの線形及び/又は非線形の組み合わせである前記ニューラルネットワークに適用する工程と、
d)区別力を示す患者バイオマーカーのみ又は誘導された二次入力を選択する工程と、
e)前記前処理された患者バイオマーカー又は誘導された二次入力を使用して、前記コンピュータベースのニューラルネットワークを訓練する工程と、を含む、コンピュータベースのシステム。 - 前記出力分類が、前記患者が抗−TNFα療法に応答するか又は応答しないかであり、前記臨床エンドポイントがASA20又はBASDAIであり、前記バイオマーカーが患者の性別、レプチン、CD40リガンド、TIMP−1、MCP−1、G−CSF、PAP、オステオカルシン、インスリン、VEGF、フェリチン、補体成分3、ICAM−1又は前記バイオマーカーの組み合わせである、請求項23に記載のコンピュータベースのシステム。
- 抗−TNFα治療で処置されるべき強直性脊椎炎と診断された患者が、1つ以上の臨床エンドポイントにより評価されるように、療法に応答するか又は応答しないかを予測するための装置であって、
a)レプチン、CD40リガンド、TIMP−1、MCP−1、G−CSF、PAP、オステオカルシン、インスリン、VEGF、フェリチン、補体成分3又はICAM−1からなる群から選択される抗−TNFα療法に対するAS患者の応答又は非応答に関連したマーカーに特異的な抗体と、検出可能なラベルで標識された二次抗体と、を含む試験ストリップと、
b)前記ラベルにより生成された信号を、前記信号を加工可能なリーダーを使用して検出し、
c)前記信号の前記加工から得られたデータを、前記サンプル中の前記マーカーの所定の濃度を示す結果に加工する、ことを含む、装置。 - 前記リーダーがヒトである、請求項25に記載の装置。
