Description
本発明は、少なくとも一部は、国立衛生研究所からの財政的援助(NIH科研費P50-AG05681、M01 RR00036、NIH RR000954、及びNIH DK056341)により完成した。従って、本発明について、合衆国政府は一定の権利を持ちうる。
アルツハイマー病(AD)は、最も一般的な痴呆の原因であり、増加している公衆衛生問題である。現時点において、合衆国内で500万人がこの病気により苦しんでいると推定され、2050年までに1300万人への増大が推定されている(Herbert et al 2001, Alzheimer Dis. Assoc. Disord. 15(4): 169-173)。ADは、他の中枢神経系(CNS)の変性疾患と同様に、タンパク質の生産、蓄積、及びクリアランスの障害に特徴付けられる。ADでは、アミロイド‐β(Aβ)というタンパク質代謝の調節異常が、疾患を伴う人々の脳内における該タンパク質の塊状の蓄積による兆候として示される。ADでは、記憶、認知機能、及び最終的には自立性が喪失される。それは、患者及びその家族に過酷な人的及び経済的打撃を与えるものである。この病気の過酷さ及び人口内での有病率の増加により、より良い治療法の開発は急を要する。
本発明の一態様は、脳障害及び臨床症状の発生に先立ち、ADのような神経系疾患及び神経変性疾患の出現及び進行を診断し、モニターする手段の提供にある。また、本発明の別の態様は、ADのような神経系疾患及び神経変性疾患の治療の効果をモニターする手段を提供する。
本発明は、対象の神経性及び神経変性の疾患、障害及び経過の早期診断及び評価の方法に関する。特に、本発明は、CNS由来生体分子の合成及びクリアランス速度を測定することにより、神経系の損傷と関連した臨床症状の発症前の診断方法を提供する。初期診断が初期治療の機会を提供し、場合によっては、これに苦しむ者の神経の深刻な損傷を防ぐといった本発明の有用性は、当業者にとって明らかである。また、本発明は、疾患を緩和する治療法の開発段階をモニターする方法又はヒトに直接的に重大な作用を及ぼすと考えられる治療法をスクリーニングする方法を提供する。例えば、ある治療法がCNS由来の生体分子の合成及びクリアランス速度を変えるものかどうかを判断することができる。最終的に、この方法は、神経性及び神経変性疾患の発病を予想する検査及びこのような疾患の進行をモニターする手法を提供する。
本発明は、神経系由来生体分子のインビボにおける代謝を測定する方法を提供する。この方法を用いることにより、当業者は、特定の病状と関連する神経系由来生体分子の代謝(合成及びクリアランス)の変化を調査することが可能となる。加えて、本発明は、被験者内での疾患を緩和する治療法の薬力学的作用の測定を可能とする。
アルツハイマー病(AD)は、アミロイド-β(Aβ)タンパク質の産生の増加、クリアランスの減少又はこれらの両方の結果として生じる中枢神経系(CNS)におけるアミロイド班により特徴づけられる衰弱性の疾患である。本願発明者らは、脳脊髄液(CSF)又は血漿中のインビボのAβ合成及びクリアランス速度を測定することによりヒトのインビボでのAβ代謝を測定する方法を開発した。該インビボAβ合成及びクリアランス速度は、被験者が、コントロール群との比較においてAβの合成及びクリアランスに異変を生じていないかどうかを評価することに用いることができる。このような比較はADの経過の初期、即ち、臨床症状の発症及び重大な神経の損傷前での診断を可能とする。更に、本発明は、アポリポ蛋白E(ApoE)がAβ代謝に変化を生じさせるか否かを決定する手段を提供する。この決定により、なぜ特定のApoE遺伝子型がADの危険因子であるのかということに対する新たな洞察を提供できるであろう。
本発明は、インビボでの神経系由来生体分子の代謝の測定方法を提供する。該生体分子は、タンパク質、脂質、核酸、又は炭水化物であってもよい。該可能性のある生体分子は、インビボでの合成中に標識でき、その代謝が測定されるサンプルを回収できるという能力によってのみ限定される。好ましい実施形態では、該生体分子はCNSで合成されるタンパク質である。例えば、測定される該タンパク質は、以下に限定されるものではないが、アミロイド-β(Aβ)及びその変異体、可溶性アミロイド前駆体タンパク質(APP)、アポリポ蛋白E(アイソフォーム2、3、又は4)、アポリポ蛋白J、タウ(ADに関連する別のタンパク質)、グリア線維酸性蛋白、α-2マクログロブリン、シヌクレイン、S100B、ミエリン塩基性タンパク質(多発性硬化症と関係する)、プリオン、インターロイキン、及び腫瘍壊死因子(TNF)でもよい。標的と成り得る更なる生体分子には、GABA作動性ニューロン、ノルアドレナリン作動性ニューロン、ヒスタミン作動性ニューロン、セロトニン作動性ニューロン、ドーパミン作動性ニューロン、コリン作動性ニューロン、及びグルタミン作動性ニューロンの産物、又はそれらニューロンと相互作用するタンパク質又はペプチドが含まれる。
目的の生体分子の標識には、種々の異なる部分を用いることができる。一般的に、本発明の方法で代表的に用いられる二種類の標識部分は、放射性同位体及び非放射性(安定)同位体である。好ましい実施形態では、非放射性同位体を用いて質量分析計により測定することができる。好ましい安定同位体には、重水素2H、13C、15N、17又ハ18O、33、34、又ハ36S、が含まれるが、主な天然型よりも多い又は少ない中性子により原子の質量を変化させるその他多くの安定同位体でも効果があることが理解される。一般的に、適切な標識は、研究対象である生体分子の質量を、質量分析計により検出可能なように変化させる。1つの実施形態において、該測定される生体分子はタンパク質であり、該標識部分は非放射性同位体(例えば、13C)を含むアミノ酸である。別の実施形態では、該測定される生体分子は核酸であり、該標識部分は非放射性同位体(例えば、15N)を含むヌクレオシド三リン酸である。また、代わりに、放射性同位体を用いることもでき、該標識された生体分子は、質量分析計ではなくシンチレーション計数器により測定できる。1つ以上の標識部分を同時に又は順に使用することができる。
標識部分は、いくつかの方法により対象に投与できる。適切な投与方法には、静脈内、動脈内、皮下、腹腔内、筋肉内、又は経口投与が含まれる。好ましい実施形態においては、該標識部位は標識アミノ酸であり、該標識アミノ酸は静脈内点滴される。別の実施形態においては、標識アミノ酸は経口摂取することができる。
本発明の方法は、生物学的サンプルが、標識された生体分子のインビボでの代謝を測定可能なように被験者から採取されることを可能とする。適切な生物学的サンプルには、脳脊髄液(CSF)、血漿、血清、尿、唾液、汗、及び涙が含まれるが、これらに限定されるものではない。本発明の1つの実施形態において、生物学的サンプルはCSFから採取される。また別の実施形態では、生物学的サンプルは尿から採取される。好ましい実施形態では、生物学的サンプルは血液から採取される。
本発明は、生物学的サンプル中の標識生体分子の量及び非標識生体分子の量の検出を用いて非標識生体分子に対する標識生体分子の比率を測定することを可能とする。一般的に、非標識生体分子に対する標識生体分子の比率は、該生体分子の代謝に正比例している。標識及び非標識生体分子の検出に適した方法は、研究対象となる生体分子及びこれを標識するために用いる標識部分の種類によって変化可能であり、また、変化するであろう。該目的の生体分子がタンパク質であり、該標識部分が非放射性標識アミノ酸である場合、検出方法は、該非標識タンパク質に対する該標識タンパク質の質量変化を検出できるほど高感度であるべきである。好ましい実施形態では、標識及び非標識タンパク質の質量の差の検出に質量分析計が用いられる。1つの実施形態では、ガスクロマトグラフィー質量分析計が用いられる。また、代わりの実施形態では、MALDI-TOF質量分析計が用いられる。好ましい実施形態では、高分解能タンデム質量分析計が用いられる。
生物学的サンプル中の標識及び非標識生体分子の量が検出されたならば、標識生体分子の比率又は百分率を決定できる。目的の生体分子がタンパク質で、生物学的サンプル中の標識及び非標識タンパク質量が測定された場合には、非標識タンパク質に対する標識タンパク質の比率を計算できる。タンパク質代謝(合成速度、クリアランス速度、遅延時間、半減期等)は、非標識タンパク質に対する標識タンパク質の経時的な比率から計算することができる。これら指標の計算には多くの適切な方法が存在する。本願発明では、標識及び非標識タンパク質(又はペプチド)を同時に測定することを可能であり、他の計算と共に非標識タンパク質に対する標識タンパク質の比率を得ることができる。当業者であれば、本発明の方法と共に使用できる標識の1次動態モデルに精通するであろう。例えば、分画合成速度(FSR)を計算することができる。該FSRは、初期における非標識タンパク質に対する標識タンパク質の増加率を前駆体濃縮度(Precursor Enrichment)で割ったものに等しい。同様にして、分画クリアランス速度(FCR)も計算できる。更に、遅延時間、同位体トレーサー定常状態といった他の指標についても決定でき、該タンパク質の代謝及び生理機能の測定値として用いることができる。また、データを複数のコンパートメントモデルに適合させて区画間の輸送を評価するモデリングを行なうこともできる。無論、選択される数学的モデリングの種類は、個々のタンパク質合成及びクリアランス指標に依存する(例えば、1‐プール、複数プール、定常状態、非定常状態、コンパートメントモデリング、等)。
本発明は、被験者の中枢神経系由来タンパク質のインビボでの代謝を測定することによる神経性及び神経変性疾患の診断、又はその経過観察、又は治療用のキットを提供する。一般的に、キットには標識アミノ酸、該標識アミノ酸を投与する手段、生物学的サンプルを経時的に採取する手段、及び代謝指数が計算できるように非標識タンパク質に対する標識タンパク質の比率を検出し測定するための使用説明書が含まれる。該代謝指数は、正常、健康な個体の代謝指数と比較することができ、又は同一の被験者から先の時点で得られた代謝指数と比較することができる。これらの比較により、従事者は、神経性又は神経変性疾患の発病の予測、神経性又は神経変性疾患の発症の診断、神経性又は神経変性疾患の経過観察、又は神経性又は神経変性疾患の治療法の効果の確認をすることが可能となる。好ましい実施形態では、該キットは、13C6‐ロイシン又は13C6‐フェニルアラニンを含み、標識されるタンパク質はAβであり、評価される疾患はADである。

実施例
原理
生化学的証拠、遺伝的証拠及び動物モデルによる証拠は、Aβ(図1)がADの病原ペプチドであることを示している。インビボでの標識でAβを測定する方法を開発するため、以下の4段階の基本工程を用いたインビトロ系が設計された:1)培養内でインビトロにAβを標識し、2)他の標識されたタンパク質からAβを単離し、3)Aβを該標識の解析が可能な断片へと特異的に切断し、そして4)標識及び非標識断片を定量する。
第一に、生体液から非標識型Aβを単離して測定する方法が開発された。Aβは、分子の中央ドメイン(13‐28番目の残基)を認識する特異性の高いモノクローナル抗体(m266)を用いて、脳脊髄液又は細胞培養用培地のサンプルから免疫沈降された。抗体ビーズは、製造業者のプロトコル通りに10mg/mlのm266抗体の濃度でm266抗体(Eli Lilly より寛大にも提供された)をCNBrセファローズビーズに共有結合させることで調整された。該抗体ビーズは、50%PBS及び0.02%アジドのスラリー中で4℃にて保存された。免疫沈降混合物は、250μlの5xRIPA、12.5μlの100xプロテアーゼ阻害剤、及び30μlの抗体ビーズスラリーでエッペンドルフチューブに入っていた。これに対し、1mlの生体サンプルが加えられ、チューブは4℃にてオーバーナイトでローテーションされた。前記ビーズは1xRIPAで1回、25mMの重炭酸アンモニウムで2回洗浄された。前記最終洗浄の後、ビーズは吸引乾燥され、Aβは、30μlの純蟻酸を用いて抗体‐ビーズ複合体から溶出された。質量分析により、Aβは直接的に特徴(分子量及びアミノ酸配列)が明らかにされた。図2に示すように、結果は以前発表された知見(Wang et al. 1996, J Biol. Chem. 271(50):31894-31902)に類似するものであった。
第二に、新たに合成されたAβを標識する方法が開発された。13C6‐ロイシンは、能動輸送により迅速に血液脳関門を横断して平衡化し (Smith et al. 1987, J Neurochem 49(5): 1651-1658)、必須アミノ酸であり、Aβの特性を変化させず、更に安全で非放射性であるから、代謝標識として用いられた。13C安定同位体は、アミノ酸又はタンパク質の化学的又は生物学的特質を変化させない:各13C標識ごとに質量が1ダルトン分増加するだけである。実際のところ、完全な生命体を純粋に13Cで育成しても何ら有害な影響はなかった。標識されたロイシンは、Aβのアミノ酸配列の第17番目及び第34番目に取り込まれる(図1参照)。
第三に、正確に標識型及び非標識型のAβを定量する方法が開発された。このために、自動注入装置を備えたウォーターズ製(ミルフォード、マサチューセッツ州)のキャピラリー液体クロマトグラフィーシステムが、エレクトロスプレーイオン化源を備えたサーモフィニガン製(サンノゼ、カリフォルニア州)LCQ-DECAに接続された(LC-ESI-タンデムMS)。各サンプルの5μl分量がヴィダック(Vydac)C-18キャピラリーカラム(0.3 x 150 mm MS 5μmカラム)に注入された。Aβ17‐28断片は、ロイシン残基を1残基含んでおり、13C6‐ロイシンの取り込みにより該断片の分子量は6ダルトン分シフトする。陽イオン走査モードにおいては、トリプシン消化された合成Aβ及び免疫沈降されたAβのLC-ESI-MS解析は、Aβ17‐28では1325.2に、13C6‐ロイシン標識されたAβ17‐28では1331.2の質量に予想される親イオンを得た(図3A及び3B)。標識Aβ(Aβ*)の割合は、標識Aβ17‐28からの全標識MS/MSイオンを、非標識Aβ17‐28からの全非標識MS/MSイオンで割った割合として計算された。マクロを備えたカスタム化されたマイクロソフト・エクセル表計算ソフトを用いて、以下の式に従い、Aβ17‐28のトレイサー/トレイシー比(tracer to tracee ratio)(TTR)を計算した:
Aβを産生するヒト神経膠腫細胞(Murphy et al. 2000, J Biol.Chem. 275(34): 26277-26284)を13C6‐標識ロイシン(ケンブリッジアイソトープラボラトリーズ、ケンブリッジ、マサチューセッツ州)又は非標識ロイシンの存在下で増殖させた。Aβは、m266抗体を用いた免疫沈降法により培養液から単離された(上記参照)。溶出されたAβは、トリプシンにより37℃にて4時間消化され、その断片はLC-ESI MSにより分析された。予想される通り、非標識ロイシンの存在下で培養された細胞から単離されたAβのAβ17‐28断片は、1325.2の分子量を有し、13C6‐標識ロイシンと培養された細胞から単離されたAβのAβ17‐28断片は、1331.2の分子量を有していた(図3B)。上記知見は、該細胞が13C6‐ロイシンをAβに取り込んでおり、13C6‐ロイシンの存在下で合成されたAβは、該標識されたアミノ酸を取り込んでいることが確認され、該ロイシンを含有するペプチドにおける6ダルトン分の分子量のシフトは、質量分析により識別可能であることを意味している。
原理
タンパク質の産生及びクリアランスは、厳密に制御され、疾患状態及び正常な生理機能を反映する重要なパラメターである。ヒトのタンパク質代謝についての先の研究は、中枢神経系(CNS)で生産されるタンパク質ではなく、全身又は周辺体のタンパク質に焦点が合せられていた。従前では、ヒトのCNSにおけるタンパク質の産生及びクリアランスの速度を定量可能な方法が存在しなかった。そのような方法は、ヒトのAβ合成及びクリアランス速度だけでなく、CNSの疾患に関連するその他の種々のタンパク質の代謝を評価するのにも有益であろう。ADの根源的な病理発生及びAβ代謝に関する決定的な問題に取り組むため、ヒトのCNSにおけるインビボでのAβ分画合成速度(FSR)及び分画クリアランス速度(FCR)の定量方法が開発された。
全ての人体調査は、ワシントン大学人体研究委員会及び総合臨床研究センター(GCRC)諮問委員会に承認されている。また、全ての参加者からインフォームドコンセントを得ている。全ての参加者が、一般的に良好な健康状態にあり神経系疾患を有していないようにふるいにかけられた。7名の男性及び3名の女性(23‐45歳)が参加した。各研究参加者は、前夜午後8時からの一晩の絶食の後、午前7:00にGCRCに入院させられた。GCRC研究調理室は午前9時、午後1時及び午後6時に食事(60%の炭水化物、20%の脂肪、20%のタンパク質、標識ロイシンの点滴期間中は低ロイシン食物)を提供し、参加者は水分については自由に摂取した。入院期間中は、全ての食料及び水分消費が看護士及びGCRC調理室により記録された。1つの静脈内カテーテルは、肘正中静脈に設置され、安定同位体で標識されたロイシンの溶液の投与に用いられた。第二の静脈内カテーテルは、血液サンプルを得るために対側性肘正中静脈(contra-lateral antecubital vein)に設置された。複数回の腰椎穿刺を行なうことなくCSFを採取できるよう、くも膜下カテーテルが、トーイー針(Touhy needle)を介してL3-L4間空に挿入された(Williams, 2002, Neurology 58: 1859-1860)。静脈内カテーテルは熟練した公認看護士により設置され、腰椎カテーテルは、豊富な腰椎穿刺経験を有する熟練した内科医により設置された。血液サンプルは、調査が36時間の場合には、最初の16時間では1時間毎に採取され、その後は2時間毎に採取され、それ以外の場合には、1時間毎に採取された。CSFサンプルについては、全調査期間に渡り、1時間毎に採取された。図5に表されるインビボでの実験プロトコルの図を参照していただきたい。参加者は、手洗所の使用以外は寝台にいるように促された。
ヒトにおいて標識Aβをインビボで産生及び検出可能かどうかを判断するため、参加者のうち1名について、標識ロイシンの24時間点滴を行なった後に腰椎穿刺によりCSFを採取した。13C6‐標識ロイシンは、一台のIVで1.8 mg/kg/hrの速度で点滴された。1時間毎に、他方のIVを通して10mlの血漿が採取された。連続点滴の24時間後、腰椎穿刺が1回行なわれ、30mlのCSFが採取された。Aβは、前記CSFサンプルから免疫沈降され、トリプシンで消化され、更に、各サンプルの5μl分量がヴィダック(Vydac)C-18キャピラリーカラム(0.3 x 150 mm MS 5μm)に注入された。陽イオン走査モードにおいては、トリプシン消化された合成Aβ及び免疫沈降されたAβのLC-ESI-MS解析で、Aβ17‐28では1325.2に、13C6‐ロイシン標識されたAβ17‐28では1331.2の質量に予想される親イオンを得た。アミノ酸配列と存在割合のデータを得るため、これらの親イオンは、衝突誘起解離され(CID 28%)、その2価荷電種([M + 2H]+2; m/z 663.6及び666.6)のタンデムMS分析は、選択反応監視モード(SRM)で走査され、従って、生じたy-及びb-シリーズイオンが同位体比定量に用いられた(図6)。加えて、血漿中及びCSF中の13C6‐ロイシンが測定され、13C6‐ロイシンAβの予想される最大量が決定された。結果から、非標識Aβ17‐28及び13C6‐標識Aβ17‐28は、ヒトのCSF中にて検出及び測定可能であることが証明された。
Aβの合成及びクリアランス速度の計算に定常状態の方程式を使用できるように検出可能なAβの13C6‐ロイシン標識が達せられて適切な期間維持されることを確認するため、標識及びサンプリングの最適な時間が測定された。13C6‐ロイシンの静脈内点滴投与量の範囲(1.8〜2.5 mg/kg/hr)、期間(6、9、又は12時間)及びCSF/血液サンプリング時間(12〜36時間の期間)について試験された(表1参照)。
最後の3名の参加者においては、13C6‐標識ロイシンは、最初に2mg/kgで10分間かけてボーラスして標識ロイシンの定常状態に達し、その後、2 mg/kg/hrの速度で9時間の連続静脈内点滴を行なった。該最後の3名の参加者については、血液及びCSFは36時間サンプリングされた。1又は2時間の間隔をあけて、12ml血液及び6mlCSFの一連のサンプルが採取された。CSFは、通常の大きさの成人で1時間に〜20mlの産生速度を有し(Fishman RA, 1992, Cerebrospinal fluid in diseases of the nervous system, Saunders, Philadelphia)、手続の間ずっと自らを補充し続けている。36時間に亘る調査において、採取された総血液量は312mlであり、採取された総CSF量は216mlであった。
血漿及びCSFのサンプルは、解析されて各液体に存在する標識ロイシンの量が測定された(図9)。血漿及びCSF13C6‐ロイシンについての非標識ロイシンに対する標識ロイシンの比率は、LC-ESI-MSよりも低質量のアミノ酸解析に適した、キャピラリーガスクロマトグラフィー‐質量分析計(CG-MS)を用いて定量された(Yarasheski et al. 2005, Am J Physiol. Endocrinol. Metab. 288: E278-284; Yarasheski et al. 1998 Am J Physiol. 275:E577-583)。血漿及びCSFの両方において、13C6‐ロイシンは1時間以内にそれぞれ14%及び10%の定常状態の濃度に達した。これにより、ロイシンは、既知の中性アミノ酸輸送系により、迅速に血液脳関門を横断して輸送されることが確認された(Smith et al. 1987 J Neurochem. 49(5): 1651-1658)。
1時間毎に採取されたCSFサンプルについて、上記の通り、免疫沈降‐MS/MSにより非標識Aβに対する標識Aβの比率が測定された。13C‐標識Aβ17‐28からのMS/MSイオンを非標識Aβ17‐28からのMS/MSイオンで割って、非標識Aβに対する標識Aβの比率を示した(上記TTRの式を参照)。各時点における標識Aβの比率の平均値及び標準誤差(n = 6)が図10に示されている。最初の4時間、測定可能な標識Aβはなかったが、これに続いて5時間目から13時間目にかけて増大した。13時間目から24時間目にかけて有意な変化はなかった。標識Aβは24時間目から36時間目にかけて減少した。
分画合成速度(FSR)は、下記に表す標準式を用いて計算された:
原理
血漿Aβの代謝については、恐らくCSFと比べて別の区画で異なる代謝速度で起こっている。ADのマウスモデルでは、血漿中で抗体により捕らえられるAβの量は、ADによる病状を定義づける特徴でありうる。従って、血漿中のAβの代謝速度は、ADの病理を定義づける特徴でありうる。更に、血漿Aβの代謝は、CSFと比較しても同等に効果的なヒトのAβ代謝の測定方法でありうる。仮に、痴呆を診断又は予測するものであることが証明されれば、ADの前臨床又は臨床的な診断試験法としてより高い可能性を有するであろう。
先の実施例でCSFについて行なわれたように、血漿中の標識及び非標識Aβを測定する方法が開発され得る。CSFとの比較において、血漿からAβを採取するには以下の2つの主な相違点がある:1)血漿には100分の1未満のAβしか存在せず、2)非Aβタンパク質の濃度が約200倍高い。免疫沈降の効率性及び特異性については、当業者に公知の方法を用いて最適化される必要があろう。免疫沈降については、リニアトラップクアドラポール(LTQ)質量分析計を用いた解析により夾雑タンパク質の数及び比較量を同定することで検査可能である。LC-ESIに対し、LTQは200倍までの感度の向上を示す。予備的結果から、1mlのヒトCSFからのAβ断片の50倍希釈において、良好なS/N比を有することが示された。
CSFのデータと同様に(実施例1及び2参照)、ヒト血漿からの標識及び非標識Aβを再現性のある定量的測定を提供可能な技術が開発できることが期待されている。前記検量線については、直線に近く、ばらつきが少ないことが期待される。ヒトのインビボ調査から得た血漿の標識Aβの検量線は、CNS/CSFの標識Aβの検量線を正確に反映することが期待される。参加者からの血漿AβのFSR及びFCRを得ることも可能である。血漿へのAβの点滴後における動物モデルで示されているように、血漿中のAβのクリアランス速度は、CSFの場合よりも十分に速くなるであろうことが予想される。
標識及び非標識血漿Aβが上に詳細に述べるように正確に測定できない場合、時点数は少なくかつ各時点におけるサンプルは多くして、用いることが出来る(1時間毎に10mlに対して2時間毎に20ml)。これにより測定の時間的分解能は低減するが、しかし、それでもFSR及びFCRを得るには十分であろう。もし、血漿に関してタンパク質のコンタミネーションが依然問題となるのであれば、HPLC、タンパク質二次元電気泳動、又は当業者にとって馴染みのあるよりストリンジェントな洗浄工程による精製が必要であろう。
原理
ApoE遺伝子型は、ADについて十分な実証を得た遺伝子の危険因子である。ADにおいては、ApoEが、細胞外アミロイド沈着と共局在していることが免疫組織学により明らかとなった。更に、ヒトの個体群においては、ApoE ε4遺伝子型がADの危険因子であることが分かった。ApoE ε2対立遺伝子は、ADのリスクの面で保護的であることが示されている。また、ApoE遺伝子型は、いくつかのADマウスモデルにおいて、ADの病態変化に劇的に影響することが示されている(Games et al. 1995 Nature 373(6514): 523-527)。
各参加者についてApoE遺伝子型を決定可能である。遠心分離された血漿からのバフィコート(白血球層)は、当業者にとって公知の標準的な手法を用いて回収し、迅速に−80℃で凍結可能である。サンプルのApoE遺伝子型は、PCR解析により決定される(Talbot et al. 1994, Lancet 343(8910): 1432-1433)。ApoE2の遺伝子量(0、1、又は2コピー)及びApoE4の遺伝子量(0、1、又は2コピー)による影響は、CSF又は血漿におけるAβ代謝のFSR又はFCRの連続変数を用いて解析することが可能である。
ApoE3との比較において、ApoE4はAβのクリアランスを減少できることが予想される。反対に、ApoE2は、ApoE3と比較して、Aβのクリアランスを増大させることが予想される。ApoE遺伝子型に基くAβの合成速度の変化は予想されない。Aβ代謝の変化が検出された場合には、ヒトのインビボでのAβ代謝に対するApoE状態による影響の証拠となるであろう。
原理
ADのマウスモデルで示されたように、CNSから血漿へのAβの輸送は、ApoE遺伝子型により影響を受けうる。ヒトにおけるこの影響の測定は、ApoEによる輸送の変化を明らかにするであろう。
各参加者についてApoE遺伝子型を決定可能である。遠心分離された血漿からのバフィコート(白血球層)は、当業者にとって公知の標準的な手法を用いて回収し、迅速に−80℃にて凍結可能である。サンプルは、実施例4で用いられた手法を用いて解析可能である。ApoE2の遺伝子量(0、1、又は2コピー)及びApoE4の遺伝子量(0、1、又は2コピー)による影響は、血漿Aβ代謝のFSR又はFCRの連続変数に対して解析することが可能である。統計解析の方法は、上記実施例4に記載されるように当業者に公知の標準的手法により作製できる。
ApoE4は、ApoE3と比較して、血漿Aβのクリアランスを減少でき、ApoE2は、ApoE3と比較して、血漿からのAβのクリアランスを増大させることが予想される。ApoE遺伝子型に基くAβの合成速度の変化が観察されるとは予想されない。しかしながら、血漿Aβ代謝の変化が検出された場合には、ヒト中のAβ代謝に対するApoE状態による影響の最初の評価となるであろう。
Aβ代謝についての血漿、CSF及びCNS区画の相関性はよく理解されていない。各区画におけるAβの割合についてはADの有無に依存した変化がある。このことは、これらの区画の間で、Aβ代謝障害が異なる影響を及ぼすことを示している。周辺の血漿Aβ代謝をCSFのAβ代謝と比較した関係は、単なるApoE遺伝子型の依存性よりも複雑なものであろう。恐らく、ADの状態だけではなく、他の要因も上記関係に作用して影響するであろう。従って、Aβ代謝の変化の明確なパターンは、ApoE遺伝子型に依存しない可能性もある。
Claims (41)
- 中枢神経系で合成される生体分子の被検者のインビボでの代謝の測定方法であって、該測定方法が、
(a)被検者の中枢神経系で生体分子が合成されるに伴い該生体分子に取り込まれることが可能であり血液脳関門を横断可能な標識部分を前記被検者に投与すること;
(b)前記部分により標識された生体分子画分と、該部分により標識されていない生体分子画分とを含む生物学的サンプルを前記被検者から採取すること;及び
(c)標識された生体分子量及び標識されていない生体分子量を検出することであって、標識されていない生体分子に対する標識された生体分子の比率が、前記被検者の前記生体分子の代謝に正比例していること、
を含む前記測定方法。 - 前記生体分子が、タンパク質、脂質、核酸及び炭水化物からなる群から選択される請求項1に記載の方法。
- 前記標識部分が、原子、又は標識された原子を有する分子である請求項1に記載の方法。
- 前記原子が、放射性同位体である請求項3に記載の方法。

(b) 前記部分により標識されたタンパク質画分と、該部分により標識されていないタンパク質画分とを含む生物学的サンプルを前記被検者から採取すること;及び
(c) 標識されたタンパク質量及び標識されていないタンパク質量を検出することであって、標識されていないタンパク質に対する標識されたタンパク質の比率が、前記被検者の前記タンパク質の代謝に正比例していること、
を含む前記測定方法。 - 前記合成されたタンパク質が、神経細胞、グリア細胞、又は中枢神経系のその他の細胞に由来する請求項13に記載の方法。

(d) 代謝指数が計算できるように標識されていないタンパク質に対する標識されたタンパク質の比率を経時的に検出し測定するための指示であって、該代謝指数は正常で健康な個体の代謝指数と比較又は同一の被験者から先の時点で得られた代謝指数と比較され得る、前記指示、
を含む前記キット。 - 前記神経性又は神経変性疾患が、アルツハイマー病、パーキンソン病、脳卒中、前頭側頭型痴呆(FTDs)、ハンチントン病、進行性核上性麻痺(PSP)、大脳皮質基底核変性症(CBD)、老化関連疾患及び痴呆、多発性硬化症、プリオン病、レビー小体病、及び筋萎縮性側索硬化症からなる群から選択される請求項29に記載のキット。
- 前記合成されたタンパク質が、神経細胞、グリア細胞、又は中枢神経系のその他の細胞に由来する請求項29に記載のキット。
