Claims (18)
- 患者の内部脈管内にステントグラフト(20)を留置するステントグラフト(20)留置システムにおいて、
前記留置システムは、
ステントグラフト(20)を具備した導入装置(1a)と、
補助ガイドワイヤ(14)と
を有し、
前記導入装置(1a)は、使用中は患者の体内に留置される近位端(6)と患者の体外に残る遠位端4とを有し、
前記補助ガイドワイヤ(14)は、導入装置(1a)の遠位端2aから導入装置(1a)を貫通して導入装置(1a)の近位端(6)に隣接する場所にまで伸び、
前記補助ガイドワイヤ(14)は、導入装置(1a)が患者の体内に留置された後、、患者の体外から操作可能であり、ステントグラフトの一部が枝動脈内に留置されるのを補助する為に、枝動脈から捕獲できるように、導入装置(1a)の近位端(6)から伸びる
ことを特徴とするステントグラフトの留置システム。 - 前記導入装置(1a)は、シース(3)を有し、
前記シース(3)は、ステントグラフト(20)を導入装置(1a)内に収縮状態で保持する
ことを特徴とする請求項1記載の留置システム。 - 前記ステントグラフト(20)は、サイドアーム・ステントグラフト(23)を有し、
前記補助ガイドワイヤ(14)が、前記サイドアーム・ステントグラフト(23)を貫通して伸びる
ことを特徴とする請求項1記載の留置システム。 - 前記ステントグラフト(20)は、開窓(75)を有し、
前記補助ガイドワイヤ(14)は、前記開窓(75)を貫通して伸びる
ことを特徴とする請求項1記載の留置システム。 - 前記開窓は、開窓から伸びるサイドアーム・ステントグラフト(23)を有する
ことを特徴とする請求項4記載の留置システム。 - 前記開窓は、前記開窓からステントグラフト内に伸びるグラフト・チューブ(77)を有する
ことを特徴とする請求項4記載の留置システム。 - 前記グラフト・チューブは、遠位方向に伸びる
ことを特徴とする請求項6記載の留置システム。 - 前記グラフト・チューブは、近位方向に伸びる
ことを特徴とする請求項6記載の留置システム。 - 前記補助ガイドワイヤは、導入装置の遠位端に伸びる補助カテーテル(13)内に保持され、
前記補助ガイドワイヤは、前記補助カテーテルに対し移動可能である
ことを特徴とする請求項1記載の留置システム。 - 前記補助ガイドワイヤは、導入装置の近位端上のノーズコーンと導入装置上のシースとの間から外に伸びる
ことを特徴とする請求項1記載の留置システム。 - 前記留置システムは、
導入装置上のステントグラフトの近位端と遠位端を保持する為の、導入装置上のステントグラフト保持装置(35)と、
トリガワイヤ(36)と、
前記遠位端と近位端の保持装置を解放するトリガワイヤ解放装置と
を有する
ことを特徴とする請求項1記載の留置システム。 - トリガワイヤ(31)に関連づけられた外径縮小結び(29)と、サイドアーム・ステントグラフト用のトリガワイヤ解放装置(18)とを具備し、
前記外径縮小結び(29)は、サイドアーム・ステントグラフト内の自己拡張ステントの力に抗して、サイドアーム・ステントグラフトの直径を収縮した状態に保持し、
前記サイドアーム・ステントグラフトは、患者の動脈の壁に不適切に当たることなく、枝動脈内に引き込まれる
ことを特徴とする請求項5記載の留置システム。 - 前記ステントグラフト(20)は、
その中を貫通するメインルーメンを具備するメイン管状本体と、
その中を貫通するサイドアームルーメンを有するサイドアーム・ステントグラフトと
を有し、
前記留置システムは、
近位端(6)から遠位端(4)に伸びるメインガイドワイヤ(11)と、
前記ガイドワイヤカテーテルの近位端にノーズコーン(8)と、
前記遠位端からノーズコーンまでその中を伸びる補助ガイドワイヤ(14)を具備する補助ガイドワイヤカテーテル(13)と、

前記ステントグラフトは、前記ノーズコーンの遠位端のメインガイドワイヤ・カテーテルに保持され、
前記メインガイドワイヤは、前記メインルーメン内を貫通して伸び、
前記補助ガイドワイヤは、前記メインルーメン内と前記サイドアームルーメン内を貫通してノーズコーンまで伸び、
前記サイドアームは、補助カテーテル(13)に保持され、
前記シースは、メインガイドワイヤカテーテルと同軸に配置され、前記ノーズコーンまで伸びて、ステントグラフトを包囲する
ことを特徴とする請求項1記載の留置システム。 - 前記ステントグラフトと、前記サイドアーム・ステントグラフトとは、自己拡張型ステント(24,27)を有する
ことを特徴とする請求項13記載の留置システム。 - 前記ノーズコーンは、補助カテーテルがシースから出られるような軸方向ノッチ(28)を有する
ことを特徴とする請求項13記載の留置システム。 - 前記ステントグラフト(20)は、
その中を貫通するメインルーメンを具備するメイン管状本体と、
前記メイン管状本体に形成された開窓(75)と
を有し、
前記留置システムは、
近位端から遠位端に伸びるメインガイドワイヤ(2)と、
前記ガイドワイヤカテーテルの近位端にノーズコーンと、
前記遠位端からノーズコーンまでその中を伸びる補助ガイドワイヤ(14)を具備する補助カテーテル(13)と、
シース(3)と
を有し、
前記ステントグラフトは、ノーズコーンの遠位端のメインガイドワイヤ・カテーテルに保持され、
前記メインガイドワイヤカテーテルは、前記メインルーメン内を貫通して伸び、
前記補助カテーテルは、前記メインルーメン内と前記開窓内を貫通して、ノーズコーンまで伸び、
前記シースは、前記メインガイドワイヤカテーテルと同軸に配置され、前記ノーズコーンまで伸びて、前記ステントグラフトを包囲する
