サイトカインが関係する疾患治療法

る。発明の分野
本発明はヌクレオシドの分野に関する。発明の背景
哺乳類の免疫系は、主要な2種のリンパ細胞を含む:骨髄に起源するBリンパ
細胞(B細胞);および胸腺に起源するTリンパ細胞(T細胞)。B細胞は主と
して体液性免疫(即ち、抗体生産)に関与し、T細胞は主として細胞仲介免疫に
関与する。
T細胞は一般に、ヘルパーT細胞と細胞傷害性T細胞の2種類に分類されると
考えられる。ヘルパーT細胞は、細胞仲介免疫に関与するサイトカインと呼ばれ
る可溶性タンパク質仲介物
質を放出することによって、B細胞および細胞傷害性T細胞を含む他のリンパ細
胞、ならびにマクロファージを活性化する。本明細書で使用されるリンホカイン
はサイトカインのサブセットである。
ヘルパーT細胞も、一般に、Th1およびTh2の2種類に分類される。Th
1細胞(1型細胞としても既知である)は、インターロイキン2(IL−2)、
腫瘍壊死因子(TNFα)およびインターフェロンγ(IFNγ)を生産し、お
よび、遅延型過敏症および抗ウイルス免疫のような細胞仲介免疫に主に関与して
いる。これと対照的に、Th2細胞(2型細胞としても既知である)は、インタ
ーロイキン(IL4、IL−5、IL−6、IL−9、IL−10およびIL−
13)を生産し、および、アレルゲンへの反応に見られるような体液性免疫反応
の補助、例えば、IgEおよびIgG4抗体アイソタイプスイッチ、に主に関与
している(Mosmann,1989,Annu Rev Immunol,7
:145−173)。
本明細書で使用されるTh1およびTh2「反応」という用語は、Th1およ
びTh2リンパ細胞の誘発からそれぞれ生じる全ての範囲の効果を包含すること
を意味する。特に、そのよ
うな反応は、転写、翻訳、分泌、および他の機構による、対応するサイトカイン
の生産における変化、対応するリンパ細胞の増殖の増加、ならびに運動効果(m
otility effects)を含む、サイトカインの生産増加に付随する
他の効果、を包含する。
参照により本明細書に取り込まれる優先権出願は、種々のヌクレオシド(本明
細書において天然ヌクレオシドの誘導体および類似体を含むものとして定義され
る)の、リンパ細胞反応を互いに関して選択的に調節することにおける効果に関
する、我々の最近の発見の側面に関係している。特に、Th1またはTh2反応
のどちらかが選択的に抑制される一方で、他方が誘発されるかまたは比較的影響
を受けないままにすることができること、およびTh1またはTh2反応のどち
らかが選択的に誘発される一方で、他方が抑制されるかまたは比較的影響を受け
ないままにすることができることを、我々は示した。Th1およびTh2の反応
を互いに関して選択的に調節することにおけるヌクレオシドの有効性が、二形式
の効果を有する傾向があるという驚くべき事実を、我々は見い出した。特に、比
較的高い用量において一般にTh1およびTh2活性の両方を抑制ま
たは誘発する傾向があるヌクレオシドは、比較的低い用量において、Th1およ
びTh2を選択的に調節する傾向がある。
ヌクレオシドおよび他の化合物が、Th1およびTh2反応を亙いに関して選
択的に調節するメカニズムは、未だ明らかにされていない。本願発明者らによっ
て考えられる1つの可能性は、有効なヌクレオシドがグアノシン三リン酸(GT
P)の貯蔵量を変化させ、次にこのことがサイトカインが生産される割合に影響
を及ぼす。この理論において、利用可能なGTPの相対的に大きい変化が、Th
1およびTh2サイトカインの両方の濃度に影響を及ぼすのに必要である一方、
利用可能なGTPの比較的小さい変化が、Th1およびTh2サイトカインの濃
度に、異なる程度に影響を及ぼす傾向がある。
2−β−D−リボフラノシルチアゾール−4−カルホキサミド(チアゾフリン
)、合成C−ヌクレオシド類似体の、GTP濃度への影響が、この考えを支持し
ている。腫瘍細胞は、高レベルのイノシン−リン酸脱水素酵素(IMP DH)
活性を特徴とし、IMP DHが、GTP生合成の律速酵素であることが既知で

309B:287−292(1991)。チアゾフリンは、IMP DH活性を
選択的にブロックし、グアニンヌクレオチド貯蔵を枯渇させ、次にこれによって

の典型的な初期用量は約4,400mg/m2であり、統合用量は1100〜3
300mg/m2である。このようなレベルにおいて、Th1およびTh2の両
反応の合成が大きく減少し、それによって、免疫系の大部分が本質的に封鎖され
る。本発明の1つの側面において、前記用量の1/2〜1/10の範囲の非常に
少ないチアゾフリンが、Th1の反応またはTh2の反応を、他方の反応を大き
く減少させずに、特異的に抑制するのに充分であると考えられる。
1−b−D−リボフラノシル−1,2,4−トリアゾール−3−カルボキサミ
ド(リバビリン)の効果も、本発明の理論を支持している。リバビリンは、有効
な、広いスペクトルの抗ウ
イルス剤であり、IMP DHを阻害することも示されている。Yamada,

93−2196(1988)。しかし、リバビリンは、IMP DHの阻害にお
いて、チアゾフリンと異なるメカニズムにおいて進行し、酵素分子の異なる部位
で作用する。リバビリンはその活性代謝物質、リバビリン−モノホスフェート(
RMP)に転化され、これがIMP DHのIMP−XMP部位において酵素を
阻害する。チアゾフリンと同様に、リバビリンの活性形態の酵素への親和性は、
天然代謝物質の親和性よりも高い。約2200mg/m2または成人に対して約
1200〜1500mg/日の比較的高い用量において、リバビリンは、Th1
およびTh2の両反応が阻害される程度に、IMP DH活性を減少させる。約
600〜1000mg/日の比較的低い投与量において、リバビリンは、Th1
の反応を促進し、Th2の反応を抑制する。
未解明メカニズムの存在にもかかわらず、Th1およびTh2反応の互いに関
して選択的な調節から、非常に有効な利点が引
き出されることを我々は見い出した。例えば、Th2に対するTh1の特異的調
節が、感染、侵襲、腫瘍および過敏症から自己免疫疾患に及ぶ広範囲な症状およ
び疾患の治療に有効であるとの結論に我々は達した。
前記の多くの疾患に関する現在の治療法が、限られた有効性、重大な副作用、
またはその両方を有するので、これらの発見は特に重要である。例えば、自己免
疫疾患の治療は、一時的軽減法、毒性抗体の除去(例えば、重症筋無力症におけ
る)、ならびにコルチコステロイド、クロロキン誘導体、抗代謝または抗腫瘍剤
、および免疫系細胞を標的とするシクロスポリンのような薬剤を包含する有害な
薬剤の投与、に限定されることが多い。発明の概要
本出願は、疾患の治療において、Th1およびTh2の反応を互いに関して選
択的に調節するために、比較的低い投与量範囲のヌクレオシドを使用することに
関する。本発明の1つの側面において、ヌクレオシドまたは他の化合物の投与は
、主要薬剤の投与量を減少させる。本発明の他の側面において、サイトカインの
1つのグループにおける反応の増加に反映される異常が、サイトカインの他のグ
ループにおける反応を増加させるヌ
クレオシドまたは他の化合物を投与することによって、治療される。本発明のさ
らに他の側面において、Th2の活性を有意に減少させずに、Th1の活性を選
択的に減少させるヌクレオシドまたは他の化合物を投与することによって、患者
が予防的に治療される。本発明のさらに他の側面において、他方の反応を有意に
減少させずにTh1またはTh2の反応の一方を選択的に減少させる程度に患者
のGTPプールを減少させる用量において、ヌクレオシドまたは他の化合物が患
者に投与される。制御放出投与形態は特に、そのような結果を得ることを意図し
ている。
このような方法において有効であると考えられるヌクレオシドの例は、(a)
式1、1−A〜1−Fのいずれかに対応する二環式ヌクレオシド;および(b)
式2〜5のいずれかに対応する単環式ヌクレオシド;のD−およびL−形態であ
る。
このような方法において有効であると考えられる主要薬剤の例は、リバビリン
、アシクロビル(acyclovir)、およびAZTTMのような抗ウイルス剤
;トルナフテート、FungizoneTM、LotriminTM、Mycele
xTM、ナイスタチン、およびアムホテラシンのような抗菌剤;
MintezolTM、NiclocideTM、VermoxTM、およびFlag
ylTMのような抗寄生虫剤;ImmodiumTM、LomotilTM、およびP
hazymeTMのような服薬;AdriamycinTM、CytoxanTM、I
muranTM、メトトレキセート、MithracinTM、Tiazofuri
nTM、TaxolTMのような抗腫瘍剤;AclovateTM、Cyclocor
tTM、DenorexTM、FloroneTM、OxsoralenTM、コールタ
ール、およびサリチル酸のような皮膚薬;エルゴタミン化合物のような偏頭痛薬
;シクロスポリン、DiprosoneTM、ヒドロコルチゾン、FloronTM

いステロイドおよび免疫抑制剤;ならびにインシュリンのような代謝剤である。発明の詳細な説明 定義
下記の用語が本明細書において使用される場合、それらは、下記の定義のよう
に使用される。
「α」および「β」という用語は、記載されている化学構造式における不斉炭
素原子における置換基の特定の立体化学配置
を意味する。
「異常」という用語は、疾患に付随する症状を意味する。従って、本明細書に
おいては、自己免疫疾患から生じるTh1及ひ/又はTh2の反応は、たとえそ
のようなサイトカインの反応が通常その疾患から生じるものであったとしても、
対応するサイトカインの異常であると見なされる。
「アリール」という用語は、ヒドロキシル、低級アルキル、クロロ、及び/又
はシアノで任意に置換されていてもよい1つの環(例えば、フェニル)または2
つの縮合環(例えば、ナフチル)を有する一価不飽和芳香族炭素環ラジカルを意
味する。
「有効量」という用語は、免疫機能を正常レベルに回復させるか、または感染
を排除するために免疫機能を正常レベルより高いレベルに増加させる式(1)の
化合物の量を意味する。
「鏡像異性体」という用語は、重ね合わせることができない互いの鏡像である
1対の立体異性体を意味する。1対の鏡像異性体の1:1の比の混合物は、「ラ
セミ」混合物である。
「複素環」という用語は、N、OまたはSのような少なくとも1つのヘテロ原
子を有する一価飽和または不飽和炭素環ラジカルを意味し、環において、環の可
能な各位置がそれぞれ、例
えば、ヒドロキシ、オキソ、アミノ、イミノ、低級アルキル、ブロモ、クロロ、
及び/又はシアノで任意に置換されていてもよい。
「免疫調節剤」という用語は、刺激または抑制によって、正常または異常免疫
系を改質することができる天然または合成生成物を意味する。
「異性体」という用語は、同じ式を有する異なる化合物を意味する。「立体異
性体」は、原子の空間での配置の仕方においてのみ異なる異性体である。
「L−配置」という用語は、本明細書全体にわたって、核塩基(nucleo
bases)に結合した化合物のリボフラノシル成分の化学配置を記述するため
に使用されている。本発明の化合物の糖成分のL−配置は、シチジン、アデノシ
ン、チミジン、グアノシン、およびウリジンのような天然ヌクレオシドのリボー
ス糖成分のD−配置と対照的である。
「低級アルキル」という用語は、メチル、エチル、n−プロピル、イソプロピ
ル、n−ブチル、t−ブチル、I−プチルまたはn−ヘキシルを意味する。この
語はさらに、環状、分岐鎖または直鎖の1〜6個の炭素原子を意味する。
「単環式」という用語は、O、N、S、SeまたはPのような少なくとも1つ

「C−ヌクレオシド」という用語は、本明細書全体にわたって、リボース糖成
分と複素環塩基との間に形成される結合型を記述するために使用されている。C
−ヌクレオシドにおいて、結合が、リボース糖成分のC−1位から起こり、複素
環塩基の炭素と結合する。C−ヌクレオシドにおいて形成される結合は、炭素−
炭素型である。
「D−ヌクレオシド」という用語は、D−リボース糖成分(例えば、アデノシ
ン)を有するヌクレオシド化合物を意味する。
「L−ヌクレオシド」という用語は、L−リボース糖成分を有するヌクレオシ
ド化合物を意味する。
「N−ヌクレオシド」という用語は、本明細書全体にわたって、リボース糖成
分と複素塩基との間に形成される結合型を記述するために使用されている。N−
ヌクレオシドにおいて、結合が、リボース糖成分のC−1位から起こり、複素環
塩基の窒素と結合する。N−ヌクレオシドにおいて形成される結合は、炭素−窒
素型である。
「ヌクレオチド」という用語は、ヌクレオシドの5’位で置換されている燐酸
エステルを意味する。
「医薬的に許容される塩」という用語は、無機および有機酸または塩基から誘
導される塩を意味する。
「保護基」という用語は、酸素または窒素が配置されている分子中の他の成分
の誘導体化の間に、酸素または窒素原子に、それの反応を防止するために付加さ
れる化学基を意味する。酸素または窒素の種々の保護基が、有機合成の当業者に
既知である。
「プリン」という用語は、式1、1−A〜1−Fに示されている窒素二環式複
素環を意味する。
「ピリミジン」という用語は、式2〜5に示されている窒素単環式複素環を意
味する。
「腫瘍」という用語は、あらゆる種類の新生物および癌を包含する、悪性にな
る場合もあり、ならない場合もあるあらゆる種類の組織の自立的疾患性成長を、
広義に意味する。
疾患を「治療する」または「治療」という用語は、疾患の徴候または症状を軽
減するために、1種またはそれ以上の薬剤を患者に投与することを含むプロトコ
ルの実施を意味する。従って、「治療する」または「治療」とは、徴候または症
状の完全な軽減を必要とせず、治癒を必要とせず、特に、患者への最低限の効果
のみを有するプロトコルが含まれる。組み合せおよび方法
本発明の1つの側面において考えられる組み合せは一般に、主要または「第一
」薬剤と、他のまたは「第二」薬剤を含み、考えられる方法は、組合せ療法にお
いて第一および第二薬剤を選択し組み合わせることを含む。好ましい実施態様に
おいて、患者において少なくとも1種のサイトカインの異常を生じるこ
とが既知な疾患が同定され、第一薬剤が、単一療法投与量において疾患を治療す
ることが立証されている化合物の中から選択され、および本明細書に記載される
ような二形式のヌクレオシド調節剤であってもよい第二薬剤が、所定の投与量範
囲で投与された場合に疾患によって生じるまさにその異常を悪化させることが既
知の化合物の中から選択される。次に、第一薬剤が単一療法投与量より少ない量
で投与され、第二薬剤が、異常を悪化させる投与量範囲外の投与量において投与
される。第二薬剤は、関心の持たれているサイトカインの少なくとも数種に関し
て二形式の活性を有するので、組み合わせは疾患を治療するのになお有効であり
、第二薬剤の投与は、主要または第一薬剤の投与量を減少させる。
式1、1−A〜1−F、および式2〜5から選択される調節剤との組み合わせ
において有効であると考えられる主要薬剤の例は、インターフェロンαおよびγ
を包含するがそれらに限定されないインターフェロン、リバビリン、アシクロビ

、8−置換グアノシン類似体、2’,3’−ジデオキシヌクレオシド、インター
ロイキンII、IαB−インターフェ
ロンのようなインターフェロン、ツカレソル、レバミソール、イソプリノシンお
よびシクロリグナンである。
本発明に有効であると考えられる第二薬剤の例は、(a)−般式1および1−
A〜1−Fに対応する二環式ヌクレオシド、および(b)式2〜5に対応する単
環式ヌクレオシドの、D−およびL−形態である。本発明に有効な他のヌクレオ
シドおよび非ヌクレオシド化合物は、PCT/US97/00600号に記載の
ように、IL−2、TNF−α、IFN−γ、IL−4およびIL−5への効果
に関してin vitroにおいてそのような化合物をスクリーニングすること
によって、容易に同定される。

本発明の他の側面において、サイトカインの1グループにおける反応の増加に
反映される異常が、サイトカインの他のグループにおける反応を増加させるヌク
レオシドまたは他の化合物を投与することによって、治療される。従って、例え
ば、一般的な即時型アレルギーが、異常に高いTh2反応を生じさせる。その異
常は、リバビリンを、Th反応が減少される用量である600mg/日〜1,0
00mg/日(一般成人の場合)において、投与することによって治療される。
この場合、Th1およびTh2がシーソー(teeter−totter)型関
係を有し、そのためにTh2反応が抑制されるので、この治療は有効である。
本発明のさらに他の側面において、Th2活性を有意に減少させずにTh1活
性を選択的に減少させるヌクレオシドまたは他の化合物を投与することによって
、患者が予防的に治療される。例えば、この予防措置は、患者を、器官または組
織移植に対して、または予期されるアレルゲンとの接触に対して、備えさせる。
本発明のさらに他の側面においては、他方の反応を有意に減
少させずにTh1またはTh2の反応の一方を選択的に減少させる程度に、GT
P貯蔵量を減少させる用量において、ヌクレオシドまたは他の化合物が投与され
る。制御放出投与形態、特に血清中の該化合物の用量を所望範囲に維持する製剤
が、このような結果を生じると考えられる。例えばリバビリンの場合、血清濃度
が約2μM〜約5μMに維持されなければならない。送達速度において、制御放
出製剤が、USP Paddle Methodによって900mL緩衝液(p
H1.6〜7.2)において100rpmで測定した場合に、1時間後に約15
〜40重量%の化合物、2時間後に約30〜約50重量%の化合物、4時間後に
約50〜70重量%の化合物、6時間後に約60〜約80重量%の化合物の、生
体内溶解速度を有するのが好都合である。使用
本発明の組み合わせを、種々の症状、実際的には1種またはそれ以上のそのよ
うな組み合わせ投与に積極的に反応する症状、を治療するのに使用することが考
えられる。特に、そのような組み合わせを、感染、侵襲、腫瘍、過敏症、または
自己免疫疾患を治療するのに使用することが考えられる。
本発明の化合物を用いて治療することが考えられる感染症は、呼吸器合胞体ウ
イルス(RSV)、B型ウイルス性肝炎(HBV)、C型ウイルス性肝炎(HC
V)、単純疱疹1および2型、陰部疱疹、疱疹性角膜炎、疱疹状脳炎、帯状ヘル
ペス、ヒト免疫不全ウイルス(HIV)、インフルエンザA型ウイルス、ハンタ
ン(hantann)ウイルス(出血熱)、ヒト乳頭腫ウイルス(HPV)、麻
疹、および真菌を包含する。特に、本発明で請求される組み合わせが、HIV、
結核、ライ病等を含む慢性ウイルスおよび細菌感染症を治療するのに有用である
と考えられる。
本発明の化合物を用いて治療することが考えられる侵襲は、細胞内原生動物侵
襲、ならびに蠕虫および他の寄生虫侵襲を包含する。ここでもまた、特に本発明
で請求される組み合わせが、慢性侵襲を治療するのに有用であると考えられる。
治療することが考えられる腫瘍はウイルスによって生じる腫瘍を包含し、その
効果は、ウイルス感染細胞の新生物状態への変質を阻害し、変質細胞から他の正
常細胞へのウイルスの拡散を阻害し、及び/又はウイルス変質細胞の増殖を抑止
することを包含する。
治療することか考えられる過敏症は、あらゆる種類のアレルギーを包含し、T
gEおよびIgGアレルギー、高IgE症候群、およびアトピー性皮膚炎のよう
な皮膚症状が包含される。本発明で請求される組み合わせを、移植拒絶反応(移
植細胞対宿主病)および移植反応を治療するのにも使用できると考えられる。
自己免疫疾患は、非器官特異性または器官特異性のどちらかに分類することが
できる。非器官特異性自己免疫疾患は、慢性関節リウマチ、通風および通風性関
節炎、全身性紅斑性狼よう(SLE)、ショーグレン症候群、硬皮症、多発性筋
炎および皮膚性筋炎、硬直脊椎炎、ならびにリウマチ熱を包含する。器官特異性
自己免疫疾患は事実土、全ての器官において知られており、インシュリン依存型
糖尿病、甲状腺疾患(バセドウ病および橋本甲状腺炎)、アジソン病、およびア
レルギーおよび喘息を含むいくつかの腎臓および肺疾患、多発性硬化症、重症筋

サイトカインの1つのグループにおける反応の増加に反映される異常は、サイ
トカインの他のグループにおける反応を増加させるヌクレオシドを投与すること
によって治療することができることも考えられる。従って、例えば、一般的Ig
Eアレルギーは主にTh2の反応に関係しているので、Th1の反応を増加させ
るリバビリンを、約500mg/日〜約1000mg/日の低い投与量において
用いることによって、アレルギーを治療することができる。
本発明のさらに他の側面において、Th2活性を有意に減少させずにTh1活
性を選択的に減少させる化合物を投与することによって、患者が予防的に治療さ
れる。予防的治療は、器官または組織移植のような将来的な事象から予期される

本発明の化合物は、適切な医薬処方において、適切なプロトコル下に、投与さ
れることが意図される。前記のように、主要または「第一」治療剤が使用される
場合、好ましい単一治療投与量およびそのような薬剤のためのプロトコルは、P
DRに記載されており、または少なくとも製造者または販売者から入手可能であ

化することが、当業者によって理解される。従って、有効投与量は、1mg/k
g体重またはそれ未満〜25mg/kg体重またはそれ以上の範囲に及ぶ。一般
に、「第二」薬剤の治療的に有効な量は、使用される化合物、治療される症状ま
たは感染および投与経路に依存して、1mg/kg患者体重より僅かに少ない量
〜約25mg/kg患者体重またはそれ以上に及ぶと考えられる。この投与量範
囲は一般に、約0.04〜約100マイクログラム/cc患者血液の範囲の、活
性化合物の有効血中レベル濃度を生じる。しかし、少量を投与し、次に、副作用
が過度に有害になるかまたは意図する効果が得られるまで、その量を増加させる
ことによって、適切な患者特異的治療法が展開されると考えられる。
本発明の化合物の投与は、経口的、非経口的(皮下注射、静脈内、筋肉内、胸

と考えられる。例えば、本発明の化合物は、
薬理的に許容される塩として経口投与することができる。本発明の化合物は、主
として水溶性であるので、生理食塩水(例えば、pH約7.2〜7.5に緩衝)
中おいて静脈内投与することできる。燐酸塩、炭酸水素塩、またはクエン酸塩の
ような通常の緩衝液を、この目的に使用することができる。当然のことであるが
、当業者は、本明細書の開示の範囲内において製剤を改質して、本発明の組成物

有効成分および医薬的に活性な薬剤の量、ならびに投与の相対的時機は、所望の

本発明の化合物の投与経路は、連続(静脈内点滴)から、1日に数回の経口投
与(例えば、Q.I.D.)に及び、経口、局部、非経口、筋肉内、静脈内、皮
下、経皮(透過強化剤を含む場合がある)、口内、および坐薬投与などを包含す
る。
本発明の治療法を準備するために、用量を製造するための通常の医薬配合技術
によって、治療的に有効な量の化合物を、医薬的に許容される担体と完全に混合
するのが好ましい。経口または非経口などの投与に所望される製剤形態に依存し
て、担体は種々の形態をとることができる。経口投与形態の医薬組成物
の製造において、通常の医薬媒体を使用することかできる。従って、懸濁剤、エ
リキシル、および液剤のような液体経口製剤に関しては、水、グリコール、油、
アルコール、香味料、保存料、着色料などを包含する適切な担体および添加剤を
使用することができる。散剤、錠剤、カプセルのような固形経口製剤、および坐

る。発明の分野
本発明はヌクレオシドの分野に関する。発明の背景
哺乳類の免疫系は、主要な2種のリンパ細胞を含む:骨髄に起源するBリンパ
細胞(B細胞);および胸腺に起源するTリンパ細胞(T細胞)。B細胞は主と
して体液性免疫(即ち、抗体生産)に関与し、T細胞は主として細胞仲介免疫に
関与する。
T細胞は一般に、ヘルパーT細胞と細胞傷害性T細胞の2種類に分類されると
考えられる。ヘルパーT細胞は、細胞仲介免疫に関与するサイトカインと呼ばれ
る可溶性タンパク質仲介物質を放出することによって、B細胞および細胞傷害性
T細胞を含む他のリンパ細胞、ならびにマクロファージを活性化する。本明細書
で使用されるリンホカインはサイトカインのサブセットである。
ヘルパーT細胞も、一般に、Th1およびTh2の2種類に分類される。Th
1細胞(1型細胞としても既知である)は、インターロイキン2(IL−2)、
腫瘍壊死因子(TNFα)およびインターフェロンγ(IFNγ)を生産し、お
よび、遅延型過敏症および抗ウイルス免疫のような細胞仲介免疫に主に関与して
いる。これと対照的に、Th2細胞(2型細胞としても既知である)は、インタ
ーロイキン(IL4、IL−5、IL−6、IL−9、IL−10およびIL−
13)を生産し、および、アレルゲンへの反応に見られるような体液性免疫反応
の補助、例えば、IgEおよびIgG4抗体アイソタイプスイッチ、に主に関与
している

本明細書で使用されるTh1およびTh2「反応」という用語は、Th1およ
びTh2リンパ細胞の誘発からそれぞれ生じる全ての範囲の効果を包含すること
を意味する。特に、そのような反応は、転写、翻訳、分泌、および他の機構によ
る、対応するサイトカインの生産における変化、対応するリンパ細胞の増殖の増
加、ならびに運動効果(motility effects)を含む、サイトカ
インの生産増加に付随する他の効果、を包含する。
参照により本明細書に取り込まれる優先権出願は、種々のヌクレオシド(本明
細書において天然ヌクレオシドの誘導体および類似体を含むものとして定義され
る)の、リンパ細胞反応を互いに関して選択的に調節することにおける効果に関
する、我々の最近の発見の側面に関係している。特に、Th1またはTh2反応
のどちらかが選択的に抑制される一方で、他方が誘発されるかまたは比較的影響
を受けないままにすることができること、およびTh1またはTh2反応のどち
らかが選択的に誘発される一方で、他方が抑制されるかまたは比較的影響を受け
ないままにすることができることを、我々は示した。Th1およびTh2の反応
を互いに関して選択的に調節することにおけるヌクレオシドの有効性が、二形式
の効果を有する傾向があるという驚くべき事実を、我々は見い出した。特に、比
較的高い用量において一般にTh1およびTh2活性の両方を抑制または誘発す
る傾向があるヌクレオシドは、比較的低い用量において、Th1およびTh2を
選択的に調節する傾向がある。
ヌクレオシドおよび他の化合物が、Th1およびTh2反応を互いに関して選
択的に調節するメカニズムは、未だ明らかにされていない。本願発明者らによっ
て考えられる1つの可能性は、有効なヌクレオシドがグアノシン三リン酸(GT
P)の貯蔵量を変化させ、次にこのことがサイトカインが生産される割合に影響
を及ぼす。この理論において、利用可能なGTPの相対的に大きい変化が、Th
1およびTh2サイトカインの両方の濃度に影響を及ぼすのに必要である一方、

チアゾフリンは、IMP DH活性を選択的にブロックし、グアニンヌクレオチ

5−95(1989)。チアゾフリンの典型的な初期用量は約4,400mg/
m2であり、統合用量は1100〜3300mg/m2である。このようなレベル
において、Th1およびTh2の両反応の合成が大きく減少し、それによって、
免疫系の大部分が本質的に封鎖される。本発明の1つの側面において、前記用量
の1/2〜1/10の範囲の非常に少ないチアゾフリンが、Th1の反応または
Th2の反応を、他方の反応を大きく減少させずに、特異的に抑制するのに充分
であると考えられる。
1−b−D−リボフラノシル−1,2,4−トリアゾール−3−カルボキサミ
ド(リバビリン)の効果も、本発明の理論を支持している。リバビリンは、有効
な、広いスペクトルの抗ウイルス剤であり、IMP DHを阻害することも示さ
れている。Yamada,Y.ら、Action of the Active Metabolites of Tiazofurin and Ribav irinon Purified TMP Dehydrogenase

ochem.27:2193−2196(1988)。しかし、リバビリンは、
IMP DHの阻害において、チアゾフリンと異なるメカニズムにおいて進行し
、酵素分子の異なる部位で作用する。リバビリンはその活性代謝物質、リバビリ
ン−モノホスフェート(RMP)に転化され、これがIMP DHのIMP−X
MP部位において酵素を阻害する。チアゾフリンと同様に、リバビリンの活性形
態の酵素への

は、Th1およびTh2の両反応が阻害される程度に、IMP DH活性を減少

は、Th1の反応を促進し、Th2の反応を抑制する。
未解明メカニズムの存在にもかかわらず、Th1およびTh2反応の互いに関
して選択的な調節から、非常に有効な利点が引き出されることを我々は見い出し
た。例えば、Th2に対するTh1の特異的調節が、感染、侵襲、腫瘍および過
敏症から自己免疫疾患に及ぶ広範囲な症状および疾患の治療に有効であるとの結
論に我々は達した。
前記の多くの疾患に関する現在の治療法が、限られた有効性、重大な副作用、
またはその両方を有するので、これらの発見は特に重要である。例えば、自己免
疫疾患の治療は、一時的軽減法、毒性抗体の除去(例えば、重症筋無力症におけ
る)、ならびにコルチコステロイド、クロロキン誘導体、抗代謝または抗腫瘍剤
、および免疫系細胞を標的とするシクロスポリンのような薬剤を包含する有害な
薬剤の投与、に限定されることが多い。発明の概要
本出願は、疾患の治療において、Th1およびTh2の反応を互いに関して選
択的に調節するために、比較的低い投与量範囲のヌクレオシドを使用することに
関する。本発明の1つの側面において、ヌクレオシドまたは他の化合物の投与は
、主要薬剤の投与量を減少させる。本発明の他の側面において、サイトカインの
1つのグループにおける反応の増加に反映される異常が、サイトカインの他のグ
ループにおける反応を増加させるヌクレオシドまたは他の化合物を投与すること
によって、治療される。本発明のさらに他の側面において、Th2の活性を有意
に減少させずに、Th1の活性を選択的に減少させるヌクレオシドまたは他の化
合物を投与することによって、患者が予防的に治療される。本発明のさらに他の
側面において、他方の反応を有意に減少させずにTh1またはTh2の反応の一
方を選択的に減少させる程度に患者のGTPプールを減少させる用量において、

る。
このような方法において有効であると考えられる主要薬剤の例は、リバビリン

疾患を「治療する」または「治療」という用語は、疾患の徴候または症状を軽
減するために、1種またはそれ以上の薬剤を患者に投与することを含むプロトコ
ルの実施を意味する。従って、「治療する」または「治療」とは、徴候または症
状の完全な軽減を必要とせず、治癒を必要とせず、特に、患者への最低限の効果

本発明の1つの側面において考えられる組み合せは一般に、主要または「第一
」薬剤と、他のまたは「第二」薬剤を含み、考えられる方法は、組合せ療法にお
いて第一および第二薬剤を選択し組み合わせることを含む。好ましい実施態様に
おいて、患者において少なくとも1種のサイトカインの異常を生じることが既知
な疾患が同定され、第一薬剤が、単一療法投与量において疾患を治療することが
立証されている化合物の中から選択され、および本明細書に記載されるような二
形式のヌクレオシド調節剤であってもよい第二薬剤が、所定の投与量範囲で投与
された場合に疾患によって生じるまさにその異常を悪化させることが既知の化合
物の中から選択される。次に、第一薬剤が単一療法投与量より少ない量で投与さ

本発明の他の側面において、サイトカインの1グループにおける反応の増加に
反映される異常が、サイトカインの他のグループにおける反応を増加させるヌク
レオシドまたは他の化合物を投与することによって、治療される。従って、例え
ば、一般的な即時型アレルギーが、異常に高いTh2反応を生じさせる。その異
常は、リバビリンを、Th反応が減少される用量である600mg/日〜1,0
00mg/日(一般成人の場合)において、投与することによって治療される。
この場合、Th1およびTh2がシーソー(teeter−totter)型関
係を有し、そのためにTh2反応が抑制されるので、この治療は有効である。
本発明のさらに他の側面において、Th2活性を有意に減少させずにTh1活
性を選択的に減少させるヌクレオシドまたは他の化合物を投与することによって
、患者が予防的に治療される。例えば、この予防措置は、患者を、器官または組
織移植に対して、または予期されるアレルゲンとの接触に対して、備えさせる。
本発明のさらに他の側面においては、他方の反応を有意に減少させずにTh1
またはTh2の反応の一方を選択的に減少させる程度に、GTP貯蔵量を減少さ
せる用量において、ヌクレオシドまたは他の化合物が投与される。制御放出投与
形態、特に血清中の該化合物の用量を所望範囲に維持する製剤が、このような結
果を生じると考えられる。例えばリバビリンの場合、血清濃度が約2μM〜約5
μMに維持されなければならない。送達速度において、制御放出製剤が、USP
Paddle Methodによって900mL緩衝液(pH1.6〜7.2)
において100rpmで測定した場合に、1時間後に約15〜40重量%の化合
物、2時間後に約30〜約50重量%の化合物、4時間後に約50〜70重量%
の化合物、6時間後に約60〜約80重量%の化合物の、生体内溶解速度を有す
るのが好都合である。使用
本発明の組み合わせを、種々の症状、実際的には1種またはそれ以上のそのよ
うな組み合わせ投与に積極的に反応する症状、を治療するのに使用することが考
えられる。特に、そのような組み合わせを、感染、侵襲、腫瘍、過敏症、または
自己免疫疾患を治療するのに使用することが考えられる。
本発明の化合物を用いて治療することが考えられる感染症は、呼吸器合胞体ウ
イルス(RSV)、B型ウイルス性肝炎(HBV)、C型ウイルス性肝炎(HC
V)、単純疱疹1および2型、陰部疱疹、疱疹性角膜炎、疱疹状脳炎、帯状ヘル
ペス、ヒト免疫不全ウイルス(HIV)、インフルエンザA型ウイルス、ハンタ
ン(hantann)ウイルス(出血熱)、ヒト乳頭腫ウイルス(HPV)、麻
疹、および真菌を包含する。特に、本発明で請求される組み合わせが、HIV、
結核、ライ病等を含む慢性ウイルスおよび細菌感染症を治療するのに有用である
と考えられる。
本発明の化合物を用いて治療することが考えられる侵襲は、細胞内原生動物侵
襲、ならびに蠕虫および他の寄生虫侵襲を包含する。ここでもまた、特に本発明
で請求される組み合わせが、慢性侵襲を治療するのに有用であると考えられる。
治療することが考えられる腫瘍はウイルスによって生じる腫瘍を包含し、その
効果は、ウイルス感染細胞の新生物状態への変質を阻害し、変質細胞から他の正
常細胞へのウイルスの拡散を阻害し、及び/又はウイルス変質細胞の増殖を抑止
することを包含する。
治療することが考えられる過敏症は、あらゆる種類のアレルギーを包含し、I
gEおよびIgGアレルギー、高IgE症候群、およびアトピー性皮膚炎のよう
な皮膚症状が包含される。本発明で請求される組み合わせを、移植拒絶反応(移
植細胞対宿主病)および移植反応を治療するのにも使用できると考えられる。
自己免疫疾患は、非器官特異性または器官特異性のどちらかに分類することが
できる。非器官特異性自己免疫疾患は、慢性関節リウマチ、通風および通風性関
節炎、全身性紅斑性狼よう(SLE)、ショーグレン症候群、硬皮症、多発性筋
炎および皮膚性筋炎、硬直脊椎炎、ならびにリウマチ熱を包含する。器官特異性
自己免疫疾患は事実上、全ての器官において知られており、インシュリン依存型
糖尿病、甲状腺疾患(バセドウ病および橋本甲状腺炎)、アジソン病、およびア
レルギーおよび喘息を含むいくつかの腎臓および肺疾患、多発性硬化症、重症筋

患、ならびに数種の男性および女性の不妊症を包含する。自己免疫過程は、HI
Vウイルスを含むウイルス感染によって刺激される場合もあり、移植拒絶反応か
ら生じる場合もあり、ある種の腫瘍に関係する場合もあり、またはある種の化学
物質への曝露によって促進される場合もある。
サイトカインの1つのグループにおける反応の増加に反映される異常は、サイ
トカインの他のグループにおける反応を増加させるヌクレオシドを投与すること
