Descrizione di Brevetto di Invenzione Industriale avente per titolo:
“PROCEDIMENTO PER LA REALIZZAZIONE DI PROTESI BIOLOGICAMENTE
COMPATIBILI”
CAMPO TECNICO DELL’INVENZIONE
L’invenzione riguarda un processo per la realizzazione di protesi biologicamente compatibili, particolarmente adatte per realizzare elementi impiantabili nel corpo umano ed animale.
STATO DELLA TECNICA ANTERIORE
Le protesi, come ad es. cotili, impianti femorali e tibiali, glenoidi ed altri componenti simili, vengono impiegate per l’impianto nel corpo umano o animale onde sostituire parti o articolazioni ossee irrimediabilmente danneggiate da traumi oppure da malattie come l’artrite e l’artrosi.
Numerose sono le tecniche di produzione. Ricordiamo le lavorazioni per asportazione di truciolo, il casting, il “Selective Laser Sintering” (SLS) e l”Electron Beam Melting” (EBM). Con queste due ultime tecniche si producono protesi mediante deposizione e sinterizzazione di strati di polvere successivi.
Alcune delle superfici esterne delle protesi sono poi solitamente trattate per conferire loro caratteristiche di porosità e/o rugosità, in modo da favorire o accelerare il processo di osteointegrazione.
Un particolare processo, di tipo sottrattivo, è descritto nella domanda di brevetto WO2009/004444. Si pone in uno stampo una quantità di materiale in polvere che contiene un secondo materiale granulare asportabile (detto anche spaziatore) avente temperatura di fusione maggiore della temperatura di sinterizzazione del primo materiale. I due materiali vengono dapprima mescolati in modo omogeneo, e successivamente sottoposti ad un processo di sinterizzazione e solidificazione. L’oggetto risultante viene immerso in acqua oppure in una soluzione acida o basica. L’immersione nella soluzione permette di sciogliere il materiale spaziatore,che lascia vuoto il volume che aveva occupato nella fase di pressatura e sinterizzazione. A dissoluzione ultimata, il pezzo, che risulta costituito solo dal primo materiale, è caratterizzato dalla presenza di alveoli e pori in superficie.
Questo processo ha limitati gradi di libertà nella realizzazione di protesi, soprattutto perché la superficie porosa e lo strato su cui giace sono costituiti dallo stesso materiale. Tuttavia – si pensi ad un acetabolo - la scelta ideale sarebbe quella di avere a disposizione servirebbe ottenere una superficie porosa costituita da un materiale biocompatibile ad alto potere osteointegrante su una base sottostante ad alta resistenza meccanica e resistente all’usura. Sistemi con queste duplici caratteristiche non sono frequenti e comunque non riescono a soddisfare completamente le caratteristiche sovraesposte.
SCOPI DELL’INVENZIONE
Scopo principale dell’invenzione è definire un processo per la realizzazione di protesi biologicamente compatibili avente maggiori gradi di libertà nella realizzazione delle protesi stesse. Altro scopo è la realizzazione di protesi biologicamente compatibili con migliore resistenza, durata e compatibilità col corpo che le riceve.
Tale scopo è raggiunto con un procedimento per la realizzazione di un componente di protesi (1) biologicamente compatibile, caratterizzato dal fatto di comprendente le fasi di (i) disporre di due o più materiali dalle caratteristiche fisico/chimiche diverse;
(ii) definire in mezzi di formatura (6) il componente come composizione di almeno due volumi di detti due o più materiali;
(iii) sinterizzare nei mezzi di formatura (6) ad una certa temperatura di sinterizzazione il componente, per ottenere un componente finito formato da parti tutte dense o da almeno una parte porosa.
L’uso di due o più materiali consente di progettare il componente con meno vincoli strutturali, oltre a poter realizzare una superficie porosa per l’osteointegrazione su una base resistente all’usura.
Preferibilmente uno o ciascuno dei due o più materiali è disponibile inizialmente sotto forma di polvere, per sfruttare al meglio le tecniche di sinterizzazione. In particolare, si può realizzare una parte porosa partendo da un primo materiale in polvere e addizionando a questo un materiale detto spaziatore, avente una temperatura di fusione maggiore della temperatura di sinterizzazione del primo materiale. Successivamente si rimuove dal componente finito il materiale spaziatore creando sul componente finito dei corrispondenti spazi vuoti (pori). Altre tecniche di formazione della parte porosa sono comunque fattibili, v. ad es. US 6 849 230. Pertanto il pezzo dopo la sinterizzazione può anche essere tutto denso, perché lo spaziatore deve essere ancora rimosso.
Si può prevedere che uno o ciascuno dei due o più materiali sia un materiale inizialmente disponibile in forma sinterizzata o pre-sinterizzata. Questo apre il campo a tantissimi nuovi prodotti, che possono sfruttare le caratteristiche fisiche/meccaniche di porzioni della protesi realizzate o preparate prima della sinterizzazione. Lo stesso vale qualora uno o ciascuno dei due o più materiali sia un materiale avente già una struttura porosa, la quale può essere associata al resto del componente sinterizzato nel processo qui descritto, ma venire prodotta in precedenza , col vantaggio di prescindere ad es. dai parametri o dai vincoli della sinterizzazione descritta.
Preferibilmente, per economia di produzione, si definisce il componente sovrapponendo i due o più materiali in strati.
Preferibilmente il processo di sinterizzazione è lo SPS (Spark Plasma Sintering).
Uno o più di detti materiali possono essere costituiti da leghe dello stesso metallo e/o di metalli diversi e/o da metalli puri.
I metalli possono essere scelti tra: Ti, Cr, Co, Mo, Fe, Zr, Nb, Ta, Mg.
Le leghe metalliche possono essere selezionate tra i gruppi che consistono di: Cobalto-Cromo, Magnesio, Titanio-Alluminio, Titanio-Nickel, Zirconio-Niobio e Tantalio, Ferro-Carbonio e ogni combinazione tra essi.
Altra variante è che uno o ciascuno di detti uno o più materiali sia un materiale ceramico o un composito ceramico. Il materiale ceramico può essere scelto dal gruppo che consiste di: Allumina, Zirconia, Zirconia stabilizzata (TZP, PSZ), Allumina Tenacizzata con Zirconia (ZTA), Zirconia Tenacizzata con Allumina (ATZ), Nitruro di Cromo, Carburo di Silicio, Nitruro di Silicio, Carburo di Titanio, Nitruro di Titanio, Carburo di Zirconio, Nitruro di Zirconio, Carburo di Tantalio, Carburo di Tungsteno, Idrossiapatite, Calciofosfati Magnesia, e ogni combinazione tra essi.
Ancora, uno o ciascuno di detti uno o più materiali può essere un materiale formato da miscele di polveri metalliche e/o ceramiche oppure da una matrice metallica o ceramica o polimerica o polimerica rinforzata da materiali fibrosi o particellari.
L’invenzione è rivolta anche alla protesi biocompatibile realizzata secondo il metodo appena descritto.
BREVE DESCRIZIONE DEI DISEGNI.
Ulteriori caratteristiche e vantaggi dell’invenzione risulteranno maggiormente evidenti dalla descrizione esemplificativa di un processo, insieme agli allegati disegni in cui:
la figura 1 mostra un acetabolo producibile con l’invenzione;
la figura 2 mostra in sezione verticale una pressa di sinterizzazione durante un processo secondo l’invenzione;
la figura 3 mostra un dettaglio ingrandito di fig.2,
la figura 4 mostra un altro oggetto producibile con l’invenzione;
la figura 5 mostra un piatto tibiale producibile con l’invenzione.
FORME DI ATTUAZIONE DELL’INVENZIONE.
La presente invenzione è adatta per realizzare protesi ortopediche in generale.
Ad esempio, il processo descritto è ideale per realizzare una protesi d’anca, ad es. una coppa acetabolare 50 (fig. 1). La coppa 50 ha una superficie esterna convessa 52 porosa (quella che andrà a contatto con la sede ossea) costituita da un primo materiale 22 e una calotta interna resistente ad usura in un secondo materiale 20 (parte densa). Il processo dell’invenzione è utilizzabile anche per realizzare protesi di rivestimento, steli femorali, un piatto tibiale 90 (fig. 5), per una protesi di ginocchio protesi monocompartimentali (e non del ginocchio), o in genere protesi ortopediche che consistono di geometrie piatte e/o curve, con o senza fori, alette, spine, etc.
I materiali 20, 22 preferibilmente hanno caratteristiche chimico/fisiche diverse e sono scelti per massimizzare con le loro proprietà una diversa funzione: massima biointegrazione per il materiale 22, massima resistenza ad usura e resistenza meccanica per il materiale 20.
La sovrapposizione dei materiali 20, 22 qualifica i componenti come FGM (Functionally Graded Materials), vale a dire che i componenti sono nell’es. formati da una parte densa (ossia compatta e piena, il materiale 20) e una porosa (ossia con spazio vuoti, il materiale 22), la cui unione crea così strutture FGM.
La coppa 50 è realizzata mediante sinterizzazione via Spark Plasma Sintering (SPS), o, in genere, mediante qualsiasi processo di sinterizzazione (fig. 2, 3). La sinterizzazione sfrutta uno stampo metallico 10 formato da un corpo 14, un punzone 12 e una matrice 18. Sulla matrice 18 si dispongono due strati sovrapposti di polvere di materiale 20, 22 previo mescolamento nel materiale 22 con un terzo materiale spaziatore asportabile 26 in granuli.
Nell’esempio si considerano due materiali ma il processo può prevederne anche di più, secondo la tipologia e la struttura progettata per la protesi da realizzare. Anche la struttura finale può essere considerata come un multistrato, avendosi ad es. n materiali per formare n strati.
Tra il punzone 12 e la matrice 18 viene collegato un generatore di tensione 16 con lo scopo di indurre una corrente tra i due pezzi. Tale corrente scorre anche nei materiali (conduttori) 20, 22 e genera localmente dei plasmi tra le loro particelle, fondendole tra loro e solidificando le polveri. Terminata la sinterizzazione, l’oggetto composto dai tre materiali 20, 22, 26 viene prelevato dallo stampo 10 e immerso in acqua o una soluzione opportuna, in modo da sciogliere il materiale 26, asportarlo dal materiale ospite 22 lasciandolo parzialmente poroso o . Lo spazio svuotato dal materiale 26 costituisce gli spazi vuoti o pori della parte porosa 52.
A seconda del tipo e numero di materiali e strati presenti nel processo di sinterizzazione, i componenti finali realizzati secondo l’invenzione potranno ad es. essere composti da o comprendere:
- materiali diversi per la parte densa e porosa che differiscano per il fatto di essere in forma pura o in lega (ad es. Ti grado 5 per la parte densa e Ti grado 1-4 per la parte porosa);
- due o più leghe dello stesso metallo (ad es. Ti6Al4V, Ti-Ni, Ti6Al7Nb, Ti13Nb13Zr, Ti12Mb6Al, leghe di Ta);
- due o più materiali diversi: preferibilmente Titanio per la parte porosa e lega di CoCrMo per la parte densa;
- un materiale multistrato composto da strati di Titanio grado 2 (strato poroso, ad es. con MgO da rimuovere), di Titanio grado 1-4 o 5 (strato denso) e di CoCr (strato denso). In questo tipo di strutture lo strato denso di titanio può avere una funzione di barriera antirilascio ionico del CoCr nel corpo, problema che da molto cerca soluzione;
- uno o più materiali in polvere 62, 64 (v. oggetto 60 in fig.4), disposti a strato o in un volume con forma qualsiasi, e uno o più materiali compositi o ceramici 66 in polvere, disposti a strato o in un volume con forma qualsiasi. Il materiale composito, a sua volta, può essere costituito da una matrice e da particelle o fibre di rinforzo. Come materiali ceramici si possono usare a tale scopo: Ossido di zirconio (o Zirconia), Y-TZP (Zirconia stabilizzata con Yttria), PSZ (Zirconia stabilizzata ad es. con MgO, CeO2, etc.), Allumina, ZTA (Zirconia Toughened Alumina), ATZ (Alumina Toughned Zirconia). La matrice può anche contenere fibre di carbonio;
- uno strato di materiale polimerico (es. PE o UHMWPE o PEEK);
- due o più porzioni porose tra loro accoppiate, ottenute ad es. tramite asportazione selettiva di materiale spaziatore da due o più strati di materiali diversi o sinterizzando tra loro due o più materiali porosi già sinterizzati;
- un materiale poroso o a griglia o a maglia (network) realizzato in precedenza (ad es. sinterizzato precedentemente) il quale viene accoppiato ad un materiale in polvere che viene sinterizzato col processo descritto o
Come materiale spaziatore 26 si può usare MgO, ma vanno bene anche ad es. cloruro di sodio, oppure idrossilapatite.
Le temperature di sinterizzazione e gli altri parametri del processo dipendono da molte variabili, tra cui il tipo di oggetto da produrre, la sua forma, i materiali costituenti ed i materiali con cui è costruito lo stampo. Ad esempio, nel caso si desideri realizzare un componente protesico avente una lega di Co-Cr come costituente della parte densa e del Titanio grado 4 come costituente della parte porosa, la temperatura di sinterizzazione può variare tra 700-2000°C, la pressione può variare tra 5-100 MPa e la durata della sinterizzazione può variare tra 1-15 minuti.
Questi valori, come per le correnti circolanti nello stampo, vanno ottimizzati di volta in volta. Essi dipendono fortemente dai materiali, dalla forma dello stampo e dalla geometria dell’oggetto da sinterizzare.
Claims (12)
- RIVENDICAZIONI 1. Procedimento per la realizzazione di un componente di protesi (1) biologicamente compatibile, caratterizzato dal fatto di comprendente le fasi di (i) disporre di due o più materiali (20, 22) dalle caratteristiche fisico/chimiche diverse; (ii) definire in mezzi di formatura (10) il componente come composizione di almeno due volumi di detti due o più materiali; (iii) sinterizzare nei mezzi di formatura (6) ad una certa temperatura di sinterizzazione il componente, per ottenere un componente finito formato da parti o tutte dense (22) o almeno una porosa (20).
- 2. Procedimento secondo la rivendicazione 1, in cui per uno o ciascuno dei due o più materiali si usano materiali sotto forma di polvere.
- 3. Procedimento secondo la rivendicazione 2, in cui - la parte porosa viene formata partendo da un materiale in polvere e addizionando a questo un materiale spaziatore avente una temperatura di fusione maggiore della temperatura di sinterizzazione (T1); e - successivamente rimuovendo dal componente finito il materiale spaziatore per lasciare sul componente finito corrispondenti spazi vuoti.
- 4. Procedimento secondo la rivendicazione 2, in cui uno o ciascuno dei due o più materiali è un materiale già sinterizzato o un materiale pre-sinterizzato.
- 5. Procedimento secondo la rivendicazione 2, in cui uno o ciascuno dei due o più materiali è un materiale avente già una struttura porosa.
- 6. Procedimento secondo una qualsiasi delle rivendicazioni precedenti, in cui si definisce il componente con strati sovrapposti dei due o più materiali.
- 7. Procedimento secondo una delle rivendicazioni precedenti, in cui uno o più di detti materiali è costituito da leghe dello stesso metallo e/o di metalli diversi e/o da metalli puri.
- 8. Procedimento secondo la rivendicazione 7, in cui il metallo è scelto nel gruppo comprendente Ti, Cr, Co, Mo, Fe, Zr, Mn, Ta, Mg e/o da leghe metalliche scelte nel gruppo comprendente: Cobalto-Cromo, Magnesio, Titanio-Alluminio, Zirconio-Niobio e Tantalio, Ferro-Carbonio e ogni combinazione tra essi.
- 9. Procedimento secondo una delle rivendicazioni precedenti, in cui uno o ciascuno di detti uno o più materiali è un materiale ceramico o un composito ceramico .
- 10. Procedimento secondo la rivendicazione 9, in cui il materiale ceramico è scelto dal gruppo che comprende: Allumina, Zirconia, Zirconia stabilizzata (Y-TZP, PSZ), Allumina Tenacizzata con Zirconia (ZTA), Zirconia Tenacizzata con Allumina (ATZ), Nitruro di Cromo, Carburo di Silicio, Nitruro di Silicio, Carburo di Titanio, Nitruro di Titanio, Carburo di Zirconio, Nitruro di Zirconio, Carburo di Tantalio, Carburo di Tungsteno, Idrossiapatite, Calciofosfati Magnesia, e ogni combinazione tra essi.
- 11. Procedimento secondo una delle rivendicazioni precedenti, in cui uno o ciascuno di detti uno o più materiali è un materiale formato da miscele di polveri metalliche e/o ceramiche oppure da una matrice metallica o ceramica o polimerica o polimerica rinforzata da materiali fibrosi o particellari.
- 12. Protesi biocompatibile realizzata secondo una qualsiasi delle rivendicazioni precedenti.
