Abstract

La présente invention concerne les têtes de prothèse. La tête selon l'invention se caractérise par le fait qu'elle comporte une calotte sphérique 2 apte à coopérer en rotation dans un cotyle 3, une patte de fixation 4 apte à être implantée dans le canal médulaire d'un os 5 tel qu'un fémur, des moyens de liaison 6 de la calotte sphérique avec une extrémité 7 de la patte de fixation, ces moyens de liaison comprenant une partie en saillie 10 solidaire d'un méplat 11 réalisé sur la calotte sphérique, un manchon 12 comportant deux percées 13, 14, une première percée ayant une section complémentaire de la partie en saillie qui est enfichée dans cette première percée, et la seconde percée ayant une section sensiblement complémentaire de l'extrémité de la patte de fixation qui est enfichée dans cette seconde percée. Description

Tête de prothèse notamment pour prothèse de hanche La présente invention
concerne les têtes de prothèses, notamment pour prothèses de hanche ou analogue. On connaît déji des têtes de prothèses comportant une calotte sphérique apte i coopérer en rotation dans un cotyle, une patte de fixation apte à être implantée dans un os, par exemple dans le canal médulaire d'un fémur, et des moyens de liaison de la calotte sphérique avec une extrémité de la
patte de fixation.
Les calottes sphériques sont généralement réalisées dans des matériaux tels que de la céramique, et même, depuis peu, dans des mQnocristaux du type saphir, rubis, etc. Dans ce cas, l'extrémité de la patte en liaison avec la calotte sphérique est sensiblement tronconique et les moyens de liaison sont constitués par un logement cylindrique réalisé dans la calotte sphérique et un manchon comportant essentiellement deux parties, une partie mâle complémentaire s'enfichant dans le logement cylindrique et une partie femelle venant, elle, s'emmancher en force
sur l'extrémité tronconique de la patte.
Pour des raisons de fiabilité, les calottes sphériques sont réalisées dans des matériaux les plus durs et solides possible. De ce fait, la réalisation du logement en creux dans ces calottes pose des problèmes. Elle nécessite en effet l'utilisation d'un outillage adapté de très grande qualité et rend difficile le contrôle de l'état de la surface intérieure du logement car cette surface est difficilement
accessible aux appareils qui peuvent effectuer un tel contrôle.
La présente invention a pour but de réaliser une tète de prothèse présentant une structure qui soit d'une réalisation plus aisée que celle des prothèses de l'art antérieur et qui, de plus, permette de contr8ler plus facilement l'état des surfaces usinées afin d'assurer aux prothèses une fiabilité maximale, ce qui est primordial dans ce genre de produit pour éviter des interventions chirurgicales
trop fréquentes.
Plus précisément, la présente invention a pour objet une tête de prothèse du type "prothèse de hanche" ou analogue, comportant une calotte sphérique apte à coopérer en rotation dans un cotyle, une patte de fixation apte à être implantée dans le canal médulaire d'un os tel qu'un fémur, et des moyens de liaison de la calotte sphérique avec une extrémité de ladite patte de fixation, caractérisée par le fait que lesdits moyens de liaison comportent: une partie en saillie solidaire d'un méplat réalisé sur ladite calotte sphérique, et un manchon comportant deux percées opposées, une première percée ayant une section complémentaire de ladite partie en saillie, ladite partie en saillie étant enfichée dans cette dite première percée, et la seconde percée ayant une section sensiblement complémentaire de ladite extrémité de ladite patte de fixation, cette
dite extrémité étant enfichée dans cette seconde percée.
D'autres caractéristiques et avantages de la présente
invention apparaîtront au cours de la description suivante donnée en
regard du dessin annexé à titre illustratif, mais nullement limitatif, dans lequel la figure unique représente, vu en coupe partielle, un
mode de réalisation d'une tête de prothèse selon l'invention.
La figure unique représente une tête de prothèse 1 du type "prothèse de hanche" ou analogue, comportant une calotte sphérique 2 apte à coopérer en rotation dans un cotyle 3, une patte de fixation 4 apte à être implantée dans le canal médulaire d'un os 5 tel qu'un fémur, et des moyens de liaison 6 de la calotte sphérique 2 avec une
extrémité 7 de la patte 4.
Les moyens de liaison 6 comportent une partie en saillie 10 solidaire d'un méplat 11 réalisé sur la calotte sphérique 2, et un manchon 12 comportant deux percées 13 et 14. Une première percée 13 possède une section complémentaire de la partie en saillie 10 qui est enfichée dans cette première percée 13, et la seconde percée 14 a une section sensiblement complémentaire de l'extrémité 7 de la patte de
fixation 4 qui est, pour sa part, enfichée dans cette seconde percée.
Dans un mode réalisation avantageux, la partie en saillie 10 a une section inférieure au diamètre du méplat 11 de façon i définir une surface portante sur laquelle pourra venir s'épauler une partie du manchon 12 comme décrit ci-après. Toujours dans le but d'obtenir une structure facilement réalisable par usinage, la partie en saillie 10 et la première percée 13 sont avantageusement de forme cylindrique de révolution. Pour permettre a la calotte d'absorber la plus grande quantité possible des efforts auxquels la prothèse implantée dans un corps humain est généralement soumise, avant qu'ils ne soient transmis au cotyle, l'ouverture extérieure 15 de la première percée 13 est délimitée par un épaulement externe 16 en forme de couronne circulaire d'un diamètre extérieur sensiblement inférieur ou égal à celui du méplat 11. De cette façon, la calotte sphérique prend bien appui sur
le manchon 12 par une première surface relativement grande.
Dans le mode de réalisation avantageux illustré, l'extrémité 7 de la patte de fixation 4 est usinée en forme de tronc de c8ne de révolution, dont la grande base 17 définit la surface de liaison entre
le corps 18 de la patte et l'extrémité 7, elle-même, de cette patte.
Dans ce cas, la seconde percée 14 est également de forme sensiblement tronconique de révolution mais d'un angle au sommet légèrement inférieur à celui du tronc de c6ne défissant l'extrémité 7, tout en ayant sa grande base 19 d'une section égale à l'une des sections de cette extrémité de la patte de fixation, par exemple une section au voisinage de celle qui est indiquée en trait interrompus 20
sur la figure unique.
Il a été mentionné ci-avant que la calotte sphérique prenait appui sur un premier épaulement 16 du manchon. Afin de mieux encore répartir les efforts exercés sur la prothèse, la surface portante de la tête de prothèse a été augmentée en réalisant, dans le fond de la première percée 13, un épaulement rentrant 21 situé à une distance de l'ouverture 22 de la première percée égale à la hauteur de la partie
en saillie 10.
Pour rendre la réalisation mécanique d'un tel manchon la plus aisée possible, il est avantageux que les fonds, respectivement des deux première 13 et seconde 14 percées, soient confondus, comme illustré sur la figure, ainsi que les axes de révolution 23, 24 de ces deux percées. De plus, la surface latérale tronconique de la seconde percée est réalisée de façon que le diamètre de sa petite base soit inférieur à celui de la première percée. La différence des sections forme ainsi directement lors de l'usinage l'épaulement 21 défini ci-dessus. Comme mentionné ci-avant, la partie en saillie 10 est solidaire de la calotte sphérique 2. Cependant, l'un des moyens O5 avantageux pour obtenir cette solidarisation est de réaliser la calotte et la partie en saillie dans la masse d'un même bloc de matériau, par exemple, un monocristal tel que du rubis, du saphir, etc. La tête de prothèse telle que décrite ci-dessus et illustrée
sur la figure unique se monte de la façon suivante.
Dès que les trois éléments sont réalisés, la calotte sphérique 2 avec sa partie en saillie 10 est montée en coopération avec le manchon 12 de façon à plonger, par cette partie en saillie, dans la première percée 13 de ce manchon avec lequel elle est solidarisée par tous moyens, notamment par collage, soudage, brasage, selon la nature des matériaux constituant respectivement la calotte sphérique, la
partie en saillie et le manchon.
L'extrémité 7 de la patte de fixation 4 est alors emmanchée dans la seconde percée 14 du manchon Jusqu'à ce que le bord de la grande base 20 de cette seconde percée vienne au contact de la surface du tronc de c6ne définissant l'extrémité de la patte. Par un effort supplémentaire d'enfoncement, la paroi du manchon se déforme au niveau de sa seconde percée de façon que la conicité de cette dernière devienne sensiblement égale à celle de l'extrémité de la patte de fixation, la paroi interne de la percée épousant alors parfaitement la surface de l'extrémité de la patte de fixation. Ceci permet de bien solidariser le manchon et la calotte sphérique avec la patte de fixation, par un effet de pincement essentiellement dû à l'élasticité
du métal utilisé, généralement un alliage à base de titane.
La structure de tête de prothèse telle que décrite ci-dessus présente de multiples avantages. Il est en effet plus facile d'usiner a l'aide d'outils tels que des meules au diamant un bloc de matériau d'une grande dureté pour obtenir des parties en saillie même de faibles dimensions, que de l'usiner avec des mèches pour y réaliser des logements. De plus, le fait que toutes les surfaces d'usinage de la calotte et de la partie en saillie soient externes rend le contr8le de leurs défauts et qualités beaucoup plus aisé que celui de surfaces
en creux.
Enfin, cette structure n'oblige pas A effectuer des percées dans la masse de la sphère et contribue ainsi à limiter l'existence de points de faiblesse de la tète de prothèse, améliorant d'autant la
fiabilité d'un tel produit.
La structure de tête de prothèse décrite ci-dessus peut cependant comporter un perfectionnement. Ce perfectionnement est constitué par un épaulement 27 situé au niveau de la grande base 17 de l'extrémité tronconique 7 de la patte 4. Cet épaulement va pouvoir constituer une butée pour limiter l'enfoncement de l'extrémité 7 dans la seconde percée 14 du manchon 12. Cette limitation évite ainsi que de trop grands efforts radiaux soient appliqués à la paroi de cette percée 14, et confère donc au manchon une durée de vie plus
importante.
Bien entendu, dans ce cas, le manchon sera coçu, notamment en fonction du matériau dans lequel il est réalisé, pour donner i la paroi de sa seconde percée 14 une épaisseur lui conférant une élasticité de déformation suffisante pour que, d'une part, la couronne circulaire 29 entourant la grande base 19 de cette seconde percée vienne buter contre l'épaulement 27 et que, d'autre part, cette paroi puisse exercer une force de pincement d'intensité suffisante pour obtenir une bonne solidarisation du manchon avec l'extrémité
tronconique de la patte.

Claims (11)

REVENDICATIONS - R E V E N D I C À T I O N S
1. Tête de prothèse (1) du type "prothèse de hanche" ou-
analogue, comportant une calotte sphérique (2) apte à coopérer en rotation dans un cotyle (3), une patte de fixation (4) apte à être implantée dans le canal médulaire d'un os (5) tel qu'un fémur, et des moyens de liaison (6) de la calotte sphérique avec une extrémité (7) de ladite patte de fixation, caractérisée par le fait que lesdits moyens de liaison comportent: une partie en saillie (10) solidaire d'un méplat (11) réalisé sur ladite calotte sphérique, un manchon (12) comportant deux percées (13,14), une première percée (13) ayant une section complémentaire de ladite partie en saillie, ladite partie en saillie étant enfichée-dans cette dite première percée, et la seconde percée (14) -ayant une section sensiblement complémentaire de ladite extrémité de ladite patte de fixation, cette dite extrémité étant enfichée dans cette seconde percée.
2. Tête selon la revendication 1, caractérisée par le fait que ladite partie en saillie (10) a une section inférieure au diamètre du
méplat (11).
3. Tête selon l'une des revendications 1 et 2, caractérisée
par le fait que ladite partie en saillie (10) et ladite première
percée (13) sont de forme cylindrique avantageusement de révolution.
4. Tête selon l'une des revendications 2 et 3, caractérisée
par le fait que l'ouverture (22) de ladite première percée comporte un épaulement externe (16) en forme de couronne circulaire d'un diamètre
extérieur sensiblement inférieur ou égal à celui du méplat (11).
5. Tête selon l'une des revendications 1 à 4, caractérisée par
le fait que ladite extrémité (7) de ladite patte (4) est en forme de tronc de c8ne de révolution dont la grande base (17) définit la surface de liaison entre le corps (18) et ladite extrémité (7) de
ladite patte de fixation.
6. Tête selon la revendication 5, caractérisée par le fait que ladite seconde percée (14) est de forme sensiblement tronconique de révolution d'angle au sommet légèrement inférieur à celui dudit tronc de cène défissant ladite extrémité (7) de ladite patte de fixation, la grande base (19) de cette seconde percée (14) ayant une section égale
à l'une des sections (20) dudit tronc de c6ne.

7. Tite selon la revendication 6, caractérisée par le fait que ladite extrémité comporte un épaulement (27) situé au niveau de sa grande base (17), ledit épaulement constituant une butée pour limiter l'enfoncement de ladite extrémité (7) de la patte dans ladite seconde
percée (14) du manchon (12).
8. Tête selon l'une des revendications 1 à 7, caractérisée par
le fait que ladite première percée comporte, dans son fond, un épaulement rentrant (21) situé à une distance de l'ouverture (22) de
ladite première percée égale à la hauteur de ladite partie en saillie.
9. Tête selon l'une des revendications 1 à 8, caractérisée par
le fait que les fonds, respectivement des deux dites première (13) et
seconde (14) percées, sont confondus.
10. Tête selon les revendications 3, 6 et 9, caractérisée par
le fait que les axes de révolution (23,24) des deux dites percées sont confondus, le diamètre de la petite base de la forme tronconique de la
seconde percée étant inférieur à celui de la première percée.
11. Tête selon l'une des revendications précédentes,
caractérisée par le fait que ladite calotte sphérique (2) et ladite partie en saillie (10) sont réalisées dans la masse d'un même bloc de matériau.
