Un sujeto en necesidad de tratamiento también puede ser diagnosticado presuntamente por pruebas séricas de las enzimas hepáticas. Por ejemplo, la esteatosis puede ser indicada por niveles séricos elevados (a menudo moderadamente elevados, p. ej., elevados aproximadamente 2, 3, 4, 5, 6, 7, 9, 10, 11 ó 12 veces por encima de los niveles normales) de las enzimas hepáticas (tal como, p. ej., alanina aminotransferasa, aspartato aminotransferasa, γ-glutamiltransferasa, fosfatasa alcalina) cuando se han eliminado otras causas (tales como, p. ej., hepatitis aguda, enfermedad autoinmune, hepatitis crónica, cirrosis, hepatitis fulminante, carcinoma hepatocelular, carcinoma metastásico, insuficiencia cardíaca derecha y hepatitis viral). o SGPT) superiores a 32, 24 ó 56 unidades por litro de suero o al menos 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12 o más veces los valores normales pueden ser indicativos de un trastorno asociado con depósitos de lípidos hepáticos, o valores de aspartato aminotransferasa (AST o SGOT) superiores a 40 unidades por litro de suero o al menos 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, o más veces los valores normales. La relación de AST a ALT es a menudo menor que uno en NAFLD, pero puede ser mayor que uno en pacientes con enfermedad hepática alcohólica o enfermedad hepática avanzada. Además, los niveles de γ-glutamiltransferasa pueden ser significativamente elevados, p. ej., al menos 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, o más veces los valores normales. El volumen corpuscular medio (MPV) puede ser mayor que, p. ej., 86, 98, 100 ó 110 femtolitros.
Un sujeto en necesidad de tratamiento también puede ser diagnosticado presuntamente por técnicas no invasivas de formación de imágenes (p. ej., ultrasonografía, tomografía computarizada y formación de imágenes por resonancia magnética) cuando la esteatosis es mayor que, p. ej., 25% o 30%. En general, puede ser difícil distinguir entre NAFLD y NASH para detectar la fibrosis, o para determinar la progresión de la enfermedad por métodos de formación de imágenes de este tipo. La NAFLD puede presentarse como una acumulación focal o difusa de lípidos, pero en la NASH el lípido es generalmente difuso. La NAFLD también puede detectarse por espectroscopía de resonancia magnética, una técnica que puede ser de valor para la determinación cuantitativa de los niveles de lípidos hepáticos. Por ejemplo, la determinación de los niveles de triglicéridos hepáticos por MRI ha demostrado que se correlaciona con resultados de la biopsia histológicos. Véase, p. ej., Kawamitsu et al., Magn. Un sujeto en necesidad de tratamiento puede ser diagnosticado definitivamente mediante biopsia del hígado. Un hígado se considera que es esteatósico cuando la biopsia revela al menos 5-10% p/p de depósitos de grasa (en la práctica, este valor puede determinarse microscópicamente como la fracción de hepatocitos llenos de lípidos). Un hígado con depósitos de grasa que comprende hasta 25% p/p puede ser considerado ligeramente esteatósico, y un hígado con depósitos de grasa que comprende más de 25% p/p puede ser considerado seriamente esteatósico. Hallazgos histológicos indicativos de NASH incluyen esteatosis, balonización de hepatocitos, inflamación lobular, cuerpos hialinos de Mallory, infiltrado inflamatorio mixto, fibrosis pericelular y fibrosis perisinusoidal. Se ha informado que el progreso de la enfermedad en NAFLD/NASH, según se evalúa mediante la fibrosis en la histología del hígado, se correlaciona con el grado de resistencia a la insulina y otras características del síndrome metabólico. Ryan y otros, Diabetes Care, 28: 1222-1224 (2005). Niveles elevados de inmunoglobulina sérica A también han sido asociados con un progreso de la enfermedad. Otros marcadores propuestos para estar relacionados con la fibrosis en pacientes de NAFLD incluyen laminina, hialuronano, colágeno tipo IV y aspartato aminotransferasa. Dos Santos et al., Braz. J. Med. Biol. Res. 38: 747-753 (2005). El género femenino también se asocia con un progreso más rápido de la enfermedad.
La eficacia del tratamiento también se puede determinar mediante la detección de una reducción de uno o más síntomas o manifestaciones clínicas de una enfermedad, así como cualquiera de los ensayos descritos anteriormente para el diagnóstico.
El uso de un inhibidor de una enzima de la síntesis de GSL, que es un compuesto de Fórmula I, puede reducir los niveles séricos de una enzima hepática (p. ej., alanina aminotransferasa, aspartato aminotransferasa, γglutamiltransferasa o fosfatasa alcalina) al menos un 10% tal como, p. ej., al menos un 15, 20, 30, 40, 50, 60, 62, 64, 66, 68 ó 70%, en comparación con el control de pre-tratamiento.
